Abstract:Objective:To evaluate multiple biopsies and cervical conization in the clinical diagnosis and treatment of cervical cancer.Methods: Two thousand and five hundred females aged 30~49 years in a high risk area for cervical cancer underwent liquid based pap smear,high risk human papilloma virus(HPV) testing(13 sub-types),visual inspection of the cervix using acetic acid(VIA) and visual inspection of the cervix using lugol's iodine(VILI).Those with positive findings in any of these four screening tests underwent digital colposcopic exam,and some had biopsies taken from 4 regions of the cervix as well as endocervical curretage.Seventy-two patients with pathological findings of cervical intraepithelial neoplasia(CIN)2 or greater(including CIN2,CIN3,Carcinoma in situ(CIS) and microinvasive cancer) underwent cervical conization(the conventional first-line treatment),with final treatment(regularly scheduled follow-up or more extensive therapy) based on surgical pathology.Results: Pathological findings of the cone biopsies were correlated with that from colposcopy-directed biopsy in 39 patients(55%).In 16 patients,the diagnosis pointed to cancer;five patients increased from CIN2 to CIN3,and one increased from CIS to microinvasive cancer.Eleven patients(15%) had less findings on surgical pathology,for a total discrepancy rate of 45%.Conclusion:(1)The use of multiple biopsies should be promoted in screening for cervical cancer.(2)Cases of CIN3 should be actively treated.(3)Cervical conization should be the first line method used to diagnose and treat cases of pathology-confirmed high grade cervical changes(CIN2 and greater).
The Effect of Cervical Canal Mucosa Ring Excision on Cervical Columnar Eversion in Loop Electrosurgical Excision Procedure

Abstract:Objective:To study the effect of cervical canal mucosa ring excision on cervical columnar eversion in Loop electrosurgical excision procedure (LEEP).Methods:A prospective randomized control trail was performed in 125 cervical intraepithelial neoplasia(CIN) patients.Cervix and cervical canal were conically excised with triangle electrode in 62 patients in the control group;in the study group,a small ring electrode was put into the cervical canal and the cervical canal mucosa was excised about 0.5～0.8cm,after excision as the control group.Follow-up was performed postoperatively in 1,3,6 months respectively.The operation time,the bleeding volume,the rate of cervical columnar eversion and cervical adhesion or stenosis were compared between the two groups.Results:The rate of cervical columnar eversion in the study group (1/63,1.59%) was lower than that in the control group (9/62,14.52%).There was significantly statistical difference between them(P=0.008).There was no statistical differences between them in the operation time,the bleeding volume,and the rate of cervical adhesion or stenosis (P0.05).Conclusions:The cervical canal mucosa ring excision in LEEP can effectively prevent postoperative cervical columnar eversion.
Loop electrosurgical excision procedure in treating 68 patients with cervical intraepithelial neoplasia

Abstract:Objective:To study the application of loop electrosurgical excision procedure(LEEP),its complications and the prevention measures.Methods:68 cases with cervical intraepithelial neoplasia(CIN)treated by LEEP were analyzed retrospectively.Results:The preoperative diagnosis of 27 cases(39.71%)were in accordance with the postoperative ones.The postoperative diagnosis were downgraded in 37 cases(54.41%),and upgraded in 4 cases(5.88%).Intraoperative bleeding amounted to 10 millilitres in 1 case.The cure rate was 95.59%.1 case with CIN II was found with persistence nature in 1 year.Postoperative pregnancy in all patients occurred 6 times.Conclusions:LEEP is a safe and effective procedure in treating CIN.Satisfactory results can be achieved by mastering surgical indications and standard procedures and paying attention to postoperative pathology.

Abstract:Objective To evaluate the effectiveness of loop electrosurgical excision procedure(LEEP) for the management of cervical intraepithelial neoplasia(CIN) grade Ⅱ. Methods A total of 28 patients were pathologically diagnosed as having CIN grade Ⅱ by colposcopic cervical biopsy.High-risk human papillomavirus(HPV) DNA testing showed positive results in 96.4% of the patients(27/28).The procedure was conducted under surface anesthesia.A loop electrode was used to cut through the cervical tissues,and then a square-shaped electrode or a small-sized loop electrode was utilized to complete the resection of lesions,including parts of the cervical canal.All the patients were followed postoperatively. Results The intraoperative blood loss was 0~20 ml,and the operation time was 5~10 min.Out of the 28 patients,postoperative abnormal vaginal bleeding was found in 3 patients.No other complications occurred.The resected tissues showed no obvious charring changes.The lesions of CIN completely disappeared in 15 patients(53.6%),subsided to grade Ⅰ in 5 patients,remained in grade Ⅱ in 5 patients,and progressed to grade Ⅲ in 3 patients.Follow-up examinations in the 28 patients for 6~24 months(mean,16 months) found no residual lesions or recurrence.The high-risk HPV DNA findings turned negative in 23 patients. Conclusions LEEP is a safe and effective procedure for the treatment of CIN grade Ⅱ,with advantages of little invasion,simplicity of performance,and no need of hospitalization.

Abstract:OBJECTIVE:The aim of this study is to review a cohort of cases with vaginal intraepithelial neoplasia (VAIN) in a Chinese tertiary centre and to analyze the relationship between VAIN and cervical neoplasm, the treatment of the disease, and the outcomes.METHODS:We retrospectively reviewed patients undergoing treatment for VAIN from January 2007 to June 2013. All the data of demographics, history, histological information, and HPV testing results were recorded. Statistical analysis was performed with SPSS 10.0 software.RESULTS:The study comprised of 184 patients. 132 out of 184 cases were diagnosed with cervical cancer or cervical intraepithelial neoplasia (CIN). 19 cases occurred after treatment of cervical neoplasia, and 33 patients were without any cervical lesion at the time of diagnosis. The average age of total patients was 48.30 ± 11.58. There were no significant differences in age, parity, and HPV infection among different groups. No significant differences were found in age, parity, and HPV infection between low-grade VAIN1 and high-grade VAIN (VAIN2 and VAIN3). The main treatment was electrofulguration and focal resection. The primary remission rate was 87.62 %. Four patients of VAIN1 were observed with the result of no recurrence in three cases.CONCLUSION:VAIN is a disease which has a close relationship with cervical neoplasia. Electrofulguration treatment with local resection is a suitable treatment with a much lower recurrence rate. For VAIN 1, observation may be a good choice especially for younger patients.

Abstract:Background: The prevention of cervical cancer can be achieved by treating high-grade cervical precancerous lesions. Treatment options for cervical precancer include excisional procedures, and ablation treatments. Despite the long pre-invasive course of the disease, literature addressing sexual function post-treatment for cervical pre-invasive lesions is scarce. This study aims to bridge this gap and assess the sexual function and the acceptability, efficacy, safety, and complications of loop electrosurgical excision procedure (LEEP) versus thermal ablation. Methods: The prospective open-label randomized controlled trial recruited women aged 22-55 with histologically confirmed Cervical Intraepithelial Neoplasia (CIN) 2 and 3 lesions. Participants were randomly allocated to either thermal ablation or LEEP. All cases were followed up with a Pap smear at three- and six-months post treatment. Sexual health assessments were conducted using a questionnaire at baseline and 3 months post-procedure. Secondary outcome measures included comparison of acceptability, pain, and side effects between the two treatment measures. Results: Out of 1356 screened cases, 60 were included in the study and randomized in two groups. The groups had similar baseline characteristics. Duration of LEEP was longer than thermal ablation (25.33 vs. 20.67 minutes), with higher pain reported 10 minutes post-procedure in the LEEP group. Three months post-procedure, both groups showed comparable acceptability and symptom relief. Sexual function parameters significantly improved in the thermal ablation group compared to LEEP, including satisfaction, desire, lubrication, flexibility, and ability to reach climax. Conclusion: LEEP and thermal ablation are effective treatments for CIN with similar efficacy at 6 months. Thermal ablation demonstrated advantages in procedure time and post-procedural pain but exhibited varying effects on sexual function, improving satisfaction and desire. In contrast, LEEP showed a decrease in satisfaction and potential alterations in lubrication and flexibility. Larger-sample, longer-term studies are recommended for further insights.
Outcome of cervical intraepithelial neoplasia 2 diagnosed by punch biopsy in 131 women

Abstract:Aim: To study the outcome of biopsy-diagnosed cervical intraepithelial neoplasia (CIN) 2 in patients treated by loop electrosurgical excision procedure (LEEP) or cold knife cone biopsy (CKC). Material and methods: Patients (n = 131) were analyzed for histological results on LEEP/CKC specimen. Demographic and risk factors, referral cytology, high risk human papilloma virus (HR HPV) test and follow-up colposcopic clinic visits were studied. Results: Excisional specimens from LEEP/CKC demonstrated no CIN in 20 (15%), CIN 1 in 17 (13%), CIN 2 in 57 (44%) and CIN 3 in 37 (28%). No invasive cancers were identified. The referral Pap smear was high grade or possible high grade in 52 (40%), low grade in 59 (45%) and inconclusive in 20 (15%). Fourteen patients (11%) had positive excisional margins, eight of the 14 were tested for HR HPV infection before LEEP, and all were positive. Post-treatment HPV testing was performed at the 12-month visit in 6 of the 14 patients who had positive excision margins, and all 6 tested were negative. Two women (1.5%) with clear margins had recurrence of CIN 2, both tested positive for HR HPV after treatment. Conclusion: Our data showed that a significant number of women (28%) with biopsy-diagnosed CIN 2 had CIN 1 or no dysplasia on subsequent excisional biopsy. The recurrence risk of high grade dysplasia in CIN 2 is low (1.5%). However, due to the high number of patients (72%) with high grade dysplasia at treatment biopsy, caution needs to be exercised when a conservative approach is adopted in the management of CIN 2.

Abstract:OBJECTIVE:to compare the effect and complications of loop electro-surgical excision procedure (LEEP) and laser CO(2) vaporization in the treatment of cervical intraepithelial neoplasia II.METHODS:a total of 338 CINII women were recruited into this multi-center comparative study. The diagnosis was confirmed by histopathological examination for cervical epithelial cell abnormalities. And colposcopic examination was submitted to LEEP (n = 195) or laser CO(2) vaporization (n = 143) respectively. A post-treatment follow-up of 3, 6 and 12 months was carried out to compare the effect of two methods.RESULTS:among 195 women undergoing LEEP, the frequency of cure, persistent and recurrent CIN was 89.2% (n = 174), 4.1% (n = 8) and 3.6% (n = 7) respectively. And among 143 women receiving laser CO(2) vaporization, the frequency of cure, persistent and recurrent CIN was 86.7% (n = 124), 4.9% (n = 7) and 0.70% (n = 1) respectively. There was no statistical difference in cure rates, persistence or recurrence of CIN (P > 0.05). The recovery time, the operative frequency and intra-operative blood loss were significantly different in two groups.CONCLUSION:both LEEP and CO(2) vaporization are both effective and reliable for the treatment of cervical intraepithelial neoplasia II. However, pathological specimens may be harvested during LEEP. It is of vital importance to conduct preoperative colposcopic assessment and standard postoperative follow-ups.

Abstract:Obstructive sleep apnea syndrome (OSAS) is a complete or partial airway obstruction that can cause significant physiologic disturbance with various clinical impacts. The etiology is multifactorial and its clinical manifestations are night snoring, headache when patient wakes up, day-time sleepiness and degrease of cognitive performance. Some recent international studies suggest that the OSAS prevalence is 2-4% in men and 1-2% in women of average age. The aim of this work was presenting a literature review in Medline concerning Obstructive sleep apnea syndrome. A review of the articles ranging from 1980 to 2014 has been done. Over 2000 articles were found and those which give useful information about etiology, diagnosis, therapy and found results after the study were selected and evaluated. In literature there is no unanimous opinion on the treatment of OSAS. According to the majority of the authors it should be multidisciplinary. The choice of therapy is predicated on the etiology, severity and natural history of the increased upper airway resistance. The importance of a praecox diagnosis and orthodontic therapy in order to re-establish normal function is underlined, since OSAS is linked with high risk of hypertension, cardiovascular diseases, daytime sleepiness, home and work-related accidents, with consequent worsening of life quality.
Effectiveness of cold coagulation in treating high-grade cervical intraepithelial neoplasia: the human papillomavirus evidence of cure

Abstract:We retrospectively analysed post-treatment human papillomavirus (HPV) results in women treated for cervical intraepithelial neoplasia (CIN) to establish if 'virological cure' rates achieved by cold coagulation matched those for large-loop excision of the transformation zone (LLETZ) in the treatment of CIN. The main outcome measure was the rate of non-detection of HPV at 6- and 18-month follow-up. HPV was not detected in 82.0% and 86.0% of women at 6- and 18-month follow-up, respectively. In women with high-grade CIN (CIN 2 or worse), there was no difference in HPV non-detection rates among the two treatment methods at six-month (81.8% for cold coagulation vs. 84.0% for LLETZ, χ2=0.23, p value=.632), and 18-month follow-up (83.3% for cold coagulation vs. 89.2% for LLETZ, χ2=1.46, p value=.227). Cervical cold coagulation provides a high 'virological cure' rate for all grades of CIN, equivalent to that seen with LLETZ.What is already known on this subject? It is well established that CIN cure rates after treatment with cold coagulation are comparable to those of excisional methods and are over 90% on cytological follow-up post-treatment. Furthermore, there are no demonstrable adverse effects on fertility and delivery in pregnancies conceived after cold coagulation according to long-term follow-up studies. In contrast, there is a positive correlation between treatment with LLETZ and the risk of subsequent subfertility and adverse pregnancy outcomes.What the results of this study add? This study provides evidence that cold coagulation achieves similar cure rates to that of LLETZ not only in cytology but also in HPV test of cure. This finding is significant as we are moving to a primary HPV cervical screening programme.What the implications are of these findings for clinical practice and/or further research? A negative high-risk HPV test provides a greater reassurance of a low risk of CIN 3 or cancer than a negative cytology result. Therefore, given parity to LLETZ in 'virological cure' rate and having no known adverse effects on fertility and pregnancy, cold coagulation should be used to treat most cases of CIN, especially in women of reproductive age.

A comparison study of post-operative infection analysis of cold-knife conization and loop electrosurgical excision procedure for cervical high-grade squamous intraepithelial lesion

Abstract:BACKGROUND: High-grade squamous intraepithelial lesion (HSIL) is a premalignant condition of the cervical cancer. Loop electrosurgical excision procedure (LEEP) and cold-knife conization (CKC) were the most effective treatment. Most studies focused on the recurrence rate, positive margin rate, residual disease rate, secondary hemorrhage or cervical stenosis of these two methods. At present, there are few researches about the post-operative infection comparing LEEP with CKC for treating HSIL.METHODS: One hundred and fourteen patients diagnosed as HSIL were underwent cold conization (n=43) or LEEP (n=71), according to 1:2 matching approximately and being divided randomly into two groups. The information, including the post-operational inflammatory complications, the leucorrhea abnormalities, the pathogens isolated from cervical secretions and pathological reports, were collected for comparison.RESULTS: There was no significant difference between them in bleeding, diameter, depth or volume of tissue between two groups. However, the operation time of the CKC group (24.81±11.09) minutes was longer than that of LEEP group (15.79±9.82) minutes. Seventeen patients of the LEEP group were admitted postoperatively as emergencies for secondary-hemorrhage. But it did not happen in CKC group. During the follow-up period, 28 patients (CKC 8 vs. LEEP 20) were diagnosis as reproductive tract infections. Fourteen patients (CKC 6 vs. LEEP 8) had leucorrhea abnormalities. Eighteen patient (CKC 3 vs. LEEP 15) isolated pathogens from their cervical secretions. There was no significant correlation between leucorrhea abnormality and cervical secretion abnormality. The positive rate of cervical secretion infection in the LEEP group was higher than the CKC group (P<0.05). The inflammatory response and process had some pathological difference in post-operation time of two groups, especially in those secondary hemorrhage cases.CONCLUSIONS: These two excision procedures for treating HSIL have their respective advantages and disadvantages. This study indicates that the incidence of persistent cervical infection after the CKC operation with Sturmdorf suturing is lower than that of after LEEP surgery. Clinicians should pay attention to the pathogen isolated from cervical post-operative secretion. It is conducive to find hidden pathogens and control subsequent surgical inflammation.

Intracervical intracapsular myomectomy using loop ligation: a proposed technique

Abstract:Performing laparoscopic myomectomy for an intracervical myoma, especially a single giant myoma, is challenging. Only skilled surgeons attempt this operation. Surgeons frequently encounter cases of massive intraoperative bleeding, difficulty suturing the cervical incision, risk of injuring the neighbouring tissues, haematoma formation and conversion to laparotomy or hysterectomy. We previously developed a the technique of loop ligation of the pseudocapsule and have been using it to treat sizeable intracervical myomas laparoscopically. Its advantages include less blood loss, lower risk of injuries to the neighbouring tissues, maintenance of the cervical canal and sparing of the pseudocapsule. This technique is safe and feasible, and facilitates fertility. TWEETABLE ABSTRACT: The loop ligation technique is safe and feasible for laparoscopic myomectomy involving intracervical myomas.

Abstract:The extraocular fibrosis syndromes are congenital ocular-motility disorders that arise from dysfunction of the oculomotor, trochlear, and abducens nerves and/or the muscles that they innervate. Each is marked by a specific form of restrictive paralytic ophthalmoplegia with or without ptosis. Individuals with the classic form of congenital fibrosis of the extraocular muscles (CFEOM1) are born with bilateral ptosis and a restrictive infraductive external ophthalmoplegia. We previously demonstrated that CFEOM1 is caused by an autosomal dominant locus on chromosome 12 and results from a developmental absence of the superior division of the oculomotor nerve. We now have mapped a variant of CFEOM, exotropic strabismus fixus ("CFEOM2"). Affected individuals are born with bilateral ptosis and restrictive ophthalmoplegia with the globes "frozen" in extreme abduction. This autosomal recessive disorder is present in members of three consanguineous Saudi Arabian families. Genetic analysis of 70 individuals (20 affected individuals) reveals linkage to markers on chromosome 11q13, with a combined LOD score of 12.3 at the single nonrecombinant marker, D11S1314. The 2.5-cM CFEOM2 critical region is flanked by D11S4196/D11S4162 and D11S4184/1369. Two of the three families share a common disease-associated haplotype, suggesting a founder effect for CFEOM2. We hypothesize that CFEOM2 results from an analogous developmental defect to CFEOM1, one that affects both the superior and inferior divisions of the oculomotor nerve and their corresponding alpha motoneurons and extraocular muscles.
Outcome after treatment of high-grade squamous intraepithelial lesions: relation between colposcopically directed biopsy, conization and cervical loop excision

Abstract:Purpose of investigation: The hypothesis that cold-knife conization performed in women with high-grade squamous intraepithelial lesions (HSIL) and unsatisfactory colposcopy is a better procedure than the loop electrosurgical procedure (LEEP) is tested. Methods: A retrospective study was conducted in conization specimens of women submitted to LEEP (n = 102) or conization (n = 245) due to HSIL. Age, biopsy, compromised surgical margins in conization, and recurrence were analysed. Results: The frequency of invasion, non-compromised margins, and recurrence in conization and LEEP were, respectively, 7.7% versus 2.9%, 64.1% versus 48% (p = 0.008), and 33.8% versus 24.1%. Eight (42.1%) and five (26.3%) of 19 women submitted to conization where invasion was found in surgical specimens were, respectively, menopausal or had unsatisfactory colposcopy. Twenty-five (96.2%) of 26 and 23 (95.8%) of 24, respectively, undergoing conization and LEEP had recurrence in the first five years. Conclusion: We recommend the use of cold-knife conization in cases where the lesion is located deep in the cervical canal.

Abstract:Abstract Background The effectiveness of surgery of high-grade squamous intraepithelial lesion in post-menopausal women needs to be investigated. This study evaluated the clinical significance of cold-knife conization in the diagnosis and surgery of cervical high-grade squamous intraepithelial lesions in post-menopausal women. Methods We conducted a retrospective analysis of post- and pre-menopausal patients with high-grade squamous intraepithelial lesion. All patients received cold-knife conization as the primary therapy. Results The satisfactory rate of colposcopy was significantly lower in the post-menopausal group than in the pre-menopausal group (38.33 vs. 71.25%; χ 2 = 36.202, P < 0.001). The overall positive margin rate of cold-knife conization (25.83 vs 12.50%; χ 2 = 10.106, P = 0.001) and rate of positive endocervical cone margins (16.67 vs. 4.58%; χ 2 = 14.843, P < 0.001) were significantly higher in the post-menopausal group. Moreover, 49 post- and 60 pre-menopausal women underwent subsequent surgical treatment (40.83 vs. 25.00%). Residual rate of positive and negative margins in patients before and after menopause was significantly different ( χ 2 = 5.711, P = 0.017; χ 2 = 12.726, P < 0.001, respectively). The recurrence rate in post-menopausal women remained 3.85%. Conclusions Cold-knife conization can be performed as a primary procedure for diagnosis and surgery of post-menopausal patients with high-grade squamous intraepithelial lesions. Sufficient deep excisions are necessary to avoid positive endocervical margins, which can reduce the residual and recurrence of postoperative lesions.

Effects of Cervical Physical Therapy and Excisional Therapy on the Prognosis of Cervical Precancerous Lesions

Abstract:Objective To investigate the prognosis of cervical intraepithelial neoplasia (CIN) Ⅱ and Ⅲ after being treated by microwave coagulation, loop electrosurgical excision procedure (LEEP) and cold knife conization. Methods From March 2000 to February 2006, 289 patients with CIN Ⅱ or Ⅲ diagnosed by colposcopic biopsy were treated and followed up according to consensus guidelines for the management of women with CIN. Results Totally 233 of the 289 patients (80.6%) completed the follow-up (3-56 months with a mean of 11.8). Among them, 77.7% (181/233) received treatment; the other 22.3% (52/233) did not. In the 181 patients who received treatment, the lesion disappeared in 160 (88.4%) cases, during the follow-up; while in those who did not received treatment, the CIN was cured spontaneously in 63.5% (33/52; χ2=17.664, P=0.000). The prognosis of CIN Ⅱ was similar among the patients who were treated with microwave coagulation, LEEP, or cold knife conization (χ2=0.008,P=0.996). In addition, in the patients with CIN Ⅲ, the prognosis after cold knife conization was not significantly different from that after LEEP (χ2=0.017,P=0.897). Conclusions The management of CIN Ⅱ and Ⅲ may increase the cure rate. However, the three therapeutic methods produce similar prognosis.

Abstract:Objective: To investigate the clinical character, diagnosis and treatment of cervical intraepithelial neoplasia III (CIN III). Methods: 115 cases of CIN III were analyzed retrospectively. Results: 115 cases submitted to hysterectomy (n=49) and cervical conization (re =66), and 2 cases submitted to subradical hysterectomy after cervical conization. Average follow-up duration was 47 months (6-132 months), and there was no recurrence until June 2005. Conclusions: Emphasis on screening is of importance for young and symptomless female patients. Cervical conization should be the first choice of treatment for CIN III, and the patients are to be followed up carefully after surgery.

Abstract:Objective To compare the efficacy of transcervical resection(TCRC)and loop electrosurgical excision procedure(LEEP)for the treatment of cervical intraepithelial neoplasia grade I(CINⅠ).Methods A total of 231 CIN I patients were divided into two groups according to their patient number to receive TCRC or LEEP.The resected specimens were sent for pathological diagnosis and human papilloma virus(HPV)-16/18 test.Results No significant difference was found in the operation time[(14.1±2.2)min vs.(13.8±2.1)min],rates of wound infection and cervical stricture[1.7%(2/115)vs.1.7%(2/116)and 0.9%(1/115)vs.1.7%(2/116)],and rates of cure and recurrence [99.1%(107/108)vs.99.1%(108/109)and 0.9%(1/108)vs.0.9%(1/109)] between the TCRC and LEEP groups(t=1.060,P=0.290;χ2=0.000,P=1.000;χ2=0.000,P=1.000;χ2=0.000,P=1.000;χ2=0.000,P=1.000).In the TCRC group,the surgical wound was healed in(5.0±0.6)weeks,which was significantly shorter than that in the LEEP group[(5.2±0.7)weeks,t=-2.331,P=0.021].The patients who had severe endocervicitis in both the groups had similar rate of residual endocervicitis[16.7%(2/12)vs.53.8%(7/13),Fisher's test:P=0.097].In both the groups,one patient respectively showed residual lesion after the procedure,in whom HPV-16 and/or HPV-18 were detected.Conclusions Both TCRC and LEEP are effective for CINI.LEEP is easier to master than TCRC.Closed follow-up is essential of the with patients positive HPV-16 or-18.
