Abstract:Undernutrition in children with cancer is associated with complications during cancer therapy. The study objective was to determine the association between specific anthropometric parameters and short-term chemotherapy-related complications and mortality. This was a hospital-based, prospective cohort study of children, age ≤12 years, with a new cancer diagnosis at the Paediatric Oncology Unit, Korle Bu Teaching Hospital, Ghana. Socio-demographic information, cancer characteristics and anthropometric measurements were obtained at enrolment. Participants were followed up for twelve weeks from commencement of chemotherapy and selected treatment-related complications such as anaemia and thrombocytopenia requiring transfusions, prolonged neutropenia resulting in treatment delays, febrile neutropenia, mucositis and death were recorded. A total of 133 participants were recruited with a median age of 4.5 years. Eighty-one (60.9%) were diagnosed with solid tumours, 31 (23.3%) had leukaemias and 21 (15.8%) had lymphomas. Of the anthropometric parameters assessed, only arm anthropometry using upper arm muscle area (UAMA) and mid-upper arm circumference (MUAC) were associated with complications. Participants with wasting were more likely to develop anaemia and mucositis. However, the incidence of prolonged neutropenia was significantly higher among participants with average UAMA (p = 0.043) and low average UAMA (p = 0.049) compared to those with low UAMA. Risk of neutropenia was also significantly less among those with wasting by MUAC compared to those well-nourished (p = 0.045). Twenty-three participants (17.3%) died with a greater proportion (11/44; 25%) occurring in those who were wasted using MUAC. These findings underscore the need for nutritional surveillance at diagnosis and during chemotherapy, particularly where co-morbid disease is prevalent.

Abstract:e23280 Background: Malnutrition is a problem affecting tumor patients greatly. To investigate the nutritional status of hospitalized patients with common malignant tumor in a cancer hospital of Southwest China. Methods: From April 2017 to May 2021, we enrolled 891 patients with cancer hospitalized for treatment in Chongqing university Cancer Hospital. These patients were diagnosed with one of the following 16 different types of malignant tumors: lung cancer, gastric cancer, liver cancer, colorectal cancer, breast cancer, esophageal cancer, cervical cancer, endometrial cancer, nasopharyngeal carcinoma, malignant lymphoma, leukemia, pancreatic cancer, ovarian cancer, prostate cancer, bladder cancer and brain tumor. Patient-generated subjective global assessment (PG-SGA), anthropometric measurements, and laboratory examination were used to evaluate the nutritional risk or nutritional status.Cancer pain status were assessed with the Numerical Rating Scale (NRS). We also investigated the nutritional therapy of these cancer patients. Results: According to the PG-SGA score, 48.7% (434/891) of the cancer patients were severe malnutrition (PG-SGA≥9), 31.2% (312/891) were moderate malnutrition (8≥PG-SGA≥4), 14.7% (131/891) were mild malnutrition (PG-SGA 2~3), and only 5.4% (48/891) patients were no malnutrition (PG-SGA 0~1) . The rate of malnutrition for gastrointestinal cancer patients is higher than Nongastrointestinal cancer patients (67.3% vs. 44.6%, χ2 = 31.48, P < 0.001). Multiple linear regression analysis, PG-SGA scores and body mass index (P < 0.001), serum total protein(P < 0.001), hemoglobin serum(P < 0.001), albumin (P < 0.001), prealbumin ( P< 0.001), calf circumference (left side, P = 0.001) were correlated. Age(≥65 years), albumin( < 40g/L), prealbumin ( < 150mg/L) and cancer pain (NRS≥4) are the risk factors of severe malnutrition. However, only 26.8% (200/746) of all the moderately and severely malnourished patients received nutritional therapy. Conclusions: 94.6% of the common malignant tumor patients enrolled in the present study were malnutrition. PG-SGA is an effective tool to assess malnutrition in cancer patients, it is recommended to conduct routine assessment of cancer patients at the beginning of admission. Nutritional therapy of malignant tumor patients with malnutrition is very low. Suggestions for patients with malignant tumor after admission nutritional risk screening, and comprehensive nutritional evaluation, including PG-SGA score, and to give the right nutritional therapy.

Abstract:: The consequences of malnutrition in Cancer patient, include an increased risk of complications, a lower quality of life, reduced survival and higher health-care costs. The Scored Patient-Generated Subjective Global Assessment (PG-SGA) is used internationally as the reference method for screening, monitoring and triaging for interventions in patients with cancer The aim of this study was to evaluate the use of the scored PG-SGA as a nutrition assessment tool in hospital patients with cancer A prospective 15 week study assessing the nutritional status and QoL(Quality of life) of ambulatory patients receiving chemotherapy, radiation therapy, medications/supplements therapy were analyzed and used to predict the magnitude of QoL in a private hospital in Bengaluru. The total sample number was around 50.Statistical Analyses were performed, using SPSS software. : The result based on total PG-SGA Score, shows, 14% required no intervention, 32% required patient and family education, 24% required intervention by dietitian and doctor and 20% required critical care management. Overall, there was no significant difference in intervention required when classified according to BMI groups (p >0.05).Surprisingly similar percentage of participants in both Group 1 and Group 2 required critical care management since most of them in both the groups were facing different eating problems. : This concludes that the PG-SGA showed high sensitivity and good specificity in predicting malnutrition among cancer patients but it can be improved further by taking up a larger sample of patients or increasing the number of sample size so that the data are made accurate.
Assessment of Nutritional Status in Patients with Head and Neck Cancer Before Radiotherapy: A Single-center, Cross-sectional Study

Abstract:Objective: It is important to assess nutritional status to determine the presence of malnutrition because poor nutritional status will reduce the efficacy and increase the side effects of radiotherapy. The aim of this research was to assess nutritional status by comparing several parameters, namely anthropometry, biochemistry, physical condition, and inflammatory parameters, with Patient Generated-Subjective Global Assessment (PG-SGA) as the gold standard. Methods: A cross-sectional study with 78 subjects was conducted at the General Hospital Dr. Sardjito Yogyakarta, Indonesia, in 2022. The Malnutrition Screening Tool, Simple Nutrition Screening Tool, PG-SGA, and objective parameter data were used in the nutritional assessment. The objective parameters were determined by analyzing anthropometric data [body weight, mid-upper arm circumference (MUAC), and body fat], biochemical data (albumin and a complete blood profile), physical data (hand grip strength), and food intake data using the 1×24-hour recall method. The data were analyzed using One-Way ANOVA and the Kruskal-Wallis test. Results: Malnutrition was found in 33.3% of pre-radiotherapy head and neck cancer (HNC) patients. Patients with good nutritional status did not experience weight loss, decreased appetite, gastrointestinal symptoms, decreased functional capacity, or fat and/or muscle deficit (p<0.05). The findings showed a significant relationship between PG-SGA and nutritional status based on body weight, weight loss, MUACs, handgrip strength, visceral fat, resting metabolic rate (RMR), and hemoglobin (p<0.05). A better nutritional status was associated with higher parameter values. Conclusions: The method for nutritional status assessment in HNC patients undergoing radiotherapy can be performed by measuring body weight, weight loss, upper arm circumference, visceral fat, hemoglobin, and RMR in addition to PG-SGA as the gold standard.

Abstract:The scored Patient-Generated Subjective Global Assessment (PG-SGA) is considered to be the most appropriate tool for detecting malnutrition in cancer patients. In particular, malignant tumours derived from the gastrointestinal tract may impair nutrient intake and absorption and cause malnutrition. We carried out a cross-sectional study to assess the nutritional status and related factors of patients with gastrointestinal cancer. Nutritional status was determined using the scored PG-SGA in patients (n 498) with advanced gastrointestinal cancer admitted to the Gastrointestinal Medical Oncology Unit at Beijing Cancer Hospital between 1 August 2012 and 28 February 2013. The possible related factors including age, sex, hospitalisation frequency and pathology were explored. We found that 98% of the patients required nutrition intervention and 54% of the patients required improved nutrition-related symptom management and/or urgent nutritional support (PG-SGA score ≥9). Factors related to malnutrition were age (r 0.103, P<0.01), hospitalisation frequency (r -0.196, P<0.01) and sex (the prevalence of malnutrition was higher in men than in women (9.88 v. 8.54, P<0.01)). Patients with rectal cancer had a lower risk of malnutrition than patients with other types of gastrointestinal cancer (F=35.895, P<0.01). More attention should be paid to the nutritional status of gastrointestinal patients, especially those at a higher risk of malnutrition, such as elderly patients, those hospitalised for the first time, male patients and those with other types of gastrointestinal cancer except rectal cancer. The nutritional status of these patients should be evaluated and they should be given proper nutrition education and nutritional support in a timely manner.

Abstract:SIR, Interferons have been used in the treatment of viral papillomas, HIV-associated Kaposi's sarcoma, inflammatory dermatoses and cutaneous tumours.' We report a case of a Merkel cell tumour that was treated with interferon-a-2b (IFN). An 85-year-old man had a subcutaneous mass in his right leg excised in March 1987 and the histology and immunohistochemistry were typical for that of a Merkel cell tumour. Postoperatively he received radiotherapy, but the tumour recurred and in December 1987 a further excision was performed. The patient later returned in March 1988 with a recurrent mass that measured 3 x 3 cm and following excision, he had a course of superficial radiotherapy. There was a further recurrence along the excision margin in May 1988. He was then treated with IFN (Intron A) receiving 3 x lO*" U subcutaneously three times a week. The tumour became softer after 1 month and disappeared after 3 months. Subsequently there has been no recurrence with 1 year of follow up. Merkel cell tumour is a rare but aggressive form of skin cancer and there is a high rate of recurrence of 3 5-40% within a year of diagnosis.^'^ Locally recurrent lesions have been treated with variable success with further excision, amputation, radiation and chemotherapy.^'^ The advanced age and the poor health of many of the patients preclude the routine use of cytotoxic drug therapy. This case report suggests that IFN may produce a relatively long remission in the treatment of Merkel cell tumour.
Evaluating Nutritional Health in Cases of Head and Neck Cancer Receiving Radiotherapy

Abstract:Introduction Patients diagnosed with head and neck cancers (HNC) are highly susceptible to malnutrition, with initial rates ranging from 35% to 60%, which can further worsen during treatment, leading to significant weight loss in up to 80% of patients. Malnutrition often remains undetected, contributing to increased morbidity, mortality, and healthcare expenses. This study sought to evaluate the nutritional status of patients with head and neck cancer receiving radiotherapy and investigate its relationship with factors related to the disease and treatment.  Materials and Methods The study is a longitudinal observational research involving 31 patients who were treated at the Radiation Oncology Unit of the National Institute of Oncology in Rabat, Morocco, between October 2023 and March 2024. Patients underwent radiation therapy using the Volumetric Modulated Arc Therapy (VMAT) technique, either alone or in combination with chemotherapy. Data, including anthropometric measurements, PG- SGA assessments, and RTOG criteria evaluations, were gathered at the start of treatment, at the end of treatment, and three months after the treatment ended.  Results The cohort consisted of 28 males and 3 females, with a mean age of 56 ± 8 years. The majority of tumors were located in the larynx (32%) and nasopharynx (30%), with 78% of patients presenting with stage III or IV cancer. Initially, 88% of patients were well- nourished; however, by the end of treatment, 79% were malnourished. The average PG-SGA score rose from 2.8 to 11.3. There was a marked decrease in food intake, with only 40.2% of patients consuming more than half of their meal portions by the end of treatment. Weight loss occurred in 85% of patients, with 27% experiencing a loss of over 10% of their initial body weight. Patients with advanced-stage cancer, oral cavity and pharyngeal cancers, and those undergoing chemo-radiotherapy were more likely to be malnourished. Higher radiation doses were associated with increased pain and swallowing difficulties.  Conclusion Radiotherapy significantly worsens the nutritional status of HNC patients, particularly by the end of treatment. The most severe nutritional declines were observed in patients with oral cavity and pharyngeal cancers, advanced-stage disease, concurrent chemotherapy, and higher radiation toxicity. Incorporating routine nutritional assessments into clinical practice and assigning dedicated staff for these evaluations could enable timely nutritional interventions.
Occurrence of Malnutrition in Cancer Patients During Radiotherapy

Abstract:Objective? To investigate the malnutrition status of cancer patients undergoing radiotherapy, and to provide research basis for later nutrition intervention. Methods? Using convenient sampling method, 380 patients with cancer treated by radiotherapy in Shanghai and Zhejiang provincial ClassⅢ Grade A hospitals from February to November 2018 were selected as the research subjects. The incidence of malnutrition, laboratory indicators on admission and discharge, and the incidence of significant weight loss during hospitalization were investigated. Malnutrition was assessed by Patient-Generated Subjective Global Assessment Scale (PG-SGA). Results? Among 380 cancer patients receiving radiotherapy, 48.9% (186/380) suffered from or suspected malnutrition at admission, of which 7.6% (29/380) suffered from severe malnutrition. At discharge, 64.2% (244/380) of radiotherapy patients developed or suspected malnutrition, of which 18.9% (72/380) suffered from severe malnutrition. In addition, 56.3% (214/380) patients lost weight during radiotherapy, and 12.4% (47/380) patients showed significant weight loss. Compared with that at admission, the levels of serum albumin and hemoglobin decreased significantly (P＜0.05). Conclusions The incidence of malnutrition is high in cancer patients during radiotherapy. Early nutritional assessment should be given at admission and early nutritional intervention should be given to malnutrition patients.
Nutrition intervention in advanced upper gastrointestinal cancer patients undergoing chemotherapy

Abstract:Objective To evaluate the effect of nutritional intervention on the nutritional status of patients with upper gastrointestinal cancer before and after chemotherapy. Methods Advanced upper gastrointestinal cancer patients (n=82) were screened and assessed by NRS 2002 and PG-SGA. According to the results of PG-SGA, the patients were divided into three groups:well-nourished (A), moderately malnourished or suspected of being malnourished (B) and severely malnourished (C). Nutritional intervention is based on the "five steps of malnutrition". Nutritional status, weight, energy and protein intakes of the patients were compared before and after chemotherapy. Results Before intervention well-nourished patient was 10.98%(8/82), moderately malnourished or suspected of being malnourished was 50.00%(41/82), severely malnourished was 39.02%(32/82). The incidence of nutritional risk and malnutrition after intervention decreased, before intervention was 52.44% and 89.02% respectively, after intervention was 35.37% and 75.61% respectively. Compared to before chemotherapy, the weight of patients in well-nourished group and moderately malnourished or suspected of being malnourished group maintained stability, [(67.88±11.25) kg vs (67.22±10.17) kg, P>0.05 and (63.00±11.50) kg vs (62.58±11.48) kg, P>0.05], respectively. While in severely malnourished group, weight loss significantly [(59.92±9.61) kg vs (59.15±9.46) kg, P<0.05]. The average daily dietary intake of energy and protein increased significantly of moderately malnourished or suspected of being malnourished group and severely malnourished group, compared to before intervention (P<0.05) well-nourished group maintained stability (P>0.05). Conclusions Nutritional intervention can reduce the incidence of nutritional risk and malnutrition of patients with advanced upper gastrointestinal cancer, decrease weight loss, improve nutritional status, and may help to improve the clinical outcome.
Prevalence of Malnutrition and Influence of an Individualized Nutritional Support on the Nutritional Status of Cancer Patients

Abstract:Background: Muscle mass loss, impaired dietary intake, and poor nutritional status are hallmarks of patients with cancer. Cancer-associated malnutrition can lead to several negative consequences, including poor prognosis, reduced survival, increased therapy toxicity, reduced tolerance and compliance to treatment, and diminished response to drugs. The main aim of this study is to understand the prevalence of malnutrition using Global Leadership Initiative on Malnutrition Criteria (GLIM Criteria) and evaluate the impact of tailor-made nutritional intervention on the nutritional status of patients with cancer. Methods and Material: An interventional prospective study was conducted among 100 adult hospitalized cancer patients. The study was based on the Nutrition Care Process model, where nutritional assessment was done using GLIM criteria and Malnutrition Universal Screening Tool (MUST), and muscle mass was assessed using the Handgrip Dynamometer. Nutritional diagnosis was made based on assessment; nutritional intervention was provided through Individualized Nutritional Counseling & Oral Nutritional Support, and nutritional monitoring was done for patients during their stay in hospital using 24-hour daily dietary recall. Nutritional evaluation was done at the time of discharge using GLIM criteria, MUST, and Handgrip Strength (HGS). Data analysis was done using SPSS v24 statistical software (developed by IBM) to find out the Mean and Standard Deviation. Paired t test, Chi-square test, and Regression Analysis were used to analyze the qualitative variables. Statistical significance was defined as P < 0.05. Results: The current study evaluated 100 participants among whom 26% of participants were malnourished as assessed by MUST and 22% participants had poor muscle strength. A paired t -test was performed and it was found that the calorie intake (1504.1 ± 158.7) and the protein intake (56.08±7.01) were almost double after the nutrition intervention. The difference was found to be highly statistically significant with P < 0.01. Thus, nutrition intervention significantly ( P < 0.01) improved calorie and protein intake. The overall mean MUST score at baseline was 0.39 which was reduced significantly ( P < 0.000) to 0.17 at the end of the study. The HGS improved significantly ( P < 0.000) from 10.02 to 25.51 after the nutrition intervention and it was evidenced by 2.5 times increase in HGS (25.51 ± 2.40) by the end of the study. Conclusion: Individualized nutritional support with continuous monitoring and counseling resulted in improvement in nutritional status as evidenced through their improved MUST score and HGS.
Assessing the risk of malnutrition in cancer patients during radiotherapy

Abstract:Radiotherapy is considered one of the most effective curative therapies for cancer, but it has multiple side effects, especially when the irradiation is on the oral or abdominal area of the body, with a negative impact on nutrition in terms of quantity and quality, as well as the way nutrients are assimilated in the body. The objective of this study was to assess the risk of malnutrition in cancer patients during radiotherapy. The methodology was based on the use of the NRS-2002 screening tool and a set of questions extracted from the EORTC QLQ-C30 questionnaire related to the patient quality of life of a group of patients during radiationtreatment between September and December 2022. Results. From the group of 46 participants, 56.52% of the patients were identified with a moderate risk of malnutrition, and 92.30% of them benefited from personalized nutritional counseling. Weight change for the total group registered an average decrease of 0.71-0.77 kg. Symptoms such as decreased appetite were found in 23.91% of participants. According to the responses of the participants, the evaluation of the general state of health had an average score of 5.17 (out of 7), and the general quality of life had an average of 4.48 (out of 7). Following the dietary survey, all participants received general dietary recommendations, according to the ESPEN guidelines, and patients identified at risk of malnutrition received personalized nutritional counseling and indications for enteral nutrition (oral or gastrostomy solutions) to meet the caloric needs. In conclusion, the study proved the importance of early identification of malnutrition and personalized nutritional intervention for reducing anxiety related to food, increasing food intake, maintaining body weight, respectively a better management of manifested adverse effects and a better compliance with medical therapy.
The Effect of Nutrition Intervention With Oral Nutritional Supplements on Ovarian Cancer Patients Undergoing Chemotherapy

Abstract:Background: Ovarian cancer is the third most common gynecological malignancy in the world and it is under a higher incidence of malnutrition. Chemotherapy is currently a common treatment for ovarian cancer, but the resulting side effects can exacerbate malnutrition. Our aim was to investigate the beneficial effects of oral nutrition supplements (ONS) on ovarian cancer patients undergoing chemotherapy. Methods: Single-blinded randomized controlled trial. Patients with ovarian cancer receiving chemotherapy were randomly assigned either to the ONS or non-ONS groups via a simple randomization. The ONS group was given 250 mL ONS each time (1.06 kcal, 0.0356 g of protein per mL), three times a day, and nutrition education. Control group received nutrition education alone. The primary outcome was the nutritional risk of the patients as assessed by the Patient-Generated Subjective Global Assessment (PG-SGA). The secondary outcome was the results of the participants' biochemical tests at each measurement time point. Data were collected (T0) at baseline, (T1) post intervention at 3 weeks, (T2) 9-week follow-up, (T3) 15-week follow-up. Generalized estimating equation models were used to compare the changes in outcomes over time between groups. Results: 60 participants (30 ONS, 30 controls) completed the trial, and data was analyzed. For baseline comparisons, no significant differences were found between the two groups. A progressive trend toward amelioration in PG-SGA scores over time was found within the ONS group, with scores decreasing from 9.27 ± 1.68 at baseline (T0) to 5.87 ± 2.06 after the intervention (T3). Furthermore, ONS group achieved a significantly greater reduction in PG-SGA score at the T1 ( p = 0.03, confidence interval −2.23 to −0.11), T2 ( p = 0.001, confidence interval −2.86 to −0.74) and T3 ( p < 0.001, confidence interval −3.81 to −1.53), than the control group. In terms of biochemical test results, patients in the ONS group had better leukocytes, lymphocytes, Hemoglobin, Albumin and Total Protein than the control group at different time points, with statistical differences between the two groups ( p < 0.05). Conclusions: The present study demonstrated that ONS can significantly reduce the nutritional risk of patients undergoing chemotherapy for ovarian cancer. In addition, we also found that nutritional education seems to have a positive effect on reducing the nutritional risk of patients especially at the beginning of chemotherapy.

Abstract:Objective To investigate the nutritional status of patients undergoing concurrent chemoradiotherapy for ma-lignant tumor and its influencing factors.Methods A self-designed questionnaire was used to collect related clinical data of the patients who underwent concurrent chemoradiotherapy for malignant tumor in Beijing Cancer Hospital from March to May,2019.Patient-generated Subjective Global Assessment(PG-SGA)was used to investigate the nutritional status of these patients and re-lated influencing factors.Results A total of 150 usable questionnaires were collected.The median PG-SGA score was 6.0(3.0,11.0)for all 150 patients.Among these 150 patients,56(37.3%)had severe malnutrition and 49(32.7%)had moderate malnutri-tion.The univariate analysis showed that old age,esophageal cancer or head and neck cancer,stage Ⅳ tumor,more than 15 ses-sions of radiotherapy,more than 4 sessions of chemotherapy,and paclitaxel+platinum regimen or platinum monotherapy were in-fluencing factors for malnutrition in these patients(Z=-3.839--2.087,P＜0.05).The binary logistic regression analysis showed that old age,esophageal cancer or head and neck cancer,and more than 15 sessions of radiotherapy were independent risk factors for severe malnutrition in patients(P＜0.05).Conclusion There is a relatively high proportion of patients with malnu-trition among the patients undergoing concurrent chemoradiotherapy for malignant tumor,especially among elderly patients,pa-tients with esophageal cancer or head and neck cancer,and patients in the late stage of concurrent chemoradiotherapy.Medical staff should dynamically assess and monitor the nutritional status of patients and give timely intervention.

Abstract:PurposeIn the oncology population where malnutrition prevalence is high, more descriptive screening tools can provide further information to assist triaging and capture acute change. The Patient-Generated Subjective Global Assessment Short Form (PG-SGA SF) is a component of a nutritional assessment tool which could be used for descriptive nutrition screening. The purpose of this study was to conduct a secondary analysis of nutrition screening and assessment data to identify the most relevant information contributing to the PG-SGA SF to identify malnutrition risk with high sensitivity and specificity.MethodsThis was an observational, cross-sectional study of 300 consecutive adult patients receiving ambulatory anti-cancer treatment at an Australian tertiary hospital. Anthropometric and patient descriptive data were collected. The scored PG-SGA generated a score for nutritional risk (PG-SGA SF) and a global rating for nutrition status. Receiver operating characteristic curves (ROC) were generated to determine optimal cut-off scores for combinations of the PG-SGA SF boxes with the greatest sensitivity and specificity for predicting malnutrition according to scored PG-SGA global rating.ResultsThe additive scores of boxes 1–3 had the highest sensitivity (90.2 %) while maintaining satisfactory specificity (67.5 %) and demonstrating high diagnostic value (AUC = 0.85, 95 % CI = 0.81–0.89). The inclusion of box 4 (PG-SGA SF) did not add further value as a screening tool (AUC = 0.85, 95 % CI = 0.80–0.89; sensitivity 80.4 %; specificity 72.3 %).ConclusionsThe validity of the PG-SGA SF in chemotherapy outpatients was confirmed. The present study however demonstrated that the functional capacity question (box 4) does not improve the overall discriminatory value of the PG-SGA SF.

Abstract:The negative influence of malnutrition on patients with malignant tumors runs through the whole course of a disease. To the best of our knowledge, the nutritional status of cancer patients has not yet been reported in a large sample size. Therefore, this study aimed to investigate the status of malnutrition and nutritional support therapy in a large sample of patients with common malignant tumors in China. In this observational multicenter study, 80 tertiary hospitals in 22 major provinces and cities were selected using a two-stage random sampling method. A total of 47,488 hospitalized patients with 16 kinds of common malignant tumors were enrolled in this study. The nutritional status of the patients was assessed by the scored Patient-Generated Subjective Global Assessment (PG-SGA). The severity of malnutrition was identified by a quantitative assessment where 0–1 was labelled as no malnutrition and 2–3, 4–8, and ≥9 were defined as suspected/mild, moderate and severe malnutrition. The PG-SGA scores of patients with different tumor characteristics were compared by a variance analysis. The incidence rate of moderate and severe malnutrition was 58.2%, of which 32.1% and 26.1% fell under the category of moderate and severe malnutrition, 22.2% were suspected/mild and only 19.6% of the sample were not malnourished. Abstract:Objective: To investigate the nutritional status and nutrient intake of the gastrointestinal cancer patients with adjuvant chemotherapy in order to provide the basis for dietary care of the patients.Methods: The subjective global assessment(SGA) was used to investigate the overall nutritional status of 62 patients and the combined inquiry and the weighing method was used to conduct a survey on the nutrient intake of the patients on any 3 days before chemotherapy,1 ~ 3 days after chemotherapy and on 8 ~ 10 days after stopping chemotherapy.The indices of traditional nutritional evaluation(BW,BMI,Hb,TLC,and ALB) and nutrient intake(energy,protein,fat,carbohydrates,dietary fiber,vitamin A,vitamin C,vitamin E,calcium,phosphorus,iron,zinc,selenium) were compared by SGA standard between the well-nourished and malnourished patients.Results: 14 patients were well-nourished(22.6%) and 48 patients were malnourished(77.4%).The differences in the comparison of BMI,Hb,TLC and ALB except BW were statistically significant between the well-nourished and malnourished patients(P<0.01);the comparison of the intake of energy,protein,fat,carbohydrates,calcium and iron was significantly different(P<0.01,P<0.05).Conclusion: The malnutrition was commonly found in the gastrointestinal cancer patients with adjuvant chemotherapy,the analysis results showed that nutrient intake plays an important role in the improvement of the nutritional status of the patients.The nurses of tumor specialty should study more nutrition-related knowledge so as to provide the professional dietary care for the patients.

Abstract:e24142 Background: Malnutrition is common in cancer and associated with greater treatment toxicity and poor clinical outcomes. Validated screening tools can identify individuals at risk; however, they are underutilized in clinical practice. It is also unclear how nutrition status is assessed and reported in oncology clinical trials. We analyzed nutrition assessment protocols in clinical trials published in major US oncology journals. Methods: Three researchers (YA, AA, VS) identified studies published in the Journal of Clinical Oncology, JAMA Oncology, and the New England Journal of Medicine under the “original reports”, “original articles”, or “’ research” categories, respectively (1/1/2022-12/31/2022). We selected studies that reported outcomes of systemic therapeutic interventions in any Phase clinical trial. This included chemotherapy, hormone therapy, and immunotherapy alone or combined with radiation therapy or surgical procedures. Systematic reviews, meta-analyses, commentaries, retrospective studies, and secondary analyses of pooled data from completed trials were excluded. Descriptive statistics summarized how malnutrition risk was assessed and nutrition data reported. Results: 521 articles were reviewed; 153 (29%) studies met eligibility criteria. Most were randomized trials (103/153, 67%) and Phase II or III (126/153, 82%). Malnutrition risk assessment was absent from all studies. Overall, 15/153 (10%) reported either baseline Body Mass Index or pre-treatment weight changes. These variables were included in the survival analysis of 5/15 studies. The primary cancer sites studied were breast (5/15), gynecologic (3/15), hematologic (3/15), multiple sites (2/15), prostate (1/15), and pancreatic (1/15). Conclusions: Most oncology clinical trials ignore and/or underreport nutrition status. Only 10% of the studies reported nutrition status data, and not all cancer sites were represented. There is a need to routinely assess nutrition status and (given the major prognostic value) investigate its potential uses for patient selection and stratification in clinical trials.

Abstract:Objective Cancer patients are at risk of malnutrition, which is associated with poor oncological outcomes. The aim of this study was to assess the incidence of malnutrition before, during, and after radiotherapy in locally advanced cervical cancer patients. In addition, we evaluated the impact of malnutrition on survival, and whether and when malnourished patients were referred to a dietitian. Methods This retrospective cohort study included cervical cancer patients who received primary or adjuvant radiotherapy with curative intent between January 2013 and January 2021. Patient and treatment characteristics, including longitudinal data on weight and dietary care, were retrieved from the electronic patient files. Malnutrition was defined by body mass index and weight loss according to the Global Leadership Initiative on Malnutrition (GLIM). Overall survival was estimated using the Kaplan–Meier method. Cox proportional hazard regression analysis was used to estimate hazard ratios for key prognostic factors. Results A total of 294 patients were included. Median follow-up was 40 months (range 7–101 months). Malnutrition occurred in 44 patients (15%) at baseline, in 132 (45%) during radiotherapy, and in 63 (21%) during follow-up. Referral to a dietician occurred in 45% of the 138 patients who were malnourished before or during radiotherapy. Malnutrition was significantly associated with worse survival after adjusting for age, performance score, diabetes, histology, International Federation of Gynecology and Obstetrics (FIGO) stage, and nodal stage. The 3 year overall survival in patients with malnutrition was 77% (95% confidence interval (CI) 70% to 85%) and without malnutrition 89% (95% CI 83% to 95%); p=0.001). Independent significant risk factors for worse overall survival were: malnutrition, age ˃52 years, adenocarcinoma, FIGO stage III/IV, and N1 disease. Conclusion Malnutrition was common in cervical cancer patients treated with radiotherapy and was associated with a shorter overall survival. Further studies are needed to evaluate the effectiveness of better monitoring of malnutrition and faster and better dietary intervention on survival and quality of life.

Abstract:Background Malnutrition screening is not widely practiced in outpatient cancer centers. This review aims to determine the validity of malnutrition screening tools and provide recommendations for clinical use. Methods Studies identified by a systematic review assessed the general validity of screening tools in adult oncology outpatients from five databases through 2022. The American Society for Parenteral and Enteral Nutrition (ASPEN) convened a working group of members from the Academy of Nutrition and Dietetics, Academy of Oncology Nurse and Patient Navigators, American Cancer Society, American Society for Clinical Oncology, American Society for Nutrition, American Society for Radiation Oncology, Association of Cancer Care Centers, and Oncology Nursing Society to answer the following questions: (1) should clinicians screen for malnutrition, (2) which malnutrition screening tools are recommended, and (3) what are the clinical applications for malnutrition risk screening in adult oncology outpatients? Results Twenty of 738 studies met the criteria and were reviewed. Six screening tools with specific cut‐points demonstrated validity and are recommended, including the Mini Nutritional Assessment (≤23.5), Malnutrition Screening Tool (MST; MST ≥ 2 and patient‐led MST ≥ 2), Malnutrition Universal Screening Tool (MUST; MUST ≥ 1 and MUST ≥ 2), Nutrition Risk Screening‐2002 (NRS‐2002; NRS‐2002 ≥ 2 and NRS‐2002 ≥ 3), NUTRISCORE ≥ 5, and Patient‐Generated Subjective Global Assessment Short Form (PG‐SGA SF; PG‐SGA SF ≥ 7 and PG‐SGA SF ≥ 8). Conclusion Six screening tools are valid for malnutrition risk identification in oncology ambulatory settings and recommended before treatment initiation and regularly thereafter, depending on treatment course. Research is needed to understand to what extent early diagnosis and management of malnutrition improves the clinical care of oncology patients.

Abstract:Introduction: Malnutrition is prevalent among individuals with gastric cancer and notably decreases their quality of life (QOL). However, the factors impacting QOL are yet to be clearly defined. This study aimed to identify essential factors impacting QOL in malnourished patients suffering from gastric cancer. Methods: By using the Patient-Generated Subjective Global Assessment (PG-SGA) to assess the nutritional status (≥4 defined malnutrition) of hospitalized cancer patients, 4,586 gastric cancer patients were ultimately defined as malnourished. Spearman method was used to calculate the relationship between clinical features and the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30). Then, univariate and multivariate logistic regression were used to observe which factors affected QOL, and subgroup analysis was performed in young and old population respectively. In addition, we used univariate and multivariate logistic regression to explore whether and how self-reported frequent symptoms in the last 2 weeks of the PG-SGA score affected QOL. Results: In multivariate logistic regression analysis of clinical features of patients with malnourished gastric cancer, women, stage II, stage IV, WL had an independent correlation with a low global QOL scores. However, BMI, secondary education, higher education, surgery, chemotherapy, HGS had an independent correlation with a high global QOL scores. In multivariate logistic regression analysis of symptoms in self-reported PG-SGA scores in patients with malnourished gastric cancer, having no problem eating had an independent correlation with a high global QOL scores. However, they have no appetite, nausea, vomiting, constipation and pain had an independent correlation with a lower global QOL scores. The p values of the above statistical results are both < 0.05. Conclusion: This study demonstrates that QOL in malnourished patients with gastric cancer is determined by female sex, stage II, stage IV, BMI, secondary and higher education or above, surgery, chemotherapy, WL, and HGS. Patients' self-reported symptoms of nearly 2 weeks, obtained by using PG-SGA, are also further predictive of malnourished gastric cancer patients. Detecting preliminary indicators of low QOL could aid in identifying patients who might benefit from an early referral to palliative care and assisted nursing.
