Abstract:Onboard, real-time, imaging techniques, from the original megavoltage planar imaging devices, to the emerging combined MRI-Linear Accelerators, have brought a huge transformation in the ability to deliver targeted radiation therapies. Each generation of these technologies enables lethal doses of radiation to be delivered to target volumes with progressively more accuracy and thus allows shrinking of necessary geometric margins, leading to reduced toxicities. Alongside these improvements in treatment delivery, advances in medical imaging, e.g., PET, and MRI, have also allowed target volumes themselves to be better defined. The development of functional and molecular imaging is now driving a conceptually larger step transformation to both better understand the cancer target and disease to be treated, as well as how tumors respond to treatment. A biological description of the tumor microenvironment is now accepted as an essential component of how to personalize and adapt treatment. This applies not only to radiation oncology but extends widely in cancer management from surgical oncology planning and interventional radiology, to evaluation of targeted drug delivery efficacy in medical oncology/immunotherapy. Here, we will discuss the role and requirements of functional and metabolic imaging techniques in the context of brain tumors and metastases to reliably provide multi-parametric imaging biomarkers of the tumor microenvironment.

Abstract:Medical image processing and analysis (also known as Radiomics) is a rapidly growing discipline that maps digital medical images into quantitative data, with the end goal of generating imaging biomarkers as decision support tools for clinical practice. The use of imaging data from routine clinical work-up has tremendous potential in improving cancer care by heightening understanding of tumor biology and aiding in the implementation of precision medicine. As a noninvasive method of assessing the tumor and its microenvironment in their entirety, radiomics allows the evaluation and monitoring of tumor characteristics such as temporal and spatial heterogeneity. One can observe a rapid increase in the number of computational medical imaging publications-milestones that have highlighted the utility of imaging biomarkers in oncology. Nevertheless, the use of radiomics as clinical biomarkers still necessitates amelioration and standardization in order to achieve routine clinical adoption. This Review addresses the critical issues to ensure the proper development of radiomics as a biomarker and facilitate its implementation in clinical practice.

Abstract:Medical imaging can assess the tumor and its environment in their entirety, which makes it suitable for monitoring the temporal and spatial characteristics of the tumor. Progress in computational methods, especially in artificial intelligence for medical image process and analysis, has converted these images into quantitative and minable data associated with clinical events in oncology management. This concept was first described as radiomics in 2012. Since then, computer scientists, radiologists, and oncologists have gravitated towards this new tool and exploited advanced methodologies to mine the information behind medical images. On the basis of a great quantity of radiographic images and novel computational technologies, researchers developed and validated radiomic models that may improve the accuracy of diagnoses and therapy response assessments. Here, we review the recent methodological developments in radiomics, including data acquisition, tumor segmentation, feature extraction, and modelling, as well as the rapidly developing deep learning technology. Moreover, we outline the main applications of radiomics in diagnosis, treatment planning and evaluations in the field of oncology with the aim of developing quantitative and personalized medicine. Finally, we discuss the challenges in the field of radiomics and the scope and clinical applicability of these methods.

Abstract:In the past decade, the field of medical image analysis has grown exponentially, with an increased number of pattern recognition tools and an increase in data set sizes. These advances have facilitated the development of processes for high-throughput extraction of quantitative features that result in the conversion of images into mineable data and the subsequent analysis of these data for decision support; this practice is termed radiomics. This is in contrast to the traditional practice of treating medical images as pictures intended solely for visual interpretation. Radiomic data contain first-, second-, and higher-order statistics. These data are combined with other patient data and are mined with sophisticated bioinformatics tools to develop models that may potentially improve diagnostic, prognostic, and predictive accuracy. Because radiomics analyses are intended to be conducted with standard of care images, it is conceivable that conversion of digital images to mineable data will eventually become routine practice. This report describes the process of radiomics, its challenges, and its potential power to facilitate better clinical decision making, particularly in the care of patients with cancer.

Abstract:The integration of machine learning (ML) with radiotherapy has emerged as a pivotal innovation in outcome prediction, bringing novel insights amid unique challenges. This review comprehensively examines the current scope of ML applications in various treatment contexts, focusing on treatment outcomes such as patient survival, disease recurrence, and treatment‐induced toxicity. It emphasizes the ascending trajectory of research efforts and the prominence of survival analysis as a clinical priority. We analyze the use of several common medical imaging modalities in conjunction with clinical data, highlighting the advantages and complexities inherent in this approach. The research reflects a commitment to advancing patient‐centered care, advocating for expanded research on abdominal and pancreatic cancers. While data collection, patient privacy, standardization, and interpretability present significant challenges, leveraging ML in radiotherapy holds remarkable promise for elevating precision medicine and improving patient care outcomes.

Abstract:Patients diagnosed with metastatic breast cancer (mBC) typically undergo several radiographic assessments during their treatment. mBC often involves multiple metastatic lesions in different organs, it is imperative to accurately track and assess these lesions to gain a comprehensive understanding of the disease's response to treatment. Computerized analysis methods that rely on lesion-level tracking have often used manual matching of corresponding lesions, a time-consuming process that is prone to errors. This paper introduces an automated lesion correspondence algorithm designed to precisely track both targets' lesions and non-targets' lesions in longitudinal data. Here we demonstrate the applicability of our algorithm on the anonymized data from two Phase III trials. The dataset contains imaging data of patients for different follow-up timepoints and the radiologist annotations for the patients enrolled in the trials. Target and non-target lesions are annotated by either one or two groups of radiologists. To facilitate accurate tracking, we have developed a registration-assisted lesion correspondence algorithm. The algorithm employs a sequential two-step pipeline: (a) Firstly, an adaptive Hungarian algorithm is used to establish correspondence among lesions within a single volumetric image series which have been annotated by multiple radiologists at a specific timepoint. (b) Secondly, after establishing correspondence and assigning unique names to the lesions, three-dimensional rigid registration is applied to various image series at the same timepoint. Registration is followed by ongoing lesion correspondence based on the adaptive Hungarian algorithm and updating lesion names for accurate tracking. Validation of our automated lesion correspondence algorithm is performed through triaxial plots based on axial, sagittal, and coronal views, confirming its efficacy in matching lesions.

For this purpose, a general structure is extracted from some of the researches in the literature. This structure includes a number of essential elements, each of which having a few design choices, namely 1) tracked features, 2) models mathematical expression and random effects and finally 3) response prediction. Two research study examples in Alzheimers disease and prostate tomography are also briefly introduced to further discuss the above design choices.

Abstract:Current clinical decision-making in oncology relies on averages of large patient populations to both assess tumor status and treatment outcomes. However, cancers exhibit an inherent evolving heterogeneity that requires an individual approach based on rigorous and precise predictions of cancer growth and treatment response. To this end, we advocate the use of quantitative in vivo imaging data to calibrate mathematical models for the personalized forecasting of tumor development. In this chapter, we summarize the main data types available from both common and emerging in vivo medical imaging technologies, and how these data can be used to obtain patient-specific parameters for common mathematical models of cancer. We then outline computational methods designed to solve these models, thereby enabling their use for producing personalized tumor forecasts in silico, which, ultimately, can be used to not only predict response, but also optimize treatment. Finally, we discuss the main barriers to making the above paradigm a clinical reality.

Abstract:Bilateral subconjunctival haemorrhage was observed on 5 separate occasions in 4 patients presenting a fulminant form of acute severe asthma. Four of these episodes required ventilatory support. All patients recovered from their asthma attack after treatment with bronchodilators, corticosteroids, and oxygen. The incidence of this previously unreported finding represents 5% of the total number of patients with acute severe asthma mechanically ventilated in our intensive care unit from 1976.

Abstract:The role of medical image computing in oncology is growing stronger, not least due to the unprecedented advancement of computational AI techniques, providing a technological bridge between radiology and oncology, which could significantly accelerate the advancement of precision medicine throughout the cancer care continuum. Medical image processing has been an active field of research for more than three decades, focusing initially on traditional image analysis tasks such as registration segmentation, fusion, and contrast optimization. However, with the advancement of model-based medical image processing, the field of imaging biomarker discovery has focused on transforming functional imaging data into meaningful biomarkers that are able to provide insight into a tumor’s pathophysiology. More recently, the advancement of high-performance computing, in conjunction with the availability of large medical imaging datasets, has enabled the deployment of sophisticated machine learning techniques in the context of radiomics and deep learning modeling. This paper reviews and discusses the evolving role of image analysis and processing through the lens of the abovementioned developments, which hold promise for accelerating precision oncology, in the sense of improved diagnosis, prognosis, and treatment planning of cancer.

Abstract:"Radiomics" refers to the extraction and analysis of large amounts of advanced quantitative imaging features with high throughput from medical images obtained with computed tomography, positron emission tomography or magnetic resonance imaging. Importantly, these data are designed to be extracted from standard-of-care images, leading to a very large potential subject pool. Radiomics data are in a mineable form that can be used to build descriptive and predictive models relating image features to phenotypes or gene-protein signatures. The core hypothesis of radiomics is that these models, which can include biological or medical data, can provide valuable diagnostic, prognostic or predictive information. The radiomics enterprise can be divided into distinct processes, each with its own challenges that need to be overcome: (a) image acquisition and reconstruction, (b) image segmentation and rendering, (c) feature extraction and feature qualification and (d) databases and data sharing for eventual (e) ad hoc informatics analyses. Each of these individual processes poses unique challenges. For example, optimum protocols for image acquisition and reconstruction have to be identified and harmonized. Also, segmentations have to be robust and involve minimal operator input. Features have to be generated that robustly reflect the complexity of the individual volumes, but cannot be overly complex or redundant. Furthermore, informatics databases that allow incorporation of image features and image annotations, along with medical and genetic data, have to be generated. Finally, the statistical approaches to analyze these data have to be optimized, as radiomics is not a mature field of study. Each of these processes will be discussed in turn, as well as some of their unique challenges and proposed approaches to solve them. The focus of this article will be on images of non-small-cell lung cancer.

Abstract:Purpose-Optimal use of established and imaging methods, such as multiparametric magnetic resonance imaging(mpMRI) can simultaneously identify key functional parameters and provide unique imaging phenotypes of breast cancer. Therefore, we have developed and implemented a new machine-learning informatic system that integrates clinical variables, derived from imaging and clinical health records, to compare with the 21-gene array assay, OncotypeDX. Materials and methods-We tested our informatics modeling in a subset of patients (n=81) who had ER+ disease and underwent OncotypeDX gene expression and breast mpMRI testing. The machine-learning informatic method is termed Integrated Radiomic Informatic System-IRIS was applied to the mpMRI, clinical and pathologic descriptors, as well as a gene array analysis. The IRIS method using an advanced graph theoretic model and quantitative metrics. Summary statistics (mean and standard deviations) for the quantitative imaging parameters were obtained. Sensitivity and specificity and Area Under the Curve were calculated for the classification of the patients. Results-The OncotypeDX classification by IRIS model had sensitivity of 95% and specificity of 89% with AUC of 0.92. The breast lesion size was larger for the high-risk groups and lower for both low risk and intermediate risk groups. There were significant differences in PK-DCE and ADC map values in each group. The ADC map values for high- and intermediate-risk groups were significantly lower than the low-risk group. Conclusion-These initial studies provide deeper understandings of imaging features and molecular gene array OncotypeDX score. This insight provides the foundation to relate these imaging features to the assessment of treatment response for improved personalized medicine.

Abstract:Background Radiomics can provide in-depth characterization of cancers for treatment outcome prediction. Conventional radiomics rely on extraction of image features within a pre-defined image region of interest (ROI) which are typically fed to a classification algorithm for prediction of a clinical endpoint. Deep learning radiomics allows for a simpler workflow where images can be used directly as input to a convolutional neural network (CNN) with or without a pre-defined ROI. Purpose The purpose of this study was to evaluate (i) conventional radiomics and (ii) deep learning radiomics for predicting overall survival (OS) and disease-free survival (DFS) for patients with head and neck squamous cell carcinoma (HNSCC) using pre-treatment 18 F-fluorodeoxuglucose positron emission tomography (FDG PET) and computed tomography (CT) images. Materials and methods FDG PET/CT images and clinical data of patients with HNSCC treated with radio(chemo)therapy at Oslo University Hospital (OUS; n = 139) and Maastricht University Medical Center (MAASTRO; n = 99) were collected retrospectively. OUS data was used for model training and initial evaluation. MAASTRO data was used for external testing to assess cross-institutional generalizability. Models trained on clinical and/or conventional radiomics features, with or without feature selection, were compared to CNNs trained on PET/CT images without or with the gross tumor volume (GTV) included. Model performance was measured using accuracy, area under the receiver operating characteristic curve (AUC), Matthew’s correlation coefficient (MCC), and the F1 score calculated for both classes separately. Results CNNs trained directly on images achieved the highest performance on external data for both endpoints. Adding both clinical and radiomics features to these image-based models increased performance further. Conventional radiomics including clinical data could achieve competitive performance. However, feature selection on clinical and radiomics data lead to overfitting and poor cross-institutional generalizability. CNNs without tumor and node contours achieved close to on-par performance with CNNs including contours. Conclusion High performance and cross-institutional generalizability can be achieved by combining clinical data, radiomics features and medical images together with deep learning models. However, deep learning models trained on images without contours can achieve competitive performance and could see potential use as an initial screening tool for high-risk patients.

Abstract:Abstract Over the past years, the quantity and complexity of imaging data available for the clinical management of patients with solid tumors has increased substantially. Without the support of methods from the field of artificial intelligence (AI) and machine learning, a complete evaluation of the available image information is hardly feasible in clinical routine. Especially in radiotherapy planning, manual detection and segmentation of lesions is laborious, time consuming, and shows significant variability among observers. Here, AI already offers techniques to support radiation oncologists, whereby ultimately, the productivity and the quality are increased, potentially leading to an improved patient outcome. Besides detection and segmentation of lesions, AI allows the extraction of a vast number of quantitative imaging features from structural or functional imaging data that are typically not accessible by means of human perception. These features can be used alone or in combination with other clinical parameters to generate mathematical models that allow, for example, prediction of the response to radiotherapy. Within the large field of AI, radiomics is the subdiscipline that deals with the extraction of quantitative image features as well as the generation of predictive or prognostic mathematical models. This review gives an overview of the basics, methods, and limitations of radiomics, with a focus on patients with brain tumors treated by radiation therapy.

Abstract:Recent advancements in signal processing and machine learning coupled with developments of electronic medical record keeping in hospitals and the availability of extensive set of medical images through internal/external communication systems, have resulted in a recent surge of significant interest in "Radiomics". Radiomics is an emerging and relatively new research field, which refers to extracting semi-quantitative and/or quantitative features from medical images with the goal of developing predictive and/or prognostic models, and is expected to become a critical component for integration of image-derived information for personalized treatment in the near future. The conventional Radiomics workflow is typically based on extracting pre-designed features (also referred to as hand-crafted or engineered features) from a segmented region of interest. Nevertheless, recent advancements in deep learning have caused trends towards deep learning-based Radiomics (also referred to as discovery Radiomics). Considering the advantages of these two approaches, there are also hybrid solutions developed to exploit the potentials of multiple data sources. Considering the variety of approaches to Radiomics, further improvements require a comprehensive and integrated sketch, which is the goal of this article. This manuscript provides a unique interdisciplinary perspective on Radiomics by discussing state-of-the-art signal processing solutions in the context of Radiomics.

A New Method for the High-Precision Assessment of Tumor Changes in Response to Treatment

Abstract:Imaging demonstrates that preclinical and human tumors are heterogeneous, i.e. a single tumor can exhibit multiple regions that behave differently during both normal development and also in response to treatment. The large variations observed in control group tumors can obscure detection of significant therapeutic effects due to the ambiguity in attributing causes of change. This can hinder development of effective therapies due to limitations in experimental design, rather than due to therapeutic failure. An improved method to model biological variation and heterogeneity in imaging signals is described. Specifically, Linear Poisson modelling (LPM) evaluates changes in apparent diffusion co-efficient (ADC) before and 72 hours after radiotherapy, in two xenograft models of colorectal cancer. The statistical significance of measured changes are compared to those attainable using a conventional t-test analysis on basic ADC distribution parameters. When LPMs were applied to treated tumors, the LPMs detected highly significant changes. The analyses were significant for all tumors, equating to a gain in power of 4 fold (i.e. equivelent to having a sample size 16 times larger), compared with the conventional approach. In contrast, highly significant changes are only detected at a cohort level using t-tests, restricting their potential use within personalised medicine and increasing the number of animals required during testing. Furthermore, LPM enabled the relative volumes of responding and non-responding tissue to be estimated for each xenograft model. Leave-one-out analysis of the treated xenografts provided quality control and identified potential outliers, raising confidence in LPM data at clinically relevant sample sizes.

Abstract:Objective: In this article, we introduce a computational model for simulating the growth of breast cancer lesions accounting for the stiffness of surrounding anatomical structures. Approach: In our model, ligaments are classified as the most rigid structures while the softer parts of the breast are occupied by fat and glandular tissues As a result of these variations in tissue elasticity, the rapidly proliferating tumor cells are met with differential resistance. It is found that these cells are likely to circumvent stiffer terrains such as ligaments, instead electing to proliferate preferentially within the more yielding confines of the breast's soft topography. By manipulating the interstitial tumor pressure in direct proportion to the elastic constants of the tissues surrounding the tumor, this model thus creates the potential for realizing a database of unique lesion morphology sculpted by the distinctive topography of each local anatomical infrastructure. We modeled the growth of simulated lesions within volumes extracted from fatty breast models, developed by Graff et al., with a resolution of 50 μm generated with the open-source and readily available VICTRE (Virtual Imaging Clinical Trials for Regulatory Evaluation) imaging pipeline. To visualize and validate the realism of the lesion models, we leveraged the imaging component of the VICTRE pipeline, which replicates the Siemens Mammomat Inspiration mammography system in a digital format. This system was instrumental in generating digital mammogram (DM) images for each breast model containing the simulated lesions. Results: By utilizing the DM images, we were able to effectively illustrate the imaging characteristics of the lesions as they integrated with the anatomical backgrounds. Significance: The lesion growth model will facilitate and enhance longitudinal in silico trials investigating the progression of breast cancer.

Abstract:Imaging plays a crucial role in the management of oncologic patients, from the initial diagnosis to staging and treatment response monitoring. Recently, it has been suggested that its importance could be further increased by accessing a new layer of previously hidden quantitative data at the pixel level. Using a multi-step process, radiomics extracts potential biomarkers from medical images that could power decision support tools. Despite the growing interest and rising number of research articles being published, radiomics is still far from fulfilling its promise of guiding oncologic imaging toward personalized medicine. This is, at least partly, due to the heterogeneous methodological quality in radiomic research, caused by the complexity of the analysis pipelines. In this review, we aim to disentangle this complexity with a stepwise approach. Specifically, we focus on challenges to face during image preprocessing and segmentation, how to handle imbalanced classes and avoid information leaks, as well as strategies for the proper validation of findings.
