Abstract:Over one in five households with children has at least one child with a special health care need (CSHCN). Child health caregiving can bleed into paid work time. This research analyzes what factors influence work decisions (who reduces work and by how much) in married-couple families with CSHCN. This article uses data from the Medical Expenditure Panel Survey to examine the specifics of changes in parental work status and a comparison of family/work trade-offs made by parents in families with and without a CSHCN. Results indicate that mothers are more likely to experience negative work changes than fathers. Both mothers and fathers with CSHCN are more likely to report missing work than parents of children without special health care needs. Overall, when children receive treatment in a primary care practice that serves as a medical home, parents are less likely to experience negative employment changes.

Abstract:One in five U.S. households with children has at least one child with a special health care need (USDHHS, 2004). Like most parents, those with children with special health care needs struggle to balance child-rearing responsibilities with employment demands. This research examines factors affecting married parents' and single-mother's employment change decisions focusing specifically on whether having a medical home influences these decisions. This study includes 38,569 children with special health care needs from birth through age 17 surveyed in the 2005-2006 National Survey of Children with Special Health Care Needs. The employment model is estimated using multinomial logistic regression with the choice of a parent to maintain their current level of employment, reduce work hours, or stop working as the dependent variable. Independent variables are those characterizing the needs of the child, the resources of the family, and the socio-demographic characteristics of the family. Components of the medical home variable include: 1) having a usual source of care; 2) care provided is "family centered"; 3) receipt of care coordination services; and 4) receipt of needed referrals. Half of the children in our sample met criteria in all four facets. If the child has a medical home, the relative risk of a parent choosing to cut hours rather than not change hours decreases by 51%. The relative risk of choosing to stop working rather than not change hours decreases by an estimated 64%. Care coordination services significantly reduce the odds of changing employment status. Our results suggest that the medical home is a moderating factor in parental decisions concerning change in employment status.

Abstract:Journal of Family Issues, Ahead of Print. Immigrant parents may struggle to secure and maintain employment if they raise children with disabilities. The current study examines data from immigrant parents responding to the 2018 National Survey of Children's Health, including 618 raising children with special health care needs (CSHCN) and 2937 parenting children needing typical care. Parents of CSHCN had higher levels of both aggravation and resilience, but less access to a medical home than parents with typical care responsibilities. Immigrants were less likely to change jobs, but more likely to cut back work hours, or quit work, if they had CSHCNs lacking a medical home, missing more school days, and living in unsafe neighborhoods. Parents of CHSHNs also spent substantial time coordinating care and providing health care at home. Results indicate the need for service providers to connect immigrant families with CSHCNs with culturally supportive organizations that can help them handle family responsibilities while maintaining employment.

Abstract:Participation in non‐parental child care arrangements is now the norm for preschool‐age children in the U.S. However, child care services are becoming increasingly expensive for many families, and quality is highly uneven across providers and sectors, raising questions about the impact of child care costs and quality on parental employment and child development. The U.S. policy landscape is dominated by three policies that subsidize costs for low‐income families or attempt to improve the safety and quality of providers: the Child Care and Development Fund, regulations, and quality rating and improvement systems. In this paper, I provide a thorough review of the evidence on each policy, focusing on how they influence a wide range of family and provider outcomes. The paper begins with a detailed description of the structure and functioning of the child care market, using the most up‐to‐date data on families' utilization of care services and provider characteristics. I then draw on a diverse set of studies across multiple fields to summarize the evidence on the impact of child care policy. In the final section of the paper, I offer recommendations for future research in each policy area.

Abstract:BACKGROUND AND OBJECTIVES: Many children with special health care needs (CSHCN) receive health care at home from family members, but the extent of this care is poorly quantified. This study’s goals were to create a profile of CSHCN who receive family-provided health care and to quantify the extent of such care. METHODS: We analyzed data from the 2009–2010 National Survey of Children with Special Health Care Needs, a nationally representative sample of 40 242 parents/guardians of CSHCN. Outcomes included sociodemographic characteristics of CSHCN and their households, time spent by family members providing health care at home to CSHCN, and the total economic cost of such care. Caregiving hours were assessed at (1) the cost of hiring an alternative caregiver (the “replacement cost” approach), and (2) caregiver wages (the “foregone earnings” approach). RESULTS: Approximately 5.6 million US CSHCN received 1.5 billion hours annually of family-provided health care. Replacement with a home health aide would have cost an estimated $35.7 billion or $6400 per child per year in 2015 dollars ($11.6 billion or $2100 per child per year at minimum wage). The associated foregone earnings were $17.6 billion or $3200 per child per year. CSHCN most likely to receive the greatest amount of family-provided health care at home were ages 0 to 5 years, were Hispanic, lived below the federal poverty level, had no parents/guardians who had finished high school, had both public and private insurance, and had severe conditions/problems. CONCLUSIONS: US families provide a significant quantity of health care at home to CSHCN, representing a substantial economic cost.

Abstract:The financial difficulties of parents have a negative impact on the health of their children. This problem is more pronounced in single mother families. There is limited research on low-income, single mothers and how interventions to help them address financial difficulties may also benefit their children. The purpose of this study was to evaluate the effect of a year-long financial education and coaching program on school absenteeism and health care utilization of children in employed, low-income, single mother households. This was a post hoc analysis of the Finances First study, a randomized controlled trial conducted in 2017–2020 examining the impact of a financial coaching and education program on economic stability and health outcomes in 345 low-income, single mothers. Either generalized estimating equations (GEEs) or generalized linear mixed models (GLMMs) were used to account for relationships between participants. For the continuous outcomes of child absenteeism, physician visits, emergency room visits, and hospitalization days, a linear mixed-effects model was used. The Finances First study demonstrated improvements in various financial strain measures. Compared to the control group, children of intervention group participants experienced 1 fewer day of school absence (p = 0.049) and 1 fewer physician visit (p = 0.032) per year, but no impact was seen on emergency room visits (p = 0.55) or hospitalizations (p = 0.92). Addressing social determinants of health in parents is necessary for improving child health outcomes.

Abstract:This paper studies the dynamic relationship between parental labor supply, children's cognitive development and intra-household bargaining power. To do so, I construct a model that incorporates preference heterogeneity across and within households, a constrained cooperative bargaining framework with explicit outside options, a dynamic technology of the child's cognitive development, and endogenous parental human capital and wages. Using detailed U.S. panel data on married households, I identify and estimate the model with the Method of Simulated Moments, and show that it replicates observable trends and heterogeneity in parental time use, the use of formal childcare arrangements and child test scores. I use the model to simulate the effects of cash transfers and formal childcare subsidies. First, I find that the cost-effectiveness of the recently expanded Child Tax Credit, in terms of fostering child cognition and reducing intra-household inequality, could be improved by giving cash to mothers directly, and by making the credit increase with the child's age. Second, I consider expansions of the Child and Dependent Care Tax Credit which offer larger formal childcare subsidies and relax the employment requirements. I estimate that a 25 percent subsidy could boost young children's skills by over 0.1 standard deviations, increase maternal labor supply even when there is no work requirement, and be financed at no additional cost through a 50 percent reduction in the Child Tax Credit for households with young children.

Abstract:An observer measure of social phobic symptoms, referred to as the Brief Social Phobia Scale, consists of 11 items, 7 evaluating commonly feared or avoided situations and 4 additional items measuring autonomic distress. Symptoms represented by the scale items are found frequently in social phobia, and the instrument demonstrates acceptable interrater and test-retest reliability, internal consistency, concurrent validity against other measures of social phobia, and the ability of patients to change as a result of treatment. It can also detect differences between active treatment and placebo treatment. A comparison of the sensitivity of this scale with other scales in detection of size of effect also is presented.

Abstract:This study examined the longitudinal contribution of four different childcare arrangements attended during the preschool years to social behaviors and academic achievement up to age 15 years. Children participating in a Canadian longitudinal survey with available information on childcare attendance between ages 3 and 5 years (N = 6,852) were measured on multiple social behaviors (hyperactivity/inattention, depression/anxiety, disruptive behaviors) and academic outcomes (mathematic skills, academic achievement) across both childhood and adolescence. We conducted a propensity score matching analysis to control the selection bias for childcare attendance and performed generalized estimating equation models for panel data among matched groups. Our results showed no clear social or academic long-term advantage for Canadian children of attending any childcare arrangement in comparison to children being exclusively cared for by their parents. In contrast, children attending daycare centers had higher levels of hyperactivity/inattention until the age of 15 years. Children also had lower mathematic skills if attending daycare centers or informal childcare at preschool age, but this effect dissipated from childhood to adolescence. Interestingly, children from low-income families had higher levels of depression/anxiety if being cared for at home by someone other than their parents or relatives. This finding supports the dual-risk hypothesis suggesting that children from already impoverished families and attending informal childcare in their own home are at greater risk for internalizing difficulties. Abstract:Abstract Objectives Mothers of school age and older children with developmental disabilities experience poorer health than mothers of typically developing children. This review assesses the evidence for the effect on mothers’ health of caring for young children with developmental disabilities, and the influence of different disability diagnoses and socioeconomic status. Methods Medline, EMBASE, PsycINFO and CINAHL were searched. Studies measuring at least one symptom, using a quantitative scale, in mothers of preschool children (0–5 years) with and without a diagnosed developmental disability were selected. Random effects meta-analysis was performed, and predictive intervals reported due to high expected heterogeneity. Results The meta-analysis included 23 estimates of association from 14 retrospective studies for the outcomes of stress (n = 11), depressive symptoms (n = 9), general health (n = 2) and fatigue (n = 1). Caring for a child with a developmental disability was associated with greater ill health (standardised mean difference 0.87; 95% predictive interval − 0.47, 2.22). The largest association was for mixed developmental disabilities (1.36; − 0.64, 3.36) and smallest for Down syndrome (0.38; − 2.17, 2.92). There was insufficient socioeconomic information to perform subgroup analysis. The small number of studies and data heterogeneity limited the precision of the estimates of association and generalizability of the findings. Conclusions for Practice Mothers of young children with developmental disabilities may have poorer health than those with typically developing children. Research is needed to identify whether the relationship is causal and, if so, interventions that could reduce the negative effect of caregiving.

Abstract:This article examines the effects of the health insurance coverage mandate for dependents on household financial portfolio decisions by focusing on the Affordable Care Act of 2010. Using the Survey of Income and Program Participation data, the author finds that the dependent coverage mandate significantly increased (decreased) the share of stocks (bonds and other interest-accruing assets) by 2.5 (1.3 and 1.1) percentage points for households having both parental employer-sponsored health insurance and dependent children aged 19 to 25 years.
[Effects of biochar application on the vertical transport of NO(3-)-N in the red soil and its simulation].

Abstract:Soil column experiments in laboratory were conducted to determine the effect of biochar application on the vertical transport of NO(3-)-N in red soil. Biochar was mixed thoroughly with soil at rates of 0, 5, 10, 20, 30 and 40 t · hm(-2), i. e., biochar/soil ratios of 0, 2.22%, 4.459%, 8.95%, 13.37% and 17.80%. The CXTFIT 2.0 model was used to simulate the breakthrough curve of NO(3-)-N. The results were as follows: the breakthrough curve of NO(3-)-N varied remarkably with the increase of biochar application rate under saturated condition. The peak values of relative concentration (C/Co) , leaching rate and cumulative loss of NO(3-)-N all significantly decreased with the increasing biochar application rate. There existed a certain prolongation of the breakthrough curves among all treatments. The more the biochar was applied, the more obviously the break-through curve was prolonged. According to the correlation analysis between the NO(3-)-N break-through curves and soil properties, biochar affected the bulk density, organic carbon, total porosity, CEC of red soil, which would exert an effect on the breakthrough curves. The simulation value and the actual obtained value of the breakthrough curves were positively correlated with the correlation coefficients being over 0. 850 in all breakthrough curves, which indicated the CXTFIT 2.0 model could best fit the prediction of nitrate-N transport and relative infiltration. These results could provide a scientific basis for predicting the effect of biochar on nitrate-N in underground water after biochar incorporation into field.
Connecting Healthcare with Income Maximisation Services: A Systematic Review on the Health, Wellbeing and Financial Impacts for Families with Young Children

Abstract:Financial counselling and income-maximisation services have the potential to reduce financial hardship and its associated burdens on health and wellbeing in High Income Countries. However, referrals to financial counselling services are not systematically integrated into existing health service platforms, thus limiting our ability to identify and link families who might be experiencing financial hardship. Review evidence on this is scarce. The purpose of this study is to review "healthcare-income maximisation" models of care in high-income countries for families of children aged between 0 and 5 years experiencing financial difficulties, and their impacts on family finances and the health and wellbeing of parent(s)/caregiver(s) or children. A systematic review of the MEDLINE, EMBase, PsycInfo, CINAHL, ProQuest, Family & Society Studies Worldwide, Cochrane Library, and Informit Online databases was conducted according to the Preferred Reporting Items for Systemic Reviews and Meta-Analyses (PRISMA) statement. A total of six studies (five unique samples) met inclusion criteria, which reported a total of 11,603 families exposed to a healthcare-income maximisation model. An average annual gain per person of £1661 and £1919 was reported in two studies reporting one Scottish before-after study, whereby health visitors/midwives referred 4805 clients to money advice services. In another UK before-after study, financial counsellors were attached to urban primary healthcare centres and reported an average annual gain per person of £1058. The randomized controlled trial included in the review reported no evidence of impacts on financial or non-financial outcomes, or maternal health outcomes, but did observe small to moderate effects on child health and well-being. Small to moderate benefits were seen in areas relating to child health, preschool education, parenting, child abuse, and early behavioral adjustment. There was a high level of bias in most studies, and insufficient evidence to evaluate the effectiveness of healthcare-income maximisation models of care. Rigorous (RCT-level) studies with clear evaluations are needed to assess efficacy and effectiveness.
The Price of Love: Understanding the Financial and Psychosocial Costs of Caring for Children with Medical Complexities

Abstract:Parents of children with medical complexities (CMCs) report significant challenges affecting their financial, emotional and social well-being in relation to caring for their child's medical needs. The Complex Care Navigator Program was designed to provide social, emotional and economic support to parents of CMCs. This paper describes the results and outcomes of the program and the challenges experienced during the evaluation process. Overall, results suggest that the program had a positive impact on the parents' psychosocial functioning and social connections. The results demonstrate the importance of providing early screening, psychosocial intervention and peer support.

Abstract:Young children in poor communities are spending more hours in nonparental care because of policy reforms and expansion of early childhood programs. Studies show positive effects of high-quality center-based care on children's cognitive growth. Yet, little is known about the effects of center care typically available in poor communities or the effects of home-based care. Using a sample of children who were between 12 and 42 months when their mothers entered welfare-to-work programs, this paper finds positive cognitive effects for children in center care. Children also display stronger cognitive growth when caregivers are more sensitive and responsive, and stronger social development when providers have education beyond high school. Children in family child care homes show more behavioral problems but no cognitive differences.

Abstract:On average, women faculty take on more childcare responsibilities, posing barriers to career success. Work-family policies represent one solution for advancing gender equity in academia as they support parents after childbirth with benefits for children, employees, and organizations. We contribute to understanding how the availability and use of dependent care policies (paid parental leave and childcare benefits) relate to long-term research productivity trends. Based on the work-home resources model, we theorize that policy availability provides contextual resources and policy use provides personal resources, leading to improvements in an individual's research productivity after they have a child. We also examine potential gender differences in the effect of dependent care policies on research productivity trends. We leverage n = 6945 yearly top publications and h -index observations for 386 business professors from 108 universities. Consistent with our hypotheses, the availability and use of paid parental leave and childcare benefits were associated with increased research productivity trends, though the effects depend on birth order, policy, and gender to some extent. Our findings have considerable theoretical and practical implications for organizations and society.

Abstract:I estimate the effect of removing children with disabilities from Supplemental Security Income (SSI) on parental earnings and household disability receipt. Using administrative data from the Social Security Administration (SSA), I implement regression discontinuity and differencein- differences designs based on changes in SSA's budget for child medical reviews. I find that parents fully offset the SSI loss with increased earnings, and the loss of the child's SSI payment reduces disability applications by parents and siblings but does not reduce their actual disability receipt. I examine alternative hypotheses for the large parental earnings response.

Abstract:Early care and education (ECE) enables parental employment and provides a context for child development. Theory suggests that lower child care costs, through subsidized care or the provision of free or low-cost arrangements, would increase the use of ECE and parents’ employment and work hours. This paper reviews the research literature examining the effects of child care costs and availability on parental employment. In general, research suggests that reduced out-of-pocket costs for ECE and increased availability of public ECE increases ECE attendance among young children, and has positive impacts on mothers’ labor force participation and work hours. However, there is considerable heterogeneity in findings. Among U.S. studies that report the elasticity of employment to ECE price, estimates range from −0.025 to −1.1, with estimates clustering near 0.05–0.25. This indicates that a 10 % reduction in the price of child care would lead to a 0.25–11 % increase in maternal employment, likely near 0.5–2.5 %. In general, studies using more recent data or data from non-U.S. countries find smaller elasticities than those using U.S. data from the 1990s. These differences may be due to historical and cross-national differences in ECE attendance, labor force attachment, and educational attainment among mothers with young children, as well as heterogeneity in the methodological approaches and data used across studies. More research in the U.S. using contemporary data is needed, particularly given recent changes in U.S. ECE policy.
