Abstract:Objective To explore bone mineral density (BMD) level and incidence of osteoporosis in elderly male patients with chronic obstructive pulmonary disease ( COPD), and to explore the possible association between COPD and OP.Methods Ninety-four elderly male patients over 60 years old who had BMD and pulmonary function examined at the same time in our hospital from January 2013 to September 2017 were selected as the objective .The BMD values (lumbar TL2-4, femoral neck TNeck, and hip TTotal) were measured using dual energy X-ray absorptiometry (DEXA).The pulmonary ventilation parameters ( FEV1%, FEV1/FVC%, VC%, MVV%) were measured using a pulmonary function instrument.Independent samples t test or Mann Whitney U test was used to analyze the quantitative data.The qualitative data were analyzed by Chi-square test and multivariate analysis using logistic regression analysis.Results The BMD values ( TNeck and TTotal) and pulmonary ventilation indexes (FEV1%, FEV1/FVC%, VC%and MVV%) were significantly lower in elderly male patients with COPD than those in the control groups, and the differences were statistically significant.The composition ratios of osteoporosis, osteopenia, and normal BMD in elderly male COPD group and the control group were 21.2%, 55.8%, 23.1% and 9.5%, 42.9%, 47.6%, respectively.The difference was statistically significant.The influencing factors of COPD in the elderly male patients were height, body weight, BMI index, TNeck, and FEV1/FVC%.Conclusion BMD of the femoral neck and hip in elderly male patients with COPD is significantly lower than that in the control group.The incidence of osteoporosis is significantly higher than that in the control group. The higher the BMD of the femoral neck, the lower the risk of COPD.
Assessing Thoracic Vertebral Bone Mineral Density (T8-T10) for Osteoporosis Diagnosis During CT Lung Cancer Screening in Older Adults

Abstract:Introduction: Osteoporosis diagnosis often utilizes quantitative computed tomography (QCT). This study explored the validity of applying lumbar bone mineral density (LBMD) standards to thoracic vertebrae (T8-T10) for osteoporosis detection during CT lung cancer screenings. This study investigated the utility of thoracic BMD (BMD-T8-T10) for detecting osteoporosis in older persons during CT lung cancer screening. Methods: We studied 701 participants who underwent QCT scans for both LBMD and BMD-T8-T10. Osteoporosis was diagnosed using ACR criteria based on LBMD. We determined BMD-T8-T10 thresholds via a receiver operating characteristic (ROC) curve and translated BMD-T8+T9+T10 to LBMD (TTBMD) using linear regression. Kappa test was used to evaluate the accuracy of BMD-T8-T10 thresholds and TTBMD in diagnosing osteoporosis. Results: Raw BMD-T8-T10 poorly identified osteoporosis (kappa = 0.51). ROC curve analysis identified BMD-T8-T10 thresholds for osteopenia (138 mg/cm3) and osteoporosis (97 mg/cm3) with areas under the curve of 0.97 and 0.99, respectively. We normalized BMD-T8-T10 to TTBMD based on the formula: TTBMD = 0.9 × BMD-T8-T10 - 2.56. These thresholds (kappa = 0.74) and TTBMD performed well in detecting osteoporosis/osteopenia (kappa = 0.74). Conclusion: Both calculating BMD-T8-T10 threshold (138.0 mg/cm3 for osteopenia and 97 mg/cm3 for osteoporosis) and normalizing BMD-T8-T10 to LBMD demonstrated good performance in identifying osteoporosis in older adults during CT lung cancer screening.

Methods: A total of 1268 patients who underwent chest CT examination and DXA within one-month period retrospectively analyzed. The CT attenuation values of trabecular bone were measured in mid-sagittal plane from thoracic vertebra 7 (T7). Receiver operating characteristic (ROC) curves were used to evaluate the ability to diagnose low-BMD. Results: The AUC for diagnosing low BMD was larger in women than in men (0.894 vs 0.744, p < 0.05). The AUC increased gradually with the increase of age but decreased gradually with the increase in height and weight (p < 0.05). In females, when specificity was adjusted to approximately 90%, a threshold of 140.25 HU has a sensitivity of 69.3%, which is higher than the sensitivity of 36.5% in males for distinguishing low-BMD from normal. At the age of 70 or more, when specificity was adjusted to approximately 90%, a threshold of 126.31 HU has a sensitivity of 76.1%, which was higher than that of other age groups. Conclusion: For patients who had completed chest CTs, the CT values were more effective in predicting low-BMD in female, elderly, lower height, and lower weight patients. Keywords: bone mineral density, chest computed tomography, dual-energy X-ray absorptiometry, attenuation value It is projected that by 2050, the incidence of osteoporotic fractures in China will surpass a staggering 5.9 million cases, with medical expenditures soaring above the astounding sum of 20 billion dollars. 1,2 At present, lumbar and hip dual-energy X-ray absorptiometry (DXA) is the most commonly used reference standard for the diagnosis of osteoporosis. 3–5 However, over 80% of patients with osteoporotic fractures do not undergo bone mineral density (BMD) testing to reduce the risk. 6 Since DXA still has some limitations, fragility fractures also occur in patients with osteopenia defined by DXA. 6,7 Therefore, more sensitive measures are needed to identify people at low and high risk for osteoporotic fractures. 8 It has been reported that the attenuation value of Hounsfield unit (HU) expression in trabecular (non-cortical) vertebrae scanned by computed tomography (CT) can correctly reflect BMD. The utility of vertebral CT attenuation value derived from CT is used to detect low bone mineral density (low-BMD), which has been confirmed. 9,10 Reduced CT attenuation value can be used as a basis for detecting decreased BMD. Furthermore, the advantage of CT attenuation value in assessing fracture risk, implant stability, and spinal fusion compared to DXA has been studied. Previous studies found that the CT attenuation value of 7th thoracic vertebra (T7) is highly correlated with osteoporosis. 11,12 Meanwhile, studies have found that T7-T8 thoracic vertebrae have a higher risk of fracture. 13 CT scans can identify vertebral compression fractures from reconstructed sagittal images, providing an opportunity for detection and proper management. Chest CT scan is widely used in the screening of lung cancer and follow-up of pulmonary nodules, and its images carry valuable potential information about vertebral quality, especially sagittal images, which play an important role in the evaluation of vertebral morphology. 14–16 However, the optimal population of low bone mineral density predicted by CT has not been defined in the previous literature. This study fills this gap by classifying the basic clinical characteristics of different populations. The aim of this study is to use the attenuation value of the T7 on CT images and to explore its value in screening various types of people with low-BMD. Abstract:Objective To explore the related factors to bone metabolism in patients with COPD in aged men. Methods Hipbone and lumbar vertebra bone mineral density in 40 elderly male patients with COPD were measured by dual energy X-ray absorptiometry and their pulmonary function were also tested. The BGP and PINP were used as markers for bone formation and bone turnover, and D-Pyr / Cr for the bone absorbable. GH, PTH, 25- (OH) D3, CT, E2, T, IL-6, IL-8, TNF-α, leptin and IGF-I were measured, Results The levels of leptin and IGF-I in osteoporotic subjects were significantly lower than that of the non-osteoporotic subjects of the COPD group. With the bone mineral density of the COPD group as dependent variance to make multiple linear regression analysis, the results showed that the standardization regression coefficient of leptin, IGF-I and FEV1/FVC% with the bone mineral density of Ward's area were higher than other factors. Conclusions Chronic obstructive pulmonary diseases induced osteoporosis is high change-over pattern osteoporosis; The osteoclastic factors such as IL-6 and TNF-α increase and the osteoblastic factors such as IGF-1 decrease in COPD patients. The more re-absorption of bone tissues over bone formation might accelerate osteoporosis.
Disturbance of the OPG/RANK/RANKL Pathway and Systemic Inflammation in COPD Patients with Emphysema and Osteoporosis

Abstract:BACKGROUND:Osteoporosis is one of the systemic features of COPD. A correlation between the emphysema phenotype of COPD and reduced bone mineral density (BMD) is suggested by some studies, however, the mechanisms underlying this relationship are unclear. Experimental studies indicate that IL-1β, IL-6 and TNF-α may play important roles in the etiology of both osteoporosis and emphysema. The OPG/RANK/RANKL system is an important regulator of bone metabolism, and participates in the development of post-menopausal osteoporosis. Whether the OPG/RANK/RANKL pathway is involved in the pathogenesis of osteoporosis in COPD has not been studied.METHODS:Eighty male patients (current or former smokers) completed a chest CT scan, pulmonary function test, dual x-ray absorptiometry measurements and questionnaires. Among these subjects, thirty patients with normal BMD and thirty patients with low BMD were selected randomly for measurement of IL-1β, IL-6, TNF-α (flow cytometry) and OPG/RANK/RANKL (ELISA). Twenty age-matched healthy volunteers were recruited as controls.RESULTS:Among these eighty patients, thirty-six had normal BMD and forty-four had low BMD. Age, BMI and CAT score showed significant differences between these two COPD groups (p < 0.05). The low-attenuation area (LAA%) in the lungs of COPD patients was negatively correlated with lumbar vertebral BMD (r = 0.741; p < 0.0001). Forward logistic regression analysis showed that only LAA% (p = 0.005) and BMI (p = 0.009) were selected as explanatory variables. The level of IL-1β was significantly higher in the COPD patients as compared to the normal controls (p < 0.05), but the difference between the two COPD groups did not reach significance. The levels of IL-6 and TNF-α among the three groups were significantly different (p < 0.05). The level of RANKL and the RANKL/OPG ratio were significantly higher in COPD patients with low BMD compared to those with normal BMD and the normal controls (p < 0.05), and correlated negatively with lumbar vertebral BMD, but positively with LAA%.CONCLUSIONS:Radiographic emphysema is correlated with low BMD in current and former smokers with COPD. IL-1β, IL-6, TNF-α, and the osteoporosis-related protein system OPG/RANK/RANKL may have some synergetic effects on emphysema and bone loss in COPD.

Abstract:Objective: To study the correlation between systemic inflammation level and emphysema degree and bone mineral density in chronic obstructive pulmonary disease (COPD) patients and its possible mechanism. Methods: 90 patients with stable COPD who met the inclusion criteria and 50 controls in the physical examination center during the same period were recruited. All the enrolled objects have collected general clinical data, analyzed peripheral blood samples, measuring the Low-attenuation area of lung and CT value of lumbar 1 vertebra (L1-CT) by chest spiral CT. According to LAA%, COPD patients were divided into 36 cases of the non-emphysema group, 32cases of mild to moderate emphysema group, and 22 cases of severe emphysema group. The correlation between L1-CT value, LAA%, peripheral blood inflammatory factors, and pulmonary function indices in each group was analyzed and compared. Results: The HU value of L1-CT (107±32) in the COPD group was significantly lower than that in the control group (153±30), and the difference was statistically significant (P<0.05). The higher the LAA% in COPD patients was, the lower the value of L1-CT was, and the difference between groups was statistically significant. Compared with COPD patients in the non-emphysema group, peripheral blood neutrophil/lymphocyte ratio (NLR), platelet/lymphocyte ratio (PLR), and C-reactive protein (CRP) were all increased in the emphysema group, and the differences were statistically significant (P<0.05). L1-CT was negatively correlated with LAA, PLR, NLR, and CRP while uncorrelated with serum concentration of calcium and phosphorus. Conclusion: The decrease in bone density in COPD patients is closely related to the degree of emphysema. It is associated with increased levels of systemic inflammation caused by COPD itself. Early and timely broad-spectrum anti-inflammatory treatment may have certain clinical significance for the prevention and treatment of comorbidity with osteoporosis.
Changes of bone mineral density and bone metabolic markers in old male patients with stable chronic obstructive pulmonary disease

Abstract:Objective To explore the features of bone mineral density(BMD) and bone metabolic markers in old male patients with stable chronic obstructive pulmonary disease(COPD).Methods From January 2016 to December 2017,41 old male patients with COPD at stationary phase (COPD group) and 42 old males without COPD(control group) were included in Beijing Jishuitan Hospital.BMD and bone metabolic markers were analyzed.Results Proportions of osteopenia and osteoporosis in the COPD group were higher than those in the control group[43.9％ (18/41)vs 31.0％ (13/42),36.6％ (15/41)vs 14.3％ (6/42)] (P ＜0.05).BMD T-values of L1-4,femoral neck and hipbone in the COPD group were lower than those in the control group[-0.3(-1.0,0.7) vs 0.6(-0.3,2.0),-2.1(-2.5,-1.4) vs-1.5(-2.5,-1.0),-1.3(-2.0,-0.8) vs-1.1(-1.4,-0.2)](P ＜ 0.05).Levels of N-terminal propeptide of type Ⅰ collagen,β cross-linked C-telopeptide of type Ⅰ collagen,N-MID osteocaltin in the COPD group were higher and 25-hydroxyvitamin D was lower than those in the control group [(40 ± 12) μg/L vs (32 ± 12) μg/L,(0.52 ± 0.21) μg/L vs (0.42 ± 0.24) μg/L,(13 ± 4) μg/L vs (11 ±4)μg/L,(12 ±4)μg/L vs (16 ±6)μgL](P＜0.05).Conclusion BMD decreases and the incidence of osteoporosis increases in old male patients with COPD;osteoporosis secondary to COPD is a high osteo-turnover type.

Abstract:Background: Osteoporosis remains substantially underdiagnosed and undertreated worldwide. Chest low-dose computed tomography (LDCT) may provide a valuable and popular opportunity for osteoporosis screening. This study sought to evaluate the feasibility of the screening of low bone mineral density (BMD) and osteoporosis with mean attenuation values of the lower thoracic compared to upper lumbar vertebrae. The cutoff thresholds of the mean attenuation values in Hounsfield units (HU) were derived to facilitate implementation of opportunistic screening using chest LDCT. Methods: The participants aged 30 years or older who underwent chest LDCT and quantitative computed tomography (QCT) examinations from August 2018 to October 2020 in our hospital were consecutively included in this retrospective study. A region of interest (ROI) was placed in the trabecular bone of each vertebral body to measure the HU values. The correlations of mean HU values of lower thoracic (T11-T12) and upper lumbar (L1-L2) vertebrae with age and lumbar BMD obtained with QCT were performed using the Pearson correlation coefficient, respectively. The area under the curve (AUC) of the receiver operator characteristic (ROC) curve was generated to determine the cutoff thresholds for distinguishing low BMD from normal and osteoporosis from non-osteoporosis. Results: A total of 1,112 participants were included in the final study cohort (743 men and 369 women, mean age 58.2±8.9 years; range, 32-88 years). The mean HU values of T11-T12 and L1-L2 were significantly different among 3 QCT-defined BMD categories of osteoporosis, osteopenia, and normal (P<0.001). The differences in HU values between T11-T12 and L1-L2 in each category of bone status were statistically significant (P<0.001). The mean HU values of T11-T12 (r=-0.453, P<0.001) and L1-L2 (r=-0.498, P<0.001) had negative correlations with age. Positive correlations were observed between the mean HU values of T11-T12 (r=0.872, P<0.001) and L1-L2 (r=0.899, P<0.001) with BMD. The optimal cutoff thresholds for distinguishing low BMD from normal were average T11-T12 ≤157 HU [AUC =0.941, 95% confidence interval (CI): 0.925-0.954, P<0.001] and L1-L2 ≤138 HU (AUC =0.950, 95% CI: 0.935-0.962, P<0.001), as well as distinguishing osteoporosis from non-osteoporosis were average T11-T12 ≤125 HU (AUC =0.960, 95% CI: 0.947-0.971, P<0.001) and L1-L2 ≤107 HU (AUC =0.961, 95% CI: 0.948-0.972, P<0.001). There was no significant difference between the AUC values of T11-T12 and L1-L2 for low BMD (P=0.07) and osteoporosis (P=0.92) screening. Conclusions: We have conducted a study on low BMD and osteoporosis screening using mean attenuation values of lower thoracic and upper lumbar vertebrae. Assessment of mean attenuation values of T11-T12 and L1-L2 can be used interchangeably for low BMD and osteoporosis screening using chest LDCT, and their cutoff thresholds were established.
Prediction of Osteoporosis from Proximal Femoral Cortical Bone Thickness and Hounsfield Unit Value with Clinical Significance.

Abstract:BackgroundUtilizing dual-energy x-ray absorptiometry (DXA) to assess bone mineral density (BMD) was not routine in many clinical scenarios, leading to missed diagnoses of osteoporosis. The objective of this study is to obtain effective parameters from hip computer tomography (CT) to screen patients with osteoporosis and predict their clinical outcomes. MethodsA total of 375 patients with hip CT scans for intertrochanteric fracture were included. Among them, 56 patients possessed the data of both hip CT scans and DXA and were settled as a training group. The cortical bone thickness (CTh) and Hounsfield unit (HU) values were abstracted from 31 regions of interest (ROIs) of the proximal femur. In the training group, the correlations between these parameters and BMD were investigated, and their diagnostic efficiency of osteoporosis was assessed. Finally, 375 patients were divided into osteoporotic and nonosteoporotic groups based on the optimal cut-off values, and the clinical difference between subgroups was evaluated. ResultsThe CTh value of ROI 21 and the HU value of ROI 14 were moderately correlated with the hip BMD [r = 0.475 and 0.445 (p < 0.001), respectively]. The best diagnostic effect could be obtained by defining osteoporosis as CTh value < 3.19 mm in ROI 21 or HU value < 424.97 HU in ROI 14, with accuracies of 0.821 and 0.883, sensitivities of 84% and 76%, and specificities of 71% and 87%, respectively. The clinical outcome of the nonosteoporotic group was better than that of the osteoporotic group regardless of the division criteria. ConclusionThe CTh and HU values of specific cortex sites in the proximal femur were positively correlated with BMD of DXA at the hip. Thresholds for osteoporosis based on CTh and HU values could be utilized to screen osteoporosis and predict clinical outcomes.

Abstract:Osteoporosis, a systemic bone disorder, is characterized by diminished bone mass and disruption of bone tissue microarchitecture, leading to heightened bone fragility and vulnerability to fractures [1]. Disturbingly, recent findings indicate a significant surge in osteoporosis prevalence among postmenopausal women and individuals aged over 70 [2,3]. Clinically, postmenopausal osteoporosis (PMOP) escalates the risk of both asymptomatic vertebral compression fractures and symptomatic fractures, encompassing the wrist or hip [4]. Although Dual-energy X-ray Absorptiometry (DXA) currently stands as the gold standard for osteoporosis diagnosis [5], its high costs and radiation exposure pose notable drawbacks. In light of this, our study harnesses Olink proteomics to identify potential biomarkers for the diagnosis, early prevention, and treatment of PMOP. Employing the diagnostic criteria outlined in the 2022 edition of the Guidelines for Diagnosis and Treatment of Primary Osteoporosis [6] and the 2018 edition of the Chinese Quantitative CT (QCT) Diagnostic Guidelines for Osteoporosis [7], Postmenopausal women were classified into three groups based on dual-energy X-ray absorptiometry and Quantitative Computed Tomography (QCT) results: the osteoporosis group (T value=-2.5) consisting of 24 cases, the bone mass loss group (-2.5<-1.0) with 20 cases, and the control group (T value=-1.0) comprising 16 cases. Fasting serum samples were collected and subjected to biochemical analysis. Using Olink proteomics technology, based on its core protein detection patent - Proximity Extension Assay (PEA), multiple protein expression values were accurately detected from each sample. The protein expression values were standardized using IPC (Inter-plate Control) Normalization. Samples and proteins were grouped and classified based on similarity to generate a clustering heatmap. Hierarchical clustering analysis of the heatmap (depicted in Figure 1A-C) revealed a consis tent trend in the expression of differentially expressed proteins within each group. However, there were significant variations in the expression of these proteins among the different groups. Illustrated in the volcano plot of differentially expressed proteins among the groups (shown in Figure 2A-C), To validate the proteomic data, six differentially expressed proteins exhibiting substantial alterations were selected, and box plots were generated (Figure 3A-F).Further, subjects from the osteopenia and osteoporosis groups were combined (Group 2-3) and compared with the control group (Group 1) characterized by normal bone mass. Notably, in Group 2-3, the expression of ADAM-TS13, ANGPT1, and CTRC increased, while the expression of IL6, IDUA, and TRAIL-R2 decreased. Their trends of variation were consistent with the outcomes of proteomic analysis. To identify proteins that have similar or related functions, we employed the STRING online analysis tool (http://www.string-db.org/) to construct an intricate Protein-Protein Interaction (PPI) network. We then computed the Pearson correlation coefficient for pairs of such proteins and created the clustering heat map shown in Figure 4 to analyze the correlations between the different proteins. Our investigation revealed 61 differentially expressed proteins among the study samples. Differential protein Venn analysis was conducted for the comparisons between osteopenia (M) and normal bone mass (C) groups (Figure 5A), osteoporosis (S) and normal bone mass (C) groups (Figure 5B), as well as osteoporosis (S) and osteopenia (M) groups (Figure 5C). The outcomes, illustrated in Figure 5. Through ROC curve analysis, we identified 12 candidate molecules with potential relevance to PMOP: CD40-L, PGF, IL-1ra, PRSS27, TF, SCF, KIM1, PRELP, HO-1, GDF-2, MMP12, and CD4. Correspondingly, ROC curves were generated for these twelve distinct proteins. As depicted in Figure B, the AUC was 1, indicating a comprehensive discriminatory ability. Our findings underscore the unique protein profiles and their potential diagnostic significance in distinguishing different osteoporosis groups. Concurrent assessment of plasma biomarkers shows promise in facilitating early detection and prognosis of PMOP. The current study attempted to establish a diagnostic threshold for thoracic spine HU in a type 2 diabetes mellitus (T2DM) population with osteoporosis. Patients and Methods: The current study retrospectively included 334 patients with T2DM. They underwent chest CT and Dual-energy X-ray (DXA) between August 2021 and January 2022 in our hospital. HU values were measured on the resulting chest CT images at thoracic spine 11 and 12 to construct regions of interest. All patients were grouped according to the lowest T-value of DXA examination: osteoporosis, osteopenia and normal bone density. HU values were compared with T-values in each group of patients, and receiver operating characteristics curves were plotted to calculate diagnostic thresholds as well as sensitivity and specificity. Results: There was a strong correlation between the HU values of chest CT and the T-values of DXA (p < 0.01). The sensitivity for osteoporosis was 88.7% for T11 attenuation≤ 98 HU and the specificity for osteoporosis was 87.5% for T12 attenuation ≤ 117HU; the specificity for normal BMD was 85.4% for T11 attenuation ≥ 147 HU and 82% for T12 attenuation ≥ 146 HU. Conclusion: Chest CT can be used to screen patients with T2DM for opportunistic osteoporosis and help determine if they need DXA screening. The current study suggests that when the HU threshold of T11 ≤ 98/T12 ≤ 117, patients may need further osteoporosis screening. The age-standardized prevalence of diabetes mellitus was 9.6 per 1000 person-years for men and 9.2 per 1000 person-years for women. 3 Osteoporosis is a chronic disease that is often overlooked and neglected. 4,5 Patients frequently discover they have osteoporosis only after experiencing a fracture, which can lead to substantial financial losses. T2DM has been found to be one of the highest risks for the occurrence of fractures. 6 T2DM predisposes 21.1% of cases to diabetic osteoporosis. 7 However, the mechanism of its occurrence is complex and may be related to abnormal bone metabolism, deterioration of bone tissue microstructure, increased bone fragility, and the level of blood glucose control. 8–11 Osteoporotic fractures have become one of the major complications of T2DM. Hence, early screening and prevention of diabetic osteoporosis are crucial. Dual-energy X-ray (DXA) is now the gold standard in diagnosing osteoporosis. 12 However, the values measured by DXA are high due to the effects of calcification of the vessel wall, hyperplasia of the articular eminence and degenerative bone spurs, resulting in false negatives. 13 At the same time, T2DM patients have a larger BMI, which corresponds to a high area BMD, causing errors in DXA testing. 14 Due to the numerous complications associated with T2DM, patients require a significant number of annual health screenings. Abstract:Opportunistic screening for osteoporosis can be performed using low‐dose computed tomography (LDCT) imaging obtained for other clinical indications. In this study we explored the CT‐derived bone mineral density (BMD) and prevalence of osteoporosis from thoracic LDCT in a large population cohort of Chinese men and women. A total of 69,095 adults (40,733 men and 28,362 women) received a thoracic LDCT scan for the purpose of lung cancer screening between 2018 and 2019, and data were obtained for analysis from the China Biobank Project, a prospective nationwide multicenter population study. Lumbar spine (L1–L2) trabecular volumetric bone mineral density (vBMD) was derived from these scans using quantitative computed tomography (QCT) software and the American College of Radiology QCT diagnostic criteria for osteoporosis were applied. Geographic regional differences in the prevalence of osteoporosis were assessed and the age‐standardized, population prevalence of osteoporosis in Chinese men and women was estimated from the 2010 China census. The prevalence of osteoporosis by QCT for the Chinese population aged >50 years was 29.0% for women and 13.5% for men, equating to 49.0 million and 22.8 million, respectively. In women, this rate is comparable to estimates from dual‐energy X‐ray absorptiometry (DXA), but in men, the prevalence is double. Prevalence varied geographically across China, with higher rates in the southwest and lower rates in the northeast. Trabecular vBMD decreased with age in both men and women. Women had higher peak trabecular vBMD (185.4 mg/cm3) than men (176.6 mg/cm3) at age 30 to 34 years, but older women had lower trabecular vBMD (62.4 mg/cm3) than men (92.1 mg/cm3) at age 80 years. We show that LDCT‐based opportunistic screening could identify large numbers of patients with low lumbar vBMD, and that future cohort studies are now required to evaluate the clinical utility of such screening in terms of fracture prevention and supporting national health economic analyses. © 2020 The Authors. Abstract:Objective. It is imperative to popularize the tertiary prevention of chronic obstructive pulmonary disease (COPD) and to improve the diagnosis and treatment. Methods. COPD patients were divided into mild (n = 18), moderate (n = 20), severe (n = 24), and extremely severe (n = 22) groups for performing high-resolution computed tomography (HRCT) and pulmonary function test. Serum procalcitonin (PCT) and high-sensitivity C-reactive protein (hs-CRP) were detected, and the occurrence rate of acute exacerbation COPD (AECOPD) was recorded during a 12-months follow-up period. Results. With an increase in the severity grade, the HRCT indexes, including emphysema index (EI), 1st and 15th percentile of inspiratory attenuation distribution (Perc1 and Perc15), ratio of expiratory/inspiratory mean lung density (MLDex/in) and lung volume (LVex/in), and ratio of the wall thickness to the outer diameter of the lumen (TDR), as well as percentage of the wall area to the total cross-sectional area (WA%) were increased with a decreased change in relative lung volume with attenuation values between −860 and −950 HU (RVC−860to −950) and lumen area (Ai). These were correlated with the ratio of forced expiratory volume in 1 sec (FEV1) over forced vital capacity (FVC) (FEV1/FVC), the percentage of FEV1 the predicted value (FEV1%), and ratio of residual volume to total lung volume (RV/TLC). Body mass index, MLDex/in, FEV1%, FEV1/FVC, and PCT had a predictive value to AECOPD, with the combined AUC of 0.812. Conclusions. HRCT imaging effectively classifies the severity of COPD, which combined with BMI, PFT, and serum PCT can predict the risk of AECOPD.

Developing Cut-off Values for Low and Very Low Bone Mineral Density at the Thoracic Spine Using Quantitative Computed Tomography

Abstract:Osteoporosis is under-diagnosed while detectable by measuring bone mineral density (BMD) using quantitative computer tomography (QCT). Opportunistic screening for low BMD has previously been suggested using lumbar QCT. However, thoracic QCT also possesses this potential to develop upper and lower cut-off values for low thoracic BMD, corresponding to the current cut-offs for lumbar BMD. In participants referred with chest pain, lumbar and thoracic BMD were measured using non-contrast lumbar- and cardiac CT scans. Lumbar BMD cut-off values for very low (< 80 mg/cm 3 ), low (80–120 mg/cm 3 ), and normal BMD (> 120 mg/cm 3 ) were used to assess the corresponding thoracic values. A linear regression enabled identification of new diagnostic thoracic BMD cut-off values. The 177 participants (mean age 61 [range 31–74] years, 51% women) had a lumbar BMD of 121.6 mg/cm 3 (95% CI 115.9–127.3) and a thoracic BMD of 137.0 mg/cm 3 (95% CI: 131.5–142.5) , p < 0.001 . Categorization of lumbar BMD revealed 14%, 35%, and 45% in each BMD category. When applied for the thoracic BMD measurements, 25% of participants were reclassified into a lower group. Linear regression predicted a relationship of Thoracic BMD = 0.85 * Lumbar BMD + 33.5, yielding adjusted thoracic cut-off values of 136 mg/cm 3 . Significant differences in BMD between lumbar and thoracic regions were found, but a linear relationship enabled the development of thoracic upper and lower cut-off values for low BMD in the thoracic spine. As Thoracic CT scans are frequent, these findings will strengthen the utilization of CT images for opportunistic detection of osteoporosis.

Abstract:Abstract Background: The interaction between osteoporosis-related bone microenvironment (BMD)and tumor may accelerate bone metastasis. However, the relationship between osteoporosis and lung cancer bone metastasis is unknown. Methods: We performed opportunistic screening for osteoporosis using chest low-dose computerized tomography (LDCT) to assess a relationship between osteoporosis and bone metastases from lung cancer. We used quantitative computed tomography (QCT) software to measure spinal trabecular BMD; we evaluated the density distribution by the American College of Radiology QCT diagnostic criteria for osteoporosis. The diagnosis of bone metastases was based on imaging methods. We obtained optimal cut-off for age by X-tile analysis. Univariate and multivariate COX survival analysis was used to compare the relationship between osteoporosis and bone metastases from lung cancer, and whether age and sex had an effect on the relationship. Results: At baseline, 401 patients with lung cancer, mean age ± standard deviation (SD) 61.7 ± 9.7 years, underwent LDCT opportunistic screening for lung cancer and osteoporosis, including 240 men (mean age ± SD 62.3 ± 9.7 years) and 161 women (mean age ± SD 60.7 ± 9.6 years). During a median follow-up of 21.3 months, 59 patients with lung cancer progressed to bone metastases. Regression analysis showed that the risk of lung cancer bone metastasis for men with osteoporosis was adjusted hazard ratios (HR) 95% confidence interval (CI)(0.27 0.14-0.50), P<0.001. After adjusting for age and sex, regression analysis showed that the lung cancer with osteoporosis for men was adjusted HR 95%CI(0.29 0.14-0.57), P<0.001 ,and greater for patients aged 63 older (adjusted HR 95%CI, 0.32 0.15-0.69 P<0.001). The progression time of bone metastases in patients with lung cancer and osteoporosis was faster, but not statistically different than the progression time of patients without osteoporosis. Conclusion: Overall, our study showed that osteoporosis diagnosed by low-dose chest CT opportunistic screening was associated with bone metastases from lung cancer. The findings proved that osteoporosis was an important risk factor for bone metastases from lung cancer, but osteoporosis cannot accelerate the time to progression of bone metastases. Our study highlights the clinical value of screening for osteoporosis in the prevention and treatment of bone metastases from lung cancer. Vertebral bone attenuation in Hounsfield Units and prevalent vertebral fractures are associated with the short-term risk of vertebral fractures in current and ex-smokers with and without COPD: a 3-year chest CT follow-up study

Abstract:CT scans performed to evaluate chronic obstructive pulmonary disease (COPD) also enable evaluation of bone attenuation (BA; a measure of bone density) and vertebral fractures (VFs). In 1239 current/former smokers with (n = 999) and without (n = 240) COPD, the combination of BA and prevalent VFs was associated with the incident VF risk. Introduction: Chest CT scans are increasingly used to evaluate pulmonary diseases, including COPD. COPD patients have increased risk of osteoporosis and VFs. BA on CT scans is correlated with bone mineral density and prevalent VFs. The aim of this study was to evaluate the association between BA and prevalent VFs on chest CT scans, and the risk of incident VFs in current and former smokers with and without COPD. Methods: In participants of the ECLIPSE study with baseline and 1-year and 3-year follow-up CT scans, we evaluated BA in vertebrae T4-T12 and prevalent and incident VFs. Results: A total of 1239 subjects were included (mean age 61.3 ± 8.0, 61.1% men, 999 (80.6%) COPD patients). The mean BA was 155.6 ± 47.5 Hounsfield Units (HU); 253 (20.5%) had a prevalent VF and 296 (23.9%) sustained an incident VF within 3 years. BA and prevalent VFs were associated with incident VFs within 1 (per - 1SD HR = 1.38 [1.08-1.76] and HR = 3.97 [2.65-5.93] resp.) and 3 years (per - 1SD HR = 1.25 [1.08-1.45] and HR = 3.10 [2.41-3.99] resp.), while age, sex, body mass index (BMI), smoking status and history, or presence of COPD was not. In subjects without prevalent VFs and BA, and for 1-year incidence, BMI values were associated with incident fractures (1 year, BA per - 1SD HR = 1.52 [1.05-2.19], BMI per SD HR = 1.54 [1.13-2.11]; 3 years, per - 1SD HR = 1.37 [1.12-1.68]). Conclusions: On CT scans performed for pulmonary evaluation in (former) smokers with and without COPD, the combination of BA and prevalent VFs was strongly associated with the short-term risk of incident VFs.
A Novel CT-Based Fracture Risk Prediction Model for COPD Patients

Abstract:Rationale and Objectives The aim of this study was to develop and validate a novel computed tomography (CT)-based fracture risk assessment model (FRCT) specifically tailored for patients suffering from chronic obstructive pulmonary disease (COPD). Methods We conducted a retrospective analysis encompassing a cohort of 284 COPD patients, extracting data on demographics, clinical profiles, pulmonary function tests, and CT-based bone quantification metrics. The Boruta feature selection algorithm was employed to identify key variables for model construction, resulting in a user-friendly nomogram. Results Our analysis revealed that 37.32% of the patients suffered fragility fractures post-follow-up. The FRCT model, integrating age, cancellous bone volume, average cancellous bone density, high-density lipoprotein levels, and prior fracture incidence, demonstrated superior predictive accuracy over the conventional fracture risk assessment tool (FRAX), with a C-index of 0.773 in the training group and 0.797 in the validation group. Calibration assessments via the Hosmer-Lemeshow test confirmed the model's excellent fit, and decision curve analysis underscored the FRCT model's substantial positive net benefit. Conclusion The FRCT model, leveraging opportunistic CT screening, offers a highly accurate and personalized approach to fracture risk prediction in COPD patients, surpassing the capabilities of existing tools. This model is poised to become an indispensable asset for clinicians in managing osteoporotic fracture risks within the COPD population.

Predicting mixed venous oxygen saturation (SvO2) impairment in COPD patients using clinical-CT radiomics data: A preliminary study

Abstract:Background: Chronic obstructive pulmonary disease (COPD) is one of the most common chronic airway diseases in the world. Objective: To predict the degree of mixed venous oxygen saturation (SvO2) impairment in patients with COPD by modeling using clinical-CT radiomics data and to provide reference for clinical decision-making. Methods: A total of 236 patients with COPD diagnosed by CT and clinical data at Xiangyang No. 1 People's Hospital (n= 157) and Xiangyang Central Hospital (n= 79) from June 2018 to September 2021 were retrospectively analyzed. The patients were divided into group A (SvO⩾2 62%, N= 107) and group B (SvO<2 62%, N= 129). We set up training set and test set at a ratio of 7/3 and time cutoff spot; In training set, Logistic regression was conducted to analyze the differences in general data (e.g. height, weight, systolic blood pressure), laboratory indicators (e.g. arterial oxygen saturation and pulmonary artery systolic pressure), and CT radiomics (radscore generated using chest CT texture parameters from 3D slicer software and LASSO regression) between these two groups. Further the risk factors screened by the above method were used to establish models for predicting the degree of hypoxia in COPD, conduct verification in test set and create a nomogram. Results: Univariate analysis demonstrated that age, smoking history, drinking history, systemic systolic pressure, digestive symptoms, right ventricular diameter (RV), mean systolic pulmonary artery pressure (sPAP), cardiac index (CI), pulmonary vascular resistance (PVR), 6-min walking distance (6MWD), WHO functional classification of pulmonary hypertension (WHOPHFC), the ratio of forced expiratory volume in the first second to the forced vital capacity (FEV1%), and radscore in group B were all significantly different from those in group A (P< 0.05). Multivariate regression demonstrated that age, smoking history, digestive symptoms, 6MWD, and radscore were independent risk factors for SvO2 impairment. The combined model established based on the abovementioned indicators exhibited a good prediction effect [AUC: 0.903; 95%CI (0.858-0.937)], higher than the general clinical model [AUC: 0.760; 95%CI (0.701-0.813), P< 0.05] and laboratory examination-radiomics model [AUC: 0.868; 95%CI (0.818-0.908), P= 0.012]. The newly created nomogram may be helpful for clinical decision-making and benefit COPD patients. Conclusion: SvO2 is an important indicator of hypoxia in COPD, and it is highly related to age, 6MWD, and radscore. The combined model is helpful for early identification of SvO2 impairment and adjustment of COPD treatment strategies.

Abstract:Introduction: Lung transplantation (LT) recipients are at risk of bone mineral density (BMD) loss. Pre- and post-LT BMD loss has been reported in some cross-sectional studies; however, there are limited studies regarding the serial BMD change in LT recipients. The aim of this study was to investigate the serial BMD changes and the clinical characteristics associated with BMD decline. Methods: This was a single-center, retrospective observational study. BMD was serially measured in thoracic vertebral bodies (Th4, 7, 10) using computed tomography (CT) before and 3 and 12 months after LT. The frequency of osteoporosis and factors associated with pre-LT osteoporosis and post-LT BMD loss were evaluated. The frequency of post-LT compression fracture and its associated factors were also analyzed. Results: This study included 128 adult LT recipients. LT recipients had decreased BMD (151.8 ± 42.2 mg/mL) before LT compared with age-, sex-, and smoking index-matched controls (176.2 ± 35.7 mg/mL). The diagnosis of COPD was associated with pre-LT osteoporosis. LT recipients experience further BMD decline after transplantation, and the percentage of recipients classified as exhibiting osteoporosis increased from 20% at baseline to 43% at 12 months. Recipients who had been taking no or small doses of glucocorticoids before LT had rapid BMD loss after LT. Early bisphosphonate use (within 3 months) after LT attenuated BMD loss and decreased new-onset compression fracture. Conclusion: LT recipients are at high risk for BMD loss and compression fracture after LT. Early bisphosphonate use may decrease BMD loss and compression fracture.

Predicting COPD exacerbations based on quantitative CT analysis: an external validation study

Abstract:Purpose Quantitative computed tomography (CT) analysis is an important method for diagnosis and severity evaluation of lung diseases. However, the association between CT-derived biomarkers and chronic obstructive pulmonary disease (COPD) exacerbations remains unclear. We aimed to investigate its potential in predicting COPD exacerbations. Methods Patients with COPD were consecutively enrolled, and their data were analyzed in this retrospective study. Body composition and thoracic abnormalities were analyzed from chest CT scans. Logistic regression analysis was performed to identify independent risk factors of exacerbation. Based on 2-year follow-up data, the deep learning system (DLS) was developed to predict future exacerbations. Receiver operating characteristic (ROC) curve analysis was conducted to assess the diagnostic performance. Finally, the survival analysis was performed to further evaluate the potential of the DLS in risk stratification. Results A total of 1,150 eligible patients were included and followed up for 2 years. Multivariate analysis revealed that CT-derived high affected lung volume/total lung capacity (ALV/TLC) ratio, high visceral adipose tissue area (VAT), and low pectoralis muscle cross-sectional area (CSA) were independent risk factors causing COPD exacerbations. The DLS outperformed exacerbation history and the BMI, airflow obstruction, dyspnea, and exercise capacity (BODE) index, with an area under the ROC (AUC) value of 0.88 (95%CI, 0.82–0.92) in the internal cohort and 0.86 (95%CI, 0.81–0.89) in the external cohort. The DeLong test revealed significance between this system and conventional scores in the test cohorts ( p < 0.05). In the survival analysis, patients with higher risk were susceptible to exacerbation events. Conclusion The DLS could allow accurate prediction of COPD exacerbations. The newly identified CT biomarkers (ALV/TLC ratio, VAT, and pectoralis muscle CSA) could potentially enable investigation into underlying mechanisms responsible for exacerbations.
