Abstract:Background and objective: Pulmonary alveolar proteinosis (PAP) is a rare pulmonary disease. It was first described in China in 1965, and more cases have been reported subsequently. A systematic review was performed on 241 cases of PAP in China and progress in the diagnosis and treatment of this disease is discussed. Methods: The Chinese biological and medical databases from 1965 to 2006 were searched and 241 cases with complete clinical and pathological data were identified. The clinical characteristics of the disease were summarized and longitudinal comparisons were made of diagnostic and treatment methods over time. Results: The morbidity associated with PAP has increased in recent years. The clinical manifestations were non-specific. Progressive dyspnoea, cough and sputum were the most common symptoms. The percentage of patients undergoing CT examination has increased over the years. The combination of bronchoscopic biopsy and bronchoalveolar lavage (BAL) was usually sufficient to establish the diagnosis. Treatment was reported for a total of 142 cases. BAL and whole lung lavage were both effective and were only required once by most patients. Conclusions: The demographic characteristics and clinical manifestations of PAP patients in China are largely consistent with previous reports. Morbidity has increased dramatically in recent years, mainly due to the broad application of bronchoscopy since 1995. CT is very important for diagnosis of the disease. The long-term effects of treatment by whole lung lavage and BAL are similar.

Abstract:Objective To analyze and evaluate clinical features, pathological characteristics, diagnosis and treatment of the identified pulmonary alveolar proteinosis (PAP) patients.Methods Retrospective study of 12 patients identified by biopsy through BALF was conducted. Focused analysis with imaging studies (chest x-ray or CT/HRCT) and clinical presentations were aimed to determine the diagnostic methods of PAP. Results Nonspecific clinical representation of PAP was composed of progressive dyspnea (100%) and nonproductive cough (91.67%). Physical examination showed signs of inner lungsawas seldom detected, such as inspiratory crackles or Velcro's rales (41.67%) and cyanosis (25%). Although, absence of specific chest X-ray findings, some typical radiological changes contributed to diagnose PAP correctly, such as patchy opacification(91.67%)and crazy paving pattern(50%), especially in HRCT. Conclusion Separability of clinical symptoms and signs, radiological results and clinical features exist as characteristic representation of PAP. Diagnostic bronchoalveolar lavage is a practical and useful technology to determine PAP.

Abstract:The pulmonary alveolar proteinosis (PAP) is a kind of causally unknown and infrequent pulmonary diseases. The pathologic feature is that the phospholipoprotein like surfactant, positive while dyed with periodic acid schiff has excessively accumulated in the alveoli cavity and the terminus of the bronchus. In this article, we selected five patients who have probably caught PAP. Vitally examed with the micro bronchoscope,their pulmonary tissue were taken,the ultra thin sections were observed by TEM. Two of these five patients could be excluded from catching PAP and the cellular ultrastructural alteration of the rest was fundamentally the same and corresponded with the pathologic alteration of the pulmonary tissue of PAP. The cellular ultrastructural feature of PAP is that of the typeⅡovergrown of alveolar epithelial cell and the abundence of that cellular chips, pulmonary surfactant, protein like structures and a number of spumous alveolar macrophages in the alveoli cavity and the terminus of the bronchus. The aboundance of these substances lead to the depression of the alveolar function and render the patients dyspnoea. The alteration of these ultrastructures sometimes is difficult to find with the LM. The use of the EM can help greatly in the clinical diagnosis.

Abstract:Pulmonary alveolar proteinosis (PAP) is a rare pulmonary disorder that is characterized by the abnormal accumulation of surfactant within the alveoli. Alveolar macrophages (AMs) have been identified as playing a pivotal role in the pathogenesis of PAP. In most of PAP cases, the disease is triggered by impaired cholesterol clearance in AMs that depend on granulocyte-macrophage colony-stimulating factor (GM-CSF), resulting in defective alveolar surfactant clearance and disruption of pulmonary homeostasis. Currently, novel pathogenesis-based therapies are being developed that target the GM-CSF signaling, cholesterol homeostasis, and immune modulation of AMs. In this review, we summarize the origin and functional role of AMs in PAP, as well as the latest therapeutic strategies aimed at addressing this disease. Our goal is to provide new perspectives and insights into the pathogenesis of PAP, and thereby identify promising new treatments for this disease.

Abstract:OBJECTIVE:To summarize and analyze the clinical manifestations of pulmonary alveolar proteinosis (PAP) and its treatment.METHODS:Retrospective analysis of 2 cases with literature was reviewed.RESULTS:PAP presented a protracted chronic clinical course with overt symptoms but few physical signs, distinct chest radiography, and was a benign process. It was manifested as primary or secondary pathological changes. Typical chest CT scan showed "map" or "cobble stone" changes. Clinically, PAP was characterized by its incompatibility between symptoms and signs, and also radiographic picture and physical signs. The examination of bronchoalveolar lavage fluid could be used to confirm the diagnosis of PAP with periodic acid-schiff staining and electro microscopic examination. Whole lung lavage was an efficient therapy for PAP.CONCLUSION:It is very important for physicians to pay much more attention on the early diagnosis and treatment of pulmonary alveolar proteinosis.

The underlying mechanisms of PAP are not fully understood, but it is believed to involve impaired clearance of surfactant and abnormal immune responses. Diagnosis of PAP typically involves imaging studies and bronchoscopy, and treatment options include whole-lung lavage, pharmacotherapy, and lung transplantation. We report PAP in a 56-year-old female who worked in a dental office and had no prior diagnosis of lung disease.

Abstract:Objective:To improve the understanding of the diagnosis and treatment of pulmonary alveolar proteinosis(PAP). Methods: A retrospective analysis of 17 cases who were diagnosed of PAP with pathological evidences from June 1996 to February 2006 was made. Results: There were 14 cases had significant improvement in clinical manifestation, chest CT scan, ABG and pulmonary function tests. Conclusion:PAP may a clinical manifestation of cough progressing dyspnea and lowered breath sound. Chest X-ray of ten shows a diffuse ground glass pattern of both lungs with patches nodules or reticular infiltrates CT scan demonstrates map-like patches with a pavestone pattern. The diagnosis should be made with pathological evidences. Upto -date, the only effective treatment is bronchoalveolar lavage.

Abstract:OBJECTIVE:To evaluate the efficacy and safety of large-volume whole lung lavage in the treatment of severe pulmonary alveolar proteinosis (PAP).METHODS:Five severe PAP patients underwent large-volume whole lung lavage under general anaesthesia using a double lumen endotracheal tube. Suction was used for returning fluid. The volume of lavage and return volume and the return rate were calculated. The efficacy was evaluated according to the improvement of symptoms, amelioration of chest radiograph or chest CT scan and the changes of pulmonary function and artery blood gas analysis.RESULTS:After lavage, the symptoms, pulmonary function and artery blood gas were markedly improved, and the chest radiographs showed dissipation of lung consolidation. No severe complications were observed in the 5 cases.CONCLUSION:Large-volume whole lung lavage is an effective and safe therapeutic method in the treatment of PAP.

Abstract:Background: Pulmonary alveolar proteinosis (PAP) is a rare syndrome caused by several distinct diseases leading to progressive dyspnoea, hypoxemia, risk of respiratory failure and early death due to accumulation of proteinaceous material in the lungs. Diagnostic strategies may include computed tomography (CT) of the lungs, bronchoalveolar lavage, evaluation of antibodies against granulocyte macrophage colony stimulating factor (GM-CSF), genetic testing, and, eventually, lung biopsy. The management options are focused at removing the proteinaceous material by whole lung lavage (WLL), augmentation therapy with GM-CSF, rituximab, plasmapheresis, and lung transplantation. The presented diagnostic and management guideline aim to provide guidance to physicians managing patients with PAP. Methods: A European Respiratory Society Task Force committee composed of clinicians, methodologists, and patients with experience in PAP developed recommendations in accordance with the ERS Handbook for Clinical Practice Guidelines and the GRADE (Grading of Recommendations, Assessment, Development and Evaluations) approach. This included a systematic review of the literature and application of the GRADE approach to assess the certainty of the evidence and strength of recommendations. The committee formulated five PICO (Patients, Intervention, Comparison, Outcomes) questions, and two narrative questions to develop specific evidence-based recommendations. Results: The Task Force committee developed recommendations for five PICOs. These included management of PAP with WLL, GM-CSF augmentation therapy, rituximab, plasmapheresis, and lung transplantation. Also, the committee made recommendations regarding the use of GM-CSF antibody testing, diagnostic bronchoalveolar lavage (BAL) and biopsy based on narrative questions.In addition to the recommendations, the committee provided information on the hierarchy of diagnostic interventions and therapy. Conclusions: The diagnosis of PAP is based on CT and BAL cytology or lung histology, whereas diagnosis of specific PAP-causing diseases requires GM-CSF antibody testing or genetic analysis. There are several therapies including WLL and augmentation therapy with GM-CSF available to treat PAP, but supporting evidence is still limited.

Abstract:To describe the clinical, radiological and pathological characteristics of idiopathic pulmonary alveolar proteinosis (I-PAP) and to evaluate the methods of diagnosis and treatment. Three patients were successfully diagnosed and treated in our hospital and the literature on the subject was reviewed. Three patients, two males and one female (mean age 46 years), were diagnosed averagely in 4 months. Two severe patients presented with progressive dyspnea and type I respiratory failure, and one mild patient only with dry cough and hypoxemia. Chest X-ray radiographs all showed perihilar "butterfly" shadow and chest CT scans showed diffused ground-glass opacities (GGO), typically with "map" changes and "crazy paving" patterns. All the patients underwent bronchoscope, branchoalveolar lavage fluid (BALF) had grossly opaque and/or milky appearance and its sediment was periodic acid-Schiff stain positive. Trans-bronchoscopic lung biopsy (TBLB) specimens were obtained and under light microscopy alveoli and some of the small bronchioles were filled with eosinophilic proteinaceous material with needle-like clefts. By electron microscopy numerous cellular debris and extracellular multilamellated bodies were found. Two severe patients were successfully treated with sequential whole-lung lavage and one required repeated lavages. I-PAP is rare and prone to be misdiagnosed. The radiological features may indicate the diagnosis and examinations of TBLB and BALF can make the accurate diagnosis. Whole-lung lavage is the most effective therapy by now and granulocyte-macrophage colony-stimulating factor (GM-CSF) may be beneficial in some patients.

Abstract:Pulmonary alveolar proteinosis (PAP) is an umbrella term used to refer to a pulmonary syndrome which is characterized by excessive accumulation of surfactant in the lungs of affected individuals. In general, PAP is a rare lung disease affecting children and adults, although its prevalence and incidence is variable among different countries. Even though PAP is a rare disease, it is a prime example on how modern medicine can lead to new therapeutic concepts, changing ways and techniques of (genetic) diagnosis which ultimately led into personalized treatments, all dedicated to improve the function of the impaired lung and thus life expectancy and quality of life in PAP patients. In fact, new technologies, such as new sequencing technologies, gene therapy approaches, new kind and sources of stem cells and completely new insights into the ontogeny of immune cells such as macrophages have increased our understanding in the onset and progression of PAP, which have paved the way for novel therapeutic concepts for PAP and beyond. As of today, classical monocyte-derived macrophages are known as important immune mediator and immune sentinels within the innate immunity. Furthermore, macrophages (known as tissue resident macrophages (TRMs)) can also be found in various tissues, introducing e. g. alveolar macrophages in the broncho-alveolar space as crucial cellular determinants in the onset of PAP and other lung disorders. Given recent insights into the onset of alveolar macrophages and knowledge about factors which impede their function, has led to the development of new therapies, which are applied in the context of PAP, with promising implications also for other diseases in which macrophages play an important role. Thus, we here summarize the latest insights into the various forms of PAP and introduce new pre-clinical work which is currently conducted in the framework of PAP, introducing new therapies for children and adults who still suffer from this severe, potentially life-threatening disease. PAP ist eine seltene Erkrankung, die sowohl Kinder als auch Erwachsene betrifft und deren Prävalenz und Inzidenz stark zwischen verschiedenen Ländern variiert. Obwohl die PAP eine seltene Erkrankung ist, so ist diese Krankheit ein gutes Beispiel wie Fortschritte in der modernen Medizin zu neuen therapeutischen Ansätzen und veränderten Methoden und Techniken in der genetischen Diagnostik führen, welches schlussendlich die Lungenfunktion der Patienten verbessern und somit eine höhere Lebenserwartung und gesteigerte Lebensqualität der Patienten ermöglichen. Zum besseren Verständnis in der Krankheitsentstehung haben aktuelle Technologien wie neuste Genomsequenzierung, moderne Gentherapieansätze, Möglichkeiten der Stammzellgewinnung sowie neuste Erkenntnisse in der Ontogenese von Immunzellen wie beispielsweise Makrophagen, maßgeblich dazu beigetragen die Lebensqualität von Patienten zu verbessern. Damit wurde der Weg für neue therapeutische Ansätze in der Behandlung von PAP geebnet, aber auch ein großer Beitrag zum allgemeinen wissenschaftlichen Verständnis erbracht. Im Bereich der PAP und als wesentliche Grundlage in der Krankheitsentstehung haben sich klassische, aus Monozyten differenzierte Makrophagen als wichtiger Wächter und Modulator der angeborenen Immunabwehr etabliert. Abstract:RATIONALE:Pulmonary alveolar proteinosis (PAP) is a rare lung disorder characterized by the abnormal accumulation of alveolar surfactant protein in alveolar spaces. Secondary PAP can result from myelodysplastic syndrome (MDS).PATIENT CONCERNS:But most reports described a single case; here we reported 2 cases of PAP secondary to MDS. One case developed secondary PAP at the same time as MDS, and the other developed during the course of MDS.DIAGNOSES:The diagnosis of PAP was made by bronchoalveolar lavage and based on the identification of periodic acid-Schiff-positive proteinaceous material. Chest high resolution CT (HRCT) scans showed variable distribution of ground glass opacities, but crazy-paving appearance was not seen in our 2 cases.INTERVENTIONS:Because the patients' general conditions were poor, whole lung lavage was not used in the 2 cases.OUTCOMES:And the 2 cases' prognoses were poor.LESSONS:In conclusion, pulmonary physicians should suspect the possibility of secondary PAP when they encounter unexplained pulmonary infiltrates with some hematologic or infectious disease that shows diffuse bilateral GGO on an HRCT scan.

Abstract:Objective To analyze the clinical and radiological characteristics of pulmonary alveolar proteinosis(PAP),and evaluate the effect of whole lung lavage treatment.Methods Clinical data of 15 patients(2009~2011) with confirmed PAP were analyzed retrospectively.Results The rate of misdiagnosis of PAP was high.5 patients had a history of metal dusts and special chemical substance exposure,and were diagnosed as secondary PAP.Clinical symptoms were atypical,dyspnea after exertion and cough were the main complains.Levels of LDH and tumor markers in blood increased in some cases.Geographic and crazy paving were the typical radiological characteristics.6 patients were treated with whole lung lavage treatment,both the clinical characteristic and the pulmonary function were improved significantly.Conclusion The characteristic changes on chest CT is useful in early diagnosis of PAP,and the whole lung lavage is the most effective and standard treatment of PAP so far.

Abstract:Pulmonary alveolar proteinosis (PAP) is a life-threatening, rare lung syndrome for which there is no cure and no approved therapies. PAP is a disease of lipid accumulation characterized by alveolar macrophage foam cell formation. While much is known about the clinical presentation, there is a paucity of information regarding temporal changes in lipids throughout the course of disease. Our objectives were to define the detailed lipid composition of alveolar macrophages in PAP patients at the time of diagnosis and during treatment. We performed comprehensive mass spectrometry to profile the lipid signature of alveolar macrophages obtained from three independent mouse models of PAP and from PAP and nonPAP patients. Additionally, we quantified changes in macrophage-associated lipids during clinical treatment of PAP patients. We found remarkable variations in lipid composition in PAP patients, which were consistent with data from three independent mouse models. Detailed lipidomic analysis revealed that the overall alveolar macrophage lipid burden inversely correlated with clinical improvement and response to therapy in PAP patients. Specifically, as PAP patients experienced clinical improvement, there was a notable decrease in the total lipid content of alveolar macrophages. This crucial observation suggests that the levels of these macrophage-associated lipids can be utilized to assess the efficacy of treatment. These findings provide valuable insights into the dysregulated lipid metabolism associated with PAP, offering the potential for lipid profiling to serve as a means of monitoring therapeutic interventions in PAP patients.

Abstract:Background: Pulmonary alveolar proteinosis (PAP) is a rare lung disease characterized by surfactant accumulation, and is caused by disruption of granulocyte/macrophage colony-stimulating factor (GM-CSF) signaling. Abnormalities in CSF2 receptor alpha (CSF2RA) were reported to cause pediatric hereditary PAP. We report here the first case of CSF2RA-mutated, elderly-onset hereditary (h) PAP. Case presentation: The patient developed dyspnea on exertion, and was diagnosed with PAP at the age of 77 years, based on findings from chest computed tomography scan and bronchoalveolar lavage. She tested negative for GM-CSF autoantibodies, with no underlying disease. Her serum GM-CSF level was elevated (91.3 pg/mL), indicating GM-CSF signaling impairment and genetic defects in the GM-CSF receptor. GM-CSF-stimulated phosphorylation in signal transducer and activator of transcription 5 (STAT5) was not observed, and GM-CSF-Rα expression was defective in her blood cells. Genetic screening revealed a homozygous, single-base C > T mutation at nt 508-a nonsense mutation that yields a stop codon (Q170X)-in exon 7 of CSF2RA. High-resolution analysis of single nucleotide polymorphism array confirmed a 22.8-Mb loss of heterozygosity region in Xp22.33p22.11, encompassing the CSF2RA gene. She was successfully treated with whole lung lavage (WLL), which reduced the serum levels of interleukin (IL)-2, IL-5, and IL-17, although IL-3 and M-CSF levels remained high. Conclusions: This is the first known report of elderly-onset hPAP associated with a CSF2RA mutation, which caused defective GM-CSF-Rα expression and impaired signaling. The analyses of serum cytokine levels during WLL suggested that GM-CSF signaling might be compensated by other signaling pathways, leading to elderly-onset PAP.

Abstract:Objective:To correlate the radi ologic manifestations of pulmonary alveolar proteinosis.Methods:The clinical histopathologic records an d CT images in 13 patients with PAP were reviewed.Thirteen patients,including 8 men and 5 woman (mean age,46 years),were proved by lavage and transbronchial bio psy and underwent bilateral whole lung lavage.Results:The radiographs showed bilateral diffus e reticular alveolar consolidation in all the patients.HRCT scans revealed crazy paving appearance (CPA) pattem with or without patchy consolidation in all the patients.The pattems of destribution were diffuse in 8,central in 3 and peripher al in 2 cases.Eight patients had clear border of lesion and positive right angle sign.Other findings included air bronchiogram in 13 and enlargement of lymph no de in 8 cases.Conclusion:PAP is a rare diffuse pulmonary pare nchymal disease.CPA on HRCT is typical and nonspecific finding for PNP.Final dia gnosis of PNP is made by microscopic studies of alveolar lavage and lung biopsy.

Abstract:OBJECTIVETo investigate the expression of granulocyte-macrophage colony-stimulating factor (GM-CSF), GM-CSF/IL-3/IL-5 receptor common beta chain (GM-CSFbetacR), surfactant protein (SP)-A and SP-D in adult patients with idiopathic pulmonary alveolar proteinosis (PAP), so as to demonstrate the possible association of GM-CSF, GM-CSFbetacR, SP-A and SP-D with human PAP.METHODSThe protein expression of GM-CSF, GM-CSFbetacR, SP-A and SP-D in the alveolar macrophages and alveolar type II epithelial cells was detected by immunohistochemical staining.RESULTSThe age was 51.8 +/- 9.5 years old for patients with idiopathic PAP (6; males 4, females 2) and 48.8 +/- 12.6 years old for the control adult individuals (6; males 4, females 2). There was no difference in the expression of GM-CSF (P = 0.818) and SP-D (P = 0.24) in alveolar macrophages between patients with idiopathic PAP and the controls. The expression of GM-CSFbetacR (P = 0.002) and SP-A (P = 0.004) in alveolar macrophages of patients with idiopathic PAP was less than that in the controls. There was no expression of GM-CSF in alveolar type II epithelial cells of patients with idiopathic PAP and the controls. There was a little expression of GM-CSFbetacR in alveolar type II epithelial cells of the controls. There was no expression of GM-CSFbetacR in alveolar type II epithelial cells of patients with idiopathic PAP. There was no difference in the expression of SP-A (P = 0.818) and SP-D (P = 0.485) in alveolar type II epithelial cells between patients with idiopathic PAP and the controls.CONCLUSIONThe decreased protein expression of GM-CSFbetacR in alveolar macrophages may be one of the mechanisms causing dysfunction of the macrophages in adult patients with idiopathic PAP.
