Abstract:With fewer than 50 reported cases, Laryngo-onycho-cutaneous syndrome is a rare condition presenting in childhood with skin and conjunctival ulcerations and laryngeal pathology. A 4-year-old girl presented with recent onset aphonia. Airway endoscopy revealed a laryngeal mass. Tracheostomy was advised but parents denied consent. Two weeks later, she developed severe stridor warranting emergent tracheostomy. The child was diagnosed as Laryngo-onycho-cutaneous syndrome, also known as Shabbir syndrome. Subsequently, she was taken up for laser excision of the mass. Here, we describe the anesthetic management in such airway procedures. Total intravenous anesthesia and local anesthetic in the form of vocal cord spray are safe and environment-friendly alternatives to inhalational anesthesia for airway endoscopies.
Anesthetic management of ventricular-peritoneal shunt implantation in osteogenesis imperfecta type IIB: A case report

Abstract:Introduction: Osteogenesis imperfecta (OI), a rare congenital disorder, has a risk of bone fracture and progressive bone deformity. OI type II is the most serious subtype, and very few reports on its anesthetic management exist. Patients face several anesthetic difficulties, of which easy fracturing of OI-affected bones is critical. Herein, we report our experience with the anesthetic management of a patient with OI type II. Patient concerns and diagnoses: Through genetic testing, a 14-month-old girl (height: 45 cm, weight: 3.9 kg) was diagnosed with OI type IIB due to COL1A gene abnormality. The clinical manifestations included hydrocephalus, blue sclera, dental dysplasia, short stature, limb deformity and shortening, thoracic hypoplasia, rabbit eye, left inguinal hernia, and tricuspid valve regurgitation. Physical examination revealed an enlarged head due to skull dysplasia and hydrocephalus. The pediatrician confirmed that mask ventilation was possible even under spontaneous breathing, and that there was no history of bone fracture during mask holding. Interventions and outcomes: The patient was scheduled for gastrostomy and ventriculo-peritoneal shunt implantation. An arterial pressure line was inserted at the neonatal intensive care unit. Propofol and remifentanil were selected for general anesthesia. Confirming that mask-assisted ventilation was possible under sleep, rocuronium was administered. Attentive mask ventilation was performed via the 2-person method to avoid fractures. We were able to intubate successfully using a Macintosh laryngoscope. A microcuff endotracheal tube was used. For ventilation, pressure control ventilation was selected and sedative dosage was adjusted using the patient state index as an indicator. The patient was sedated, intubated, and returned to the neonatal intensive care unit. She was extubated on the sixth postoperative day. No bone fractures were noted. Conclusion: OI type II is the most severe subtype with high mortality, and there are few reports on its anesthetic management. Easy fractures can be a problem in airway maintenance, blood pressure measurement, and repositioning. We performed the procedure attentively, avoiding jaw and cervical fractures during mask ventilation and endotracheal intubation. For respiratory management, we chose pressure control ventilation using a cuffed tracheal tube and circulatory control was attained via an arterial line inserted preoperatively. No complications occurred.

Abstract:INTRODUCTION: Mucopolysaccharidoses (MPSs) are a group of heredity storage diseases, transmitted in an autosomal recessive manner, associated with the accumulation of glycosaminoglycans (GAGs) in various tissues and organs. The concerned patients have multiple concomitant hereditary anomalies. Considering the craniofacial abnormality in these patients, airway management may be difficult for anesthesiologists. In these patients, preanesthetic assessment is necessary and performed with the accurate assessment of airways, consisting of the physical exam and radiography, MRI or CT of head and neck. An anesthesiologist should set up a "difficult intubation set" with a flexible fiber-optic bronchoscope and also, it may be necessary to discuss with an ear-nose and throat (ENT) specialist if required, for unpredicted emergency situations.CASE PRESENTATION: In this case-report we presented a 2-year-old boy with known MPSs with psychomotor retardation, bilateral corneal opacities, impaired hearing and vision, inguinal hernia, severe mitral regurgitation, micrognathia, coarse facial feature, stiff and short neck and restricted mouth opening. He scheduled for left inguinal hernia repair surgery.DISCUSSION: The patient's difficult airway was managed successfully and the anesthesia of his surgical procedure had an uneventful course.

Abstract:Tracheobronchopathia osteochondroplastica (TO) is a rare disease that may cause unexpected difficult intubation. There is no available consensus on the management of difficult intubation that is associated with TO. A 45-year-old woman was scheduled for modified radical mastoidectomy, canaloplasty, and tympanoplasty under general anesthesia. We encountered significant resistance during tracheal intubation, although the laryngeal view was normal with the video laryngoscope. A fiberoptic bronchoscope was then used to facilitate intubation, and we noted that the trachea was obviously narrowed due to cartilaginous ring hypertrophy. The tracheal tube was fully lubricated with tetracaine gel, and smoothly inserted into the trachea. After the operation, bronchoscopy and a computed tomography (CT) scan were performed to confirm the diagnosis of TO. Fiberoptic bronchoscopy-assisted tracheal intubation is safe and effective choice for the patients in whom subglottic intubation is difficult. CT scan and bronchoscopy might be helpful for preoperative airway assessment. Identifying patients with TO is important to avoid unexpected tracheal intubation impediment. Assessment of the subglottic airway should also be taken seriously.

Abstract:Introduction Upper airway obstruction (UAO) management in children, particularly those with associated anatomical mid‐face structural differences, poses a significant challenge. This study describes using a nasopharyngeal airway (NPA) in managing infants with severe upper airway obstruction. Methods Infants discharged home from Queensland Children's Hospital with an NPA in situ for the management of upper airway obstruction were retrospectively evaluated for clinical and demographic parameters, underlying diagnoses and respiratory support, including NPA insertion details and comorbidities. Results Sixty‐seven children (24 male) were included with a median age at NPA insertion of 24 (IQR 7‐59) days and a median duration of NPA use of 229.1 (IQR 151.50‐297.75) days. Fifty‐two (77.6%) had Pierre Robin sequence. Other diagnoses included CHARGE syndrome, Treacher Collins syndrome, Stickler syndrome, Crouzon syndrome, Bohring–Opitz syndrome, isolated cleft palate, Beckwith–Wiedemann syndrome, Chromosome 3 deletion and VACTERL association. Four infants required tracheostomy, and no complications related to NPA use were reported. Those with oximetry and polysomnography data showed improving indices following NPA insertion [pre NPA median (IQR) SpO2: 95.9% (94.73–97.73), AHI: 51.40 (44.50‐69.45), OAHI 65.9 (56.45‐73.35) and TcCO2 (mmHg) 65.70 (61.95‐67.30) vs. post‐NPA median (IQR) SpO2: 97.1% (96.19–97.9), AHI: 12.20 (11.25‐24.35), OAHI 10.20 (6.12‐5.62) and TcCO2 (mmHg) 52.40 (47.90‐58.40)]. Conclusion Our findings highlight the effectiveness and safety of NPA as a noninvasive management option for severe upper airway obstruction in children, particularly in those with anatomical midface structural differences. Further research and larger studies are warranted to confirm these findings and optimise management strategies for these patients.

Abstract:Abstract Introduction Morquio Syndrome is a lysosomal storage disease (mucopolysaccharidosis type 4) characterized by the accumulation of glycosaminoglycans (GAG) and causes craniofacial, skeletal, and soft-tissue abnormalities. The pathophysiology of obstructive sleep apnea (OSA) is multifactorial due to GAG accumulation in upper airways, adenoids, trachea, and bronchi causing narrowing and collapsibility of the airways. Report of case(s) A nine-year-old male with Morquio Syndrome presented to the emergency department with difficulty staying awake for 5 days and an upper respiratory tract infection. He would doze off and wake up every few minutes several times a day. He had no prior history of somnolence. He was admitted to the ICU given the severity of the presentation and was clinically started on BiPAP. There was also a history of loud snoring but no witnessed apneas. An in-laboratory polysomnogram was performed to confirm his diagnosis of severe OSA and sleep-related hypoxemia, with an apnea-hypopnea index of 34.1, minimum oxygen saturation 78%, and time < 90% of 74.8 minutes. After a titration study, he was started on noninvasive ventilation (NIV) with bilevel PAP. He remained compliant with outpatient NIV therapy and did not have any hospital admission for respiratory illness. Seven years later, he had a repeat titration study and was switched to NIV with AVAPS. He had one emergency visit for dyspnea and tested positive for influenza. A CT at that time showed severe tracheal stenosis of 8 mm at the thoracic inlet. On direct visualization with laryngoscopy, the stenosis was noted to be around 3 mm, probably due to dynamic airway obstruction. He improved with symptomatic treatment and was discharged the next day. Despite having significant airway abnormalities, he has never required intubation for respiratory failure, likely attributable to consistent use of NIV therapy. Conclusion Symptoms of OSA can be subtle, and if unrecognized, can potentially progress to respiratory failure. It is crucial, particularly in children with airway abnormalities, to actively screen for symptoms of OSA and initiate appropriate treatment. Our case highlights successful long-term treatment spanning more than a decade in an individual with Morquio Syndrome and avoidance of tracheostomy using NIV with bilevel PAP and AVAPS therapy. Bilateral TMJ Ankylosis with limited mouth opening in pediatric patients: An anesthetic challenge

Abstract:Restricted mouth opening is a challenging airway in pediatric patients with temperomandibular joint (TMJ) ankylosis. The fiber-optic bronchoscopic nasotracheal intubation technique continues to be the gold standard for difficult airway, among the techniques available such as submandibular intubation, retrograde intubation, and tracheostomy. However, awake fiber-optic bronchoscopy (FOB) is difficult to achieve in pediatric patients. Prior planning of the anesthetic method and effective collaboration with the surgeon are crucial for excellent outcomes in such challenging airway cases. We present a successful awake fiber-optic bronchoscopy with high-flow nasal oxygen (HFNO), airway blocks, and deep sedation in the case of bilateral TMJ ankylosis of a pediatric age group with reduced mouth opening. We conclude that awake intubation using HFNO and airway blocks helps to achieve oxygenation and ease of intubation in difficult airway management.
Airway management with Hi-flow nasal cannula oxygen in children with severe laryngeal obstruction

Abstract:Background: The aim of this study was to report our initial experience in airway management in young children with severe laryngeal obstruction. Hi-flow nasal cannula oxygen (HFNO) with spontaneous respiration was used as a new airway management strategy in young children undergoing suspension laryngoscopic surgery. Methods: Children aged between 1 day and 24 months scheduled for suspension laryngoscopy were retrospectively studied. The data collected included the patients' age, gender, American Society of Anaesthesiologists physical status classification, comorbidities, preoperative physiological status, methods of induction and maintenance of anesthesia, course of the disease and surgical options, lowest oxygen saturation recorded, transcutaneous CO2, duration of operation, and patients' need for rescue methods. Results: A total of 38 patients successfully underwent suspension laryngoscopy under HFNO with spontaneous respiration. 19 patients were less than 1 year old (7 neonates), while the other half were less than or equal to 2 years old. The median [IQR (range)] lowest oxygen saturation recorded during the operation was 98 [93-99 (91-99)] %. The median [IQR (range)] duration of HFNO with spontaneous respiration was 65 [45-100 (30-200)] minutes. The median [IQR (range)] PCO2/PtcCO2 at the end of the spontaneous ventilation period was 54 [48-63 (39-70)] mmHg, which was the same as the preoperative PCO2 despite a long operation time. Conclusions: HFNO with spontaneous respiration emerged as a new airway management strategy in young children with severe laryngeal obstruction that was beneficial in maintaining oxygenation and was superior to transnasal humidified rapid insufflation ventilatory exchange (THRIVE) in terms of the rising rate of PCO2 in these patients, thereby prolonging the safety time of the operation.
Stridor and severe subglottic stenosis: emergency tracheostomy for a neonate with a pinhole trachea

Abstract:Congenital subglottic stenosis is a rare but potentially catastrophic condition. In this report, we describe the management of a term neonate who was noted to have biphasic stridor during preassessment for correction of an imperforate anus at 26 hours of life. The neonate was found to have a pinhole trachea secondary to congenital subglottic stenosis. It was impossible to pass an endotracheal tube, so the neonate underwent an emergency surgical tracheostomy with a good outcome. A high index of suspicion led to appropriate steps being taken to safely anaesthetise the neonate.

Abstract:Background: Mandibular hypoplasia, deafness, progeroid features, and lipodystrophy (MDPL) syndrome is a rare autosomal dominant disorder that presents unique challenges for anesthetic management due to its multisystemic manifestations. This report outlines the anesthetic considerations for MDPL patients based on our case experience. Case presentation: A 15-year-old male with MDPL syndrome underwent testicular extraction under general anesthesia. Insertion of a peripheral venous catheter was challenging due to scleroderma-like skin. Although the facial features of MDPL syndrome suggested a difficult airway, intubation with a McGrath™ Mac video laryngoscope was successful. Despite MDPL syndrome's association with hypertriglyceridemia due to lipodystrophy, this patient's triglyceride levels were normal. Thiamylal and sevoflurane were used without issues such as delayed emergence from anesthesia. Conclusions: MDPL syndrome requires careful preoperative assessment and tailored anesthetic management due to potential airway challenges arising from its distinctive facial features and the possibility of altered anesthetic pharmacokinetics associated with lipodystrophy.
1145 Stride Along with Stridor in the Setting of CSA: An Unusual Presentation of Pediatric Cervicomedullary Ganglioma

Abstract:Abstract Introduction We describe an unusual presentation of a cervicomedullary ganglioglioma secondary to findings of stridor despite positive airway pressure (PAP) therapy and the presence of treatment emergent central sleep apnea in a pediatric patient. Report of case(s) Patient is an 8 year old male with a history of congenital subglottic stenosis and no history of neurologic problems who presented to the sleep clinic for stridor despite adequate adherence to CPAP. Split night polysomnogram (PSG) showed moderate OSA (OHI 8.3/h) and treatment emergent central sleep apnea. Patient was unable to tolerate BPAP ST and wanted to go back to CPAP. Drug induced sleep endoscopy findings were significant for mild collapse of upper airway with no snoring. Repeat PSG - 6 months later - was significant for severe obstructive and central sleep apnea (OAHI 10.4, CAI 19, REM CAI 54, AHI of 30). MRI of the brain showed cervicomedullary tumor with marked effacement of the foramen magnum. Further workup showed right vocal cord paralysis. Biopsy confirmed ganglioglioma. Tumor was not amenable for surgical resection hence proton radiation therapy was pursued. Repeat titration study was performed which showed BPAP ST settings of 12/6 with BUR of 12 to be effective with residual AHI of 0.7. Patient developed radiation induced tumor psuedoprogression and tumor necrosis with worsening ataxia and recurrence of central and mixed apneas requiring AVAPS for further respiratory support due to progressive neuromuscular weakness. Stridor resolved after AVAPS support. Conclusion Stidor in a pediatric patient can involve various anatomical abnormalities of the upper airway. However, in the setting of central sleep apnea, abnormalities of the brain stem can lead to stridor. We present a patient with a prolonged period of central sleep apnea of unknown etiology and persistent stridor despite adequate adherence to PAP therapy that ultimately led to a diagnosis of a cervicomedullary ganglioma before neurologic symptoms developed. Abstract:Abstract Background Orthopedic surgery for cervical torticollis poses potential threat to airway management both in tracheal intubation and extubation. Klippel-Feil syndrome (KFS) is a complex syndrome of osseous and visceral anomalies. The anatomical characteristics of KFS might have significant implications for airway management. Case presentation This is a rare case of an 8-year-old boy presenting with osseous torticollis, congenital occipito-atlantal deformity, congenital basilar invagination and KFS undergone elective torticollis correction surgery. Though with difficulty, tracheal intubation was successfully performed. Extubation failed twice on postoperative day 2 and 10, and required tracheostomy. Based on radiological findings, we speculated that prolonged airway edema accounted for the main reason of the failed extubation, the hypertrophic tonsil and occipito-cervical fusion resulted in reduced oropharyngeal space and limited cervical range of motion. Moreover, the Chiari malformation and KFS complicated the airway condition and lead to prolonged airway obstruction. The tracheostomy casing was removed 1 month later. Conclusions Cautions should be taken in extubation of pediatric patients undergone major osseous torticollis surgery. Reintubation should be prepared in case of failed extubation. Severe post-operative airway edema, complicated with hypertrophic tonsil, the structural abnormalities in the oropharyngeal cavity, and occipito-cervical deformities constituted the decreased oropharyngeal space and resulted in failed extubation. For severe airway compromise and prolonged intubation, tracheostomy should be considered.

Abstract:Mounier-Kuhn-syndrome patients have markedly dilated trachea and main bronchi due to an atrophy or absence of elastic fibers and thinning of smooth muscle layers in the tracheobronchial tree. Although this syndrome is rare, airway management is challenging and general anesthesia may produce fatal results. However, only a few cases have been reported and this condition is not widely known among anesthesiologists. We present the case of a tracheobronchomegaly patient undergoing an emergency off-pump coronary artery bypass. Although the trachea was markedly dilated with numerous tracheal diverticuli, there was an undilated 2 cm portion below the vocal cords found on the preoperative CT. Under a preparation of extracorporeal membrane oxygenation, we intubated and placed the balloon of an endotracheal tube (I.D. 9 mm) at this portion, and maintained ventilation during the operation. This case showed that a precise preoperative evaluation and anesthetic plan is essential for successful anesthetic management.

The most common causes of stridor in this population are laryngotracheobronchitis (viral croup) and foreign body aspiration. This article gives an overview of the signs, symptoms and potential causes of stridor and the signs and symptoms of, and diagnostic criteria for, viral croup. The author also discusses evidence-based assessment criteria and treatments and criteria for admission or discharge. A case study is used for illustration.

Abstract:Airway ultrasound (US) is an easily available, portable, radiation‐free imaging modality for quick, non‐invasive, dynamic evaluation of the airway without sedation. This is useful in children with stridor, which is an emergency due to upper airway obstruction requiring immediate management. Several causes of stridor including laryngomalacia, laryngeal cyst, subglottic hemangioma, vocal cord palsy, and lymphatic malformations can be evaluated accurately. Thin musculature and unossified cartilages in children provide a good acoustic window. Thus, airway US is valuable, but underutilized for the evaluation of children with stridor. In this case‐based review, we describe the technique, indications, anatomy, and pathologies on airway US.

Role of Hysteroscopy and Laparoscopy in Evaluation of Abnormal Uterine Bleeding

Abstract:Abnormal uterine bleeding poses a real challenge for the gynecologists Dilatation and curettage has been the gold standard for differentiating benign from malignant endometrium. Hysteroscopy and laparoscopy are the recent addition to the armamentarium of investigations for abnormal uterine bleeding. This study was done to evaluate the role of hysteroscopy and laparoscopy in 75 women with abnormal uterine bleeding. Hysteroscopy findings were compared with histopathology in the diagnosis and etiology of abnormal uterine bleeding. Majority of the patients presented with menorrhagia (40%) and metrorrhagia (20%). Hysteroscopy showed abnormality detection rate of 66% as compared to 26.6% with traditional curettage. Diagnostic accuracy of hysteroscopy was higher in patients with polyps and myoma. Hysteroscopy allows the exclusion of intra uterine pathology with greater precision. Hysteroscopy is superior to curettage, though not a substitute for tissue diagnosis. Laparoscopy was performed in 50 cases including 16 of coexistent infertility, 25 cases with normal hysteroscopy findings and in 9 patients symptoms and signs indicative of pelvic inflammatory diseases were present. An abnormaliity was detected in 40% of these cases in the form of adhesions, leiomyomas, ovarian cysts, misplaced IUCDs and endometriosis. Thus laparoscopy can be complimentary to hysteroscopy in evaluation and management of patients with abnormal uterine bleeding.
Difficult airway associated with bifid glottis and coexistent subglottic stenosis in a patient with Pallister–Hall syndrome: a case report

Abstract:BackgroundPallister–Hall syndrome is a rare disorder characterized by hypothalamic hamartoma, hypopituitarism, bifid epiglottis, and micrognathia.Case presentationWe describe the airway management under general anesthesia of a 15-year-old female with Pallister–Hall syndrome whose airway was compromised with bifid epiglottis and acquired subglottic stenosis. The three options considered for airway management were tracheal intubation, a supraglottic device, and surgical tracheotomy. Tracheal intubation provides a secured airway, but extubation can be difficult. A supraglottic device minimizes airway injury, but it does not completely protect the airway from aspiration.ConclusionsThe patient’s airway was successfully managed using a supraglottic device with aspiration prophylaxis. Airway management devices should be selected according to each patients’ individual circumstances.
The Prevalence of Difficult Airway and Associated Risk Factors in Pediatric Patients: A Cross-sessional Observational Study

Abstract:Background: Difficult airway remains a great challenge in pediatric anesthesia practice. Previously published data show the prevalence of difficult airways in pediatric population varies in a wide range. However, there is a lack of studies in the Asian region. Methods: This cross-sectional single-center study was conducted in a tertiary pediatric hospital in China from October 2022 to October 2023. The patients who underwent elective surgery under general anesthesia with tracheal intubation were recruited consecutively. Data on patient characteristics, airway assessment, and airway management information were collected. Multivariable logistic regression analysis was performed to detect the independent variables of difficult airway in pediatric patients. Results: A total of 18,491 pediatric patients were included in this study. The overall incidence of difficult airways was 0.22%, 39% of whom were unanticipated. Very few previous airway management information was available in the patients presented with a known difficult airway. Patients with younger age, higher American Society of Anesthesiologists (ASA) physical status classification grade, and presented for craniofacial and thoracic surgery were associated with higher incidence of difficult airway. Further multivariable logistic regression analysis revealed that age ≤28 days (OR=50.48), age between 28days and 1 year (OR=6.053), craniofacial surgery (OR=1.81), and thoracic surgery (OR=0.2465) were independent risk factors of increased incidence of difficult airway. Conclusions: Our study showed the prevalence of difficult airways in pediatric surgical patients. Patient characteristics, age, and type of surgery were identified as the independent factors associated with increased occurrence of difficult airways. Unanticipated difficult airway was not unusual in our study population, even for the patients with previous surgical history.
