Abstract:Objective. The purpose of this study was to assess the effects of glucose fluctuation targeted intervention on neurologic function, independent living skills, and quality of life in type 2 diabetes patients following the first episode of cerebral infarction (CI). Methods. This was a randomized control trial. Following confirmed cerebral infarction, 75 patients with type 2 diabetes were randomized into 2 groups: control group (n=37) with usual care, focused on hemoglobin A1c (HbA1c) control, targeting A1c<7%, and intervention group (n=38), targeting both A1c<7%and daily glycemic fluctuation (largest amplitude of glycemic excursions (LAGE<80 mg/dL)). Results. After 6 months, data from 63 patients were analyzed (30 in the control group, 33 in the intervention group). There was no difference (P>0.05) in the reduction of A1c between the 2 groups, but the reductions of LAGE (P=0.030), 1,5-anhydroglucitol (1,5-AG) (P=0.023), 2-hour postprandial blood glucose (2hPG) (P=0.041), and low-density lipoprotein cholesterol (LDL-c) (P=0.046) were significantly different. The National Institutes of Health Stroke Scale (NIHSS) score of the intervention group was better than the control group (2.35±0.81 vs. 3.50±2.24, P=0.047). In terms of quality of life, there was no significant between-group difference in total Stroke Impact Scale (SIS) score, but in the intervention group, scores in the strength, hand function, and participation dimensions were higher than those in the control group (P=0.041, P=0.049, and P=0.048, respectively). Conclusion. Glucose fluctuation targeted intervention can improve nerve function for patients with T2DM following the first CI episode. This trial is registered with NCT03932084.

Background:Acute hyperglycemia may increase the rate of death or disability among acute ischemic stroke patients undergoing endovascular procedures.Objective:To explore the association between serum glucose fluctuation on post treatment symptomatic intracranial hemorrhage (sICH) and mortality.Methods:We analyzed consecutive acute ischemic stroke patients who underwent endovascular treatment over 30 months at a comprehensive stroke center. Serum glucose levels within the initial 24 hours were used to calculate the mean and standard deviation (SD). Patients were grouped by the standard deviation [SD] of the mean value of serum glucose, whether above or below 25%. Outcomes included sICH, and inpatient mortality, adjusted for confounders which included age, gender, baseline NIHSS, perfusion grade (TICI) intravenous thrombolysis, history of diabetes and HBA1c.Results:A total of 224 patients (mean age was 68.7 years (±14.3); 44.2% were women) underwent endovascular treatment. The proportion of sICH (16.7% vs. 3.28%; p=0.0006) and inpatient mortality (18.6% vs. 7.4%; p=0.0112) were higher in patients with SD exceeding 25. After adjusting for potential confounders, serum glucose SD above 25 was significantly associated with increased rate of sICH (odds ratio [OR] 5.2; 95% confidence interval [CI] 1.5-18.5; p=0.0112) and inpatient mortality (OR 3.5; 95% CI 1.3-9.8; p=0.0154).Conclusion:Serum glucose fluctuations within first 24 hours maybe be therapeutic target due to its association with increased risks of sICH and inpatient mortality following endovascular treatment in acute ischemic stroke.

Abstract:Objective To investigate the association between glucose parameters and outcome measures in acute ischemic stroke (AIS) treated with intravenous thrombolysis (IVT).Methods Two hundred and two eligible cases were enrolled in the cohort study.Blood glucose (BG),fasting BG and glycosylated hemoglobin A1c (HbA1c) determined on admission were taken as continuous variables,therapeutic outcome measures [e.g.,nodified Rankin Scale (mRS) ≤2 at discharge,early neurological improvement (ENI) or deterioration (END),and recanalization status] and safety outcome measures [symptomatic intracranial hemorrhage (SICH) and all intracranial hemorrhage (ICH)] were taken as endpoint indicators.Results Of the recruited cases,the median age was 61 years,25.2％ female,median National Institute of Health Stoke Scale (NIHSS) 9 scores and onset to needle (OTN) time 230 min.The median fasting BGs were much lower in subgroups with favorable outcome (mRS ≤ 2) or recanalization than those in the related reference subgroups,while the comparisons according to ICH and SICH didnt reach significance.Significantly higher admission BG was only found in the comparison according to SICH and the HbAlc levels were similar in all comparisons.Adjusted for age,sex,OTN and baseline NIHSS,admission BG [odds ratio (OR),1.24;95％ confidence interval (CI):1.03-1.50] was the only independent predictor of SICH,while fasting BG was negatively associated with all therapeutic outcome measures,with an OR of 0.72 (0.58-0.90) for mRS ≤ 2 at discharge,an OR of 0.69 (0.55-0.87) for ENI,and an OR of 0.80 (0.66-0.98) for recanalization.Elevated fasting BG was only significantly associated with Non-SICH related END (OR:1.42,95％ CI:1.04-1.95) after adjustment for explanatory variables.Conclusion Acute glucose parameters could be independent predictors of outcomes in AIS after IVT and elevated fasting BG negatively affected the functional outcome probably through the mechanism of an impaired response of IVT.
Hyperglycaemia in Acute Lacunar Stroke: A Chinese Hospital-Based Study

Abstract:Admission hyperglycaemia is very common in acute ischaemic stroke (IS) patients and is associated with worse outcome in non-lacunar infarction, but its association with lacunar infarction remains controversial. We investigated the association between hyperglycaemia and clinical outcome in patients with acute lacunar stroke in a hospitalised Chinese population. Consecutive patients with acute IS were enrolled and classified into lacunar and non-lacunar stroke groups based on clinical criteria according to the Oxfordshire Community Stroke Project (OCSP) and findings on brain computed tomography (CT) or magnetic resonance imaging (MRI) scan. Hyperglycaemia was defined as blood glucose > 6.1 mmol/L. Univariate and multivariate analyses were used to examine the association between hyperglycaemia and outcomes in diabetic and non-diabetic groups. The main outcome measure was a poor outcome [defined as modified Rankin Scale (mRS) score ≥ 3] at 12 months after stroke. Of the 2020 acute ISs, 689 (34.1%) were acute lacunar strokes, of which 159 (23%) were diabetic and the other 530 (77%) were non-diabetic. In non-lacunar stroke, admission hyperglycaemia was independently associated with increased risk of poor outcome at 1 year (odds ratio (OR) = 1.782; 95% confidence interval (CI) = 1.340–2.370, p = 0.0005). In lacunar stroke, hyperglycaemia was not associated with functional outcome (OR = 1.337; 95% CI = 0.939–1.093, p = 0.086) irrespective of the diabetic status.
Association between short-term glucose fluctuations and cognition in patients with acute ischemic stroke complicated by type 2 diabetes mellitus

Abstract:Objectives: Glucose fluctuations are more harmful than persistent hyperglycemia for chronic complications of diabetes. However, the relationship between cognition and glucose fluctuations in patients with acute ischemic stroke (AIS) complicated by type 2 diabetes mellitus (T2DM) remains unclear. We aimed to evaluate the association between short-term glucose fluctuations and cognition in patients with AIS complicated by T2DM. Design: A cohort study with a 2-year follow-up. Setting and participants: We included 554 patients with mild AIS (mean age: 62 years; 170 females and 384 males). Measurements: Glucose variability (GV) was evaluated using glycated hemoglobin (HbA1c), stress hyperglycemia (SHR), standard deviation of blood glucose (SDBG), mean postprandial blood glucose (MPBG), mean amplitude of glycemic excursion (MAGE), and time in range (TIR). We evaluated the relationship between GV, fasting blood glucose (FBG) and cognition during the acute phase using linear regression analysis. We evaluated the relationship between GV, FBG and the occurrence of post-stroke cognitive impairment (PSCI) using a logistic regression model. Mediation analyses were fitted to explore whether the relationships of HbA1c with cognition were mediated by cerebral small vessel disease (CSVD). Results: A clear pattern of age-related GV was observed. Higher SHR in middle-aged participants; higher HbA1c, and lower TIR in older participants; and higher MAGE, MPBG, and SDBG across a broad age range (50-80 years) were associated with cognitive impairment in the acute phase of AIS. Higher SHR and SDBG together with lower TIR in middle-aged participants, higher HbA1c in older participants, and higher FBG, MPBG, and MAGE across a broad age range (50-80 years) were associated with the occurrence of PSCI. The association between HbA1c and cognition was partially mediated (proportion: 7-16%) by CSVD. Conclusions: Short-term glucose fluctuations are associated with cognition and a higher risk of PSCI in patients with AIS complicated by T2DM. CSVD might play an important role in the relationship between short-term glucose fluctuations and cognition.

Abstract:An In/sub 0.52/AlAs/In/sub 0.53/GaAs HEMT with 5 nm InAs-inserted-channel into the In/sub 0.53/GaAs channel was fabricated using 0.05 /spl mu/m conventional T-shaped gate technology. The measured maximum transconductance (G/sub m,max/) is above 1 S/mm even at a lower drain voltage of 0.7 V and current gain cutoff frequency (f/sub T/) is 409 GHz. This excellent performance is attributed to the combination of the suppression of short-channel effect and the superior transport in 5 nm InAs-inserted-channel design concept. Higher f/sub T/ above 500 GHz is to be expected using the InAs-inserted-channel scheme, if reducing L/sub g/ down to sub-0.05/spl mu/m range.

Abstract:Background Hyperglycemia has been associated with poor outcomes in acute ischemic stroke patients undergoing endovascular treatment. We analyzed the effect of intensive glucose control on death and disability rates in patients with acute ischemic stroke undergoing endovascular treatment. Methods We analyzed the effect of intensive (serum glucose <110 mg/dL) glucose treatment (compared with standard treatment, serum glucose <180 mg/dL) in patients who received endovascular treatment in the Stroke Hyperglycemia Insulin Network Effort (SHINE) trial. We further analyzed the effect of area under the curve (AUC) of serum glucose, proportion of the time blood glucose was <140 mg/dL, and glucose variability defined as the glucose range during 72 hours. The primary outcomes were neurological deterioration within 72 hours and outcome at 90 days. Results A total of 146 patients (mean age 68.1±13.9 years, 50.7% men) underwent endovascular treatment for acute ischemic stroke; 72 and 74 patients were randomized to intensive and standard treatments, respectively. The rates of death (20.3% and 22.2%), favorable 90-day primary outcome (17.6% and 19.4%), and serious adverse events (41.9% and 56.98%) were similar between the two groups. The AUC of serum glucose was not associated with death within 90 days (OR 1, 95% CI 1 to 1) or favorable outcome at 90 days (OR 1, 95% CI 1 to 1). Glucose variability was not associated with death or favorable outcome at 90 days. Conclusion We did not identify any beneficial effect of intensive glucose reduction on rates of death or favorable outcomes at 90 days among acute ischemic stroke patients undergoing endovascular treatment.

Abstract:Opinion statementThere is considerable clinical evidence that hyperglycemia at the onset of acute ischemic stroke may negatively impact not only acute morbidity but also brain recovery. Establishing hyperglycemia treatment protocols is challenging, given the variation among patients and acute stroke care settings. Relatively few randomized trials have examined glycemic control protocols in this population, and there is not yet any clear evidence that “correcting” hyperglycemia in patients with acute stroke leads to better functional outcomes. Intensification of glucose regimens, using lower glucose targets, leads to more hypoglycemic events, but the immediate and long-term impact of these events on the acutely ischemic brain is unknown. It is reasonable to treat patients with acute ischemic stroke according to the American Diabetes Association inpatient glycemic control guidelines, initiating therapy to achieve glucose targets of 140 to 180 mg/dL if fasting glucose is greater than 140 mg/dL or random glucose is consistently higher than 180 mg/dL. Lower glucose targets (<140 mg/dL) may be appropriate for patients with well-controlled diabetes and those with stress hyperglycemia who were not known to be diabetic before admission, but glucose levels less than 80 mg/dL should be avoided. Patients who present with extreme or persistent hyperglycemia, are critically ill, or who are treated with thrombolytic therapy should be started on an established and standardized intravenous insulin protocol to improve blood glucose control for at least the first 24 to 48 h of hospitalization. They should then be transitioned to a subcutaneous insulin regimen that includes basal long-acting insulin along with correction rapid-acting insulin for glucose that is out of range. Prandial (meal) insulin should be added for patients who are eating; this would preferably be a rapid-acting insulin analogue that can be administered immediately before or after the meal. Caution and close glucose monitoring are necessary, especially for patients prone to hypoglycemia, such as those with type 1 diabetes mellitus or hepatic or renal impairment, or those on complicated feeding regimens.

Abstract:Abstract Background Hyperglycemia was associated with delayed cerebral infraction (DCI) and poor outcomes in aneurysmal subarachnoid hemorrhage (aSAH) patients. However, the influence of glucose fluctuation on DCI and mortality in aSAH patients was still unclear. Objective This study aimed to clarify the relationship of glucose fluctuation and DCI occurrence or 30 days mortality in aSAH patients who were admitted in Neurocritical Unit (NCU). Methods A total of 341 patients with aSAH were retrospectively recruited and studied. Continuous 14-day fasting blood glucose was collected and divided into four groups: stable, unstable, well-controlled, bad-controlled. Risk factors that were associated with glucose fluctuation, DCI occurrence and 30 days mortality was analyzed.Results Many risk factors, such as Age (59.09±11.95), Diabetes history, SBP (162.91±20.85), BUN (5.50±1.80), HA1c (7.12±1.65), BGV (0.32±0.11), Hunt-Hess Score (2.59±0.97), GCS Score (10.68±4.23), Hydrocephalus in Forth ventricle, Third ventricle and Lateral ventricles, are associated with glucose fluctuation. Bad controlled group has the highest mortality (15.91%), then unstable (13.04%), well-controlled (6.45%), and stable group (1.42%). The unstable group has the highest incidence of DCI (39.13%), then bad-controlled (29.55%), the stable (17.92%) and well-controlled (17.74%). Conclusions Unstable and bad-controlled glucose during NCU admission was associated with DCI and 30 days mortality of aSAH patients.
[Hyperglycemia: a predictor of poor prognosis in acute stroke]

Abstract:Hyperglycemia is common and involves up to 50% of the acute stroke patients. Several clinical studies have revealed that hyperglycemia is associated with a poor outcome in terms of mortality and neurological recovery. The results obtained from experimental studies have shown that hyperglycemia exacerbates the ischemic lesions and is associated with an increase of the edema and size of the infarct, as well as a decrease in the cerebral blood flow. In contradiction with animal models no increase in hemorrhagic transformation of the brain infarct after reperfusion is observed among patients with hyperglycemia, unless they are treated with rt-PA. Elevated plasma glucose levels (glycemia > 8 mmol/L) predict poor prognosis, irrespective of age, severity, or stroke sub-type. The benefit to treat a glycemia >=11 mmol/l has been established in terms of mortality for patients with myocardial infarction. Although the clinical benefit of plasma glucose lowering therapy remains unknown regarding acute stroke, based on current evidence, we recommend to treat moderate to mild hyperglycemia with glucose potassium insulin infusions.

Abstract:The development of a multicellular organism from a single zygote depends on the differentiation of progenitor cells to specialized cell types. The differentiation of these cell types is associated with changes in gene expression and the underlying chromatin landscape. To understand how these processes are regulated, it is desirable to understand how the chromatin features that constitute the epigenome differ between cell types at any given time during development. INTACT, a method for the cell type‐specific purification of nuclei that can be used for the isolation of both RNA and chromatin, has emerged as a powerful tool to simultaneously study gene expression and chromatin profiles specifically in cell types of interest. Association of Admission Glucose and Outcomes After Endovascular Treatment for Acute Stroke: Data From the ANGEL-ACT Registry

Abstract:Background: The association between admission glucose levels and clinical outcomes after stroke has not been effectively elucidated. This study assessed the association among admission glucose levels, admission hyperglycemia, diabetes mellitus, and 90-day neurological outcomes in patients with acute ischemic stroke undergoing endovascular therapy. Methods: The ANGEL-ACT registry enrolled adults with acute ischemic stroke undergoing endovascular therapy between 2017 and 2019 in China and patients with available admission glucose data were included. Restricted cubic spline regression was used to determine the knots of blood glucose levels. Binary or ordinal logistic regression models were used to examine the impact of different admission glucose levels on neurological outcomes and 90-day mortality. Results: In total, 1684 participants with available admission glucose concentrations were evaluated. The admission glucose level was divided into 4 levels according to the restricted cubic spline curves: level 1 (<5.3 mmol/L), level 2 (5.3 to 7.0 mmol/L), level 3 (7.0 to 11.6 mmol/L), and level 4 (≥11.6 mmol/L). Level 4 admission glucose was associated with a decreased incidence of a modified Rankin scale score of 0 to 2 (hazard ratio, 0.59; 95% CI, 0.40-0.87) and an increased risk of mortality (hazard ratio, 1.74; 95% CI, 1.06-2.85). Levels 3 and 4, hyperglycemia, and diabetes mellitus independently predicted symptomatic intracranial hemorrhage (sICH). Admission glucose levels showed J-shaped relationships with sICH. Conclusions: Higher admission glucose levels (≥11.6 mmol/L) were associated with a decreased likelihood of a modified Rankin scale score of 0 to 2 and an increased risk of mortality and sICH.

Abstract:OBJECTIVE To inventory the diagnostic methods used in patients with clinically occult, radiologically suspect breast lesions. DESIGN Enquiry. METHOD The departments of radiology of all Dutch hospitals were sent a list in January 2000 containing questions concerning the number of thread localizations in 1999 and the use of cytological or histological needle diagnostics with image monitoring prior to surgical intervention in clinically occult, radiologically suspect breast lesions. Of the 120 questionnaires mailed, 74 (62%) were completed and returned by clinics throughout the country. RESULTS Fifty-one of the 74 hospitals (69%) had prior to operation carried out histological or cytological examinations and in these 51 hospitals this was done in 1743 of the 2857 lesions (61%): fine-needle aspiration cytology was performed in 1046 (/1743 = 60%; /4140 lesions in all 74 hospitals = 25%) and/or histological needle biopsy in 784 (45%; /4140 = 19%). CONCLUSION In less than half of all non-palpable breast abnormalities non-surgical methods of diagnosis are used, histological needle biopsy less often than fine needle aspiration cytology.

Conclusion: We did not identify any beneficial effect of intensive glucose reduction on rates of death or favorable outcomes at 90 days among acute ischemic stroke patients undergoing endovascular treatment. We also did not observe any relationship between various glycemic parameters and death or favorable outcomes at 90 days.

Abstract:Introduction: Hyperglycemia in acute ischemic stroke (AIS) is frequent and associated with worse outcome. Yet, strict glycemic control in AIS patients has failed to yield beneficial outcome. So far, the underlying pathophysiological mechanisms of admission hyperglycemia in AIS have remained not fully understood. We aimed to evaluate the yet equivocal association of hyperglycemia with computed tomographic perfusion (CTP) deficit volumes. Patients and methods: We included 832 consecutive AIS and transient ischemic attack (TIA) patients who underwent CTP as a part of screening for recanalization treatment (stroke code) between 3/2018 and 10/2020, from the prospective cohort of Helsinki Stroke Quality Registry. Associations of admission glucose level (AGL) and CTP deficit volumes, namely ischemic core, defined as relative cerebral blood flow <30%, and hypoperfusion lesions Time-to-maximum (Tmax) >6 s and Tmax >10s, as determined with RAPID® software, were analyzed with a linear regression model adjusted for age, sex, C-reactive protein, and time from symptom onset to imaging. Results: AGL median was 6.8 mmol/L (interquartile range 5.9-8.0 mmol/L), and 222 (27%) patients were hyperglycemic (glucose >7.8 mmol/L) on admission. In non-diabetic patients (643 [77%]), AGL was significantly associated with volume of Tmax. >6 s (regression coefficient [RC] 4.8, 95% confidence interval [CI] 0.49-9.1), of Tmax >10s (RC 4.6, 95% CI 1.2-8.1), and of ischemic core (RC 2.6, 95% CI 0.64-4.6). No significant associations were shown in diabetic patients. Conclusion: Admission hyperglycemia appears to be associated with both larger volume of hypoperfusion lesions and of ischemic core in non-diabetic stroke code patients with AIS and TIA.

Abstract:Background: The role of tight glycemic control in the management of acute ischemic stroke remains uncertain. Objective: Our goal is to evaluate the effects of tight glucose control with insulin therapy after acute ischemic stroke. Methods: We searched PubMed, CENTRAL, and EMBASE for randomized controlled trials (RCTs) that evaluated the effects of tight glycemic control (70 - 135 mg/dL) in acute ischemic stroke. Analysis was performed using fixed- and random-effects models. Outcomes were death, independence and modified Rankin Scale (mRS) score at ≥90 days follow-up, and symptomatic or severe hypoglycemia during treatment. Results: Twelve RCTs including 2,734 patients were included. When compared to conventional therapy or placebo, tight glycemic control was associated with similar rates of mortality at ≥90 days follow-up (pooled odds ratio [pOR], 0.99 [95% CI, 0.79 - 1.22], I 2 0%), independence at ≥90 days follow-up (pOR, 0.95 [0.79 - 1.14], I 2 0%) and mRS scores at ≥90 days follow-up (standardized mean difference, 0.014 [-0.15 - 0.17], I 2 0%). In contrast, tight glycemic control was associated with increased rates of symptomatic or severe hypoglycemia during treatment (pOR 5.2 [1.87 - 14.42], I 2 20%). Conclusion: Tight glucose control after acute ischemic stroke is not associated with improvements in mortality, independence or mRS score and leads to higher rates of symptomatic or severe hypoglycemia.

Abstract:OBJECTIVE:To assess the impact of diabetes on neurological outcome and recanalization in acute ischemic stroke (AIS) after intravenous thrombolysis (IVT). METHODS:Clinical data of 419 consecutive patients with AIS who received IVT between June 2009 and April 2014. Based on the medical history and new diagnosis, the patients were divided into groups with and without diabetes. Neurological outcomes at 24 h, 7 days and 3 months after IVT were evaluated. Favorable outcome was defined as National Institutes of Health Stroke Scale (NIHSS) score decrease ≥4 points from baseline or 0 at 24 h, NIHSS decrease ≥8 points or 0 at day 7, or modified Rankin scale ≤1 at 3 months after IVT. Recanalization on non-invasive imaging was evaluated in patients with large vessel occlusion (LVO) according to thrombolysis in myocardial infarction grades. RESULTS:Among 419 patients, 98 (23.4%) had diabetes. Multivariable analyses showed that comorbidity of diabetes was an independent predictor of unfavorable outcome at 24 h (OR=0.534, 95% CI 0.316 to 0.903, p=0.019), at day 7 (OR=0.382, 95% CI 0.220 to 0.665, p=0.001), and at 3 months (OR=0.464, 95% CI 0.266 to 0.808, p=0.007). In patients with LVO, diabetes was an independent predictor of incomplete recanalization 24 h after IVT (OR=0.268, 95% CI 0.075 to 0.955, p=0.042). CONCLUSIONS:Diabetic patients with AIS had unfavorable neurological outcome, potentially linked to incomplete recanalization after IVT.

Objective:This study aimed to evaluate the correlation between the stress hyperglycemia and clinical outcomes after large vessel occlusion stroke (LVO) at 90 days.Methods:The RESCUE BT trial was a multicenter, randomized, double-blind, placebo-controlled clinical trial, consisting of 948 patients from 55 centers in China. In this subgroup study, 542 consecutive stroke patients with glucose or glycated hemoglobin values were included. Stress hyperglycemia was evaluated both as a tri-categorical variable (≤1.07 vs 1.08-1.29 vs ≥1.30) and a continuous variable. The primary outcome was favorable functional outcome (modified Rankin Scale [mRS] ≤ 2) at 90 days. The secondary outcome contained excellent functional outcome (modified Rankin Scale [mRS] ≤ 1) and safety outcomes such as 90-day mortality and intracranial hemorrhage.Results:542 patients were included. Compared to patients with the lowest tertiles of stress hyperglycemia, calculated as glucose/GA ratio, the highest tertiles of stress hyperglycemia was obviously associated with a higher rate of poor functional outcome at 90-day (modified Rankin Scale [mRS] score 3-6) after adjusted for potential covariates (adjusted hazard ratio, 0.44; 95% confidence interval, 0.28-0.69, P < 0.001). This result was also consistent in the excellent clinical outcome of 90-day mRS score 2-6 (adjusted hazard ratio, 0.48; 95% confidence interval, 0.29-0.79, P =0.004).Conclusion:In patients with acute ischemic stroke, lower stress hyperglycemia, as measured by glucose/GA ratio, was more likely to have favorable outcome at 90-day.Trial registrationChinese Clinical Trial Registry Identifier: ChiCTR-IOR-17014167, December 27, 2017.
