Abstract:(1) Background: People living with type 1 diabetes (T1D) require self-management to maintain blood glucose (BG) levels in a therapeutic range through the delivery of exogenous insulin. However, due to the various variability, uncertainty and complex glucose dynamics, optimizing the doses of insulin delivery to minimize the risk of hyperglycemia and hypoglycemia is still an open problem. (2) Methods: In this work, we propose a novel insulin bolus advisor which uses deep reinforcement learning (DRL) and continuous glucose monitoring to optimize insulin dosing at mealtime. In particular, an actor-critic model based on deep deterministic policy gradient is designed to compute mealtime insulin doses. The proposed system architecture uses a two-step learning framework, in which a population model is first obtained and then personalized by subject-specific data. Prioritized memory replay is adopted to accelerate the training process in clinical practice. To validate the algorithm, we employ a customized version of the FDA-accepted UVA/Padova T1D simulator to perform in silico trials on 10 adult subjects and 10 adolescent subjects. (3) Results: Compared to a standard bolus calculator as the baseline, the DRL insulin bolus advisor significantly improved the average percentage time in target range (70-180 mg/dL) from 74.1%±8.4% to 80.9%±6.9% (p<0.01) and 54.9%±12.4% to 61.6%±14.1% (p<0.01) in the the adult and adolescent cohorts, respectively, while reducing hypoglycemia. (4) Conclusions: The proposed algorithm has the potential to improve mealtime bolus insulin delivery in people with T1D and is a feasible candidate for future clinical validation.

Abstract:We propose a dual-hormone delivery strategy by exploiting deep reinforcement learning (RL) for people with Type 1 Diabetes (T1D). Specifically, double dilated recurrent neural networks (RNN) are used to learn the hormone delivery strategy, trained by a variant of Q-learning, whose inputs are raw data of glucose \& meal carbohydrate and outputs are dual-hormone (insulin and glucagon) delivery. Without prior knowledge of the glucose-insulin metabolism, we run the method on the UVA/Padova simulator. Hundreds days of self-play are performed to obtain a generalized model, then importance sampling is adopted to customize the model for personal use. \emph{In-silico} the proposed strategy achieves glucose time in target range (TIR) $93\%$ for adults and $83\%$ for adolescents given standard bolus, outperforming previous approaches significantly. The results indicate that deep RL is effective in building personalized hormone delivery strategy for people with T1D.

Abstract:Several closed or hybrid loop controllers for Blood Glucose (BG) regulation, which are also known as Artificial Pancreas (AP) Systems or Automated Insulin Delivery systems (AIDs), are in development worldwide. Most AIDs are designed and evaluated for short-term performance, with a particular emphasis on the post-meal period. However, if controllers are not adapted properly to account for variations in physiology that affect Insulin Sensitivity (IS), the AIDs may perform inadequately. In this work, the performance of two Reinforcement Learning (RL) agents trained under both piecewise and continuous reward functions is evaluated in-silico for long-term adaptation of a Fully Automated Insulin Delivery (fAID) system. An automatic adaptive discretization scheme that expands the state space as needed is also implemented to avoid disproportionate state space exploration. The proposed agents are evaluated for long-term adaptation of the Automatic Regulation of Glucose (ARG) algorithm, considering variations in IS. Results show that both RL agents have improved performance compared to a rule-based decision-making approach and the baseline controller for the majority of the adult population. Moreover, the use of a continuous shaped reward function proves to enhance the performance of the agents further than a piecewise one.

Abstract:People with Type 1 diabetes (T1D) require regular exogenous infusion of insulin to maintain their blood glucose concentration in a therapeutically adequate target range. Although the artificial pancreas and continuous glucose monitoring have been proven to be effective in achieving closed-loop control, significant challenges still remain due to the high complexity of glucose dynamics and limitations in the technology. In this work, we propose a novel deep reinforcement learning model for single-hormone (insulin) and dual-hormone (insulin and glucagon) delivery. In particular, the delivery strategies are developed by double Q-learning with dilated recurrent neural networks. For designing and testing purposes, the FDA-accepted UVA/Padova Type 1 simulator was employed. First, we performed long-term generalized training to obtain a population model. Then, this model was personalized with a small data-set of subject-specific data. In silico results show that the single and dual-hormone delivery strategies achieve good glucose control when compared to a standard basal-bolus therapy with low-glucose insulin suspension. Specifically, in the adult cohort (n = 10), percentage time in target range 70, 180 mg/dL improved from 77.6% to 80.9% with single-hormone control, and to 85.6% with dual-hormone control. In the adolescent cohort (n = 10), percentage time in target range improved from 55.5% to [Formula: see text] with single-hormone control, and to 78.8% with dual-hormone control. In all scenarios, a significant decrease in hypoglycemia was observed. These results show that the use of deep reinforcement learning is a viable approach for closed-loop glucose control in T1D.

A Deep Learning Framework for Automatic Meal Detection and Estimation in Artificial Pancreas Systems

Abstract:Current artificial pancreas (AP) systems are hybrid closed-loop systems that require manual meal announcements to manage postprandial glucose control effectively. This poses a cognitive burden and challenge to users with T1D since this relies on frequent user engagement to maintain tight glucose control. In order to move towards fully automated closed-loop glucose control, we propose an algorithm based on a deep learning framework that performs multitask quantile regression, for both meal detection and carbohydrate estimation. Our proposed method is evaluated in silico on 10 adult subjects from the UVa/Padova simulator with a Bio-inspired Artificial Pancreas (BiAP) control algorithm over a 2 month period. Three different configurations of the AP are evaluated -BiAP without meal announcement (BiAP-NMA), BiAP with meal announcement (BiAP-MA), and BiAP with meal detection (BiAP-MD). We present results showing an improvement of BiAP-MD over BiAP-NMA, demonstrating 144.5 ± 6.8 mg/dL mean blood glucose level (−4.4 mg/dL, p< 0.01) and 77.8 ± 6.3% mean time between 70 and 180 mg/dL (+3.9%, p< 0.001). This improvement in control is realised without a significant increase in mean in hypoglycaemia (+0.1%, p= 0.4). In terms of detection of meals and snacks, the proposed method on average achieves 93% precision and 76% recall with a detection delay time of 38 ± 15 min (92% precision, 92% recall, and 37 min detection time for meals only). Furthermore, BiAP-MD handles hypoglycaemia better than BiAP-MA based on CVGA assessment with fewer control errors (10% vs. 20%). This study suggests that multitask quantile regression can improve the capability of AP systems for postprandial glucose control without increasing hypoglycaemia.

Abstract:The existing adaptive basal-bolus advisor (ABBA) was further developed to benefit patients under insulin therapy with multiple daily injections (MDI). Three different in silico experiments were conducted with the DMMS.R simulator to validate the approach of combined use of self-monitoring of blood glucose (SMBG) and insulin injection devices, e.g. insulin pen, as are used by the majority of type 1 diabetes patients under insulin therapy. The proposed approach outperforms the conventional method, as it increases the time spent within the target range and simultaneously reduces the risks of hyperglycaemic and hypoglycaemic events.

Abstract:The use of automated insulin delivery (AID) has led to a decrease in the burden of diabetes, allowing for better sleep, decreased anxiety about hypoglycemia, and automatic corrections doses, and meal recognition algorithms have provided "forgiveness" for imprecise carbohydrate (CHO) entries and missed or late meal boluses. We provide a case report and review of the current literature assessing the effect of AID on the burden of meal bolus. The case also demonstrates how sensor and pump data provide insight into insulin bolus behavior, and access to integrated cloud-based data has allowed for virtual patient visits. Glucose sensor metrics provides time in range and time below range, and the sensor-derived glucose management indicator provides an assessment of the long-term risk of complications when a laboratory glycated hemoglobin is not available.

Performance of an Automated Insulin Delivery System: Results of Early Phase Feasibility Studies

Abstract:Background: Automated insulin delivery (AID) systems have demonstrated improvements in time-in-range (TIR, blood glucose 70-180 mg/dL) without increasing hypoglycemia. Testing a closed-loop system in an inpatient environment with supervised challenges allows for initial evaluation of performance and safety of the system. Methods: Adults with type 1 diabetes (T1D) were enrolled into two similar studies (n = 10 per study), with 3-day inpatient analysis periods. Participants tested a Lilly hybrid closed-loop (HCL) system comprising an investigational insulin pump, insulin lispro, a pump-embedded model predictive control algorithm, a continuous glucose monitor (CGM), and an external dedicated controller. Each protocol included meal-related and exercise challenges to simulate real-world diabetes self-management errors. Only study staff interacted with the HCL system. Performance was assessed using standard CGM metrics overall and within prespecified periods. Results: Participants (25% male) had mean ± standard deviation (SD) age 44.7 ± 14.2 years, T1D duration 30.2 ± 11.1 years, A1C 7.2% ± 0.8%, and insulin usage 0.53 ± 0.21 U/(kg·day). Percentage TIR 70-180 mg/dL (mean ± SD) was 81.2 ± 8.4 overall, 85.2 ± 8.1 outside of challenge periods, 97.3 ± 5.3 during the nocturnal periods, and 74.5 ± 16.2 for the postprandial periods. During challenge periods, percentage TIR for the overbolus challenge was 65.4 ± 29.2 and that for the delayed bolus challenge was 57.1 ± 25.1. No adverse events (AEs), serious AEs, or unanticipated adverse device events occurred while participants were using the HCL system. Conclusions: In participants with T1D, Lilly AID system demonstrated expected algorithm performance and safety with satisfactory glycemic outcomes overall and in response to simulated diabetes management challenges. Abstract:Self-monitoring of blood glucose (SMBG) and continuous glucose monitoring (CGM) are commonly used by type 1 diabetes (T1D) patients to measure glucose concentrations. The proposed adaptive basal-bolus algorithm (ABBA) supports inputs from either SMBG or CGM devices to provide personalised suggestions for the daily basal rate and prandial insulin doses on the basis of the patients' glucose level on the previous day. The ABBA is based on reinforcement learning, a type of artificial intelligence, and was validated in silico with an FDA-accepted population of 100 adults under different realistic scenarios lasting three simulated months. The scenarios involve three main meals and one bedtime snack per day, along with different variabilities and uncertainties for insulin sensitivity, mealtime, carbohydrate amount, and glucose measurement time. The results indicate that the proposed approach achieves comparable performance with CGM or SMBG as input signals, without influencing the total daily insulin dose. The results are a promising indication that AI algorithmic approaches can provide personalised adaptive insulin optimization and achieve glucose control-independent of the type of glucose monitoring technology.
Offline Deep Reinforcement Learning and Off-Policy Evaluation for Personalized Basal Insulin Control in Type 1 Diabetes

Abstract:Recent advancements in hybrid closed-loop systems, also known as the artificial pancreas (AP), have been shown to optimize glucose control and reduce the self-management burdens for people living with type 1 diabetes (T1D). AP systems can adjust the basal infusion rates of insulin pumps, facilitated by real-time communication with continuous glucose monitoring. Deep reinforcement learning (DRL) has introduced new paradigms of basal insulin control algorithms. However, all the existing DRL-based AP controllers require extensive random online interactions between the agent and environment. While this can be validated in T1D simulators, it becomes impractical in real-world clinical settings. To this end, we propose an offline DRL framework that can develop and validate models for basal insulin control entirely offline. It comprises a DRL model based on the twin delayed deep deterministic policy gradient and behavior cloning, as well as off-policy evaluation (OPE) using fitted Q evaluation. We evaluated the proposed framework on an in silico dataset generated by the UVA/Padova T1D simulator, and the OhioT1DM dataset, a real clinical dataset. The performance on the in silico dataset shows that the offline DRL algorithm significantly increased time in range while reducing time below range and time above range for both adult and adolescent groups. Then, we used the OPE to estimate model performance on the clinical dataset, where a notable increase in policy values was observed for each subject. The results demonstrate that the proposed framework is a viable and safe method for improving personalized basal insulin control in T1D.
Simulation-driven optimization of insulin therapy profiles in a commercial hybrid closed-loop system

Abstract:Background: Automated insulin delivery (AID) has represented a breakthrough in managing type 1 diabetes (T1D), showing safe and effective glucose control extensively across the board. However, metabolic variability still poses a challenge to commercial hybrid closed-loop (HCL) solutions, whose performance depends on customizable insulin therapy profiles. In this work, we propose an Identification-Replay-Optimization (IRO) approach to optimize gradually and safely such profiles for the Control-IQ AID algorithm. Methods: Closed-loop data are generated using the full adult cohort of the UVA/Padova T1D simulation platform in diverse glycemic scenarios. For each subject, daily records are processed and used to estimate a personalized model of the underlying insulin-glucose dynamics. Every two weeks, all identified models are integrated into an optimization procedure where daily basal and bolus profiles are adjusted so as to minimize the risks for hypo- and hyperglycemia. The proposed strategy is tested under different scenarios of metabolic and behavioral variability in order to evaluate the efficacy and convergence of the proposed strategy. Finally, glycemic metrics between cycles are compared using paired t-tests with p<0.05 as the significance threshold. Results: Simulations reveal that the proposed IRO approach was able to improve glucose control over time by safely mitigating the risks for both hypo- and hyperglycemia. Furthermore, smaller changes were recommended at each cycle, indicating convergence when simulation conditions were maintained. Conclusions: The use of reliable simulation-driven tools capable of accurately reproducing field-collected data and predicting changes can substantially shorten the process of optimizing insulin therapy, adjusting it to metabolic changes and leading to improved glucose control.
An ensemble machine learning approach for the detection of unannounced meals to enhance postprandial glucose control

Abstract:Background: Hybrid automated insulin delivery systems enhance postprandial glucose control in type 1 diabetes, however, meal announcements are burdensome. To overcome this, we propose a machine learning-based automated meal detection approach; Methods: A heterogeneous ensemble method combining an artificial neural network, random forest, and logistic regression was employed. Trained and tested on data from two in-silico cohorts comprising 20 and 47 patients. It accounted for various meal sizes (moderate to high) and glucose appearance rates (slow and rapid absorbing). To produce an optimal prediction model, three ensemble configurations were used: logical AND, majority voting, and logical OR. In addition to the in-silico data, the proposed meal detector was also trained and tested using the OhioT1DM dataset. Finally, the meal detector is combined with a bolus insulin compensation scheme; Results: The ensemble majority voting obtained the best meal detector results for both the in-silico and OhioT1DM cohorts with a sensitivity of 77%, 94%, 61%, precision of 96%, 89%, 72%, F1-score of 85%, 91%, 66%, and with false positives per day values of 0.05, 0.19, 0.17, respectively. Automatic meal detection with insulin compensation has been performed in open-loop insulin therapy using the AND ensemble, chosen for its lower false positive rate. Time-in-range has significantly increased 10.48% and 16.03%, time above range was reduced by 5.16% and 11.85%, with a minimal time below range increase of 0.35% and 2.69% for both in-silico cohorts, respectively, compared to the results without a meal detector; Conclusion: To increase the overall accuracy and robustness of the predictions, this ensemble methodology aims to take advantage of each base model's strengths. All of the results point to the potential application of the proposed meal detector as a separate module for the detection of meals in automated insulin delivery systems to achieve improved glycemic control.

Intermittent Control for Safe Long-Acting Insulin Intensification for Type 2 Diabetes: In-Silico Experiment

Abstract:Around a third of type 2 diabetes patients (T2D) are escalated to basal insulin injections. Basal insulin dose is titrated to achieve a tight glycemic target without undue hypoglycemic risk. In the standard of care (SoC), titration is based on intermittent fasting blood glucose (FBG) measurements. Lack of adherence and the day-to-day variabilities in FBG measurements are limiting factors to the existing insulin titration procedure. We propose an adaptive receding horizon control strategy where a glucose-insulin fasting model is identified and used to predict the optimal basal insulin dose. This algorithm is evaluated in \textit{in-silico} experiments using the new UVA virtual lab (UVlab) and a set of T2D avatars matched to clinical data (NCT01336023). Compared to SoC, we show that this control strategy can achieve the same glucose targets faster (as soon as week 8) and safer (increased hypoglycemia protection and robustness to missing FBG measurements). Specifically, when insulin is titrated daily, a time-in-range (TIR, 70--180 mg/dL) of 71.4$\pm$20.0\% can be achieved at week eight and maintained at week 52 (72.6$\pm$19.6%) without an increased hypoglycemia risk as measured by time under 70 mg/dL (TBR, week 8: 1.3$\pm$1.9% and week 52: 1.2$\pm$1.9%), when compared to the SoC (TIR at week 8: 59.3$\pm$28.0% and week:52 72.1$\pm$22.3%, TBR at week 8: 0.5$\pm$1.3% and week 52: 2.8$\pm$3.4%). Such an approach can potentially reduce treatment inertia and prescription complexity, resulting in improved glycemic outcomes for T2D using basal insulin injections.

Abstract:While a typical way for diabetes therapy is discrete insulin infusion based on long-time interval measurement, in this paper, we design a closed-loop control system for continuous drug infusion to improve the traditional discrete methods and make diabetes therapy automatic in practice. By exploring the accumulative function of drug to insulin, a continuous injection model is proposed. Based on this model, proportional-integral-derivative (PID) and fuzzy logic controllers are designed to tackle a control problem of the resulting highly nonlinear plant. Even with serious disturbance of glucose, such as nutrition absorption at meal time, the proposed scheme can perform well in simulation experiments.

Abstract:Automated insulin delivery (AID) systems enhance glucose management by lowering mean glucose level, reducing hyperglycemia, and minimizing hypoglycemia. One feature of most AID systems is that they allow the user to view "insulin on board" (IOB) to help confirm a recent bolus and limit insulin stacking. This metric, along with viewing glucose concentrations from a continuous glucose monitoring system, helps the user understand bolus insulin action and the future "threat" of hypoglycemia. However, the current presentation of IOB in AID systems can be misleading, as it does not reflect true insulin action or automatic, dynamic insulin adjustments. This commentary examines the evolution of IOB from a bolus-specific metric to its contemporary use in AID systems, highlighting its limitations in capturing real-time insulin modulation during varying physiological states.

Abstract:Patients with diabetes who are self-monitoring have to decide right before each meal how much insulin they should take. A standard bolus advisor exists, but has never actually been proven to be optimal in any sense. We challenged this rule applying Reinforcement Learning techniques on data simulated with T1DM, an FDA-approved simulator developed by Kovatchev et al. modeling the gluco-insulin interaction. Results show that the optimal bolus rule is fairly different from the standard bolus advisor, and if followed can actually avoid hypoglycemia episodes.

Attention Networks for Personalized Mealtime Insulin Dosing in People with Type 1 Diabetes

Abstract:Calculating mealtime insulin doses poses a significant challenge for individuals with Type 1 Diabetes (T1D). Doses should perfectly compensate for expected post-meal glucose excursions, requiring a profound understanding of the individual's insulin sensitivity and the meal macronutrients'. Usually, people rely on intuition and experience to develop this understanding. In this work, we demonstrate how a reinforcement learning agent, employing a self-attention encoder network, can effectively mimic and enhance this intuitive process. Trained on 80 virtual subjects from the FDA-approved UVA/Padova T1D adult cohort and tested on twenty, self-attention demonstrates superior performance compared to other network architectures. Results reveal a significant reduction in glycemic risk, from 16.5 to 9.6 in scenarios using sensor-augmented pump and from 9.1 to 6.7 in scenarios using automated insulin delivery. This new paradigm bypasses conventional therapy parameters, offering the potential to simplify treatment and promising improved quality of life and glycemic outcomes for people with T1D.

Data-Driven Robust Control for Type 1 Diabetes Under Meal and Exercise Uncertainties

Abstract:We present a fully closed-loop design for an artificial pancreas (AP) which regulates the delivery of insulin for the control of Type I diabetes. Our AP controller operates in a fully automated fashion, without requiring any manual interaction (e.g. in the form of meal announcements) with the patient. A major obstacle to achieving closed-loop insulin control is the uncertainty in those aspects of a patient's daily behavior that significantly affect blood glucose, especially in relation to meals and physical activity. To handle such uncertainties, we develop a data-driven robust model-predictive control framework, where we capture a wide range of individual meal and exercise patterns using uncertainty sets learned from historical data. These sets are then used in the controller and state estimator to achieve automated, precise, and personalized insulin therapy. We provide an extensive in silico evaluation of our robust AP design, demonstrating the potential of this approach, without explicit meal announcements, to support high carbohydrate disturbances and to regulate glucose levels in large clusters of virtual patients learned from population-wide survey data.
