Abstract:The utilisation of Information and Communication Technology (ICT) and networked information infrastructure in a wide range of businesses and activities has increased the need for fast and reliable information. The information is vital for innovation, competition, and decision making. To date information gathering is no longer absolute from the library, nonetheless the information can be retrieved from all around the world through Internet technology. However, information searching is a tedious process. Knowledge has been identified in this paper as the important factor that affects the searching behaviour. User knowledge is measured by topic and search system understanding while searching behaviour is measured by the breadth and depth of the search query. Both knowledge reflects the true understanding of what and how to search. The breadth and depth of search query show the users' effort in completing the search. In this paper, the impact of user knowledge on search behaviour is accessed on the education domain. Final year undergraduate students were identified as the population that "grown with the web" as they are actively using the Internet to fulfil their study requirement. The findings show that user knowledge does have an impact on the search behaviour.

Validation of cross-sectional and longitudinal ComBat harmonization methods for magnetic resonance imaging data on a travelling subject cohort

Abstract:Background: The growth in multi-center neuroimaging studies generated a need for methods that mitigate the differences in hardware and acquisition protocols across sites i.e., scanner effects. ComBat harmonization methods have shown promise but have not yet been tested on all the data types commonly studied with magnetic resonance imaging (MRI). This study aimed to validate neuroCombat, longCombat and gamCombat on both structural and diffusion metrics in both cross-sectional and longitudinal data. Methods: We used a travelling subject design whereby 73 healthy volunteers contributed 161 scans across two sites and four machines using one T1 and five diffusion MRI protocols. Scanner was defined as a composite of site, machine and protocol. A common pipeline extracted two structural metrics (volumes and cortical thickness) and two diffusion tensor imaging metrics (mean diffusivity and fractional anisotropy) for seven regions of interest including gray and (except for cortical thickness) white matter regions. Results: Structural data exhibited no significant scanner effect and therefore did not benefit from harmonization in our particular cohort. Indeed, attempting harmonization obscured the true biological effect for some regions of interest. Diffusion data contained marked scanner effects and was successfully harmonized by all methods, resulting in smaller scanner effects and better detection of true biological effects. LongCombat less effectively reduced the scanner effect for cross-sectional white matter data but had a slightly lower probability of incorrectly finding group differences in simulations, compared to neuroCombat and gamCombat. False positive rates for all methods and all metrics did not significantly exceed 5%. Conclusions: Statistical harmonization of structural data is not always necessary and harmonization in the absence of a scanner effect may be harmful. Harmonization of diffusion MRI data is highly recommended with neuroCombat, longCombat and gamCombat performing well in cross-sectional and longitudinal settings.

Abstract:Diffusion-weighted magnetic resonance imaging (dMRI) of brain has helped elucidate the microstructural changes of psychiatric and neurodegenerative disorders. Inconsistency between MRI models has hampered clinical application of dMRI-based metrics. Using harmonized dMRI data of 300 scans from 69 traveling subjects (TS) scanning the same individuals at multiple conditions with 13 MRI models and 2 protocols, the widely-used metrics such as diffusion tensor imaging (DTI) and neurite orientation dispersion and density imaging (NODDI) were evaluated before and after harmonization with a combined association test (ComBat) or TS-based general linear model (TS-GLM). Results showed that both ComBat and TS-GLM significantly reduced the effects of the MRI site, model, and protocol for diffusion metrics while maintaining the intersubject biological effects. The harmonization power of TS-GLM based on TS data model is more powerful than that of ComBat. In conclusion, our research demonstrated that although ComBat and TS-GLM harmonization approaches were effective at reducing the scanner effects of the site, model, and protocol for DTI and NODDI metrics in WM, they exhibited high retainability of biological effects. Therefore, we suggest that, after harmonizing DTI and NODDI metrics, a multisite study with large cohorts can accurately detect small pathological changes by retaining pathological effects.
Bundle Analytics based Data Harmonization for Multi-Site Diffusion MRI Tractometry

Abstract:The neural pathways of the living human brain can be tracked using diffusion MRI-based tractometry. Alongtract statistical analysis of microstructural metrics can reveal the effects of neurological and psychiatric diseases with 3D spatial precision. To maximize statistical power to detect disease effects and factors that influence them, data from multiple sites and scanners must often be combined, yet scanning protocols and hardware may vary widely. For simple scalar metrics, data harmonization methods - such as ComBat and its variants - allow modeling of disease effects on derived brain metrics, while adjusting for effects of scanning site or protocol. Here, we extend this method to pointwise segment analyses of 3D fiber bundles by integrating ComBat into the BUndle ANalytics (BUAN) tractometry pipeline. In a study of the effects of mild cognitive impairment (MCI) and Alzheimer’s disease (AD) on 38 white matter tracts, we merge data from 7 different scanning protocols used in the Alzheimer’s Disease Neuroimaging Initiative, which vary in voxel size and angular resolution. By incorporating ComBat harmonization, we model site- and scanner-specific effects, ensuring the reliability and comparability of results by mitigating confounding variables. We also evaluate choices that arise in extending batch adjustment to tracts, such as the regions used to estimate the correction. We also compare the approach to the simpler approach of modeling the site as a random effect. To the best of our knowledge, this is one of the first applications to adapt harmonization to 3D tractometry.

Improved inter-subject alignment of the lumbosacral cord for group-level in vivo gray and white matter assessments: A scan-rescan MRI study at 3T

Abstract:Introduction Magnetic resonance imaging (MRI) enables the investigation of pathological changes in gray and white matter at the lumbosacral enlargement (LSE) and conus medullaris (CM). However, conducting group-level analyses of MRI metrics in the lumbosacral spinal cord is challenging due to variability in CM length, lack of established image-based landmarks, and unknown scan-rescan reliability. This study aimed to improve inter-subject alignment of the lumbosacral cord to facilitate group-level analyses of MRI metrics. Additionally, we evaluated the scan-rescan reliability of MRI-based cross-sectional area (CSA) measurements and diffusion tensor imaging (DTI) metrics. Methods Fifteen participants (10 healthy volunteers and 5 patients with spinal cord injury) underwent axial T2*-weighted and diffusion MRI at 3T. We assessed the reliability of spinal cord and gray matter-based landmarks for inter-subject alignment of the lumbosacral cord, the inter-subject variability of MRI metrics before and after adjusting for the CM length, the intra- and inter-rater reliability of CSA measurements, and the scan-rescan reliability of CSA measurements and DTI metrics. Results The slice with the largest gray matter CSA as an LSE landmark exhibited the highest reliability, both within and across raters. Adjusting for the CM length greatly reduced the inter-subject variability of MRI metrics. The intra-rater, inter-rater, and scan-rescan reliability of MRI metrics were the highest at and around the LSE (scan-rescan coefficient of variation <3% for CSA measurements and <7% for DTI metrics within the white matter) and decreased considerably caudal to it. Conclusions To facilitate group-level analyses, we recommend using the slice with the largest gray matter CSA as a reliable LSE landmark, along with an adjustment for the CM length. We also stress the significance of the anatomical location within the lumbosacral cord in relation to the reliability of MRI metrics. The scan-rescan reliability values serve as valuable guides for power and sample size calculations in future longitudinal studies.

Harmonizing multisite neonatal diffusion-weighted brain MRI data for developmental neuroscience

Abstract:Large diffusion-weighted brain MRI (dMRI) studies in neonates are crucial for developmental neuroscience. Our aim was to investigate the utility of ComBat, and empirical Bayes tool for multisite harmonization, for removing site effects from white matter (WM) dMRI measures in healthy infants born 37-42+6 weeks from the Theirworld Edinburgh Birth Cohort (n=86) and Developing Human Connectome Project (n=287). Skeletonized fractional anisotropy (FA), mean, axial and radial diffusivity (MD, AD, RD) maps were harmonized. The differences between voxel-wise metrics, skeleton means and histogram widths (5th-95th percentile) were assessed before and after harmonization, as well as variance associated with gestational age at birth. Before harmonization, large cohort differences were observed in all measures. Harmonization removed all voxel-wise differences from MD maps and all metric means and histogram widths, however small voxel-wise differences (<1.5% of voxels) remained in FA, AD and RD. We detected significant relationships between GA at birth and all metrics. When comparing single site and multi-site harmonized datasets of equal sample sizes, harmonized data resulted in smaller standardized regression coefficients. ComBat will enable unprecedented sample sizes in developmental neuroscience, offering new horizons for biomarker discovery and validation, understanding typical and atypical brain development, and assessment of neuroprotective therapies.

High-fidelity Diffusion Tensor Imaging of the Cervical Spinal Cord Using Point-Spread-function Encoded EPI

Abstract:Diffusion tensor imaging (DTI) of the spinal cord is technically challenging due to the size of its structure and susceptibility-induced field inhomogeneity, which impedes clinical applications. This study aimed to achieve high-fidelity spinal cord DTI with reasonable SNR and practical acquisition efficiency. Particularly, a distortion-free multi-shot EPI technique, namely point-spread-function encoded EPI (PSF-EPI), was adopted for diffusion imaging of the cervical spinal cord (CSC). The shot number can be reduced to six for sagittal scans through titled-CAIPI acceleration and partial Fourier undersampling, consequently rendering this technique beneficial in clinics. Fifteen healthy volunteers and seven patients with metallic implants underwent sagittal scans using tilted-CAIPI PSF-EPI at 3T. Unsuppressed fat signals were further removed by retrospective water/fat separation using the intrinsic chemical-shift encoded signals. Compared with multi-shot interleaved EPI method, highly accelerated PSF-EPI method provided evidently improved distortion reduction and higher consistency with anatomical references even with metallic implants. Additionally, axial DTI scans using PSF-EPI were also evaluated quantitatively, and the measured DTI metrics are similar to those obtained from the zonal oblique multi-slice EPI (ZOOM-EPI) method and reported values. The high anatomical consistency, practical scan time and quantitative reliability indicate PSF-EPI's clinical potential for CSC diffusion imaging.

Abstract:Purpose: To introduce a framework that automatically identifies regions of anatomical abnormality within anatomical MR images and uses those regions in hypothesis-driven selection of seed points for fiber tracking with diffusion tensor (DT) imaging (DTI). Materials and Methods: Regions of interest (ROIs) are first extracted from MR images using an automated algorithm for volume-preserved warping (VPW) that identifies localized volumetric differences across groups. ROIs then serve as seed points for fiber tracking in coregistered DT images. Another algorithm automatically clusters and compares morphologies of detected fiber bundles. We tested our framework using datasets from a group of patients with Tourette's syndrome (TS) and normal controls. Results: Our framework automatically identified regions of localized volumetric differences across groups and then used those regions as seed points for fiber tracking. In our applied example, a comparison of fiber tracts in the two diagnostic groups showed that most fiber tracts failed to correspond across groups, suggesting that anatomical connectivity was severely disrupted in fiber bundles leading from regions of known anatomical abnormality. Conclusion: Our framework automatically detects volumetric abnormalities in anatomical MRIs to aid in generating a priori hypotheses concerning anatomical connectivity that then can be tested using DTI. Additionally, automation enhances the reliability of ROIs, fiber tracking, and fiber clustering.

Abstract:INTRODUCTION: Traditional spine imaging technique present disparity between degenerative cervical myelopathy (DCM) disease severity and prognosis. Advancement in spine neuroimaging has unveiled key changes that enhance our understanding of DCM pathogenesis. We investigated region- and tract-specific SC white matter (WM) damage and their relationship to motor deficit in DCM. METHODS: A cohort of 20 DCM and 23 age-matched HC participants were recruited in a cross-sectional study. We utilized a consensus MT-imaging protocol, Spinal cord Toolbox, and objective motor assessment tool battery to assess the extent of WM injury and functional deficits in DCM. The SC WM region- and tract-based MTR were calculated in three different ROIs: maximum compressed level (MCL), caudal uncompressed level (C7), and central region of compression. RESULTS: DCM patients demonstrated significant functional deficits compared to HC in motor assessments. DCM exhibited reduced MTR measures denoting WM injury in the anterior region of the SC and within selected motor tracts in the ROIs. MTR values in reticulospinal, vestibulospinal, and lateral corticospinal tracts were significantly associated with dexterity and grip strength measures. CONCLUSIONS: Our methodology enables precise mapping of ROIs within the SC allowing us to demonstrate an association between tract-specific WM damage and functional motor deficits in DCM patients. Additionally, the observation of decreased MTR measures in seemingly uncompressed SC regions indicates the pathophysiology of DCM that may provide insight into the varying presentations of DCM. Identification of specific WM injury may help better understand DCM deficits that can be targeted for effective interventions to improve patient outcomes

Assessing the Agreement Between Diffusion Tension Imaging (DTI) and T2-Weighted MRI Sequence for Biometry of the Fetal Corpus Callosum

Abstract:Background/Objectives: Little is known about the advantages of Diffusion Tensor Imaging (DTI) when evaluating the fetal corpus callosum (CC), a sensitive indicator for normal brain development. This study evaluates the contribution of DTI compared to T2-weighted imaging to assess fetal CC biometry. Methods: Data from the fetal MRI exams of singleton pregnancies between July 2017 and 2019 were retrospectively analyzed. Mid-sagittal sections were used to measure the CC biometry, and inter- and intra-observer agreements were assessed using the interclass correlation coefficient (ICC), targeting an ICC above 0.85. Results: The results from 100 patients (mean gestational age, 32.24 weeks) indicated excellent inter-observer reliability for DTI (ICC = 0.904, 95% CI = 0.815–0.952) and moderate agreement for T2-weighted imaging (ICC = 0.719, 95% CI = 0.556–0.842). Intra-observer assessments showed excellent reliability for both DTI and T2-weighted imaging (ICC = 0.967, 95% CI = 0.933–0.984 and ICC = 0.942, 95% CI = 0.884–0.971, respectively). However, a comparison between DTI and T2-weighted images for CC biometry showed poor agreement (ICC = 0.290, 95% CI = 0.071–0.476). Conclusions: In conclusion, the study highlights a lack of agreement between DTI and T2-weighted imaging in fetal CC biometry, suggesting the need for further research to understand this discrepancy and the role of DTI in fetal health.

High-resolution diffusion tensor imaging in cervical spondylotic myelopathy: a preliminary follow-up study.

Abstract:Diffusion imaging is a promising technique as it can provide microstructural tissue information and thus potentially show viable changes in spinal cord. However, the traditional single-shot imaging method is limited as a result of various image artifacts. In order to improve measurement accuracy, we used a newly developed, multi-shot, high-resolution, diffusion tensor imaging (DTI) method to investigate diffusion metric changes and compare them with T-2-weighted (T2W) images before and after decompressive surgery for cervical spondylotic myelopathy (CSM). T2W imaging, single-shot DTI and multi-shot DTI were employed to scan seven patients with CSM before and 3 months after decompressive surgery. High signal intensities were scored using the T2 W images. DTI metrics, including fractional anisotropy (FA), axial diffusivity (AD), radial diffusivity (RD) and mean diffusivity (MD), were quantified and compared pre-and post-surgery. In addition, the relationship between imaging metrics and neurological assessments was examined. The reproducibility of multi-shot DTI was also assessed in 10 healthy volunteers. Post-surgery, the mean grade of cervical canal stenosis was reduced from grade 3 to normal after 3 months. Compared with single-shot DTI, multi-shot DTI provided better images with lower artifact levels, especially following surgery, as a result of reduced artifacts from metal implants. The new method also showed acceptable reproducibility. Both FA and RD values from the new acquisition showed significant differences post-surgery (FA, p = 0.026; RD, p = 0.048). These changes were consistent with neurological assessments. In contrast, T2W images did not show significant changes before and after surgery. Multi-shot diffusion imaging showed improved image quality over single-shot DWI, and presented superior performance in diagnosis and recovery monitoring for patients with CSM compared with T2W imaging. DTI metrics can reflect the pathological conditions of spondylotic spinal cord quantitatively and may serve as a sensitive biomarker for potential CSM management.
ComBating inter‐site differences in field strength: harmonizing preclinical traumatic brain injury MRI data

Abstract:Rats underwent longitudinal 4.7 or 9.4 T MRI following moderate/severe traumatic brain injury (TBI) or sham injury. Mixed‐effect analysis of summary values revealed a significant effect of site on FA and ADC values, which was effectively removed following ComBat harmonization. QSM was largely unaffected by inter‐site differences. Our findings demonstrate the need for harmonizing multi‐site preclinical MRI data and show that this can be successfully achieved using ComBat while preserving phenotypical changes due to TBI. Integrating datasets from multiple sites and scanners can increase statistical power for neuroimaging studies but can also introduce significant inter‐site confounds. We evaluated the effectiveness of ComBat, an empirical Bayes approach, to combine longitudinal preclinical MRI data acquired at 4.7 or 9.4 T at two different sites in Australia. Male Sprague Dawley rats underwent MRI on Days 2, 9, 28, and 150 following moderate/severe traumatic brain injury (TBI) or sham injury as part of Project 1 of the NIH/NINDS‐funded Centre Without Walls EpiBioS4Rx project. Diffusion‐weighted and multiple‐gradient‐echo images were acquired, and outcomes included QSM, FA, and ADC. Acute injury measures including apnea and self‐righting reflex were consistent between sites. Mixed‐effect analysis of ipsilateral and contralateral corpus callosum (CC) summary values revealed a significant effect of site on FA and ADC values, which was removed following ComBat harmonization. Bland–Altman plots for each metric showed reduced variability across sites following ComBat harmonization, including for QSM, despite appearing to be largely unaffected by inter‐site differences and no effect of site observed. Following harmonization, the combined inter‐site data revealed significant differences in the imaging metrics consistent with previously reported outcomes. TBI resulted in significantly reduced FA and increased susceptibility in the ipsilateral CC, and significantly reduced FA in the contralateral CC compared with sham‐injured rats. Additionally, TBI rats also exhibited a reversal in ipsilateral CC ADC values over time with significantly reduced ADC at Day 9, followed by increased ADC 150 days after injury. Our findings demonstrate the need for harmonizing multi‐site preclinical MRI data and show that this can be successfully achieved using ComBat while preserving phenotypical changes due to TBI.

Abstract:Diffusion-weighted magnetic resonance imaging (DW-MRI) is a promising technique for assessing spinal cord injury that has historically been challenged by the presence of metallic stabilization hardware. This study leverages recent advances in metal-artifact resistant multi-spectral DW-MRI to enable diffusion quantification throughout the spinal cord even after fusion stabilization. Twelve cervical spinal cord injury subjects treated with fusion stabilization and 49 asymptomatic controls underwent multi-spectral DW-MRI evaluation. Apparent diffusion coefficient (ADC) values were calculated in axial cord sections. Statistical modeling assessed ADC differences across cohorts and within distinct cord regions of the SCI subjects (at, above or below injured level). Computed mod- els accounted for subject demographics and injury characteristics. ADC was found to be elevated at injured levels compared to non-injured levels (z=3.2, p = 0.001), with ADC at injured levels decreasing over time since injury (z=-9.2, p<0.001). Below the injury level, ADC was reduced relative to controls (z=-4.4, p<0.001), with greater reductions after more severe injuries that cor- related with lower extremity motor scores (z=2.56, p=0.012). By enabling diffusion analysis near fusion hardware, the multi-spectral DW-MRI technique allowed intuitive quantification of cord dif- fusion changes after spinal cord injury both at and away from injured levels. This demonstrates the approach's potential for assessing post-surgical spinal cord integrity throughout stabilized regions.

Investigation of ComBat Harmonization on Radiomic and Deep Features from Multi-Center Abdominal MRI Data

Abstract:ComBat harmonization has been developed to remove non-biological variations for data in multi-center research applying artificial intelligence (AI). We investigated the effectiveness of ComBat harmonization on radiomic and deep features extracted from large, multi-center abdominal MRI data. A retrospective study was conducted on T2-weighted (T2W) abdominal MRI data retrieved from individual patients with suspected or known chronic liver disease at three study sites. MRI data were acquired using systems from three manufacturers and two field strengths. Radiomic features and deep features were extracted using the PyRadiomics pipeline and a Swin Transformer. ComBat was used to harmonize radiomic and deep features across different manufacturers and field strengths. Student's t-test, ANOVA test, and Cohen's F score were applied to assess the difference in individual features before and after ComBat harmonization. Between two field strengths, 76.7%, 52.9%, and 26.7% of radiomic features, and 89.0%, 56.5%, and 0.1% of deep features from three manufacturers were significantly different. Among the three manufacturers, 90.1% and 75.0% of radiomic features and 89.3% and 84.1% of deep features from two field strengths were significantly different. After ComBat harmonization, there were no significant differences in radiomic and deep features among manufacturers or field strengths based on t-tests or ANOVA tests. Reduced Cohen's F scores were consistently observed after ComBat harmonization. ComBat harmonization effectively harmonizes radiomic and deep features by removing the non-biological variations due to system manufacturers and/or field strengths in large multi-center clinical abdominal MRI datasets.
Diffusion Tensor Imaging Identifies Cervical Spondylosis, Myelitis, and Spinal Cord Tumors

Abstract:Background: Diffusion tensor imaging (DTI) has been increasingly recognized for its capability to study microstructural changes in the neuropathology of brain diseases. However, the optimal DTI metric and its diagnostic utility for a variety of spinal cord diseases are still under investigation. Purpose: To evaluate the diagnostic efficacy of DTI metrics for differentiating between cervical spondylosis, myelitis, and spinal tumors. Methods: This retrospective study analyzed DTI scans from 68 patients (22 with cervical spondylosis, 23 with myelitis, and 23 with spinal tumors). DTI indicators, including fractional anisotropy (FA), mean diffusivity (MD), radial diffusivity (RD) and axial diffusivity (AD), were calculated. The Kruskal–Wallis test was used to compare these indicators, followed by Receiver Operating Characteristic (ROC) curve analysis, to evaluate the diagnostic efficacy of each indicator across disease pairs. Additionally, we explored the correlations of DTI indicators with specific clinical measurements. Results: FA values were significantly lower in tumor patients compared to those with cervical spondylosis (p < 0.0001) and myelitis (p < 0.05). Additionally, tumor patients exhibited significantly elevated MD and RD values relative to the spondylosis and myelitis groups. ROC curve analysis underscored FA's superior discriminative performance, with an area under the curve (AUC) of 0.902 for differentiating tumors from cervical spondylosis, and an AUC of 0.748 for distinguishing cervical myelitis from spondylosis. Furthermore, a significant negative correlation was observed between FA values and Expanded Disability Status Scores (EDSSs) in myelitis patients (r = −0.62, p = 0.002), as well as between FA values and Ki-67 scores in tumor patients (r = −0.71, p = 0.0002). Conclusion: DTI indicators, especially FA, have the potential in distinguishing spondylosis, myelitis, and spinal cord tumors. The significant correlation between FA values and clinical indicators highlights the value of FA in the clinical assessment and prognosis of spinal diseases and may be applied in diagnostic protocols in the future.

Abstract:We proposed a personalized framework to effectively address the confounding caused by the misalignment of neuroanatomy in dMRI harmonization. This approach is applied to harmonize the dMRI data from the Adolescent Brain Cognitive Development study. We demonstrated that our method achieved superior performance in reducing inter‐site variations and preserving sex‐related biological variability. One fundamental challenge in diffusion magnetic resonance imaging (dMRI) harmonization is to disentangle the contributions of scanner‐related effects from the variable brain anatomy for the observed imaging signals. Conventional harmonization methods rely on establishing an atlas space to resolve anatomical variability and generate a unified inter‐site mapping function. However, this approach is limited in accounting for the misalignment of neuroanatomy that still widely persists even after registration, especially in regions close to cortical boundaries. To overcome this challenge, we propose a personalized framework in this paper to more effectively address the confounding from the misalignment of neuroanatomy in dMRI harmonization. Instead of using a common template representing site‐effects for all subjects, the main novelty of our method is the adaptive computation of personalized templates for both source and target scanning sites to estimate the inter‐site mapping function. We integrate our method with the rotation invariant spherical harmonics (RISH) features to achieve the harmonization of dMRI signals between sites. In our experiments, the proposed approach is applied to harmonize the dMRI data acquired from two scanning platforms: Siemens Prisma and GE MR750 from the Adolescent Brain Cognitive Development dataset and compared with a state‐of‐the‐art method based on RISH features. Our results indicate that the proposed harmonization framework achieves superior performance not only in reducing inter‐site variations due to scanner differences but also in preserving sex‐related biological variability in original cohorts. Moreover, we assess the impact of harmonization on the estimation of fiber orientation distributions and show the robustness of the personalized harmonization procedure in preserving the fiber orientation of original dMRI signals.

Abstract:Background: Magnetic field inhomogeneities generate important geometric distortions in reconstructed echo-planar images. Various procedures were proposed for correcting these distortions on brain images; yet, few neuroimaging studies tailored and incorporated the use of these techniques in spinal cord diffusion MRI. Purpose: We present a comparative evaluation of distortion correction methods that use the reversed gradient polarity technique on spinal cord. We propose novel geometric metrics to measure the alignment of the reconstructed diffusion model with the apparent centerline of the spinal cord. Subjects: 95 subjects, among which 29 healthy controls and 66 multiple sclerosis patients. Assessment: Geometric distortions were corrected using 4 state-of-the-art methods. We measured the alignment of the principal direction of diffusion with the apparent centerline of the spine after correction and the correlation with the reference anatomical image. Results are computed per vertebral level, to evaluate the impact on different portions of the spine. Besides, subjective evaluation of the quality of the correction of healthy subjects images was performed by three expert raters. Results: As a result of distortion correction, the diffusion directions are better aligned locally with the centerline, in particular at both ends of the acquisition window. The cross-correlation with anatomical image is also improved by Hyperelastic Susceptibility Artefact Correction (HySCO) and block-matching. The subjective evaluation for HySCO is significantly better (p < 0.05) than for Block-Matching; TOPUP performs significantly worse than the three other methods. Conclusion: Correction based on HySCO provide best results among the selected methods.
A Follow-up Study of Postoperative DCM Patients Using Diffusion MRI with DTI and NODDI

Abstract:Study Design. A retrospective cohort study. Objective. The aim of this study was to investigate the relationship between spinal cord microstructures and spinal cord dysfunction in degenerative cervical myelopathy (DCM) patients; a follow-up study was carried out using diffusion tensor imaging (DTI) and neurite orientation dispersion and density imaging (NODDI), an advanced diffusion model. Summary of Background Data. DTI has been used for diagnosis of DCM. Although DTI has advantages of high sensitivity and quantitative features, it is limited by its low specificity for measuring microstructures. Methods. Fifty-eight postoperative DCM patients with 12 to 14 months of surgical recovery were recruited, as well as 14 healthy volunteers for control group. All DTI and NODDI metrics were measured at the most stenotic levels of patients and at all levels of control group. A t test was used to compare the metrics between patient and control groups, and Spearman correlation was used to test the relationship between the metrics and clinical assessment, modified Japanese Orthopedic Association (mJOA) scores. Results. t test showed that DTI metrics, neurite density Vic, and free water fraction Viso had significant differences between control group and the most stenotic levels in patients. For DTI metrics, fractional anisotropy (FA), radial diffusivity (RD), and mean diffusivity (MD) in patients were significantly correlated with mJOA. For NODDI metrics, only Vic is positively correlated with mJOA. Conclusion. The results of t test and correlation with mJOA suggest that DTI and NODDI are valuable for evaluating spinal cord function. Results of NODDI indicate that the reason for FA reduction in DCM may be decreased neurite density, not increased orientation dispersion. Level of Evidence: 3
