Abstract:Background: This study aimed to investigate the relationship between the severity of autism, emotional and behavioral problems of autistic children, internalized stigma, depressive symptoms, and primary caregiver parental stress. Specifically, we explored the mediating role of internal stigmatization and total difficulties of individuals with autism on parenting stress and depressive symptoms of the primary caregiver. Method: Mothers of 93 children with autism were included in the study. The mothers were given the Beck Depression Inventory (BDI), The Internalized Stigma of Mental Illness Scale (ISMI), the Autism Behavior Checklist, the Parenting Stress Index-Short Form, Strength, and Difficulties Questionnaire -Parent Form (SDQ-P). Results: As a result of our study, the emotional and behavioral problems of the child and the internalized stigmatization felt by the parent played a mediator role in the relationship between the child's autism severity and the parent's stress and depressive symptoms. Conclusions: Our findings highlight that internalized stigmatization and behavioral characteristics of individuals with autism are among the most critical problems for their primary caregivers. These results have important implications for the development of interventions aimed at reducing the internalized stigma experienced by primary caregivers of individuals with autism and for improving their mental health outcomes.
Sense of coherence, its components and depressive and anxiety symptoms in expecting women and their partners - A FinnBrain Birth Cohort Study

Abstract:Objective: Expecting mothers with high sense of coherence (SOC) exhibit improved physical, emotional, and childbearing health. However, the dimensions of SOC and the factor structure of the SOC-13 scale during prenatal period is slightly known. Especially the differences in experiencing SOC and its components (comprehensibility, manageability, meaningfulness) among both expecting parents (mothers and fathers) is poorly understood. The association between SOC and mood disorder symptoms (depression and anxiety) during pregnancy is scarcely studied. Methods: The structure of the SOC-13 scale, differences in SOC experiences, and the associations between SOC and depressive and anxiety symptoms were studied in a sample of 2784 pregnant women (mothers) and 1661 men/partners (fathers) belonging to the FinnBrain Birth Cohort Study. Self-reports (SOC-13, EPDS, SCL-90: ANX) from gestational week 24 were used. Confirmatory factor analysis (CFA) and invariance testing was carried out to investigate the factorial structure of SOC-13 among both groups (mothers and fathers). Group comparisons were used to study differences in the level of SOC among mothers vs. fathers, low vs. high depression and anxiety subgroups, and multiparous vs. nulliparous mothers. Results: A two-factor model for SOC-13 consisting of comprehensibility-manageability and meaningfulness fitted the data best. Mothers reported higher levels of meaningfulness, whereas fathers reported higher levels of comprehensibility-manageability. SOC was significantly higher among fathers vs. mothers, but mothers with depressive symptoms reported higher SOC than fathers with depressive symptoms. Conclusions: During pregnancy, SOC can be viewed as a two-dimensional (vs. one- or three-dimensional) concept, and mothers and fathers have differences in the components of SOC. Importantly, mothers vs. fathers with depressive symptoms express higher overall SOC indicating that pregnancy may relate to higher than usual SOC especially among women with psychological distress. Understanding how expecting mothers and fathers experience SOC during pregnancy, particularly in relation to depressive symptoms, helps midwives and maternity care providers to focus health promoting support more precisely.
Association between Maternal Depression and Autistic Child’s Characteristics A Mediating Role of Treatment

Abstract:A child's diagnosis of autism spectrum disorder (ASD) alters family dynamics, heightens marital discord and parenting stress, and may result in parental psychopathology. This may impact the long-term prospects for their children. This study aimed to evaluate the prevalence of depression among parents of autistic children and the factors that predict these conditions. Two hundred and thirty file children were diagnosed with autism and their mothers were enlisted from the Sindh Institute of Physical Medicine & Rehabilitation (SIPM&R), Karachi Sindh Pakistan. Beck Depression Inventory (BDI) and The Childhood Autism Rating Scale (CARS) were used to assess depression and the severity of autism, respectively. The demographic information was composed through a predesigned questionnaire. Data were analysed on SPSS version 20. Descriptive and correlation analyses were used for statistical evaluations. The analysis outcomes exhibited that mainstream mothers hold a master’s degree (54.5%), belong to middle-class families (72.8%), have a nuclear family setup (67.2%), and are housewives (86.8%). The mothers of these children tended to experience moderate depression (65.1%). The severity of an ASD child’s IQ level, adaptive behaviour, and communication characteristics were significantly correlated with depression in the mother (p<0.01). The mothers of children who were receiving treatment had significantly higher BDI scores than those mothers whose children were not under treatment (p<0.01). Hence, offering mental health support to mothers experiencing distress along with researching to explore the root causes of depression is found to be crucial. There is a need to strategise financial and moral support for couples with ASD children to help reduce depression among them.

Abstract:Abstract Autistic children experience high rates of mental health challenges, and links have been found between child mental health and the parent-child relationship. As parents of autistic children are often actively involved in their child’s treatment, it is important to consider aspects of the parent-child relationship within this context. The present study investigated changes in a component of the parent-child relationship, the coherence of parental representations, following participation in a 10-week cognitive behavioural therapy intervention designed to address autistic children’s mental health challenges. Relationships were examined between coherence and child characteristics (i.e., autism symptoms, mental health), and associations with child treatment outcomes (i.e., mental health). Participants included 81 children (89% boys) aged 8 to 13 years and their parents (85% mothers) aged 35 to 54 years. Baseline levels of coherence were related to children’s mental health symptoms but not autism symptoms. Although there were no significant changes in overall coherence across therapy, subscale-level improvements (i.e., concern, acceptance) emerged. Changes in coherence across therapy were linked with children’s post-intervention behavioural symptoms and were approaching significance for internalizing problems, but were not associated with externalizing problems. It is critical to investigate factors that shape the coherence of parents’ representations of their children, as this may provide insight into potential targets for intervention. Ascertaining whether participation in therapy improves parental coherence, and consequently child treatment outcomes, can advocate for parent-involved therapy, which will ultimately benefit the well-being of autistic children.

Abstract:Patients with thrombotic microangiopathies (TMAs), which include thrombotic thrombocytopenic purpura (TTP), atypical hemolytic uremic syndrome (aHUS), and Shiga toxin-producing Escherichia coli hemolytic uremic syndrome (STEC-HUS), exhibit multiple symptoms. These include thrombocytopenia (platelet count <15×104/µl), microangiopathic hemolytic anemia (hemoglobin <10 g/dl), and organ damage, such as renal dysfunction or neurological abnormalities. Guidelines for the management of TTP, aHUS, and STEC-HUS have been published. Diagnosis of TTP, STEC-HUS, or aHUS requires ADAMTS13 activity <10%, the detection of STEC or Shiga toxins, or a genetic analysis to investigate the complement regulatory system, respectively. In the established management of TMAs, fresh frozen plasma is used for congenital TTP to supplement ADAMTS13. Plasma exchange (PE) is used in acquired TTP (ATTP) to remove inhibitors and replace ADAMTS13. Steroid therapy is administered in ATTP to suppress inhibitor production, and rituximab is administered in ATTP without response to PE or with high titers of ADAMTS13 inhibitors. Eculizumab, a monoclonal antibody for complement protein 5, is used for the treatment of aHUS. Supportive therapy is required for some TMAs, particularly STEC-HUS, but platelet transfusion is generally contraindicated for other TMAs, such as TTP. Treatments should be administered in accordance with these guidelines to improve the outcome of TMAs.

Abstract:The "sense of coherence" incorporates the notion that when life seems comprehensive, manageable, and meaningful for an individual, even under tremendous adversity, this accounts for stamina and confidence. Its absence is associated with mental health problems, including depression. The current analysis aimed to explore the relationship between the sense of coherence and depression through a network analysis approach in a sample of 181 people with depression. The mean age of the individuals in the sample was 35.59 (11.50) years, and 76.8% were females ( n = 139). All participants had at least one previous depressive episode; the duration of the current episode was 30.30 (77.30) days. Sense of coherence was assessed using the 13-item Sense of Coherence Scale (SOC-13). The severity of depression was quantified with the Beck Depression Inventory (BDI-I). The correlations between the two scales, three networks (i.e., SOC-13; BDI-I and joint SOC-13/BDI-I), and their centrality indices were calculated. The mean SOC-13 was 40.56 ± 9.99, and the mean BDI-I was 49.90 ± 9.26. There was a strong correlation-concordance (− 0.65; 95% CI: − 0.72 to − 0.55) index between the two scales. We identified detailed relationship dynamics between symptoms, clusters, and domains through the novel network analysis approach. The analysis of SOC-13 nodes revealed the pivotal role of social relationships in the network. However, in the depression network, we found a role for affection (in contrast to neglect) and joy (as opposed to boredom). In conclusion, solid and sustainable personal relationships in distress and adversity stand against the burden of depression.

Abstract:The increased prevalence of autism spectrum disorder (ASD) has placed a significant emotional and psychological burden on mothers. We explored the association between the severity of ASD symptoms in children and the mental health of their mothers during the COVID-19 pandemic. Our study included 1,924 mothers of children with ASD, enrolled in a web-based cross-sectional survey over 85 consecutive days to gather clinical and sociodemographic data. The severity of ASD symptoms was obtained according to the children's age. Using the Depression, Anxiety, and Stress Scales (DASS-21) scale, we found that 35.8 percent of mothers experienced both anxiety and depression. A high education level and a high family income reduced the chance of concurrent anxiety and depression. Conversely, unemployment, a child using psychiatric medication, and higher severity of ASD symptoms increased the chance. Notably, the severity of the ASD symptom was the sole predictor of maternal co-occurring anxiety and depression across all age groups (<3 years aOR = 2.04, 95%CI 1.07–3.89; 3–5 years aOR = 2.76, 95%CI 1.67–4.56; ≥ 6 years aOR = 1.61, 95%CI 1.04–2.50). Recognizing the challenges associated with ASD leads to greater acceptance and tailored interventions, ultimately improving the overall well-being of both individuals with ASD and their mothers.

Abstract:This study examined the relationship between perceived social support, occupational balance and family functionality in rehabilitation parents of children with autism. The sample of this cross-sectional study, in which descriptive correlational data analysis was used, consists of 138 parents reached by the easily accessible sampling method. Data collection tools in the study were the Turkish version of Multidimensional Scale of Perceived Social Support, the Turkish version of Occupational Balance Questionnaire and the Turkish Family Functioning Questionnaire in Rehabilitation. Pearson Correlation Coefficient Analysis and Spearman Correlation Analysis were used. The relationships between the perceived social support with occupational balance and family functionality in the rehabilitation of parents were examined. The parents received social support from the significant other, the family member and the friend, respectively. They thought they had no occupational balance and their family functionality in rehabilitation was good. In addition, perceived social support had a relationship with occupational balance, as well as family functionality in rehabilitation. Future studies could examine these relationships in parents of individuals with autism in different age groups or children with different special needs. Furthermore, the examination of occupational balance and participation of parents is recommended.

Abstract:The quantitative study assessed subjective burden, depression, and the moderating effect of social support in mothers of children with autism spectrum disorder (ASD) in India. Seventy mothers were interviewed using a structured interview schedule, which measured their subjective burden, depression, and social support from family, friends, and significant others. Data was analyzed using descriptive statistics and multiple regression analysis with interaction terms. Half of the mothers in the study reported depression of clinical significance. Higher subjective burden significantly predicted higher depression. Of the three sources of support, only medium/high family support had a direct impact on depression and also moderated the impact of the subjective burden of depression. Implications for practice and policy are discussed.

Abstract:Caregiving for children with autism spectrum disorder (ASD) poses significant stress for parents, necessitating an exploration of mitigating factors. This study investigates the interplay between Family Sense of Coherence, Psychological Capital and caregiver stress in this context. A total of 205 caregivers of children with ASD participated in this cross‐sectional study. Data were collected on Family Sense of coherence, Psychological Capital (encompassing hope, resilience, optimism and self‐efficacy) and caregiver stress. Structural equation modelling was employed to test the mediation effect of psychological capital between family sense of coherence and caregiver stress. The results indicated a strong positive correlation between family sense of coherence and all subdomains of psychological capital, with coefficients ranging from 0.541 to 0.610. Conversely, psychological capital demonstrated significant negative correlations with various domains of the Kingstone Caregiver Stress Scale, including caregiving, family issues and financial issues (coefficients from −0.443 to −0.427). Abstract:Raising a child with an autism spectrum disorder (ASD) can be a stressful experience for parents. When left unmanaged, high stress levels can lead to the development of depressive symptomatology, highlighting the importance of coping supports. The current paper examined the stress level and psychological wellbeing of mothers with a child with ASD in a national survey. After adjusting for child, mother and family level characteristics, it was determined that mothers of children with ASDs were at greater risk for poor mental health and high stress levels compared to mothers of children without ASDs. The presence of maternal coping strategies, in the form of emotional and neighborhood social supports, as well as strong coping skills, reduced these risks between models.

Abstract:This study explores both behavioral and skill disparities between children diagnosed with Autism Spectrum Disorder (ASD) and their typically developing (TD) peers, assessing how these differences influence the quality of life (QoL) of mothers with children who have ASD. The sample of participants included the mothers of 46 children with ASD (27 boys and 19 girls) and the mothers of 45 TD children (26 boys and 19 girls). The average age of the children with ASD was 57.96 ± 12.42 months. The average age of the TD children was 53.64 ± 13.13 months. Comparative analyses were conducted using the Aberrant Behavior Checklist (ABC) scale, the Vineland Adaptive Behavior Scale 3, The PedsQL (Pediatric Quality of Life Inventory) Family Impact Module, and Sleep Disturbance Scale for Children (SDSC). Analyses of mothers' QoL revealed that behavioral challenges in children with ASD negatively affect the physical, emotional, social, cognitive, and communicative aspects of mothers' lives. Significant correlations and multiple regression analyses highlighted the relationships between children's behavioral characteristics and the QoL of their mothers. Notably, sleep difficulties in children with ASD, such as issues with initiation and maintenance, were found to adversely impact the physical and emotional health of mothers. The study demonstrates the extensive impact of ASD, not only on the individuals but also on their mothers, suggesting a need for a more inclusive approach to the management of ASD.

Abstract:Mothers of children with autism report higher levels of depression than mothers of children with other developmental disabilities. We explored the relations between child characteristics of diagnostic severity and problem behaviors, parenting stress, relationship quality, and depressive symptoms in 70 mothers of young children with autism. We hypothesized that relationship quality and parenting stress would relate to maternal depression beyond contributions of child characteristics. Multiple regression analysis revealed a main effect of parenting stress above and beyond child problem behaviors and autism severity. A significant interaction emerged, with relationship quality buffering the effect of parenting stress on depression. Results suggest that the relation between child problem behaviors and maternal depression should be considered in conjunction with other measures of marriage and family stress. Relationship quality and parenting stress may also represent important factors to be explicitly considered within intervention paradigms for young children with autism spectrum disorders.

Abstract:PURPOSE: This study aims to determine the depression, stress and anxiety levels of mothers with preschool children during the pandemic and to determine their coping strategies with stress.DESIGN AND METHODS: This descriptive study sampled 300 mothers with kindergarten children in the 2021-2022 academic year. The data were collected with a personal information form, The Depression, Anxiety and Stress Scale-21 (DASS-21) and Stress-Coping Styles Scale (SCSS). Numerical variables were presented as mean and standard deviation, and categorical variables were presented as numbers and percentages. The Mann Whitney U test was used for the dual group and the Kruskal Wallis test for more than two groups in the comparison of the scale mean scores. The relationship between the two scales was determined by Pearson's correlation analysis. p < 0.05 value was accepted statistically significant.RESULTS: The depression, anxiety and stress mean scores of the mothers in the study were high (10.01 ± 4.79; 9.07 ± 5.09; 10.28 ± 4.28, respectively). There is a high level of negative correlation between the SCSS self-confidence approach scores, optimistic approach scores and depression, anxiety and stress scores, and a high level of positive correlation between the helpless/self-accusatory approach, submissive approach, optimistic approach and depression, anxiety and stress scores.CONCLUSION: Mothers with pre-school children have been found to experience intense depression, anxiety and stress during the pandemic, and as their depression, stress and anxiety levels increase, they move away from self-confident and optimistic approaches, and they embrace more the helpless/self-accusatory approach and submissive approach, seeking social support.IMPLICATIONS TO PRACTICE: In the event of a future pandemic, psychiatry and pediatric nurses should strengthen positive coping styles and make mothers aware of social support resources to ensure that mothers, who are the most vulnerable group in terms of mental health, and thus children, do not experience psychiatric problems. Thus, nurses, as mandated by the holistic care approach, provide the qualified care that the individual and his family deserve.

Abstract:This study aimed to investigate the different effects of fatigue and health-related quality of life in the Turkish mothers of children with autism spectrum disorder (ASD) through comparisons with mothers of typically developing children, and to evaluate the relationship between different aspects of maternal fatigue, depression, and maternal health-related quality of life. The study included a total of 103 mothers, comprising 60 mothers of children with ASD and 43 mothers of typically developing children. The Nottingham Health Profile and Quality of Life in Autism Questionnaire-Parent Version were used to assess the health-related quality of life. Fatigue was assessed comprehensively with the Fatigue Impact Scale and the Fatigue Severity Scale. The Nottingham Health Profile total and physical mobility domain and the quality of life were the strongest factors, explaining with a variance of 66.7% the fatigue impact on cognitive function. The mothers' quality of life accounted for 64.8% of the variance in factors that explained fatigue. Physical mobility, social isolation and quality of life were associated with the physical impact of fatigue and were explained with a variance of 52.4%. Emotional reactions and quality of life were explained with a variance of 52.7% in the fatigue of psychosocial function. The current study has highlighted that the fatigue of Turkish mothers of children with ASD has a greater impact on cognitive, physical and psychosocial functions. Furthermore, the quality of life, physical mobility, social isolation, and emotional reactions of mothers have a significant impact on maternal fatigue.

Abstract:For most parents, the birth of their child is a reason for joy because the child is the center of their Universe. He is light, hope, smile, change, and future. Raising a child with a disability can have a profound impact on the family. This causes great stress for parents, especially for the mother. Prolonged stress negatively influences their quality of life, causing health problems and having a high level of fatigue, anxiety, and depression. This research aimed to investigate the level of parental stress among mothers of children with autism spectrum disorder respectively, among mothers of children with Down syndrome. The results of this study demonstrated significant differences regarding the measured variables among mothers of children with ASD and Down syndrome. Regarding coping mechanisms, mothers of children with ASD use religious coping more frequently compared to mothers of children with Down syndrome, who use the following coping mechanisms: active coping, planning, seeking instrumental support, seeking emotional support, acceptance, mental disengagement, and venting. Additionally, the level of parental stress is higher among mothers of children with ASD, and their quality of life is lower; the tendency of social isolation is at a fairly high level.

The parenting stress questionnaire, coping orientation to problems experienced scale, strengths and difficulties questionnaire, and Duke-UNC social support questionnaire were administered to 52 mothers, who also estimated the ASD severity symptoms of their children. Correlation analyses revealed that parenting stress was positively correlated with the children's ASD symptoms and behavioral problems. On the other hand, parenting stress was negatively correlated with the engagement coping and social functional support reported by the mothers. Multiple mediation analysis indicated that engagement coping and behavioral difficulties were significant mediators in the relationship between ASD symptoms and parenting stress, with the engagement variable having a larger effect. The findings illustrate the need to promote the mothers' engagement coping orientation and the application of behavioral strategies with their children to help them to buffer the impact of stress.

Raising an autistic child: perspectives from Turkish mothers

Abstract:Problem: Autism is an illness with severe deficits in reciprocal social interactions, imagination, communication, and restricted or unusual behavioral repertories that affect all areas of a child's life such as daily living activities, home/school life, and relationships with family members and others. Yet, there is much to be learned about the impact of this disorder on parents' experiences, and ways in which their lives are altered and ways that they can be assisted to better manage the home environment. Methods: The purpose of this study was to explore and categorize the experiences of mothers having an autistic child using a phenomenological design in the qualitative tradition. Semistructured interviews were conducted with 43 mothers regarding their experiences with their autistic children. Data were analyzed with deductive content analysis based on coding, and were grouped into categories based on common themes. The distributions of common responses in categories were presented as percentages. Findings: Based on this qualitative study, mothers expressed feelings of burden and stress because of their child's behaviors associated with autism, as well as their own role and future expectations, and the complexity of care needed by their children at home. Conclusions: This study provided new knowledge about the difficulties and experiences of mothers of autistic children in Turkey. The results underscored the needs of mothers who require support from family members, healthcare organizations, and society. The findings support the need for training programs for mothers and parents conducted by nurses and other healthcare professionals who have special knowledge and skill to provide education and modeling of therapeutic interventions.

Abstract:The aim of this study is to determine the functioning of adults with autism spectrum disorders (ASDs) diagnosed in childhood and depression and burnout levels among their parents. A total of 261 adults with ASDs and their parents were recruited for the study. Both parents completed the Beck Depression and Maslach Burnout Inventories and reported the functioning of their adult offspring with ASDs. Only 5.4 % of our sample reported "good" or "very good" outcomes. The most common psychiatric comorbidities were intellectual disabilities and attention-deficit/hyperactivity disorder. Maternal burnout and depression scores were significantly elevated compared to those of fathers. There is an undeniable urgent need for more research to identify the needs of adults and families suffering from ASD. Modifications for those with ASD may have to be made for support in workplaces, achieving driving licenses, using public transportation and attendance at tertiary education.
