Abstract:BACKGROUND AND AIM: Few studies have demonstrated the potential effects of metal mixtures on metabolic dysfunction-associated fatty liver disease (MAFLD). Here, we examined the associations between whole blood metal mixtures and prevalence of MAFLD among an aged population in China. METHODS: a cross-sectional study of community-dwelling elderly adults were conducted in China. Whole blood concentrations of fourteen metals were measured by inductively coupled plasma mass spectrometry (ICP-MS). MAFLD was ascertained based on ultrasonographic evidence of hepatic steatosis and the presence of overweight/obesity, type 2 diabetes mellitus, or metabolic dysregulation. We performed logistic regression and restricted cubic spline (RCS) models to independently estimate the associations between metal concentrations and MAFLD prevalence. Environmental Risk Score (ERS) constructed by adaptive elastic net (AENET) and Bayesian kernel machine regression (BKMR) analysis were utilized to examine joint effects of metal mixtures and metal interactions on MAFLD. RESULTS: In the models adjusting for multiple covariates, lower blood copper and higher selenium, iron and mercury were significantly associated with the elevated prevalence of MAFLD. Non-linear effects of copper, iron, magnesium, manganese and selenium on MAFLD were observed in RCS models. Similar metals (copper, selenium, zinc, iron, cadmium and mercury) were selected as crucial predictors for MAFLD prevalence by AENET and BKMR models. The association between ERS and MAFLD was estimated and the ERS was significantly associated with the elevated OR for MAFLD. CONCLUSIONS: Our findings provide evidence that exposure to blood copper, iron, selenium and mercury are associated with the prevalence of MAFLD. Multiple metals exposure may adversely affect MAFLD, with copper, selenium, zinc, iron, cadmium and mercury being the major contributors. Further researches are needed to clarify the cumulative effects of metal mixture in a larger population. Abstract:BACKGROUND AND AIM: Metal exposures are suspected to associate with the risk of chronic kidney disease (CKD). Copper (Cu) is an essential yet toxic trace element in humans. Epidemiological evidence on the association of Cu exposure with CKD remains inconsistent. We aim to investigate the association of blood Cu and its co-exposure to other metals with CKD. METHODS: A cross-sectional study was conducted among 3,286 Chinese elderly adults in Zhejiang, China. The whole blood levels of Cu as well as other 10 metals were measured by inductively coupled plasma mass spectrometry (ICP-MS). CKD events were defined as the presence of albuminuria or <60 mL/min/1.73m2 estimated glomerular filtration rate (eGFR). Multivariable logistic regression and linear regression models were applied to assess the single-metal association of Cu with kidney function. The Bayesian kernel machine regression (BKMR) was used to estimate associations of the metal mixture with CKD. RESULTS: In the logistic models, we observed a positive dose-dependent association of blood Cu with CKD prevalence after adjusting for multiple covariates. Positive linear dose-response associations with blood Cu were also found for CKD and negative for eGFR. The positive association between blood Cu and CKD risk also observed in BKMR model and stronger at lower level of manganese (Mn). CONCLUSIONS: The whole blood level of Cu was remarkably associated with the prevalence of CKD and showed positive dose-response relationships in the elderly Chinese population. The toxicity of Cu on kidney function could be antagonized by the exposure of Mn. These findings have provided new clues of the potential roles of Cu in kidney health, and the needs to develop not only recommended intake but also safe limits for Cu. Sex-specific associations of plasma metals and metal mixtures with glucose metabolism: An occupational population-based study in China

Abstract:<p>Studies with multi-pollutant approach on the relationships between multiple metals and fasting plasma glucose (FPG) are limited. Few studies are available on the potential sex-specific associations between metal exposures and glucose metabolism. We explored the associations between 22 plasma metals and FPG level among the 769 participants from the manganese-exposed workers healthy cohort in China. We applied a sparse partial least squares (sPLS) regression followed by ordinary least-squares regression to evaluate multi-pollutant association. Bayesian kernel machine regression (BKMR) model was used to deal with metal mixtures and evaluate their joint effects on FPG level. In the sPLS model, negative associations on FPG levels were observed for plasma iron (belta = −0.066), cobalt (belta = −0.075), barium (belta = −0.109), and positive associations for strontium (belta = 0.082), and selenium (belta = 0.057) in men, which overlapped with the results among the overall participants. Among women, plasma copper (belta = 0.112) and antimony (belta = 0.137) were positively associated with elevated FPG level. Plasma magnesium was negatively associated with FPG level in both sexes (belta = −0.071 in men and belta = −0.144 in women). The results of overlapped for plasma magnesium was selected as the significant contributor to decreasing FPG level in the multi-pollutant, single-metal, and multi-metal models. BKMR model showed a significantly negative over-all effect of six metal mixtures (magnesium, iron, cobalt, selenium, strontium and barium) on FPG level among the overall participants from all the metals fixed at 50th percentile. In summary, our findings underline the probable role of metals in glucose homeostasis with potential sex-dependent heterogeneities, and suggest more researches are needed to explore the sex-specific associations of metal exposures with risk of diabetes.</p>

Abstract:Abstract Embryonic stem cells have the ability to differentiate into nearly all cell types. However, the molecular mechanism of its pluripotency is still unclear. Oct3/4, Sox2 and Nanog are important factors of pluripotency. Oct3/4 (hereafter referred to as Oct4), in particular, has been an irreplaceable factor in the induction of pluripotency in adult cells. Proteins interacting with Oct4 and Nanog have been identified via affinity purification and mass spectrometry. These data, together with iterative purifications of interacting proteins allowed a protein interaction network to be constructed. The network currently includes 77 transcription factors, all of which are interconnected in one network. In-depth studies of some of these transcription factors show that they all recruit the NuRD complex. Hence, transcription factor clustering and chromosomal remodeling are key mechanism used by embryonic stem cells. Studies using RNA interference suggest that more pluripotency genes are yet to be discovered via protein-protein interactions. More work is required to complete and curate the embryonic stem cell protein interaction network. Analysis of a saturated protein interaction network by system biology tools can greatly aid in the understanding of the embryonic stem cell pluripotency network.

Abstract:The long-term associations between multiple metals and incident diabetes are uncertain. We aimed to examine the relationship between plasma concentrations of 23 metals and the incidence of type 2 diabetes among Chinese senior adults. We quantified fasting plasma concentrations of 23 metals by inductively coupled plasma mass spectrometry among 1039 incident diabetes cases and 1039 controls (age and sex matched) nested in a prospective study, the Dongfeng-Tongji cohort. Both cases and controls were free of diabetes at baseline (2008–2010), incident diabetes were identified using the following criteria: fasting glucose ≥ 7.0 mmoL/l; or hemoglobin A1c (HbA1c) ≥ 6.5%; or self-reported physician diagnosis of diabetes or use of anti-diabetic medication during the follow-up visits in 2013. In the conditional logistic regression models, the multivariable adjusted ORs (95% CIs) of diabetes across quartiles (Q1–Q4) of metal concentrations were as follows: titanium, 1.00, 0.92, 1.31, 1.38 (1.00–1.91, Ptrend = 0.011); selenium, 1.00, 1.08, 1.45, 1.27 (0.93–1.74, Ptrend = 0.05); and antimony, 1.00, 0.79, 0.77, 0.60 (0.44–0.83, Ptrend = 0.002). Arsenic was significantly associated with diabetes in the crude model (ORs comparing extreme quartiles 1.30; 1.02–1.65; Ptrend = 0.006), but was not significant after adjustment for socio-demographic factors. No significant associations were found for other metals. In conclusion, titanium and selenium were positively while antimony was negatively associated with incident diabetes.

Abstract:BACKGROUND: Epidemiological studies about the effect of essential metal mixture on fasting plasma glucose (FPG) levels among elderly people are sparse. The object of this study was to examine the associations of single essential metals and essential metal mixture with FPG levels in Chinese community-dwelling elderly people.METHODS: The study recruited 2348 community-dwelling elderly people in total. Inductively coupled plasma-mass spectrometry was adopted to detect the levels of vanadium (V), selenium (Se), magnesium (Mg), cobalt (Co), calcium (Ca), and molybdenum (Mo) in urine. The relationships between single essential metals and essential metal mixture and FPG levels were evaluated by linear regression and Bayesian kernel machine regression (BKMR) models, respectively.RESULTS: In multiple-metal linear regression models, urine V and Mg were negatively related to the FPG levels (β = - 0.016, 95 % CI: - 0.030 to - 0.003 for V; β = - 0.021, 95 % CI: - 0.033 to - 0.009 for Mg), and urine Se was positively related to the FPG levels (β = 0.024, 95 % CI: 0.014-0.034). In BKMR model, the significant relationships of Se and Mg with the FPG levels were also found. The essential metal mixture was negatively associated with FPG levels in a dose-response pattern, and Mg had the maximum posterior inclusion probability (PIP) value (PIP = 1.0000), followed by Se (PIP = 0.9968). Besides, Co showed a significant association with decreased FPG levels in older adults without hyperlipemia and in women.CONCLUSIONS: Both Mg and Se were associated with FPG levels, individually and as a mixture. The essential metal mixture displayed a linear dose-response relationship with reduced FPG levels, with Mg having the largest contribution to FPG levels, followed by Se. Further prospective investigations are necessary to validate these exploratory findings.

Abstract:China has undergone rapid socioeconomic transition accompanied by lifestyle changes that are expected to have a profound impact on the health of its population. However, there is limited evidence from large nationwide studies about the relevance of socioeconomic status (SES) to risk of diabetes. We describe the associations of two key measures of SES with prevalent and incident diabetes in Chinese men and women. The China Kadoorie Biobank study included 0.5 million adults aged 30–79 years recruited from ten diverse areas in China during 2004–2008. SES was assessed using the highest educational level attained and annual household income. Prevalent diabetes was identified from self-report and plasma glucose measurements. Incident diabetes was identified from linkage to disease and death registries and national health insurance claim databases. We estimated adjusted ORs and HRs for prevalent and incident diabetes associated with SES using logistic and Cox regression models, respectively. At baseline, 30,066 (5.9%) participants had previously diagnosed (3.1%) or screen-detected (2.8%) diabetes among 510,219 participants included for cross-sectional analyses. There were 480,153 people without prevalent diabetes at baseline, of whom 9544 (2.0%) had new-onset diabetes during follow-up (median 7 years). Adjusted ORs (95% CIs) for prevalent diabetes, comparing highest vs lowest educational level, were 1.21 (1.09, 1.35) in men and 0.69 (0.63, 0.76) in women; for incident diabetes, the corresponding HRs were 1.27 (1.07, 1.51) and 0.80 (0.67, 0.95), respectively. For household income, the adjusted ORs for prevalent diabetes, comparing highest vs lowest categories, were 1.45 (1.34, 1.56) in men and 1.26 (1.19, 1.34) in women; for incident diabetes, the HRs were 1.36 (1.19, 1.55) and 1.06 (0.95, 1.17), respectively. Among Chinese adults, the associations between education and diabetes prevalence and incidence differed qualitatively between men and women, whereas higher household income was positively associated with diabetes prevalence and incidence in both sexes, with a stronger relationship in men than in women.

Abstract:BACKGROUND: Existing evidence on the associations of urinary element profiles with related food intake and cardiometabolic diseases has been limited in China.OBJECTIVES: To examine the associations of urinary toxic metals and other elements with food intakes and with the prevalence of cardiometabolic diseases.METHODS: Inductively coupled plasma mass spectrometry was used to measure the concentrations of cadmium (Cd), arsenic (As), nickel (Ni), aluminum (Al), copper (Cu), and 16 other elements in spot urine samples collected from 19,380 adults in 10 geographically diverse areas of China during 2013-2014. The levels of creatinine-corrected elements were used to analyze their correlations with self-reported dietary intake and associations with prevalent diabetes (n = 1862), stroke (n = 1322) and ischemic heart disease (IHD) (n = 1690).RESULTS: Overall, the mean (SD) age was 59.2 (10.1) years with a mean BMI of 24.2 (3.5) kg/m<sup>2</sup>. Of the 21 elements, the median (IQR) concentrations varied from 0.49 (0.31-0.82) μg/g creatinine for vanadium (V) to 1666 (1189-2321) mg/g creatinine for potassium (K). Nine urinary elements [Cd, As, Ni, lead (Pb), boron (B), magnesium (Mg), rubidium (Rb), strontium (Sr), and cesium (Cs); all rs > 0.20, p < 0.001] were positively correlated with staple food intake, five [Cd, As, selenium (Se), Rb, and Cs; all rs > 0.20, p < 0.001] with animal-sourced food group, and one (Cd; r = 0.21, p < 0.05) with pickled vegetable intake. For diabetes, adjusted prevalence ratios (PRs) per SD of specific element levels were 1.10 [95% confidence interval (CI): 1.03-1.18] for Cd, 1.24 (1.18-1.31) for As, 1.33 (1.27-1.39) for Ni, 1.14 (1.09-1.20) for Al, and 1.24 (1.18-1.30) for Cu. Cd was positively associated with stroke (PR per SD = 1.13, 1.04-1.23), while none of the elements were significantly associated with IHD.CONCLUSION: In China, the urinary levels of several toxic metals were significantly associated with the consumption of specific food groups and the risk of cardiometabolic diseases including diabetes and stroke.

Abstract:Objective: The relationship between plasma copper concentration and prevalence of diabetes in adults with hypertension is unclear. We aimed to determine the association between plasma copper concentration and prevalence of diabetes in Chinese adults with hypertension. Methods: A total of 2,579 participants (697 cases and 1,882 controls) was included in this cross-sectional study. Plasma copper concentrations were determined by inductively coupled plasma mass spectrometry. Multivariable logistic regression model was used to determine the association between plasma copper concentration and prevalence of diabetes. Results: According to the logistic regression analyses, the adjusted OR for the prevalence of diabetes in participants with plasma copper concentration ≥109.4 μg/dL was 1.26 (1.00, 1.58) compared with those with plasma copper concentration <109.4 μg/dL ( P = 0.048). The association was no longer significant following further adjusting for serum high-density lipoprotein cholesterol (HDL-C) concentration as a potential confounder. Stratified analyses demonstrated that serum HDL-C concentration significantly modified the association between plasma copper concentration and prevalence of diabetes ( P-interaction = 0.043). In the strata of serum HDL-C concentration ≥1.2 mmol/L, a 56% increased prevalence of diabetes was observed in participants with plasma copper concentration ≥109.4 μg/dL compared with those with plasma copper concentration <109.4 μg/dL ( P = 0.008). No significant relationship between plasma copper concentration and prevalence of diabetes was found in other strata. Conclusion: Our findings suggested that high plasma copper concentration (≥109.4 μg/dL) was associated with increased prevalence of diabetes in Chinese hypertensive adults with serum HDL-C concentration ≥1.2 mmol/L.

Abstract:The evidence about the effect of non-essential metal mixture on fasting plasma glucose (FPG) levels among older adults without diabetes is limited. This study aims to estimate the individual and joint relationship between five non-essential metals and FPG levels in Chinese older adults without diabetes. This study included 2362 older adults without diabetes. Urinary concentrations of five non-essential metals, i.e., cesium (Cs), aluminum (Al), thallium (Tl), cadmium (Cd), and arsenic (As), were detected by inductively coupled plasma mass spectrometry (ICP-MS). The associations of single metals and the metal mixture with FPG levels were assessed using linear regression and Bayesian kernel machine regression (BKMR) models, respectively. Adjusted single-metal linear regression models showed positive associations of urinary Al ( β = 0.016, 95% CI : 0.001–0.030) and Cs ( β = 0.018, 95% CI : 0.006–0.031) with FPG levels. When comparing the 2th, 3th, and 4th quartiles of urine Cs to its 1th quartile, the significant associations between Cs and FPG levels were found and presented as an "inverted U" trend ( β Q2 vs. Q1 : 0.034; β Q3 vs. Q1 :0.054; β Q4 vs. Q1 : 0.040; all P <0.05). BKMR analyses showed urinary level of Cs exhibited an "inverted U" shape association with FPG levels. Moreover, the FPG levels increased linearly with the raised levels of the non-essential metal mixture, and the posterior inclusion probability (PIP) of Cs was the highest (0.92). Potential positive interaction of As and Cs on FPG levels was found in BKMR model. Stratified analysis displayed significant interactions of hyperlipidemia and urine Cs or Tl on FPG levels. An inverse U-shaped association between Cs and FPG was found, individually and as mixture. The FPG levels increased with the raised levels of the non-essential metal mixture, and Cs was the most contributor to FPG levels. Further research is required to confirm the correlation between non-essential metals and FPG levels and to clarify the underlying mechanisms.

Alterations of serum trace elements in patients with type 2 diabetes

Abstract:Objective: The aim of the present study is to investigate the association of trace elements and the risk of type 2 diabetes mellitus. Design and methods: The 1837 participants (637 men and 1200 women) aged 40-70 were from a cross-sectional community-based study performed in downtown Shanghai. All the participants without diabetes mellitus history underwent a 75-g OGTT. The participants with diabetes mellitus took 100g steamed bread as the substitute. The fasting and OGTT 2h or postprandial 2h venous blood samples were collected. Blood glucose levels, fasting serum insulin concentrations, lipid profiles, HbA1C and 19 trace elements including magnesium, copper, zinc and selenium and so on were assayed. Results: Of all the 1837 studied subjects, 510 subjects had diabetes mellitus (191 male, 319 female). Serum magnesium levels were decreased statistically (p<0.05), but serum copper, zinc and selenium levels were significantly increased in subjects with diabetes mellitus compared to non-diabetic subjects (p<0.01 for copper, p<0.001 for zinc and selenium). Logistic regression analysis showed that serum magnesium was negatively associated with diabetes (p<0.05) and serum copper, zinc, and selenium were all positively associated with diabetes (p<0.05 for copper, p<0.001 for both zinc and selenium). Correlation analysis showed a remarkable correlation between blood glucose, HbA1C and serum magnesium, copper, zinc, and selenium (p<0.01 for zinc, p<0.001 for copper, zinc and selenium). Conclusions: Serum magnesium levels are decreased and serum copper, zinc and selenium levels are elevated in patients with type 2 diabetes mellitus.

Investigation regarding multi-element exposure among subjects with different exposure level is warranted to confirm the association and further explore dose-dependent relationship.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>A total of 3488 participants were recruited from four counties of Hunan province, South China. Basic characteristics were collected by face to face interview and 23 elements in plasma were determined by inductively coupled plasma mass spectrometry. We applied fully adjusted generalized linear regression model and multivariable restricted cubic spline function to test the association and dose-response relationship of FSG with 23 elements.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>The results indicated that FSG was positively associated with plasma<sup>78</sup>selenium level [regression coefficient (β), 0.001; 95 % confidence interval (CI), 0.001, 0.001] in a dose-dependent manner, robust to the adjustment for suspected covariates and stratification by age, gender, BMI and smoking status. A negative association was found between FSG and plasma <sup>208</sup>lead (β, -0.004; 95 % CI, -0.016, -0.002), <sup>52</sup>chromium (β, -0.002; 95 % CI, -0.004, -0.001) and <sup>47</sup>titanium (β, -0.001; 95 % CI, -0.002, -0.001).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p><sup>78</sup>selenium was positively while <sup>208</sup>lead, <sup>52</sup>chromium and <sup>47</sup>titanium were negatively associated with FSG in the present study. However, prospective studies are needed to confirm the results.</p>

Abstract:Introduction: Although several studies have explored the associations between single essential metals and serum uric acid (SUA), the study about the essential metal mixture and the interactions of metals for hyperuricemia remains unclear. Methods: We performed a cross-sectional study to explore the association of the SUA levels with the blood essential metal mixture, including magnesium (Mg), calcium (Ca), iron (Fe), copper (Cu), zinc (Zn), manganese (Mn) in Chinese community-dwelling adults (n=1039). The multivariable linear regression, the weighted quantile sum (WQS) regression and Bayesian kernel machine regression (BKMR) were conducted to estimate the associations of blood essential metals with SUA levels and the BKMR model was also conducted to estimate the interactions of the essential metals on SUA. Results: In the multivariable linear regression, the association of blood Mg, Mn, and Cu with SUA was statistically significant, both in considering multiple metals and a single metal. In WQS regression [β=13.59 (95%CI: 5.57, 21.60)] and BKMR models, a positive association was found between the mixture of essential metals in blood and SUA. Specifically, blood Mg and Cu showed a positive association with SUA, while blood Mn showed a negative association. Additionally, no interactions between individual metals on SUA were observed. Discussion: In conclusion, further attention should be paid to the relationship between the mixture of essential metals in blood and SUA. However, more studies are needed to confirm these findings.

Abstract:Background: The relationship between plasma metal elements and cognitive function is unclear, especially in extremely older individuals. This present study aimed to explore the association between plasma metal concentrations and the risk of cognitive impairment (CI) in Chinese extremely older adults. Methods: Individuals aged ≥90 years with plasm metal concentration data from the fifth wave of the 2008 Chinese Longitudinal Healthy Longevity Survey were included. Plasma selenium (Se), manganese (Mn), magnesium (Mg), calcium (Ca), iron (Fe), copper (Cu), and zinc (Zn) concentrations were measured using inductively coupled plasma optical emission spectroscopy. Cognitive function was assessed by the Chinese version of the mini-mental state examination. Results: The study enrolled 408 participants. Participants with CI had significantly lower plasma Se, Mn, and Fe levels and higher Ca levels than those with normal cognitive function ( p < 0.05). Plasma Se, Mn, Ca, and Fe concentrations were significantly associated with CI risk in both single- and multiple-element logistic regression models. Additionally, the multiple-element model results showed that the adjusted odds ratios for CI were 0.042 (95% confidence interval 0.016–0.109), 0.106 (0.044–0.255), 7.629 (3.211–18.124) and 0.092 (0.036–0.233) for the highest quartiles compared to the lowest quartiles of Se, Mn, Ca, and Fe, respectively. Moreover, subgroup analyses by age, sex, and body mass index suggested a consistent significant correlation ( p < 0.05). Conclusion: Therefore, decreased plasma Se, Mn, and Fe and increased plasma Ca levels were associated with CI risk in Chinese older adults. These findings are of great significance for the development of programs to delay cognitive decline in the elderly.

Abstract:BACKGROUND: A balanced intake of trace elements is beneficial for chronic diseases such as hypertension. However, the available information regarding trace elements that may be independently associated with hypertension is limited, and the relationship between this disorder and element ratios also remains unclear.METHODS: A total of 6,754 subjects from rural China were selected, after exclusion of patients who were under 18, had incomplete data or had additional related disorders, by multi-stage simple random and cluster sampling (participation rate: 95.22 %). Subjects were divided into a hypertensive (H) and a control (C) group. Data were collected on blood pressure and 12 serum trace elements were measured by flame atomic absorption spectrometry and inductively coupled plasma-mass spectrometry. Other basic information was collated from questionnaires and biochemical indicators were measured via kits.RESULTS: Differences in serum levels of magnesium (Mg(mg/l): H: 27.43 ± 12.72; C: 26.33 ± 12.16), iron (Fe(mg/l): H: 1.99 ± 1.24; C: 1.84 ± 1.16), copper (Cu(mg/l): H: 1.19 ± 0.37; C: 1.10 ± 0.36), boron (B(μg/l): H: 50.00 ± 25.21; C: 47.57 ± 26.25), selenium (Se(μg/l): H: 125.12 ± 32.81; C: 118.80 ± 29.72) and chromium (Cr(μg/l): H: 8.77 ± 10.12; C: 10.12 ± 10.72) between the hypertensive and control groups were found. There were no differences in serum contents of calcium (Ca(mg/l): H: 112.43 ± 58.25; C: 111.00 ± 59.49), zinc (Zn(mg/l): H: 1.50 ± 1.97; C: 1.44 ± 1.88), arsenic (As(μg/l): H: 4.17 ± 3.94; C: 4.10 ± 4.00), manganese (Mn(μg/l): H: 4.15 ± 4.03; C: 4.07 ± 4.05), cadmium (Cd(μg/l): H: 1.14 ± 1.11; C: 1.18 ± 1.12) or lead (Pb(μg/l): H: 4.22 ± 8.90; C: 4.26 ± 10.25). The serum Cr and Cd concentrations of hypertensive men were lower than that of male controls while Mg, Cu, Ca and Se concentrations in male controls were lower. Further differences were apparent and Fe, B, Se, Mg and Cu all showed higher levels in hypertensive females whereas Cr concentrations were higher in female controls. Serum Zn and B levels showed age-related variations among hypertensive patients and concentrations of serum Cu, Zn, Se and B showed age-related variations among control subjects. For hypertensive patients, the odds ratio (OR) and 95 % confidence interval (CI) for the association of serum Cu, Se and Cr levels with hypertension were Cu: 1.36 (1.12-1.66); Se: 1.03 (1.01-1.05); Cr: 0.89 (0.83-0.96). Moreover, when the participants in the grouping with the highest copper/zinc (Cu/Zn) and magnesium/manganese (Mg/Mn) ratios were compared with the reference group, the OR and 95 % CI for hypertension were 1.22 (1.04-1.44) and 1.20 (1.01-1.42), respectively.CONCLUSIONS: Levels of serum trace elements showed age- and sex-related differences in a group of rural Chinese adults with hypertension and healthy participants. Serum concentrations of Cu, Se and Cr may be independently associated with hypertension. Higher serum ratios of Cu:Zn and Mg:Mn may also be associated with hypertension. Further randomized trials are necessary to elucidate the true relationship between levels of Cu, Se, Cr, Cu:Zn, Mg:Mn and hypertension.

Abstract:Several studies have suggested an association between exposure to various metals and the onset of type 2 diabetes (T2D). However, the results vary across different studies. We aimed to investigate the associations between serum metal concentrations and the risk of developing T2D among 8734 participants using a prospective cohort study design. We utilized inductively coupled plasmamass spectrometry (ICP-MS) to assess the serum concentrations of 27 metals. Cox regression was applied to calculate the hazard ratios (HRs) for the associations between serum metal concentrations on the risk of developing T2D. Additionally, 196 incident T2D cases and 208 healthy control participants were randomly selected for serum metabolite measurement using an untargeted metabolomics approach to evaluate the mediating role of serum metabolite in the relationship between serum metal concentrations and the risk of developing T2D with a nested casecontrol study design. In the cohort study, after Bonferroni correction, the serum concentrations of zinc (Zn), mercury (Hg), and thallium (Tl) were positively associated with the risk of developing T2D, whereas the serum concentrations of manganese (Mn), molybdenum (Mo), barium (Ba), lutetium (Lu), and lead (Pb) were negatively associated with the risk of developing T2D. After adding these eight metals, the predictive ability increased significantly compared with that of the traditional clinical model (AUC: 0.791 vs. 0.772, P=8.85×10-5). In the nested casecontrol study, a machine learning analysis revealed that the serum concentrations of 14 out of 1579 detected metabolites were associated with the risk of developing T2D. According to generalized linear regression models, 7 of these metabolites were significantly associated with the serum concentrations of the identified metals. The mediation analysis showed that two metabolites (2-methyl-1,2-dihydrophthalazin-1-one and mestranol) mediated 46.81% and 58.70%, respectively, of the association between the serum Pb concentration and the risk of developing T2D. Our study suggested that serum Mn, Zn, Mo, Ba, Lu, Hg, Tl, and Pb were associated with T2D risk. Two metabolites mediated the associations between the serum Pb concentration and the risk of developing T2D.

Abstract:BACKGROUND: Metal exposures have been reported to be related to the progress of metabolic syndrome (MetS), however, the currents results were still controversial, and the evidence about the effect of multi-metal exposure on MetS were limited. In this study, we intended to evaluate the relationships between metal mixture exposure and the prevalence of MetS in a mid-aged and older population of China.METHODS: The plasma levels of 13 metals (aluminum, magnesium, calcium, iron, manganese, cobalt, copper, arsenic, zinc, selenium, cadmium, molybdenum and thallium) were detected by inductively coupled plasma mass spectrometry (ICP-MS) in 1277 adults recruited from the Eighth Affiliated Hospital of Sun Yat-Sen University (Shenzhen, China). Logistic regression, the adaptive least absolute shrinkage and selectionator operator (LASSO) penalized regression analysis and restricted cubic spline (RCS) analysis were used to explore the associations and dose-response relationships of plasma metals with MetS. To evaluate the cumulative effect of metals, the Bayesian Kernel Machine Regression (BKMR) model was applied.RESULTS: The concentrations of magnesium and molybdenum were lower in the MetS group (p < 0.05). In the single-metal model, the adjusted ORs (95%CI) in the highest quartiles were 0.44 (0.35, 0.76) for magnesium and 0.30 (0.17, 0.51) for molybdenum compared with the lowest quartile. The negative associations and dose-dependent relationships of magnesium and molybdenum with MetS were further validated by the stepwise model, adaptive LASSO penalized regression and RCS analysis. The BKMR models showed that the metal mixture were associated with decreased MetS when the chemical mixtures were≥ 25th percentile compared to their medians, and Mg, Mo were the major contributors to the combined effect. Moreover, concentrations of magnesium were significantly related to blood glucose, and molybdenum was related with BMI, blood glucose and blood pressure.CONCLUSIONS: Elevated levels of plasma magnesium and molybdenum were associated with decreased prevalence of MetS. Further investigations in larger perspective cohorts are needed to confirm our findings.

Abstract:Manganese (Mn) is an essential trace metal element that is associated with diabetes; however, the results of previous studies are inconsistent. Furthermore, few studies have been conducted in a hypertensive population. The purpose of this study is to explore the relationship between manganese and diabetes in a population with hypertension. A cross-sectional study was conducted, including 2575 hypertensive individuals from 14 provinces in China. Serum manganese concentrations were measured by the inductively coupled plasma mass spectrometry (ICP-MS) method. And logistic regression models were used to analyze the association between serum manganese and the risk of diabetes. The prevalence of diabetes was 27.0% in this hypertensive population. In logistic regression models, the odds ratios (95% confidence interval) for diabetes in tertile subgroups were 1.40 (1.12, 1.76) and 1.32 (1.05, 1.65) for tertiles 1 and tertiles 3, respectively, compared to tertile 2 (reference). Additionally, an interaction between sex and manganese was observed. The odds ratios (95% confidence interval) for diabetes were 1.29 (0.95, 1.75) and 0.96 (0.70, 1.31) for tertiles 1 and tertiles 3 among males, and 1.44 (1.01, 2.04) and 1.81 (1.29, 2.55) for tertiles 1 and tertiles 3 among females, respectively, compared to tertile 2. In conclusion, a U-shaped association between serum manganese and diabetes was observed in a Chinese population with hypertension, and the association was modified by sex.

Abstract:Metal exposure has been reported to be associated with metabolic syndrome (MetS), however, the evidence remains inconclusive, particularly in elderly individuals. From May to July 2016, serum levels of 16 metals were measured using inductively coupled plasma mass spectrometry (ICP-MS) in 852 elderly individuals (≥65 years) residing in Wuhan, China. Biological detection and disease recognition were based on individual surveys conducted during health check-ups. Spearman's rank correlation analysis was performed to identify the correlation among serum metals. The data were Ln-transformed to fit a normal distribution for further analyses. Linear and logistic regression were applied to explore the associations between metals and diseases. Restricted cubic spline (RCS) analysis was utilized to examine dose-response relationships. The Weighted Quantile Sum (WQS) score was applied to determine the empirical weights of each heavy metal in the context of their combined effect on metabolic diseases. The prevalence of MetS, hypertension, diabetes, and hyperlipidemia were 46.36 %, 68.90 %, 24.65 %, and 21.60 %, respectively. Serum metal mixture was positively associated with the prevalence of MetS (OR = 1.92, 95 % CI: 1.30-2.82), hypertension (OR = 1.50, 95 % CI: 1.01-2.23), and diabetes (OR = 2.18, 95 % CI: 1.48-3.22). In single metal models, we found that serum zinc levels were associated with an increased risk of MetS, while rubidium had a protective effect against MetS. Interestingly, different metals had distinct effects on specific diseases in this study: lithium and barium were more likely to influence blood pressure, while selenium had a more significant effect on blood glucose. Lipids were more susceptible to the effects of zinc, selenium, and strontium. Platelet count (PLT) and lymphocyte count (LYM) mediated the association between selenium exposure and hyperlipidemia, while neutrophil count (NEU) mediated the relationship between serum rubidium exposure and MetS. Our findings offer valuable etiological insights into the relationship between serum heavy metals and the prevalence of MetS, suggesting that peripheral blood cells may play a mediating role in this association.

Abstract:Background: Although several studies have examined the relationships between lead (Pb) exposure and serum lipid profiles, the associations of the metal mixture, including lead (Pb) and essential metals with lipid profiles remain unclear. Objective: To investigate the associations of the metal mixture including Pb and essential metals [magnesium (Mg), manganese (Mn), copper (Cu), iron (Fe), zinc (Zn), and calcium (Ca)] with serum lipid profiles [total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C) and high-density lipoprotein cholesterol (HDL-C)], as well as the potential interactions among the metals. Methods: 998 Chinese community-dwelling adults completed a questionnaire and underwent checkups of anthropometric parameters, serum lipid profile levels (TC, TG, LDL-C, and HDL-C), blood metal concentrations (Pb, Mg, Mn, Cu, Fe, Zn, and Ca). The multivariable linear regression, weighted quantile sum (WQS) regression, and Bayesian kernel machine regression (BKMR) were applied to evaluate the single and combined associations of blood Pb and essential metals with serum lipid profiles. Results: In the multivariable linear regression model, the blood Pb was positively associated with TC, LDL-C, and HDL-C (P <0.05, all), and the blood Mg were positively associated with serum TC, LDL-C, and Ln TG (P <0.05, all). In the WQS regression and BKMR models, the metal mixture of blood Pb and the essential metals was positively associated with all of the serum lipid profiles. In addition, an inverse U-shaped association of Pb with Ln TG and the positive interactive effect between blood Pb and Mg levels on TC and LDL-C were found. Conclusions: The levels of blood Pb, together with the essential metals, especially Mg levels, are suggested to be considered when assessing dyslipidemia risk. However, more evidence is still needed to validate the conclusions.
