Abstract:Goal: Impulse control disorders (ICDs) are frequent non-motor symptoms occurring during the course of Parkinson's disease (PD). The objective of this study was to estimate the predictability of the future occurrence of these disorders using longitudinal data, the first study using cross-validation and replication in an independent cohort. Methods: We used data from two longitudinal PD cohorts (training set: PPMI, Parkinson's Progression Markers Initiative; test set: DIGPD, Drug Interaction With Genes in Parkinson's Disease). We included 380 PD subjects from PPMI and 388 PD subjects from DIGPD, with at least two visits and with clinical and genetic data available, in our analyses. We trained three logistic regressions and a recurrent neural network to predict ICDs at the next visit using clinical risk factors and genetic variants previously associated with ICDs. We quantified performance using the area under the receiver operating characteristic curve (ROC AUC) and average precision. We compared these models to a trivial model predicting ICDs at the next visit with the status at the most recent visit. Results: The recurrent neural network (PPMI: 0.85 [0.80 - 0.90], DIGPD: 0.802 [0.78 - 0.83]) was the only model to be significantly better than the trivial model (PPMI: ROC AUC = 0.75 [0.69 - 0.81]; DIGPD: 0.78 [0.75 - 0.80]) on both cohorts. We showed that ICDs in PD can be predicted with better accuracy with a recurrent neural network model than a trivial model. The improvement in terms of ROC AUC was higher on PPMI than on DIGPD data, but not clinically relevant in both cohorts. Conclusions: Our results indicate that machine learning methods are potentially useful for predicting ICDs, but further works are required to reach clinical relevance.
Advances in magnetic resonance imaging of impulsive-compulsive behavior in Parkinson′s disease

Abstract:Impulsive-compulsive behaviors (ICBs), one of the non-motor symptoms of Parkinson′s disease (PD), may be attributed to dopaminergic dysfunction and dopamine replacement therapy. At present, understanding of ICBs, mechanisms and exploration of treatment largely depend on neuroimaging research. Despite contradictions among the findings, most evidence indicated that PD patients with ICBs showed decreased thickness in frontal and temporal cortex and the volume of subcortical nuclei when compared to those without ICBs. And the occurrence of ICBs is associated with relatively preserved white-matter integrity in the brain regions involved in reward-related behaviors. This article will give an overview of the magnetic resonance research of PD related ICBs including cortical thickness, diffusion tensor imaging, resting state and task state functional magnetic resonance imaging analysis, which lays a foundation for further imaging studies, revealing the pathogenesis and exploring the potential treatment options.
Subjective feelings associated with expectations and rewards during risky decision-making in impulse control disorder

Abstract:Impulse Control Disorder (ICD) in Parkinson's disease is a behavioral addiction induced by dopaminergic therapies, but otherwise unclear etiology. The current study investigates the interaction of reward processing variables, dopaminergic therapy, and risky decision-making and subjective feelings in patients with versus without ICD. Patients with (n = 18) and without (n = 12) ICD performed a risky decision-making task both 'on' and 'off' standard-of-care dopaminergic therapies (the task was performed on 2 different days with the order of on and off visits randomized for each patient). During each trial of the task, participants choose between two options, a gamble or a certain reward, and reported how they felt about decision outcomes. Subjective feelings of 'pleasure' are differentially driven by expectations of possible outcomes in patients with, versus without ICD. While off medication, the influence of expectations about risky-decisions on subjective feelings is reduced in patients with ICD versus without ICD. While on medication, the influence of expected outcomes in patients with ICD versus without ICD becomes similar. Computational modeling of behavior supports the idea that latent decision-making factors drive subjective feelings in patients with Parkinson's disease and that ICD status is associated with a change in the relationship between factors associated with risky behavior and subjective feelings about the experienced outcomes. Our results also suggest that dopaminergic medications modulate the impact expectations have on the participants' subjective reports. Altogether our results suggest that expectations about risky decisions may be decoupled from subjective feelings in patients with ICD, and that dopaminergic medications may reengage these circuits and increase emotional reactivity in patients with ICD.

Abstract:Over the last decade, we have gained a better understanding of impulse control disorder in Parkinson's disease (PD-ICD), a medication complication in PD. Researchers were aware of its complexity and took efforts to learn more about its diagnostic and treatment possibilities. Nevertheless, clinical management for it is currently neglected. We conducted a narrative overview of literature published from 2012 to October 2023 on various aspects of clinical management for PD-ICD. A potential "susceptibility-catalytic-stress" model in the development of PD-ICD was proposed and a profile encoding predictors for PD-ICD was created. Based on these predictors, some methods for prediction were recently developed for better prediction, such as the polymorphic dopamine genetic risk score and the clinic-genetic ICD-risk score. A variety of treatment options, including dose reduction of dopamine receptor agonists (DAs), DAs removal, DAs switch, and add-on therapy, are investigated with inconsistent reports. Based on current findings, we developed a clinical management model prototype centered on prevention, consisting of prediction, prevention, follow-up and monitoring, therapy, and recurrence prevention, for clinical reference, and further proposed 4 key clinical management principles, including standardization, prediction centered, persistence, and whole course.

Abstract:A substantial subset of Parkinson's disease (PD) patients suffers from impulse control disorders (ICDs), which are side effects of dopaminergic medication. Dopamine plays a key role in reinforcement learning processes. One class of reinforcement learning models, known as the actor-critic model, suggests that two components are involved in these reinforcement learning processes: a critic, which estimates values of stimuli and calculates prediction errors, and an actor, which estimates values of potential actions. To understand the information processing mechanism underlying impulsive behavior, we investigated stimulus and action value learning from reward and punishment in four groups of participants: on-medication PD patients with ICD, on-medication PD patients without ICD, off-medication PD patients without ICD, and healthy controls. Analysis of responses suggested that participants used an actor-critic learning strategy and computed prediction errors based on stimulus values rather than action values. Quantitative model fits also revealed that an actor-critic model of the basal ganglia with different learning rates for positive and negative prediction errors best matched the choice data. Moreover, whereas ICDs were associated with model parameters related to stimulus valuation (critic), PD was associated with parameters related to action valuation (actor). Specifically, PD patients with ICD exhibited lower learning from negative prediction errors in the critic, resulting in an underestimation of adverse consequences associated with stimuli. These findings offer a specific neurocomputational account of the nature of compulsive behaviors induced by dopaminergic drugs.

Abstract:Abstract Cognitive impairment is a debilitating symptom in Parkinson’s disease (PD). We aimed to establish an accurate multivariate machine learning (ML) model to predict cognitive outcome in newly diagnosed PD cases from the Parkinson’s Progression Markers Initiative (PPMI). Annual cognitive assessments over an 8-year time span were used to define two cognitive outcomes of (i) cognitive impairment, and (ii) dementia conversion. Selected baseline variables were organized into three subsets of clinical, biofluid and genetic/epigenetic measures and tested using four different ML algorithms. Irrespective of the ML algorithm used, the models consisting of the clinical variables performed best and showed better prediction of cognitive impairment outcome over dementia conversion. We observed a marginal improvement in the prediction performance when clinical, biofluid, and epigenetic/genetic variables were all included in one model. Several cerebrospinal fluid measures and an epigenetic marker showed high predictive weighting in multiple models when included alongside clinical variables.

Abstract:IMPORTANCE Major depressive disorder is prevalent and impairing. Parsing neurocomputational substrates of reinforcement learning in individuals with depression may facilitate a mechanistic understanding of the disorder and suggest new cognitive therapeutic targets. OBJECTIVE To determine associations among computational model-derived reinforcement learning parameters, depression symptoms, and symptom changes after treatment. DESIGN, SETTING, AND PARTICIPANTS In this mixed cross-sectional-cohort study, individuals performed reward and loss variants of a probabilistic learning task during functional magnetic resonance imaging at baseline and follow-up. A volunteer sample with and without a depression diagnosis was recruited from the community. Participants were assessed from July 2011 to February 2017, and data were analyzed from May 2017 to May 2021. MAIN OUTCOMES AND MEASURES Computational model-based analyses of participants' choices assessed a priori hypotheses about associations between components of reward-based and loss-based learning with depression symptoms. Changes in both learning parameters and symptoms were then assessed in a subset of participants who received cognitive behavioral therapy (CBT). RESULTS Of 101 included adults, 69 (68.3%) were female, and the mean (SD) age was 34.4 (11.2) years. A total of 69 participants with a depression diagnosis and 32 participants without a depression diagnosis were included at baseline; 48 participants (28 with depression who received CBT and 20 without depression) were included at follow-up (mean [SD] of 115.1 [15.6] days). Computational model-based analyses of behavioral choices and neural data identified associations of learning with symptoms during reward learning and loss learning, respectively. During reward learning only, anhedonia (and not negative affect or arousal) was associated with model-derived learning parameters (learning rate: posterior mean regression beta = -0.14; 95% credible interval [CrI], -0.12 to -0.03; outcome sensitivity: posterior mean regression beta = 0.18; 95% CrI, 0.02 to 0.37) and neural learning signals (moderation of association between striatal prediction error and expected value signals: t(97) = -2.10; P =.04). During loss learning only, negative affect (and not anhedonia or arousal) was associated with learning parameters (outcome shift: posterior mean regression beta = -0.11; 95% CrI, -0.20 to -0.01) and disrupted neural encoding of learning signals (association with subgenual anterior cingulate prediction error signals: r = -0.28; P =.005). Symptom improvement following CBT was associated with normalization of learning parameters that were disrupted at baseline (reward learning rate: posterior mean regression beta = 0.15; 90% CrI, 0.001 to 0.41; loss outcome shift: posterior mean regression beta = 0.42; 90% CrI, 0.09 to 0.77). CONCLUSIONS AND RELEVANCE In this study, the mapping of reinforcement learning components to symptoms of major depression revealed mechanistic features associated with these symptoms and points to possible learning-based therapeutic processes and targets.
Assessing impulse control behaviors in early Parkinson's disease: a longitudinal study

Abstract:Objective: Impulse control behaviors (ICBs) frequently coexist with Parkinson's disease (PD). However, the predictors of ICBs in PD remain unclear, and there is limited data on the biological correlates of ICBs in PD. In this study, we examined clinical, imaging, and biological variables to identify factors associated with longitudinal changes in ICBs in early-stage PD. Methods: The data for this study were obtained from the Parkinson's Progression Markers Initiative, an international prospective cohort study that evaluates markers of disease progression in PD. We examined clinical, imaging, and biological variables to determine their associations with ICBs over a period of up to 5 years. Cox regression models were employed to investigate the predictors of ICBs in early-stage, untreated PD. Results: The study enrolled 401 individuals with PD and 185 healthy controls (HC). At baseline, 83 PD subjects (20.7%) and 36 HC (19.5%) exhibited ICBs. Over the course of 5 years, the prevalence of ICBs increased in PD (from 20.7% to 27.3%, p < 0.001), while it decreased in HC (from 19.5% to 15.2%, p < 0.001). Longitudinally, the presence of ICBs in PD was associated with depression, anxiety, autonomic dysfunction, and excessive daytime sleepiness (EDS). However, there was no significant association observed with cognitive dysfunction or motor severity. Treatment with dopamine agonists was linked to ICBs at years 3 and 4. Conversely, there was no association found between ICBs and presynaptic dopaminergic dysfunction. Additionally, biofluid markers in baseline and the first year did not show a significant association with ICBs. A predictive index for ICBs was generated, incorporating three baseline characteristics: anxiety, rapid eye movement sleep behavior disorder (RBD), and p-tau levels in cerebrospinal fluid (CSF). Conclusion: During the early stages of PD, there is a notable increase in ICBs over time. These ICBs are associated with depression, anxiety, autonomic dysfunction, EDS, and the use of dopaminergic medications, particularly dopamine agonists. Anxiety, RBD, and p-tau levels in CSF are identified as predictors for the incident development of ICBs in early PD. Further longitudinal analyses will provide a more comprehensive understanding of the associations between ICBs and imaging findings, as well as biomarkers. These analyses will help to better characterize the relationships and implications of these factors in the context of ICBs in early PD.

Abstract:Background: The course of impulse control disorders (ICD) varies in the early stage of Parkinson's disease (PD). Aim: We aimed to delineate the association between the evolution pattern of ICD and the progression of PD. Methods: A total of 321 de novo PD patients from the Parkinson's Progression Markers Initiative database were included. Patients were followed up for a mean of 6.8 years and were classified into different groups according to the evolution patterns of ICD. Disease progression was compared among groups using survival analysis, in which the endpoint was defined as progression to Hoehn and Yahr stage 3 or higher for motor progression and progression to mild cognitive impairment for cognitive decline. In the fourth year of follow-up, four types of ICD evolution patterns were identified: (1) non-ICD-stable (68.2%), a patient who is consistently free of ICD; (2) late-ICD (14.6%), ICD developed during the follow-up of patients; (3) ICD-stable (11.5%), patients showed persistent ICD; and (4) ICD-reversion (5.6%), baseline ICD disappeared during the follow-up of patients with ICD. Results: The ICD-reversion type shows daily life non-motor symptoms [Movement Disorder Society-Unified Parkinson Disease Rating Scale (MDS-UPDRS) part I], daily life motor symptoms (MDS-UPDRS part II), rapid eye movement sleep behavior disorder, and anxiety symptoms has a greater impact. PD patients with different ICD evolution patterns had different changes in white matter microstructure at the onset of the disease. Those relevant brain regions are involved in ICD and non-motor functions. Conclusion: Four early ICD evolution patterns are identified in de novo PD, with different prognoses and brain white matter microstructural damage patterns, and they may predict motor progression and cognitive decline in PD patients.

Preliminary Results of Neuromorphic Controller Design and a Parkinson's Disease Dataset Building for Closed-Loop Deep Brain Stimulation

Abstract:Parkinson's Disease afflicts millions of individuals globally. Emerging as a promising brain rehabilitation therapy for Parkinson's Disease, Closed-loop Deep Brain Stimulation (CL-DBS) aims to alleviate motor symptoms. The CL-DBS system comprises an implanted battery-powered medical device in the chest that sends stimulation signals to the brains of patients. These electrical stimulation signals are delivered to targeted brain regions via electrodes, with the magnitude of stimuli adjustable. However, current CL-DBS systems utilize energy-inefficient approaches, including reinforcement learning, fuzzy interface, and field-programmable gate array (FPGA), among others. These approaches make the traditional CL-DBS system impractical for implanted and wearable medical devices. This research proposes a novel neuromorphic approach that builds upon Leaky Integrate and Fire neuron (LIF) controllers to adjust the magnitude of DBS electric signals according to the various severities of PD patients. Our neuromorphic controllers, on-off LIF controller, and dual LIF controller, successfully reduced the power consumption of CL-DBS systems by 19% and 56%, respectively. Meanwhile, the suppression efficiency increased by 4.7% and 6.77%. Additionally, to address the data scarcity of Parkinson's Disease symptoms, we built Parkinson's Disease datasets that include the raw neural activities from the subthalamic nucleus at beta oscillations, which are typical physiological biomarkers for Parkinson's Disease.

Abstract:Introduction: Impulse control disorders (ICDs) refer to the common neuropsychiatric complication of Parkinson's disease (PD). The white matter (WM) topological organization and its impact on brain networks remain to be established. Methods: A total of 17 PD patients with ICD (PD-ICD), 17 without ICD (PD-NICD), and 18 healthy controls (HCs) were recruited. Graph theoretic analyses and Granger causality analyses were combined to investigate WM topological organization and the directional connection patterns of key regions. Results: Compared to PD-NICD, ICD patients showed abnormal global properties, including decreased shortest path length (Lp) and increased global efficiency (Eg). Locally, the ICD group manifested abnormal nodal topological parameters predominantly in the left middle cingulate gyrus (MCG) and left superior cerebellum. Decreased directional connectivity from the left MCG to the right medial superior frontal gyrus was observed in the PD-ICD group. ICD severity was significantly correlated with Lp and Eg. Discussion: Our findings reflected that ICD patients had excessively optimized WM topological organization, abnormally strengthened nodal structure connections within the reward network, and aberrant causal connectivity in specific cortical– limbic circuits. We hypothesized that the aberrant reward and motor inhibition circuit could play a crucial role in the emergence of ICDs.

Pharmacological and pupillary evidence for the noradrenergic contribution to reinforcement learning in Parkinson's disease

Abstract:Abstract Noradrenaline plays an integral role in learning, by optimising behavioural strategies and facilitating choice execution. Testing the noradrenergic framework of learning in the context of human diseases offers a test bed for current normative neuroscience theories and may also indicate therapeutic potential. Parkinson's disease is often considered as a model of dopamine deficits, including dopamine’s role in reinforcement learning. However, noradrenergic function is also severely impaired by Parkinson's disease, contributing to cognitive deficits. Using a single dose of the noradrenaline reuptake inhibitor atomoxetine, in people with Parkinson's disease (n=19, randomised double-blind placebo-controlled crossover design), we show improvements in learning compared to placebo. Computational modelling confirmed a substantial shift in the decision noise parameter, indicative of more exploitative choices. This response pattern closely resembled that of age-matched controls (n=26) and simulations of optimal response strategies. Pupillometry revealed increased baseline pupil diameter under atomoxetine, which correlated with behavioural improvements, and a heightened phasic pupillary response to feedback. Our findings confirm the noradrenergic contribution to reinforcement learning, and in doing so they challenge the simple interpretation of tonic-phasic locus coeruleus firing patterns based on pupillometry. Noradrenergic modulation is a potential treatment strategy for cognitive symptoms in Parkinson's disease and related disorders.
A Machine Learning Approach Employing Common Clinical Variables

Abstract:Background: Parkinson's Disease significantly impacts health-related quality of life, with the Parkinson's Disease Questionnaire-39 extensively used for its assessment. However, predicting such outcomes remains a challenge due to the subjective nature and variability in patient experiences. This study develops a machine learning model using accessible clinical data to enable predictions of life-quality outcomes in Parkinson's Disease and utilizes explainable machine learning techniques to identify key influencing factors, offering actionable insights for clinicians. Methods: Data from the Parkinson's Real-world Impact Assessment study (PRISM), involving 861 patients across six European countries, were analyzed. After excluding incomplete data, 627 complete observations were used for the analysis. An ensemble machine learning model was developed with a 90% training and 10% validation split. Results: The model demonstrated a Mean Absolute Error of 4.82, a Root Mean Squared Error of 8.09, and an R2 of 0.75 in the training set, indicating a strong model fit. In the validation set, the model achieved a Mean Absolute Error of 11.22, a Root Mean Squared Error of 13.99, and an R2 of 0.36, showcasing moderate variation. Key predictors such as age at diagnosis, patient's country, dementia, and patient's age were identified, providing insights into the model's decision-making process. Conclusions: This study presents a robust model capable of predicting the impact of Parkinson's Disease on patients' quality of life using common clinical variables. These results demonstrate the potential of machine learning to enhance clinical decision-making and patient care, suggesting directions for future research to improve model generalizability and applicability.

Low-Frequency Activity in Dorsal Subthalamic Nucleus Predicts Impulsivity Improvement Following Deep Brain Stimulation in Parkinsonian Patients

However, the link between the subthalamic neural activity at the single-neuron level and the postoperative evolution of ICB remains unclear. In this study, we investigated neural features associated with postoperative ICB recovery and their spatial distribution within the subthalamic nucleus (STN). Approach. We examined neural activity extracted from intraoperative microelectrode recordings within the STN of 22 PD patients undergoing STN-DBS. Ten patients were diagnosed with ICB, with half of them showing recovery (ICB-R) from impulsive symptoms following implantation, while the other half remained stable (ICB-S). Both groups presented similar motor symptoms and received similar drug treatments pre- and post-operatively. Following, we compared beta [12-30 Hz] and theta [4-8 Hz] oscillations, firing rate, regularity, and spiking patterns in non-ICB, recovered, and stable patients across STN regions. We adopted linear discriminant algorithms to classify the postoperative state at both single neuron and patient levels. Main results. We observed significantly weaker beta and theta oscillations and increased spiking regularity at the single neuron level (p<0.05, Mann-Whitney U test) in patients who displayed postoperative ICB recovery. Of note, this difference was significant only on the dorsal portion of the STN, close to the stimulation target region. The discrimination algorithms based on these features correctly classified the postoperative state of 9/10 ICB patients. Significance. We showed that low-frequency subthalamic neural activity next to the stimulation target could be an effective biomarker for the evolution of ICBs following STN-DBS surgery, independently from other clinical aspects. Our results also support broader implications of beta activity in PD pathology beyond the motor domain.
Bayesian Approaches for Revealing Complex Neural Network Dynamics in Parkinson's Disease

Abstract:Parkinson's disease (PD) belongs to the class of neurodegenerative disorders that affect the central nervous system. It is usually defined as the gradual loss of dopaminergic neurons in the substantia nigra pars compacta, which causes both motor and non-motor symptoms. Understanding the neuronal processes that underlie PD is critical for creating successful therapies. This study combines machine learning (ML), stochastic modelling, and Bayesian inference with connectomic data to analyze the brain networks involved in PD. We use modern computational methods to study large-scale neural networks to identify neuronal activity patterns related to PD development. We aim to define the subtle structural and functional connection changes in PD brains by combining connectomic with stochastic noises. Stochastic modelling approaches reflect brain dynamics' intrinsic variability and unpredictability, shedding light on the origin and spread of pathogenic events in PD. We employ a novel hybrid model to assess how stochastic noise impacts the cortex-basal ganglia-thalamus (CBGTH) network, using data from the Human Connectome Project (HCP). Bayesian inference allows us to quantify uncertainty in model parameters, improving the accuracy of our predictions. Our findings reveal that stochastic disturbances increase thalamus activity, even under deep brain stimulation (DBS). Bayesian analysis suggests that reducing these disturbances could enhance healthy brain states, providing insights for potential therapeutic interventions. This approach offers a deeper understanding of PD dynamics and paves the way for personalized treatment strategies. This is an extended version of our work presented at the ICCS-2024 conference.

Abstract:This study presents a comprehensive framework for analyzing Parkinson's disease (PD) progression and optimizing medication recommendations based on multimodal data integration and reinforcement learning (RL) techniques. The research framework integrates diverse biomarkers from structured clinical assessments, imaging data, and biospecimen information, creating various single, bimodal, and trimodal datasets. Deep learning-based embedding networks are employed to transform high-dimensional data into lower-dimensional representations, capturing essential patterns related to PD progression. The learned representations are evaluated using visualization methods and predictive modeling to differentiate clinically relevant outcomes. Subsequently, medication recommendation models are developed based on the learned representations, assisting clinicians in making informed decisions to manage PD progression effectively. Optimal medication policies are derived considering different modality settings, revealing insights into medication prescribing tendencies across modalities. The findings highlight the significance of integrating diverse data modalities in understanding and managing PD progression, showcasing the potential of RL-based medication recommendation systems in clinical decision-making. This research contributes to the advancement of personalized medicine strategies for PD patients, emphasizing the importance of tailored medication regimes based on multimodal data insights.

Design of a Parkinsonian Biomarkers Combination Optimization Method Using Rodent Model.

Abstract:Adaptive Deep Brain Stimulation (aDBS) has been proposed in literature to avoid the negative consequences associated with the continuous stimulation delivered through traditional deep brain stimulation. This work seeks to determine a group of neural biomarkers that a classification algorithm could use on an aDBS device using rodent animal models. The neural activities were acquired from the primary motor cortex of four Parkinsonian model rats and four healthy rats from a control group. To overcome the variability introduced from the small rat sample size, this work proposes a novel method for combining and running Genetic Feature Selection and Forward Stepwise Feature Selection in an environment where classification accuracy varies greatly based on how the folds are organized before cross-validation. Three separate classification algorithms, Logistic Regression, k-Nearest Neighbor, and Random Forest are used to verify the proposed method. For Logistic Regression, the set of Alpha Power (7-12 Hz), High Beta Power (20-30 Hz), and 55-95 Hz Gamma Power shows the best performance in classification. For k-Nearest Neighbor, the characterizing features are Low Beta Power (12-20 Hz), High Beta Power, All Beta Power (12-30 Hz), 55-95 Hz Gamma Power, and 95-105 Hz Gamma Power. For Random Forest, they are High Beta Power, All Beta Power, 55-95 Hz Gamma Power, 95-105 Hz Gamma Power, and 300-350 Hz High-Frequency Oscillations Power. With the selected feature set, experimental results show an increasing classification accuracy from 59.08% to 77.69% for Logistic Regression, from 49.53% to 73.44% for k-Nearest Neighbor, and from 54.10% to 71.15% for Random Forest. Clinical Relevance- This experiment provides a method for determining the most effective biomarkers from a larger set for classifying Parkinsonian behavior for an aDBS device.
Impulse control disorders in Parkinson’s disease: a national Swedish registry study on high-risk treatments and vulnerable patient groups

Abstract:Background Impulse control disorders (ICDs) are known psychiatric conditions in Parkinson’s disease (PD), especially as a side effect of antiparkinsonian therapy. Screening for vulnerable patients and avoiding high-risk treatments can be an effective approach to reduce the ICD burden in patients with PD. Thus, our goal was to identify risk factors for ICDs in PD in the Swedish total population. Methods Our longitudinal study was based on records of all patients with PD in the Swedish National Patient Registries and the Prescribed Drug Register (n=55 235). Patients with incident gambling disorder and other ICDs were compared with a control group on demographic factors, psychiatric comorbidity, antiparkinsonian dopaminergic treatment and therapies for advanced disease. Potential risk factors were analysed using logistic regressions and relative frequency comparisons (Fisher’s exact test). Results Main predictors for incident gambling disorder were treatment with dopamine agonists (Frequency ratio 1.4, p=0.058), monoamine oxidase B (MAO-B) inhibitors (Frequency ratio 1.8, p=0.006) and a prescription for drugs used in addictive disorders (OR 5.85, 95% CI 2.00 to 17.10). Main predictors for other ICDs were dopamine agonist treatment (frequency ratio 1.6, p=0.003), anxiety disorders (OR 7.04, 95% CI 2.96 to 16.71) and substance use disorders other than alcohol (OR 5.66, 95% CI 1.75 to 18.23). Conclusions Our results support possible risk factors for incident ICDs that had previously been identified, like dopamine agonist treatment and raise additional attention for risk factors like MAO-B inhibitor treatment and specific psychiatric comorbidities. These findings enable tailoring antiparkinsonian therapy to individual patient-specific risk profiles.

Abstract:Background: Aspects of reinforcement learning have been associated with specific depression symptoms and may inform the course of depressive illness. Methods: We applied support vector machines to investigate whether blood‑oxygen-level dependent (BOLD) responses linked with neural prediction error (nPE) and neural expected value (nEV) from a probabilistic learning task could forecast depression remission. We investigated whether predictions were moderated by treatment use or symptoms. Participants included 55 individuals (n = 39 female) with a depression diagnosis at baseline; 36 of these individuals completed standard cognitive behavioral therapy and 19 were followed during naturalistic course of illness. All participants were assessed for depression diagnosis at a follow-up visit. Results: Both nPE and nEV classifiers forecasted remission significantly better than null classifiers. The nEV classifier performed significantly better than the nPE classifier. We found no main or interaction effects of treatment status on nPE or nEV accuracy. We found a significant interaction between nPE-forecasted remission status and anhedonia, but not for negative affect or anxious arousal, when controlling for nEV-forecasted remission status. Limitations: Our sample size, while comparable to that of other studies, limits options for maximizing and evaluating model performance. We addressed this with two standard methods for optimizing model performance (90:10 train and test scheme and bootstrapped sampling). Conclusions: Results support nEV and nPE as relevant biobehavioral signals for understanding depression outcome independent of treatment status, with nEV being stronger than nPE as a predictor of remission. Reinforcement learning variables may be useful components of an individualized medicine framework for depression healthcare.
