Abstract:Objective To explore the effects of maternal prenatal administration of dexamethasone on the auditory systems of newborns by analyzing the distortion production otoacoustic emission (DPOAE) results.Methods The clinical data and DPOAE results of 1021 newborns were recorded retrospectively. The newborns were divided into observation group and control group according to whether their mothers were exposed to dexamethasone before delivery. The two groups were 1:1 pair matched according to genders, number of fetus, gestational age, birth weight, Apgar score at 1 min, neonatal hypoxia and family history of congenital hearing loss. The pass rate and the response amplitude of DPOAE at each frequency were analyzed by McNemar test and paired-samples t-test for the successfully paired 168 cases.Results (1) For the observation group, the pass rates of right, left and both ears were 89.29% , 85.71% and 79.76%, respectively. For the control group, the pass rates of right, left and both ears were 94.05%, 95.23% and 91.67%, significantly higher than those of observation group. (2) The DPOAE amplitudes of the left ears of the observation group were significantly lower than that of control group at 5649 Hz, but no significant difference was found between the two groups in the DPOAE amplitudes at the other frequencies of left ears and all frequencies of right ears.Conclusion For the newborns whose mothers are exposed to dexamethasone before delivery, the DPOAE pass rate is lower and amplitude is smaller at some frequencies. More attention to the possible adverse effects of prenatal administration of glucocorticoids on auditory system is warranted.
Changes in Maternal Glucose Metabolism after the Administration of Dexamethasone for Fetal Lung Development

Abstract:Aims. Antenatal dexamethasone administration for fetal lung development may impair maternal glucose tolerance. In this study, we investigated whether glucose and insulin levels differed among singleton and twin pregnancies and pregnancies with impaired glucose tolerance (IGT) after treatment with dexamethasone.Methods. Singleton pregnancies, twin pregnancies, and pregnancies with IGT between 28 and 33 weeks of gestation whose mothers were treated with dexamethasone were enrolled in this study. Exclusion criteria included gestational hypertension, diabetes, renal disorders, and infectious diseases. The fasting plasma glucose and insulin levels were checked before administration and 24 h, 48 h, and 72 h after treatment was completed.Results. Mean glucose levels were significantly higher in the twin pregnancy and IGT groups at 24 h and 48 h after the administration of dexamethasone than those in the singleton pregnancy group (P<0.05). Although there was no significant difference in glucose levels before administration and 72 h after dexamethasone administration among the three groups, insulin levels in the IGT group were significantly higher (P<0.05). Insulin levels in the singleton pregnancy group at 24 h and 48 h after treatment were significantly lower than in the twin and IGT groups.Conclusion. The effects on maternal fasting blood glucose and insulin levels of dexamethasone administrated to promote fetal lung maturation correlated with embryo number and the presence of IGT.
[Effects of dexamethasone on short-term and long-term outcomes in late preterm infants with twin pregnancy: an observational study]

Abstract:Objective: To investigate the effect of prenatal dexamethasone on short-term outcomes and long-term neurological development in late preterm infants with twin pregnancy. Methods: A total of 315 pregnant women with twin pregnancy and their preterm infants who delivered in Peking University Third Hospital from January 2019 to December 2022 were retrospectively analyzed. The clinical data of pregnant women and preterm infants were collected. They were divided into non-medication group (93 pregnant women and 186 preterm infants), medication after 34 weeks group (123 pregnant women and 246 preterm infants), and medication before 34 weeks group (99 pregnant women and 198 preterm infants). Short-term outcomes of preterm infants were analyzed, including the incidence of neonatal respiratory distress syndrome (NRDS), wet lung, hypoglycemia, neonatal septicemia, intraventricular hemorrhage (IVH), bronchopulmonary dysplasia (BPD) and neonatal necrotizing enterocolitis (NEC). "Ages and Stages Questionnaire-Third Edition (ASQ-3) scale" was used to follow up the late neurological development of preterm infants at the corrected age of 6-54 months, and the level of neurological development was compared. Results: (1) General conditions: the gestational age at delivery in the non-medication group [36.1 weeks (35.6, 36.6 weeks)] was later than that in the medication after 34 weeks group [36.1 weeks (35.2, 36.4 weeks)] and medication before 34 weeks group [35.2 weeks (34.2, 36.2 weeks)] groups, and the differences were statistically significant (all P<0.05). After correcting for gestational age, there was no significant difference in birth weight among the three groups (H=3.808, P=0.149). There were no significant differences in gender and the proportion of small for gestational age among the three groups (all P>0.05). (2) Short-term outcome: the incidence of wet lung was 7.0% (13/186), 11.0% (27/246) and 16.2% (32/198) in the non-medication group, medication after 34 weeks group and medication before 34 weeks group, respectively, and the difference was statistically significant (P=0.018). There were no significant differences in the incidence rates of NRDS, hypoglycemia, sepsis, IVH, BPD, and NEC among the three groups (all P>0.05). Logistic regression analysis with gestational age and newborn birth weight as confounding factors showed that early gestational age (OR=0.884, 95%CI: 0.837-0.933, P<0.001) and increased incidence of selective intrauterine growth restriction type I (OR=2.967, 95%CI: 1.153-7.639, P=0.024) could both lead to an increased incidence of wet lung. (3) Long-term outcomes: a total of 109 pregnant women completed the follow-up, and 218 preterm infants with a corrected age of 6-54 months at the end of follow-up were enrolled, including 86 cases in the non-medication group, 66 cases in the medication after 34 weeks group, and 66 cases in the medication before 34 weeks group. There were no significant differences in the scores of communication, gross motor, fine motor, problem solving and personal-social among the three groups (all P>0.05). Conclusion: Prenatal administration of a single course of dexamethasone does not affect the neonatal birth weight and short-term outcomes of twin late preterm infants, and has no adverse effect on the neurological development of twin late preterm infants with a corrected age of 6-54 months.
Follow-up at 15 years of preterm infants from a controlled trial of moderately early dexamethasone for the prevention of chronic lung disease

Abstract:Objective: Postnatal dexamethasone treatment of ventilator-dependent preterm infants results in rapid improvement in lung function and reduction in chronic lung disease. However, limited data are available on long-term outcomes after such therapy. We studied growth, neurodevelopmental, and pulmonary outcomes at adolescence in children who had participated in a double-blind, placebo-controlled trial of dexamethasone beginning at 2 weeks of age for the prevention of chronic lung disease. Methods: Thirty-six infants (birth weight < or =1250 g and gestational age < or =30 weeks) who were dependent on mechanical ventilation at 2 weeks of age received a 42-day course of dexamethasone, an 18-day course of dexamethasone, or saline placebo. Twenty-two children survived to 15 years (69% of the 42-day dexamethasone group, 67% of the 18-day dexamethasone group and 45% of the control group), and all were evaluated. Intact survival was defined as survival with normal neurologic examination, IQ >70, and receiving education in the regular classroom. Results: There were no differences among groups for growth or incidence of neurologic abnormalities. The mean IQ for the 42-day dexamethasone group was 85 +/- 10 compared with 60 +/- 20 for the 18-day dexamethasone group and 73 +/- 23 for the control group. All children in the 42-day dexamethasone group were receiving education in the regular classroom compared with only 50% of the 18-day dexamethasone group and 40% of the control group. As a result, intact survival was significantly greater for the 42-day dexamethasone group (69%) than for either the 18-day dexamethasone group (25%) or the control group (18%). Pulmonary function was significantly better for the 42-day dexamethasone group compared with the 18-day dexamethasone group (eg, forced expiratory volume in 1 second: 90 +/- 16 vs 71 +/- 15% predicted, respectively). Conclusion: A 42-day course of dexamethasone therapy beginning at 2 weeks of age in preterm infants who are at high risk for severe chronic lung disease was associated with improved long-term neurodevelopmental outcome. Although additional research is needed to establish the optimal steroid preparation, dosage, and duration of therapy, these data support the view that moderately early (beginning at 1-2 weeks) corticosteroid treatment is advantageous for a select group of ventilator-dependent preterm infants.
The influence of prenatal dexamethasone administration before scheduled full-term cesarean delivery on short-term adverse neonatal outcomes: a retrospective single-center cohort study

Abstract:Objective: There has been a gradual increase in the prevalence of cesarean section deliveries and more healthcare professionals are considering the prophylactic use of corticosteroids before planned full-term cesarean sections. However, the association between dexamethasone administration before full-term cesarean delivery and short-term adverse neonatal outcomes is unclear. This study analyzed the disparities in short-term adverse neonatal effects in neonates born via full-term elective cesarean delivery with or without antenatal dexamethasone treatment. Study design: This single-center retrospective cohort study involved neonates aged 37–39 weeks. The primary neonatal outcomes included various short-term adverse events, including neonatal admission to the neonatal intensive care unit, neonatal access to the special care baby unit, transient neonatal respiratory distress, respiratory distress syndrome, and the requirement of intravenous antibiotics or ventilatory support. Multiple logistic regression analysis was used to assess the association between these outcomes and dexamethasone exposure while adjusting for covariates. Results: Of the 543 neonates included in the study, 121 (22.2%) had been exposed to prenatal dexamethasone. When compared with the control group, the dexamethasone-exposed group exhibited significantly higher rates of transient neonatal respiratory distress, respiratory distress syndrome, administration of intravenous antibiotics, the need for ventilatory support, and longer duration of neonatal hospitalization ( P < 0.05). The association between dexamethasone exposure and short-term adverse neonatal outcomes remained significant after adjusting for potential confounders (odds ratio: 12.76, 95% confidence interval: 6.9–23.62, P < 0.001). Conclusion: The dexamethasone-exposed group had a higher likelihood of experiencing short-term adverse outcomes when compared with non-exposed neonates, suggesting that dexamethasone may have detrimental effects on infants delivered at full term. This implies the importance of exercising caution when contemplating the use of antenatal corticosteroids.

Abstract:BACKGROUND: It has been well recognized that antenatal administration of dexamethasone to pregnant women at risk of preterm delivery may markedly accelerate fetal maturation and reduce the risk of adverse perinatal outcomes in their preterm infants, particularly for births before 34 weeks of gestation. Since 2015, antenatal corticosteroid administration has been extended beyond 34 weeks of gestation by clinical guidelines, as it might have beneficial effects on fetal maturation and perinatal outcomes. However, concerns regarding the potential influence of antenatal corticosteroid treatment on offspring neurodevelopment have been raised. OBJECTIVE: This study aimed to investigate whether maternal antenatal corticosteroid administration was associated with neurodevelopment in infants at 1 year of age. STUDY DESIGN: In this prospective and longitudinal birth cohort study, women were followed up throughout gestation, and their infants underwent a Bayley Scales of Infant and Toddler Development, Third Edition, screening test at 1 year of age between December 2018 and September 2020. Finally, 1609 pregnant women and 1759 infants were included in the current study. Using a generalized linear mixed model, we examined the association between antenatal corticosteroid exposure and infant neurodevelopment in cognitive, receptive communication, expressive communication, fine motor, and gross motor functions. RESULTS: Of the 1759 infants eligible for this study, 1453 (82.6%) were singletons. A total of 710 infants were exposed to antenatal corticosteroids, among whom 415 were dexamethasone exposed and 483 were prednisone exposed. Dexamethasone was prescribed most often in late pregnancy, whereas prednisone was often used before 8 weeks of gestation among women who conceived through assisted reproductive technology. Compared with those who had no exposure, antenatal corticosteroid exposure was associated with an increased risk of infants being noncompetent in the cognitive development domain after adjusting for conventional risk factors (adjusted risk ratio, 1.53; 95% confidence interval, 1.08-2.18; P=.017). For medication-specific exposure, those exposed vs not exposed to antenatal dexamethasone were 1.62-fold (95% confidence interval, 1.10-2.38; P=.014) more likely to be noncompetent in the cognitive development domain at 1 year. The association did not vary markedly between preterm and term infants, singletons and twins, or assisted reproductive technologyeconceived and spontaneously conceived infants (all P>.05 for heterogeneity). In contrast, a null association was observed for the risk of being noncompetent in any domain of neurodevelopment with antenatal prednisone exposure at early pregnancy. CONCLUSION: Here, antenatal corticosteroid, particularly dexamethasone exposure, was markedly associated with an increased risk of infants being noncompetent in the cognitive development domain at 1 year of age. These findings may provide new information when weighing the benefits and potential risks of maternal antenatal corticosteroid administration.
Effects of antenatal dexamethasone administration in the infant: long-term follow-up

Abstract:Follow-up evaluations during the first 3 years of life (at 9, 18, and 36 months) were carried out in infants born to women enrolled in a double-blind, randomized trial to evaluate the efficacy and safety of antenatal dexamethasone administration for the prevention of respiratory distress syndrome. Of 739 infants born to 678 mothers, 93 (12.7%), equally distributed between placebo and steroid groups had died, and 240 (32.5%) were lost to follow-up at the end of 3 years. Weight and height at 18 and 36 months, respectively, were slightly greater in infants in the steroid group. No statistically significant differences were observed between the placebo and steroid groups with regard to head circumference and neurologic abnormalities. The Mental Development Index and the Psychomotor Development Index of the Bayley Scales at 9 and 18 months, and the General Cognitive Index or any subscales of the McCarthy Scales of Children's Abilities at 36 months also indicated comparable development within the two groups. No treatment effect was observed on developmental outcomes in boys or girls, in different racial groups, or in different socioeconomic groups. As might be expected, however, MDI, PDI and GCI were significantly influenced by socioeconomic levels. It is concluded that no detectable growth, physical, motor, or developmental deficiencies within the first 3 years of life can be attributed to antenatal dexamethasone therapy in the mothers just prior to delivery.
A Retrospective Analysis of the Effect of Antenatal Corticosteroids Administration on the Incidence of HMD in Preterm Neonates

Abstract:Objective To explore the effect of antenatal corticosteroids administration on the incidence of hyaline membrane disease(HMD) in preterm neonates. Methods Maternal and neonatal medical records were reviewed between 28～36 weeks gestation over the period of January 1, 1995 to August 31, 1998. One hundred and twenty cases received steroid treatment, 60 cases did not. The incidences of HMD in two groups were compared, Alveolo arterial oxygen partial pressure difference (Aa DO 2) and respiratory index (RI) were measured. Results (1) The statistics showed some differences in the incidences of HMD in the optimal steroid compared to the group without the steroid treatment, the incidence of HMD in the optimal was reduced from 57.1% to 16.7%. (2) Aa DO 2 and RI of preterm neonates with HMD in optimal steroid group were (227±60) mmHg and 5±4, and were significantly lower than those without the treatment, (347±126)mmHg and 9±6. (3) The therapy with corticosteroids showed a reduction in the incidence of HMD in preterm multiple pregnancy. The incidences of HMD in the two groups with and without steroid treatments were 0% and 66.7%, respectively. In the optimal steroid group the incidence of HMD in female infants was 0%, which was lower than that in male infants, 10.3%. Conclusions Antenatal corticosteroids did not only reduce the incidence and severity of HMD, but also had effects on multiple pregnancy. The sensitivity of antenatal corticosteroids in female preterm neonates was higher than that in male preterm neonates.
Multi-organ developmental toxicity and its characteristics in fetal mice induced by dexamethasone at different doses, stages, and courses during pregnancy

Abstract:Dexamethasone is widely used in pregnant women at risk of preterm birth to reduce the occurrence of neonatal respiratory distress syndrome and subsequently reduce neonatal mortality. Studies have suggested that dexamethasone has developmental toxicity, but there is a notable absence of systematic investigations about its characteristics. In this study, we examined the effects of prenatal dexamethasone exposure (PDE) on mother/fetal mice at different doses (0.2, 0.4, or 0.8 mg/kg b.i.d), stages (gestational day 14–15 or 16–17) and courses (single- or double-course) based on the clinical practice. Results showed that PDE increased intrauterine growth retardation rate, and disordered the serum glucose, lipid and cholesterol metabolic phenotypes, and sex hormone level of mother/fetal mice. PDE was further discovered to interfere with the development of fetal lung, hippocampus and bone, inhibits steroid synthesis in adrenal and testis, and promotes steroid synthesis in the ovary and lipid synthesis in the liver, with significant effects observed at high dose, early stage and double course. The order of severity might be: ovary > lung > hippocampus/bone > others. Correlation analysis revealed that the decreased serum corticosterone and insulin-like growth factor 1 (IGF1) levels were closely related to PDE-induced low birth weight and abnormal multi-organ development in offspring. In conclusion, this study systematically confirmed PDE-induced multi-organ developmental toxicity, elucidated its characteristics, and proposed the potential "glucocorticoid (GC)-IGF1" axis programming mechanism. This research provided an experimental foundation for a comprehensive understanding of the effect and characteristics of dexamethasone on fetal multi-organ development, thereby guiding the application of "precision medicine" during pregnancy.

Association of different doses of antenatal corticosteroids exposure with early major outcomes and early weight loss percentage in extremely preterm infants or extremely low birthweight infants: a multicentre cohort study

Abstract:Objectives: To determine the dose-dependent associations between antenatal corticosteroids (ANS) exposure and the rates of major morbidities, and the early weight loss percentage (EWLP) in hospital among extremely preterm infants (EPI) or extremely low birthweight infants (ELBWI). Methods: A multicentre, retrospective cohort study of EPI or ELBWI born between 2017 and 2018 was conducted. Infants were classified into no ANS, partial ANS and complete ANS exposure group; three subgroups were generated by gestational age and birth weight. Multiple logistic regression and multiple linear regression were performed. Results: There were 725 infants included from 32 centres. Among no ANS, partial ANS and complete ANS exposure, there were significant differences in the proportions of bronchopulmonary dysplasia (BPD) (24.5%, 25.4% and 16.1%), necrotising enterocolitis (NEC) (6.7%, 2.0% and 2.0%) and death (29.6%, 18.5% and 13.5%), and insignificant differences in the proportions of intraventricular haemorrhage (IVH) (12.5%, 13.2% and 12.2%), and extrauterine growth restriction (EUGR) (50.0%, 56.6% and 59.5%). In the logistic regression, compared with no ANS exposure, complete ANS reduced the risk of BPD (OR 0.58, 95% CI 0.37 to 0.91), NEC (OR 0.21, 95% CI 0.08 to 0.57) and death (OR 0.36, 95% CI 0.23 to 0.56), and partial ANS reduced the risk of NEC (OR 0.23, 95% CI 0.07 to 0.72) and death (OR 0.54, 95% CI 0.34 to 0.87). Compared with partial ANS exposure, complete ANS decreased the risk of BPD (OR 0.58, 95% CI 0.37 to 0.91). There were insignificant associations between ANS exposure and IVH, EUGR. In the multiple linear regression, partial and complete ANS exposure increased EWLP only in the ≥28 weeks (w) and <1000 g subgroup (p<0.05). Conclusions: Different doses of ANS (dexamethasone) exposure were protectively associated with BPD, NEC, death in hospital, but not EUGR at discharge among EPI or ELBWI. Beneficial dose-dependent associations between ANS (dexamethasone) exposure and BPD existed. ANS exposure increased EWLP only in the ≥28 w and<1000 g subgroup. ANS administration, especially complete ANS, is encouraged before preterm birth. Trial registration number: NCT06082414.
Effect of Low Dose Glucocorticoid Inhalation on Bronchopulmonary Dysplasia in Premature Infants

Abstract:The aim of the study was to explore the effect of low dose glucocorticoid on bronchopulmonary dysplasia in premature infants, to provide new ideas for clinical prevention and cure of bronchopulmonary dysplasia in premature infants. The 144 cases of premature infants were divided into 72 each: control group and experimental group. Control group received routine clinical prevention and cure, while experimental group was received low dose glucocorticoid on the basis of control group. The serum interleukin-10 (IL-10) , interleukin-8 (IL-8), and transforming growth factor-1 (TGF-β1) before and after treatment were compared between two groups. The incidence and severity of bronchopulmonary dysplasia was compared between two groups. The mechanical ventilation time, oxygen inhalation time and hospitalization time in two groups were recorded, and the body mass, head circumference and body length at 30 days after birth were assessed in both groups. After treatment, the serum IL-10 level in experimental group was increased and IL-8, TGF-β1 levels were decreased compared with control group (p <0.05). The incidence rate of bronchopulmonary dysplasia in experimental group was 13.89% and the disease severity in experimental group was significantly reduced (p 0.05). Low-dose glucocorticoids have a significant preventive and therapeutic effect on bronchopulmonary dysplasia in preterm infants, and have a high safety, showing high clinical application value for bronchopulmonary dysplasia in preterm infants. Abstract:Introduction: Despite widespread use, many herbicides and fungicides are not well studied for neurological effects. Fetal and infant brains are rapidly developing, yet the effects of early-life exposure to these classes of pesticides on visual and auditory function are unknown. Here we examined the effects of prenatal herbicide and fungicide exposure on infant grating visual acuity (VA) and auditory brainstem response (ABR). Methods: 9 herbicides and 13 fungicides were measured in umbilical cord blood plasma from a cohort of infants in Fuyang County, China (n = 232). Grating VA and ABR latencies for waves I, III, V were measured at 3 time points: 6 weeks, 9 months, and 18 months. Outcomes included VA score, ABR wave V latency and ABR central conduction time (CCT [wave V - wave I]). Pesticides were analyzed as 3-level ordinal (non-detect [ND]/medium/high), or dichotomous (ND/detect), depending on detection rates. Linear mixed models were used to evaluate relations between pesticides and VA and ABR outcomes. Results: 2,4-dichloroacetic acid (2,4-D), prometryn, simazine, and tetrahydrophthalamide (THPI, a metabolite of captan) were detected in 27%, 81%, 17%, and 16% of samples, respectively. Infants prenatally exposed to 2,4-D had slower auditory response times at 6 weeks. Infants with cord levels of 2,4-D > 1.17 ng/mL had wave V latencies that were 0.12 (95% CI: 0.03, 0.22) ms slower (p = 0.01) and overall CCTs that were 0.15 (95% CI:0.05, 0.25) ms slower (p = 0.003) than infants with non-detectable 2,4-D in their cord blood. No other statistically significant findings were observed for the other herbicides and fungicides or for the grating VA outcome. Conclusions: Prenatal exposure to the herbicide 2,4-D was associated with slower auditory signal transmission in early infancy. ABR latencies reflect auditory pathway maturation and longer latencies may indicate delayed auditory development.
[Preliminary Study on Intratympanic Dexamethasone Injection for Management of Patients with Profound Sudden Hearing Loss].

Abstract:OBJECTIVE:To evaluate the effects of intratympanic dexamethasone injection on patients with profound sudden hearing loss.METHODS:All patients in the present study were profound sudden hearing loss, with initial hearing loss of more than 90 dB, but no previous interventions was conducted on the 78 patients within 2 weeks onset of sudden hearing loss. Patients were assigned to 3 groups according to patients' choice, Group I (local and general dexamethasone administration, 22 cases), Group II (Intravenous dexamethasone injection, 44 cases), Group III (intratympanic dexamethasone injection, 12 cases). In addition, vessel dilation drugs, neurotrophic and hyperbaric oxygen therapy were also conducted on all patients. Intravenous dexamethasone was applied 15 mg/dx3 d, 10 mg/dx3 d and 5 mg/dx3 d, respectively. Intratympanic dexamethasone (5 mg/ml, 0.8 ml) injection was performed during 10 days (1 injection/2 days). Pure tone test was conducted on 10th, 20th, and 30th day after intervention.RESULTS:The factors which may impact on the prognosis were matched in all three groups. The threshold improvement more than 30 dB was 81.82% in group I, 83.3% in group II and 88.64% in group III. Statistical study showed there was no significant different among 3 groups (P=0.726). On the 30th day after intervention, pure tone threshold improvement was 41.36 dB in group I (local and general dexamethasone administration), 43.08 dB in group II (intravenous dexamethasone injection) and 51.70 dB in group III (intratympanic dexamethasone injection). Furthermore, pure tone threshold improvement among the 3 groups was no statistical different (F=1.58, P=0.2133). Obvious hearing improvement was noted on the 10th day after intervention, but no further improvement showed after 20 days intervention. More hearing improvement was revealed in the low frequency, while less hearing improvement was achieved in the high frequency.CONCLUSIONS:Comparison with intravenous dexamethasone injection, intratympanic dexamethasone injection did not provide more hearing improvement on patients with profound sudden hearing loss.

Abstract:Dexamethasone is frequently prescribed for preterm infants to wean from respiratory support and/or to facilitate extubation. This pre-/postintervention prospective study ascertained the impact on clinical (respiratory support) and echocardiographic parameters after dexamethasone therapy in preterm fetal growth restriction (FGR) infants compared with appropriate for gestational age (AGA) infants. Echocardiography was performed within 24 h before the start and after completion of 10-day therapy. Parameters assessed included those reflecting pulmonary vascular resistance and right ventricular output. Seventeen FGR infants (birth gestation and birth weight, 25.2 ± 1.1 wk and 497 ± 92 g, respectively) were compared with 22 AGA infants (gestation and birth weight, 24.5 ± 0.8 and 663 ± 100 g, respectively). Baseline respiratory severity score (mean airway pressure × fractional inspired oxygen) was comparable between the groups, (median [interquartile range] FGR, 10 [6, 13] vs. AGA, 8 ± 2.8, P = 0.08). Pre-dexamethasone parameters of pulmonary vascular resistance (FGR, 0.19 ± 0.03 vs. AGA, 0.2 ± 0.03, P = 0.16) and right ventricular output (FGR, 171 ± 20 vs. 174 ± 17 mL/kg/min, P = 0.6) were statistically comparable. At post-dexamethasone assessments, the decrease in the respiratory severity score was significantly greater in AGA infants (median [interquartile range] FGR, 10 [6, 13] to 9 [2.6, 13.5], P = 0.009 vs. AGA, 8 ± 2.8 to 3 ± 1, P < 0.0001). Improvement in measures of pulmonary vascular resistance (ratio of time to peak velocity to right ventricular ejection time) was greater in AGA infants (FGR, 0.19 ± 0.03 to 0.2 ± 0.03, P = 0.13 vs. AGA 0.2 ± 0.03 to 0.25 ± 0.03, P < 0.0001). The improvement in right ventricular output was significantly greater in AGA infants (171 ± 20 to 190 ± 21, P = 0.014 vs. 174 ± 17 to 203 ± 22, P < 0.0001). This highlights differential cardiorespiratory responsiveness to dexamethasone in extremely preterm FGR infants, which may reflect the in utero maladaptive state.NEW & NOTEWORTHY Dexamethasone (DEX) is frequently used in preterm infants dependent on ventilator support. Differences in vascular structure and function that may have developed prenatally arising from the chronic intrauterine hypoxemia in FGR infants may adversely affect responsiveness. The clinical efficacy of DEX was significantly less in FGR (birth weight < 10th centile) infants, compared with appropriate for gestational age (AGA) infants. Echocardiography showed significantly less improvement in pulmonary vascular resistance in FGR, compared with AGA infants.

Abstract:Dexamethasone has been widely used in clinical practice to promote fetal lung maturity and reduce neonatal respiratory distress syndrome and perinatal mortality. Nevertheless, its administration is a double-edged sword, as a large number of studies have shown that there are obvious disadvantages in pregnant women and fetal development. In this review, we comprehensively retrospect the latest literature on the toxicological effects and mechanisms of dexamethasone on fetal development, in an attempt to provide a valuable basis for further studies and clinical trials in the future. Overall, prenatal dexamethasone exposure could lead to some adverse consequences on fetal organ systems through intrauterine programming based on the results of current animal and human researches. Potential sequelae include osteoarthritis, hypertension, fatty liver, glomerulosclerosis, depression, diabetes and infertility, some of which can pass on to the next generation. It must be noted that the evidence in humans is preliminary and limited by the small sample size. More studies in large-scale populations are needed to confirm if it raises the risk of sequelae in humans. In addition, we strongly support the application of dexamethasone as a pharmaceutical therapy in pregnant women with coronavirus disease 2019 before a better therapy is developed. However, the adverse side effects that may arise also cannot be ignored.

[Multi-center comparative study of methylprednisolone and dexamethasone in treatment of all-frequency sudden hearing loss].

Abstract:OBJECTIVE:Through a prospective cohort study, to assess the clinical efficacy of methylprednisolone (MP) and dexamethasone (DXM) in treatment of all-frequency sudden hearing loss. METHODS:A total of 76 cases of all-frequency sudden hearing loss were included in this study and divided into two groups. The MP group (n = 40) was treated with MP 40 mg qd, for 5 days, combined with conventional treatment. The DXM group (n = 36) was treated with DXM 10 mg qd, for 5 days, combined with conventional treatment. The total period of treatment was 14 days. RESULTS:After the treatment for 14 days, in the MP group,17 cases were cured (42.5%), 7 cases were markedly improved (17.5%), 9 cases were effective (22.5%), and 7 cases were invalid (17.5%), the total effective rate was 82.5%. As for the patients in the DXM group, 13 cases were cured (36.1%), 6 cases were markedly improved (16.7%), 8 cases were effective (24.2%), and 9 cases were invalid (25%), the total effective rate was 75.0%. The pure tone audiometry in all-frequency was improved (31.5 ± 17.8) dB in the MP group, and (33.1 ± 24.2) dB in the DXM group. The speech recognition rate was improved (41.7 ± 29.8) %, and (42.0 ± 39.1) % in the DXM group. There were no significant differences between two groups. CONCLUSION:There is no significant difference of therapeutic efficacy between the low-dose MP group and High-dose DXM group.

Abstract:Objective: To study the sensitivity and specificity of targeted neonatal hearing screening for the single-session distortion product otoacoustic emissions (DPOAE) technique and the combined DPOAE/automated auditory brain-stem response (AABR) technique.Methods: 3000 high-risk newborns were studied at Children's Hospital of Fudan University. They were required to take two different screening procedures separately. The first procedure consisted of DPOAE alone and the second consisted of DPOAE combined with the AABR. Based upon the etiology in high-risk babies, they were divided into four groups. In group I there were 670 very-low-birth-weight (VLBW) newborns (1340 ears), and in group II there were 890 preterm babies (1780 ears). 850 babies (1700 ears) suffered from hyperbilirubinemia in group III, whereas 790 babies (1580 ears) with asphyxia were in group IV. The babies in groups II, Ill, and IV came from the neonatal intensive-care unit (NICU) of our hospital. The study protocols consisted of the DPOAE alone and DPOAE combined with AABR hearing screening at an age of less than 1 month, and a diagnostic stage at the age of 2 months.Results: With single-session DPOAE screening, the referral rate (8% of the NICU babies), the false-positive rate (4.96%) and the false-negative rate (0.8%) were higher. The different etiologies in NICU babies had significantly different referral rates (F-test, p < 0.01). A 4.46% referral rate of hyperbilirubinemi babies was much lower. The combined DPOAE/AABR screening technique revealed a referral rate of 5.03%, a false-positive rate of 2% and a false-negative rate of 0.06%. The false-positive rate was well below the suggested 3% of the American Academy of Pediatric. Comparisons of the referral rate, false-positive rate and false-negative rate of two hearing screening protocols (DPOAE alone and combined DPOAE/AABR) revealed significant differences (t-test, p < 0.05, p < 0.01, p < 0.01). 91 infants (3.03% of the NICU babies) who failed the combined DPOAE/AABR screening were confirmed on hearing impairment. Of 22 babies who passed DPOAE screening but failed the AABR screening had a severe to profound hearing loss based on classic ABR. These patients (24% of the NICU babies with hearing losses) with hyperbilirubinemia and asphyxia problems at newborn stage were diagnosed as auditory neuropathy based on evaluations of DPOAE screening passed, abnormal ABR and absent middle-ear muscle reflexes (MMR).Conclusion: Our study demonstrates the use of a combination of DPOAE and AABR testing ensures high sensitivity and acceptable specificity, and predict the AN profile in NICU babies. Our efforts identified 22 NICU babies with auditory neuropathy who hopefully will benefit from early remediation of their hearing deficit. (C) 2011 Elsevier Ireland Ltd. All rights reserved.
Effects of Intra-Amniotic and Maternal Intramuscular Injection of Glucocorticoid on Fetal Lung Maturity and Blood Glucose Level in Women with Gestational Diabetes Mellitus

Abstract:Objective To compare the effects of intra-amniotic and maternal intramuscular injection of glucocorticoid on fetal lung maturity and blood glucose level in gestational diabetes mellitus (GDM) and pregestational diabetes mellitus (PGDM).Methods A total of 60 GDM and PGDM women,whose blood glucose control was inadequate and who needed glucocorticoid treatment for fetal lung maturity,were selected in Peking University First Hospital from January 2013 to April 2015.They received intramuscular (29 women) or intraamniotic (31 women) injection of dexamethasone.Amniotic fluid samples were collected from the two groups during cesarean section for the oscillation test and for determining the surfactant protein-A level.Outcome of the neonates was followed up.Their blood glucose levels and after treatment,fetal lung maturity and neonatal outcomes were analyzed and compared.Two-sample t test,sum-rank,Chi-square and Fisher exact tests were used for statistical analysis.Results (1) Before treatment,the intra-amniotic injection group had a higher preprandial blood glucose level than intramuscular injection group [(5.8 ± 0.6) vs (5.3 ± 0.7) mmol/L,t=-2.730,P=0.008].On the first day after treatment,preprandial blood glucose level in intramuscular injection group was (6.5 ±0.8) mmol/L,and elevated by (1.2±0.9) mmol/L compared with before treatment;preprandial blood glucose in intra amniotic injection group was (6.7± 1.3) mmol/L,and increased by (1.1 ± 1.2) mmol/L.There was no statistical difference in the increment of preprandial blood glucose between the two groups (t=0.548,P=0.586).On the second day after treatment,preprandial blood glucose in intramuscular injection group was higher than before treatment (t=9.070,P=0.000),while preprandial blood glucose in intra-amniotic injection group restored to the pre-treatment level (t=0.516,P=0.612),and preprandial blood glucose in intramuscular injection group was higher than in intra-amniotic injection group [(6.9±0.9) vs (5.6±0.8) mmol/L,t=4.676,P=0.000].On the third day after treatment,preprandial blood glucose in intramuscular injection group decreased to (5.7±0.4) mmol/L,but was still higher than before treatment (t=3.000,P=0.034),while there were no differences in intra-amniotic injection group compared with the level before treatment [(5.9±2.0) vs (5.8± 0.6) mmol/L,t=-0.226,P=0.826].The preprandial and postprandial blood glucose showed a similar trend of change in the two groups.(2) There were no differences in the mature rate [86％ (24/28) vs 88％ (23/26),Fisher exact test] by the oscillation test and surfactant protein-A level [(1 489.1 ±669.3) vs (1 495.2±535.7) μ g/L,t=-0.037],the gender ratio,birth weight,and incidences of macrosomia,neonatal asphyxia,hypoglycemia and hyperbilirubinemia between intramuscular injection group and intra-amniotic injection group (all P＞0.05).No neonatal respiration distress syndrome occurred in the two groups.Conclusions Intra-amniotic injection and intramuscular injection of dexamethasone have the same effects on elevating maternal blood glucose level,but the duration of the effects of intra-amniotic injection is shorter.

Abstract:Objective To assess the efficacy and safety of prenatal dexamethasone treatment in offspring at risk for congenital adrenal hyperplasia. Methods MEDLINE, EMBASE, the Cochrane Library, the clinicaltrials.gov website databases were systematically searched from inception through March 2019. WMD and SMD with 95%CIs were calculated using random or fixed effects models. Results There was a significant reduction in virilization in the DEX-treated group (WMD: -2.39, 95%CI: -3.31,-1.47). No significant differences were found in newborn physical outcomes for birth weight (WMD: 0.09, 95%CI: -0.09, 0.27) and birth length (WMD = 0.27, 95%CI: -0.68, 1.21). Concerning cognitive functions, no significant differences in the domains of psychometric intelligence (SMD: 0.05, 95%CI: -0.74, 0.83), verbal memory (SMD: -0.17, 95%CI: -0.58, 0.23), visual memory (SMD: 0.10, 95%CI: -0.14, 0.34), learning (SMD: -0.02, 95%CI: -0.27, 0.22) and verbal processing (SMD: -0.38, 95%CI: -0.93, 0.17). Regarding behavioural problems, no significant differences in the domains of internalizing problems (SMD: 0.16, 95%CI: -0.49, 0.81), externalizing problems (SMD: 0.07, 95%CI: -0.30, 0.43) and total problems (SMD: 0.14, 95%CI: -0.23, 0.51). With respect to temperament, no significant differences in the domains of emotionality (SMD: 0.13, 95%CI: -0.79, 1.05), activity (SMD: 0.04, 95%CI: -0.32, 0.39), shyness (SMD: 0.25, 95%CI: -0.70, 1.20) and sociability (SMD: -0.23, 95%CI: -0.90, 0.44). Conclusions Prenatal DEX treatment reduced virilization with no significant differences in newborn physical outcomes, cognitive functions, behavioural problems and temperament. The results need to be interpreted cautiously due to the existence of limitations.
