Abstract:Stroke results in the formation of a cavity in the infarcted brain tissue. Angiogenesis and neurogenesis are poor in the cavity, preventing brain-tissue regeneration for stroke therapy. To regenerate brain tissue in the cavity, filamentous phages, the human-safe nanofiber-like bacteria-specific viruses, are genetically engineered to display many copies of RGD peptide on the sidewalls. The viral nanofibers, electrostatically coated on biocompatible injectable silk protein microparticles, not only promote adhesion, proliferation, and infiltration of neural stem cells (NSCs), but also induce NSCs to differentiate preferentially into neurons in basal medium within 3 d. After the NSC-loaded microparticles are injected into the stroke cavity of rat models, the phage nanofibers on the microparticles stimulate angiogenesis and neurogenesis in the stroke sites within two weeks for brain regeneration, leading to functional recovery of limb motor control of rats within 12 weeks. The viral nanofibers also brought about the desired outcomes for stroke therapy, such as reducing inflammatory response, decreasing thickness of astrocytes scars, and increasing neuroblasts response in the subventricular zone. As virtually any functional peptide can be displayed on the phage by genetic means, the phage nanofibers hold promise as a unique and effective injectable biomaterial for stroke therapy.

Abstract:Nucleic acid-based therapy has emerged as a revolutionary methodology for treatment of the diseases related to protein dysfunction; however, lack of systemically applicable synthetic delivery systems limits its current usage in local applications, particularly for DNA-based therapy with regard to the poor bioavailability in the systemic administrations. To overcome this obstacle, we compiled multiple chemistry-based strategies into the manufacture of the gene delivery formulations to pursue improved tolerability of DNA to the enzymatic degradation in the biological milieu and prolonged retention in the systemic circulation. Here, we constructed a distinctive multilayered functional architecture: plasmid DNA (pDNA) was electrostatically complexed with cationic poly(lysine) (polyplex) as the interior pDNA reservoir, which was further cross-linked by redox-responsive disulfide cross-linking to minimize the occurrence of polyplex disassembly through exchange reaction with the biological charged,components. Still, the pDNA reservoir was spatially protected by a sequential thermoresponsive poly(N-isopropylacrylamide) palisade as the intermediate barrier and a biocompatible hydrophilic poly(ethylene glycol) (PEG) shell with the aim of preventing the accessibility of the biological species, particularly the nuclease degradation to the pDNA payload. Subsequent investigations validated the utilities of these strategies in accomplishing prolonged blood retention. In an attempt to apply this method for tumor therapy, ligand cyclic (Arg-Gly-Asp) peptide was attached at the distal end of PEG, validating prompted tumor-targeted delivery and gene expression of the loaded antiangiogenic gene at the targeted tumor cells and accordingly exerting antiangiogenesis of the tumors for abrogation of tumor growth. Together with its excellent safe profile, the proposed formulation suggests potential utility as a practical gene delivery system for treatment of intractable diseases.
"Molecular Bypass": The Application of vascular endothelial growth factor to gene therapy of experimental limb ischemia

Abstract:Vascular endothelial growth factor (VEGF) is an endothelial cell-spcecific angiogenic growth factor. It can stimulate focal angiogenesis. To investigate the feasibility of using VEGF gene for treating limb ischemia, we constructed an eukaryotic expression vector pcDNA3/VEGF that the human VEGF121 CDNA placed under the transcriptional control of cytomegalovirus immediate-early promoter/enhancer. Utilizing gene suture, the recombinant plasmid was transferred into the hind-limb's adductor of rat of which the distal extreme of external iliac artery was ligated and the femoral artery was completely excised. With RT-PCR, Western blot, and immunohistochemistry analysis, the transcription and expression of VEGF gene were detected. Besides, results from angiography, collateral vessel measuring, and clinical symptom observing showed that the transfer of VEGF gene into the rat with experimental limb ischemia can stimulate the formation of focal neovessels, establish the collateral circulation, augment blood perfusion, and ameliorate distal limb necrosis. These findings reveal that the "Molecular Bypass" using VEGF gene may represent a new therapeutic modality for treating tissue ischemia. ? 1997 Woodland Publications.
Reiterated Targeting Peptides on the Nanoparticle Surface Significantly Promote Targeted Vascular Endothelial Growth Factor Gene Delivery to Stem Cells.

Abstract:Nonviral gene delivery vectors hold great promise for gene therapy due to the safety concerns with viral vectors. However, the application of nonviral vectors is hindered by their low transfection efficiency. Herein, in order to tackle this challenge, we developed a nonviral vector integrating lipids, sleeping beauty transposon system and 8-mer stem cell targeting peptides for safe and efficient gene delivery to hard-to-transfect mesenchymal stem cells (MSCs). The 8-mer MSC-targeting peptides, when synthetically reiterated in three folds and chemically presented on the surface, significantly promoted the resultant lipid-based nanoparticles (LBNs) to deliver VEGF gene into MSCs with a high transfection efficiency (∼52%) and long-lasting gene expression (for longer than 170 h) when compared to nonreiterated peptides. However, the reiterated stem cell targeting peptides do not enable the highly efficient gene transfer to other control cells. This work suggests that the surface presentation of the reiterated stem cell-targeting peptides on the nonviral vectors is a promising method for improving the efficiency of cell-specific nonviral gene transfection in stem cells.
Vascular Regeneration in Ischemic Hindlimb by Adeno‐Associated Virus Expressing Conditionally Silenced Vascular Endothelial Growth Factor

Abstract:Background Critical limb ischemia (CLI) is the extreme manifestation of peripheral artery disease, a major unmet clinical need for which lower limb amputation is the only option for many patients. After 2 decades in development, therapeutic angiogenesis has been tested clinically via intramuscular delivery of proangiogenic proteins, genes, and stem cells. Efficacy has been modest to absent, and the largest phase 3 trial of gene therapy for CLI reported a worsening trend of plasmid fibroblast growth factor. In all clinical trials to date, gene therapy has used unregulated vectors with limited duration of expression. Only unregulated extended expression vectors such as adeno‐associated virus (AAV) and lentivirus have been tested in preclinical models. Methods and Results We present preclinical results of ischemia (hypoxia)‐regulated conditionally silenced (CS) AAV–human vascular endothelial growth factor (hVEGF) gene delivery that shows efficacy and safety in a setting where other strategies fail. In a BALB/c mouse model of CLI, we show that gene therapy with AAV‐CS‐hVEGF, but not unregulated AAV or plasmid, vectors conferred limb salvage, protection from necrosis, and vascular regeneration when delivered via intramuscular or intra‐arterial routes. All vector treatments conferred increased capillary density, but organized longitudinal arteries were selectively generated by AAV‐CS‐hVEGF. AAV‐CS‐hVEGF therapy reversibly activated angiogenic and vasculogenic genes, including Notch,SDF1, Angiopoietin, and Ephrin‐B2. Reoxygenation extinguished VEGF expression and inactivated the program with no apparent adverse side effects. Conclusions Restriction of angiogenic growth factor expression to regions of ischemia supports the safe and stable reperfusion of hindlimbs in a clinically relevant murine model of CLI.
"All in one" lipid-polymer nanodelivery system for gene therapy of ischemic diseases

Abstract:Gene therapy offers a promising avenue for treating ischemic diseases, yet its clinical efficacy is hindered by the limitations of single gene therapy and the high oxidative stress microenvironment characteristic of such conditions. Lipid-polymer hybrid vectors represent a novel approach to enhance the effectiveness of gene therapy by harnessing the combined advantages of lipids and polymers. In this study, we engineered lipid-polymer hybrid nanocarriers with tailored structural modifications to create a versatile membrane fusion lipid-nuclear targeted polymer nanodelivery system (FLNPs) optimized for gene delivery. Our results demonstrate that FLNPs facilitate efficient cellular uptake and gene transfection via membrane fusion, lysosome avoidance, and nuclear targeting mechanisms. Upon encapsulating Hepatocyte Growth Factor plasmid (pHGF) and Catalase plasmid (pCAT), HGF/CAT-FLNPs were prepared, which significantly enhanced the resistance of C2C12 cells to H2O2-induced injury in vitro. In vivo studies further revealed that HGF/CAT-FLNPs effectively alleviated hindlimb ischemia-induced gangrene, restored motor function, and promoted blood perfusion recovery in mice. Metabolomics analysis indicated that FLNPs didn't induce metabolic disturbances during gene transfection. In conclusion, FLNPs represent a versatile platform for multi-dimensional assisted gene delivery, significantly improving the efficiency of gene delivery and holding promise for effective synergistic treatment of lower limb ischemia using pHGF and pCAT.

Abstract:Current therapies for most neurodegenerative disorders are only symptomatic in nature and do not change the course of the disease. Gene therapy plays an important role in disease modifying therapeutic strategies. Herein, we have designed and optimized a series of highly branched poly(β-amino ester)s (HPAEs) containing biodegradable disulfide units in the HPAE backbone (HPAESS) and guanidine moieties (HPAESG) at the extremities. The optimized polymers are used to deliver minicircle DNA to multipotent adipose derived stem cells (ADSCs) and astrocytes, and high transfection efficiency is achieved (77% in human ADSCs and 52% in primary astrocytes) whilst preserving over 90% cell viability. Furthermore, the top-performing candidate mediates high levels of nerve growth factor (NGF) secretion from astrocytes, causing neurite outgrowth from a model neuron cell line. This synergistic gene delivery system provides a viable method for highly efficient non-viral transfection of ADSCs and astrocytes.
Optimization of Adenovirus-Mediated Endothelial Nitric Oxide Synthase Delivery in Rat Hindlimb Ischemia

Abstract:Adenovirus-mediated overexpression of endothelial nitric oxide synthase (eNOS) induces collateral artery development and substantially increases blood flow after induction of experimental acute hindlimb ischemia. However, the optimal technique of gene delivery for this or any other form of gene therapy in limb ischemia is still unknown. The purpose of this study was to determine the effect of the two most commonly used techniques, intra-arterial and intramuscular injection, on blood flow recovery, collateral artery development, and preservation of muscle mass. We compared intra-arterial injection under vascular isolation, intra-arterial injection under transient vascular occlusion, and intramuscular injection of phosphate buffered saline (PBS) or adenovirus encoding either the eNOS (AdeNOS) or LacZ (AdlacZ) gene after induction of acute hindlimb ischemia. Delivery of AdeNOS by both intra-arterial injection techniques increased eNOS activity (22.30 versus 10.56, P<0.01), blood flow (0.90±0.02 versus 0.69±0.07, P<0.001) and collateral artery development (17.56484 versus 13.74259, P<0.05) more than by intramuscular delivery. Intra-arterial injection under transient vascular occlusion led to better preservation of muscle mass, muscle architecture, and clinical ischemic index, but led to greater transgene expression in distant organs and contralateral limb muscles. Intra-arterial injection of AdeNOS under transient vascular occlusion is the optimal technique to reverse severe hindlimb ischemia in the rat. This is the first systematic comparison of different delivery techniques used in gene therapy of experimental hindlimb ischemia.

Abstract:Rationale and Objectives. We evaluated the potential of using intravascular magnetic resonance (MR)/radiofrequency (RF) to enhance vascular endothelial growth factor (VEGF) gene therapy of in-stent neointimal hyperplasia.Materials and Methods. By using a catheter-based approach, VEGF/lentivirus was locally transferred into 10 (five paired) bilateral femoral-iliac arteries of five hypercholesterolemic pigs, whereas the right arteries were heated up to approximately 41 degrees C by using an intravascular MR/RF system. Then, identical stents were placed immediately into the bilateral VEGF-targeted arteries to create in-stent neointimal hyperplasia. At day 60 after gene/stent interventions, the targeted arteries were harvested for histological correlation.Results. X-Ray angiography-detectable in-stent stenoses were found in three of the arteries treated with VEGF genes only, whereas there were no in-stent stenoses in arteries treated by using MR/RF-heated VEGF genes. Correlative histological examination confirmed a 138% reduction in average thickness of neointimal hyperplasia in VEGF/RF-treated arteries compared with VEGF-only-treated arteries (P < .01).Conclusion. We report a potential method of using an intravascular MR/RF heating technique to enhance gene therapy of in-stent restenosis.

Abstract:Interventional radiology by balloon catheterization is frequently used to address vessel atherosclerosis. In addition, drug-loaded particles may be infused through the perforated balloon for vascular cell targeting. This may also be an interesting approach for vascular gene therapy. In an effort to understand therapeutic catheter-based gene transfer, we investigated the delivery of the coagulation gene human factor VIII (FVIII) as a model gene in a porcine model. Using ultrasound imaging, a balloon catheter was inserted into a section of femoral artery for the delivery of chitosan nanoparticles loaded with FVIII DNA. A volume of 1.5 ml (containing 75 g of DNA in approximately 300 g of nanoparticles) was infused at each of four vessel sites. Human factor VIII protein was detected in plasma samples following DNA-nanoparticle treatment, with a maximum of 4.6 ng/ml at day 14. This level of expression achieved in the pig represents approximately 3% of the normal human FVIII plasma level (100–200 ng/ml). At the time of sacrifice (day 31), approximately 3 ng/ml plasma FVIII was detected, indicating the persistence of transgene expression. Vessel sections were harvested and examined for plasmid transfection. Both human factor VIII DNA and mRNA were observed and were localized to the sections of the vessel in contact with the catheter. Plasmid copy number determined by real-time PCR was almost 1000 copies per cell in a sample taken from the middle of the vessel section in contact with the catheter, in contrast to less than 0.05 copies from a section taken slightly beyond the catheter location. These data suggest that therapeutic genes in the form of nanoparticles can be delivered through a balloon catheter in a highly localized manner and result in sustained protein expression in a large animal model. Such a delivery system may be particularly relevant for the delivery of anti-proliferative or anti-stenotic genes.
Cell-modified Bioprinted Microspheres for Vascular Regeneration

Abstract:Cell therapy is a promising strategy in which living cells or cellular materials are delivered to treat a variety of diseases. Here, we developed an electrospray bioprinting method to rapidly generate cell-laden hydrogel microspheres, which limit the migration of the captured cells and provide an immunologically privileged microenvironment for cell survival in vivo. Currently, therapeutic angiogenesis aims to induce collateral vessel formation after limb ischemia. However, the clinical application of gene and cell therapy has been impeded by concerns regarding its inefficacy, as well as the associated risk of immunogenicity and oncogenicity. In this study, hydrogel microspheres encapsulating VEGF-overexpressing HEK293T cells showed good safety via subcutaneously injecting into male C57BL/6 mice. In addition, these cell-modified microspheres effectively promoted angiogenesis in a mouse hind-limb ischemia model. Therefore, we demonstrated the great therapeutic potential of this approach to induce angiogenesis in limb ischemia, indicating that bioprinting has a bright future in cell therapy.
Cell-mediated Delivery of VEGF Modified Mrna Enhances Blood Vessel Regeneration and Ameliorates Murine Critical Limb Ischemia.

Abstract:Synthetic chemically modified mRNAs (modRNA) encoding vascular endothelial growth factor (VEGF) represents an alternative to gene therapy for the treatment of ischemic cardiovascular injuries. However, novel delivery approaches of modRNA are needed to improve therapeutic efficacy in the diseased setting. We hypothesized that cell-mediated modRNA delivery may enhance the in vivo expression kinetics of VEGF protein thus promoting more potent angiogenic effects. Here, we employed skin fibroblasts as a "proof of concept" to probe the therapeutic potential of a cell-mediated mRNA delivery system in a murine model of critical limb ischemia (CLI). We show that fibroblasts pre-treated with VEGF modRNA have the potential to fully salvage ischemic limbs. Using detailed molecular analysis we reveal that a fibroblast-VEGF modRNA combinatorial treatment significantly reduced tissue necrosis and dramatically improved vascular densities in CLI-injured limbs when compared to control and vehicle groups. Furthermore, fibroblast-delivered VEGF modRNA treatment increased the presence of Pax7+ satellite cells, indicating a possible correlation between VEGF and satellite cell activity. Our study is the first to demonstrate that a cell-mediated modRNA therapy could be an alternative advanced strategy for cardiovascular diseases.
Use of Cationic Microbubbles Targeted to P-selectin to Improve Ultrasound-Mediated Gene Transfection of Hvegf165 to the Ischemic Myocardium

Abstract:Gene therapies have been applied to the treatment of cardiovascular disease, but their use is limited by the need to deliver them to the right target. We have employed targeted contrast ultrasound-mediated gene transfection (TCUMGT) via ultrasound-targeted microbubble destruction (UTMD) to transfer therapeutic genes to specific anatomic and pathological targets. Phospholipid microbubbles (MBs) with pcDNA3.1-human vascular endothelial growth factor 165 (pcDNA3.1-hVEGF165) plasmids targeted to P-selectin (MB+P+VEGFp) were created by conjugating monoclonal antibodies against P-selectin to the lipid shell. These microbubbles were divided into four groups: microbubble only (MB), microbubble+P-selectin (MB+P), microbubble+pcDNA3.1-hVEGF165 plasmid (MB+VEGFp), and microbubble+ P-selectin+pcDNA3.1-hVEGF165 plasmid (MB+P+VEGFp). The reverse transcription polymerase chain reaction (RT-PCR) and enzyme-linked immunosorbent assay (ELISA) results showed that the VEGF gene was successfully transfected by TCUMGT and the efficiency is increased with P-selectin targeting moiety. UTMD-mediated delivery of VEGF increased myocardial vascular density and improved cardiac function, and MB+P+VEGFp delivery showed greater improvement than MB+VEGFp. This study drew support from TCUGMT technology and took advantage of targeted ultrasound contrast agent to identify ischemic myocardium, release pcDNA3.1-hVEGF165 recombinant plasmid, and improve the myocardial microenvironment, so promoting the restoration of myocardial function.

Abstract:A biomimic reconstituted high density lipoprotein (rHDL) based system, rHDL/Stearic-PEI/VEGF complexes, was fabricated as an advanced nanovector for delivering VEGF plasmid. Here, Stearic-PEI was utilized to effectively condense VEGF plasmid and to incorporate the plasmid into rHDL. The rHDL/Stearic-PEI/VEGF complexes with diameter under 100 nm and neutral surface charge demonstrated enhanced stability under the presence of bovine serum albumin. Moreover,in vitrocytotoxicity and transfection assays on H9C2 cells further revealed their superiority, as they displayed lower cytotoxicity with much higher transfection efficiency when compared to PEI 10K/VEGF and Lipos/Stearic-PEI/VEGF complexes. In addition,in vivoinvestigation on ischemia/reperfusion rat model implied that rHDL/Stearic-PEI/VEGF complexes possessed high transgene capacity and strong therapeutic activity. These findings indicated that rHDL/Stearic-PEI/VEGF complexes could be an ideal gene delivery system for enhanced VEGF gene therapy of myocardial ischemia, which might be a new promising strategy for effective myocardial ischemia treatment.

Abstract:Background — Angiopoietin–1 (Ang1) and vascular endothelial growth factor (VEGF) are endothelial cell-specific growth factors. In the current study, we tested the hypothesis that co-expression of Ang1 and VEGF could have an effect on development of leakage-resistant vessels. Methods and results — Expression plasmids, pcD2 (control plasmid), pcD2/Ang1, pcD2/VEGF121, or pcD2/Ang1 +pcD2/VEGF121, were injected intramuscularly into an ischaemic hindlimb rat model, followed by electroporation. Collateral vessel development and skeletal muscle atrophy were assessed before and 7, 14, 28 days after treatment. Capillary density was significantly increased in the rats transfected with Ang1 or VEGF compared with that in the rats transfected with pcD2 alone (P < 0.05). Rats transfected with Ang1 + VEGF had the highest capillary density (P < 0.05).The mean perimeter ratio of ligated hindlimb to non-ligated hindlimb was lower with pcD2 treatment rats compared with that in the rats transfected with Ang1, VEGF or Ang1 +VEGF (P < 0.01). Limb necrosis was observed in two of 33 control rats, but none in the Ang1 and/or VEGF gene-transferred rats.The amount of extravasated Evans blue in rat ligated hindlimb muscle (7 days after treatment) treated with both Angl and VEGF was significantly less compared with that in the rats transfected with VEGF alone (P < 0.01). Conclusions —The current study demonstrated that co-expression of Ang1 and VEGF genes in the ischaemic muscle effectively develops leakage-resistant vessels in the rat model. Therefore, this approach may provide a more appropriate therapeutic strategy in ischaemic vascular diseases.

Abstract:Gene therapy has great potential in the treatment of peripheral arterial diseases (PAD). However, the commercial used carrier for gene therapy, polyethyleneimine (PEI) 25 kDa is not efficient in transfection in vivo and is difficult to degrade. In this work, PEI 1.8 kDa was cross-linked with glutaraldehyde, and glutaraldehyde-PEI (GAPEI) was obtained by a simple wet chemistry synthetic process. The prostacyclin synthase (PTGIS) plasmid was further wrapped by GAPEI to form a polymer with a size of ∼150 nm and a positive potential for transfection. The western blotting experiment showed that the level of PTGIS protein synthesis in cells transfected by GAPEI was 1.48 times higher than that of PEI25kDa. He cytotoxicity test showed that GAPEI had lower cytotoxicity. In vitro and in vivo experiments showed a significant increase in locally secreted prostacyclin (PGI2) after transfection of PTGIS plasmid. The ability of cells to establish tube and chemotaxis to stem cells increased, and these phenotypic changes may be involved in the vascular endothelial growth factor (VEGF), transforming growth factor β1 (TGF-β1), and fibroblast growth factor 2 (FGF2) pathways. This work promotes the potential applications of GAPEI-PTGIS nanoparticles for PAD therapy and provides design concepts for the synthesis of therapeutic agents for PAD therapy.

Abstract:Crude coumarin constituents of Peucedanum praeruptorum Duun. including praeruptorin A (=Pd-Ia)(2) and B (=Pd-II)(11) had an antagonistic effect on a specific platelet activating factor (PAF) in the platelet aggregation induced by several aggregating agents. This provides a new class of PAF antagonists. Studies of the structure-activity relationship with khellactone (1) derivatives and PAF antagonistic activity indicate that the cis isomers of 1 at the C-3' and C-4' positions are more favorable than trans isomers, and the acyl moiety at the C-3' position of 1 requires an appropriate molecular size.

Abstract:In this study, the effects of different combinations of the genes Vegf, Ang, and Gdnf injected both using direct virus-mediated injection (adenovirus, Ad5) and umbilical cord blood mononuclear cells (UCBCs) on the processes of stimulation of post-ischemic innervation, angiogenesis, and regeneration in skeletal muscle were investigated in a rat hindlimb chronic ischemia model. It was shown that more pronounced stimulation of angiogenesis and restoration of post-ischemic innervation were achieved both in the early (28 days post-ischemia, dpi) and late (42 dpi) terms of the experiment in the calf muscle when UCBCs delivered the combination of Ad5-Vegf and Ad5-Ang compared to the direct injection of the same vector combination into the area of ischemia. At the same time, the inclusion of Ad5-Gdnf in the combination of Ad5-Vegf and Ad5-Ang directly injected or administered by UCBCs provided a significant increase in the number of centronuclear muscle fibers, indicating stimulation of post-ischemic reparative myogenesis. This study allowed us to determine the most effective gene combinations for angiogenesis and neurogenesis, which, in the future, may serve as the basis for the development of gene and gene cell products for the treatment of chronic lower limb ischemia.

Abstract:Therapeutic angiogenesis is one promising strategy for the treatment of ischemic heart disease, which is the leading cause of death globally. In recent years, extracellular vesicles (EVs) have quickly gained much attention as a cell-free approach to stimulate angiogenesis. However, clinical applications of EVs are limited by their insufficient targeting capability. Herein, we introduce a method to enhance therapeutic angiogenesis based on platelet membrane-engineered EVs.Methods: Platelet-mimetic EVs (P-EVs) were fabricated by fusing the membranes of EVs with platelet membranes by extrusion. A mouse model of myocardial ischemia reperfusion (MI/R) was established and injected with PBS, EVs, and P-EVs to evaluate their targeting ability and therapeutic angiogenesis efficacy.Results: P-EVs inherited the adhesive proteins and natural targeting ability to injured vasculature of platelets and retained the pro-angiogenic potential of EVs. In the MI/R model, P-EVs preferentially accumulated in the injured endothelium of the ischemic hearts and enhanced the angiogenesis potency of EVs.Conclusions: This engineering strategy to modify pre-isolated EVs with platelet membranes by membrane fusion bestows EVs with the targeting ability of platelets and offers an exciting opportunity to design other targeted EVs fused with cell membranes from different sources for therapeutic angiogenesis.

Abstract:This study introduces a spatially differential plasmid DNA dual‐delivery nano hydrogel conduit fabricated with HA‐GMA/PEI (HP) for restoring extensive segmental nerve defects. It utilizes transforming growth factor β(TGF‐β) delivery via HP (TGF‐β@HP) in the proximal region to enhance nerve regeneration and nicotinamide mononucleotide adenylyltransferase 2 (NMNAT2) via HP (NMNAT2@HP) in the distal region to prevent Wallerian degeneration. The design promotes selective nerve fiber alignment, aiming for improved neuromotor and sensory function recovery. Peripheral nerve deficits give rise to motor and sensory impairments within the limb. The clinical restoration of extensive segmental nerve defects through autologous nerve transplantation often encounters challenges such as axonal mismatch and suboptimal functional recovery. These issues may stem from the limited regenerative capacity of proximal axons and the subsequent Wallerian degeneration of distal axons. To achieve the integration of sensory and motor functions, a spatially differential plasmid DNA (pDNA) dual‐delivery nanohydrogel conduit scaffold is devised. This innovative scaffold facilitates the localized administration of the transforming growth factor β (TGF‐β) gene in the proximal region to accelerate nerve regeneration, while simultaneously delivering nicotinamide mononucleotide adenylyltransferase 2 (NMNAT2) to the distal region to mitigate Wallerian degeneration. By promoting autonomous and selective alignment of nerve fiber gap sutures via structure design, the approach aims to achieve a harmonious unification of nerve regeneration, neuromotor function, and sensory recovery. It is anticipated that this groundbreaking technology will establish a robust platform for gene delivery in tissue engineering.
