Abstract:Brazil holds the third highest general and fifth female incarcerated population worldwide. Despite the incarceration ecosystem that may favor the spreading of zoonotic diseases, particularly when unattended animals are present, no comprehensive study has focused on toxoplasmosis dynamics in such environment. Accordingly, the present study has aimed to serologically assess anti-Toxoplasma gondii (IgG) antibodies by indirect immunofluorescent antibody test in inmates, correctional officers, and feral cats at the Women’s State Penitentiary of Parana, southern Brazil. In overall, 230/506 (45.5%; CI 95%: 41.2–49.8) incarcerated women, 31/91 (34.1%; 95% CI: 25.2–44.3) correctional officers, and 23/39 (59.0%; CI 95%: 43.2–72.9) cats were seropositive to anti-T. gondii antibodies. Logistic regression revealed that seropositivity likelihood increased with consumption of raw meat ( p = 0.040 ) and decreased with elementary educational level ( p = 0.001 ). No statistical difference was found comparing seropositivity between inmates and correctional officers ( p = 0.057 ). As women inmates have been considered among the most vulnerable groups in disease morbidity and mortality, seropositivity observed herein may be directly related to vulnerability and high T. gondii oocyst exposure dispersed in cat feces during incarceration.

Abstract:Background Regarding the leprosy transmission through the upper airways, overcrowded locations such as prisons can become a risk to get sick. Like the leprosy hidden endemic demonstrated in male prison population, being interesting to assess the leprosy scene also among confined women. Methods A prospective descriptive study conducted at Female Penitentiary, Brazil. Leprosy Suspicion Questionnaire (LSQ) were applied to the participants, and submitted to specialized dermatoneurological exam, peripheral nerve ultrasonography, and anti-PGL-I serology. Findings 404 female inmates were evaluated, 14 new cases were diagnosed (LG-leprosy group), a new case detection rate (NCDR) of 3.4%, 13 multibacillary, while another 390 constituted the Non-Leprosy group (NLG). Leprosy cases were followed up during multidrug therapy with clinical improvement. The confinement time median was 31 months in LG, similar to NLG, less than the time of leprosy incubation. Regarding LSQ, the neurological symptoms reached the highest x 2 values as Q1–numbness (5.6), Q3–anesthetizes areas in the skin (7.5), Q5–Stinging sensation (5.8), and Q7–pain in the nerves (34.7), while Q4-spots on the skin was 4.94. When more than one question were marked in the LSQ means a 12.8-fold higher to have the disease than a subject who marked only one or none. The high 34% rate of anti-PGL-I seropositivity in the penitentiary, higher levels in LG than NLG. Three additional leprosy cases each were diagnosed on the second (n = 66) and third (n = 14) reevaluations 18 and 36 months after the initial one. Semmes-Weinstein monofilaments demonstrated lower limbs (32.2%) more affected than the upper limbs (25%) with improvement during the follow-up. Interpretation The NCDR in this population showed an hidden endemic of leprosy as well as the efficacy of a search action on the part of a specialized team with the aid of the LSQ and anti-PGL-I serology as an auxiliary tracking tools.

Abstract:This study investigated access to health care among female prisoners in the state of Ceará, Brazil, and screened for common mental disorders. We conducted an analytical cross-sectional study in the only female prison in the state. Ninety detainees participated in the study. All participants were either pregnant or postpartum women or had one or more of the following health problems: hypertension, diabetes mellitus, tuberculosis, syphilis, hepatitis B, HIV/AIDS. The data were collected using a structured questionnaire. The findings reveal that access to health care was limited, violating the fundamental rights of the prisoners. Screening for diseases on admission to prison was limited, especially among non-pregnant women. Differences in health care were found between health conditions, with priority being given to pregnant and postpartum women. Most of the inmates (68.24%) were found to be at risk for common mental disorders (SRQ score > 7). A positive correlation was found between age and mental health problems (p = 0.0002). Despite legislation guaranteeing access to health care in prisons, the prison system is unprepared to meet the health needs of female prisoners with comorbidities and pregnant and puerperal women.

Abstract:Introduction: Studies on human T-lymphotropic virus 1/2 (HTLV-1/2) infection are scarce in incarcerated population. Therefore, this study estimated the prevalence of HTLV-1/2 infection among prisoners of the major penitentiary complex of Goiás State, Central-West Brazil, comparing it with available data from other Brazilian regions. Methods: A cross-sectional study was conducted with 910 prisoners of the major penitentiary complex in the State of Goiás, Central-West Brazil. All participants were interviewed, and their serum samples were tested for anti-HTLV-1/2 using an enzyme-linked immunosorbent assay (ELISA; Murex HTLV-I + II, DiaSorin, Dartford, UK). Seropositive samples were submitted for confirmation by a line immunoassay (INNO-LIA HTLV I/II, Fujirebio, Europe N.V., Belgium). Results: The majority of participants were males (83.1%), between 25 and 39 years old (56.1%; mean age: 31.98 years), self-reported brown ethnicity (56.2%) and reported 9 years or less of formal education (41.4%). Most reported using non-injectable illicit drugs and various sexual behaviors that present risk for sexually transmitted infections (STIs). The prevalence of anti-HTLV-1/2 was 0.33% (95% CI: 0.07–0.96), HTLV-1 (0.22%) and HTLV-2 (0.11%). The two HTLV-1 seropositive prisoners reported high-risk sexual behaviors, and the HTLV-2 seropositive individual was breastfed during childhood (> 6 months) by her mother and three other women. Conclusion: These data revealed a relatively low seroprevalence of HTLV-1/2 in prisoners in Central-West Brazil, and evidence of HTLV-1 and HTLV-2 circulation in the major penitentiary complex of Goiás State. Given the prevalence of high-risk sexual behaviors, there is a crucial need to intensify education and health programs in prisons to effectively control and prevent HTLV-1/2 and other STIs.

Abstract:This study examines the epidemiological and genomic characteristics, along with the transmission dynamics, of SARS-CoV-2 within prison units I and II in Campo Grande, Mato Grosso do Sul, Brazil. Conducted between May and October 2022, it reveals how the virus spreads in the confined settings of prisons, emphasizing the roles of overcrowded cells, frequent transfers, and limited healthcare access. The research involved 1927 participants (83.93% of the total prison population) and utilized nasopharyngeal swabs and RT-qPCR testing for detection. Contact tracing monitored exposure within cells. Out of 2108 samples, 66 positive cases were identified (3.13%), mostly asymptomatic (77.27%), with the majority aged 21–29 and varying vaccination statuses. Next-generation sequencing generated 28 whole genome sequences, identifying the Omicron variant (subtypes BA.2 and BA.5) with 99% average coverage. Additionally, the study seeks to determine the relationship between immunization levels and the incidence of SARS-CoV-2 cases within this enclosed population. The findings underscore the necessity of comprehensive control strategies in prisons, including rigorous screening, isolation protocols, vaccination, epidemiological monitoring, and genomic surveillance to mitigate disease transmission and protect both the incarcerated population and the broader community.

Abstract:Background Imported strongyloidiasis in non-endemic countries has increasingly been diagnosed. The aim of the present study is to describe the main epidemiological and clinical characteristics of patients with imported strongyloidiasis attended in a referral International Health Unit and to detect trend changes over a 12-year period. Methods This is an observational retrospective study including all imported strongyloidiasis cases seen at the International Health Unit Vall d'Hebron-Drassanes (Barcelona, Spain) from January 2009 to December 2020. Epidemiological and clinical characteristics from included patients were collected. Results Overall, 865 cases of imported strongyloidiasis were diagnosed, of whom 472 (54.6 %) were men and mean age was 38.7 (SD 13.4) years. Most cases were diagnosed in migrants (830, 96 %). The distribution of the geographic origin was: Latin America (561, 67.6 %), Sub-Saharan Africa (148, 17.8 %), Asia (113, 13.6 %), North Africa (5, 0.6 %), Eastern Europe (2, 0.2 %), and North America (1, 0.1 %). The main reasons for consultation at the Unit were screening of health status (371, 42.9 %), laboratory test alteration (367, 42.4 %), gastrointestinal symptoms (56, 6.5 %), cutaneous symptoms (26, 3 %), and other clinical symptoms (45, 5.2 %). An increase in the number of cases was observed in the last years of the study period. Conclusions Imported strongyloidiasis has increasingly been diagnosed in our referral unit, mostly due to screening strategies implementation. Most of the patients were young migrants coming from Latin America, with no symptoms at the time of diagnosis. The optimization of screening strategies will increase the detection and treatment of cases, reducing potential complications.

Optic nerve sheath diameter measurement using bedside ultrasound: Is it accurate?

Abstract:Raised intracranial pressure (ICP) is a complication not just of traumatic brain injury and other acute cerebral insults, but also of a number of general medical conditions. Monitoring of ICP is of paramount importance in Neurointensive Care Unit. Increased ICP causes brain insult which may be associated with increased mortality and poor neurological outcomes.[1,2,3] Bedside clinical diagnosis can be difficult as early clinical signs may be misinterpreted, and heavy reliance on cross-sectional imaging studies may further delay diagnosis. Ultrasound is a bedside imaging modality that is increasingly available in most critical care units. The diameter of the optic nerve sheath has been found to be a strong predictor of raised ICP, with a high sensitivity and specificity in multiple studies and in a systematic review.[4] The optic nerve sheath is bound more loosely to the optic nerve closer to the globe. This loose binding creates a much larger, and potentially more distensible, subarachnoid space in this region, which can appear bulbous on ultrasound.[5] Consistent with this, the optic nerve sheath is at its most distensible anteriorly, where it is potentially most reflective of raised ICP. As per a 1996 study, using modern ultrasonography (USG) techniques showed that optic nerve sheath diameter (ONSD) increased by up to 60% at a distance of 3mm behind the globe in comparison to only 35% at 10 mm.[6] This has been confirmed in subsequent studies, indicating that a position 3 mm behind the globe is preferred for measurement.[7] Measurements made at this point are more reproducible since ultrasound contrast is greater at this depth with a linear probe. While papilloedema viewed by funduscopy may take the time to develop, dilatation of the optic nerve sheath occurs much earlier and may be a near instantaneous manifestation of raised ICP.[6,8] Studies of sonographic ONSD have mainly correlated findings with clinical and radiological signs and symptoms of elevated ICP. Intraventricular measurement is the gold standard for measuring ICP. These measurement devices carry many risks, including hemorrhage and infection. These complications partly account for increasing interest in noninvasive methods such as neuroimaging, transcranial Doppler sonography, ONSD USG and computed tomography/magnetic resonance imaging (CT/MRI). In addition, invasive methods may not be available to individual patients due to contraindications, such as coagulopathy, thrombocytopenia, or the lack of local facilities. The optic nerve and its surrounding sheath can be accurately imaged and measured on MRI using a fat-suppressed T2-weighted sequence. MRI is often spoken of as a reference test for ONSD, as it has a higher spatial resolution, and the images offer a more representative calculation of the mean diameter than CT. While Lagreze et al. contend the accuracy of MRI exceeds sonographic methods for determining ONSD,[9] Bauerle showed good scan-rescan reproducibility and good observer agreement in 15 healthy volunteers.[10] Two questions that need to be answered when we talk about bedside measurement of ONSD by ultrasound for diagnosis of raised ICP: First, does the bedside ultrasound measurement of ONSD correlate strongly with a neuroimaging technique such as an orbital MRI; and secondly what should be the cut-off of ONSD values to determine raised ICP. The authors of the current study[11] have attempted to resolve the first question regarding the correlation of ONSD measured by bedside ultrasound and MRI. They have studied 100 adult patients of either sex, diagnosed with meningoencephalitis who were to undergo MRI of the brain and measured the ONSD 3 mm behind the globe using a bedside ultrasound and correlated its measurement with that measured by MRI at the same point. They found a high level of correlation between the ultrasound measurement and MRI measurement of ONSD. A Bland-Altman analysis was done to show agreement between both methods of measurement of ONSD, the analysis showed a significant correlation between ocular sonography and MRI. Previous Studies performed between both modalities to measure ONSD were done in subjects with normal ICP or in cadavers. This is the first study done with a relatively large group subjects with meningoencephalitis and it adds further proof that bedside ultrasound readings of ONSD may be as accurate as MRI, given the proper technique and training. With regards to the second question, there is wide variation in the literature regarding in the optimal cut-off values, when ONSD was compared with invasive ICP monitoring, ranging from 4.8 to 5.9 mm.[12,13] The reasons cited for these varied cut-off include: Heterogeneous patient populations, varied methods for confirming raised ICP, and ethnic difference among the patient population. The current authors have selected cut-off values of 4.6 mm for females, and 4.8 mm for males in accordance to a study by Shirodkar et al.[14] on 101 Indian patients who demonstrated a high level of sensitivity and specificity with these cut-off values for the diagnosis of intracranial hypertension as evident on CT or MRI imaging. However, the current authors have limited themselves to comparing the numerical accuracy of ONSD values obtained with USG and MRI. They have not commented on the presence or absence of MRI findings of raised ICP and hence have not corroborated the cut-off values of ONSD for raised ICP in Indian patients which as per Shirodkar et al.[14] was found to be somewhat less than those from previous studies. Nevertheless, the high level of correlation between MRI and a bedside ultrasound found in this study suggests that ultrasound can be used to detect acute rise in ICP with a high level of accuracy and despite its limitations, USG ONSD measurement is likely to be more reliable than clinical assessment in the diagnosis of intracranial hypertension and has the potential to become one of the first line modalities in detecting increased ICP especially in situations when altered sensorium or administration of sedative medication precludes such assessment.[15] Further, larger studies need to be performed on Indian population to decide an optimal cut-off value for raised ICP and that needs to be correlated with a standard which could be either direct measurement of ICP or by means of characteristic findings on neuroimaging.
Serosurvey of Toxoplasma gondii and Toxocara spp. co-infection in pregnant women in low-income areas of Brazil

Abstract:Introduction: Despite human toxoplasmosis and toxocariasis having been listed among the top six most neglected parasitic zoonoses worldwide, presenting similar associated risk factors and transmission routes, few studies have been conducted in pregnant women and the consequences of concurrent infection remain to be fully established. Accordingly, the present study has serologically assessed the co-infection and associated risk factors for Toxoplasma gondii and Toxocara spp. in pregnant women, assisted by the public Unified National Health System (SUS) in southeastern Brazil. Materials and Methods: Blood samples were collected and tested for IgG antibodies against Toxoplasma gondii by chemiluminescence immunoassay and against Toxocara spp. by enzyme-linked immunosorbent assay (ELISA). An epidemiological questionnaire was applied to gather socioeconomic information to assess the risk factors associated with seropositivity to toxocariasis/toxoplasmosis by univariate analysis followed by logistic regression. Results: Overall, seropositivity was 69/280 (24.6, 95% CI: 19.96–30.01) for T. gondii and 56/280 (20.0, 95% CI: 15.73–25.08) for Toxocara spp. Co-infection was observed in 25/280 (8.9, 95% CI: 6.12–12.85) pregnant women, with increased odds (OR: 3.3, CI 95%: 1.77–6.14, p = 0.0002). Logistic regression revealed that a higher educational level (high school or college) significantly reduced the likelihood of co-infection seropositivity, owning cats increased the odds of toxocariasis, and older pregnant women presented significantly higher T. gondii seropositivity. Conclusion: Co-infection herein highlights the importance of educational programs in the prevention of toxocariasis and toxoplasmosis in pregnant women and other high-risk populations.

Abstract:Strongyloides stercoralis is a widely distributed nematode more frequent in tropical areas and particularly severe in immunosuppressed patients. The aim of this study was to determine factors associated with strongyloidiasis in migrants living in a non-endemic area and to assess the response to treatment and follow-up in those diagnosed with the infection. We performed a multicenter case-control study with 158 cases and 294 controls matched 1:2 by a department service. Participants were recruited simultaneously at six hospitals or clinics in Spain. A paired-match analysis was then performed looking for associations and odds ratios in sociodemographic characteristics, pathological background, clinical presentation and analytical details. Cases outcomes after a six-month follow-up visit were also registered and their particularities described. Most cases and controls came from Latin America (63%–47%) or sub-Saharan Africa (26%–35%). The number of years residing in Spain (9.9 vs. 9.8, p = 0.9) and immunosuppression status (30% vs. 36.3%, p = 0.2) were also similar in both groups. Clinical symptoms such as diffuse abdominal pain (21% vs. 13%, p = 0.02), and epigastralgia (29% vs. 18%, p < 0.001); along with a higher eosinophil count (483 vs. 224 cells/mL in cases and controls, p < 0.001) and the mean total Immunoglobulin E (IgE) (354 U/L vs. 157.9 U/L; p < 0.001) were associated with having strongyloidiasis. Finally, 98.2% percent of the cases were treated with ivermectin in different schedules, and 94.5% met the cure criteria at least six months after their first consultation. Abdominal pain, epigastralgia, eosinophilia, increased levels of IgE and Latin American origin remain the main features associated with S. stercoralis infection, although this association is less evident in immunosuppressed patients. The appropriate follow-up time to evaluate treatment response based on serology titers should be extended beyond 6 months if the cure criteria are not achieved.

Abstract:Strongyloidiasis is a widespread parasitic disease that can be life-threatening in immunosuppressed people. In the Mediterranean basin, autochthonous cases coexist with imported ones. We aimed to assess the utility of different screening methods, along with the frequency of strongyloidiasis and its associated risk factors in migrants and the native population. This cross-sectional study took place from 2019 to 2022 in the area of the Vega Baja Hospital in Alicante, Spain. Screening was performed in people who were immunosuppressed, at risk of immunosuppression, with blood asymptomatic eosinophilia, and in asymptomatic people from highly endemic countries. Screening methods were serological techniques (ELISA), stool parasitological tests (fecal concentration methods and agar plate culture), and a stool molecular test (PCR). Of the 168 participants (62.5% males, 53.0% migrants, 36.3% immunosuppressed, median age 57 years), 14 (8.3%) had confirmed strongyloidiasis, where 6 were confirmed by serology, 4 by PCR, and 4 by both methods. Overall, 9% of the migrants and 7.6% of the native-born patients were infected. Elevated IgE and hemoglobin and Latin American origin were associated with strongyloidiasis diagnosis. Screening with serology alone would have missed 28.6% of cases. We conclude that strongyloidiasis prevalence is high in our population, both in native and migrant groups, and stool PCR is a useful tool to increase case detection.
The Seroepidemiology of Strongyloides stercoralis Infection in Jamaica

Abstract:Strongyloides stercoralis is a soil‐transmitted helminth which is distributed predominately throughout tropical and subtropical regions and is considered a neglected tropical disease. Due to low larval output, traditional microscopic methods lack sensitivity, especially in areas of low endemicity. Serological assays present an opportunity to study the epidemiology of S. stercoralis in areas of low endemicity such as Jamaica. The current study evaluated the seroprevalence of S. stercoralis in a selected subpopulation in Jamaica. An analysis was conducted on 311 archived serum samples previously submitted for investigating viral infections during a fever epidemic between 2014 and 2015. Randomly selected, anonymized sera were tested for the presence of S. stercoralis IgG antibodies using the AccuDiag Strongyloides IgG ELISA Kit. Data including age, sex, clinical diagnosis, and the geographic location of sample submission were recorded to delineate trends in demographic variables. The seropositivity rate of S. stercoralis was 15.43%. The rate among females and males was 16.45% and 14.47%, respectively (χ 2 = 0.2339, p = 0.629). The highest rate was found in middle adulthood (31–50 years) (26.53%; 13/49). The seroprevalence of S. stercoralis was significantly highest in a rural Regional Health Authority (33.33%; 14/42) and least within an urban Health Authority (9.71%; 17/175). Exposure to S. stercoralis appears to be highest in the rural Regional Health Authorities with an island‐wide exposure rate of 15.43%. The rapid ELISA testing method for the detection of IgG antibodies to S. stercoralis used in this study may be useful as part of a combined approach to elucidate the epidemiology of this soil‐transmitted parasite in Jamaica.

Seroprevalence of Treponema pallidum infection among high-risk populations from Brazil

Abstract:Syphilis is a significant public health concern worldwide. According to the 2020 estimates, nearly 7.1 million new cases of syphilis have been reported globally, with over 30% of these cases reported from American nations, particularly Brazil. Concerns have been raised regarding the susceptibility of specific groups to syphilis due to challenges and vulnerabilities that place these groups at a higher risk of infections or complications in the treatment outcomes. The present study aimed to compare the seroprevalence and the factors associated with syphilis among such high-risk groups. The study was designed as a cross-sectional one and was conducted with pregnant women, people living with HIV (PLHIV), people living with tuberculosis (PLTB), indigenous and healthy populations in Mato Grosso do Sul, Brazil. The study was conducted between June 2019 and August 2022, during which the included patients were subjected to treponemal and non-treponemal serological assays. The study also included a survey conducted through a self-reported questionnaire to collect information regarding the participants' demographics and sexual behaviors. A total of 550 samples were collected, with 110 participants in each of the five groups. The results of the study revealed that the seroprevalence of Treponema pallidum infection in pregnant women, PLHIV, PLTB, indigenous and healthy populations of the study region was 10% (n = 11/110), 41.81% (n = 46/110), 17.27% (n = 19/110), 5.45% (n = 6/110), and 8.18% (n = 9/110), respectively. Homosexual orientation (p = 0.04) and a history of sexually transmitted infection (STI) (p = 0.01) were associated with the seroprevalence of T. pallidum infection in PLHIV. However, no such associations were noted in the remaining four groups. The seroprevalence of T. pallidum infection was observed to vary significantly among the different high-risk groups, which highlighted the persistent concern of syphilis, particularly among vulnerable populations. These findings underscore the significance of focused interventions and public health strategies customized to the specific requirements of each of the groups evaluated in the present study to decrease the number of cases of syphilis and thereby prevent future complications in patients with other serious infections.

Abstract:This study describes and compares tuberculosis (TB) data among persons deprived of liberty and the general Brazilian population, from 2007 to 2019, using the Joinpoint tool to observe changes in trends. This study focuses on women and older adults, for HIV testing, and on the number of detainees according to prison capacity. This is a retrospective, quantitative, and analytical study, which uses methods of regression of time series data from secondary data of unrestricted access collected from the Brazilian Information System for Notifiable Diseases (SINAN), Brazilian Institute of Geography and Statistics (IBGE), and from analytical reports made available by the Brazilian National Penitentiary Department (DEPEN). The results show a considerably higher increase in the prevalence of TB in persons deprived of liberty in all perspectives analyzed; increased HIV testing; and a debatable trend of stability in the number of detainees according to prison capacity. When analyzing trends in prevalence, services, and determinants, it is curious to see the temporal non-coincidence in most cases. Clearly, national policies against TB do not have the same effect within prisons and even the National Policy for Comprehensive Health Care for People Deprived of Liberty in the Prison System (PNAISP) showed restricted effects in view of the health situation herein analyzed. Despite working with secondary data of variable reliability, comparisons were reached that can impact health decisions and actions. Although lacking complete and definitive answers, it was possible to launch a new point-of-view on the evolution of questions for which reflection is essential.
Hepatitis B Virus Infection among Prison Inmates in Borno State: Determination of Prevalence of Surface Antigen (HBsAg) and Possible Risk Factors of Disease

Abstract:An epidemiological study on hepatitis B virus infection among prison inmates in Borno state, Nigeria, was carried out using a questionnaire survey to determine the possible risk factors of the disease and serological method [using an Enzyme Linked Immunosorbent Assay (ELISA)] to determine the prevalence of hepatitis B surface antigen (HBsAg) among the inmates. The study was conducted in correctional facilities in Maiduguri, Biu and Bama local government area of Borno State. Out of a total of 300 sera tested, 49 (16.3%) had detectable ELISA antibody to HBsAg. A significant difference (p<0.001) in seroprevalence of HBsAg among inmates of different educational qualifications was observed and those with higher education had the highest prevalence (20.9%). Although there was no significant difference in prevalence of HBsAg among the different age groups, the age group 60-69 years had the highest infection rate (33.3%). Homosexuality and duration of stay in prison are shown to be significantly associated with HBsAg infection (P<0.05) among inmates. A significant gender difference was noted among inmates with the males (16.1%) having higher prevalence when compared with females (20.0%). Analysis of spatial distribution of prevalence of HBsAg showed that Maiduguri prison had (20.5%), followed by Biu prison (12.7%) and Bama prison (6.7%) and there was significant difference (P 0.05) association of prevalence of HBsAg with marital status or occupation of prison inmates. In conclusion, hepatitis b virus infection was found to be prevalent among prison inmates in Borno state with homosexuality and duration of stay in prison as risk factors.

Abstract:Soil transmitted helminths are highly prevalent worldwide. Globally, approximately 1.5 billion people are infected with Ascaris lumbricoides, Trichuris trichiura or hookworm. Endemic countries carry out periodic mass treatment of at-risk populations with albendazole or mebendazole as a control measure. Most prevalence studies have focused on school aged children and therefore control programs are implemented at school level, not at community level. In this study, the prevalence of intestinal helminths, including Strongyloides stercoralis, was examined using a comprehensive laboratory approach in a community in north-western Ethiopia.A cross-sectional survey was conducted on 792 individuals ≥5 years old in randomly selected houses in a rural district. Stools were examined using three techniques: a formol-ether concentration, the Baermann technique and a real time polymerase chain reaction test (these last two specific for S. stercoralis). Statistical analyses were performed between two large age groups, children (≤14 years old) and adults (≥15 years old).The prevalence of helminths was 91.3%; (95% CI: 89.3–93.3%). Hookworm was the most prevalent, 78.7% (95% CI 75.6–81.4%), followed by S. stercoralis 55.7% (95% CI 52.2–59.1%). Co-infection with both was detected in 45.4% (95% CI 42.0–49.0%) of the participants. The mean age of hookworm-infected individuals was significantly higher than non-infected ones (p = 0.003). Also, S. stercoralis infection was significantly associated with age, being more prevalent in adults (p = 0.002).This is the highest prevalence of S. stercoralis detected in Ethiopia so far. Our results highlight the need of searching specifically for infection by this parasite since it usually goes unnoticed if helminth studies rely only on conventional diagnostic techniques, i.e. Kato-Katz. Moreover, the focus of these programs on children undermines the actual prevalence of hookworm. The adult population acts as a reservoir for both hookworm and S. stercoralis and this fact may negatively impact the current control programs in Ethiopia which only target treatment of school aged children. This reservoir, together with a lack of adequate water, sanitation and hygiene, increases the probability of re-infection in children. Finally, the high prevalence of S. stercoralis found calls for a comprehensive diagnostic approach in endemic areas in addition to a revision of control measures that is, adding ivermectin to current albendazole/mebendazole, since it is the drug of choice for S. stercoralis.Soil-transmitted-helminthiases and schistosomiasis have major programs for control within the neglected tropical diseases agenda. Deworming programs, that is, periodic mass drug administration, with albendazole or mebendazole and praziquantel, for at-risk populations, are the main strategy for control and success in interrupting the transmission of these diseases. But despite huge efforts made, programs are still unsuccessful in controlling STH in endemic areas in Africa. In Ethiopia most parts of the country are suitable for helminth transmission. Mapping for identification of risk areas for hookworm, A. lumbricoides, T. trichiura and Schistosoma spp., has been conducted by the government, in order to prioritize the health agenda. The focus of the interventions are school- aged children. Another species of soil-transmitted helminth, Strongyloides stercoralis, of high morbidity, which requires special laboratory methods for diagnosis and a different treatment drug, is highly underestimated and not included in control programs. With the aim of knowing the prevalence of S. stercoralis and other intestinal helminths at the community level, a population in the rural area of north-western Ethiopia was studied using parasitological and molecular techniques. A high prevalence of helminths at the community level and specifically a high prevalence of S. stercoralis were detected. These results suggest there is a need to revise control programs in order to be able to cut transmission and to ensure universal health coverage that "leaves no one behind".

Abstract:Abstract Background Strongyloides stercoralis is an intestinal nematode most commonly found in subtropical and tropical locations. Military service members are believed to be at increased risk of exposure due to their unique occupational exposures in endemic regions. Methods Burden, clinical course, and risk factors associated with all Strongyloides infections within the US Military Health System from fiscal years 2012 to 2019 were assessed by manual chart review of records with International Classification of Diseases, Ninth Revision/Tenth Revision codes for Strongyloides infection. Infection risk in demographic subgroups based on region of birth, military occupation, and age was quantified with univariate analysis and multivariate logistic regression. Results We reviewed 243 charts based on diagnosis coding, yielding 210 confirmed diagnoses (86.4%). Immigrant patients born in Latin America/Caribbean, sub-Saharan Africa, and East Asia/Pacific regions had statistically significant increased risk ratios of infection at 34.4, 32.0, and 22.4, respectively, when compared to patients born in Europe and North America. In univariate analysis, active duty members in the healthcare occupational category had a statistically significant increased risk ratio of infection at 2.31 compared to those outside this occupation. Multivariate logistic regression analysis demonstrated that occupational categories of healthcare, admininstrative/support, warfighter/combat specialist, and engineering/repair/maintenance occupations, being an immigrant patient, and age ≥65 were all associated with statistically significant increased odds ratios for infection. Conclusions In the Military Health System, occupational exposures, region of birth, and age serve as risk factors for Strongyloides infection. Because infections may be chronic, the impact of targeted screening programs to complement routine medical care should be considered.

Methods: This cross-sectional study determined the prevalence of syphilis and its associated risk factors among recyclable waste collectors in Central Brazil. Results: A lifetime syphilis prevalence rate of 7.91% (95% confidence interval: 5.25-11.75) was found among 278 participants. Low educational level, history of sexually transmitted infection, and age ≥ 36 years were associated with a high prevalence of lifetime syphilis. Conclusions: These findings emphasize the need for syphilis prevention, screening, and treatment among recyclable waste collectors, highlighting the potential for the spread of infection in vulnerable populations.
