Abstract:Introduction. An important element of the implementation of the concept of sustainable development in Ukraine and the concept of the development of the system of financial support in the field of health care is the modernization of the financial mechanism of medical institutions, the development of new models of financial activity in the context of ensuring their stability and sustainability. The purpose of the article is to study the main theoretical and methodological approaches to the analysis of the financial stability of health care institutions in the context of sustainable development. Methods. The set goal was achieved using general scientific, special and empirical methods of learning processes and phenomena. In particular, the methods of analysis and synthesis, comparison, and generalization are used to understand the sustainable development of society and the enterprise, the financial stability of health care institutions; scientific abstraction, structural-logical and tabular methods, structural-dynamic and coefficient analysis - to determine and analyze the financial stability of medical institutions. Results. The theoretical foundations and methodical approaches of the analysis of financial stability have been studied. The methodology was adapted to modern health care institutions that have acquired features of an entrepreneurial nature, an analysis of the medical institution was carried out, and indicators were calculated to determine the level of financial status. Conclussions. Therefore, the financial stability of communal non-profit institutions is the state of financial resources and their management, which ensures the balance of assets and liabilities in a changing external and internal environment and ensures constant solvency and investment attractiveness within the limits of an acceptable level of risk. Analysis of the financial stability of health care institutions allows us to assess their readiness to repay their debts, financial independence, trends in the level of this independence, the rationality of managing state and self-raised funds. The algorithm for determining financial stability includes the use of a balance sheet model of financial equilibrium, a three-component indicator, determination of the optimal degrees of coverage of sources of financial support for health care, and calculation of absolute and relative indicators.

Abstract:The HiT reviews are country-based reports that provide a detailed description of a health system and of policy initiatives in progress or under development. HiTs examine different approaches to the organization, financing and delivery of health services and the role of the main actors in health systems; describe the institutional framework, process, content and implementation of health and health care policies; and highlight challenges and areas that require more in-depth analysis. At independence from the Soviet Union in 1991, the Russian health system inherited an extensive, centralized Semashko system, but was quick to reform health financing by adopting a mandatory health insurance (MHI) model in 1993. MHI was introduced in order to open up an earmarked stream of funding for health care in the face of severe fiscal constraints. While the health system has evolved and changed significantly since the early 1990 s, the legacy of having been a highly centralized system focused on universal access to basic care remains. High energy prices on world markets have ensured greater macroeconomic stability, a budget surplus and improvements in living standards for most of the Russian population. However, despite an overall reduction in the poverty rate, there is a marked urban rural split and rural populations have worse health and poorer access to health services than urban populations. The increase in budgetary resources available to policy-makers have led to a number of recent federal-level health programmes that have focused on the delivery of services and increasing funding for priority areas including primary care provision in rural areas. Nevertheless, public health spending in the Russian Federation remains relatively low given the resources available. However, it is also clear that, even with the current level of financing, the performance of the health system could be improved. Provider payment mechanisms are the main obstacle to improving technical efficiency in the Russian health system, as most budget funding channelled through local government is input based. For this reason, the most recent reforms as well as legislation in the pipeline seek to ensure all health care funding is channelled through a strengthened MHI system with contracts for provider payments being made using output-based measures.
Unified system and classification of indicators for integral assessment of the performance and effectiveness of medical care organization at the regional level in the Russian Federation

Abstract:Objective: development of a unified system and classification of indicators for an integral assessment of performance and effectiveness of the organization of medical care (MC) at the regional level in the Russian Federation (RF). Material and methods. A systematic search for domestic and foreign scientific publications and a comprehensive analysis of the current regulatory legal acts in the field of health care for the presence of indicators characterizing the effectiveness of MC organization in the RF regions were performed. The search for indicators of effectiveness was carried out in analytical reports based on the results of field events of national medical research centers (NMRC) in the constituent entities of the RF for 2019, 2020 and 2021 in various profiles of MC. In total, 3019 analytical reports on the results of field events and 97 annual public reports on the results of the NMRC activities were analyzed. Results. Three options for classifying indicators were formed: according to the method of obtaining (primary, calculated, secondary qualitative); in relation to the final result (resulting, process (surrogate)); classification of parameters that determine the MC system, including in the context of profiles, forms, types, conditions of MC (18 positions). The proposed unified system and classification of indicators has a number of advantages compared with the Organisation for Economic Cooperation and Development, Centers for Medicare & Medicaid Services and Commonwealth Fund indicators adopted in a number of countries. It was established that international systems use mainly resulting indicators, while the domestic health care system uses exclusively process indicators that are not combined into a single system and are monitored by various departments. Conclusion. The presented approach to a unified system and classification of indicators for an integrated assessment of performance and effectiveness of MC organization allows to determine the priorities for the development of a monitoring system. At the same time, it should be noted that this approach requires further discussion and improvement.

Abstract:Background: The medical performance evaluation, which provides a basis for rational decision-making, is an important part of medical service research. Current progress with health services reform in China is far from satisfactory, without sufficient regulation. To achieve better progress, an effective tool for evaluating medical performance needs to be established. In view of this, this study attempted to develop such a tool appropriate for the Chinese context.Methods: Data was collected from the front pages of medical records (FPMR) of all large general public hospitals (21 hospitals) in the third and fourth quarter of 2007. Locally developed Diagnosis Related Groups (DRGs) were introduced as a tool for risk adjustment and performance evaluation indicators were established: Charge Efficiency Index (CEI), Time Efficiency Index (TEI) and inpatient mortality of low-risk group cases (IMLRG), to reflect respectively work efficiency and medical service quality. Using these indicators, the inpatient services' performance was horizontally compared among hospitals. Case-mix Index (CMI) was used to adjust efficiency indices and then produce adjusted CEI (aCEI) and adjusted TEI (aTEI). Poisson distribution analysis was used to test the statistical significance of the IMLRG differences between different hospitals.Results: Using the aCEI, aTEI and IMLRG scores for the 21 hospitals, Hospital A and C had relatively good overall performance because their medical charges were lower, LOS shorter and IMLRG smaller. The performance of Hospital P and Q was the worst due to their relatively high charge level, long LOS and high IMLRG. Various performance problems also existed in the other hospitals.Conclusion: It is possible to develop an accurate and easy to run performance evaluation system using Case-Mix as the tool for risk adjustment, choosing indicators close to consumers and managers, and utilizing routine report forms as the basic information source. To keep such a system running effectively, it is necessary to improve the reliability of clinical information and the risk-adjustment ability of Case-Mix.

Abstract:In Russia disputes on the need to abandon Compulsory Health Insurance (CHI) and return to the tax-based financing are yet to subside. At present, after the statement of the President of the Russian Federation V. Putin about the possibility to establish a state health care corporation, discussions on the issue have only escalated. Purpose. To conduct a comparative assessment of the public health financing model impact on the access and structural characteristics of health care delivery in the developed countries. Material and methods. Assessment of the potential impact of public funding models on the health system performance is carried out by analyzing variations in the main indicators of financial access, health care uptake and health status of the population, achieved in the developed countries with different health financing models. Results. Health care expenditures in countries with CHI are higher than in countries with the tax-based financing model. In countries with CHI the share of administrative expenses is slightly higher than in countries with the tax-based financing system. The share of spending on preventive care is slightly higher in countries with the tax-based financing system. There is a slightly lower level of outpatient and inpatient care uptake in countries with the tax-based financing system compared to countries with CHI. The premature mortality rate in countries with CHI is slightly lower than in countries with the tax-based system. Conclusion. The obtained data indicate that there are no significant differences in the access and structural characteristics of medical care in the health care system of the developed countries with different financing models. The main difference remains the level of health expenditures. In countries with CHI, the level of health expenditures is higher than in countries with the tax-based financing, which is largely due to the existence of a separate source of funding. The level of administrative costs in countries with CHI is also higher than in countries with the tax-based system.

Abstract:Innovations in health policy are critical to ensuring the delivery of high-quality, affordable and effective health services, consistent with the relevance of the research topic. The aim of this article is to determine the approaches to the interpretation of innovation policy in health care, as well as to consider specific problems of innovation policy of health care in the Russian Federation, European countries and the US. The study uses general scientific research methods: comparison, analysis, generalization, specification. Five directions of approaches to the definition of innovation policy in health care organizations are considered: systemic, market, patient-oriented, equity-oriented, as well as technological determinism. Specific problems of innovative healthcare policy in the Russian Federation, European countries and the USA are considered, and possible solutions to them are proposed. It is concluded that, regardless of the level of achievements in the field of innovation in health care, all countries are faced with a list of specific problems that require coordinated action in finding a joint solution for all stakeholders.

Complex Issues of Cardiovascular Diseases

Abstract:Highlights . For the first time, mapping models of morbidity and mortality from the blood circulation system diseases for assessing the performance of the health protection system in federal districts of the Russian Federation were proposed. The method makes it possible to identify low-quality health systems, and outsider regions, where it is necessary to develop measures in order to identify modifiable and non-modifiable risk factors and improve the performance of health care system both in federal districts and in the Russian Federation as a whole. Aim . To develop mapping models of morbidity and mortality from the blood circulation system diseases (BCSD) and to propose indices for assessing the performance of the health care system in different federal districts of Russia over a 10-year period (from 2010 to 2019). Methods . Reporting forms of federal statistical observation in the field of public health were used. Results . The incidence of BCSD among the population of the Russian Federation has been steadily increasing over the past two decades, increasing 2.047-fold from 2000 to 2019, while the mortality rate of the Russian population decreased 2.073-fold over the same period. Mapping models of morbidity and mortality from BCSD have been developed and indices for assessing the performance of the health protection system in different federal districts of Russia over a 10-year period (from 2010 to 2019) have been proposed. Conclusion . The mapping of low and ultra-low incidence rates of BCSD combined with high and ultra-high mortality rates from BSC among the population of the federal districts indicates low detection rates and low performance of the primary health care system in detecting BSC among the population assigned to receive primary health care according to the territorial principle. The correlation of indicators of general morbidity and mortality from HIPC in relation to health care resources allows timely assessment and finding resources in those areas that contribute to the maximum efficiency of medical organizations at minimum cost.

Abstract:The article analyzes modern approaches to medical organizations rating based on national and foreign literature, The groups of indices that are responsible for effectiveness of medical organizations from point of view of consumers of medical services and health care administration are established. According analyzed publications, rating values consider effectiveness of single processes rather than organization in whole. This served as a basis for scientific search of evaluation system that can reflect performance of medical organization contributing to its sustainable development. In 2012-2018, the Novosibirsk State Medical University carried out a study of potential of sustainable development of health care organizations. On the basis of legal analysis, sociological and expert research of opinions of administrators the key directions of activity contributing to improving efficiency of medical organizations functioning were identified. The article presents original approach to comparative assessment of health care organizations providing different types of medical care to population based on concept of sustainable development using the T. L. Saati hierarchy analysis methodology. The strategy of sustainable development supposes achievement of long-term goals of medical organizations, including not only indices of patient satisfaction with quality of medical care and organization of safe environment for their stay in health care facility, but also optimization of all processes, rational resource support, monitoring and analysis of internal environment of organization for continuous activity improvement, staff training, implementation of organizational innovations, etc. It is established that medical organizations implementing strategy of sustainable development demonstrated the best results in resource management, increasing of consumer satisfaction with quality and safety of medical care, optimization of processes to increase the number of treated patients, etc. Thus, health care institutions with clear-cut plan of actions and set of management practices to achieve long-term goals acquire ability to improve medical social indices and, as a result, population health indices.

Abstract:Significance. Changes in the social and economic development of any society pose fundamentally new challenges for the medical industry aimed at creating an accessible and effective health care system. Currently, the whole world is looking for ways to reasonably regulate the health сare system in a market economy. The purpose of the study is to conduct a chronological description of the main models of health care systems in the world and an assessment of modern trends in medical care provision to the population Material and methods. To analyze scientific literature, results of modern research on implementing various models in health care systems of the highly developed countries were used. Results. The main characteristics of the models of health care systems in the world, which are fundamentally different from each other, have been outlined: the state-funded (budgetary, public system), which is most comprehensively and conspicuously realized in the UK; predominantly the insurance system (most fully and clearly implemented in Germany and Israel) and the private (paid) system (most typical of the U.S. healthcare). Conclusion. Analysis of the global trends in health care development shows that there is no ideal model of the health care system in the world. Therefore, in order to organize an effective system of public health, it is necessary to adopt the experience of the developed countries on the basis of the state regulation of the health care system, taking into account various trends in the market economy.
Performance Evaluation of a Health Insurance in Nigeria Using Optimal Resource Use: Health Care Providers Perspectives

Abstract:BACKGROUND:Performance measures are often neglected during the transition period of national health insurance scheme implementation in many low and middle income countries. These measurements evaluate the extent to which various aspects of the schemes meet their key objectives. This study assesses the implementation of a health insurance scheme using optimal resource use domains and examines possible factors that influence each domain, according to providers' perspectives.METHODS:A retrospective, cross-sectional survey was done between August and December 2010 in Kaduna state, and 466 health care provider personnel were interviewed. Optimal-resource-use was defined in four domains: provider payment mechanism (capitation and fee-for-service payment methods), benefit package, administrative efficiency, and active monitoring mechanism. Logistic regression analysis was used to identify provider factors that may influence each domain.RESULTS:In the provider payment mechanism domain, capitation payment method (95%) performed better than fee-for-service payment method (62%). Benefit package domain performed strongly (97%), while active monitoring mechanism performed weakly (37%). In the administrative efficiency domain, both promptness of referral system (80%) and prompt arrival of funds (93%) performed well. At the individual level, providers with fewer enrolees encountered difficulties with reimbursement. Other factors significantly influenced each of the optimal-resource-use domains.CONCLUSIONS:Fee-for-service payment method and claims review, in the provider payment and active monitoring mechanisms, respectively, performed weakly according to the providers' (at individual-level) perspectives. A short-fall on the supply-side of health insurance could lead to a direct or indirect adverse effect on the demand-side of the scheme. Capitation payment per enrolees should be revised to conform to economic circumstances. Performance indicators and providers' characteristics and experiences associated with resource use can assist policy makers to monitor and evaluate health insurance implementation.

Abstract:Aim: The aim of this study is to analyze the Indian public healthcare delivery system and compare it with other countries based on their gross domestic product (GDP) classification. Materials and Methods: The study was conducted to assess the Indian healthcare delivery system and the GDP among global nations based on their GDP potential as criteria. The data from 24 high-, middle-, and low-income countries were gathered from the World Bank from year 2018 to 2021. The parameters included GDP, GDP per capita, total number of population, and healthcare delivery system. The data were collected from various electronic sources of the World Health Organization, the International Monetary Fund, and the US Census Bureau’s international data, National Health Profile 2021. The collected data were compiled and tabulated. The descriptive analysis was done using Microsoft Excel version 2016 and SPSS version 20. Results: India ranks in the high-income group based on the GDP, whereas when ranked based on GDP per capita, it drops down to the lower-middle-income group. This lag may be because of a larger population and lack of priority in public health delivery. In terms of overall expenditure, India spends US$63 million in total health expenditure, of which only US$20 million is spent by the government sector. On the other hand, the United States holds the highest rank with US$10,921 million of total health expenditure, of which US$5,552 million spent by the government sector and US$5,368 million by the private sector. Conclusion: The Indian healthcare delivery system is suffering from a shortage of public health workforce and infrastructure. In the last decade, India has seen remarkable economic development, particularly in terms of the GDP growth rate. Unfortunately, this progress does not accompany with the Indian healthcare delivery system. Still there is a shortage in public health workforce and infrastructure. India should prioritize the expenditure in healthcare delivery.
Comparative Analysis of the Healthcare Delivery System in China and Nigeria: A Review

Abstract:Objectives: Equitable distribution of quality healthcare services among different subpopulations in China and Nigeria is inconsistent. This necessitates an examination of potential avenues for improving healthcare provision in both countries. A comparative analysis of both health systems was carried out to provide insights into the progress each nation has made, the obstacles they face, and areas both countries can learn from each other. Methods: We assessed 3 key dimensions of these systems using the Hsiao analytical framework. First, we scrutinized the policy levers of the health systems. Next, we evaluated intermediate outcomes, such as access to healthcare and the quality of care. Lastly, we assessed the extent to which the health systems achieve their ultimate goals, including improving citizen health status and enhancing overall patient satisfaction with the healthcare services. Results: China surpasses Nigeria in terms of better birth outcomes, life expectancy, and management of communicable diseases. Both countries offer minimal protection against out-of-pocket spending on medical bills, which consequently exacerbates the poverty rate. Conclusions: China surpasses Nigeria in every health metrics. However, Nigeria's healthcare system offers services to a larger proportion of its populace because of the involvement of the private sector in healthcare delivery. Both countries continue to experience significant challenges, such as limited access to healthcare services in rural areas and overall patient dissatisfaction with the quality of care provided.

Abstract:Nowadays, the reforming of health care system in the Russian Federation is the most actual since country survives difficult economic conditions and is in need of development of new enhanced scientific and methodological approaches to various aspects of public health and health care, targeting increase of all types of efficiency in this system (medical, social, economic ones). For proper functioning of health care system, not only physicians, but also such specialists as physicists, engineers, mathematicians, biologists, chemists, lawyers, economists, psychologists, teachers and others are to be involved in process of developing new approaches. The multidisciplinary approach and effective interdisciplinary interaction can input into improvement of health care system based on needs of patients. To achieve the highest benefit for patient, development of comfortable conditions for all participants of diagnostic and treatment process, appropriate quality of medical care and maximal level of satisfaction of subjects in given system, it is necessary to consider segmentation of medical services market i.e. characteristics of consumers and providers. The systematic approach is to be applied to achieve this target, involving organizations of private and public sector of practical health care and scientific and educational organizations. it is needed to develop methods of early diagnostic, disease prevention programs and training of personnel in health care system, taking into account approaches of integrative and preventive medicine.

Abstract:Objective. The objective of the present article is to analyze the health care system in Ukraine at different levels, the development of medical care in Ukraine, to determine the purpose and objectives of primary health care on the basis of family medicine and the classification of medical services. Methods. The research methods are historical-logical and system-structural approaches to the analysis of the medical field of Ukraine. Modern methods of analysis, method of expert assessments, comparative analysis, etc. are used. The information base of the research is scientific works of domestic and foreign scientists, Laws of Ukraine, resolutions of the Cabinet of Ministers of Ukraine, regulations and statistical materials of the State Statistics Service of Ukraine, World Data Bank, Eurostat and the Center for Medical Statistics of Ukraine, research and analytical materials, Internet resource. Results. the current state of the primary health care system on the basis of family medicine in Ukraine and Europe has been studied. The development of the health care system from social medicine to the current health care system has been studied. The structure of the Ministry of Health has been studied, and the responsibility of the sectors for the development of modern medicine in Ukraine has been studied. The hierarchy of the health care system was studied, it was found that it includes national, regional and local levels. The purpose and objectives of medical care are identified, namely - to ensure a guaranteed amount of affordable, timely, quality and effective primary care to the patient, improve public health, preserve, restore and strengthen the health of the population. The classification of medical services is determined. The basic principles of family medicine are revealed, which include - family approach, patient orientation, preventive orientation, accessibility, equality and non-discrimination, multidisciplinary, continuity, longevity and coordination. The assessment of medical care of the population in the countries of Europe and Ukraine is carried out. To do this, we analyzed the cost of health care. International experience shows that the optimal level of health care spending is 8-10% of GDP and data from EU member states confirm this. In Ukraine, per capita health expenditures have been steadily increasing during 2010-2016, with an average of 3.8% of GDP in recent years.

Abstract:Health-related quality of life is becoming the standard measure of personal well-being and human’s health worldwide. Indicators of the population quality of life allow taking into account the subjective incidence, which is often overlooked during routine medical care. In Russia, the health-related quality of life is becoming particularly relevant in connection with the problems of an ageing population, the development of national strategies and programs in the social and healthcare systems, and the increasing prevalence of chronic diseases. The basis of the HRQoL measurement should be based on a proven and reliable methodology that generates comparable estimates of human well-being. Like any complex subjective indicator, HRQoL does not have an unambiguous direct method of measurement. The aim of this paper is a comprehensive description of the methodology for conducting and presenting the results of the study of the HRQoL using the European Quality of Life Questionnaire, EQ-5D. The paper discusses the measurement of HRQoL as a critical component of a modern health care system, describes in detail the administration of HRQoL data using the EQ-5D-3L questionnaire, and provides a methodology for analyzing and presenting the collected data in studies of population and patient health. We address the discussion to clinicians, healthcare providers, and researchers dealing with the problems of studying and measuring the health-related quality of life.

Abstract:Newcastle disease virus (NDV) has been developed as a vector for vaccine and gene therapy purposes. However, the optimal insertion site for foreign gene expression remained to be determined. In the present study, we inserted the green fluorescence protein (GFP) gene into five different intergenic regions of the enterotropic NDV VG/GA vaccine strain using reverse genetics technology. The rescued recombinant viruses retained lentogenic pathotype and displayed delayed growth dynamics, particularly when the GFP gene was inserted between the NP and P genes of the virus. The GFP mRNA level was most abundant when the gene was inserted closer to the 3' end and gradually decreased as the gene was inserted closer to the 5' end. Measurement of the GFP fluorescence intensity in recombinant virus-infected cells demonstrated that the non-coding region between the P and M genes is the optimal insertion site for foreign gene expression in the VG/GA vaccine vector.

Abstract:BACKGROUND: Hospitals are the most costly operational and really important units of health system because they consume about 50%-89% of total health resources. Therefore efficient use of resources could help in saving and reallocating the financial and physical resources.OBJECTIVES: The aim of this study was to obtain an overview of hospitals' performance status by applying different techniques, to compare similarities and differences between these methods and suggest the most comprehensive and practical method of appraisal for managers and policy makers.PATIENTS AND METHODS: This is a cross sectional study conducted in all hospitals of Ahvaz (eight hospitals affiliated with Jundishapur University of Medical Sciences and eight non-affiliated hospitals) during 2007 to 2011. Two kinds of data were collected through separate special checklists. Excel 2007 and Windeap 2.1 software were applied for data analysis.RESULTS: The present findings show that the average of bed occupancy rate (BOR) in the studied hospitals was about 65.91 ± 1.16. The maximum number of inefficient hospitals in the present study happened in the years 2007, 2008 and 2010 (four hospitals) but there were two hospitals in the third part of the present graph which had maximum level of efficiency and optimal level of productivity in the years 2007 and 2009. Data Envelopment Analysis (DEA) showed that the mean score of technical efficiency for the studied hospitals is 0.924 ± 0.105 with the minimum of 0.585 ± 0.905 for hospital number 1. Furthermore It shows that only five hospitals (31.25%) reach complete technical efficiency (TE) scores across all five years of 2007-11 (TE = 1).CONCLUSIONS: Results of the present and similar studies should be considered for the future planning and resource allocation of Iranian public hospitals. At the same time it is very important to consider need assessment results for each region according to its potentials, population under the coverage and other geographical and cultural indices. Furthermore because of potential limitations of each of the above models it is highly recommended to apply different methods of performance evaluation to reach a complete and real status view of the hospitals for future planning.

Abstract:The development of digital and information and communication technologies contributes to fundamental changes in all spheres of human life. Thanks to the Internet of Things technologies, medicine has moved to a new level of efficiency, convenience and quality of providing electronic medical services. The problems of the introduction of electronic services in the context of the development of e-governance require special consideration, potential threats in the collection, processing and storage of data in the field of healthcare are of particular concern. P u r p o s e o f t h e s t u d y is to conduct a comparative analysis of the use of modern trends in the introduction of electronic services in the healthcare sector of foreign countries and Russia with the use of Internet of Things technologies in the conditions of digitalization of the industry. M a t e r i a l s a n d m e t h o d s . The following methods were used in the work: analysis – when searching and familiarizing with various web resources, relevant programs and portals on electronic management and the Internet of Things; comparison – when comparing the stages and mechanisms of using Internet of Things technologies in the development of digitalization of healthcare in the world; analogy and extrapolation – to determine the possibilities, implementation of foreign experience. R e s u l t s . The best foreign practices of providing electronic services in the field of healthcare with the use of Internet of Things technologies were revealed. A trend has been established for the appearance and use of various devices using Internet of Things technology that are used to provide medical services, in particular those that have the functions of medical equipment and help to conduct an online examination. The US experience in the introduction of electronic medical records and the maintenance of relevant databases is disclosed. Separately, attention is focused on the experience of countries that are trying to prevent excessive personalization and identification of data. С o n c l u s i o n s . Thus, electronic services are services that are provided in electronic format online. In Russia, they are regulated by a significant list of regulatory legal acts. At the same time, for the effective use of Internet of Things technologies in the provision of electronic medical services, first of all, it is necessary to solve the problem of de-identification (depersonalization) of data that is collected to meet this service.

Abstract:The subject of the research is healthcare financing systems in European countries and Russia. The relevance of the problem stems from the possibility of using successful foreign experience for the organization of healthcare financing in Russia. The purpose of the research was to examine the specific features of the establishing and funding healthcare models in countries of Europe and the possibilities of their experience in organizing healthcare financing in the Russian Federation. The paper examines the current state of the two models of health financing in the EU — budget funding and insurance. By the example of individual European countries, the operation specifics of each model are analyzed, their advantages and disadvantages are assessed. Particular attention is paid to the specifics of the healthcare financing system in Russia and its difference from foreign systems. Taking into account the foreign experience and the need to improve the Russian compulsory healthcare insurance (CHI) system, the paper proposes ways to improve the Russian model of healthcare financing: establishment of a unified organizational and financial CHI model; introduction of a system of equal participation of the employer and employee in the financing of compulsory healthcare insurance; introduction of a family insurance system; promotion of voluntary healthcare insurance programs. It is concluded that the keypoint in the CHI system restructuring should be the search for socially acceptable and economically justified forms of attracting public funds. At the same time, it is extremely important that the funds received by medical institutions do not increase the shadow incomes in the economy but are spent on the development of the healthcare system.
