Abstract:Background Microvascular invasion (MVI) has been shown to be closely associated with postoperative recurrence and metastasis in patients with intrahepatic cholangiocarcinoma (ICC). We aimed to develop a radiomics prediction model based on contrast-enhanced CT (CECT) to distinguish MVI in patients with mass-forming ICC. Methods 157 patients were included and randomly divided into training (n=110) and test (n=47) datasets. Radiomic signatures were built based on the recursive feature elimination support vector machine (Rfe-SVM) algorithm. Significant clinical-radiologic factors were screened, and a clinical model was built by multivariate logistic regression. A nomogram was developed by integrating radiomics signature and the significant clinical risk factors. Results The portal phase image radiomics signature with 6 features was constructed and provided an area under the receiver operating characteristic curve (AUC) of 0.804 in the training and 0.769 in the test datasets. Three significant predictors, including satellite nodules (odds ratio [OR]=13.73), arterial hypo-enhancement (OR=4.31), and tumor contour (OR=4.99), were identified by multivariate analysis. The clinical model using these predictors exhibited an AUC of 0.822 in the training and 0.756 in the test datasets. The nomogram combining significant clinical factors and radiomics signature achieved satisfactory prediction efficacy, showing an AUC of 0.886 in the training and 0.80 in the test datasets. Conclusions Both CECT radiomics analysis and radiologic factors have the potential for MVI prediction in mass-forming ICC patients. The nomogram can further improve the prediction efficacy.
Delta-Radiomics Based on Dynamic Contrast-Enhanced MRI for Predicting Lymphovascular Invasion in Invasive Breast Cancer

Abstract:Rationale and Objectives Treatment strategies for invasive breast cancer require accurate lymphovascular invasion (LVI) predictions. This study aimed to investigate the effectiveness of delta radiomics based on dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) for assessing LVI and develop a nomogram to aid treatment decisions. Materials and Methods Overall, 293 patients with resectable invasive breast cancer underwent preoperative DCE-MRI. Radiomic features were extracted from pre-contrast (A0), first post-contrast (A1), and subtracted images of A0 and A1. Three radiomics models were developed using several data analyses; logistic analyses were performed to identify radiological features to predict the LVI status. A hybrid model integrating both radiological features and optimal radiomics was developed. Receiver operating characteristic analysis was employed to evaluate model performance, using the area under the curve (AUC) as a quantitative metric for discriminative ability. Results In the test set, the Radiomics-Delta model, with 17 radiomic features, had an AUC of 0.781 and accuracy of 0.705. Radiomics-A0, with 10 features, had an AUC of 0.619 and accuracy of 0.523, while Radiomics-A1, with 8 features, had an AUC of 0.715 and accuracy of 0.591. The hybrid model exhibited better performance, with an AUC of 0.868 and accuracy of 0.875, than the radiological and Radiomics-Delta models, with an AUC of 0.759 and 0.781, respectively, and accuracy of 0.773 and 0.705, respectively. Conclusion Compared to Radiomics-A0 and Radiomics-A1, Radiomics-Delta demonstrated superior performance. Moreover, the hybrid model incorporating Radiomics-Delta and radiological features exhibited excellent performance in determining the LVI status in cases of invasive breast cancer.

Abstract:ObjectiveTo develop a multiparametric MRI-based radiomics nomogram for predicting lymphovascular invasion (LVI) status and clinical outcomes in patients with breast invasive ductal carcinoma (IDC).MethodsA total of 160 patients with pathologically confirmed breast IDC (training cohort: n = 112; validation cohort: n = 48) who underwent preoperative breast MRI were included. Imaging features were extracted from T2-weighted imaging (T2WI), apparent diffusion coefficient (ADC) maps, and contrast-enhanced T1-weighted imaging (cT1WI) sequences. A four-step procedure was applied for feature selection and radiomics signature building. Univariate and multivariate logistic regression analyses were conducted to identify the features associated with LVI, which were then incorporated into the radiomics nomogram. The performance of the nomogram was evaluated by its discrimination, calibration, and clinical usefulness. Kaplan–Meier survival curves based on the two radiomics models were used to estimate disease-free survival (DFS).ResultsThe fusion radiomics signature of the T2WI, cT1WI, and ADC maps achieved a better predictive efficacy for LVI than either of them alone. The proposed radiomics nomogram, incorporating the fusion radiomics signature and MRI-reported peritumoral edema, showed satisfactory capabilities of calibration and discrimination in both training and validation datasets, with AUCs of 0.919 (95% CI: 0.871–0.967) and 0.863 (95% CI: 0.726–0.999), respectively. The radiomics signature and nomogram-defined high-risk groups had a shorter DFS than those in the low-risk groups (both p < 0.05). Higher Rad-scores were independently associated with a worse DFS in the whole cohort (p < 0.05).ConclusionsThe proposed nomogram, incorporating multiparametric MRI-based radiomics signature and MRI-reported peritumoral edema, achieved a satisfactory preoperative prediction of LVI and clinical outcomes in IDC patients.Key Points• The fusion radiomics signature of the T2WI, cT1WI, and ADC maps achieved a better predictive efficacy for LVI than either of them alone.• The proposed nomogram achieved a favorable prediction of LVI in IDC patients with AUCs of 0.919 and 0.863 in the training and validation datasets, respectively.• The radiomics model could classify patients into high- and low-risk groups with significant differences in DFS.

Abstract:Rationale and objectives: Dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) radiomics had been used to evaluate lymphovascular invasion (LVI) in patients with breast cancer. However, no studies had explored the associations between features from delayed phase as well as multiphases DCE-MRI and the LVI status. Thus, we aimed to develop an efficient nomogram based on multiphases DCE-MRI to predict the LVI status in invasive (IBC) breast cancer patients. Materials and methods: A retrospective analysis was conducted on preoperative clinical, pathological, and DCE-MRI data of 173 breast cancer patients. All patients were randomly assigned into training set (n=121) and validation set (n=52) in 7:3 ratio. The clinical, pathologic, and conventional MRI characteristics were then subjected to univariate and multivariate logistic regression analysis, and the clinical risk factors with P < 0.05 in the multivariate logistic regression were used to build clinical models. Different single-phase models (early phase, peak phase, and terminal phase), as well as a multiphases model integrating radiomics features from multiple phases, were established. Furthermore, a preoperative radiomics nomogram model was constructed by combining the rad-score of the multiphases model with clinicopathologic independent risk factors. Finally, the performance of the multiphases model, clinical model, and rad-score was compared using receiver operating characteristic (ROC) curves, area under the curve (AUC) values, and decision curve analysis (DCA). The clinical utility of the rad-score was evaluated using calibration curves, and Delong test was used to compare the differences in AUC values among the different models. Results: The axillary lymph nodes (ALN) status and Ki-67 had been identified as clinicopathologic independent predictors and a clinical model had been constructed. Image features that were extracted from the terminal phase of the DCE-MRI exhibited notably superior predictive performances compared to features from the other single phases. Particularly, in the multiphases model, terminal phase features were identified as potentially providing more predictive information. Among the nine features that were found to be associated with LVI in the multiphase model, one was derived from the early phase, two from the peak phase, and six from the terminal phase, indicating that terminal phase features contributed significantly more information towards predicting LVI. Evaluation of the nomogram performance revealed promising results in both the training set (AUCs: clinical model vs. multiphase model vs. nomogram=0.734 vs. 0.840 vs. 0.876) and the validation set (AUCs: clinical model vs. multiphase model vs. nomogram=0.765 vs. 0.753 vs. 0.832). Conclusion: The DCE-MRI-based radiomics model demonstrated utility in predicting LVI status, features of the terminal phase offered more valuable information particularly. The preoperative radiomics nomogram enhanced the diagnostic capability of identifying LVI status in IBC patients, and might aid clinicians in making personalized treatment decisions.
Dynamic Contrast-Enhanced MRI Radiomics Nomogram for Predicting Axillary Lymph Node Metastasis in Breast Cancer

Abstract:Purpose The goal of this study is to develop and validate a radiomics nomogram integrating the radiomics features from DCE-MRI and clinical factors for the preoperative diagnosis of axillary lymph node (ALN) metastasis in breast cancer patients. Procedures A total of 432 patients with breast cancer were enrolled in this retrospective study and divided into a training cohort (n = 296) and a validation cohort (n = 136). Radiomics features were extracted from the second phase of dynamic contrast enhanced (DCE) MRI images. The least absolute shrinkage and selection operator (LASSO) regression method was used to screen optimal features and construct a radiomics signature in the training cohort. Multivariable logistic regression analysis was used to establish a radiomics nomogram model based on the radiomics signature and clinical factors. The predictive performance of the nomogram was quantified with respect to discrimination and calibration, which was further evaluated in the independent validation cohort. Results Fourteen ALN metastasis-related features were selected to construct the radiomics signature, with an area under the curve (AUC) of 0.847 and 0.805 in the training and validation cohorts, respectively. The nomogram was established by incorporating the histological grade, multifocality, MRI report lymph node status and radiomics signature and showed good calibration and excellent performance for ALN detection (AUC of 0.907 and 0.874 in the training and validation cohorts, respectively). The decision curve, which demonstrated the radiomics nomogram, displayed promising clinical utility. Conclusions The radiomics nomogram can be used as a noninvasive and reliable tool to assist clinicians in accurately predicting ALN metastasis in breast cancer preoperatively.

Abstract:PurposeThis study aimed to establish and validate a radiomics nomogram based on dynamic contrast-enhanced (DCE)-MRI for predicting axillary lymph node (ALN) metastasis in breast cancer.MethodThis retrospective study included 296 patients with breast cancer who underwent DCE-MRI examinations between July 2017 and June 2018. A total of 396 radiomics features were extracted from primary tumor. In addition, the least absolute shrinkage and selection operator (LASSO) algorithm was used to select the features. Radiomics signature and independent risk factors were incorporated to build a radiomics nomogram model. Calibration and receiver operator characteristic (ROC) curves were used to confirm the performance of the nomogram in the training and validation sets. The clinical usefulness of the nomogram was evaluated by decision curve analysis (DCA).ResultsThe radiomics signature consisted of three ALN-status-related features, and the nomogram model included the radiomics signature and the MR-reported lymph node (LN) status. The model showed good calibration and discrimination with areas under the ROC curve (AUC) of 0.92 [95% confidence interval (CI), 0.87–0.97] in the training set and 0.90 (95% CI, 0.85–0.95) in the validation set. In the MR-reported LN-negative (cN0) subgroup, the nomogram model also exhibited favorable discriminatory ability (AUC, 0.79; 95% CI, 0.70–0.87). DCA findings indicated that the nomogram model was clinically useful.ConclusionsThe MRI-based radiomics nomogram model could be used to preoperatively predict the ALN metastasis of breast cancer.

Abstract:Background Radiomics has been applied for assessing lymphovascular invasion (LVI) in patients with breast cancer. However, associations between features from peritumoral regions and the LVI status were not investigated. Purpose To investigate the value of intra‐ and peritumoral radiomics for assessing LVI, and to develop a nomogram to assist in making treatment decisions. Study Type Retrospective. Population Three hundred and sixteen patients were enrolled from two centers and divided into training (N = 165), internal validation (N = 83), and external validation (N = 68) cohorts. Field Strength/Sequence 1.5 T and 3.0 T/dynamic contrast‐enhanced (DCE) and diffusion‐weighted imaging (DWI). Assessment Radiomics features were extracted and selected based on intra‐ and peritumoral breast regions in two magnetic resonance imaging (MRI) sequences to create the multiparametric MRI combined radiomics signature (RS‐DCE plus DWI). The clinical model was built with MRI‐axillary lymph nodes (MRI ALN), MRI‐reported peritumoral edema (MPE), and apparent diffusion coefficient (ADC). The nomogram was constructed with RS‐DCE plus DWI, MRI ALN, MPE, and ADC. Statistical Tests Intra‐ and interclass correlation coefficient analysis, Mann–Whitney U test, and least absolute shrinkage and selection operator regression were used for feature selection. Receiver operating characteristic and decision curve analyses were applied to compare performance of the RS‐DCE plus DWI, clinical model, and nomogram. Results A total of 10 features were found to be associated with LVI, 3 from intra‐ and 7 from peritumoral areas. The nomogram showed good performance in the training (AUCs, nomogram vs. clinical model vs. RS‐DCE plus DWI, 0.884 vs. 0.695 vs. 0.870), internal validation (AUCs, nomogram vs. clinical model vs. RS‐DCE plus DWI, 0.813 vs. 0.695 vs. 0.794), and external validation (AUCs, nomogram vs. clinical model vs. RS‐DCE plus DWI, 0.862 vs. 0.601 vs. 0.849) cohorts. Development of an Intratumoral and Peritumoral Radiomics Nomogram Using Digital Breast Tomosynthesis for Preoperative Assessment of Lymphovascular Invasion in Invasive Breast Cancer

Abstract:Rationale and objectives: This study aimed to create a nomogram model that combines clinical factors with radiomics analysis of both intra- and peritumoral regions extracted from preoperative digital breast tomosynthesis (DBT) images, in order to develop a reliable method for predicting the lymphovascular invasion (LVI) status in invasive breast cancer (IBC) patients. Materials and methods: A total of 178 patients were randomly split into a training dataset (N = 124) and a validation dataset (N = 54). Comprehensive clinical data, encompassing DBT features, were gathered for all cases. Radiomics features were extracted and selected from intra- and peritumoral region to establish radiomics signature (Radscore). To construct the clinical model and nomogram model, univariate and multivariate logistic regression analyses were utilized to identify independent risk factors. To assess and validate these models, various analytical methods were employed, including receiver operating characteristic (ROC) curve analysis, calibration curve analysis, decision curve analysis (DCA), net reclassification improvement (NRI), and integrated discriminatory improvement (IDI). Results: The clinical model is constructed based on two independent risk factors: tumor margin and the DBT-reported lymph node metastasis (DBT_reported_LNM). Incorporating Radscore_Combine (utilizing both intra- and peritumoral radiomics features), tumor margin, and DBT_reported_LNM into the nomogram achieved a reliable predictive performance, with area under the curve (AUC) values of 0.906 and 0.905 in both datasets, respectively. The significant improvement demonstrated by the NRI and IDI indicates that the Radscore_Combine could be a valuable biomarker for effectively predicting the status of LVI. Conclusion: The nomogram demonstrated a reliable ability to predict LVI in IBC patients.
Preoperative Prediction of Lymphovascular Space Invasion in Cervical Cancer With Radiomics –Based Nomogram

Abstract:Purpose To build and evaluate a radiomics-based nomogram that improves the predictive performance of the LVSI in cervical cancer non-invasively before the operation. Method This study involved 149 patients who underwent surgery with cervical cancer from February 2017 to October 2019. Radiomics features were extracted from T2 weighted imaging (T2WI). The radiomic features were selected by logistic regression with the least absolute shrinkage and selection operator (LASSO) penalty in the training cohort. Based on the selected features, support vector machine (SVM) algorithm was used to build the radiomics signature on the training cohort. Incorporating radiomics signature and clinical risk factors, the radiomics-based nomogram was developed. The sensitivity, specificity, accuracy, and area under the curve (AUC) and Receiver operating characteristic (ROC) curve were calculated to assess these models. Result The radiomics model performed much better than the clinical model in both training (AUCs 0.925 vs. 0.786, accuracies 87.5% vs. 70.5%, sensitivities 83.6% vs. 41.7% and specificities 90.9% vs. 94.7%) and testing (AUCs 0.911 vs. 0.706, accuracies 84.0% vs. 71.3%, sensitivities 81.1% vs. 43.4% and specificities 86.4% vs. 95.0%). The combined model based on the radiomics signature and tumor stage, tumor infiltration depth and tumor pathology yielded the best performance (training cohort, AUC = 0.943, accuracies 89.5%, sensitivities 85.4% and specificities 92.9%; testing cohort, AUC = 0.923, accuracies 84.6%, sensitivities 84.0% and specificities 85.1%). Conclusion Radiomics-based nomogram was a useful tool for predicting LVSI of cervical cancer. This would aid the selection of the optimal therapeutic strategy and clinical decision-making for individuals.

Abstract:Purpose: The aim of this study was to develop a radiomics nomogram based on grayscale ultrasound (US) for preoperatively predicting Lymphovascular invasion (LVI) in patients with pathologically confirmed T1 (pT1) breast invasive ductal carcinoma (IDC). Methods: One hundred and ninety-two patients with pT1 IDC between September 2020 and August 2022 were analyzed retrospectively. Study population was randomly divided in a 7: 3 ratio into a training dataset of 134 patients (37 patients with LVI-positive) and a validation dataset of 58 patients (19 patients with LVI-positive). Clinical information and conventional US (CUS) features (called clinic_CUS features) were recorded and evaluated to predict LVI. In the training dataset, independent predictors of clinic_CUS features were obtained by univariate and multivariate logistic regression analyses and incorporated into a clinic_CUS prediction model. In addition, radiomics features were extracted from the grayscale US images, and the radiomics score (Radscore) was constructed after radiomics feature selection. Subsequent multivariate logistic regression analysis was also performed for Radscore and the independent predictors of clinic_CUS features, and a radiomics nomogram was developed. The performance of the nomogram model was evaluated via its discrimination, calibration, and clinical usefulness. Results: The US reported axillary lymph node metastasis (LNM) (US_LNM) status and tumor margin were determined as independent risk factors, which were combined for the construction of clinic_CUS prediction model for LVI in pT1 IDC. Moreover, tumor margin, US_LNM status and Radscore were independent predictors, incorporated as the radiomics nomogram model, which achieved a superior discrimination to the clinic_CUS model in the training dataset (AUC: 0.849 vs. 0.747; P < 0.001) and validation dataset (AUC: 0.854 vs. 0.713; P = 0.001). Calibration curve for the radiomic nomogram showed good concordance between predicted and actual probability. Furthermore, decision curve analysis (DCA) confirmed that the radiomics nomogram had higher clinical net benefit than the clinic_CUS model. Conclusion: The US-based radiomics nomogram, incorporating tumor margin, US_LNM status and Radscore, showed a satisfactory preoperative prediction of LVI in pT1 IDC patients.

Abstract:Objective: To explore the application of multiparametric MRI-based radiomic nomogram for assessing HER-2 2+ status of breast cancer (BC). Methods: Patients with pathology-proven HER-2 2+ invasive BC, who underwent preoperative MRI were divided into training (72 patients, 21 HER-2-positive and 51 HER-2-negative) and validation (32 patients, 9 HER-2-positive and 23 HER-2-negative) sets by randomization. All were classified as HER-2 2+ FISH-positive (HER-2-positive) or -negative (HER-2-negative) according to IHC and FISH. The 3D VOI was drawn on MR images by two radiologists. ADC, T2WI, and DCE images were analyzed separately to extract features (n = 1906). L1 regularization, F-test, and other methods were used to reduce dimensionality. Binary radiomics prediction models using features from single or combined imaging sequences were constructed using logistic regression (LR) classifier then and validated on a validation dataset. To build a radiomics nomogram, multivariate LR analysis was conducted to identify independent indicators. An evaluation of the model's predictive efficacy was made using AUC. Results: On the basis of combined ADC, T2WI, and DCE images, ten radiomic features were extracted following feature dimensionality reduction. There was superior diagnostic efficiency of radiomic signature using all three sequences compared to either one or two sequences (AUC for training group: 0.883; AUC for validation group: 0.816). Based on multivariate LR analysis, radiomic signature and peritumoral edema were independent predictors for identifying HER-2 2 +. In both training and validation datasets, nomograms combining peritumoral edema and radiomics signature demonstrated an effective discrimination (AUCs were respectively 0.966 and 0. 884). Conclusion: The nomogram that incorporated peritumoral edema and multiparametric MRI-based radiomic signature can be used to effectively predict the HER-2 2+ status of BC.
Ultrasound-Based Deep Learning Radiomics Nomogram for the Assessment of Lymphovascular Invasion in Invasive Breast Cancer: A Multicenter Study

Abstract:Rationale and Objectives The aim of this study was to develop a deep learning radiomics nomogram (DLRN) based on B-mode ultrasound (BMUS) and color doppler flow imaging (CDFI) images for preoperative assessment of lymphovascular invasion (LVI) status in invasive breast cancer (IBC). Materials and Methods In this multicenter, retrospective study, 832 pathologically confirmed IBC patients were recruited from eight hospitals. The samples were divided into training, internal test, and external test sets. Deep learning and handcrafted radiomics features reflecting tumor phenotypes on BMUS and CDFI images were extracted. The BMUS score and CDFI score were calculated after radiomics feature selection. Subsequently, a DLRN was developed based on the scores and independent clinic-ultrasonic risk variables. The performance of the DLRN was evaluated for calibration, discrimination, and clinical usefulness. Results The DLRN predicted the LVI with accuracy, achieving an area under the receiver operating characteristic curve of 0.93 (95% CI 0.90–0.95), 0.91 (95% CI 0.87–0.95), and 0.91 (95% CI 0.86–0.94) in the training, internal test, and external test sets, respectively, with good calibration. The DLRN demonstrated superior performance compared to the clinical model and single scores across all three sets ( p < 0.05). Decision curve analysis and clinical impact curve confirmed the clinical utility of the model. Furthermore, significant enhancements in net reclassification improvement (NRI) and integrated discrimination improvement (IDI) indicated that the two scores could serve as highly valuable biomarkers for assessing LVI. Conclusion The DLRN exhibited strong predictive value for LVI in IBC, providing valuable information for individualized treatment decisions.

Abstract:Purpose: To develop and validate a multiparametric magnetic resonance imaging (mpMRI)-based radiomics model for predicting lymph-vascular space invasion (LVSI) of cervical cancer (CC). Methods: The data of 177 CC patients were retrospectively collected and randomly divided into the training cohort (n=123) and testing cohort (n = 54). All patients received preoperative MRI. Feature selection and radiomics model construction were performed using max-relevance and min-redundancy (mRMR) and the least absolute shrinkage and selection operator (LASSO) on the training cohort. The models were established based on the extracted features. The optimal model was selected and combined with clinical independent risk factors to establish the radiomics fusion model and the nomogram. The diagnostic performance of the model was assessed by the area under the curve. Results: Feature selection extracted the thirteen most important features for model construction. These radiomics features and one clinical characteristic were selected showed favorable discrimination between LVSI and non-LVSI groups. The AUCs of the radiomics nomogram and the mpMRI radiomics model were 0.838 and 0.835 in the training cohort, and 0.837 and 0.817 in the testing cohort. Conclusion: The nomogram model based on mpMRI radiomics has high diagnostic performance for preoperative prediction of LVSI in patients with CC.

Abstract:Background Assessment of lymphovascular invasion (LVI) in breast cancer (BC) primarily relies on preoperative needle biopsy. There is an urgent need to develop a non‐invasive assessment method. Purpose To develop an effective model to assess the LVI status in patients with BC using magnetic resonance imaging morphological features (MRI‐MF), Radiomics, and deep learning (DL) approaches based on dynamic contrast‐enhanced MRI (DCE‐MRI). Study Type Cross‐sectional retrospective cohort study. Population The study included 206 BC patients, with 136 in the training set [97 LVI(−) and 39 LVI(+) cases; median age: 51.5 years] and 70 in the test set [52 LVI(−) and 18 LVI(+) cases; median age: 48 years]. Field Strength/Sequence 1.5 T/T1‐weighted images, fat‐suppressed T2‐weighted images, diffusion‐weighted imaging (DWI), and DCE‐MRI. Assessment The MRI‐MF model was developed with conventional MR features using logistic analyses. The Radiomic feature extraction process involved collecting data from categorized DCE‐MRI datasets, specifically the first and second post‐contrast images (A1 and A2). Next, a DL model was implemented to determine LVI. Finally, we established a joint diagnosis model by combining the MRI‐MF, Radiomics, and DL approaches. Statistical Tests Diagnostic performance was compared using receiver operating characteristic curve analysis, confusion matrix, and decision curve analysis. Results Rim sign and peritumoral edema features were used to develop the MRI‐MF model, while six Radiomics signature from the A1 and A2 images were used for the Radiomics model. The joint model (MRI‐MF + Radiomics + DL models) achieved the highest accuracy (area under the curve [AUC] = 0.857), being significantly superior to the MRI‐MF (AUC = 0.724), Radiomics (AUC = 0.736), or DL (AUC = 0.740) model. Furthermore, it also outperformed the pairwise combination models: Radiomics + MRI‐MF (AUC = 0.796), DL + MRI‐MF (AUC = 0.796), or DL + Radiomics (AUC = 0.826). Data Conclusion The joint model incorporating MRI‐MF, Radiomics, and DL approaches can effectively determine the LVI status in patients with BC before surgery. Abstract:Background Overtreatment of axillary lymph node dissection (ALND) may occur in patients with axillary positive sentinel lymph node (SLN) but negative non-SLN (NSLN). Developing a magnetic resonance imaging (MRI)-based radiomics nomogram to predict axillary NSLN metastasis in patients with SLN-positive breast cancer could effectively decrease the probability of overtreatment and optimize a personalized axillary surgical strategy. Methods This retrospective study included 285 patients with positive SLN breast cancer. Fifty five of them had metastatic NSLNs and 230 had non-metastatic NSLNs. MRI-based radiomic features of primary tumors were extracted and MRI morphologic findings of the primary tumor and axillary lymph nodes were assessed. Four models, namely, a radiomics signature, an MRI-clinical nomogram, and two MRI-clinical-radiomics nomograms were established based on MRI morphologic findings, clinicopathologic characteristics, and MRI-based radiomic features to predict the NSLN status. The optimal predictors in each model were selected using the 5-fold cross-validation (CV) method. Their predictive performances were determined by the receiver operating characteristic (ROC) curves analysis. The area under the curves (AUCs) of different models was compared by the Delong test. Their discrimination capability, calibration curve, and clinical usefulness were also assessed. Results The 5-fold CV analysis showed that the AUCs ranged from 0.770 to 0.847 for the radiomics signature, from 0.720 to 0.824 for the MRI-clinical nomogram, from 0.843 to 0.932 for the MRI-clinical-radiomics nomogram. The optimal predictive factors in the radiomics signature, MRI-clinical nomogram, and MRI-clinical-radiomics nomogram were one texture feature of diffusion-weighted imaging (DWI), two clinicopathologic features together with one MRI morphologic finding, and the DWI-based texture feature together with the two clinicopathologic features plus the one MRI morphologic finding, respectively. The MRI-clinical-radiomics nomogram with CA 15-3 included achieved the highest AUC compared with the radiomics signature (0.868 vs . 0.806, P < 0.001) and MRI-clinical nomogram (0.868 vs . 0.761; P < 0.001). In addition, the MRI-clinical-radiomics nomogram without CA 15-3 showed a higher performance than that of the radiomics signature (AUC, 0.852 vs . 0.806, P = 0.016) and the MRI-clinical nomogram (AUC, 0.852 vs . 0.761, P = 0.007). The MRI-clinical-radiomics nomograms showed good discrimination and good calibration. Decision curve analysis demonstrated that the MRI-clinical-radiomics nomograms were clinically useful. Conclusion The MRI-clinical-radiomics nomograms developed in our study showed high predictive performance, which can be used to predict the axillary NSLN status in SLN-positive breast cancer patients before surgery.

Abstract:Objective: To develop and validate a radiomics predictive model based on multiparameter MR imaging features and clinical features to predict lymph node metastasis (LNM) in patients with cervical cancer.Material and Methods: A total of 168 consecutive patients with cervical cancer from two centers were enrolled in our retrospective study. A total of 3,930 imaging features were extracted from T2-weighted (T2W), ADC, and contrast-enhanced T1-weighted (cT1W) images for each patient. Four-step procedures, mainly minimum redundancy maximum relevance (MRMR) and least absolute shrinkage and selection operator (LASSO) regression, were applied for feature selection and radiomics signature building in the training set from center I (n = 115). Combining clinical risk factors, a radiomics nomogram was then constructed. The models were then validated in the external validation set comprising 53 patients from center II. The predictive performance was determined by its calibration, discrimination, and clinical usefulness.Results: The radiomics signature derived from the combination of T2W, ADC, and cT1W images, composed of six LN-status-related features, was significantly associated with LNM and showed better predictive performance than signatures derived from either of them alone in both sets. Encouragingly, the radiomics signature also showed good discrimination in the MRI-reported LN-negative subgroup, with AUC of 0.825 (95% CI: 0.732–0.919). The radiomics nomogram that incorporated radiomics signature and MRI-reported LN status also showed good calibration and discrimination in both sets, with AUCs of 0.865 (95% CI: 0.794–0.936) and 0.861 (95% CI: 0.733–0.990), respectively. Decision curve analysis confirmed its clinical usefulness.Conclusion: The proposed MRI-based radiomics nomogram has good performance for predicting LN metastasis in cervical cancer and may be useful for improving clinical decision making.

Abstract:Objective: Preoperative identification of lymphovascular invasion (LVI) in patients with invasive breast cancer is challenging due to absence of reliable biomarkers or tools in clinical settings. We aimed to establish and validate multiparametric magnetic resonance imaging (MRI)-based radiomic models to predict the risk of lymphovascular invasion (LVI) in patients with invasive breast cancer. Methods: This retrospective study included a total of 175 patients with confirmed invasive breast cancer who had known LVI status and preoperative MRI from two tertiary centers. The patients from center 1 was randomly divided into a training set (n=99) and a validation set (n = 26), while the patients from center 2 was used as a test set (n=50). A total of 1409 radiomic features were extracted from the T2-weighted imaging (T2WI), dynamic contrast-enhanced (DCE) imaging, diffusion-weighted imaging (DWI), and apparent diffusion coefficient (ADC), respectively. A three-step feature selection including SelectKBest, interclass correlation coefficients (ICC), and least absolute shrinkage and selection operator (LASSO) was performed to identify the features most associated with LVI. Subsequently, a Support Vector Machine (SVM) classifier was trained to develop single-layer radiomic models and fusion radiomic models. Model performance was evaluated and compared by the area under the curve (AUC), sensitivity, and specificity. Results: Based on one feature of wavelet-HLH_gldm_GrayLevelVariance, the ADC radiomic model achieved an AUC of 0.87 (95% confidence interval [CI]: 0.80–0.94) in the training set, 0.87 (0.70-1.00) in the validation set, and 0.77 (95%CI: 0.64-0.86) in the test set. However, the combination of radiomic features derived from other MR sequences failed to yield incremental value. Conclusions: ADC-based radiomic model demonstrated a favorable performance in predicting LVI prior to surgery in patients with invasive breast cancer. Such model holds the potential for improving clinical decision-making regarding treatment for breast cancer.

Abstract:Background As lymphovascular space invasion (LVSI) was closely related to lymph node metastasis and prognosis, the preoperative assessment of LVSI in early‐stage cervical cancer is crucial for patients. Purpose To develop and validate nomogram based on multimodal MR radiomics to assess LVSI status in cervical cancer patients. Study Type Retrospective. Population The study included 168 cervical cancer patients, of whom 129 cases (age 51.36 ± 9.99 years) from institution 1 were included as the training cohort and 39 cases (age 52.59 ± 10.23 years) from institution 2 were included as the external test cohort. Field Strength/Sequence There were 1.5 T and 3.0 T MRI scans (T1‐weighted imaging [T1WI], fat‐saturated T2‐weighted imaging [FS‐T2WI], and contrast‐enhanced [CE]). Assessment Six machine learning models were built and selected to construct the radiomics signature. The nomogram model was constructed by combining the radiomics signature with the clinical signature, which was then validated for discrimination, calibration, and clinical usefulness. Statistical Tests The clinical characteristics were compared using t‐tests, Mann–Whitney U tests, or chi‐square tests. The Spearman and LASSO methods were used to select radiomics features. The receiver operating characteristic (ROC) analysis was performed, and the area under the curve (AUC), accuracy, sensitivity, and specificity were calculated. Results The logistic regression (LR) model performed best in each sequence. The AUC of CE‐T1‐T2WI‐combined was the highest in the LR model, with an AUC of 0.775 (95% CI: 0.570–0.979) in external test cohort. The nomogram showed high predictive performance in the training (AUC: 0.883 [95% CI: 0.823–0.943]) and test cohort (AUC: 0.830 [95% CI: 0.657–1.000]) for predicting LVSI. Decision curve analysis demonstrated that the nomogram was clinically useful. Data Conclusion Our findings suggest that the proposed nomogram model based on multimodal MRI of CE T1WI‐T2WI‐combined could be used to assess LVSI status in early cervical cancer. Evidence Level 4. Abstract:Purpose To develop a nomogram based on radiomics signature and deep-learning signature for predicting the axillary lymph node (ALN) metastasis in breast cancer. Methods A total of 151 patients were assigned to a training cohort (n = 106) and a test cohort (n = 45) in this study. Radiomics features were extracted from DCE-MRI images, and deep-learning features were extracted by VGG-16 algorithm. Seven machine learning models were built using the selected features to evaluate the predictive value of radiomics or deep-learning features for the ALN metastasis in breast cancer. A nomogram was then constructed based on the multivariate logistic regression model incorporating radiomics signature, deep-learning signature, and clinical risk factors. Results Five radiomics features and two deep-learning features were selected for machine learning model construction. In the test cohort, the AUC was above 0.80 for most of the radiomics models except DecisionTree and ExtraTrees. In addition, the K-nearest neighbor (KNN), XGBoost, and LightGBM models using deep-learning features had AUCs above 0.80 in the test cohort. The nomogram, which incorporated the radiomics signature, deep-learning signature, and MRI-reported LN status, showed good calibration and performance with the AUC of 0.90 (0.85-0.96) in the training cohort and 0.90 (0.80-0.99) in the test cohort. The DCA showed that the nomogram could offer more net benefit than radiomics signature or deep-learning signature. Conclusions Both radiomics and deep-learning features are diagnostic for predicting ALN metastasis in breast cancer. The nomogram incorporating radiomics and deep-learning signatures can achieve better prediction performance than every signature used alone.

Abstract:AIM To investigate the value of multiparametric magnetic resonance imaging (MRI)-based radiomics nomograms for predicting the hormone receptor (HR) status of HER2-positive breast cancer. MATERIALS AND METHODS Patients with HER2-positive invasive breast cancer were divided randomly into training (68 patients) and validation (30 patients) sets. All were classified as either HR-positive (HR+) or negative (HR-) at histopathology. Two radiologists outlined the three-dimensional (3D) volumetric regions of interest (VOI) on the MRI images. Features ( n= 1,096) were extracted from the T2-weighted imaging (WI), apparent diffusion coefficient (ADC), and dynamic contrast-enhanced (DCE) images separately. Dimensionality was reduced using feature screening. Binary radiomics prediction models were established using a logistic regression classifier and were validated in the validation set. To construct a nomogram, independent predictors were identified using multivariate logistic regression analysis. The predictive efficacy of the model was assessed using the area under the receiver operating characteristic curve (AUC). RESULTS Ten radiomics features were obtained after feature dimensionality reduction based on the merged T2WI, ADC, and DCE images. The diagnostic efficacy of the radiomics signature using the three sequences was better than that of any single sequence (training set AUC: 0.797; validation set AUC: 0.75). Using multivariate logistic regression analysis, the independent predictors for identifying HR status were combined radiomics signature and peritumoural oedema. Nomograms constructed by combining the radiomics signature and peritumoural oedema showed good discrimination in both the training and validation sets (AUC: 0.815 and 0. 805, respectively). CONCLUSION A multiparametric MRI-based nomogram incorporating the radiomics signature and peritumoural oedema can assess the HR status of HER2-positive breast cancer. The resulting model can improve diagnostic accuracy, improving patient outcomes.
