Abstract:Background: Plasma concentrations of fluoropyrimidine exhibit a wide interindividual variability that depends mainly on the activity of dihydropyrimidine dehydrogenase, its major catabolic enzyme. Patients with low dihydropyrimidine dehydrogenase activity are at an increased risk of overexposure and often severe, sometimes lethal, toxicity. This study aimed to develop a quick and easy bioanalytical method for the simultaneous determination of endogenous uracil (U), exogenous 5-fluorouracil (5-FU), and their respective 5,6-dihydro-metabolite in human plasma using Liquid chromatography coupled to tandem mass spectrometry (LC-MS/MS). Methods: After protein precipitation, the compounds were purified using liquid-liquid extraction. Chromatographic separation was conducted using a Cortecs T3 column and binary gradient elution. Detection and quantification were performed in the positive electrospray ionization and selected the reaction monitoring mode after 2 transitions per analyte and 1 per internal standard. The data obtained with this technique were retrospectively gathered for uracil metabolism phenotyping before fluoropyrimidine treatment (as enforced by national regulations) in a large group of 526 patients. Results: The analytical response was linear (r > 0.99 for all compounds), and it yielded a lower limit of quantification of 2 ng·mL for U and UH2 as well as 4 ng·mL for 5-FU and 5,6-dihydro-5-FUH2. The median uracil concentration in 526 patients was 10.6 mcg/L, with extreme values of 3.9 and 81.6 mcg/L; 78 patients (15%) had uracil concentration ≥16 mcg/L, that is, above the threshold of decreased enzyme activity and initial dose reduction.
Quantification of uracil, dihydrouracil, thymine and dihydrothymine for reliable dihydropyrimidine dehydrogenase (DPD) phenotyping critically depend on blood and plasma storage conditions

Abstract:Establishing dihydropyrimidine dehydrogenase (DPD) activity is highly important in determining the correct starting dose of fluoropyrimidines such as 5-fluorouracil and capecitabine. The concentration ratio of endogenous uracil with its metabolite dihydrouracil (DHU) is a well-known parameter that is linked to DPD activity. Concentration ratios such as thymine over its DPD-converted metabolite dihydrothymine (DHT) is less described and may serve as an alternative diagnostic biomarker for DPD activity. In this study, we describe the development and validation of an ultra-high performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) assay for the quantification of uracil, DHU, thymine, and DHT in human plasma. In addition, stability experiments were performed. Uracil and thymine were quantified up to 80.0 ng/mL and DHU and DHT up to 800 ng/mL. Intra- and inter-assay precision were maximum 8.0 % and 7.6 %. respectively. Also, recovery was adequate and significant matrix-effects and carry-over were excluded. Stability experiments showed that uracil concentrations increased with 27-52 % when stored for 1 or 2 h at ambient temperatures compared to cold storage. Thymine, DHU, and DHT concentrations remained stable, thymine after 1 h in plasma excluded, showing the DHT:T ratio might be a more robust marker for DPD activity than DHU:U. In conclusion, we present here a novel assay capable of quantifying uracil, thymine, DHU and DHT in a single analytical run. We provide additional data showing increased stability for DHU, thymine and DHT compared to uracil. This assay may be used as a diagnostic test in future studies, establishing the association of these endogenous biomarker concentrations with DPD activity and safety to treatment with fluoropyrimidines. In addition, future research should also be focused on reducing pre-analytical instability. Standardization in this field is essential to set proper reference values and to allow inter-study comparison on clinical outcomes.
Vaginal Delivery for the Selected Frank Breech Infant at Term

Abstract:A retrospective analysis of 254 term breech deliveries was done, with term breech presentations managed by a protocol in which cesarean section was done for nonfrank breech presentation, or estimated fetal weight in excess of 4000 g. Patients with frank breech presentation were assigned to one of three groups based on x-ray pelvimetry and estimated fetal weight. Of 70 group 1 patients (adequate pelvis with estimated fetal weight <3600 g), 79% had a vaginal delivery. Of 21 group 2 patients (borderline pelvis or estimated fetal weight of 3600 to 4000 g), 67% delivered vaginally. In group 3 (contracted pelvis or estimated fetal weight >4000 g), all patients were delivered by cesarean section. The overall cesarean section rate for frank breech infants was 36%. Apgar scores were not significantly different for infants delivered vaginally or abdominally. The crude perinatal mortality rate was 11.8; the corrected perinatal mortality rate was 0. These findings further substantiate the safety of these criteria in management of term breech presentations.
Influence of metastatic disease on the usefulness of uracil pharmacokinetics as a screening tool for DPD activity in colorectal cancer patients

Abstract:Purpose: Dihydropyrimidine dehydrogenase (DPD) deficiency can lead to severe toxicity in patients treated with a standard dose of a fluoropyrimidine such as 5-fluorouracil or capecitabine (CAP). Administration of oral uracil and subsequent measurement of uracil and dihydrouracil (DHU) plasma concentrations has been used to identify patients with DPD deficiency. Liver metastasis might influence systemic DPD activity. The aim of the study was to investigate the effect of metastatic disease on the pharmacokinetics of uracil and DHU after oral administration of uracil. Methods: 500 mg/m(2) uracil was administered orally to 12 subjects with stages II-III colorectal cancer (CRC) who were treated in the adjuvant setting and to 12 subjects with stage IV metastasized CRC, all treated with CAP containing therapy. All subjects had a normal DPD activity defined as >6 nmol/mg/h determined in peripheral blood mononuclear cells. Results: The mean uracil clearance [CL 51.7 (SD 6.4) vs. 46.7 (SD 13.0) l/h], area under the curve [AUC0-220min 20.6 (SD 6.4) vs. 21.0 (SD 5.7) h mg/l], elimination half-life [t 1/2 21 (SD 7) vs. 21 (SD 8) min], maximum concentration time [T max 27 (SD 9) vs. 25 (SD 9) min], volume of distribution [V 26.58 (SD 10.11) vs. 21.10 (SD 8.48) l] and the elimination constant [k el 2.01 (SD 0.56) vs. 2.41 (SD 0.72) h(-1)] did not differ significantly (p > 0.05) non-metastatic CRD versus metastatic CRC. Conclusions: Metastasis does not alter uracil pharmacokinetics and is similar in CRC patients with and without metastasis. Therefore, the uracil test dose could be used as a DPD phenotype test in both adjuvantly treated and metastatic CRC patients using similar cutoff criteria to identify patients with DPD deficiency.

Abstract:Fluorouracil is among the most used antimetabolite drugs for the chemotherapeutic treatment of various types of gastrointestinal malignancies. Dihydropyrimidine dehydrogenase (DPYD) genotyping prior to fluorouracil treatment is considered standard practice in most European countries. Yet, current pre-therapeutic DPYD genotyping procedures do not identify all dihydropyrimidine dehydrogenase (DPD)-deficient patients. Alternatively, DPD activity can be estimated by determining the DPD phenotype by quantification of plasma concentrations of the endogenous uracil and thymine concentrations and their respective metabolites dihydrouracil (DHU) and dihydrothymine (DHT). Liquid chromatography - mass spectrometry (LC-MS) detection is currently considered as the most adequate method for quantification of low-molecular weight molecules, although the sample preparation method is highly critical for analytical outcome. It was hypothesized that during protein precipitation, the recovery of the molecule of interest highly depends on the choice of precipitation agent and the extent of protein binding in plasma. In this work, the effect of protein precipitation using acetonitrile (ACN) compared to strong acid perchloric acid (PCA) on the recovery of uracil, thymine, DHU and DHT is demonstrated. Upon the analysis of plasma samples, PCA precipitation showed higher concentrations of uracil and thymine as compared to ACN precipitation. Using ultrafiltration, it was shown that uracil and thymine are significantly (60-65 %) bound to proteins compared to DHU and DHT. This shows that before harmonized cut-off levels of DPD phenotyping can be applied in clinical practice, the analytical methodology requires extensive further optimization.

Abstract:Reliable, rapid, highly selective and sensitive analytical methods for the determination of antineoplastic agent 5-fluorouracil (5-FU) in human body fluids (blood serum/plasma and urine) are required to improve the chemotherapy regimen to reduce its toxicity and improve efficacy. Nowadays, electrochemical techniques provide a powerful analytical tool for 5-FU detection systems. This comprehensive review covers the advances in the development of electrochemical sensors for the quantitative determination of 5-FU, mainly focused on original studies reported from 2015 to date. We have summarized recent trends in the electrochemical sensor systems applied for the analysis of 5-FU in pharmaceutical formulations and biological samples, and critically evaluated the key performance metrics of these sensors (limit of detection, linear range, stability and recovery). Challenges and future outlooks in this field have also been discussed.

Abstract:A case of bilateral persistent hyperplastic primary vitreous (PHPV) in a 3-month-old male infant, who had bilateral leukokoria, is presented. The child was referred for imaging with a clinical suspicion of retinoblastoma. Gray-scale ultrasound evaluation revealed an echogenic band in the posterior segment of both globes, extending from the posterior surface of the lens capsule to the optic disc. Doppler examination revealed the presence of arterial flow in the band in both globes. Associated echogenic hemorrhage was also seen, which was confirmed by computed tomography. Most cases of PHPV are sporadic and unilateral, and bilateral PHPV is rare. The imaging features in this case suggest the diagnosis of bilateral PHPV and differentiate it from retinoblastoma. This entity, although infrequent, should be considered in the differential diagnosis while evaluating bilateral leukokoria.

Endogenous metabolic markers for predicting the activity of dihydropyrimidine dehydrogenase

Abstract:Five-fluorouracil (5-FU) is a chemotherapeutic agent that is mainly metabolized by the rate-limiting enzyme dihydropyrimidine dehydrogenase (DPD). The DPD enzyme activity deficiency involves a wide range of severities. Previous studies have demonstrated the effect of a DPYD single nucleotide polymorphism on 5-FU efficacy and highlighted the importance of studying such genes for cancer treatment. Common polymorphisms of DPYD in European ancestry populations are less frequently present in Koreans. DPD is also responsible for the conversion of endogenous uracil (U) into dihydrouracil (DHU). We quantified U and DHU in plasma samples of healthy male Korean subjects, and samples were classified into two groups based on DHU/U ratio. The calculated DHU/U ratios ranged from 0.52 to 7.12, and the two groups were classified into the 10th percentile and 90th percentile for untargeted metabolomics analysis using liquid chromatography-quantitative time-of-flight-mass spectrometry. A total of 4440 compounds were detected and filtered out based on a coefficient of variation below 30%. Our results revealed that six metabolites differed significantly between the high activity group and low activity group (false discovery rate q-value < 0.05). Uridine was significantly higher in the low DPD activity group and is a precursor of U involved in pyrimidine metabolism; therefore, we speculated that DPD deficiency can influence uridine levels in plasma. Furthermore, the cutoff values for detecting DPD deficient patients from previous studies were unsuitable for Koreans. Our metabolomics approach is the first study that reported the DHU/U ratio distribution in healthy Korean subjects and identified a new biomarker of DPD deficiency.
Renal impairment and abnormal liver function tests in pre-therapeutic phenotype-based DPD deficiency screening using uracilemia: a comprehensive population-based study in 1138 patients

Abstract:Background: Dihydropyrimidine dehydrogenase (DPD) deficiency screening is a pre-therapeutic standard to prevent severe fluoropyrimidine-related toxicity. Although several screening methods exist, the accuracy of their results remains debatable. In France, the uracilemia measurement is considered the standard in DPD deficiency screening. The objective of this study was to describe the hyperuracilemia (⩾16 ng/mL) rate and investigate the influence of hepatic and renal impairment in uracilemia measurements since the guidelines were implemented. Patients and methods: Using a cohort of 1138 patients screened between 18 October 2018 and 18 October 2021, basic demographic characteristics, date of blood sampling, and potential biological confounders including liver function tests [aspartate aminotransaminase (AST), alanine aminotransaminase (ALT), gamma-glutamyl transferase (γGT), alkaline phosphatase (ALP), and bilirubin] and estimated glomerular filtration rate (eGFR) were collected. The second same-patient uracilemia analysis was also performed. Temporal change was graphically represented while potential confounders were stratified to show linearity when suspected. Results: Hyperuracilemia was diagnosed in 12.7% ( n = 150) samples with 6.7%, 5.4%, 0.5%, and 0.08% between 16 and 20 ng/mL, 20 and 50 ng/mL, 50 and 150 ng/mL, and >150 ng/mL, respectively. The median uracilemia concentration was 9.4 ng/mL (range: 1.2 and 172.3 ng/mL) and the monthly hyperuracilemia rate decreased steadily from >30% to around 9%. Older age, normalized AST, γGT, ALP results, bilirubin levels, and decreased eGFR were linearly associated with higher plasma uracil concentrations (all p < 0.001). In the adjusted multivariate linear model, AST, eGFR, and ALP remained associated with uracilemia ( p < 0.05). When measured twice in 39 patients, the median uracilemia rate of change was −2.5%, which subsequently changed the diagnosis in nine patients (23.1%). Conclusions: Better respect of pre-analytical conditions may explain the steady decrease in monthly hyperuracilemia rates over the 3 years. Elevated AST, ALP levels, and reduced eGFR could induce a false increase in uracilemia and second uracilemia measurements modified the first DPD deficiency diagnosis in almost 25% of the patients.

Abstract:In this study, a carbon paste electrode was modified with a composite material of CeO 2 –CuO to fabricate an electrochemical sensor . The fabricated sensor was then employed for the analysis of 5-fluorouracil (5-FU), an important anticancer drug, in a phosphate buffer solution (PBS) with pH 7.0, using cyclic voltammetry (CV) and differential potential voltammetry (DPV) techniques. The effect of critical parameters such as catalyst loading, scan rate, type, and pH of the supporting electrolyte on the electrochemical responses of the modified electrode was assessed and optimized. The electrochemical response of the fabricated sensor towards 5-FU remained unaffected in the presence of various potent interfering compounds, indicating its anti-interference capability. The DPV responses of the prepared sensor was linear with the concentration of 5-FU in the range of 0.1–1.0 μmol L −1 and 1.0–40.0 μmol L −1 under optimal experimental conditions, with a detection limit of 0.01 μmol L −1 . The electrochemical sensor developed in this study was employed to determine 5-FU in human urine samples with the acceptable recovery. The fabricated sensor demonstrated favorable characteristics such as high sensitivity, repeatability, reproducibility, and stability.

Abstract:Inborn errors of pyrimidine degradation, dihydropyrimidine dehydrogenase deficiency and dihydropyrimidinase deficiency, are less rare than has generally been assumed. Many asymptomatic cases have been reported, and in patients with symptoms, the clinical abnormalities are variable and nonspecific. Withdrawal of pyrimidine analogues such as 5-fluorouracil (5FU), a commonly used anticancer drug, from the cancer chemotherapy regimens of patients with pyrimidine degradation deficiencies, however, is critical because 5FU is degraded in vivo by pyrimidine-degradative enzymes. Patients with these deficiencies suffer from severe neurotoxicity, sometimes leading to death, following administration of 5FU, and even otherwise asymptomatic homozygotes or heterozygotes may develop severe clinical symptoms upon administration of such medication. Therefore, a rapid and specific method for identifying cancer patients with these enzyme deficiencies prior to treatment with 5FU is critical. To address this problem, we established methods for highly sensitive yet specific determinations of thymine, uracil, dihydrothymine, dihydrouracil, orotate and creatinine simultaneously in 0.1-ml liquid urine or filter-paper urine. This method involves stable isotope dilution, a simplified urease treatment previously described and gas chromatography-mass spectrometry without prior fractionation. The high recovery and low C.V. values were obtained and healthy control values were also determined for these metabolites. Using artificially prepared urine specimens simulating these disorders. the chemical diagnosis can be made clearly, and no further analysis appears to be required for differential chemical diagnosis.

Abstract:The identification of formylpyrimidines in DNA is crucial for a better understanding of epigenetics. Although many techniques have been explored to detect their content, more accurate methods of formylpyrimidine determination are still required due to the relatively lower sensitivity or lack of selectivity in current methods. Herein, an electrochemical method based on the covalent bonding of the azido derivative of (2-benzimidazolyl) acetonitrile (azi-BIAN) and the aldehyde group of 5-formyluracil (5fU) was proposed for the selective detection of 5fU in the presence of 5-formylcytosine (5fC) and apyrimidinic (AP) sites. Target DNA containing 5fU was first treated with azi-BIAN and then incubated with DBCO-PEG4-Biotin to introduce a biotin group by copper-free click chemistry. Next, the sulfhydryl group was attached to the 5' end of above DNA through T4 polynucleotide kinase-catalyzed reaction. Subsequently, the labeled DNA was assembled onto the AuNPs-modified glassy carbon electrode (AuNPs/GCE) through Au-S bonds, and the streptavidin-horseradish peroxidase conjugate (SA-HRP) was further immobilized onto the surface of the above electrode by specific recognition between biotin and streptavidin. Finally, HRP catalyzed hydroquinone oxidation to benzoquinone to enhance the current signal, which was related to the amount of 5fU in nucleic acids. This method demonstrated a good linear relationship with 5fU concentrations ranging from 0.1 to 10 nM. Moreover, the level of 5fU in γ-irradiated nucleic acids was also successfully detected, indicating that the combination of molecule-depended chemical recognition and electrochemical sensing is a promising method for the selective and sensitive detection of 5fU.

Establishment and Clinical Application of Liquid Chromatography-Tandem Mass Spectrometric Method for Simultaneous Determination of Plasma 5-Fluorouracil

Abstract:OBJECTIVE:To develop a robust liquid chromatography-tandem mass spectrometric (LC-MS/MS) method for determination of 5-fluorouracil (5-FU) in blood plasma and evaluate the use of 5-FU for treatment response surveillance as well as toxicity prediction in malignant gastrointestinal tumors. METHODS:A LC-MS/MS method was used, with signal linearity, lower limits of quantitation, precision, accuracy and stability being evaluated according to guideline US Food and Drug Administration (FDA)'s guidance for industry bioanalytical method validation.Analysis of 5-FU was performed in 35 gastrointestinal cancer patients admitted to Zhongshan Hospital of Fudan University from April 2013 to December 2013. The relationship between 5-FU with toxicity and treatment effect was compared. RESULTS:The linear ranges of 5-FU was 49-9 800 ng/ml, the lower limit of quantitation was 49.0 ng/ml. The within-run and between-run coefficients of variation (CV) of 5-FU was <3% and <6%.The recovery rates of low, medium and high level quality controls were 103.36%, 88.12% and 91.26% respectively; with standard added recovery of 109.69%, 91.06% and 88.81% respectively. The control added recoveries were 112.16%, 99.12% and 92.28% respectively. The bias was -11.69%, 2.42% and -8.09% when samples were repeated freezing and thawing twice (-80 ℃). The results had a bias -11.97%, 1.42%, -10.91% and 0.56%, 0.14%, 3.82% when samples were kept at 2-8 ℃ for 2 days and 14 days. In 35 gastrointestinal cancer patients, there was no correlation between initial dose of 5-FU and 44 h AUC (concentration 3.44-53.43 mg/L·h). The risk of chemotherapy-related adverse effect in 5-Fu 44 h AUC> 30 mg/L·h group was significantly higher than 44 h AUC < 30 mg/L·h group (χ(2)=12.600, P<0.01). While the chemotherapy response of AUC > 20 mg/L·h group was significantly satisfactory than AUC < 20 mg/L·h group (χ(2)=5.358, P<0.05). CONCLUSIONS:A robust and reliable LC-MS/MS method for the determination of 5-FU in blood plasma has been developed and it is suitable for clinical application. Detecting 5-FU may guide individualized treatment and predict adverse events.
Dihydropyrimidine dehydrogenase activity in human blood mononuclear cells

Abstract:Dihydropyrimidine dehydrogenase (DPD; EC 1.3.1.2) catalyzes the rate-limiting reaction in the catabolism of endogenous uracil and thymine and exogenous fluoropyrimidines. DPD activity was studied in human blood mononuclear cell supernatants utilizing a new and sensitive radiochromatographic assay. Total DPD activity showed a linear correlation with supernatant protein concentration. The affinity constants (Km) for NADPH and thymine were approximately 10 and 1 mumol/l, respectively. Maximal activity (Vmax) was observed at 0.25 mmol/l NADPH and 10 mumol/l thymine, respectively. DPD activity in normal individuals was 8.0 +/- (SD) 2.2 nmol/mg protein/h, and ranged from 4.4 to 12.3 nmol/mg/h (n = 25). This activity range was quite similar to values obtained in patients with metastatic solid tumors treated with fluorodeoxyuridine (FUdR; n = 33, p = 0.57). No correlation was found to exist between mononuclear leucocyte DPD activity and the observed toxicity of FUdR in the tested patients. A bimodal distribution of DPD activity was observed in the patients and in normal individuals. The entire study population tested could be divided into two groups with respect to DPD activity; one group with high (greater than 8 nmol/mg/h) activity and another with low (less than 8 nmol/mg/h) activity. The possibility that sex differences may have been responsible for this distribution of DPD activity could not be excluded. The findings of this study are relevant to the pharmacogenetics of fluoropyrimidines in humans.
Development of an HPLC-DAD Method for the Extraction and Quantification of 5-Fluorouracil, Uracil, and 5-Fluorodeoxyuridin Monophosphate in Cells and Culture Media of Lactococcus lactis

Abstract:The drug 5-fluorouracil (5-FU) is a common cancer chemotherapeutic, presenting toxicity. Mild toxicity is treated with administration of probiotics. The interaction of these probiotics with the drug may have a crucial effect on its therapeutic efficacy. In the present work, a method for the quantification of uracil, 5-FU, and its active metabolite 5-fluorodeoxyuridin monophosphate in cells and culture medium of the probiotic L. lactis is presented. Extraction using H2O containing 0.05% v/v formic acid (1:5 v/v) was followed by ammonium sulphate protein precipitation and SPE. Analysis was conducted in a Nucleosil column using a gradient of water, formic acid, and acetonitrile. Calibration curves were constructed for 5-FU (5–100 μg/mL), uracil (5–20 μg/mL), and 5-fluorodeoxyuridin monophosphate (5–20 μg/mL) using 5-bromouracil as the internal standard (R2 ≥ 0.999). The photodegradation of 5-FU amounted to 36.2% at 96 h. An administration experiment in the dark revealed a decline in 5-FU concentration in the culture media (88.3%) and uptake by the cells, while the uracil and FdUMP levels increased in the cells. The inactive metabolite 5,6 dihydrofluorouracil was detected in the medium. Our results demonstrate that uptake and metabolism of 5-FU in L. lactis cells leads to a decline in the drug levels and in the formation of both the active and the inactive metabolites of the drug.

4,5‐Diamino‐2‐Thiouracil‐Powered Dual‐Mode Biosensor for Sensitive, Nonenzymatic Determination of Saliva Uric Acid Levels

Abstract:Nonenzymatic and rapid monitoring of uric acid levels is of great value for early diagnosis, prevention, and management of oxidative stress–associated diseases. However, fast, convenient, and low‐cost uric acid detection remains challenging, especially in resource‐limited settings. In this study, a novel and rapid biosensing approach was developed for the simultaneous visualization and quantification of uric acid levels by using the unique surface plasmon resonance and photothermal property of 4,5‐diamino‐2‐thiouracil (DT)‐capped gold nanoparticles (AuNPs). With the presence of uric acid, DT‐capped AuNPs rapidly aggregated, and a visible color/photothermal change was used for uric acid quantification within 15 min. The limit of detection was determined to be 11.3 and 6.6 μM for the dual‐mode biosensor, leveraging the unique structure of DT to optimize reaction kinetics. Moreover, the sensor exhibited excellent anti‐interference capabilities and demonstrated potential for detecting a wide range of uric acid concentrations in complex samples, thereby reducing the need for extensive sample dilution and complex material synthesis procedures. Furthermore, validation against gold standard testing indicates that this biosensor could serve as a highly sensitive assay for quantifying uric acid levels in point‐of‐care applications, particularly in resource‐limited settings.

Abstract:To the Editor, With great interest, we read the article by Launay et al. [1], which compared results of pre-emptive dihydropyrimidine dehydrogenase (DPD) deficiency testing for fluoropyrimidine treatment using both DPYD genotyping and plasma uracil levels. The impressive sample size demonstrates that routine pre-treatment DPD deficiency testing is feasible and has been fully adapted into clinical practice in France. However, we have several comments and concerns regarding the data and its interpretation. Firstly, the authors opted to use a genotyping panel that omits haplotype B3 (HapB3; c.1129–5923C>G (rs75017182) and the commonly tested variant in linkage p.E412E; c.1236G>A (rs56038477)). Although indeed conflicting results regarding the adequate dose for HapB3 carriers exist, its association with increased toxicity rates, such as severe neutropenia and diarrhea, remains undisputed. In meta-analysis a relative risk rate for severe grade ≥3 fluoropyrimidine related toxicity of 1.72 (1.22–2.42) was found [2], and even among 51 patients treated at a 75 % dose intensity, an increased incidence of grade ≥3 adverse events of 39 % persisted [3]. The exclusion of HapB3 from the current analysis therefore hampers adequate comparison between both tests. Secondly, the authors argue that targeted genotyping is not sufficiently reliable in the detection of patients who have a complete and profound DPD deficiency. In support of this argument, details are provided of five patients who had a complete DPD deficiency according to French health authority recommendations, with only one of them having a DPYD variant genotype (see ref [1], table 1, patient C: homozygous *2A genotype). Interestingly, this is also the only patient who had unmeasurable dihydrouracil levels. Reliable measurement of uracil and dihydrouracil levels, as noted by the authors, is heavily dependent on pre-analytical handling. Lack of adherence to recommendations regarding sample handling can result in false positive DPD deficiency diagnoses and subsequent undertreatment due to unnecessary dose-reductions. Moreover, renal impairment and abnormal liver function may also result in false positive results. Notably, one of the five described patients (i.e. patient B) had a very high dihydrouracil level, resulting in legitimate concerns of the authors regarding the reliability of the completely deficient status in this specific case. Regrettably, data on sample handling as well as clinical data relating to kidney and liver function were unavailable to the authors, making a final conclusion regarding the patient's DPD status difficult. Nonetheless, we have concerns regarding the validity of the diagnosis of a completely deficient DPD status in the remaining three patients as well, suggesting that, in contrast to the authors' conclusion, both genotyping and phenotyping are able to identify completely deficient patients. Literature on patients with a complete DPD deficiency is sparse. However, case-reports of four completely DPD deficient patients (genotype confirmed) show that these patients not only share the inability to form 5-FU metabolites (DHFU, FUPA or FBAL being not quantifiable), but also have undetectable levels of dihydrouracil [4], [5], [6]. Since the remaining three patients who were identified as being completely DPD deficient (i.e. patients A, D, and E) had detectable dihydrouracil levels, with two of the patients even having dihydrouracil levels within the physiological range, we consider it unlikely that these patients are truly completely DPD deficient. Although the patients match the criteria for a complete DPD deficiency based on uracil testing guidelines, we believe they are more likely to be partially deficient patients, in which case a major treatment line (i.e. fluoropyrimidines) has been unnecessarily withheld from them. Still, the absence of common DPYD variants in these patients highlights an important limitation of targeted DPYD genotyping: its inability to detect rare or unknown genetic variants. In that regard, DPYD sequencing of these patients would greatly aid interpretation of the results, as presence of rare DPYD variants could support diagnosis of a DPD deficiency. Consequently, this would demonstrate the added value of combining DPYD genotyping with uracil-based phenotyping, especially for patients with extremely elevated uracil levels. For now however, hard conclusions cannot be drawn from these cases due to the absence of additional data. Finally, the lack of clinical outcomes or a corresponding DPD enzyme activity measured in peripheral blood mononuclear cells greatly hampers the interpretation of the results. Currently, prospective evidence for dose-reductions based on pre-emptive DPD testing is only available for genotyping [3, 7]. The evidence for uracil-based dosing is largely retrospective [7]. Abstract:Herein this study, a facile, efficient and disposable electrochemical sensor has been prepared by electropolymerization of glycine (poly(GLY)) on the surface of pencil graphite electrode (PGE). The surface topology of the equipped poly(GLY) modified pencil graphite electrode (poly(GLY)/PGE) and bare pencil graphite electrode (BPGE) has been characterized by the scanning electron microscopy (SEM) combined with energy dispersive x-ray analysis (EDX) and charge transfer behaviour was measured by electron impedance spectroscopy (EIS) method. The voltammetric behaviour of anticancer, 5-fluorouracil (5-FU) in the presence of theophylline (THP) has been carried out in 0.1 M phosphate buffer solution (PBS) of physiological pH 7.0 using different techniques such as cyclic voltammetry (CV), linear sweep voltammetry (LSV) and differential pulse voltammetry (DPV). The proposed poly(GLY)/PGE shows augmented peak current for 5-FU at lower potential side over the BPGE due to the electrocatalytic behaviour of modifier layers wrapped on the electrode surface. The kinetic behaviour of 5-FU at modified electrode surface was studied by varying different parameters such as pH, scan rate and concentration study in 0.1 M PBS used as a supporting electrolyte. The limit of detection (LOD) for 5-FU was attained using DPV method with different concentrations (1.0-13.0 μM) and it was found to be 0.012 μM. The possible electrochemical reaction of 5-FU was proposed and it was incorporated by two electrons and two protons mechanism at modified electrode surface. The voltammetric response of poly(GLY)/PGE towards the determination of 5-FU was unaffected in the presence of some excipients in addition to the remarkable stability and reproducibility. The applicability of the proposed sensor has been performed by real sample investigation of 5-FU with a substantial percentage of recovery results in all optimized conditions.
