Abstract:Background: Fetal heart evaluation has been proven to have the utility in the differential diagnosis of cardiomyopathies or prediction of perinatal mortality in congenital heart disease. The purpose of this study is to introduce a novel method that can measure the global and regional longitudinal strain of left ventricle in fetuses, described as automated cardiac motion quantitation (aCMQ). Furthermore, to evaluate the feasibility and value of aCMQ. Methods: A cross-sectional study was conducted in 138 fetuses with normal heart structure, then were divided into four groups by gestational age, standard four-chamber view clips were collected, the global and segmental longitudinal peak systolic strain of the left ventricle were measured by aCMQ, interobserver and intraobserver variability was analyzed. Results: The success rate of aCMQ analysis was 90.6% (125/138). aCMQ can obtain more comprehensive data to evaluate fetal cardiac function by on-line analysis, including the left ventricular end-systolic volume (LVESV, 0.64???0.16?ml), the left ventricular end-diastolic volume (LVEDV, 0.85???0.35?ml), strain and so on. There was no significant correlation between the gestational age and the global and segmental longitudinal peak strain of the left ventricle (all p?>?.05). The base and middle segmental strain values of left ventricular free wall were similar to those of the interventricular septum. No significant differences were found in base segments and middle segments for left ventricular free wall (LVLW) and the interventricular septum (IVS) (p?>?.05). The strain of the apex segment was higher than that of the base and middle segment (p?<?.05), and there were significant differences between the apex and the base and the middle (p?<?.05). Interobserver and intraobserver variability showed a small bias among the observers. Conclusions: The global and regional strain values of fetus have no correlation with gestational ages, and the strain values of apical segment are higher than those of basal and middle segments. The base and middle segmental strain values of left ventricular free wall were similar to those of the interventricular septum. aCMQ is a novel method of two dimensional speckle tracking echocardiography to assess the global and regional systolic function of fetuses. It?s a feasible and reproducible approach to evaluate normal cardiac function of fetus quantitatively and may have potential in fetuses with congenital heart diseases.
New Insight from Using Spatiotemporal Image Correlation in Prenatal Screening of Fetal Conotruncal Defects

Abstract:Background: To establish the reference range of the angle between ascending aorta and main pulmonary artery of fetus in the second and third trimester using spatiotemporal image correlation (STIC), and to investigate the value of this angle in prenatal screening of conotruncal defects (CTDs). Materials and Methods: Volume images of 311 normal fetuses along with 20 fetuses with congenital heart diseases were recruited in this cross-sectional study. An offline analysis of acquired volume datasets was carried out with multiplanar mode. The angle between aorta and pulmonary artery was measured by navigating the pivot point and rotating axes and the reference range was established. The images of ascending aorta and main pulmonary artery in fetuses with congenital heart diseases were observed by rotating the axes within the normal angle reference range. Results: The angle between ascending aorta and main pulmonary artery of the normal fetus (range: 59.1˚~97.0˚, mean ± SD: 78.0˚ ± 9.7˚) was negatively correlated with gestational age (r = -0.52; p < 0.01). By rotating the normal angle range corresponding to gestational age, the fetuses with CTD could not display views of their left ventricular long axis and main pulmonary trunk correctly. Conclusion: The left ventricular long axis and main pulmonary trunk views can be displayed using STIC so that the echocardiographic protocol of the cardiovascular joint could be standardized. The reference range of the angle between ascending aorta and main pulmonary artery is clinically useful in prenatal screening of CTD and provides a reliable quantitative standard to estimate the spatial relationship of the large arteries of fetus.
Study of Xplane Imaging in Quantifying the Maximal Volume of the Left and Right Atria and Evaluating Atrial Function in Fetuses with Abnormal Heart Morphology

Abstract:Objective To explore the feasibility of real-time three-dimensional ultrasound Xplane imaging in quantifying left and right atrial diastolic maximal volume (LAVmax,RAVmax) and evaluating cardiac diastolic function in fetuses with cardiac disease in second and later trimesters.Methods One hundred and forty-four fetuses with abnormal heart morphology at 16-34 weeks of gestational age were included and divided into 3 groups according to the influence of pathological changes on atrial volume:group A with symmetrical left and right atrial volume,group B with decreased left atrial volume and increased right atrial volume and group C with increased left atrial volume and decreased right atrial volume,and the fetus were also divided into 2 groups according to the law of fetal development:the middle pregnancy group (16-27+6 weeks) and the late pregnancy group (28-34+6 weeks).Using the "Xplane" mode of volume probe,the maximal atrial volume was calculated automatically by tracing method and three-path line method.The correlation between the two methods in quantitative LAVmax was validated by paired sample t test and Pearson correlation analysis.The correlation between LAVmax,RAVmax and gestational age were analyzed by curve fitting.The volumes of bilateral chambers and the average weekly growth rates of E peak,A peak and E/A value of mitral and tricuspid orifices were calculated and compared.Results There was no significant statistical difference between the maximal volume of the left atrium obtained by the tracing method and the three-diameter line method (P ＞0.05),and there was high correlation between the two methods in the comparison of the maximal volume of the left atrium (r =0.90,0.88,0.85;all P ＜0.01).The data of group A,B and C showed that LAVmax and RAVmax could increase with the increase of gestational weeks in a certain period of abnormal state,and had a good correlation with them(LAVmax:r =0.78,0.74,0.78,all P ＜0.005;RAVmax:r =0.79,0.77,0.78,all P ＜0.005).The average weekly growth rate of RAVmax showed an advantage in group A,B and C.Especially in group C with reduced right atrium,the growth rate of right atrium was 8.15 ％,which was higher than that of group B with decreased left atrium by 5.06％.The weekly growth rates of E peak and E/A in tricuspid orifice were also higher than those in mitral orifice.The E peak and E/A values of tricuspid orifice in group C were 4.05％ and 0.60％,respectively,higher than those in group B,which were 2.58％ and 0.02％.Peak A showed an increase in growth rate in group B and group C with decreased atria,peak A values in group B and group C increased by 4.01 ％ and 2.19％,respectively.Conclusions The right ventricular dominance of fetal heart can still be reflected in certain stages of disease,and the atrial active systolic may play a regulatory role in the filling of cardiac blood flow.Real-time three-dimensional ultrasound Xplane imaging could be used to quantify the atrial volume of fetuses with abnormal heart morphology in second and later trimesters and to preliminary assess atrial function combined with the changes of atrioventricular valve orifice hemodynamics.Real-time three-dimensional ultrasound Xplane imaging technology has obvious advantages of simple,safe,non-invasive,simultaneous and high repeatability in measuring fetal atrial volume.

Abstract:Objective:To evaluate the application value of fetal intelligent navigation echocardiography (FINE, 5D Heart) combined with virtual intelligent sonographer assistance (VIS-Assistance?) in the diagnosis of fetal tetralogy of Fallot (TOF).Methods:From February to January 2020, 57 pregnant women who received echocardiography examination for fetuses at Sir Run Run Shaw Hospital were selected as subjects in this study. Doctor A with rich fetal echocardiography experience collected two to five spatio-temporal image correlation (STIC) volume datasets for each fetal heart. The inexperienced doctor B and the more experienced doctor C screened out the best volume dataset in each fetus, and applied 5D Heart and VIS-Assistance? for post-processing. According to different spine positions, 57 TOF fetuses were divided into three subgroups: A (spine 2-4 o'clock), B (spine 5-7 o'clock), and C (spine 8-10 o'clock). Five diagnostic views (three-vessel tracheal view, four-chamber view, left ventricular outflow view, right ventricular outflow view, and abdomen/stomach view) and 17 main diagnostic elements of TOF fetuses were scored, and in particular the following parameters were investigated: the display rates of five diagnostic views and 17 diagnostic elements in the three subgroups and the differences between subgroups; comparison of the display rates of 17 diagnostic elements twice by two doctors with different experience; and repeatability and consistency test of two different senior doctors' scoring results in five diagnostic views.Results:With the application of 5D Heart and VIS-Assistance?, there was no significant differences in the display rates of five diagnostic views and 17 diagnostic elements between the three subgroups (P＞0.05). The display rates of the diagnostic elements of two different senior doctors are as follows: in the three-vessel tracheal view, the display rates of diagnostic elements (dilated aorta, stenosis of pulmonary artery, trachea, and superior vena cava) were 94.7%-96.5%, 93.0%-98.2%, 70.2%-84.2%, and 86.0%-91.2%, respectively. In the four-chamber view, the display rates of the diagnostic elements (four chambers, atrioventricular valves, mitral valve-tricuspid valve distance, ovale foramen, cardiac cross, ventricular septum, and moderator band) were 100.0%, 100.0%, 93.0%-96.5%, 78.9%-82.5%, 94.7%-96.5%, 100.0%, and 100.0%, respectively. In the left ventricular outflow view, the display rates of diagnostic elements (ventricular septal defect, dilated aorta, and the aorta overriding ventricular septal defect) were 96.5%-100.0%, 100.0%, and 96.5%-98.2%, respectively. In the right ventricular outflow view, the display rates of stenosis of the pulmonary artery was 80.7%-91.2%. In the abdomen/stomach view, the display rate of the diagnostic elements (stomach and descending aorta) was 100.0%. There was no statistical differences for the results between two doctors with different experience, and for scores of the five diagnostic views by the same doctor 1 month before and after (P＞0.05).Conclusion:5D Heart combined with VIS-Assistance? has high display rates in various diagnostic views and diagnostic elements in fetal TOF, and has good repeatability and consistency. It has relatively less dependence on the experience of the operating physician, which may has potentials in the prenatal diagnosis of TOF. Using 5D Heart, the fetal spine position within a certain range seems not to affect the TOF image display rate.

Abstract:To evaluate the usefulness of free angle M-mode echocardiography (FAM) in obtaining tricuspid annular plane systolic excursion (TAPSE) for assessing the fetal right ventricular function and to compare the results of measurements of TAPSE by conventional M-mode (CM) and FAM. 243 normal fetuses in second and late trimester were divided into 5 groups by gestational age: 20 ∼ 24 weeks, 24+1 ∼ 28 weeks,28+1 ∼ 32 weeks, 32+1 ∼ 36 weeks, 36+1 ∼ 40 weeks, the TAPSE were measured by CM and FAM echocardiography, meanwhile multiple parameters for evaluating right ventricular function were obtained by using myocardial Doppler tissue imaging (DTI) and the correlation between TAPSE and other parameters were analysed by linear correlation. ANOVA was used to compare CM-TAPSE, FAM-TAPSE, Em, Sm and FS with different gestational age. Regression equation estimate was used to compare the relationship of FAM-TAPSE with gestational ages. Independent sample T test was used to compare CM-TAPSE with FAM-TAPSE. There were significant differences in CM-TAPSE, FAM-TAPSE, Em and Sm among 5 groups (P = 0.000), the measured value of FAM-TAPSE was higher than that of CM-TAPSE (P = 0.000). Both FAM-TPASE and CM-TAPSE showed significant positive correlations with GA, Em and Sm, but FAM-TAPSE demonstrated better correlation than CM-TAPSE. By adjusting sample line of FAM, measuring the maximum displacement of tricuspid annular plane is feasible and FAM may evaluate fetal right ventricular function more accurately and effectively.

Preliminary study of cardiac morphology and function in fetuses with tetralogy of Fallot using fetal heart quantification

Abstract:Objective:To evaluate the fetal heart shape and function in tetralogy of Fallot (TOF) by fetal heart quantitative analysis (fetal HQ).Methods:A total of 52 fetuses with TOF diagnosed by fetal echocardiography and 200 normal fetuses matched with their gestational weeks from March 2020 to March 2022 at Sir Run Run Shaw Hospital, Zhejiang University were retrospectively evaluated. The basic parameters of fetal cardiac blood vessels in the two groups were measured by fetal HQ technology and conventional M-mode ultrasound technology: aortic valve diameter(AV), pulmonary artery valve diameter(PV), main pulmonary artery diameter (MPA) and Z-score. The overall morphometric measurements including end-diastolic length diameter, transverse diameter, area, and global spherical index (GSI) of the fetal heart in the 4-chamber view(4CV), area and length of the right and left ventricles and their ratios. Measurements of left and right ventricular function included ejection fraction (EF), fraction area change rate (FAC), tricuspid annular plane systolic excursion (TAPSE), left and right ventricular global longitudinal strain (GLS), and left and right ventricular end-diastolic diameter (ED), spherical index (SI), and fractional shortening rate (FS) of 24-segments. The differences of above parameters between TOF group and control group were compared. In addition, the relationships between the absolute value of left and right ventricular GLS of TOF fetus and PV/AV, PV Z-score and MPA Z-score were analyzed. The optimal critical values of GSI, left ventricular EF and left ventricular FAC of TOF fetus were determined by ROC curve, and their corresponding sensitivity and specificity were obtained.Results:Compared with control group, there were significant differences in 4CV end-diastolic length, area, GSI, left ventricular area, left ventricular length, left ventricular EF, left ventricular FAC and left ventricular GLS in TOF group (all P<0.05). There were significant differences in ED between left ventricular 15-24 segments and right ventricular 1-21 segments (all P<0.05). There were significant differences in SI between left ventricular 1-16 segments, 21-24 segments and all segments of right ventricle (all P<0.05). The differences in FS were statistically significant (all P<0.05) when comparing all segments of the left ventricle and 1-2 segments of the right ventricular, and the remaining parameters were not statistically significant (all P>0.05). The left ventricular GLS absolute value of TOF fetuses was positively correlated with PV/AV, PV Z-score and MPA Z-score( rs=0.338, 0.441 and 0.458, all P<0.05), the right ventricular GLS absolute value of TOF fetuses was positively correlated with PV AV, PV Z-score and MPA Z-score( rs=0.418, 0.368 and 0.338, all P<0.05). The optimal critical values of GSI, left ventricular EF, and left ventricular FAC in the diagnosis of fetal TOF were 1.19, 59.05%, and 44.4%, respectively. At this time, the sensitivities of diagnosis of TOF were 0.78, 0.75, and 0.80, respectively. The specificities were 0.88, 0.88 and 0.83, respectively. The areas under ROC curve were 0.89, 0.88 and 0.89, respectively. Conclusions:Fetal HQ technology can provide a simple and reliable quantitative evaluation of fetal heart shape and function, and provide certain theoretical parameters for the study of fetal heart shape and function.
A Preliminary Study of Fetal Intelligent Navigation Echocardiography in the Display of 9 Views in the Normal Fetuses by a New Sonographer

Abstract:Objective To explore the applicability of fetal intelligent navigation echocardiography (FINE, 5D Heart) in displaying the normal fetal heart structure by a new sonographer and assess the value of this technique in fetal cardiac ultrasonography. Methods Eighty cases of normal fetal heart at the second trimester were scanned by an echocardiographic expert. STIC volume datasets with satisfactory from apical four-chamber and common 2D modality with 9 standard views of fetal echocardiogram were achieved for each case. The STIC volume images were post-processed with 5D heart software by two physicians with different levels of echocardiographic examination. The quality of the section images of 9 standard views from 2D 5D heart and were analyzed. The intraobserver and interobserver with common 2D modality repeatability were analyzed for the qualification of section images. The time spent in post-processing the images was record and analyzed. Results The qualified 9 section images with 2D were 75~80 cases (degree of qualification 93.8%~100.0%). The data were assessed twice by two observers and qualified 9 section images were got in 73~80 cases (degree of qualification 91.3%~100.0%). The intra-observer agreement by two observers was absolute concord in four chamber view and abdomen/stomach view (Kappa=1); excellent in three vessel trachea view (3VT, Kappa=0.851~0.882); good to excellent in five chamber view (Kappa=0.787~0.882); good in aortic arch view (Kappa=0.738~0.787), left ventricular outflow tract view (Kappa=0.709~0.787), right ventricular outflow tract view (Kappa=0.655~0.794), superior and inferior vena cava view (Kappa=0.647~0.649) and ductal arch view (Kappa=0.640~0.707). The inter-observer agreement by each two observers and 2D was absolute concord in four chamber view and abdomen/stomach view (Kappa=1); excellent in five chamber view (Kappa=0.851~0.902); good to excellent in ductal arch view (Kappa=0.749~0.820), aortic arch view (Kappa=0.707~0.882), right ventricular outflow tract view (Kappa=0.661~0.851) and 3VT (Kappa=0.655~0.851); good in left ventricular outflow tract view (Kappa=0.737~0.749), superior and inferior vena cava view (Kappa=0.655~0.794). The average time spent in post-processing the images for first time by three observers was (2.64±1.35) min, and there was no significant difference during the each observers (P>0.05). Conclusions 5D Heart represents a single method in obtaining 9 fetal diagnosis planes, which can be used easily by a sonographer lack of skills, and has high reliability and repeatability. Initial Feasibility Study on Live Three-dimensional Fetal Echocardiography in Normal Fetuses

Abstract:Objective The purpose of this study was to investigate the feasibility of using live three-dimensional fetal echocardiography (Live-3DE)in normal fetuses.Methods Twenty one normal fetuses at various gestational ages of 17-36 weeks (mean,27weeks)were investigated using a novel Live-3DE system.The fetal cardiovascular morphological views and anatomic correlation were systematically assessed.The fetal Live-3DE images were further evaluated independently by 2 experienced observers.Their visualization rates of the cardiac structures were calculated and the agreement between them was assessed with the K statistics.Results Live-3DE provided a good comprehensive assessment of 4-chamber views,5-chamber-views,views of left ventricular outlet in 19 fetuses,pulmonary artery-ductus arteriosus views in 7,3-vessel views in 5,aortic arch views in 3 and in 1 fetus with isolated LV intracardiac echogenic focus.Live-3DE offered clear and comprehensive visualization of the focus located at the level of chordae tendneae.The interobserver agreement was substantial to almost perfect for the assessment of 4-chamber,5-chamber and 3-vessel views,left ventricular outlet and 3-vessel views to observe the intact ventricular septum,foramen ovale,mitral valves,tricuspid valves and the anatomic correlation of mitral valves with tricuspid valves and both left atrium and the aorta located on the left of the spine.However,the agreement indicated moderate when visualization of the left outflow tract and moderator band in the right ventricle.Conclusions Our preliminary investigation has demonstrated the feasibility of using Live-3DE in normal fetuses.More studies are necessary to validate the clinical utility of the novel tool for prenatal diagnosis and assessment of congenital heart disease in the human fetuses.
Assessment of Fetal Left Atrial Volume and Function Using a Novel Left Atrial Volume Tracking Method

Abstract:BACKGROUND:Several fetal cardiovascular structural defects may alter the hemodynamics of the cardiac chambers resulting in changes in chamber sizes. Quantitative measurements of the sizes of cardiac chambers can augment the diagnostic power of fetal echocardiography.AIMS:Using a new left atrial volume tracking (LAVT) method, time-left atrial volume curves (TLAVCs) can be automatically obtained. The goal of this study was to examine whether this method can be used to evaluate left atrial volume (LAV) and provide reference values for LAV and indices of left atrial function in normal human fetuses.METHODS:Two hundred and four normal human fetuses were enrolled. Using LAVT, the maximal left atrial volume (LAVmax) and minimal left atrial volume (LAVmin) were measured from TLAVCs. Left atrial ejection fraction (EF) was calculated. The maximal left atrial area (LAAmax) and minimal left atrial area (LAAmin) were measured using manual method tracing.RESULTS:Between 21 and 40 weeks, mean LAVmax increased from 0.27 ml to 4.15 ml, and mean LAVmin increased from 0.13 ml to 2.26 ml, respectively, while the EF remained stable at around 0.43. From 21 to 40 weeks, mean LAAmax increased from 0.61 cm2 to 2.64 cm2, and mean LAAmin increased from 0.34 cm2 to 1.53 cm2.CONCLUSIONS:This study establishes reference values for fetal LAV during the second half of gestation. The LAVT method appears to be feasible in estimating fetal LAV and shows potential for assessing left atrial function.

The STIC volume images were processed by three independent physicians of different hierarchies via 5D Heart software afterwards ,and the quality of the STIC diagnosis as well as the consistence among the physicians were analyzed according to the gold standard of postpartum ultrasonic follow-up or surgery results . Results Twelve out of 16 CTD cases were finally included in the study ,and the volume data were analyzed . Senior physicians A could apply 5D Heart diagnosis to achieve 2D-US diagnosis efficiency and gold standard consistently( P = 0 .157 ,0 .083) ,medium and low seniority physician B and C got roughly equal score using 5D Heart diagnosis( P = 0 .705) ,but the diagnostic performance could not reach 2D-US ( P =0 .000 ,0 .000) and gold standard( P = 0 .000 ,0 .000) . Conclusions Fetal cardiac anomalies of CTD can be diagnosed correctly by an expert using 5D Heart ,which has important application prospects in remote consultation , training and scientific research and teaching of congenital heart disease of fetus .
Novel Foetal Echocardiographic Image Processing Software (5D Heart) Improves the Display of Key Diagnostic Elements in Foetal Echocardiography

Abstract:Background To evaluate the clinical value of foetal intelligent navigation echocardiography (5D Heart) for the display of key diagnostic elements in basic sections. Methods 3D volume datasets of 182 normal singleton foetuses were acquired with a four chamber view by using a volume probe. After processing the datasets by using 5D Heart, eight cardiac diagnostic planes were demonstrated, and the image qualities of the key diagnostic elements were graded by 3 doctors with different experiences in performing foetal echocardiography. Results A total of 231 volume datasets acquired from the 182 normal foetuses were used for 5D Heart analysis and display. The success rate of 8 standard diagnostic views was 88.2%, and the success rate of each diagnostic view was 55.8–99.2% and 70.7–99.0% for the random four chamber view as the initial section and for the apical four chamber view as the initial section, respectively. The success rate of each diagnostic element in the 8 diagnostic sections obtained by 5D Heart was 58.9%~ 100%. Excellent agreement was found between experienced sonographers and less-experienced sonographers (kappa> 0.769). Inter- and intra-observer agreement were substantial to near-perfect, kappa values ranging from 0.612 to 1.000 (Cohen’s kappa). Conclusions 5D Heart can significantly improve the image quality of key diagnostic elements in foetal echocardiography with low operator dependency and good reproducibility.

Abstract:Objective To assess the longitudinal mitral annular plane systolic excursion ( M APSE) of different directions in normal fetuses during mid‐late pregnancy based on two‐dimensional speckle tracking imaging ( ST I) . Methods Seventy‐six normal fetuses during middle and late pregnancy were selected at 26－32 weeks of gestation . T he peak M APSE was measured by free angle M‐mode echocardiography ( FAM ) perpendicular to the lateral annulus in the mitral annular plane . The time‐displacement curves of interventricular septal mitral annulus in three different directions including points A ,B and C through transverse level of apex were recorded by STI . T he peak M APSE of interventricular septal mitral annulus ( SEPT‐M APSE‐A ,SEPT‐M APSE‐B ,SEPT‐M APSE‐C) in three different directions including points A ,B and C and the time to peak ( T T P :SEPT‐T T P‐A ,SEPT‐T T P‐B ,SEPT‐T T P‐C) were recorded respectively . T he time‐displacement curves of lateral mitral annulus in three different directions including points A ,B and C through transverse level of apex were recorded by STI . T he peak M APSE of lateral mitral annulus ( LAT‐M APSE‐A ,LAT‐MAPSE‐B ,LAT‐MAPSE‐C) in three different directions including points A ,B and C ,the time to peak( LA T‐T T P‐A ,LA T‐T T P‐B ,LA T‐T T P‐C) were recorded respectively . Finally ,the data were analyzed statistically . Results T he peak M APSE of the lateral mitral annulus in 3 different directions including points A ,B and C[ LA T‐M APSE‐A ( 3 .62 ± 1 .01) mm ,LA T‐M APSE‐B ( 3 .95 ± 1 .04) mm ,LAT‐M APSE‐C ( 4 .45 ± 1 .05) mm ] were greater than those of the interventricular septum mitral annulus[ SEPT‐MAPSE‐A (3 .41 ± 0 .63)mm ,SEPT‐MAPSE‐B (3 .07 ± 0 .50) mm ,SEPT‐MAPSE‐C (2 .82 ± 0 .51) mm] . LAT‐M APSE‐C and SEPT‐M APSE‐A were the largest longitudinal excursions of mitral annulus . T he differences were statistically significant in points B and C ( P ＜0 .05) . T here was no significant difference in point A ( P ＞0 .05) . LA T‐M APSE‐C was less than FAM‐M APSE [ ( 6 .06 ± 1 .35 ) mm ] . T here was a significant difference between them ( P ＜0 .05 ) . Strong correlation was found between them ( r ＝0 .896 , P＜0 .05) . T here were no significant differences in the time to peak of interventricular septal mitral annulus [ SEPT‐T T P‐A ( 0 .210 ± 0 .008 ) s ,SEPT‐T T P‐B ( 0 .213 ± 0 .008 ) s ,SEPT‐T T P‐C ( 0 .210 ± 0 .005 ) s] in directions including points A ,B ,C ( P＞ 0 .05 ) . T here were no significant differences in time to peak of lateral mitral annulus [ LAT‐T T P‐A ( 0 .210 ± 0 .008 ) s , LAT‐T T P‐B ( 0 .213 ± 0 .006 ) s , LAT‐T T P‐C ( 0 .210 ± 0 .007) s] in directions inclucling points A ,B ,C ( P ＞0 .05) . Conclusions Longitudinal systolic motion of fetal left ventricular wall during mid‐late pregnancy has good synchronization . Longitudinal motion of fetal mitral annulus is a comprehensive movement of multiple directions and different degrees of displacement ,with the movement perpendicular to the annulus as the maximum displacement direction . T he displacement parameters of mitral annulus measured by ST I can reflect the left ventricular longitudinal systolic function and have clinical application value in evaluating the left ventricular longitudinal systolic function of fetuses .

Abstract:Objective To evaluate the application value of fetal heart echocardiography intelligent navigation (5D Heart) in the display of key diagnostic elements in five rapid screening fetal echocardiographic views.Methods Three hundred and eighty-four 3D volume datasets of 220 normal singleton fetuses in the second and third trimesters were acquired.After processing the datasets using 5D Heart,five rapid screening views were acquired,and 21 key diagnostic elements of the five cardiac diagnostic sections were graded.The 2 test was used to compare the difference in the key diagnostics elements of the five rapid screening views between 5D heart and the traditional technique.Pearson correlation was used to compare the detection rate of key diagnostic elements between 5D Heart technique and traditional two-dimensional echocardiography in normal fetuses and fetuses with congenital heart disease.The rank sum test was used to detect the consistency and reliability of the results.Results A total of 384 fetal volume datasets were obtained from 220 fetuses,of whom 317 heart volume datasets were available for analysis,with a success rate of 82.6％.There was no difference in the key diagnostic elements of the five rapid screening views between 5D heart and the traditional technique (x2=5.17,P=0.27).Compared with the abnormal fetal heart group (89.6％),the detection rate of diagnostic elements in the normal fetal heart group was 85.2％ (r=0.94,P=0.02).No significant differences were found when comparing the grading results of one observer in different periods of time,the grading results between two experienced doctors,or the grading results among the experienced doctor and one relatively inexperienced doctor (P ＞ 0.05).Conclusions Fetal cardiac ultrasound intelligent navigation technique has high reliability and repeatability in obtaining fetal rapid screening views and displaying fetal elemental diagnostic information.It has tremendous potential in standardized data collection and analysis of cardiac diagnostic sections in fetuses with congenial heart disease,as well as in training and teaching,remote medical consultation,and screening in primary hospitals.

Abstract:Objective:To evaluate the clinical value of sonography based volume computer aided display heart (SonoVCADheart) in the display of key diagnostic elements in basic fetal echocardiographic views.Methods:4D volume data based on fetal four-chamber view of 80 singleton fetuses (including 57 normal fetuses and 23 fetuses with heart abnormalities) were collected by using a volumetric probe from the Sir Run Run Shaw Hospital, Zhejiang University College of Medicine from January 8-22, 2019. Four to five volume datasets based on four-chamber view of the heart were rapidly acquired from each fetus. Three doctors (Doctor A: Engaged in fetal echocardiography diagnosis for more than 15 years, Doctor B and C: 6 months of regular fetal echocardiography training but lack of experience) with different fetal echocardiographic experiences performed off-line processing using SonoVCADheart combined tomographic ultrasound imaging (TUI) at different times to obtain eight standard echocardiographic diagnostic views, and to score the elementary contents of each echocardiographic view. The scores of the same doctor at different times, the scores of less experienced doctors and experienced doctor, and the scores between normal and abnormal fetuses, and the time required for analysis and diagnosis among all doctors were analyzed and compared, respectively. The diagnostic coincidence rates of SonoVCADheart for fetuses with cardiac malformations were also assessed.Results:A total of 279 volume datasets obtained from 57 normal fetuses, an average of 4.89/fetus, and 109 volume datasets obtained from 23 fetuses with cardiovascular abnormalities, an average of 4.74/fetus, and all volume datasets were used for SonoVCADheart analysis. The volume percentage of all the elements in the 8 diagnostic views with image quality≥2 points shown by SonoVCADheart in the normal fetuses was about 70.61%-74.91%, in the abnormal fetuses was about 53.21%-55.96%. There were no significant differences in the scores between the same doctor at different times, the scores between inexperienced doctors, and the scores among experienced and less experienced doctors(all P>0.05). There were significant differences in the scores between normal and abnormal fetuses except for all of the superior and inferior vena cava view and the aorticarch view of doctors A and C(all P<0.05). The time required for experienced doctor A and inexperienced doctors B and C to obtain 8 diagnostic views and to complete the diagnosis was statistically significant ( P<0.05). There was no significant difference in the time required for the inexperienced doctors B and C to obtain 8 diagnostic sections and to complete the diagnosis ( P>0.05). There was statistically significant difference in the diagnosis time required for doctor A using SonoVCADheart and two-dimensional echocardiography in fetuses with cardiovascular malformations ( P<0.05). However, there was no statistically significant difference in the time required in the normal fetus between SonoVCADheart and two-dimensional echocardiography ( P>0.05). The diagnostic coincidence rate of SonoVCADheart for fetuses with cardiac malformations is about 89.91%-90.83%. Conclusions:SonoVCADheart is a repeatable and stable novel fetal heart processing tool enabling displaying eight standard diagnostic sections of the fetal heart, and has potential clinical application value in the standardization of image acquisition and sequence display.
Preliminary Study on the Evaluation of Mitral Annulus Displacement in Normal Fetuses by Automated Cardiac Motion Quantitation

Abstract:OBJECTIVES:The aim of this study was to assess the characteristics of mitral annular plane systolic excursion (MAPSE) in different longitudinal directions in normal fetuses using a new method, automatic cardiac motion quantification (aCMQ). METHODS:A cross-sectional study was conducted in 164 fetuses with structurally normal hearts. The time-displacement curves of the septal mitral annulus (SMA) in three directions, including point A, B and C (MAPSE-SMA-A, MAPSE-SMA-B, MAPSE-SMA-C), were recorded by aCMQ. The time to peak (TTP) in three directions, including point A, B and C (TTP-SMA-A, TTP-SMA-B, TTP-SMA-C) were recorded. In the same way, various parameters of the lateral mitral annulus (LMA) were obtained including MAPSE-LMA-A, MAPSE-LMA-B, MAPSE-LMA-C, TTP-LMA-A, TTP-LMA-B and TTP-LMA-C. Free angle M-mode echocardiography (FAM) was used to obtain MAPSE of LMA (FAM-MAPSE). Finally, all the data were analyzed statistically. RESULTS:MAPSE was positively correlated with gestational age, and the difference between the second- and third-trimester groups was statistically significant. MAPSE-LMA in point B and C were greater than those of SMA. MAPSE-LMA-C and MAPSE-SMA-A were the largest in three directions.The difference between point B and C were statistically significant (p < .05), with no significant difference at point A (p > .05). There was no significant difference found in all TTP (all p > .05). The MAPSE-LMA-C was less than the FAM-MAPSE, and the differences were found significantly (p < .05), but there was better correlation (p < .05). CONCLUSIONS:The longitudinal movement of the fetal mitral annulus is comprehensive, with multiple directions and different displacements. Perpendicular to the mitral annulus is the maximum displacement. It is positively related to the gestational age. From the second trimester, the longitudinal contraction of the left ventricle wall has good synchronization. It possesses clinical value in selecting different methods and parameters during evaluating left ventricular function.

Abstract:Fetal two‐dimensional speckle tracking echocardiography (2D‐STE) is a novel technique that provides information on fetal heart function by measuring global longitudinal strain (GLS) and global longitudinal strain rate (GLSR). These features assess the longitudinal deformity of the fetal cardiac wall. 2D‐STE is shown to be of prognostic value in children and adults with congenital heart disease (CHD). Therefore, its importance in fetal life should also be considered. This systematic review and meta‐analysis provides an overview of the literature on 2D‐STE (GLS/GLSR) in fetuses with CHD, focusing on the left and right ventricles (LV/RV). Findings indicated that LV‐GLS was significantly lower in fetuses with coarctation of the aorta (CoA) and Tetralogy of Fallot (ToF) compared to controls. Conversely, fetuses with a single left ventricle exhibited higher LV‐GLS. RV‐GLS was significantly lower in fetuses with hypoplastic left heart syndrome (HLHS) and ToF compared to controls. LV‐GLSR was significantly lower in fetuses with CoA. Overall, considerable heterogeneity was observed, possibly due to differences in study design. More prospective longitudinal studies on 2D‐STE in fetuses with CHD, considering heterogeneity parameters, could offer better insights into this promising technique.

Abstract:Background: Cardiovascular magnetic resonance (CMR) is an emerging imaging modality for assessing the anatomy and function of the fetal heart in congenital heart disease (CHD). This study aimed to evaluate myocardial strain using fetal CMR feature tracking (FT) in different subtypes of CHD. Methods: Fetal CMR FT analysis was retrospectively performed on four-chamber cine images acquired with Doppler ultrasound gating at 3T. Left ventricular (LV) global longitudinal strain (GLS), LV global radial strain (GRS), LV global longitudinal systolic strain rate, and right ventricular (RV) GLS were quantified using dedicated software optimized for fetal strain analysis. Analysis was performed in normal fetuses and different CHD subtypes (d-transposition of the great arteries [dTGA], hypoplastic left heart syndrome [HLHS], coarctation of the aorta [CoA], tetralogy of Fallot [TOF], RV-dominant atrioventricular septal defect [AVSD], and critical pulmonary stenosis or atresia [PS/PA]). Analysis of variance with Tukey post-hoc test was used for group comparisons. Results: A total of 60 fetuses were analyzed (8/60 (13%) without CHD, 52/60 (87%) with CHD). Myocardial strain was successfully assessed in 113/120 ventricles (94%). Compared to controls, LV GLS was significantly reduced in fetuses with HLHS (-18.6±2.7% vs -6.2±5.6%; p<0.001) and RV-dominant AVSD (-18.6±2.7% vs -7.7±5.0%; p = 0.003) and higher in fetuses with CoA (-18.6±2.7% vs -25.0±4.3%; p = 0.038). LV GRS was significantly reduced in fetuses with HLHS (25.7±7.5% vs 11.4±9.7%; p = 0.024). Compared to controls, RV GRS was significantly reduced in fetuses with PS/PA (-16.1±2.8% vs -8.3±4.2%; p = 0.007). Across all strain parameters, no significant differences were present between controls and fetuses diagnosed with dTGA and TOF. Conclusion: Fetal myocardial strain assessment with CMR FT in CHD is feasible. Distinct differences are present between various types of CHD, suggesting potential implications for clinical decision-making and prognostication in fetal CHD.

Abstract:ObjectivesTwo-dimensional speckle tracking echocardiography (2D-STE) is a relatively new, promising technique to assess cardiac function in the fetus. Fetal cardiac function can be used in the evaluation of non-cardiac disease states in pregnancy. However, reliable reference values for deformation parameters measured with speckle tracking in the fetus are needed before it can be introduced into clinical practice.This study aimed to obtain reference values for fetal global longitudinal strain (GLS) and strain rate (GLSR) measured with 2D-STE and to compare the right ventricle values to those of the left ventricle. MethodsIn this longitudinal prospective cohort study, echocardiography was performed four weekly from inclusion (18-21 weeks) until delivery to obtain four-chamber loops of the fetal heart. Left and right ventricular GLS and GLSR were measured with 2D-STE at each examination. Using Bayesian mixed-effects models, reference values with a lower and an upper 5% prediction limit during pregnancy were calculated. Right and left ventricular GLS values during pregnancy were compared with a Wilcoxon signed-rank test. ResultsA total of 592 and 566 assessments of GLS and GLSR for the left and right ventricle respectively, were included. Reference values were obtained for both ventricles per gestational age week for the uncomplicated pregnancy in non-anomalous, appropriately grown fetuses. GLS and GLSR values in both ventricles increased (i.e. mathematical less negative) significantly during pregnancy. Right ventricular GLS and GLSR values were significantly higher (i.e. less negative) than the left ventricular values at every gestational age measured. ConclusionsReference values were obtained for fetal GLS and GLSR. GLS and GLSR values increased significantly for both ventricles from the second trimester until delivery, with significantly higher values for the right ventricle compared to the left ventricle. Future studies are needed to assess whether the obtained reference values are helpful in clinical practice in the assessment of pregnancy complications, such as fetal growth restriction or cardiac anomalies.This article is protected by copyright. Abstract:PURPOSE OF REVIEW: One of the difficulties of conventional two-dimensional cardiac imaging is the inability to examine fetal cardiac anatomy from multiple angle planes. Three-dimensional and four-dimensional ultrasound allows the fetal examiner to more accurately accomplish this task. Currently, multiple disciplines may be involved in the examination of the fetal heart (pediatric cardiologists, obstetricians, maternal-fetal medicine specialists, and radiologists). The three-dimensional and four-dimensional imaging equipment used by these specialty physicians varies greatly. The purpose of this communication is to review techniques using three-dimensional and four-dimensional imaging that the pediatric cardiologist may not be exposed to in the clinical environment, however, in consulting with colleagues needs to have an understanding of these imaging modalities.RECENT FINDINGS: The reconstruction of cardiac structures using this technology allows the examiner to view cardiac anatomy in a manner that was limited by previous two-dimensional imaging. Volume datasets are obtained in the three-dimensional static mode (no cardiac motion) or using four-dimensional - the three-dimensional heart is observed contracting during one or multiple cardiac cycles. Therefore, the fourth dimension is time. Using either three-dimensional or four-dimensional technology datasets are acquired, followed by image reconstruction. The image reconstruction enables the examiner to evaluate a two-dimensional image using multiple views, evaluate intracardiac anatomy at different depth planes, and recreate casts of blood flow of the chambers and great vessels.SUMMARY: This new technology has enhanced the ability of the examiner to identify normal and complex fetal heart anatomy during the early second to the late third trimesters of pregnancy.
