Abstract:Abstract Artificial intelligence (AI) has made significant advances in radiology. Nonetheless, challenges in AI development, validation, and reproducibility persist, primarily due to the lack of high-quality, large-scale, standardized data across the world. Addressing these challenges requires comprehensive standardization of medical imaging data and seamless integration with structured medical data. Developed by the Observational Health Data Sciences and Informatics community, the OMOP Common Data Model enables large-scale international collaborations with structured medical data. It ensures syntactic and semantic interoperability, while supporting the privacy-protected distribution of research across borders. The recently proposed Medical Imaging Common Data Model is designed to encompass all DICOM-formatted medical imaging data and integrate imaging-derived features with clinical data, ensuring their provenance. The harmonization of medical imaging data and its seamless integration with structured clinical data at a global scale will pave the way for advanced AI research in radiology. This standardization will enable federated learning, ensuring privacy-preserving collaboration across institutions and promoting equitable AI through the inclusion of diverse patient populations. Moreover, it will facilitate the development of foundation models trained on large-scale, multimodal datasets, serving as powerful starting points for specialized AI applications. Objective and transparent algorithm validation on a standardized data infrastructure will enhance reproducibility and interoperability of AI systems, driving innovation and reliability in clinical applications.

Abstract:The rapid growth of artificial intelligence (AI) and deep learning techniques require access to large inter-institutional cohorts of data to enable the development of robust models, e.g., targeting the identification of disease biomarkers and quantifying disease progression and treatment efficacy. The Observational Medical Outcomes Partnership Common Data Model (OMOP CDM) has been designed to accommodate a harmonized representation of observational healthcare data. This study proposes the Medical Imaging CDM (MI-CDM) extension, adding two new tables and two vocabularies to the OMOP CDM to address the structural and semantic requirements to support imaging research. The tables provide the capabilities of linking DICOM data sources as well as tracking the provenance of imaging features derived from those images. The implementation of the extension enables phenotype definitions using imaging features and expanding standardized computable imaging biomarkers. This proposal offers a comprehensive and unified approach for conducting imaging research and outcome studies utilizing imaging features.

Abstract:Background and Objectives Artificial intelligence (AI) models trained on multi-centric and multi-device studies can provide more robust insights and research findings compared to single-center studies. However, variability in acquisition protocols and equipment can introduce inconsistencies that hamper the effective pooling of multi-source datasets. This systematic review evaluates strategies for image harmonization, which standardizes appearances to enable reliable AI analysis of multi-source medical imaging. Methods A literature search using PRISMA guidelines was conducted to identify relevant papers published between 2013-2023 analyzing multi-centric and multi-device medical imaging studies that utilized image harmonization approaches. Results Common image harmonization techniques included grayscale normalization (improving classification accuracy by up to 24.42%), resampling (increasing the percentage of robust radiomics features from 59.5% to 89.25%), and color normalization (enhancing AUC by up to 0.25 in external test sets). Initially, mathematical and statistical methods dominated, but machine and deep learning adoption has risen recently. Color imaging modalities like digital pathology and dermatology have remained prominent application areas, though harmonization efforts have expanded to diverse fields including radiology, nuclear medicine, and ultrasound imaging. In all the modalities covered by this review, image harmonization improved AI performance, with increasing of up to 24.42% in classification accuracy and 47% in segmentation Dice scores. Conclusions Continued progress in image harmonization represents a promising strategy for advancing healthcare by enabling large-scale, reliable analysis of integrated multi-source datasets using AI. Standardizing imaging data across clinical settings can help realize personalized, evidence-based care supported by data-driven technologies while mitigating biases associated with specific populations or acquisition protocols.

Abstract:This paper reviews state-of-the-art research solutions across the spectrum of medical imaging informatics, discusses clinical translation, and provides future directions for advancing clinical practice. More specifically, it summarizes advances in medical imaging acquisition technologies for different modalities, highlighting the necessity for efficient medical data management strategies in the context of AI in big healthcare data analytics. It then provides a synopsis of contemporary and emerging algorithmic methods for disease classification and organ/ tissue segmentation, focusing on AI and deep learning architectures that have already become the de facto approach. The clinical benefits of in-silico modelling advances linked with evolving 3D reconstruction and visualization applications are further documented. Concluding, integrative analytics approaches driven by associate research branches highlighted in this study promise to revolutionize imaging informatics as known today across the healthcare continuum for both radiology and digital pathology applications. The latter, is projected to enable informed, more accurate diagnosis, timely prognosis, and effective treatment planning, underpinning precision medicine.

Abstract:The National Institutes of Health in 2018 identified key focus areas for the future of artificial intelligence in medical imaging, creating a foundational roadmap for research in image acquisition, algorithms, data standardization and translatable clinical decision support systems. Among the key issues raised in the report, data availability, the need for novel computing architectures and explainable artificial intelligence algorithms are still relevant, despite the tremendous progress made over the past few years alone. Furthermore, translational goals of data sharing, validation of performance for regulatory approval, generalizability and mitigation of unintended bias must be accounted for early in the development process. In this Perspective, we explore challenges unique to high-dimensional clinical imaging data, in addition to highlighting some of the technical and ethical considerations involved in developing machine learning systems that better represent the high-dimensional nature of many imaging modalities. Furthermore, we argue that methods that attempt to address explainability, uncertainty and bias should be treated as core components of any clinical machine learning system.

Abstract:The integration, open accessing of healthcare data, and the use of artificial intelligence to organize and analyze fragmented medical information can improve medical and health services, promote the level of rational government decision-making, and reduce the inequality in the allocation of medical and health resources. This paper summarizes the current status of technologies and applica-tions of artificial intelligence in the field of medical information semantic fusion and in the field of image analysis, and analyzes current problems and challenges. The first is the standardized representation and structural integration of medical information to merge nation-al and widely-used clinical terminologies, which is key to realizing auxiliary diagnosis based on 'big data' artificial intelligent. The second is the use of massive medical knowledge to construct an intelligent diagnosis and treatment model with the ability to combine multimodal data analysis and structured knowledge reasoning. Thus, we propose a national-level healthcare open data cloud platform that can help open up new data markets, improve the integration of healthcare data, and provide the new service of knowledge dis-covery and services. We also suggest to establish some basic industry standards for medical and health information, to strengthen the research and development of domestic medical devices, to promote the development of intelligent medical devices and smart wearable devices, and to guide the industry to open up new markets on the combination of artificial intelligence and medical devices.

Abstract:With promising Artificial Intelligence (AI) algorithms receiving FDA (Food and Drug Administration) clearance, the potential impact of these models on clinical outcomes must be evaluated locally before their integration into routine workflows. Robust validation infrastructures are pivotal to inspecting the accuracy and generalizability of these deep learning algorithms to ensure both patient safety and health equity. Protected Health Information (PHI) concerns, intellectual property rights, and diverse requirements of models impede the development of rigorous external validation infrastructures. Our work proposes various suggestions for addressing the challenges associated with the development of efficient, customizable, and cost-effective infrastructures for the external validation of AI models at large medical centers and institutions. We present comprehensive steps to establish an AI inferencing infrastructure outside clinical systems to examine the local performance of AI algorithms before health practice- or system-wide implementation and promote an evidence-based approach for adopting AI models that can enhance radiology workflows and improve patient outcomes.

Abstract:Abstract Artificial intelligence (AI) is transforming the field of medical imaging and has the potential to bring medicine from the era of ‘sick-care’ to the era of healthcare and prevention. The development of AI requires access to large, complete, and harmonized real-world datasets, representative of the population, and disease diversity. However, to date, efforts are fragmented, based on single–institution, size-limited, and annotation-limited datasets. Available public datasets ( e.g. , The Cancer Imaging Archive, TCIA, USA) are limited in scope, making model generalizability really difficult. In this direction, five European Union projects are currently working on the development of big data infrastructures that will enable European, ethically and General Data Protection Regulation-compliant, quality-controlled, cancer-related, medical imaging platforms, in which both large-scale data and AI algorithms will coexist. The vision is to create sustainable AI cloud-based platforms for the development, implementation, verification, and validation of trustable, usable, and reliable AI models for addressing specific unmet needs regarding cancer care provision. In this paper, we present an overview of the development efforts highlighting challenges and approaches selected providing valuable feedback to future attempts in the area. Abstract:Abstract Radiomics is a novel technique in which quantitative phenotypes or features are extracted from medical images. Machine learning enables analysis of large quantities of medical imaging data generated by radiomic feature extraction. A growing number of studies based on these methods have developed tools for neuro-oncology applications. Despite the initial promises, many of these imaging tools remain far from clinical implementation. One major limitation hindering the use of these models is their lack of reproducibility when applied across different institutions and clinical settings. In this article, we discuss the importance of standardization of methodology and reporting in our effort to improve reproducibility. Ongoing efforts of standardization for neuro-oncological imaging are reviewed. Challenges related to standardization and potential disadvantages in over-standardization are also described. Ultimately, greater multi-institutional collaborative effort is needed to provide and implement standards for data acquisition and analysis methods to facilitate research results to be interoperable and reliable for integration into different practice environments.

Abstract:Artificial intelligence (AI) in radiology is a rapidly developing field with several prospective clinical studies demonstrating its benefits in clinical practice. In 2022, the Korean Society of Radiology held a forum to discuss the challenges and drawbacks in AI development and implementation. Various barriers hinder the successful application and widespread adoption of AI in radiology, such as limited annotated data, data privacy and security, data heterogeneity, imbalanced data, model interpretability, overfitting, and integration with clinical workflows. In this review, some of the various possible solutions to these challenges are presented and discussed; these include training with longitudinal and multimodal datasets, dense training with multitask learning and multimodal learning, self-supervised contrastive learning, various image modifications and syntheses using generative models, explainable AI, causal learning, federated learning with large data models, and digital twins.

Abstract:The Radiological Society of North of America (RSNA) and the Medical Image Computing and Computer Assisted Intervention (MICCAI) Society have led a series of joint panels and seminars focused on the present impact and future directions of artificial intelligence (AI) in radiology. These conversations have collected viewpoints from multidisciplinary experts in radiology, medical imaging, and machine learning on the current clinical penetration of AI technology in radiology and how it is impacted by trust, reproducibility, explainability, and accountability. The collective points-both practical and philosophical-define the cultural changes for radiologists and AI scientists working together and describe the challenges ahead for AI technologies to meet broad approval. This article presents the perspectives of experts from MICCAI and RSNA on the clinical, cultural, computational, and regulatory considerations-coupled with recommended reading materials-essential to adopt AI technology successfully in radiology and, more generally, in clinical practice. The report emphasizes the importance of collaboration to improve clinical deployment, highlights the need to integrate clinical and medical imaging data, and introduces strategies to ensure smooth and incentivized integration. Abstract:Medical imaging technology plays a key role in modern medical diagnosis and treatment, and the integration of artificial intelligence (AI) technology has revolutionised the field.AI uses deep learning and machine learning algorithms to analyse medical imaging data, improving the accuracy of lesion identification and disease prediction, and thus significantly improving the efficiency of diagnostic work. The scope of application of AI has also expanded to include the optimisation of treatment planning, the prediction of disease progression, and the assessment of patient prognosis. The application of AI has also been extended to the optimisation of treatment plans, prediction of disease progression and assessment of patient prognosis. Although AI shows great potential in medical imaging diagnosis, its clinical application still faces challenges. The quality, accessibility, sensitivity, and privacy of medical image data, as well as the "black box" nature of AI models, pose obstacles to the widespread application of AI technology. In addition, data security and privacy protection are also issues that need to be addressed. This paper reviews the current status of AI application in medical imaging diagnosis, analyses the main problems faced, and discusses the future development direction. Examples of AI applications in different medical imaging fields are discussed in the paper, and challenges such as data quality, laws and regulations, model interpretability and data security are explored in depth, and solution strategies such as enhancing data management, improving model generalisation and interpretability, strengthening data security techniques, and promoting interdisciplinary cooperation are proposed. This paper aims to provide reference for researchers and practitioners of AI in medical imaging diagnosis to promote the healthy development of the field, and calls on experts, scholars, policy makers and technology developers to work together to overcome the challenges and to realize the potential of AI technology in improving the quality and efficiency of healthcare services and safeguarding patients' health.

Abstract:Radiology has been essential to accurately diagnosing diseases and assessing responses to treatment. The challenge however lies in the shortage of radiologists globally. As a response to this, a number of Artificial Intelligence solutions are being developed. The challenge Artificial Intelligence radiological solutions however face is the lack of a benchmarking and evaluation standard, and the difficulties of collecting diverse data to truly assess the ability of such systems to generalise and properly handle edge cases. We are proposing a radiograph-agnostic platform and framework that would allow any Artificial Intelligence radiological solution to be assessed on its ability to generalise across diverse geographical location, gender and age groups.

Abstract:Artificial intelligence (AI), the wide spectrum of technologies aiming to give machines or computers the ability to perform human-like cognitive functions, began in the 1940s with the first abstract models of intelligent machines. Soon after, in the 1950s and 1960s, machine learning algorithms such as neural networks and decision trees ignited significant enthusiasm. More recent advancements include the refinement of learning algorithms, the development of convolutional neural networks to efficiently analyze images, and methods to synthesize new images. This renewed enthusiasm was also due to the increase in computational power with graphical processing units and the availability of large digital databases to be mined by neural networks. AI soon began to be applied in medicine, first through expert systems designed to support the clinician's decision and later with neural networks for the detection, classification, or segmentation of malignant lesions in medical images. A recent prospective clinical trial demonstrated the non-inferiority of AI alone compared with a double reading by two radiologists on screening mammography. Natural language processing, recurrent neural networks, transformers, and generative models have both improved the capabilities of making an automated reading of medical images and moved AI to new domains, including the text analysis of electronic health records, image self-labeling, and self-reporting. The availability of open-source and free libraries, as well as powerful computing resources, has greatly facilitated the adoption of deep learning by researchers and clinicians. Key concerns surrounding AI in healthcare include the need for clinical trials to demonstrate efficacy, the perception of AI tools as 'black boxes' that require greater interpretability and explainability, and ethical issues related to ensuring fairness and trustworthiness in AI systems. Thanks to its versatility and impressive results, AI is one of the most promising resources for frontier research and applications in medicine, in particular for oncological applications.

Abstract:Abstract The rise of digital data and computing power have contributed to significant advancements in artificial intelligence (AI), leading to the use of classification and prediction models in health care to enhance clinical decision-making for diagnosis, treatment and prognosis. However, such advances are limited by the lack of reporting standards for the data used to develop those models, the model architecture, and the model evaluation and validation processes. Here, we present MINIMAR (MINimum Information for Medical AI Reporting), a proposal describing the minimum information necessary to understand intended predictions, target populations, and hidden biases, and the ability to generalize these emerging technologies. We call for a standard to accurately and responsibly report on AI in health care. This will facilitate the design and implementation of these models and promote the development and use of associated clinical decision support tools, as well as manage concerns regarding accuracy and bias.

Abstract:Artificial intelligence (AI) and machine learning (ML) are increasingly used in radiology research to deal with large and complex imaging data sets. Nowadays, ML tools have become easily accessible to anyone. Such a low threshold to accessibility might lead to inappropriate usage and misinterpretation, without a clear intention. Therefore, ensuring methodological rigor is of paramount importance. Getting closer to the real-world clinical implementation of AI, a basic understanding of the main concepts should be a must for every radiology professional. In this respect, simplified explanations of the key concepts along with pitfalls and recommendations would be helpful for general radiology community to develop and improve their AI mindset. In this work, twenty-two key issues are reviewed within three categories: pre-modeling, modeling, and post-modeling. Firstly, the concept is shortly defined for each issue. Then, related common pitfalls and best practices are provided. Specifically, the issues included in this paper were validity of scientific question, unrepresentative samples, sample size, missing data, quality of reference standard, batch effect, reliability of features, feature scaling, multi-collinearity, class imbalance, data and target leakage, high-dimensional data, optimization, overfitting, generalization, performance metrics, clinical utility, comparison with conventional statistical and clinical methods, interpretability and explainability, randomness, transparent reporting, and sharing data.

Abstract:Digital well-being records are multimodal and high-dimensional (HD). Better theradiagnostics stem from new computationally thorough and edgy technologies, i.e., hyperspectral (HSI) imaging, super-resolution, and nanoimaging, but advance mess data portrayal and retrieval. A patient's state involves multiple signals, medical imaging (MI) modalities, clinical variables, dialogs between clinicians and patients, metadata, genome sequencing, and signals from wearables. Patients' high volume, personalized data amassed over time have advanced artificial intelligence (AI) models for higher-precision inferences, prognosis, and tracking. AI promises are undeniable, but with slow spreading and adoption, given partly unstable AI model performance after real-world use. The HD data is a rate-limiting factor for AI algorithms generalizing real-world scenarios. This paper studies many health data challenges to robust AI models' growth, aka the dimensionality curse (DC). This paper overviews DC in the MIs' context, tackles the negative out-of-sample influence and stresses important worries for algorithm designers. AI demands data mixing skills absent in regular systems, requiring hardware/software speed and flexible storage. A partner or service can fulfill anomaly detection, predictive analysis, and ensemble modeling.

Abstract:"Garbage in, garbage out" summarises well the importance of high-quality data in machine learning and artificial intelligence. All data used to train and validate models should indeed be consistent, standardised, traceable, correctly annotated, and de-identified, considering local regulations. This narrative review presents a summary of the techniques that are used to ensure that all these requirements are fulfilled, with special emphasis on radiological imaging and freely available software solutions that can be directly employed by the interested researcher. Topics discussed include key imaging concepts, such as image resolution and pixel depth; file formats for medical image data storage; free software solutions for medical image processing; anonymisation and pseudonymisation to protect patient privacy, including compliance with regulations such as the Regulation (EU) 2016/679 "General Data Protection Regulation" (GDPR) and the 1996 United States Act of Congress "Health Insurance Portability and Accountability Act" (HIPAA); methods to eliminate patient-identifying features within images, like facial structures; free and commercial tools for image annotation; and techniques for data harmonisation and normalisation.Relevance statement This review provides an overview of the methods and tools that can be used to ensure high-quality data for machine learning and artificial intelligence applications in radiology.Key points• High-quality datasets are essential for reliable artificial intelligence algorithms in medical imaging.• Software tools like ImageJ and 3D Slicer aid in processing medical images for AI research.• Anonymisation techniques protect patient privacy during dataset preparation.• Machine learning models can accelerate image annotation, enhancing efficiency and accuracy.• Data curation ensures dataset integrity, compliance, and quality for artificial intelligence development.
Integrating Clinical Data and Medical Imaging in Lung Cancer: Feasibility Study Using the Observational Medical Outcomes Partnership Common Data Model Extension

Abstract:Background: Digital transformation, particularly the integration of medical imaging with clinical data, is vital in personalized medicine. The Observational Medical Outcomes Partnership (OMOP) Common Data Model (CDM) standardizes health data. However, integrating medical imaging remains a challenge. Objective: This study proposes a method for combining medical imaging data with the OMOP CDM to improve multimodal research. Methods: Our approach included the analysis and selection of digital imaging and communications in medicine header tags, validation of data formats, and alignment according to the OMOP CDM framework. The Fast Healthcare Interoperability Resources ImagingStudy profile guided our consistency in column naming and definitions. Imaging Common Data Model (I-CDM), constructed using the entity-attribute-value model, facilitates scalable and efficient medical imaging data management. For patients with lung cancer diagnosed between 2010 and 2017, we introduced 4 new tables—IMAGING_STUDY, IMAGING_SERIES, IMAGING_ANNOTATION, and FILEPATH—to standardize various imaging-related data and link to clinical data. Results: This framework underscores the effectiveness of I-CDM in enhancing our understanding of lung cancer diagnostics and treatment strategies. The implementation of the I-CDM tables enabled the structured organization of a comprehensive data set, including 282,098 IMAGING_STUDY, 5,674,425 IMAGING_SERIES, and 48,536 IMAGING_ANNOTATION records, illustrating the extensive scope and depth of the approach. A scenario-based analysis using actual data from patients with lung cancer underscored the feasibility of our approach. A data quality check applying 44 specific rules confirmed the high integrity of the constructed data set, with all checks successfully passed, underscoring the reliability of our findings. Conclusions: These findings indicate that I-CDM can improve the integration and analysis of medical imaging and clinical data. By addressing the challenges in data standardization and management, our approach contributes toward enhancing diagnostics and treatment strategies. Future research should expand the application of I-CDM to diverse disease populations and explore its wide-ranging utility for medical conditions.
