Abstract:The intrinsic hydrophilicity of conventional dressings cannot achieve effective management of excessive biofluid around the wound bed, which inevitably causes infection and hinders wound healing. In addition, present dressings such as medical gauze or band aids have a limited stretching capability, which does not comply well with the skin deformation during muscle movement, thus impacting patient comfort. Herein, a Janus wound dressing is reported by assembling an external hydrophobic (HP) adhesive tape, a filter paper, and a polydimethylsiloxane (PDMS) Janus film. The PDMS Janus film as the primary dressing can unidirectionally remove biofluid away from the wound bed. The mechanism of the unidirectional biofluid transport is investigated, demonstrating that the stretching or bending of the Janus dressing is beneficial for unidirectional biofluid draining. It indicates that the Janus PDMS film has potential for practical applications on stretched or bended skin surface. In addition, in order to prevent bacterial infection, amoxicillin powder is uniformly encapsulated on the HP layer of Janus film, resulting in faster wound healing. This study is valuable for designing and fabricating next-generation dressings with high performance for clinical applications.

Abstract:The healing process of infected skin lesions is often delayed by many factors including bacterial infection, excessive accumulation of wound exudate, poor local perfusion, and low cell recruitment. Development of wound dressing for efficient wound management and promotion of wound healing is a great challenge. Herein, we constructed a Janus nanofiber dressing with the comprehensive capacities of bacterial killing, piezoelectrical stimulation for promoting fibroblast migration, and wound exudate removal via unidirectional liquid delivery. The rationally designed Janus nanofibrous dressing with ultrathin, flexible, breathable, and piezoelectric characteristics is fabricated via the facile layer-by-layer electrostatic spinning technology. The hydrophilic layer of the dressing is composed of randomly arranged polycaprolactone/gelatin (PCL/Gel) nanofibers, while the hydrophobic layer consists of well-aligned Ag nanoparticles (Ag NPs)-doped piezoelectric polyvinylidene fluoride (PVDF/Ag) nanofibers. The in vitro and in vivo experimental results demonstrate that the Janus dressing can not only drain excess wound exudate out of the wound unidirectionally and kill local bacteria, but also generate dynamic piezopotential under normal body motions to promote fibroblast proliferation and migration, collagen deposition, angiogenesis, and re-epithelialization, which accelerates rapid wound healing on mice. The smart Janus dressing will provide a new approach for acceleration of wound healing and wound management.

A poly (lactic-co-glycolic acid) self-pumping Janus dressing with bidirectional biofluid transport for diabetic wound healing via anti-bacteria and pro-angiogenesis

Abstract:Diabetic wound healing poses a substantial challenge owing to bacterial infections, insufficient angiogenesis, and excessive exudates. Currently, most of the clinical dressings used for diabetic wounds are still conventional dressings such as gauze. In this study, a three-layer Janus dressing was developed via continuous electrostatic spinning. The top-layer was composed of polylactic acid-glycolic acid and hydroxyapatite doped with silver ions and silicate. The hydrophobic top-layer prevented the adhesion of foreign bacteria. The mid-layer was composed of polyethylene glycol, polylactic acid-glycolic acid and hydroxyapatite doped with silver ions and silicate facilitated exudate absorption and bioactive ion release. The modified sub-layer containing polylactic acid-glycolic acid, hydroxyapatite doped with silver ions and silicate and sodium alginate microspheres enabled both the transport of wound exudate from the wound bed to dressing and the backflow of bioactive silver ions and silicate to the wound bed, thereby reducing infection and stimulating angiogenesis. Through in vivo and in vivo experiments, the Janus dressing showed to have antimicrobial, angiogenic, and exudate-control properties that accelerate healing in diabetic wounds. As a novel dressing, the multifunctional, self-pumping Janus wound dressing with bi-directional biofluidic transport offers a new approach to diabetic wound healing.
Janus polyurethane sponge as an antibiofouling, antibacterial, and exudate-managing dressing for accelerated wound healing

Abstract:The non-fouling condition, bacteria-free environment and suitable moisture at wound site are crucial for chronic wound healing. However, it remains highly meaningful yet challenging to develop wound dressings that can simultaneously achieve these desirable functions. In this work, a kind of multifunctional Janus polyurethane sponge (Janus-PU) was designed and fabricated by coating near-infrared (NIR)-responsive and superhydrophobic nanoparticles (F-ZnO@Ag NPs) on one surface of sponge. The nano-functionalized outer layer can endow Janus-PU with superhydrophobic antifouling property for preventing bacterial colonization and broad-spectrum antibacterial activity due to the presence of Ag NPs. Especially, the synergistic combination of asymmetric structure and strong NIR photothermal effect can impart Janus-PU with NIR-controlled unidirectional exudate removal, thus achieving an optimal wetting environment for wound healing. The mice full-thickness skin acute wounds treated with Janus-PU under NIR irradiation showed superior anti-infection and healing effect compared to the commercial dressings. Significantly, the treatment using Janus-PU with NIR irradiation can accelerate the recovery of methicillin-resistant Staphylococcus aureus (MRSA)-infected diabetic chronic wounds due to the synergistic effect of antibiofouling, antibacterial and exudate-managing. The Janus-PU as a promising multifunctional dressing can prevent bacterial invasion and create an appropriate environment for wound healing, providing an effective solution for intractable wounds and infections. STATEMENT OF SIGNIFICANCE: The development of advanced wound dressings to ensure non-fouling condition, bacteria-free environment and suitable moisture is crucial for chronic wound healing. However, it remains a considerable challenge to simultaneously integrate antibiofouling, antibacterial and exudate-managing properties into a single dressing. In this work, we developed a kind of multifunctional Janus polyurethane sponge (Janus-PU) by a single-sided superhydrophobic modification strategy, which can simultaneously achieve superhydrophobic antifouling property, effective broad-spectrum antibacterial and near-infrared controlled exudate removal. The Janus-PU designed herein can not only create an optimal environment for accelerated wound healing, but also avoid frequent dressing replacement, thus providing an ideal material system for intractable wounds and infections.

Abstract:Traditional hydrophilic wound dressings are not effective in draining excessive wound exudate, generating ideal conditions for bacterial breeding. Furthermore, emerging hydrophobic‐hydrophilic Janus‐type wound dressings cannot maintain a moisture‐balanced wound surrounding because the hydrophobic side contacting the wound bed develops a severely dry environment by continuously self‐pumping the wound biofluids. Moreover, antibiotics are embedded in wound dressings with fixed concentrations regardless of infection type, significantly reducing therapeutic effectiveness while that misuse and overuse of antibiotics accelerate antibiotic resistance. Herein, a single‐layered Janus paper was innovatively used as a wound dressing to realize efficient absorption of the exudate and effective delivery of the right antibiotic with the exact dose. Unlike the typical Janus‐type technique, our wound dressing placed the lower hydrophilic side on the wound bed while the antibiotic was introduced through the upper hydrophobic side. To manage the excessive wound exudate, the hydrophilic area was extended to have four additional absorbent pads. The correct information on the antibiotic efficacy and the right doses for treatment was provided by clinically relevant antibiotic susceptibility testing while the appropriate antibiotic was manually introduced but automatically transferred unidirectionally right to the wound bed. In vitro evaluation with bacterial cultures on solid agar and ex vivo assay with porcine skin demonstrated that our Janus wound dressing provided excellent drainage of wound exudate and inhibited the growth and reproduction of pathogens. Abstract: An intravenous infusion of (PGE2) prostaglandin E was given for 8 hours on 2-3 consecutive days to 7 normally cycling women during various periods of the luteal phase, and to 1 woman whose luteal phase was prolonged by hCG administration. 3 women were infused with saline solution only. During PGE2 infusion, 2 hourly blood samples showed some variations in progesterone and estradiol levels regardless of the period of the luteal phase; no clear-cut depression was noted. These changes were similar to the spontaneous variations seen in controls. PGE2 did not induce intermenstrual bleeding or premature menstruation, although uterine contractility was markedly increased. In 1 subject, PGE2 was given during the early postovulatory period without interfering with implantation and subsequent normal pregnancy. An increase of cortisol and GH blood levels was also observed during PGE2 infusion. We conclude that PGE in the dosage used is not luteolytic in humans.

Abstract:The dynamic management of wound exudate can be achieved by utilizing a hydrophobic layer whose thickness is capable of adjusting in response to temperature stimulation. This is achieved by using an upper critical solution temperature (UCST)‐type polymer to create a liquid gate and introducing it between the hydrophilic and hydrophobic layers of the Janus structure through electrospinning technology. Exudate plays a crucial role in wound healing, but an excessive amount can lead to over‐hydration of tissue and aggravating the infection and the injury. On the other hand, inadequate exudate can cause scarring and hinder healing. Traditional wound dressings are unable to regulate exudate levels based on the specific needs of the wound. To address this issue, a liquid‐gated trilayered fibrous wound dressing capable of pumping fluid in a temperature‐dependent manner is developed. This dressing comprises a hydrophilic cotton layer, a thermo‐sensitive (TPPU) layer, and a hydrophobic layer of polyurethane (PU) nanofibers. The TPPU layer is constituted by nanofibers that are composed of an upper critical solution temperature (UCST)‐type polymer, PU, and silver nanoparticles. The intermediate TPPU layer exhibits changes in its wettability upon heating, adjusting the thickness of the hydrophobic layer and ultimately achieving the appropriate structure for guiding spontaneous fluid transport. The positive effect of this novel dressing on diabetic wounds is observed, as it enhanced epithelialization and collagen synthesis while reducing inflammation, ultimately accelerating the wound healing process. This dressing has the potential to provide a groundbreaking solution for managing exudate, achieving moisture balance and promoting wound healing in clinical settings.

Abstract:Abstract Self‐pumping dressings become one of the optimal solutions for the controlled management of chronic diabetic wound exudate and wound healing. However, present self‐pumping dressings are not only prone to breakage of the loose hydrophobic layer but also have cumbersome and complicated preparation steps, which hinder the application of self‐pumping dressings in diabetic wound treatment. Herein, a novel self‐pumping structure of superabsorbent Janus dressing is designed to improve the strength of the hydrophobic layer and promote diabetic wound healing. The Janus dressing consists of a hydrophobic layer with a drainage agent (drainage layer) and a fluffy 3D nanofiber cotton (absorbent layer). Regardless of the thickness of the drainage layer, the drainage agent in the drainage layer provides the fluid to penetrate the drainage layer to the absorbent layer for unidirectional fluid draining. In design proof, the superabsorbent Janus dressing provides unidirectional drainage of inflammatory exudate and regulation of macrophage polarization, resulting in faster diabetic wound healing than single‐layer dressings. Thus, the Janus dressing demonstrates important clinical implications to offer a novel design and preparation strategy for accelerating diabetic wound healing.

An Asymmetric Janus Membrane with Anti-Bacteria Adhesion and Rapid Hemostasis Properties for Wound Healing

Abstract:Currently, commercial intraoral dressings have limitations as they only serve a single purpose and fail to meet various needs, such as hemostasis, wound protection, and dressing fixation. Here, we successfully fabricated a multifunctional polycaprolactone-chitosan Janus membrane (PCJM) with simultaneous hydrophobic and hydrophilic properties. The hydrophobic layer consists of a dense and disordered polycaprolactone, preventing bacterial adhesion, resisting fouling, and enhancing mechanical strength. The hydrophilic layer is a highly porous chitosan sponge, which facilitates cell and protein adhesion, as well as rapid absorption of blood and activation of the coagulation process. These results are confirmed by hemostasis experiments, which demonstrate that the PCJM had superior coagulation ability compared to commercial gelatin sponges and gauze, and by an early gingival healing model of beagle tooth extraction wounds, which exhibited excellent performance in promoting wound healing. Our work demonstrates the multifunctional PCJM wound dressing not only protects wounds from bacterial infections but also facilitates rapid hemostasis and early healing of beagle gingival soft tissue, which holds significant potential for clinical translation and large-scale production. Abstract:Proper management of exudate is of great clinical value for reducing wound infection and promoting wound healing, thus various dressings have been studied to address this widespread medical challenge. Herein, we present a novel sandwich‐structured composite wound dressing (SCWD), integrating of a superlyophobic (SLO) polydimethylsiloxane (PDMS) layer, a superlyophilic gauze layer and a lyophobic PDMS layer, with particular unidirectional droplet drainage and stable anti‐adhesion capabilities, which realizes effective management of wound exudate and provides a favorable environment for wound healing. Thanks to the stable SLO property on the PDMS surface with hierarchical micro/nanostructures, the continuously accumulated wound exudate at the interface between dressing and wound surface is gradually deformed, eventually passing through SLO PDMS layer through milli‐scale channels and being absorbed by gauze layer. Experimental results show that the application of SCWD could significantly reduce the occurrence of wound infection, avoid the tearing of wound tissues when replacing dressings, and accelerate wound healing by approximately 20%. The combination of SCWD and lyophilized powders of stem cells supernatant (LPSCS) is verified to better accelerate the healing process. Janus Intelligent Antibacterial Hydrogel Dressings for Chronic Wound Healing in Diabetes

Abstract:Long-term recurrent infections are one of the significant reasons why chronic diabetic wounds are difficult to heal, and the treatment of chronic wounds remains a major clinical challenge. In this work, we developed a kind of Janus hydrogel dressing capable of adjusting the complex microenvironment of wounds for promoting the healing of diabetic chronic wounds. The Janus hydrogel dressings are made of zwitterionic polymers and gentamicin sulfate-modified hyaluronic acid (HA-GS). One side of the hydrogel dressings was brushed with chitosan to act as a bridge between wound dressings and wound tissues. Thus, the wound dressings showed Janus properties with an antifouling surface on one side and an adhesive surface on the other, where the antifouling surface of zwitterionic hydrogels can avoid the adhesion of external bacteria, while the adhesive surface helps the dressings to be fixed on the wound. Moreover, the antibiotic GS was connected to HA through Schiff’s base, and thus it can be intelligently released in response to the acidic environment of infected wounds to kill the internal bacteria. The Janus hydrogel dressings were carefully characterized and investigated. In vivo results of a diabetic rat infected wound model showed that the Janus hydrogel dressings could effectively inhibit inflammation, accelerate the formation of granulation tissues and collagen deposition, and promote wound healing. Therefore, this multifunctional hydrogel dressing is a promising candidate for chronic wound healing in clinical practice.

Abstract:A timely antibacterial dressing that can adapt to dynamic wound environments poses a significant challenge in burn research and management. Herein, a four-layered composite dressing is reported. At its core, it features a modified micropore arrayed Janus membrane with a super-hydrophobic polyurethane layer on one side and a super-hydrophilic hydrogel on the other. This unique design enables the dressing to maintain stable and durable self-pumping capabilities, effectively transferring wound exudate to the upper foam layer. The self-pumping rate exceeds that of traditional foam dressings, being approximately 1.39 times more efficient in absorbing excess wound exudate. The composite dressing demonstrates an impressive water donation capacity when tested on 45% gelatin-simulated skin, achieving a water donation rate of (59.20 ± 2.66)%. Furthermore, the water vapor permeability of the composite dressing outperforms hydrocolloids by approximately 3.66 times. Additionally, the composite dressing, when loaded with super-hydrophilic nanometer zinc oxide, exhibits a potent antibacterial effect. The dressing's surface reduces cellular, platelet, fibrinogen, and bovine serum albumin adhesion, reducing the risk of tissue adhesion. During a 20-day trial covering deep II-degree burn wounds, the dressing demonstrated excellent biocompatibility. It effectively compresses the inflammatory phase, promotes vascularization, and significantly accelerates the regeneration of skin accessory organs. This innovative dressing exhibits great potential for use throughout the entire wound-healing process in future burn treatments.

Abstract:Wound dressings play a critical role in the wound healing process; however, conventional dressings often address singular functions, lacking versatility in meeting diverse wound healing requirements. Herein, a dual-network, multifunctional hydrogel (PSA/CS-GA) has been designed, synthesized through a one-pot approach. The in vitro and in vivo experiments demonstrate that the optimized hydrogel has exceptional antifouling properties, potent antibacterial effects and rapid hemostatic capabilities. Notably, in a full-thickness rat wound model, the hydrogel group displays a remarkable wound healing rate exceeding 95% at day 10, surpassing both the control group and the commercially 3M group. Furthermore, the hydrogel exerts an anti-inflammatory effect by reducing inflammatory factors IL-6 and tumor necrosis factor-α (TNF-α), enhances the release of vascular endothelial growth factor (VEGF) to promote blood vessel proliferation, and augments collagen deposition in the wound, thus effectively accelerating wound healing in vivo. This innovative hydrogel presents a novel and highly effective approach to wound healing.

Abstract:Cellulose‐based pH‐responsive Janus dressings with unidirectional moisture drainage, nonadhesive, and antibacterial capabilities for exudate management, visualized monitoring, and diabetic wound healing that can significantly simplify the medical care of diabetic wound and can effectively alleviate the inflammation, promote the tissue regeneration, and therefore accelerate the healing process of diabetic wound. Diabetic wounds with hyperglycemic tissue exudates and bacterial infections have become a great challenge that seriously delays the healing of diabetic wounds. Therefore, multifunctional dressings for exudate management of diabetic wound are urgently needed. Herein, a smart dressing with rapid moisture‐drained and non‐adhesive, pH‐responsive, and antibacterial capabilities is proposed for exudate management and wound monitoring. The Janus dressing is assembled with an antioxidant, pH‐sensitive, and hydrophilic cellulose cover and an antibacterial hydrophobic polycaprolactone bottom layer that directly contacts with the wounds, which can unidirectionally and irreversibly drain the wound exudates and weaken the wet adhesion to the wound. In response to the humoral pH variations (5‐9) during the healing process ofdiabetic wounds, the healing process can be in situ monitored according to the distinct pH‐responsive colors of the Janus dressing at different healing stages. In vivo assays and histopathological studies suggest that the Janus dressing has a superior pro‐healing rate, collagen deposition, and angiogenesis than commercial gauze. Notably, such a mulitfunctional dressing provides real‐time wound monitoring simply through a smartphone integrated with Python‐RGB programs, which can significantly alleviate the hyperglycemic exudate‐flooded environment as an alternative strategy for diabetic wound treatment.

Abstract:Acute and infected wounds resulting from accidents, battlefield trauma, or surgical interventions have become a global healthcare burden due to the complex bacterial infection environment. However, conventional gauze dressings present insufficient contact with irregular wounds and lack antibacterial activity against multi‐drug‐resistant bacteria. In this study, we develop in situ nanofibrous dressings tailored to fit wounds of various shapes and sizes while providing nanoscale comfort and excellent antibacterial properties. Our approach involves the fabrication of these dressings using a handheld electrospinning device that allows for the direct deposition of nanofiber dressings onto specific irregular wound sites, resulting in perfect conformal wound closure without any mismatch in 2 min. The nanofibrous dressings are loaded with multi‐armed antibiotics that exhibit outstanding antibacterial activity against Staphylococcus aureus (S. aureus) and methicillin‐resistant S. aureus. Compared to conventional vancomycin, this in situ nanofibrous dressing shows great antibacterial performance against up to 98% of multi‐drug‐resistant bacteria. In vitro and in vivo experiments demonstrate the ability of in situ nanofibrous dressings to prevent multi‐drug‐resistant bacterial infection, greatly alleviate inflammation, and promote wound healing. Our findings highlight the potential of these personalized nanofibrous dressings for clinical applications, including emergency, accident, and surgical healthcare treatment.

Abstract:Optimal wound healing requires a wet microenvironment without over-hydration. Inspired by capillarity and transpiration, we have developed a sandwich-like fibers/sponge dressing with continuous exudate drainage to maintain appropriate wound moisture. This dressing is prepared by integrating a three-layer structure using the freeze-drying method. Layer I, as the side that contacts with the skin directly, consists of a hydrophobic silk fibroin membrane; Layer II, providing the pumping action, is made of superabsorbent chitosan-konjac glucomannan sponge; Layer III, accelerating evaporation sixfold compared to natural evaporation, is constructed with a graphene oxide soaked hydrophilic cellulose acetate membrane. Animal experiments showed that the composite dressing had superior wound-healing characteristics, with wounds decreasing to 24.8% of their original size compared to 28.5% for the commercial dressing and 43.2% for the control. The enhanced wound healing can be ascribed to the hierarchical porous structure serves as the fluid-driving factor in this effort; the hydrophilicity of a membrane composed of silk fibroin nanofibers is adjustable to regulate fluid-transporting capacity; and the photothermal effect of graphene oxide guarantees exudates that have migrated to the top layer to evaporate continuously. These findings indicate the unidirectional wicking dressing has the potential to become the next generation of clinical dressings.

No Parallel Between the Biochemical Course of Acute Pancreatitis and Morphologic Findings

Abstract:The clinical, biochemical (serum enzymes), and morphologic course of acute pancreatitis was studied in 83 consecutive patients from time of admission until 3 months after discharge. At discharge, all 75 surviving patients had no more symptoms of acute pancreatitis, even though amylase (in 11%) and lipase (in 25%) were still elevated, computed tomography (CT) findings still abnormal (in 81%), and necrosis amounting up to 50% of the gland still present (in 24%). There was no significant correlation between serum enzymes and CT findings at discharge. Three months later, with the exception of one patient with an acute relapse of the disease, all followed-up patients were free of symptoms and had normal serum amylase, whereas lipase was still elevated in lo%, and CT findings abnormal in 23%. Subsiding symptoms provided a more accurate picture of the patients' recovery than the biochemical markers, and even more so that the morphologic findings.
Preparation of biocompatible wound dressings with dual release of antibiotic and platelet-rich plasma for enhancing infected wound healing

Abstract:The ever-growing threats of bacterial infection and chronic wound healing have provoked an urgent need for novel antibacterial wound dressings. In this study, we developed a wound dressing for the treatment of infected wounds, which can reduce the inflammatory period (through the use of gentamycin sulfate (GS)) and enhance the granulation stage (through the addition of platelet-rich plasma (PRP)). Herein, the sustained antimicrobial CMC/GMs@GS/PRP wound dressings were developed by using gelatin microspheres (GMs) loading GS and PRP, covalent bonding to carboxymethyl chitosan (CMC). The prepared dressings exhibited high water uptake capability, appropriate porosity, excellent mechanical properties, sustain release of PRP and GS. Meanwhile, the wound dressing showed good biocompatibility and excellent antibacterial ability against Gram-negative and Gram-positive bacteria. Moreover, in vivo experiments further demonstrated that the prepared dressings could accelerate the healing process of E. coli and S. aureus-infected full-thickness wounds i n vivo, reepithelialization, collagen deposition and angiogenesis. In addition, the treatment of CMC/GMs@GS/PRP wound dressing could reduce bacterial count, inhibit pro-inflammatory factors (TNF-α, IL-1β and IL-6), and enhance anti-inflammatory factors (TGF-β1). The findings of this study suggested that biocompatible wound dressings with dual release of GS and PRP have great potential in the treatment of chronic and infected wounds.

Abstract:Adhesive hydrogels have attracted wide attention as wound dressings. However, conventional wound dressings that passively promote healing usually show limited efficacy. Although various drugs or cells are introduced to drive the healing process, such strategies are limited by the complicated fabrication process. Here, a mechanically active adhesive and immune regulative dressing is developed, mainly consisting of temperature-sensitive hydrogel with semi-interpenetrating network structure. This hydrogel could strongly adhere to and actively contract the wound in a ready-to-use form, where the freeze-dried hydrogel and sterile saline are sealed in two separated packages. More interestingly, the interspersed poly(methacrylic acid) could further promote wound healing via increasing the formation of new blood vessels and modulating inflammatory responses. In vivo mouse and porcine wound models validate the accelerated wound healing enabled by active contraction and modulated inflammatory responses. This mechanobiological and immune regulative dressing may provide a new avenue for wound management.
