Abstract:Abstract Here we report two patients with a car accident injury who suffered a cerebral infarction following a missed diagnosis of blunt cerebrovascular injury. Difficulty in diagnosis of these cases because of no neurological signs or symptoms on initial assessment is a lesson for physicians from emergency room. Our conclusion upon dealing with the situation is that traumatic blunt cerebrovascular injury has potentially devastating consequences. There is often a delay between the trauma and the onset of symptoms of neurological deficit. Therefore, clinicians should be cautious in this situation. Early detection and intervention are needed to prevent the development of ischemic stroke secondary to blunt cerebrovascular injury in combination with cerebral infarction, thereby improving the prognosis of patients.

Abstract:Cerebral venous thrombosis is an important etiology of stroke in young patients. Its clinical manifestations are variable and based on different involved venous or sinus processes. Cerebral venous thrombosis could mimic ischemic infarction and is easy to misdiagnose. Although many patients have favorable outcomes, delayed or incorrect diagnosis due to atypical symptoms may lead to a poor prognosis. Here we present a case of a 33-year-old woman with transient headache and recurrent right extremity weakness whose symptoms progressed and were sustained in the hospital. She was diagnosed with ischemic infarction and recombinant tissue plasminogen activator (rtPA) thrombolysis was performed. However, her symptoms progressed, and intracranial hematoma was found on a computed tomography scan. Ruling out other hemorrhage etiology, we confirmed the presence of cerebral venous thrombosis using magnetic resonance venography. She underwent mechanical thrombectomy and her condition improved thereafter. This case raises the awareness that in young woman patients on oral contraceptives with neurological deficits and headache, cerebral venous thrombosis is a considerable diagnosis. A contrast CT or MRI scan should be ordered in the early course of evaluation, which can help the physician to make the right clinical decision.
Abstract TP47: Intracranial Intraplaque Hemorrhage in Acute Stroke: Interim Results of the Stroke Imaging Package Study

Abstract:Introdution: Intracranial artery atherosclerosis is an important cause of ischemic stroke, especially in people of Asian origin. Intraplaque hemorrhage is supposed to be a predictor of ischemic event. The goal of this study was to examine the occurrence of intraplaque hemorrhage in a large cohort of Chinese patients with acute ischemic stroke. Methods: Patients with first ever stroke within 72 hours from onset, confirmed by diffusion weighted imaging, were recruited from 16 medical centers. Conventional MRI, magnetic resonance angiography, HRMRI (including 3-dimentional T1 images and 2-dimentional T2 images), and SWI were performed. Intraplaque hemorrhage was identified if hyperintense signals on 3-dimentional T1 images of HRMRI were observed within intracranial plaques. Results: Six hundred and one patients (70% male, mean age 61±16 years old, mean NIHSS 6± 5) were enrolled. Median time from symptom onset to MRI was 44 ± 20 hours. Of them, 240(40%) were diagnosed with large artery atherosclerosis (LAA) stroke. Intracranial intraplaque hemorrhage was identified in 15 intracranial plaques of 13 patients with LAA stroke. Six plaques with intraplaque hemorrhage were asymptomatic and irrelevant to ischemic infarct lesions. In 2 patients, both symptomatic and asymptomatic intraplaque hemorrhage were observed. Conclusions: The prevalence of intracranial intraplaque hemorrhage was low (5.4%)in acute stroke patients. The clinical importance of asymptomatic intraplaque hemorrhage need further investigations.

Abstract:A 40-year-old man suffered severe brain injury and received left side subdural hematoma evacuation with decompressive craniectomy. Intraoperative brain swelling had occurred during the surgery. Postoperative computed tomography (CT) scan was done immediately and showed a contralateral epidural hematoma resulting in herniation. Secondary hematoma evacuation was performed and found a linear fracture near a bleeding meningeal artery. 2 days later CT scan showed cerebral infarction mainly in right posterior cerebral artery distribution. Early diagnosis by postoperative CT scan or other potential ways such as intraoperative sonography is important to prompt treatments and interrupt the pathophysiological chain of the serial attacks.

Abstract:BACKGROUND Cardiac injury may occur after acute pathology of central nervous system（CNS）without any evidence of primary cardiac diseases.The resulting structural and/or functional changes are called cerebrocardiac syndrome（CCS）.The great majority of studies have been performed in patients with subarachnoid hemorrhage（SAH）, while CCS data after intracerebral hemorrhage（ICH） are rare.It may cause diagnostic and therapeutic pitfalls for the clinician due to a lack of specific clinical manifestations and diagnostic methods.Understanding the underlying pathophysiological and molecular mechanism（s） following cerebrovascular incidents will help to implement prevention and treatment strategies to improve the prognosis.CASE SUMMARY A 37-year-old man with a history of hypertension presented to our department on an emergency basis because of a sudden dizziness and left limb weakness.Cerebral computed tomography（CT） suggested ICH in the occipital and parietal lobes, and the chosen emergency treatment was hematoma evacuation.Left ventricular（LV） dysfunction occurred after the next 48 h and the electrocardiogram（ECG） showed non-ST elevation myocardial infarction.CCS was suspected first in the context of ICH due to the negative result of the coronary CT angiogram.CONCLUSION Misinterpretation of ischemic-like ECGs may lead to unnecessary or hazardous interventions and cause undue delay of rehabilitation after stroke.Our objective is to highlight the clinical implications of CCS and we hope the differential diagnoses will be considered in patients with acute CNS diseases.

Abstract:A 43-year-old male patient with no known past medical history presented to the emergency department with new-onset bitemporal headache, dizziness, and bilateral lower extremity weakness for 1 day. The patient denied chest pain, shortness of breath, cough, or recent exposure to sick individuals. He was not on any medications and denied alcohol or illicit drug use. Vital signs were unremarkable. Physical examination was notable for a left-sided pronator drift and bilateral dysmetria that was more pronounced on the left. Results of routine laboratory workup, including complete blood count, metabolic panel, and high-sensitivity troponin level, were normal. An electrocardiogram revealed sinus tachycardia with a heart rate of 102 beats per minute, T-wave inversions in the inferior leads, left axis deviation, incomplete right bundle branch block, and frequent premature ventricular contractions. A radiograph of the chest was unremarkable. CT of the head without contrast enhancement demonstrated no acute intracranial abnormities. MRI of the brain without contrast enhancement revealed multiple acute infarcts involving left posterior inferior cerebellar artery distribution, right cerebellar hemisphere, right mesial temporal lobe, and right posterior limb of the internal capsule. CT angiography of the head and neck showed an occlusion of the right posterior cerebral artery near its origin, with a trace of distal flow. Given that these findings were concerning for a cardioembolic etiology of acute ischemic stroke, transesophageal echocardiography was performed. This showed mild left ventricular systolic dysfunction with an ejection fraction of 40%, mild global hypokinesis, and an additional finding also seen at subsequent cardiac CT and MRI that will be disclosed in part 2 of the case. The patient was started on systemic anticoagulation and guideline-directed medical therapy for heart failure with reduced ejection fraction. CT of the chest showed no evidence of lymphadenopathy or abnormalities in the lung parenchyma or interstitium. Coronary CT angiography was performed (Fig 1), followed by cardiac MRI (Fig 2).
Reversible cerebral vasoconstriction syndrome with basilar artery stenosis: A case report

Abstract:Introduction: Acute severe headaches in young patients may be associated with fatal neurological complications that necessitate imaging examinations. Among acute severe headaches, a thunderclap headache may indicate the rupture of a cerebral aneurysm or the onset of reversible cerebral vasoconstriction syndrome for which emergent evaluation is required. Patient concerns: We report the case of a 36-year-old man who presented to our hospital with an acute severe headache after excessive exercise the previous day. He was prescribed a pain reliever and discharged under the suspicion of vestibular migraine but returned to the emergency room after 4 hours due to right hemiparesis, right facial palsy, severe dysarthria, and a mild drowsy mental status. Diagnosis: After cerebral angiography, we diagnosed basilar artery stenosis with acute infarction in the posterior circulation due to reversible cerebral vasoconstriction syndrome. Interventions: Brain computed tomography angiography revealed complete occlusion of the vertebrobasilar artery. Transfemoral cerebral angiography showed spontaneous improvement in the occlusion before thrombectomy. Outcomes: Ten months later, high-resolution vessel wall magnetic resonance angiography showed persisting severe stenosis of the basilar artery. Conclusions: A headache in young patients with risk factors of atherosclerosis, such as smoking history, uncontrolled hypertension, and dyslipidemia may be caused by reversible cerebral vasoconstriction syndrome or ischemic stroke, which has fatal neurological complications. Therefore, reversible cerebral vasoconstriction syndrome or ischemic stroke should be suspected and appropriately evaluated in such patients, even if the headache is not the thunderclap type.

Abstract:A 50-year-old man presented with headache. Examination showed left sided ataxic hemiparesis and elevated blood pressure. Brain imaging revealed an acute intracerebral hemorrhage in the right lentiform nucleus, deep and periventricular white matter hyperintensities, and predominantly deep cerebral microbleeds. Fundus examination showed important arteriolar tortuosity involving several blood vessels. In this young patient, we explain the diagnostic approach to intracerebral hemorrhage, the causes of cerebral small vessel disease, and the interpretation of biomolecular tests.

Abstract:A 36-year-old woman presented with sudden onset of a right-sided headache that awoke her from sleep. She had no episodes of trauma or abuse. She was initially able to speak, but fell into a coma within an hour. The right pupil was dilated, with slow pupillary reflexes to light on both sides, and she showed left hemiparalysis. Computed tomography scan showed a right acute epidural hematoma, approximately 4 cm in thickness, and there were no findings of trauma such as skin wounds, subcutaneous hematomas, or skull fractures. In the emergency room, decompression of intracranial pressure by one burr hole was performed, and her dilated right pupil improved to normal size. She was then moved to the operating room, and hematoma removal was performed by craniotomy. Her blood pressure trended downward despite rapid blood transfusion and vasopressor therapy. There were no abnormal findings apparent intraoperatively, except for oozing from the whole surface of the dura mater and epidural space. Her consciousness improved postoperatively, and her left hemiparalysis improved within a few days. No causative diseases, risk factors, or vascular abnormalities were found on laboratory and radiological surveys. Two months postoperatively, the bone flap was removed because of infection. Eight months postoperatively, a cranioplasty using artificial skull was performed, and her postoperative course was uneventful. One year after the initial surgery, she has no neurological deficits, and there has been no recurrence of epidural hematoma.

History of Present Illness: A 53-year-old man was admitted for acute onset of left hemiparesis, left facial droop and dysarthria witnessed by his wife (a nurse) while they were watching TV that evening. She reported the patient had no previous history of coronary artery disease or cerebral vascular disease, prior to an admission occurring three weeks earlier. The patient presented at that time with acute, severe left-sided chest pain that began while he was doing some heavy yardwork. While being evaluated in the emergency department (ED), he developed left-sided facial numbness, hemiparesis and dysarthria. A CT scan of the brain was normal. Neurological symptoms resolved before lytic therapy could be administered. Troponins and EKG were normal. Relationship between Headache and Characteristics of Cerebral Small Vessel Disease Imaging in Young Stroke Patients

Abstract:Objective The study aimed to investigate the prevalence of cerebral small vessel disease (SVD), the prevalence of headache, and the relationship between SVD and headache in non-hereditary young stroke patients. Methods A retrospective analysis was made upon 200 patients who were hospitalized in department of neurology of Peking University Third Hospital due to first-ever ischemic stroke during January 2006 and January 2015. All patients were aged 18 to 45 years old, who underwent MR imaging (MRI) and MR angiography (MRA) within 7 days after stroke onset. Patients were divided into two groups: those with headache and those without headache. The prevalence of ischemic SVD in 2 groups, including asymptomatic lacunar infarcts (ALAs), white matter lesions (WMLs), and enlarged perivascular spaces (EPVS), were compared to study headache associated factors.Results Of the 200 patients,male had higher prevalence(male vs female=6.41:1).Of all,32.5% (65/200) had ALAs, 25.5% (51/200) had WMLs, and 30.5% (61/200) had EPVS. There were 11% (22/200) had headache during acute stroke onset. ALAs and EPVS were less frequently associated with headache (P=0.045 and P=0.021 respectively). The percentage of Non-large artery atherosclerosis type in Trial of Org 10 172 in Acute Stroke Treatment (Toast) was significantly higher in headache group than that in non-headache group[63.6%(14/22)vs 26.4%(47/178),P=0.001].The lesions located under the tentorium (brain stem or cerebellum) were significantly higher in headache group than that in non-headache group[63.6%(14/22)vs 26.4%(47/178),P<0.001].Culprit vessels in the posterior circulation were significantly higher in headache group than that in non-headache group [68.2%(15/22)vs 37.6%(67/178),P=0.006]. Conclusion Our data suggest that the lesions in young patients with headache onset during acute stroke mainly located under the tentorium (brain stem or cerebellum) and culprit vessels mostly in the posterior circulation. Onset of headache is significantly lower in the SVD group. Headache is significantly higher in the non-large artery atherosclerosis type in TOAST.

Impact of sleep disturbance on longitudinal cognitive performance in patients with transient ischemic attack or mild stroke

Abstract:Introduction: Sleep disturbances including obstructive sleep apnea (OSA) and poor sleep quality are common after stroke, while its association with cognitive changes following transient ischemic attack (TIA) or mild stroke remains unclear. We aim to determine whether sleep duration, OSA parameters, or nocturnal hypoxemia is associated with a greater cognitive decline after stroke. Methods: We prospectively followed-up patients with acute TIA/mild stroke [National Institute Health Stroke Scale (NIHSS) < 7] who underwent baseline sleep questionnaire [Pittsburgh Sleep Quality Index (PSQI)], and serial cognitive assessments [Montreal Cognitive Assessment (MoCA) 5-min, Stroop Test] at baseline and one-year. We also evaluated apnea-hypopnea index (AHI) and nocturnal hypoxemia by Home Sleep Apnea Test (HSAT) at one-year. Primary outcome was one-year change in MoCA 5-min score. Results: One hundred and five patients with TIA/mild stroke (mean age 63 years, 65 % male) were included. Baseline short sleep (< 6 hour/night) and AHI ≥ 20/hour at one-year were independently associated with a decline in the MoCA 5-min total score after covariates adjustment [short sleep: β = -2.36 95 % confidence interval (CI) (-4.13, -0.59), p = 0.009; AHI ≥ 20/hour: β = -1.79 (-3.26, -0.32), p = 0.017; remained significant after multiple comparisons correction]. A lower mean MinSpO2 was associated with a decline in executive function [Stroop interference index: β = 0.29 (0.04, 0.53), p = 0.021], but not with MoCA 5-min score at one-year. Moderation analysis indicated AHI ≥ 20/hour was associated with a pronounced decline in executive function only in men. Conclusions: Short sleep after stroke onset, AHI ≥ 20/hour and nocturnal hypoxemia at one-year contributed to an impaired cognitive trajectory at one-year following stroke in patients with TIA/mild stroke.
Cerebral hemodynamics and stroke risks in symptomatic intracranial atherosclerotic stenosis with internal versus cortical borderzone infarcts: A computational fluid dynamics study

Abstract:There may be different mechanisms underlying internal (IBZ) and cortical (CBZ) borderzone infarcts in intracranial atherosclerotic stenosis. In 84 patients with symptomatic, 50-99% atherosclerotic stenosis of M1 middle cerebral artery (MCA-M1) with acute borderzone infarcts in diffusion-weighted imaging, we classified the infarct patterns as isolated IBZ (n = 37), isolated CBZ (n = 31), and IBZ+CBZ (n = 16) infarcts. CT angiography-based computational fluid dynamics models were constructed to quantify translesional, post-stenotic to pre-stenotic pressure ratio (PR) in the MCA-M1 lesion. Those with IBZ infarcts were more likely to have a low PR (indicating impaired antegrade flow across the lesion) than those without (p = 0.012), and those with CBZ infarcts were more likely to have coexisting small cortical infarcts (indicating possible embolism) than those without (p = 0.004). In those with isolated IBZ or CBZ infarcts, low PR was independently associated with isolated IBZ infarcts (adjusted odds ratio = 4.223; p = 0.026). These two groups may also have different trajectories in the stroke risks under current medical treatment regimen, with a higher risk of same-territory ischemic stroke recurrence within 3 months in patients with isolated IBZ infarcts than isolated CBZ infarcts (17.9% versus 0.0%; log-rank p = 0.023), but similar risks later in 1 year.

Abstract:Patient 1 (82 years old) had recurrent weakness and numbness in the left upper extremity, Patient 2 (71 years old) had transient dysarthria, and Patient 3 (70 years old) had transient apraxia of speech. Transient ischemic attack (TIA) was suspected; however, all three women had a history of head trauma before the symptom onset, and magnetic resonance imaging revealed subdural hematomas near the responsible lesions associated with sulcal hyperintensity (SHI) in the cerebral sulcus near the hematoma. Patients 1 and 2 improved spontaneously, whereas Patient 3 improved with antiseizure medication. Subdural hematomas associated with SHI may have transient focal neurological deficits that manifest through a mechanism unlike TIA.

Abstract:Headaches are common after ischemic stroke (IS). Unlike primary headaches, headaches attributed to IS have specific clinical features. This review describes the epidemiology, clinical characteristics, risk factors, and influence of IS headaches. Previous reports were summarized to show the correlations between headaches and structural lesions in the cerebral cortex, subcortical white matter, deep gray matter nuclei, brainstem, and cerebellum. However, the substantial heterogeneity of IS, subjective evaluations of headaches, and inadequate cohort studies make it difficult to explore the pathophysiology of headaches attributed to IS. In our recommendation, favorable imaging techniques, such as magnetic resonance imaging and positron emission tomography, may provide new insights into mechanical studies of IS headaches from structure to function. It may also be helpful to extend the research field by targeting several shared signal transducers between headaches and IS. These markers might be neuropeptides, vasoactive substances, ion channels, or electrophysiologic changes.

Abstract:Acute ischemic stroke in young adults typically carries significant implications for morbidity, mortality, and long-term disability. In this study, we describe the case of a 34-year-old male with no prior medical history who presented with symptoms of right-sided weakness and slurred speech, suggesting an acute ischemic stroke. Initial CT angiography revealed an occlusion in the left M2 segment middle cerebral artery (MCA). The occlusion was successfully recanalized through emergent endovascular thrombectomy, which also identified a dissection as the cause of the stroke. Follow-up assessments at 3 days and three months, which included advanced vessel wall MRI, highlighted the critical role of intracranial artery dissection in strokes among young adults and provided essential images for ongoing evaluation.

Abstract:A 27-year-old man with a history of migraines, epilepsy and pulmonary stenosis presented to the emergency department with symptoms of vomiting, headache, visual disturbance and problems with balance. The team considered the possibility of intracranial pathology and an urgent CT head with contrast showed what appeared to be a large posterior fossa mass with an appearance suggestive of a primary haemangioblastoma, which was causing considerable mass effect. The patient had neurosurgery to relieve the obstruction and a biopsy of the area showed the mass to be an ischaemic infarct rather than a tumour. Further investigations following the stroke confirmed that the cause was due to having antiphospholipid syndrome and a patent foramen ovale. The patient made a good recovery following the operation and remains well.

Abstract:Objective To investigate the etiology and risk factors of ischemic stroke in young adults.MethodsRetrospective analysis the clinical data of 45 cases of ischemic stroke in young adults and comparative analysis of ischemic stroke data with 33 elderly patients.Results The causes and risks of ischemic stroke in young adults followed by hypertension,hyperlipidemia,atherosclerosis,smoking,drinking,hyperfibringenemia,hyperhomocysteinaemia,diabetes,migraine headache,artery dissection,patent foramen ovale and arteritis.The hyperlipodemia,migraine headacke and artery dissection are statistical significance in young group compared with the older one.Conclusion The etiology and risks in young stroke are not identical with the older group.So active treatment of hyperlipidemia,migraine and arterial dissection will be more meaningful of prevention of ischemic stroke in young people.
