Abstract:For parents, experiencing disasters such as earthquakes with a child with autism spectrum disorder (ASD) involves various difficulties and increases vulnerability. But despite its importance for both children with ASD and their parents, the literature does not provide any explanation for the parenting experiences of parents with children with ASD in an earthquake. The aim of this study is to explore the parenting experiences of parents who experienced the 2023 Kahramanmaraş Earthquakes, the second most fatal earthquake in the world, with their children with ASD. In this study, which was conducted with a qualitative phenomenology design, semi-structured interviews were conducted with 18 parents (16 mothers, 2 fathers) living in eleven earthquake-affected cities and analyzed by qualitative content analysis. The findings of the study revealed the themes of (a) the effects of the earthquake on life, (b) the issues that are difficult to cope with, (c) the effects of the earthquake on parenting a child with ASD, and (d) the requirements on the earthquake. As a result, it is seen that parents experienced more difficulties in the earthquake due to the autism of their children. These difficulties wear out the parents, and ultimately, these difficulties reflect negatively on the parent-child relationship. The findings are discussed in detail in the text.

Abstract:This research aims to investigate the potential emotional stress and behavioural issues that may arise in children in the early aftermath of the Kahramanmaraş earthquake in Turkey. This cross-sectional study was conducted with 447 parents of children aged 4–10 living in 11 provinces in Turkey affected by the Kahramanmaraş earthquake. The data were collected using the "Demographic Information Form," the "Preschool Age Post-Traumatic Emotional Stress Scale," and the "Strengths and Difficulties Questionnaire-Parent Version" between March and July 2023. Multiple regression analysis was used to determine predictors of emotional stress and strengths and difficulties based on children's earthquake-related experiences. Among the children, 51.5% were male, and the mean age was 6.52 ± 1.58. The results indicated a positive correlation between emotional stress and behavioral problems ( r = 0.461, p < 0.001). Losing a family member, being injured in the earthquake, having injured family members, hospitalisation, family members hospitalisation, and home damage were identified as significant determinants of emotional stress and behavioural problems. This research provides valuable insights for tailoring interventions to children directly exposed to earthquakes. The full extent of the psychological damage caused by the earthquake that resulted in significant damage in Turkey has yet to be fully revealed. Similar studies conducted in the future will contribute to our understanding of the psychological consequences of the 2023 earthquake and aid in planning and implementing appropriate interventions for children.

Abstract:The individual, environmental and systemic physical activity (PA) barriers experienced by individuals with autism spectrum disorder (ASD) in daily life may become more complex after a devastating disaster such as an earthquake. In order to effectively combat the high level of inactivity that may occur after an earthquake in individuals with ASD, PA barriers must be determined before the earthquake. However, no description is provided in the literature regarding the PA barriers experienced by individuals with ASD after the earthquake. The purpose of this phenomenological study is to examine parents' experiences regarding PA barriers experienced by individuals with ASD after the 2023 Kahramanmaraş earthquakes in Türkiye. Fifteen parents of individuals aged 8 and 31 with ASD were interviewed. The interviews were analyzed using both inductive and deductive content analysis approach. Three overarching themes were found: (1) the effects of PA on individuals with ASD, (2) post-earthquake PA barriers, and (3) the consequences of lack of PA in individuals with ASD. The results revealed that parents were aware of the benefits of PA and directed their children with ASD to PA before the earthquake. However, after the earthquake, parents encountered PA barriers that they had to overcome, and this negatively affected the children with ASD.

Abstract:Individuals with intellectual disabilities and their parents, who encounter many barriers in daily life, may experience even more serious problems when faced with a devastating disaster such as an earthquake. For effective earthquake preparation, it is important to identify these problems before the earthquake. However, studies examining the earthquake experiences of individuals with intellectual disabilities and their parents are quite limited. In this context, the aim of the current study was to explore the experiences of parents of individuals with intellectual disabilities who were affected by the 2023 Kahramanmaraş earthquakes in Türkiye. This study used the phenomenological qualitative research design. In this study, there were 16 parents, who have children with intellectual disabilities and participated in face-to-face interviews. Interview data were analyzed thematically. Two overarching themes were found: (1) low level of earthquake preparedness, and (2) inaccessible needs. The findings revealed that the majority of parents of individuals with intellectual disabilities made few preparations for the earthquake, could not access many of their needs during and after the earthquake, and could not find a special unit for individuals with disabilities to explain their problems.

Abstract:The virus-specific components (nsP1–nsP4) of Semliki Forest virus RNA polymerase are synthesized as a large polyprotein (P1234), which is cleaved by a virus-encoded protease. Based on mutagenesis studies, nsP2 has been implicated as the protease moiety of P1234. Here, we show that purified nsP2 (799 amino acids) and its C-terminal domain Pro39 (amino acids 459–799) specifically process P1234 and its cleavage intermediates. Analysis of cleavage products ofin vitro synthesized P12, P23, and P34 revealed cleavages at sites 1/2, 2/3, and 3/4. The cleavage regions of P1/2, P2/3, and P3/4 were expressed as thioredoxin fusion proteins (Trx12, Trx23, and Trx34), containing ∼20 amino acids on each side of the cleavage sites. After exposure of these purified fusion proteins to nsP2 or Pro39, the reaction products were analyzed by SDS-polyacrylamide gel electrophoresis, mass spectrometry, and amino-terminal sequencing. The expected amino termini of nsP2, nsP3, and nsP4 were detected. The cleavage at 3/4 site was most efficient, whereas cleavage at 1/2 site required 5000-fold more of Pro39, and 2/3 site was almost resistant to cleavage. The activity of Pro39 was inhibited byN-ethylmaleimide, Zn2+, and Cu2+, but not by EDTA, phenylmethylsulfonyl fluoride, or pepstatin, in accordance with the thiol proteinase nature of nsP2.

Abstract:Given the profound impact of natural disasters on mental health, particularly in vulnerable groups such as adolescent and young adult, understanding their psychological results is crucial for providing necessary support and designing effective interventions. The aim of this study was to investigate the relationships between earthquake exposure, posttraumatic stress symptoms, coping mechanisms, and social support in adolescents and young adults who survived the 2023 earthquake in Turkey. The research was conducted in Kırıkhan district of Hatay province, which was severely damaged after the earthquake that occurred in Turkey in 2023. The sample for the descriptive and cross-sectional study consisted of 204 participants. Convenience sampling method was used for data collection. Data were collected using the Personal Information Form (PIF), Impact of Events Scale (IES), Stress Coping Strategies Inventory (CSSI), and Multidimensional Perceived Social Support Scale (MPSSS). Hierarchical multiple regression analysis was used to determine the factors associated with IES-R scores. Three models were tested to understand the factors predicting levels of posttraumatic stress (IES-R scores). The findings suggest that exposure to earthquake, problem-focused coping and seeking support play an important role in understanding posttraumatic stress levels. This study provides valuable insights into the psychological impact of the earthquake on adolescents and young adults, as well as the development of effective intervention strategies and support programs for young earthquake survivors.

Abstract:Adolescents exposed to natural disasters are among the most vulnerable groups to the devastating effects of these events, including experiencing mental health issues such as Post-Traumatic Stress Disorder (PTSD), depression, and anxiety disorders. The study aims to evaluate the impact of an early-term psychoeducational intervention (2 months after the disaster) on adolescents who were exposed to the February 6th earthquakes in Turkey regarding symptoms of post-traumatic stress disorder (PTSD), depression, anxiety, and coping styles. The study employed AB experimental design with a single-group, pre-test, post-test, and follow-up test evaluation. The sample consisted of 8 individuals aged between 12 and 18, relocated to another city (Istanbul) with their families to a container camp after exposure to the earthquake. Participants underwent an 8-session (one session per week) psychoeducational program developed by researchers composed of psychologists 2 months after the earthquake. To assess the effectiveness of the psychoeducational intervention, the Child and Adolescent Post-Traumatic Stress Disorder Reaction Index, Brief Symptom Inventory, and Coping Styles Scale-Brief Form was employed at three different time points (pre-intervention, post-intervention, and 4-month follow-up). Percentage distribution and the Friedman Test for comparisons were done for the data to be analyzed. Following the psychoeducational intervention, a statistically non-significant decrease in depression symptoms was observed, but the effect of the intervention was maintained over the 4-month follow-up period. While no change in anxiety symptoms was noted after the intervention, a significant decrease was observed in the follow-up. However, no improvement was observed in PTSD symptoms. Participants showed a statistically significant increase in religious coping, whereas a non-significant increase in planning, using emotional social support, and positive reinterpretation coping styles were observed. After large-scale disasters affecting numerous individuals, early psychological interventions should be planned and tailored for specific groups and specific needs instead of including every member of the group would be more time and cost-efficient.

Psychological Impact of the 2023 Kahramanmaraş Earthquakes on Non-Victims

Abstract:The 2023 Kahramanmaraş earthquakes, with magnitudes of 7.7 and 7.6, caused extensive destruction and psychological distress across southeastern Turkey. This study explores the psychological impact on non-victims, particularly Istanbul residents, focusing on mental health outcomes and coping mechanisms. A cross-sectional survey was conducted from March to May 2024 with 721 participants from various Turkish cities, including a significant portion from Istanbul. Validated psychological scales such as the Beck Depression Inventory-II (BDI-II), Beck Anxiety Inventory (BAI), Warwick-Edinburgh Mental Well-being Scale (WEMWBS), and PTSD Checklist for DSM-5 (PCL-5) measured depression, anxiety, well-being, and PTSD symptoms. Sociodemographic factors like age, gender, occupation, income, education level, and previous earthquake experience were also analyzed. Results showed significant psychological distress among non-victims: 51.9% reported high levels of distress, with 24% meeting PTSD criteria, 30% exhibiting moderate to severe depression, and 28% experiencing significant anxiety. Higher income and education levels correlated with better mental health outcomes. Higher education levels were linked to lower PTSD risk (β = -0.20, p < 0.01) and fewer depression symptoms (β = -0.15, p < 0.05). Higher income was associated with lower depression scores (β = -0.20, p < 0.01) and fewer PTSD symptoms (β = -0.15, p < 0.05). Age positively correlated with well-being (r = 0.68, p < 0.001) and negatively with PTSD symptoms (r = -0.15, p < 0.05). Comparisons with victim studies of major earthquakes, such as the 1995 Great Hanshin-Awaji earthquake, the 1999 Marmara earthquake, the 2008 Wenchuan earthquake, and the 2000 Iceland earthquakes, revealed similar profound psychological impacts. This highlights the need for comprehensive mental health interventions for both direct and indirect exposures. This study underscores the necessity for inclusive mental health strategies to enhance resilience and well-being, ensuring robust recovery after catastrophic events.

WHY EXPERIMENTERS SHOULD NOT RANDOMIZE, AND WHAT THEY SHOULD DO INSTEAD

Abstract:This paper discusses experimental design for the case that (i) we are given a distribution of covariates from a pre-selected random sample, and (ii) we are interested in the average treatment effect (ATE) of some binary treatment. We show that in general there is a unique optimal non-random treatment assignment if there are continuous covariates. We argue that experimenters should choose this assignment. The optimal assignment minimizes the risk (e.g., expected squared error) of treatment effects estimators. We provide explicit expressions for the risk, and discuss algorithms which minimize it. The objective of controlled trials is to have treatment groups which are similar a priori (balanced), so we can ``compare apples with apples.'' The expressions for risk derived in this paper provide an operationalization of the notion of balance. The intuition for our non-randomization result is similar to the reasons for not using randomized estimators - adding noise can never decrease risk. The formal setup we consider is decision-theoretic and nonparametric. In simulations and an application to project STAR we find that optimal designs have mean squared errors of up to 20% less than randomized designs and up to 14% less than stratified designs..

Abstract:On February 6, 2023, an earthquake registering 7.8 on the Richter scale struck southeast Turkey, and this was followed by a second earthquake of magnitude 7.5. The earthquake resulted in widespread loss, devastation, and destruction, and is anticipated to bear a significant psychological toll on survivors. This study aimed to investigate the prevalence and correlates of ICD-11 posttraumatic stress disorder (PTSD) in those living in earthquake-affected areas of Turkey. The sample was a convenience sample of 527 adults living in earthquake-affected areas of Turkey. Prevalence rates and gender differences in exposure to earthquake-related traumatic events were investigated. Moreover, prevalence rates of ICD-11 PTSD were computed, and demographic and trauma-related correlates of ICD-11 PTSD were assessed using logistic regression analyses. Most of the sample (99.2%; n = 523) experienced at least one earthquake-related traumatic event, with the majority experiencing multiple events. Findings demonstrated high endorsement of the ICD-11 PTSD symptom clusters, with 54.1% meeting criteria for diagnosis of probable ICD-11 PTSD. Correlates associated with increased risk of PTSD include female gender, physical injury in the earthquake, physical injury of family or loved ones in the earthquake and being in the higher earthquake-related traumatic events exposure quartiles. These findings indicate high levels of trauma exposure and PTSD among survivors of the recent earthquakes which occurred in Turkey. The identification of correlates associated with increased risk of PTSD is useful for identifying Turkish earthquake survivors who are most likely to need psychological support.

Abstract:Current US statistics indicate that 1 in 68 children is diagnosed with an autistic spectrum disorder (Centers for Disease Control (2014) Prevalence of autism spectrum disorder among children aged 8 years—autism and developmental disabilities monitoring network, 11 Sites, United States, 2010. Morbidity and Mortality Weekly Report (MMWR)). The lived experience of parents with children diagnosed with autism spectrum disorder is important to know since quantitative studies have indicated that higher rates of mental disorders exist in this population as compared to parents of typically developing children (Yirmiya and Shaked (2005) Psychiatric disorders in parents of children with autism: a meta-analysis. Journal of Child Psychology and Psychiatry 46: 69–83). This study was a meta-synthesis of the qualitative literature in this area embedded within a systematic review. A comprehensive search and review yielded 14 studies. A total of six major themes were identified: (a) emotional stress and strain; (b) adaptation; (c) impact on the family; (d) services; (e) stigmatization; and (f) appreciating the little things. Implications of these results are discussed.

Abstract:This study investigated the prevalence and risk factors of probable posttraumatic stress disorder (PTSD), depression, and complicated grief among 529 adults residing in a container city, 4 months after the 2023 earthquakes in Türkiye. Participants were assessed using the PTSD Checklist for Diagnostic and Statistical Manual of Mental Disorders, 5th Edition , the Hamilton Depression Scale, and the Inventory of Complicated Grief. The prevalence rates of probable PTSD, depression, and complicated grief were 21.4%, 15.9%, and 64.7%, respectively. Logistic regression analyses indicated that PTSD was significantly associated with the loss of a loved one, fear of future earthquakes, and decreased perceived life control. Similarly, depression was associated with being trapped under debris, experiencing fear during the earthquake, and reduced perceived life control. These findings suggest that individuals presenting these risk factors warrant closer follow-up for potential psychological issues. Additionally, the impact of fear related to the earthquake and the role of perceived life control emerge as crucial considerations for psychological interventions.

Teacher-mediated intervention after disaster: a controlled three-year follow-up of children's functioning

Abstract:Background: Child survivors of a catastrophic earthquake in Turkey were evaluated three and a half years after the event, and three years after a sub-group participated in a teacher-mediated intervention developed by the authors. The goal of this follow-up study was to determine the long-term effectiveness of the original intervention. Methods: Subjects who participated in the intervention were compared with a control group of children similar in terms of demographics, risk and exposure. All children were evaluated in terms of posttraumatic, grief and dissociative symptomatology, as well as adaptive functioning (academic performance, social behavior and general conduct). Results: The severity of post-traumatic, grief and dissociative symptoms of the two groups was comparable. Teachers blind to group assignment rated participating children significantly higher than the control group in terms of adaptive functioning. Conclusions: Early post-disaster intervention addressing children and their educational milieu provides children with significant symptomatic reduction, allowing the mobilization of adaptive coping, thereby enhancing their overall functioning as observed in school.

Abstract:Background On 6 February, 2023, Turkey experienced a disaster resulting from two major earthquakes. The elderly population stands out as one of the most vulnerable and least adaptable. This study aims to identify the quality of life (QOL) and post‐traumatic stress disorder (PTSD) symptoms among elderly earthquake survivors in Turkey. Methods This research was conducted between 1 October, 2023 and 15 December, 2023 as a cross‐sectional design. During the data collection process, survey questions prepared by the researchers (29 questions) and the Elderly Quality of Life Scale (OPQOL‐Bref) and the Impact of Events Scale (IES‐R) were used. Results Three hundred and eighty‐four elderly people participated in the study. PTSD was more common in those diagnosed with chronic diseases after the earthquake, in those who had a drug‐free period after the earthquake, in those who smoked and quit smoking, and in those who started psychiatric medication after the earthquake. Post‐traumatic stress was seen more frequently in those with low QOL scores and in those who had a longer period of not using medication after the earthquake. It was observed that QOL was better in married people, high school graduates, those with average income levels, those who did not have chronic diseases before and after the earthquake, those who did not have a drug‐free period after the earthquake, those who were physically active, and those who did not use psychiatric medication after the earthquake. Conclusion Chronic disease diagnosis post‐earthquake, drug‐free periods, smoking habits, and psychiatric medication usage were identified as influential factors in PTSD occurrence. Additionally, the study shed light on the profound effect of earthquake‐related experiences, such as house damage, injuries, deaths in first‐degree relatives, displacement, and changes in living conditions, on PTSD prevalence.

Abstract:Objective This cross-sectional study investigates the immediate psychological effects of disaster relief operations on team members following 2 consecutive major earthquakes in Turkey. Methods A total of 170 participants, including professional firefighters, search and rescue (SAR) workers, and volunteers, were surveyed approximately 1 month after the conclusion of active SAR operations. The study utilizes the DSM-V criteria and the Posttraumatic Stress Disorder Checklist (PCL-5) to assess symptoms of post-traumatic stress disorder (PTSD) among participants. Results The findings reveal a point prevalence of 35.3% for probable PTSD, highlighting the substantial psychological impact on disaster relief teams. Factors such as age, residency in affected areas, and active SAR involvement significantly influenced probable PTSD rates. Interestingly, actively engaged SAR members had lower probable PTSD rates, possibly due to their training. Those who directly witnessed the earthquakes had higher scores, highlighting the impact of firsthand exposure. Additionally, individuals aged 50 and above displayed a higher mean total severity score compared to younger participants. Conclusions This research contributes to understanding the mental well-being of disaster relief professionals. The study's findings underscore the importance of timely mental health support and training for these responders, emphasizing the need for preparedness in disaster relief teams.

Abstract:This study investigated the relationship between parental psychological wellbeing, parenting, children's psychological difficulties, and prosocial behavior in Kahramanmaraş earthquake‐affected families living in Türkiye in 2023. To this end, a mediation model was proposed for parental psychological distress that was hypothesized to exert an indirect effect on a child's psychological difficulties and prosocial behavior through parenting. Participants were 358 preschoolers between 4 and 6 years old and their one parent (father or mother) who completed a set of validated self‐report surveys in a cross‐sectional design study. Results showed positive associations between parental psychological distress and child's psychological difficulties but negative association with child's prosocial behavior. Furthermore, quality of parenting mediated the association between parental psychological distress and child's psychological difficulties and prosocial behavior. Our findings suggest that positive parenting may serve as a protective mechanism that mediates the association between parental psychological distress and a child's psychological difficulties and prosocial behaviors among families displaced by a natural disaster like an earthquake. These findings point to the need for supporting positive parent–child relationships in addition to decreasing the psychological distress of parents when exposed to potentially traumatizing events like this.
