Abstract:Interprofessional collaborative practice is increasingly recognized as an essential model in health care. This study lends preliminary support to the notion that medical students (including residents) and social work students develop a broader understanding of one another's roles and contributions to enhancing community-dwelling geriatric patients' health, and develop a more thorough understanding of the inherent complexities and unique aspects of geriatric health care. Wilcoxon Signed Rank Tests of participants' scores on the Index of Interdisciplinary Collaboration (IIC) indicated the training made significant changes to the students' perception of interprofessional collaboration. Qualitative analysis of participants' statements illustrated (1) benefits of the IPE experience, including complementary roles in holistic interventions; and (2) challenges to collaboration. The findings suggest that interprofessional educational experiences have a positive impact upon students' learning and strategies for enhanced care of geriatric patients.

Abstract:Journal of Applied Gerontology, Ahead of Print. Interprofessional geriatric education programs enhance trainees' knowledge of older adults, and the valuable contributions health and social care practitioners make to their well-being when specialists work collaboratively. In response to the 2020 COVID-19 pandemic restrictions, in-person geriatric interprofessional education (IPE) programs were redesigned for virtual delivery. Nineteen virtual programs were held between September 2020 and December 2022. Of the 369 health and social care trainees who participated, 67.2% completed both pre- and post-program surveys. Survey instruments included the Interprofessional Collaborative Competency Attainment Survey (ICASS), which measures perceptions associated with patient-centered, team-based, collaborative care. Significant differences were obtained across ICASS domains, including communication, conflict management/resolution, and team functioning, suggesting that virtual programs may enhance attitudes and perceived abilities for interprofessional collaborative practice. Furthermore, participants' perceived understanding of older adult needs improved, as did their interest in geriatrics. Results illustrate that virtual geriatric interprofessional (IP) programs may be viable alternatives to in-person opportunities.

Abstract:Interprofessional education (IPE) and research enhances learning, team-based communication, and cross-disciplinary collaborations, which can result in higher quality care for older adults. Despite the importance of IPE, it remains underutilized in higher education, demonstrating the need for extracurricular IPE opportunities. This study describes an interprofessional research project that brought together faculty, undergraduate, and graduate students from several health and social science disciplines to design and deliver a 15-week healthy aging program for older adults living in the urban Circumpolar North. Five faculty and one graduate research assistant led the project while eight students team-taught weekly, 1-hour sessions in the community focusing on healthy lifestyles within a framework of Persuasive Hope Theory. This paper describes the project, the student training procedures, and reports the results of the student satisfaction survey regarding their involvement with the research. Using a student self-assessment survey, students report gains in thinking like a scientist, increased confidence conducting research tasks, benefits from teamwork, and greater consideration of the needs of older adults in their field of study. Despite a small sample size, this study also suggests that students may be more likely to consider a career working with older adults if given hands-on experiences.

Abstract:Background Assessing the impact of interprofessional education (IPE) on health systems is necessary to understand the value of IPE on workforce factors impacting learners and employers. Purpose To assess the impact of IPE at the University of Kansas Medical Center (KUMC) on workforce recruitment, collaborative practice, and culture through a mixed-methods analysis of responses from workforce stakeholders including graduating students, new alumni, residency program directors (RPDs), and employers. Methods Voluntary participants included graduating health professions students, alumni, RPDs, and employers of KUMC graduates. Data were collected through surveys and focus groups that were administered in 2018–2020. The study used R Studio® and Dedoose® for quantitative and qualitative analysis and was approved by the Institutional Review Board. Results A total of 801 graduating students, 174 alumni, and 42 RPDs completed the surveys. Forty-nine employers participated in focus groups. Data showed 87.6% of graduating students and 54% of alumni agreed that a workplace culture that supports interprofessional collaboration was an important factor when considering place of employment. Seventy-eight percent of RPDs agreed that interprofessional collaborative practice helps with recruitment; similar findings emerged from the employers. Data showed 62.6% of alumni had facilitated interprofessional collaboration at their workplace. Employers discussed the impact on interprofessional collaboration through teaming practices (e.g., huddles/rounds). Data indicate that 96.7% of RPDs agreed that graduates' skills impacted workplace culture. Employers discussed graduates’ team-focused attitudes, and confidence to collaborate and develop relationships with team members. Discussion This innovative assessment approach evaluating the impact of IPE on workforce recruitment, collaboration and culture in the healthcare system can be a model for other IPE programs. The findings about new graduates’ desire for interprofessional collaboration can be a lever for advancing team-based practice models with employers and health-system leadership. Conclusion Diverse workforce stakeholders agree that graduates of IPE programs have a positive impact on interprofessional collaborative practice and workplace culture. Interprofessional collaboration may be important for recruitment of future employees.

Abstract:The vision was audacious. By the turn of the century, a fleet of huge spacecraft bristling with sophisticated instruments would be circling the globe and transmitting back a torrent of data on every slight change in the planet's biosphere. Like a team of nurses and doctors hovering over a celebrity patient with a chronic illness, scientists from around the world would analyze the data and come up with diagnoses of Earth's condition. Politicians could then use the results of this global checkup to set environmental, economic, and scientific policies well into the 21st century. That strategy was laid out in 1989, when President George Bush made the multibillion-dollar Earth Observing System (EOS) the centerpiece of the new U.S. Global Change Research Program. But 6 years and three major redesigns later, managers of the National Aeronautics and Space Administration (NASA) program are struggling to define a less ambitious but more down-to-earth vision that will be more popular with politicians and satisfy unhappy scientists. Whether they succeed will become apparent in the coming weeks, when the National Academy of Sciences (NAS) releases a comprehensive report on the program's status and Congress determines the program's 1996 budget. Much is riding on the outcome. The EOS program nearly rivals the space station in cost-estimates vary from $7 billion to $33 billion-and complexity. It funds thousands of researchers investigating the interaction of land, sea, and atmosphere and how human activity figures into the equation. But the NASA effort lacks strong and broad support from the scientific community, and it now faces a tough challenge in Congress. Many House Republicans dislike it both for its expense and its environmental focus. In addition, they fear the results could lead to more To counter both the political and scientific criticism, NASA managers intend to shift the balance of the program from longterm monitoring to research on shorter term effects. That means a move away from studying decadal global change-modeling and geodynamics, for example-and toward acquiring information immediately useful to such groups as farmers and the fishing industry. They also plan to shrink further the instruments and spacecraft and are considering use of commercial communications satellites. "You had this grand comprehensive vision that started EOS off," says Charles Kennel, NASA's associate administrator for Mission to Planet Earth (MTPE), of which EOS is a part. "But people said, 'Hey-what will EOS do for us next year?' "Robert Price, who heads the mission at Goddard Space Flight Center in Maryland, agrees that the early rhetoric was too esoteric. "There is [immediate] societal value that we never emphasized," he says. "We made it seem there would be no results for 15 years. We didn't do a good job of showing there would be short-term results." The changes are meant to make EOS seem more practical to lawmakers. Senators from farm states are particularly enthusiastic about the shift to a shorter payoff and the focus on climate. And researchers say the near-term focus is more realistic, given the complexity of the global system. "What we want is for EOS to be the springboard for a long-term monitoring system," says Dudley Chelton, an Oregon State University oceanographer. Abstract:Effective teamwork and collaboration among health professionals is a well-recognized strategy toward enhancing patient outcomes. However, there is a lack of understanding on how to best prepare health professionals for collaborative practice. The aim of this research is to gain a better understanding of how graduates of five health professions (dentistry, medicine, nursing, pharmacy, physiotherapy) perceive and experience interprofessional education for collaborative practice (IPECP) throughout their health professions journey, with a focus on transition to practice. This longitudinal study employed an interpretive, narrative methodology to understand interprofessional identity development of 24 individuals who had recently graduated from a health professions program (dentistry, medicine, nursing, pharmacy, physiotherapy) at a Canadian university. Participant experiences were analyzed using narrative analysis. Participants' narratives provided insight into the context, factors and curricular experiences needed for interprofessional identity development and preparedness for collaborative practice. Participants identified the importance of socialization and connection with others, collaborative role models and exposure to collaborative experiences and settings for interprofessional practice. Participants expressed some dissatisfaction with their earliest IPECP experiences and most valued their exposure to 'real-life' practice examples and clinical scenarios. Participants desired more authentic experiences of interprofessional collaboration during their programs. Improving health professionals' interprofessional socialization and collaborative experiences within IPECP is critical to improving patient outcomes. Study findings can inform future curricula and IPECP strategies that create conditions to enhance collaborative practice and ensure the preparedness of a future health workforce with a strong collaborative identity.
The vascular territories in the cerebellum and brainstem: CT and MR study.

Abstract:More than 100 CT and 15 MR studies of infarcts in the cerebellum and brainstem were reviewed to define the most typical distribution of infarcts in the different vascular territories. Posterior inferior cerebellar artery and anterior inferior cerebellar artery territories are variable in size and are in a sort of equilibrium with each other. The posterior inferior cerebellar artery territory in transverse sections reveals a characteristic posterior crescent caused by its cranial posterior extension. The anterior inferior cerebellar artery territory may be limited to the lateral inferior pontine and floccular regions but usually extends over the whole petrosal surface of the cerebellum up to the lateral angle. Superior cerebellar artery territory is the most extensive territory and includes the largest part of the deep white matter. Infarcts in a single-branch distribution, vermian or hemispheric, have a characteristic sagittal or oblique orientation. Watershed cerebellar infarcts can also be recognized. In the brainstem, paramedian, lateral, and dorsal penetrating arteries have characteristic distributions at the medullary, pontine, and mesencephalic levels. With MR, lateral medullary infarcts can be demonstrated. Paramedian penetrating arteries are paired, and symmetric and small infarcts at medullary and pontine levels are sharply delimited on the midline. At the mesencephalic level, infarcts in this distribution usually involve all the arteries originating from the tip of the basilar artery and from the precommunicating segment of the posterior cerebral arteries, resulting in a central mesencephalic infarct with bilateral upward extension in the thalami. The different vascular territories in the cerebellum and in the brainstem are illustrated in schematic drawings in transverse, coronal, and sagittal planes. Knowledge of the vascular territories gained by the multiplanar capabilities of MR, and knowledge of the CT patterns of enhancement and evolution, will improve recognition and definition of infarcts.

Abstract:An interprofessional education (IPE) simulation-based geriatric palliative care training was developed to educate health professions students in team communication. In health care, interprofessional communication is critical to team collaboration and patient and family caregiver outcomes. Studies suggest that acquiring skills to work on health care teams and communicate with team members should occur during the early stage of professional education. The Interprofessional Education Collaborative (IPEC®) competency-based framework was used to inform the training. An evaluation examined attitudes toward health care teams, self-efficacy in communication skills, interprofessional collaboration, and participant satisfaction with the training experience. One-hundred and eleven participants completed pre- and post-training surveys. Overall, a majority of participants (97.3%) were satisfied with the training and reported more positive attitudes toward health care teams and greater self-efficacy in team communication skills. IPE participants had higher collaboration scores compared to observer learners. Further research is needed to explore long-term effects of IPE in clinical practice.

Abstract:Interprofessional education (IPE) aims to prepare health professional students with the knowledge, attitudes and skills required for collaborative healthcare practice. Although positive outcomes have been documented at the completion of university-based IPE experiences, or longitudinally across health care degrees, the literature is unclear on how university-based IPE influences graduate practice. This study therefore explores how health professional graduates experience interprofessional interactions in practice and how these may be connected to their university-based IPE experiences. Interviews with seven health professional graduates who had participated in an 11-week IPE course as part of their pre-licensure degrees were analyzed using interpretative phenomenological analysis. The participants were able to articulate instances of effective and less effective collaboration from their professional experiences, making sense of these experiences with explicit reference to the themes of role understanding, collaborative working relationships, interprofessional communication, patient-centered care and contextual influences; all ideas introduced in their university-based IPE. They connected their understanding of roles, collaborative working relationships and interprofessional communication explicitly to their prior university-based IPE, identifying these learnings as foundational knowledge. This connection was not as explicit for patient-centered care and contextual issues. These findings highlight the critical importance of IPE in preparing health professionals for high-quality contemporary practice.

Abstract:Few post-graduate training programs offer a comprehensive curriculum that includes structured clinical experiences to teach interprofessional care. To address this need, the United States Department of Veterans Affairs, Office of Academic Affiliations funded the Centers of Excellence in Primary Care Education (CoEPCE) from 2011–2019 to provide interprofessional curricula for health profession trainees (HPTs), including physician residents, nurse practitioner residents, pharmacy residents, and psychology residents. We examined changes over time in curricular domains, system impacts, and program practices based on HPT survey data and the qualitative evaluation of narrative feedback. An annual survey was administered to participants. Indirect standardized ratios were calculated for interprofessional professional education (IPE) program domains, system impacts, and program practices. Qualitative responses were coded based on curricular domains and key program components. The study cohort included 369 HPTs. Site and profession standardized indirect ratios across all professions indicated improvements in curricular domains, system impacts, and program practices, with significant differences observed for associated health HPTs as compared to other HPTs for performance improvement. Qualitative data indicated that profession was associated with differences in perceptions of the curriculum. Although improvements occurred over time, our findings support the need for the thoughtful consideration of profession-specific identity characteristics when designing interprofessional curricula.

Abstract:Aims: This study investigated occupational and physical therapy students’ knowledge of gerontology and self-reported interest in working with the geriatric population before and after an inter-professional course in gerontology. Methods: This retrospective analysis used a single-group pretest–posttest design. Participants included 204 occupational and physical therapy students who took a graduate course in gerontology. Results: Increased student knowledge of gerontology was indicated by course content pre- and post-test scores. There was a post-test correlation between subjective knowledge and course content knowledge. Increased content knowledge did not lead to increased interest. The primary factor influencing a change in interest was the course project. Students who did group projects were more likely to report increased interest in working with older adults. Conclusions: An inter-professional course in gerontology positively influenced OT and PT student knowledge of gerontology. Group activities involving interaction with older adults may enhance interest in geriatrics.

Abstract:Interprofessional education is one of the interventions used to increase health care students' motivation for working with older patients. Previous research about such interventions has been conducted without the use of control groups and has given inconclusive results. The objective of the present curricular resource was: Does geriatric paper-based interprofessional education influence students' interest in treating older people? During a one-year period, undergraduate fourth-year medical and third-year nursing students wrote four health care plans for four different paper-based older patient cases. In the intervention group students were paired up in interprofessional couples. In the control group students made the assignment alone. Interest for working with older patients was measured on a 5-point Likert scale before and one year after the intervention. In both groups, no significant change was found. Before-interest score of the interprofessional group was relatively high (3.8) so the non-significant results may be due to a ceiling effect. Nursing students' interest in treating older people at the start of the research was higher than medical students' interest.

Abstract:In Lausanne, Switzerland, Interprofessional Education (IPE) is embedded in the curriculum of every undergraduate healthcare student. Since 2011, five educational and healthcare institutions have implemented a short interprofessional education course to bring together 2307 undergraduates from six different disciplines (medicine, midwifery, nursing, occupational therapy, physiotherapy, medical radiology technician) between 2017 and 2020. This pre-post study aimed to explore how this course influenced students' attitudes toward IPE using a French translation of the validated questionnaire called the "University West of England Interprofessional Questionnaire." Students were asked to complete an online survey prior to and at the end of the IPE course to measure students' attitudes toward interprofessional (IP) relationships and collaborative learning. A total of 942 students answered the survey between 2017 and 2020, before and after the course. Each year, students' attitudes toward IP relationships improved after the course whereas a positive change in students' attitudes toward IP learning was observed only in 2020. A short exposure to an IPE course could improve students' attitudes toward IPE and, more specifically, toward IP relationships. Our findings could inform IP leaders to design repetitive, various, and longitudinal IPE experiences to balance the development of uniprofessional and interprofessional identity.

Abstract:Public Health has been a key contributor in the Interprofessional Education Collaborative expert panel for Interprofessional Education (IPE) programs; however, limited programs have directly addressed population health concepts such as social determinants of health and health disparities in healthcare, which directly impact the health and well-being of patients. Thus, this study presents the development and implementation of population health concepts within a traditional, clinically focused IPE program. Within the IPE program conducted in 2016, more than 575 students from 12 health-related disciplines participated in a 4-month academic course. A new curriculum was developed that included a new module on social determinants of health. Previously developed sessions on roles and responsibilities, standardized patient interviewing, patient care planning, and patient safety were re-examined to incorporate the concepts of social determinants of health and health disparities. Course evaluations reported higher mean scores for each new session when compared to scores from previous years evaluation, when social determinants of health had not been addressed. Findings from this evaluation highlight the importance of developing innovative experiential learning experiences that include public health concepts within IPE in order to create a more fulfilling and enriched curriculum to better prepare healthcare students to address the social determinants of health that they will be encountering in their future practice.

Abstract:Background: Frameworks in higher education can support strategic curriculum change in complex systems. The impact of these frameworks in achieving their stated purpose is less known. An interprofessional education (IPE) framework and related multi-activity curriculum designed to develop health profession graduates with the requisite skills for collaborative care, was introduced in a large university, across eleven health professions. Objective: To determine the utility of an interprofessional framework and impact upon perceived work readiness for collaborative practice. Method: A multimethod design using the context, input, process, product (CIPP) evaluation model was selected taking a social constructivist theoretical stance. Data collection included staffing allocation to IPE, curriculum audit, and reflections from representatives of all health professions courses offered at the institution. Data was analyzed using framework analysis. Participants: Interviews or focus groups were undertaken with academic Faculty (n = 13), recent graduates (n = 24) and clinical supervisors/employers of recent graduates (n = 17). Results: The framework assisted the systematic implementation of interprofessional curriculum across the different health courses at the university. Collaborative work-ready learning outcomes were identified in graduates where targeted curriculum had been implemented across all four domains of the framework. Gaps identified in framework implementation were consistent with gaps identified in graduate knowledge and skills related to collaborative practice. The combination of formal university-based IPE and informal workplace learning as part of clinical placements contributed to achieving the desired learning outcomes. Conclusions: These findings offer insights into the use of shared frameworks to drive specific learning activities related to collaborative practice.

Abstract:Background Little is known about the influence of interprofessional education (IPE) on interprofessional collaboration (IPC), particularly following the COVID 19 pandemic. Aim To examine nursing students' perceived ability to practice IPC after exposure to virtual lectures and immersive clinical IPE activities founded on the competencies from the National Interprofessional Competency Framework. Design A cross-sectional design was used. Methods Third year nursing students (n=105) enrolled in a pediatric theory and clinical completed surveys including: Demographic items, the revised Interprofessional Collaborative Competency Attainment Survey (ICCAS) and the Interprofessional Education Collaborative Competency Self-Assessment (IPEC) (Version 3). Students completed reflective questions about their IPE experiences. Descriptive statistics, paired t-tests, confidence intervals and odds ratio measured the difference from pre- to post-participation for all items of the ICCAS and IPEC. Results Participants reported improved IPC competence on every item and overall, after the IPE activities. Participants from urban campuses had lower ICCAS scores (OR 0.38, 95% CI, 0.12-0.65) than those from rural campuses (OR 0.45 95% CI 0.18 - 0.72). Students who identified as men had significantly higher team functioning scores before participating in the IPE activities than women (OR 0.50; 95% CI: 0.05 - 0.95). The IPEC Interaction items mean score was 4.03 ±.50 and IPE values items were higher with a mean of 4.62 ± 0.46. Conclusions The findings can guide development and implementation of IPE experiences for nursing and allied healthcare students.

Abstract:Introduction: Given the aging population and the benefits of comprehensive geriatric assessment to this subset of patients, an interprofessional education training approach may be advantageous for learners from a number of different health professions.Methods: Through intercollegiate collaborations involving seven different colleges, an interprofessional simulation using standardized patients was developed and instituted for learners in medicine, nursing, pharmacy, occupational therapy, physical therapy, dental hygiene, and dietitian programs. Herein, we describe the design of the simulation experience and examine its impact on students, as assessed primarily via written reflective comments provided via exit slips at the conclusion of the activity.Results: Of the 340 student participants, 83% submitted exit slips describing something gained from the interprofessional session that would not have occurred if students had completed the activity with only students from their own discipline. Three key themes were identified from these reflections: new understanding of roles and responsibilities of other disciplines, new knowledge or skills pertaining to geriatric assessments, and the value of teamwork.Discussion: Reflective comments from students regarding the interprofessional experience are evidence of this initiative's benefits, which include increasing knowledge of geriatric medical and allied health-provided care and attainment of interprofessional competencies.

Abstract:Abstract Background Understanding human factors involved in patient care is a focus of the Geriatric Medicine Training curriculum including the leadership of and communication within an interprofessional team. Geriatric Medicine Specialty Trainees (ST4+) need advanced communication skills and confidence in managing complex situations. Simulation-enhanced Interprofessional Education (Sim-IPE) provides insight into other disciplines roles (1). We hypothesised that the implementation of a Sim-IPE programme dedicated to communication skills could improve confidence. Method We organised a one-day Sim-IPE programme for our Geriatric Medicine ST4+. Faculty consisted of doctors, advanced nurse practitioner, social worker, occupational therapist, physiotherapist and speech and language therapist. Communication scenarios, with faculty as simulated participants, were undertaken including establishing a ceiling of treatment, management of delirium, breaking bad news, ethical feeding dilemmas, complex discharge planning and capacity assessment. Debrief discussions followed each scenario. Mixed-method evaluation was used with questionnaires collected from learners’ pre and post session in a 10-point Likert scale to compare confidence levels. Qualitative feedback was collected from learners and faculty. Results Ten Geriatric Medicine ST4+ attended plus nine faculty including two facilitators. Positive feedback was received from learners and faculty. There was improvement in confidence across all six scenarios (one being “not at all confident” and ten “extremely confident”). Pre-session average confidence was 7.2, post-session 8.7. All trainees would recommend the programme to a colleague. Common learning themes included gaining feedback from debrief discussions and shared learning from our allied-health professionals. Learners and faculty commented on the enjoyment, usefulness and value of learning within a safe team environment. Conclusion Sim-IPE can improve confidence in complex communication scenarios and human factor skills. Feedback highlighted that this programme would be useful for all General Internal Medicine Specialty Trainees.
