Abstract:BackgroundAcne vulgaris affects up to 54% of Chinese adolescents. Combination therapy has become the recommended standard of care for acne.ObjectiveThe aim of this study was to compare the efficacy and safety of clindamycin (1%) and benzoyl peroxide (5%) (CDP/BPO) gel once daily vs. clindamycin (1%) (CDP) monotherapy gel twice daily in Chinese patients with mild to moderate acne.Methods1020 patients (aged 12-45 years) with mild to moderate acne were randomized (1 : 1); 1016 patients were treated with CDP/BPO (n = 500) or CDP (n = 516) for a 12-week treatment period. Efficacy assessments were performed at baseline, and at weeks 1, 2, 4, 8 and 12; and primarily included change in total lesion count (inflammatory and non-inflammatory lesions), and proportion of patients with a minimum 2-grade improvement in Investigator's Static Global Assessment (ISGA) score. Patient safety and local tolerability were also evaluated.ResultsPatients in CDP/BPO group showed a greater per cent reduction in total lesion count compared with patients in CDP group at week 12 (delta = -0.05; 95% CI = -0.09, -0.02; P = 0.003); statistically significant reduction in lesion count was noted as early as week 1 and continued through week 12. A greater proportion of patients in CDP/BPO group showed a 2-grade improvement in ISGA score at week 12 compared with CDP group (30.2% vs. 22.7%; P = 0.018). Overall, the incidence of adverse events (AEs) was higher in the CDP/BPO group (14.4%) than in the CDP group (7.9%); the most commonly reported events were generally related to application site reactions (erythema, pruritus and swelling). Incidence of drug-related AEs was 8.6% in CDP/BPO group and 1.2% in CDP group. Both groups showed trends towards reduction in investigator and subject rated local tolerability scores.ConclusionCDP/BPO gel demonstrated superior efficacy over CDP gel along with acceptable safety and tolerability in Chinese patients with mild to moderate acne.Clinical Trials.gov number: NCT01915732
A comparative study on the effectiveness of herbal extracts vs 2.5% benzoyl peroxide in the treatment of mild to moderate acne vulgaris

Abstract:Background: Although there is a standard guideline for the treatment of acne, it is still a common skin disease, and suboptimal medication adherence is a major reason for treatment failure. Herbal extracts are an interesting alternative medicine because they consist of a variety of active ingredients. Moreover, herbal extracts may have improved therapeutic efficacy because of the combination of various herbs. Objectives: To evaluate the effectiveness of herbal extracts for the treatment of mild to moderate acne vulgaris. Methods: A total of 77 patients were randomized to receive either an herbal extract or 2.5% benzoyl peroxide, which were applied for a period of 12 weeks. Acne lesion counts, adherence, porphyrin counts, the Dermatology Life Quality Index, satisfaction and side effects were assessed. Result: At the 12-week point, the acne lesion counts decreased, with statistically significant differences from the baseline values in both groups and for all types of acne (P-value < 0.001). The adherence rate was significantly higher in the patients using the herbal extract than in the patients using 2.5% benzoyl peroxide (P-value = 0.002). There was no statistically significant difference in terms of porphyrin counts, spot scores, the Dermatology Life Quality Index or satisfaction with efficacy between the groups; however, satisfaction with drug administration was significantly higher in the patients using the herbal extract (P-value = 0.001). Conclusion: Herbal extracts could be beneficial for anti-acne pharmaceutical preparations and may be used as an alternative medicine for patients with mild to moderate acne vulgaris who do not adhere to benzoyl peroxide treatment.

Abstract:To The Editor: Acne vulgaris is a chronic inflammatory skin disease involving the sebaceous glands. It has an incidence of approximately 9.4%, often occurring in adolescents. [1] The disease itself and the scars left after treatment can cause psychological stress in adolescents and seriously affect patients' quality of life. [2] At present, skin care products, either alone or in combination with medical drugs, have become important means for treating acne. This single-center, single-blind (clinician-blind), masculine-paralleled, randomized study (Registration number: ChiCTR2100051398) evaluated the efficacy and safety of a cream containing octyl salicylic acid, salicylic acid, linoleic acid, nicotinamide, and piroctone olamine (Duo+, La Roche-Posay, Paris, France) alone and in combination with benzoyl peroxide (BPO, Galderma, Paris, France) for the treatment of mild-to-moderate acne. The research was approved by the Ethics Committee of Shanghai Skin Disease Hospital, China (No. 2020-13 ke). All participants provided written informed consent.

Abstract:Topical dapsone is an alternative medication for acne treatment. However, which topical dapsone formulation is superior remains uncertain. Furthermore, data on the efficacy of dapsone compared with other topical acne treatments are lacking. We aimed to review the efficacy and safety of topical dapsone at different concentrations and to compare it with other topical acne treatments. We systematically reviewed literature related to the use of topical dapsone in treating acne published from January 2005 to September 2022. We searched databases from selected research studies for inclusion criteria and performed a network meta‐analysis to compare the efficacy of using dapsone at different concentrations. Nine eligible studies were identified; among these, two studies with 7,350 patients were analyzed using network meta‐analysis. At 12 weeks, the percentage of achieving a Global Acne Assessment Score and the mean percentage reduction in inflammatory acne were higher with 7.5% than with 5% dapsone, but the difference was not statistically significant. However, the mean percentage reduction in noninflammatory acne and total acne lesion‐count at 12 weeks was lower with 7.5% than with 5% dapsone, but the difference was not statistically significant. Both concentrations of dapsone were more effective in treating inflammatory than comedonal acne and particularly effective in female patients and those aged ≥18 years. The side effects of dapsone were mild. Thus, topical dapsone is an effective alternative treatment for acne. Both concentrations of topical dapsone are effective in treating acne with no significant difference in efficacy and minimal side effects.

DMT310, a novel once-weekly topical treatment for patients with moderate-to-severe acne vulgaris: Results of a phase 2b randomized, double-blind, placebo-controlled trial

Abstract:Background: Poor patient adherence with antiacne medications is a common clinical challenge. DMT310, a natural, topical product with a once-weekly application schedule, may alleviate this obstacle. Objective: Evaluate the safety, tolerability, and efficacy of DMT310 in treating moderate-to-severe acne. Methods: This 12-week, randomized, double-blind, placebo-controlled, multicenter clinical trial enrolled participants 12 years and older with moderate-to-severe acne. Results: The intent-to-treat population included a total of 181 participants (DMT310, N = 91; placebo, N = 90). Participants who received DMT310 vs participants treated with placebo demonstrated a statistically significant greater reduction in the number of inflammatory and noninflammatory lesions at all time points: inflammatory lesion counts at week 12 (-15.64 vs -10.84, P < .001); noninflammatory lesion counts at week 12 (-18.26 vs -12.41, P < .001). DMT310-treated participants also had higher rates of Investigator's Global Assessment treatment success than participants in the placebo group at all time points: Investigator's Global Assessment at week 12 (44.40% vs 17.78%; P < .001). No serious treatment related adverse events occurred. Conclusions: DMT310 once-weekly topical treatment significantly reduced both inflammatory and noninflammatory lesions and yielded a greater proportion of Investigator's Global Assessment treatment success at all time points in participants with moderate-to-severe acne.

Abstract:Acne is a common skin condition affecting adolescents and young adults, characterized by the blockage of sebaceous glands along with bacterial colonization of the follicular duct and inflammation. Collagen and tissue damage secondary to acne inflammation brings about severe skin texture changes, including scarring. Timely treatment initiation is crucial to prevent acne progression to scarring. Topical retinoids, topical oral antimicrobials, or a combination of both are recommended as the first-line therapy for acne. The objective of the present survey was to assess the role of 0.3% adapalene/2.5% benzoyl peroxide (BPO) combination in those with mild-moderate acne. The objective of the present survey was to assess the role of 0.3% adapalene/2.5% BPO combination in those with mild-moderate acne. : A total of 438 healthcare professionals (dermatologists and cosmetologists) participated in the survey, through which their patients' experiences with the product were recorded. : A significant proportion of the patient population (31%) experienced improvement just after 2 weeks of treatment with the combination. Most patients (84%) also found it easy to include the product in their skincare regime. Most healthcare professionals and patients (>80%) rated the effectiveness and tolerability of the intervention as 'good' to 'excellent'. : The survey confirms the 0.3% adapalene/2.5% BPO combination as an efficacious, easy-to-use topical treatment option for moderate to severe acne with the potential to impact acne scars, in line with findings from other studies examining the effect of this combination.

Abstract:Introduction and objective Acne vulgaris is a common, chronic and inflammatory skin disease of the folliculopilosebaceous unit. According to the literature, acne affects 85% adolescents and 50% adulthood, with an increasing tendency. The symptoms of acne are skin lesions such as comedones, pustules, papules as well as nodules and they generally appear on face, shoulders, chest and back. Based on its severity acne can be divided into following categories: mild, moderate and severe. The pathogenesis of acne is multivariate. Significant importance is attributed to hormonal changes, the role of Cutibacterium acnes bacteria and the balance between C. acnes and Staphylococcus epidermidis, keratinization disorders, excessive sebum production, circadian rhythm disorders, a lot of stress in life and improper nutrition as a result of imbalanced diet. Finding the cause of acne is necessary to cure the disease. The study shows that supplementation seems to be a good solution in the case of acne vulgaris. The aim of the study was to present the most commonly used substances in supplements for acne skin, their properties and possibilities in the treatment of acne vulgaris. The intention of the authors was to draw attention to the possibilities of supplementing dermatological acne treatments with supplements. Pharmacists are often the first point of contact for acne vulgaris patients looking for a way to improve their skin condition. Review methods The PubMed database was reviewed for the most commonly used substances in therapy of acne skin. Publications from the period 2010-2022 were analyzed in terms of the following keywords: „vitamin D”, „omega acids”, „antioxidants”, „berberine”, „lactoferrin” and „inositol” in collocation with „acne”. A review of 31 articles on the possibility of acne supplementation were reviewed. Supplementation of vitamin D, omega acids, antioxidants, berberine, probiotics, inositol and lactoferrin is discussed. Abbreviated description of the state of knowledge The effectiveness of taking supplements containing antioxidants, omega acids, berberine, probiotics, lactoferrin, inositol and vitamin D was discussed. Summary 1. Deficiencies of vitamin D3, vitamins A and E are often noted in patients with acne. 2. Supporting dermatological acne treatments with supplementation with omega acids and probiotics alleviates the side effects of treatment. 3. In the analyzed studies, there were no differences in the effectiveness of omega-3 acids compared to omega-6 acids in supplementation in patients with acne. 4. Multi-component preparations seem to be a better solution than supplements containing only one active ingredient. 5. More research is needed to be able to assess the real effectiveness of individual substances and determine their appropriate dosage.

Abstract:Acne is a chronic inflammatory and immune‐mediated disease of the pilosebaceous unit (the skin structure consisting of a hair follicle and its associated sebaceous gland). It is characterised by non‐inflammatory lesions (open and closed comedones) and inflammatory lesions (papules, pustules, nodules, and cysts). Lesions may be present on the face, thorax, and back, with variable severity. Acne exhibits a global distribution and has a growing prevalence. Acne vulgaris is the most common form. Acne gives rise to complications such as scars and can seriously affect people's mental health, especially those with severe acne. Acne has a huge impact on the quality of life and self‐esteem of those affected. To synthesise the existing evidence on the efficacy and safety of non‐systemic pharmacological interventions and non‐pharmacological interventions (physical therapy and complementary therapies) in the treatment of acne vulgaris and related skin complications. We searched the Cochrane Database of Systematic Reviews, Epistemonikos, MEDLINE, and Embase to 2 December 2021, and checked the reference lists of included reviews. At least two authors were responsible for screening, data extraction, and critical appraisal. We excluded reviews with high risk of bias as assessed with the ROBIS tool. We evaluated the overall certainty of the evidence according to GRADE (as carried out by the authors of the included reviews or ourselves). We provide comprehensive evidence from the review data, including summary of findings tables, summary of results tables, and evidence maps. We retrieved and assessed a total of 733 records; however, only six reviews (five Cochrane reviews and one non‐Cochrane review) with low risk of bias met the overview inclusion criteria. The six reviews involved 40,910 people with acne from 275 trials and 1316 people with acne scars from 37 trials. The age of the participants ranged from 10 to 59 years, with an average age range from 9.8 to 30 years. Four reviews included original trials involving only female participants and three reviews included original trials with only male participants. Main results for clinically important comparisons: Benzoyl peroxide versus placebo or no treatment: In two trials involving 1012 participants over 12 weeks, benzoyl peroxide may reduce the total (mean difference (MD) ‐16.14, 95% confidence interval (CI) ‐26.51 to ‐5.78), inflammatory (MD ‐6.12, 95% CI ‐11.02 to ‐1.22), and non‐inflammatory lesion counts (MD ‐9.69, 95% CI ‐15.08 to ‐4.29) when compared to placebo (long‐term treatment), but the evidence is very uncertain (very low‐certainty evidence). Two trials including 1073 participants (time point: 10 and 12 weeks) suggested benzoyl peroxide may have little to no effect in improving participants' global self‐assessment compared to placebo (long‐term treatment), but the evidence is very uncertain (risk ratio (RR) 1.44, 95% CI 0.94 to 2.22; very low‐certainty evidence). Very low‐certainty evidence suggested that benzoyl peroxide may improve investigators' global assessment (RR 1.77, 95% CI 1.37 to 2.28; 6 trials, 4110 participants, long‐term treatment (12 weeks)) compared to placebo. Thirteen trials including 4287 participants over 10 to 12 weeks suggested benzoyl peroxide may increase the risk of a less serious adverse event compared to placebo (long‐term treatment), but the evidence is very uncertain (RR 1.46, 95% CI 1.01 to 2.11; very low‐certainty evidence). Benzoyl peroxide versus topical retinoids: Benzoyl peroxide may increase the percentage change in total lesion count compared to adapalene (long‐term treatment), but the evidence is very uncertain (MD 10.8, 95% CI 3.38 to 18.22; 1 trial, 205 participants, 12 weeks; very low‐certainty evidence). Sebocytes have been identified as target cells for bioactive vitamin D. Experimental studies on animal models have demonstrated the potent comedolytic effects of topical vitamin D. However, further research is required to specifically evaluate the impact of vitamin D on inflammatory lesions in acne vulgaris (AV). Objective: To evaluate the effectiveness of topical vitamin D in treating acne vulgaris (AV) lesions by investigating its anti-inflammatory effects on pro-inflammatory cytokine modulation, specifically assessing the correlation between IL-1β levels in acne lesions and the reduction in AV severity. Materials and Methods: This study is a double-blind, randomized, placebo-controlled clinical trial with a 2-arm design over an 8-week intervention period. Participants were randomly assigned to either the topical vitamin D group (cholecalciferol 50 mcg) or the topical placebo group, with each group comprising 32 subjects. All participants received concomitant treatment with topical adapalene 0.1%. Cytokine levels within acne lesions were assessed using Luminex Polystyrene Screening Assays to detect and quantify IL-1β levels. The effectiveness of the treatment was evaluated by monitoring the reduction in the number of inflammatory lesions, while the safety of topical vitamin D was assessed by documenting and analyzing any reported side effects. Results: The study found a significant correlation between the reduction in IL-1β levels within acne lesions and the decrease in moderate and severe inflammatory lesions in acne vulgaris (p = 0.028). The topical application of vitamin D led to a significant reduction in inflammatory AV lesions (p = 0.045). No significant topical side effects were observed in either the vitamin D or placebo groups. Conclusion: This study demonstrates that the topical administration of vitamin D in acne vulgaris (AV) lesions is effective in reducing pro-inflammatory cytokine levels within acne lesions and in decreasing the severity of AV. Hypertension and risk of brain metastasis from small cell lung cancer: a retrospective follow-up study.

Abstract:BACKGROUND Although metastatic brain cancer is one of the most common forms of cancer, little is known about the factors associated with its development. We examined the hypothesis that hypertension may increase the risk for brain metastasis (BM) in patients with primary small cell lung cancer (SCLC). MATERIALS AND METHODS A retrospective review of medical charts of patients diagnosed with SCLC between June, 1986 and June, 2003 at MeritCare in Fargo, ND, USA, was done to determine which of these patients subsequently developed brain metastases. The effects of hypertension, age, gender, body mass index and the site of SCLC on the risk of developing BM were examined using both univariate and multivariable Cox proportional-hazards regression models. Two-way interactions between hypertension and other covariates were also included in the analyses. RESULTS Two hundred and thirty-two patients were identified with SCLC and 185 patients were eligible for this study. Eighty-five (45.9%) patients developed BM. Over 54% of SCLC occurred in the right lobe and more than 70% of the patients with BM had them in multiple locations. The risk of BM is significantly higher in younger patients (p-value < 0.03). Univariate analysis showed a hazard ratio (HR) for hypertension of 1.01 (95% Confidence Interval (CI) 0.6-1.6) for BM from SCLC. The multivariable Cox model showed an adjusted HR for hypertension of 1.06 (95%CI 0.7-1.6) for BM from SCLC CONCLUSION: As has been consistently observed for other lung cancers, SCLC is more common in the right lung. The higher incidence of BM in younger patients suggests that more aggressive therapy is needed in these patients. Hypertension does not appear to increase the risk of BM from SCLC.

Abstract:The Global Burden of Disease Study 2020 ranked acne vulgaris as the eighth most common skin condition, with a global estimated prevalence of 9.4%. The clinical presentation of acne vulgaris ranges from comedones to nodules and cysts. The main treatment options for acne are retinoids, antibiotics, and benzoyl peroxide (BPO). This study aimed to evaluate the reported efficacy and safety of the combination treatment with retinoid and benzoyl peroxide. Three databases (ScienceDirect, Google Scholar, and PubMed) were searched using keywords such as acne, adapalene/benzoyl peroxide, and randomized controlled trial, and specific search moods such as "Title and abstract" for PubMed and "Title, abstract, and keywords" for ScienceDirect. On Google Scholar, the "allintitle" option was utilized. The articles were searched for the years between 2003 and 2023. The eight trials selected for systematic review reported randomized controlled trials in 4,596 individuals with the first trials being conducted in February 2007 and the last one in May 2022. There were 52.8% females in the trials, and the trials reported a primary reduction of acne lesions (efficacy) from 27.5% to 70.2%. The occurrence of adverse effects was also variable (ranging from 2.7% to 57.9%), but these effects were mild and vanished with time. The results showed that the combination of adapalene and benzoyl peroxide is a safe and effective treatment.

Abstract:Background Acne is a chronic inflammatory skin disease that negatively affects patients' quality of life. Increasing antibiotic resistance is making acne less responsive to treatment. Probiotics are live microorganisms that can provide health benefits by fighting pathogens and maintaining intestinal homeostasis and skin microbiome balance. This study investigates the effects of probiotics in the treatment of acne vulgaris. Methods In this randomized controlled clinical trial, 80 patients with moderate acne were divided into two groups of 40. All patients received the same topical treatment, which consisted of a daily antibacterial face wash and Adapalene gel every other night. The control group received one capsule of doxycycline (100 mg) daily, whereas the intervention group received one probiotic capsule daily in addition to doxycycline. Patients underwent photography of facial acne lesions, and treatment response was assessed using the global acne grading system (GAGS) and acne grading method at baseline, as well as during follow‐up visits at 1, 2, and 3 months. Results The global acne grading system indicated that both groups showed improvement. However, analyses revealed that outcomes were significantly better in the doxycycline plus probiotics group for the forehead (p = 0.018), chin (p = 0.021), and nose (p = 0.021). No significant differences were observed for the left and right cheeks, back, and chest areas, with the mean GAGS score reduction between the two groups differing by only 2%. Abstract:Background: Acne tarda (AT) is typically known as adult acne and most preferably affected 25-45 age groups. Used isotretinoid monotherapy is not effective and also produces side effects from long treatment. Aim: The present study assesses the effectiveness of combined treatment of oral isotretinoid with topical chemical peeling (20% salicylic and 10% mandelic acid known as SM peeling) by a comparative double-blind randomized single-center interventional open-label study. Materials and methods: A total fifty-eight participants AT outpatients (25-45 age groups) attending for diagnosis in two groups and effectiveness of proposed combined treatments were determined at baseline and follow-up within 4-week interval. Briefly group A (n = 28) received 0.5 mg/kg of oral isotretinoin once daily for 16 weeks and group B (n = 30) received the same oral dose, addition with SM peeling every 4 weeks interval for 16 weeks. After 4 weeks, treatment efficacy was assessed based on Michelson's acne severity index (MASI) and visual analog scale (VAS), further validated using statistical tools. Results: Based on MASI and VAS scores, combined treatment was significantly effective than the monotherapy. The pre- and post-treatment analyses of response to scarring, inflammatory components at baseline and at the end of treatment were also statistically significant with p > 0.008. Conclusion: The above investigation revealed that the combination of oral isotretinoin with SM peeling was highly effective and could be used as newer therapy against AT without any serious side effects.

Abstract:Introduction: Vitamin D3 plays an important role in the immune system, and its deficiency has been implicated in various skin diseases, including atopic dermatitis and psoriasis. Acne is a common inflammatory skin disease of adolescence and young adulthood that affects the face and upper trunk. The scarring caused by acne can have a significant psychological impact on patients and greatly affect their quality of life. However, the association between the severity of acne and vitamin D3 levels remains unclear. Aim: To assess the role of vitamin D3 in the severity of acne vulgaris in adolescents. Materials and Methods: This prospective cross-sectional study was conducted at the Department of Dermatology and Biochemistry, VIMSAR, Burla, Sambalpur, Odisha, India, from August 2022 to November 2022. Adolescents between the ages of 10 and 19 years, of either sex, with acne were clinically examined, and the severity of acne was evaluated using the Global Acne Grading System (GAGS) score. Serum vitamin D3 levels were measured using a Chemiluminescence Immunoassay (CLIA) analyser (Electra FA). Statistical analysis was performed using Statistical Package for the Social Sciences (SPSS) software version 21.0 (SPSS IBM Corporation, Armonk, New York), and a p-value <0.05 was considered statistically significant. Results: A total of 150 clinically diagnosed cases of acne vulgaris patients were evaluated. The study included 73 males and 77 females. The mean age of males was 19.1±2.3 years, and for females, it was 18.2±2.4 years. Serum vitamin D3 levels were lower in patients with severe acne (18.03 ng/mL) compared to those with moderate acne (23.57 ng/mL) and mild acne (28.02 ng/mL). The association between serum vitamin D3 concentration and the different degrees of acne vulgaris was statistically significant (p<0.05). There was a strong negative correlation between severe acne and serum vitamin D3 levels (r=-0.042, p-value=0.002). Conclusion: Vitamin D3 deficiency was more prevalent in patients with severe acne, and serum vitamin D3 levels were inversely correlated with the severity of acne vulgaris.

Combining active ingredients targeting the different mediators of acne pathogenesis may yield optimal outcomes. Purpose: The purpose of this study was to evaluate the clinical effectiveness, safety and tolerability of a new topical medical device in cream containing benzoylperoxide 4%, pure retinol 0.05%, palmitate retinol 0.5%, mandelic acid 1% and glycyrrhetic acid on patients with mild acne. Patients and methods: Twenty consecutive patients of both sexes with mild acne were included in the study. The topical treatment was self-applied twice a day for 12 weeks. Evaluations included: Global Acne Grading System (GAGS); inflammatory and non-inflammatory lesions count; reflectance confocal microscopy; seborrhea and hydration degree; photographic documentation; a questionnaire to assess tolerability. Results: The GAGS score showed a 39% reduction from T0 to T1 and 69.20% from T0 to T2. The count of comedonic lesions showed a 44% reduction from T0 to T1 and 65% from T0 to T2. The count of papular lesions diminished by 49.4% from T0 to T1 and by 62% from T0 to T2. The count of pustular lesions decreased by 43% from T0 to T1 and by 80% from T0 to T2. Improvement of hydration and a decrease of seborrhea degree were even observed. These clinical results were confirmed by reflectance confocal microscopy exam. Conclusion: The topical medical device has shown to be clinically effective and well tolerated for the treatment of mild acne. Side effects were mild, transient and well tolerated. The results of our study demonstrated a high tolerability of this new combination of benzoylperoxide 4% and retinol. Furthermore, our results suggested that the studied compound could be considered as a "maintenance treatment" after specific pharmacological treatment, even in more severe types of acne. Abstract:Background: Moderate to severe acne vulgaris is often treated with a combination of an oral antibiotic, topical antibiotic/retinoid, and benzoyl peroxide (BP), but data are limited on the efficacy of this and other combination regimens that incorporate both oral and topical therapies.<BR> Methods: Patients were required to be aged 12-30 years with moderate to severe acne (grades 3-4 acne on the Investigator's Global Assessment [IGA]) and deemed potential candidates for treatment with isotretinoin. Enrolled patients were given triple-combination therapy, defined in this study as oral minocycline HCl extended release 1 mg/kg QD, 6% BP foaming cloths used QD, and clindamycin phosphate 1.2%/tretinoin 0.025% gel applied QD, and were evaluated at baseline and weeks 2, 4, 8, and 12.<BR> Results: A total of 97 patients were enrolled in the study. At week 12, 89% of patients had at least a one-grade improvement from baseline IGA and 96% had at least a one-grade improvement from baseline Global Aesthetic Improvement Scale score. Mean ± SD in- flammatory, non-inflammatory, and total lesion counts decreased from baseline by 61.8% ± 38.3%, 48.8% ± 34.5%, and 56.5% ± 29.9%, respectively. The percentage of patients evaluated as candidates for isotretinoin by independent photographic review was 77% (69/90) at baseline and only 16% (14/90) at week 12. Treatment-related adverse events (AEs) occurred in eight of 97 (8%) patients. Triplecombination therapy was not associated with any serious AEs or AEs leading to discontinuation.<BR> Conclusion: Triple-combination therapy was well tolerated and substantially reduced facial acne lesion counts, with 84% of patients judged to no longer be candidates for isotretinoin therapy by study end. These data support the clinical observation that a triple-combination regimen incorporating oral minocycline (dosed by patient weight), BP foaming cloths 6% QD, and clindamycin phosphate 1.2%/ tretinoin 0.025% gel QD can substantially improve moderate to severe acne vulgaris.

Abstract:BACKGROUND: First-line medications for acne vulgaris include retinoids and antibiotics. Dapsone is a topical drug approved by the U.S. Food and Drug Administration for the treatment of acne. However, due to its side effects, the clinical application of dapsone has not been promoted, and the value of the medication is still unclear. The aim of this study is to determine the efficacy and safety of dapsone gel in patients with acne.METHODS: Systematic searches were performed using the following databases on January 4, 2020: PubMed, EMBASE, Cochrane Central Register of Controlled Trials, China National Knowledge Infrastructure (CNKI), China Biomedical Literature Service System (SinoMed), China Science and Technology Journal Database (CQVIP), and Wanfang Data Knowledge Service Platform. A meta-analysis of randomized controlled trials was then conducted to analyze the efficacy and adverse events of dapsone gel treatment compared with excipient and other drug therapies. RevMan 5.3 software was used to calculate the odds ratio (OR), and the confidence interval (CI) was 95%.RESULTS: Data of 11,424 participants across 7 trials which met the inclusion criteria were analyzed. Meta-analysis showed that dapsone gel alone or dapsone gel combined with isotretinoin was superior to excipient alone or oral isotretinoin alone in the treatment of acne (OR =1.51, 95% CI: 1.38-1.66, P<0.0001 random effects model, I2=0%). This indicates that dapsone gel is effective for the treatment of acne. We also found that dapsone gel is a more effective treatment for females (OR =1.80, 95% CI: 1.46-2.23). There was no significant difference in the incidence of adverse events between the dapsone group and the control group (OR =0.94, 95% CI: 0.82-1.14, P=0.24 random effects model; I2=29%). The common local adverse reactions in the dapsone group, such as dryness, heat, and eczema, were not statistically significant compared with those in the control group, and the side effects were transient.DISCUSSION: Dapsone gel is effective in treating acne, and there is no significant difference in adverse events compared with other drugs.

Abstract:BackgroundProbiotics provide benefits for reducing acne. Previous studies showed an anti‐inflammatory effect of Lactobacillus paracasei. However, the clinical evidence of topical probiotic lotion and acne treatment is still lacking.ObjectiveTo evaluate the efficacy and safety of probiotic‐derived lotion compared to 2.5 % benzoyl peroxide in the treatment of mild to moderate acne vulgaris.MethodsTopical probiotic‐derived lotion was formulated from cell‐free supernatant of L. paracasei MSMC 39‐1. In vivo study showed the ability of the supernatant to inhibit both antibiotic‐resistance and susceptibility strains of C. acnes and inhibit tumor necrosis factor‐α. The patients with mild to moderate acne vulgaris on the face were randomized to receive topical probiotic‐derived lotion or 2.5 % benzoyl peroxide. Acne lesion counts, erythema index, and side effects were assessed after 2 and 4 weeks of treatment.ResultsOne hundred and four acne vulgaris patients were enrolled. After four weeks of treatment, the inflammatory acne lesion counts and erythema index significantly decreased compared to baseline in both the probiotic lotion group and 2.5 % benzoyl peroxide group (p < 0.001 in both groups) without statistically significant difference between the two groups (p > 0.05). However, the comedones were not affected in both groups. Four patients (7.69%) treated with probiotic‐derived lotion and 14 patients (26.92%) treated with 2.5% benzoyl peroxide reported treatment‐associated side effects.ConclusionProbiotic‐derived lotion is safe and effective for treating mild to moderate acne vulgaris, a comparable outcome with 2.5% benzoyl peroxide. It could be an alternative treatment of acne with more minor side effects.
