Abstract:Bacterial cellulose is a bacterially derived polymer with great potential for application in wound healing due to its innate properties such as high biocompatibility and biodegradability. In addition to this, it is naturally biosynthesized by bacteria as a hydrogel, which makes it an optimal substrate for the treatment of dry wounds, where additional moisture is required to facilitate the healing process. However, this polymer lacks antibacterial properties. As bacterial infections are becoming increasingly common and difficult to treat due to antimicrobial resistance, it is of crucial importance to develop strategies for the modification of cellulose to ensure protection against bacterial contamination. In this study, a green-chemistry approach was proposed for the functionalization of cellulose to introduce antibacterial functional groups. Two different active agents, namely glycidyl trimethylammonium chloride and glycidyl hexadecyl ether, were used for the covalent derivatization of the hydroxyl groups of glucose through a heterogeneous reaction in basic aqueous conditions. The modified material was chemically and mechanically characterized by solid-state techniques and rheological measurements. A biological assessment was then carried out both using bacterial cells and human keratinocytes. It was observed that the functionalization performed induced a reduction of approximately half of the bacterial population within 24 h of direct contact withStaphylococcus aureussubsp. aureus Rosenbach 6538P(TM)andEscherichia coli(Migula) Castellani and Chalmers ATCC(R)8739(TM)(respectively, a reduction of 53% and 43% in the cell number was registered for the two strains). In parallel, cytotoxicity studies performed on keratinocytes (HaCaT cell line) showed cell viability in the range of 90 to 100% for up to 6 days of direct contact with both unmodified and modified samples. The morphology of the cells was also visually evaluated, and no significant difference was noted as compared to the control. Finally, thein vitroscratch assay evidenced good wound closure rates in the presence of the samples, with complete coverage of the scratched area after 5 days for both the modified cellulose and the positive control (i.e., keratinocytes growth medium). Overall, the modified hydrogel showed promising features, confirming its potential as an alternative substrate to develop a sustainable, antibacterial and biocompatible wound dressing.

Abstract:Biofilms are complex bacterial colonies embedded in an extracellular matrix. These pose a major obstacle to wound healing and are noticeable in chronic wounds. It protects the bacteria from the host's immune system and conventional antibiotic treatments. The biofilm's protective matrix prevents essential nutrients and oxygen from diffusing into the surrounding healthy tissue. In addition, microbes living in biofilms naturally have increased resistance to antibiotics, which reduces the effectiveness of traditional therapies. As such, biofilms serve as persistent reservoirs of infection, which further disrupts the normal course of wound healing. In this review, the current formulation strategies such as hydrogels, polymeric nanoparticles, and nanofibers that are used in wound healing to counteract biofilms have been comprehensively discussed. The formulations have been meticulously designed and developed to disturb the biofilm matrix, prevent the growth of microorganisms, and increase the potency of antimicrobials and antibiotics. The mechanism of action, advantages and limitations associated with the existing formulation strategies have been reviewed. The formulation strategies that have been translated into clinical applications and patented are also discussed in this paper.

Dissolvable Microneedles Coupled with Nanofiber Dressings Eradicate Biofilms via Effectively Delivering a Database-Designed Antimicrobial Peptide

Abstract:Biofilms in chronic wounds, including diabetic foot ulcers, pressure ulcers, and venous leg ulcers, pose a major challenge to wound management. Herein, we report a Janus-type antimicrobial dressing for eradication of biofilms in chronic wounds. The dressing consists of electrospun nanofiber membranes coupled with dissolvable microneedle arrays to enable effective delivery of a database-designed antimicrobial peptide to both inside and outside biofilms. This antimicrobial dressing exhibited high efficacy against a broad spectrum of resistant pathogens in vitro. Importantly, such a dressing was able to eradicate methicillin-resistant Staphylococcus aureus (MRSA) biofilms in both an ex vivo human skin wound infection model and a type II diabetic mouse wound infection model after daily treatment without applying surgical debridement. Most importantly, the dressing can also completely remove the Pseudomonas aeruginosa and MRSA, dual-species biofilm in an ex vivo human skin infection model. In addition, our computational simulations also suggested that microneedles were more effective in the delivery of peptides to the biofilms than free drugs. Abstract:Silent information regulator 1 (SIRT1) mediates many effects of caloric restriction (CR) on an organism's lifespan and metabolic pathways. Recent reports have also emphasized its role in vascular function. The present study was designed to investigate the effects of SIRT1 on the properties of mouse spleen derived endothelial progenitor cells (EPCs). SIRT1 in EPCs was significantly increased by serum and by vascular endothelial growth factor (VEGF). Moreover, an adenovirus (Ad) vector expressing SIRT1 (Ad-SIRT1)-mediated overexpression of SIRT1 directly enhanced migration and proliferation of EPCs, whereas silencing of endogenous SIRT1 in EPCs inhibited cell functions. In addition, LY294002 (a PI3K inhibitor), sc-221226 (an Akt inhibitor), and L-NAME (an NOS inhibitor) abolished Ad-SIRT1-induced migration and proliferation of EPCs, and prevented nitric oxide (NO) production. Phosphorylation of Akt, PI3K, and endothelial nitricoxide synthase (eNOS) were up-regulated by Ad-SIRT1, which was attenuated by LY294002, sc-221226, and L-NAME. Together, the results suggested that through the PI3K/Akt/eNOS signaling pathway, SIRT1 plays an important role in the biological properties of EPCs.
Carboxymethyl chitosan/sodium alginate hydrogels with polydopamine coatings as promising dressings for eliminating biofilm and multidrug-resistant bacteria induced wound healing

Abstract:Microorganisms induced wound infection and the accompanying excessive inflammatory response is the daunting problems in wound treatment. Due to the lack of corresponding biological functions, traditional wound dressings cannot effectively protect the wound and are prone to induce local infection, excessive inflammation, and vascular damage, resulting in prolonged unhealing. Here, a mussel-inspired strategy was adopted to prepare a multifunctional hydrogel created by H2O2/CuSO4-induced rapid polydopamine (PDA) deposition on carboxymethyl chitosan (CMC)/sodium alginate (Alg) based hydrogel, termed as CAC/PDA/Cu(H2O2). The prepared CAC/PDA/Cu(H2O2) hydrogel features excellent biocompatibility, adequate mechanical properties, and good degradability. Moreover, the CAC/PDA/Cu(H2O2) hydrogel can not only realize antibacterial, and anti-inflammatory effects, but also promote angiogenesis to accelerate wound healing in vitro thanks to the composite PDA/Cu(H2O2) coatings. Significantly, CAC/PDA/Cu(H2O2) hydrogel illustrates excellent therapeutic effects in Methicillin-resistant Staphylococcus aureus (MRSA) induced-rat infection models, which can efficiently eliminate MRSA, dramatically reduce inflammatory expression, promote angiogenesis, and ultimately shorten the wound healing time. CAC/PDA/Cu(H2O2) hydrogel exhibited the best wound healing rate on days 7 (80.63 ± 2.44 %), 11 (92.45 ± 2.26 %), and 14 (97.86 ± 0.66 %). Thus, the multifunctional hydrogel provides a facile and efficient approach to wound management and represents promising potential in the therapy for wound healing.
Designing an antimicrobial film for wound applications incorporating bacteriophages and ε-poly-l-lysine

Abstract:Alginate-based dressings have been shown to promote wound healing, leveraging the unique properties of alginate. This work aimed to develop and characterize flexible individual and bilayered films to deliver bacteriophages (phages) and ε-Poly-l-lysine (ε-PLL). Films varied in different properties. The moisture content, swelling and solubility increased with higher alginate concentrations. The water vapour permeability, crucial in biomedical films to balance moisture levels for effective wound healing, reached optimal levels in bilayer films, indicating these will be able to sustain an ideal moist environment. The bilayer films showed improved ductility (lower tensile strength and increased elongation at break) compared to individual films. The incorporated phages maintained viability for 12 weeks under vacuum and refrigerated conditions, and their release was sustained and gradual. Antibacterial immersion tests showed that films with phages and ε-PLL significantly inhibited Pseudomonas aeruginosa PAO1 growth (>3.1 Log CFU/cm2). Particle release was influenced by the swelling degree and diffusional processes within the polymer network, providing insights into controlled release mechanisms for particles of varying size (50 nm to 6 μm) and charge. The films developed, demonstrated modulated release capabilities for active agents, and may show potential as controlled delivery systems for phages and wound healing adjuvants.

Abstract:Abstract. This paper describes a line-based classification method, which labels TLS point clouds into vertical object, ground, tree and low objects. A local classifier implements labeling task on individual site independently of its neighborhood, the inference of which often suffers from similar local appearance across different object classes. In this paper, we describe an approach using contextual information as postclassification improvement to a local generative classifier. The contextual information is expected to compensate for ambiguity in objects' visual appearance. A generative classifier is produced using Gaussian Mixture Model (GMM), model parameters of which are iteratively optimized with Expectation-Maximization (EM). The model we use to incorporate contextual information is the Conditional Random Field (CRF), which improves the classification results obtained from GMM-EM classifier by incorporating neighborhood interactions among labeled objects as well as local appearance. The proposed method was validated with three TLS datasets acquired from RIEGL LMS-Z390i scanner using cross validation.

Abstract:Bacterial infections are a constant challenge in the management of acute and chronic wounds. Chronic wounds, such as diabetic foot ulcers, have increased significantly in the last few years due to the rise of an aging population. A better understanding of the infectious pathophysiological mechanisms is urgently needed along with new options for the treatment of wound infections and wound-healing disorders. New advances in the preparation of biocompatible dressing materials that can be loaded with antimicrobial drugs may improve the topical treatment of infected wounds. In this study, we investigated the antimicrobial activity of polyvinylpyrrolidone (PVP) foils loaded with ciprofloxacin (Cipro-foils) in the presence of acetic acid as a co-solvent. We used ex vivo human wounds that were infected with two bacterial strains: methicillin-resistant Staphylococcus aureus (MRSA) or Pseudomonas aeruginosa (PAO1). The effectiveness of the treatment was demonstrated by the quantification of the living bacteria extracted from the wound and the detection of released immunological mediators in skin extracts and in the skin culture media. We found that Cipro-foils effectively treated the infection with both PAO1 and MRSA. Other than PAO1, MRSA had no lytic activity toward skin proteins. MRSA infections increased cytokines' expression and release. Interestingly, treatment with Cipro-foils could partially counteract these effects.
Engineering Sustainable Antimicrobial Release in Silica-Cellulose Membrane with CaCO3-Aided Processing for Wound Dressing Application

Abstract:The sustained release of antimicrobial therapeutics for wound dressing has become an attractive design strategy for prolonging the timespan of wound dressings and for reducing the risk of chronic wound infection. Recently, cellulose-based membrane has become a preferred option of wound dressings for the treatment of burn wounds and skin ulcers. In this work, novel cellulose membrane incorporated with mesoporous silica particles (SBA-15) was developed as an antimicrobial wound dressing with desirable sustained release functionality for targeting persistent bacterial pathogens. Attributed to a coated layer of calcium carbonate (CaCO3), SBA-15 particles were free from corrosion in alkaline condition during the preparation of cellulose-based composite membranes. SEM, TEM and BET results showed that the morphology, specific surface area, pore size and pore volume of pristine SBA-15 were preserved after the incorporation of CaCO3-coated SBA-15 into the cellulose matrix, while the mesoporous structure of SBA-15 was significantly disrupted without the use of CaCO3 coating. The resultant composite membranes containing 30 wt% SBA-15 (denoted as CM-Ca2-SBA(30%)) achieved 3.6 wt% of antimicrobial drug loading. Interestingly, CM-Ca2-SBA(30%) demonstrated the sustained release property of chloramphenicol for 270 h, driven by a two-stage drug release processes of SBA-15/cellulose. The water vapor permeability (WVTR) and swelling properties of composite membranes were shown to have complied with the primary requirements of wound dressing. Antibacterial assays revealed that strong antibacterial activities (144 h) of the composite membranes against Staphylococcus aureus and Eschericia coli were achieved. All results displayed that the strategy of coating silica with CaCO3 helps to obtain cellulose–silica composite membranes with desirable sustained release profiles and strong antibacterial activities. The antibacterial SBA-15/cellulose composite membranes show potential for the application of wound dressing.

Abstract:The early detection of wound infection in situ can dramatically improve patient care pathways and clinical outcomes. There is increasing evidence that within an infected wound the main bacterial mode of living is a biofilm: a confluent community of adherent bacteria encased in an extracellular polymeric matrix. Here we have reported the development of a prototype wound dressing, which switches on a fluorescent color when in contact with pathogenic wound biofilms. The dressing is made of a hydrated agarose film in which the fluorescent dye containing vesicles were mixed with agarose and dispersed within the hydrogel matrix. The static and dynamic models of wound biofilms, from clinical strains of Escherichia coli, Pseudomonas aeruginosa, Staphylococcus aureus, and Enterococcus faecalis, were established on nanoporous polycarbonate membrane for 24, 48, and 72 h, and the dressing response to the biofilms on the prototype dressing evaluated. The dressing indicated a clear fluorescent/color response within 4 h, only observed when in contact with biofilms produced by a pathogenic strain. The sensitivity of the dressing to biofilms was dependent on the species and strain types of the bacterial pathogens involved, but a relatively higher response was observed in strains considered good biofilm formers. There was a clear difference in the levels of dressing response, when dressings were tested on bacteria grown in biofilm or in planktonic cultures, suggesting that the level of expression of virulence factors is different depending of the growth mode. Colorimetric detection on wound biofilms of prevalent pathogens (S. aureus, P. aeruginosa, and E. faecalis) is also demonstrated using an ex vivo porcine skin model of burn wound infection.

Abstract:Abstract Chronic nonhealing wounds, such as pressure sores, venous leg ulcers and diabetic foot ulcers are a silent pandemic, and represent a significant economic and health burden, with billions exhausted per annum in the UK and USA alone. Chronic wounds are at high risk of infection with biofilms that are difficult to treat with broad-spectrum antibiotics. Another option is to develop a nontoxic dressing that can physically remove the live bacteria, thus reducing the risk of antimicrobial resistance (AMR). Thousands of wound dressings are commercially available to combat various degrees of wounding. Polydimethylsiloxane (PDMS), a biocompatible and inexpensive polymer to fabricate, can undergo various modifications. The ability of fabricated, patterned polymers to attract the opportunist Staphylococcus aureus and Pseudomonas aeruginosa, either from the planktonic state or while sessile in biofilms, on ex vivo human skin was investigated. These are the most common species isolated from chronic wounds, contributing to prolific biofilm formation, and decreasing antimicrobial efficiency. Different combinations of PDMS polymers patterned (PT) or flat (FL) ± triclosan (1% or 10%) were assessed with regard to their ability for selective bacterial attachment. Patterned PDMS + 10% triclosan (PT 10%) attracted significantly more live S. aureus and P. aeruginosa, as determined via colony-forming units (CFU) mL–1 (P < 0.01), scanning electron microscopy (P < 0.01) and confocal scanning laser microscopy (P < 0.01). The released triclosan was not cytotoxic against either type of bacteria (P < 0.05) or primary cultures of human dermal fibroblasts (P < 0.05). High performance liquid chromatography analysis of the culture medium confirmed a low level of phase release. Dermal fibroblast viability increased in the presence of PDMS, when subject to infection in co-culture via the Boyden chamber assay (P < 0.05). Compared to all the other polymer combinations, PT 10% demonstrated superior removal of biofilms that had been established on human ex vivo epidermis (P < 0.5). Histological examination of ex vivo full-thickness human skin confirmed the superior attraction qualities of PT 10% on the removal of the bacterial biofilm. In summary, the unique topography of PDMS combined with triclosan attracted a substantial number of live bacteria, in their planktonic state, and, significantly, when in an established biofilm. A coinfection model demonstrated the protective nature of the polymer on cultured dermal fibroblast viability by drawing the planktonic bacteria away from them in a co-culture model. Successful establishment of biofilms on epidermal sheets from full-thickness skin allowed the demonstration of the favourable polymer topography for biofilm removal from human skin. Defined patterned polymers also presented with higher attachment rates than nonpatterned polymers, highlighting the potential for engineering specific patterned polymers to transfer biofilms physically from wounds. Significantly, these polymers lack bactericidal properties, which is vital in the quest to combat antimicrobial resistance.

Abstract:Biofilm development is a potent inducer for many microorganisms that inflict chronic infections. Conventional antimicrobials face significant hurdles in dealing with biofilms and drug resistance because of the multi-factor nature of these processes. Despite this, bacterial biofilms remain a mystery, and their management methods are still lacking. Biofilms may lead to persistent illness and need surgical excision of affected regions if standard antimicrobial treatments fail to eliminate them. One of the most significant concerns in antibacterial medication research is discovering effective treatments for biofilm infections. Recent wound biofilm infection therapy research focuses on nanotechnology-based medication delivery systems and combination therapies. Biofilm-targeting medications are still in the early stages of research, although several promising techniques are already being tested. Current understanding of biofilm physiology and pathology is reviewed and discussed here, as is how a thorough understanding of biofilm physical and biological properties might influence treatment techniques and molecular targets for the creation of anti-biofilm medications. Specifically, we are interested in treatment approaches targeting the extracellular matrix, dormant cells, additional crucial microbial biofilm structure, functional features, and drug tolerance mechanisms. In many clinical investigations, silver has been found to improve the healing of chronic, non-healing wounds, including silver nanoparticles. The purpose of this review is to provide an overview of recent developments in silver nanoparticle-based biofilm therapeutics and to understand the current development path.

Abstract:Chronic wounds, especially diabetic, foot and pressure ulcers are a major health problem affecting >10 % of the world's populace. Calcium phosphate materials, particularly, bioactive glasses (BG), used as a potential material for hard and soft tissue repair. This study combines nanostructured 45S5 BG with titania (TiO 2 ) and alumina (Al 2 O 3 ) into a composite via simple sol-gel method. Prepared composites with alginate (Alg) formed a bioactive nanocomposite hydrogel membrane via freezing method. X-ray diffraction revealed formation of two phases such as Na 1.8 Ca 1.1 Si 6 O 14 and β-Na 2 Ca 4 (PO 4 ) 2 SiO 4 in the silica network. Fourier transformed InfraRed spectroscopy confirmed the network formation and cross-linking between composite and alginate. <2 % hemolysis, optimal in vitro degradation and porosity was systematically evaluated up to 7 days, resulting in increasing membrane bioactivity. Significant cytocompatibility, cell migration and proliferation and a 3–4-fold increase in Collagen (Col) and Vascular Endothelial Growth Factor (VEGF) expression were obtained. Sustained delivery of 80 % Dox in 24 h and effective growth reduction of S. aureus and destruction of biofilm development against E. coli and S. aureus within 24 h. Anatomical fin regeneration, rapid re-epithelialization and wound closure were achieved within 14 days in both zebrafish and in streptozotocin (STZ) induced rat in vivo animal models with optimal blood glucose levels. Hence, the fabricated bioactive membrane can act as effective wound dressing material, for diabetic chronic infectious wounds.

Abstract:Recombinant human thrombopoietin, or pegylated recombinant human megakaryocyte growth and development factor, produces log-linear increases in megakaryocytopiesis and platelet production. Recent clinical studies investigating the efficacy, safety, and cost benefit of administering platelet growth factors indicate that appropriate dosing of pegylated recombinant human megakaryocyte growth and development factor, or recombinant human thrombopoietin, during chemotherapeutic marrow suppression produces these increases in four ways. First, these factors improve ensuant thrombocytopenia without producing platelet-dependent thrombo-occlusive complications. Second, recombinant human thrombopoietin or pegylated recombinant human megakaryocyte growth and development factor mobilize hematopoietic progenitor cells into the peripheral blood. Third, chronic dosing of HIV-infected thrombocytopenic patients with pegylated recombinant human megakaryocyte growth and development factor normalizes peripheral platelet counts without antibody formation. Fourth, the administration of pegylated recombinant human megakaryocyte growth and development factor to normal human volunteer platelet donors increases platelet yields nearly fourfold, with corresponding increases in peripheral platelet counts when transfused into thrombocytopenic recipients. The clinical application of platelet growth factors is continuing to be defined.

Abstract:Chronic wounds containing opportunistic bacterial pathogens are a growing problem, as they are the primary cause of morbidity and mortality in developing and developed nations. Bacteria can adhere to almost every surface, forming architecturally complex communities called biofilms that are tolerant to an individual's immune response and traditional treatments. Wound dressings are a primary source and potential treatment avenue for biofilm infections, and research has recently focused on using nanoparticles with antimicrobial activity for infection control. This Review categorizes nanoparticle-based approaches into four main types, each leveraging unique mechanisms against biofilms. Metallic nanoparticles, such as silver and copper, show promising data due to their ability to disrupt bacterial cell membranes and induce oxidative stress, although their effectiveness can vary based on particle size and composition. Phototherapy-based nanoparticles, utilizing either photodynamic or photothermal therapy, offer targeted microbial destruction by generating reactive oxygen species or localized heat, respectively. However, their efficacy depends on the presence of light and oxygen, potentially limiting their use in deeper or more shielded biofilms. Nanoparticles designed to disrupt extracellular polymeric substances directly target the biofilm structure, enhancing the penetration and efficacy of antimicrobial agents. Lastly, nanoparticles that induce biofilm dispersion represent a novel strategy, aiming to weaken the biofilm's defense and restore susceptibility to antimicrobials. While each method has its advantages, the selection of an appropriate nanoparticle-based treatment depends on the specific requirements of the wound environment and the type of biofilm involved. The integration of these nanoparticles into wound dressings not only promises enhanced treatment outcomes but also offers a reduction in the overall use of antibiotics, aligning with the urgent need for innovative solutions in the fight against antibiotic-tolerant infections. The overarching objective of employing these diverse nanoparticle strategies is to replace antibiotics or substantially reduce their required dosages, providing promising avenues for biofilm infection management.
