Abstract:Objective To explore CT diagnostic the worth of intussusception in adults.Methods CT manifest of 15 cases intussusception in adults which were confirmed by pathology and operation were analysled retrospectively.Rusults The cause of 15 cases included colonic carcinomas(n=3);lymphoma(n=2);hemangioma(n=2)polylipoma(n=4);metastatic tumors(n=1); leiomyoma(n=2),colonic polypus(n=1).There were ileocolonic intussusceptions (n=5), small bowel intussusceptions (n=3) and colocolonic intussusception(n=7).Direct signs such as "target sign"(n=12) and "comet tail(n=7) and kidney sign"(n=4).Indirect sign of intussusception like increased bowel wall thickness(n=7),dilataltion of proximal intestintal cavity (n=9),intestintal obstruction(n=6) and mesentery infiltration(n=1) were observed.8 primary lesions(8/15)were found on CT.Conclusion CT has important diagnostic worth and it provide main reference in pathogenic diagnosis of intussusception in adult before operation.

Abstract:Intestinal lymphangiectasia (IL) is a rare disease characterized by dilated lymphatic vessles in the intestinal wall and small bowel mesentery which induce loss of protein and lymphocytes into bowel lumen. Because it most often occurs in the intestine and cannot be detected by upper gastroendoscopy or colonoscopy, and the value of common image examinations such as X-ray and computerized tomography (CT) are limited, the diagnosis of IL is difficult, usually needing the help of surgery. Capsule endoscopy is useful in diagnosing intestinal diseases, such as IL. We here report a case of IL in a female patient who was admitted for the complaint of recurrent edema accompanied with diarrhea and abdominal pain over the last twenty years, and aggravated ten days ago. She was diagnosed by M2A capsule endoscopy as a primary IL and confirmed by surgical and pathological examination.

Abstract:BACKGROUND Colonoscopy is the most frequently used diagnostic and therapeutic tool for the treatment of colorectal diseases. Although the complication rate is low, it can be potentially serious. Intussusception is a rare and severe complication often associated with polypectomy. Only a handful of post-colonoscopy intussusception cases have been reported, making this study a valuable addition to the medical literature. CASE SUMMARY Case 1: A 61-year-old man underwent colonoscopy with polypectomy for chronic abdominal pain. The patient experienced abdominal pain 11 hours later but was still discharged after pain management. He was readmitted due to recurring pain. Computed tomography (CT) showed colo-colonic intussusception. Initial conservative management and attempts at endoscopic reduction failed; therefore, laparoscopic right hemicolectomy was performed. Histopathological examination revealed tubular adenomas in the polyps and inflammation in the resected specimens. Case 2: A 59-year-old woman underwent colonoscopy with polypectomy for a polyp in the transverse colon. She experienced upper abdominal pain, fever, nausea, and vomiting 9 hours after the procedure. Emergency CT and blood tests revealed a colo-colonic intussusception near the hepatic flexure and an elevated white blood cell count. Initial attempts at endoscopic reduction failed and conservative treatment showed no improvement. She underwent successful laparoscopic reduction and recovered uneventfully. Histopathological examination of the resected polyp revealed hyperplasia. CONCLUSION Post-colonoscopy intussusception in adults is rare, and polypectomy may contribute to its occurrence. Early diagnosis is crucial, with prompt CT examination serving as key. After excluding malignancies, conservative management and reduction of intussusception should be considered before surgical bowel resection.

Abstract:Extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue (EMZL), also known as MALT lymphoma, is an extranodal multiorgan-invasive proliferative lymphoma composed of small B cells with variable morphology. It most commonly occurs in the digestive tract, with a high prevalence in the stomach, but EMZL originating in the small intestine is rare and lacks specificity in clinical manifestations, which makes it easy to be misdiagnosed. Herein, we report a rare case of small intestinal EMZL presentation as intussusception in a 32-year-old man. A colonoscopy performed at the local hospital revealed a pedicled polyp about 5 cm × 5 cm in size with a rough surface, and hyperemia was seen in the ileocecal region. He was admitted to our hospital for a polypectomy. A contrast-enhanced computed tomographic (CT) scan suggested ileocolic intussusception, which was subsequently confirmed by a colonoscopy in our hospital. Adult intussusception is relatively rare, with 90% of cases having a known causative mechanism and 40% of cases caused by primary or secondary malignancies. Therefore, we performed a laparoscopic-assisted right hemicolectomy for the patient. The resected specimen showed that the terminal ileum was intussuscepted into the ascending colon, and the intussusception was hyperemia and edema. A 2.5 cm × 2.5 cm × 1.5 cm mass was seen at the end of the intussusception. Postoperative pathology revealed that the mass was EMZL, partially transformed into a large B-cell lymphoma. The patient was transferred to the hematology department and completed a PET-CT showing postoperative manifestations of primary intestinal lymphoma, Lugano staging IE2. Although EMZL was an indolent lymphoma and the patient was in the early stages, the rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone (R-CHOP) regimen was given in view of the histological transformation. The patient is in regular follow-up. This was a rare case of small intestinal mass due to EMZL presented as intussusception in adults, which highlighted laparoscopic-assisted enterectomy as a potential therapeutic approach in the multidisciplinary collaborative therapy of small intestine EMZL.

Abstract:BACKGROUND Monomorphic epitheliotropic intestinal T-cell lymphoma (MEITL) is a rare and rapidly progressive intestinal T-cell non-Hodgkin lymphoma associated with a very poor prognosis and a median survival of 7 mo. Advances in the identification of MEITL over the last two decades have led to its recognition as a separate entity. MEITL patients, predominantly male, typically present with vague and nonspecific symptoms and diagnosis is predominantly confirmed at laparotomy. Currently, there are no standardized treatment protocols, and the optimal therapy remains unclear. CASE SUMMARY We report a case of MEITL that was initially considered to be gastrointestinal stromal tumor (GIST) and Imatinib was administered for one cycle. The 62-year-old man presented with abdominal pain, abdominal distension, and weight loss of 20 pounds. Within 2 wk, the size of the mass considerably increased on computed tomography scans. The patient underwent surgery followed by chemotherapy with CHOP (cyclophosphamide, doxorubicin, vincristine, and prednisone) and stem-cell transplant. A correct diagnosis of MEITL was established based on postoperative pathology. Immunophenotypically, the neoplastic cells fulfilled the diagnostic criteria for MEITL as they were CD3(+), CD4(+), CD8(+), CD56(+), and TIA-1(+). CONCLUSION Given that MEITL has no predisposing factor and presents with vague symptoms with rapid progression, the concomitant presence of abdominal symptoms and B symptoms (weight loss, fever, and night sweats) with hypoalbuminemia, anemia, low lymphocytic count and endoscopic findings of diffuse infiltrating type lesions should alert physicians to this rare disease, especially when it comes to Asian patients. Immediate laparotomy should then be carried out followed by chemotherapy and stem-cell transplant.

Abstract:A 65-year-old man presented with recurrent abdominal pain and vomiting for 10 years. He had undergone inguinal hernia repair 20 years before. A mass was palpated in the right lower abdomen. Laboratory values were notable for C-reactive protein of 21.8 mg/L (<10 mg/L), albumin of 32.6 g/L (35∼52 g/L), and fecal calprotectin of 1070.84 μg/g (<200 μg/g). Computed tomography showed a 3.7 × 3.4 cm liquid-gas entity adhering to the distal ileum (Figure A, arrows). Colonoscopy showed a large ulcer in the terminal ileum (Figure B). Crohn’s disease was suspected, based on the clinical and endoscopic findings. After discussing the pros (a confirmed diagnosis and relief of symptoms) and cons (potential surgical complications) of surgery, the patient, who had been experiencing poor quality of life for years, chose exploratory surgery. During laparoscopic exploration, the distal ileum was found adhering to a mesh and was removed with complete mesh excision (Figure C). The pathology revealed transmural fibrinoid necrosis and inflammatory exudation in the subserosa and serosa (Figure D). The patient was eventually diagnosed with a mesh-related intestinal fistula and had an uneventful postoperative course. Mesh-related visceral complications are rare. The sigmoid, bladder, and small bowel are the 3 most frequently involved organs. Foreign body reaction induced by mesh can result in erosion into adjacent tissue and fistula formation. The chronic process can mimic Crohn's disease, and the invisibility of the mesh material on computed tomography imaging can make it difficult to establish a causal relationship with the previous surgery. Attention to previous surgical history should be part of the work-up for suspected Crohn’s disease.

Adult Colo-Colonic Intussusception in the Setting of Invasive Mucinous Adenocarcinoma: A Case Report

Abstract:BACKGROUND Colonic intussusception is a very rare disease in adults, and if present, is usually manifested by another pathology, such as malignancy. This report describes the diagnosis and treatment of the underlying cause of intussusception, which was spontaneously reduced. CASE REPORT A 39-year-old woman with no significant past medical history presented to St. Joseph's University Medical Center on July 2022 with gradually worsening abdominal pains for 1 year and hematochezia for 3 months. Physical examination was positive for left lower quadrant abdominal tenderness to palpation. A computed tomography scan of the abdomen and pelvis without contrast showed a long segment of intussusception involving the sigmoid colon and rectum, without any noticeable lesions. A repeat computed tomography scan with rectal contrast showed a 2.1×1.1-cm mesenteric mass in the sigmoid colon at the region of the intussusception. The patient was taken for a laparoscopic sigmoid resection with primary anastomosis, showing a 5-cm mass in the sigmoid colon, and surgical pathology confirming neoplastic etiology of intussusception. The patient recovered well after surgery, and was referred for oncological intervention soon afterward. CONCLUSIONS This report displays the importance of the type of imaging modalities with and without contrast to diagnosis and determine underlying causes of intussusception and further guide treatment options.

Abstract:BACKGROUND Appendiceal intussusception is a pathological condition in which the appendix is inverted into the cecum, which may cause symptoms that resemble those of other gastrointestinal disorders and may induce intestinal obstruction. The rarity of this case presentation is the co-occurrence of appendiceal intussusception and cecal adenocarcinoma, a combination that to our knowledge has not previously been reported in the medical literature. This case provides new insights into the complexities of diagnosing and managing overlapping pathologies. CASE SUMMARY A 25-year-old woman presented with persistent periumbilical pain and bloody stools. An initial biopsy showed cecal cancer; however, subsequent colonoscopy and computed tomography findings raised the suspicion of appendiceal intussusception, which was later confirmed postoperatively. This unique case was characterized by a combination of intussusception and adenocarcinoma of the cecum. The intervention included a laparoscopic right hemicolectomy, which led to the histopathological diagnosis of mucinous adenocarcinoma with appendiceal intussusception. The patient recovered well postoperatively and was advised to initiate adjuvant chemotherapy. This case highlights not only the importance of considering appendiceal intussusception in the differential diagnosis, but also the possibility of appendicitis and the atypical presentation of neoplastic lesions. CONCLUSIONS Physicians should consider the possibility of appendiceal intussusception in cases of atypical appendicitis, particularly when associated with neoplastic presentation.

Abstract:Introduction and importance: Intussusception in adults is rare, constituting a miniscule number of bowel obstruction pathologies. Clinical practice often considers it a last-resort diagnosis, as other causes of mechanical small bowel obstruction are more common. The diagnosis is a challenging one to make, as the "telescoping" motion of the intestines fluctuates in a waxing-and-waning nature. In adults, the etiology is predominantly a pathological lead point being either benign or malignant. When encountering an obscure cause of abdominal pain, intussusception caused by a small bowel lipoma may be the culprit. Case presentation: In this unique case, we present the journey of a 55-year-old Emirati male who presented with severe abdominal pain, nausea and vomiting. The patient underwent various imaging modalities, initially an "incidental" lipoma finding in the terminal ileum until a diagnosis of ileo-ileal intussusception emerged in later imaging, given its unusual and fluctuating nature. Ultimately, a resection of the bowel segment with side-to-side anastomosis was done. Clinical discussion: This case report aims to illuminate the diverse clues and incidental findings encountered during our patient's path to a diagnosis. By exploring the elements of this patient's journey to a diagnosis, we aspire to aid future clinicians in navigating the challenges of identifying obstructive pathologies and considering intussusception as a rare yet crucial differential diagnosis. Conclusion: This case highlights the importance of a comprehensive approach to diagnosis and management of patients presenting with obstructive symptoms, incorporating both clinical insight and imaging modalities such as CT to ensure optimal patient outcomes. When confronting such an obscure cause of abdominal pain, intussusception caused by a small bowel lipoma may be the culprit.

Abstract:Primary mantle cell lymphoma (MCL) in the gastrointestinal tract is rare, accounting for 4‐9% of all reported cases of gastrointestinal non‐Hodgkin lymphoma. Furthermore, involvement of the entire gastrointestinal tract in MCL is rare. The present report describes an example of MCL characterized by numerous diffuse polypoid lesions along the whole digestive tract. In particular, there was a focus on the endoscopic presentation of the digestive tract. The patient initially received a treatment regimen of rituximab combined with cyclophosphamide, doxorubicin, vincristine and prednisone. After two cycles of treatment, the regimen was changed to rituximab combined with etoposide, oxaliplatin and ifosfamide, with the addition of ibrutinib capsules. Patients with MCL have a poor prognosis; however, complete response can be achieved after treatment.

Abstract:Adult intussusception is an unusual condition that is infrequently associated with malignancy. We experienced a rare occurrence of ileocecal intussusception in a 31-year-old male, secondary to metastatic melanoma. The patient, with a medical history of scalp malignant melanoma treated surgically and with chemotherapy, presented with persistent lower abdominal pain. Diagnostic modalities, including computed tomography (CT) and colonoscopy, revealed ileocecal intussusception. Subsequent laparoscopic surgery and pathological examination identified metastatic melanoma as the etiological factor for the intussusception, leading to the surgical resection of the involved bowel segment. The postoperative period was marked by a smooth recovery with no complications. This case shows the importance of considering malignancy in the differential diagnosis of adult intussusception and underscores the urgency of surgical intervention in the management of such cases.

Abstract:We present primers and amplification conditions for 15 microsatellite loci developed for the Cope's giant salamander (Dicamptodon copei), 14 of which are tetranucleotide repeats. Cross‐species amplification revealed 10 of these loci to also be polymorphic in the Pacific giant salamander (Dicamptodon tenebrosus). Several loci produced nonoverlapping allelic ranges between the two species and may be useful in species identification. These polymorphic microsatellite loci are potentially useful for future studies of population genetics in dicamptodontid salamanders.

Abstract:Rationale: Intestinal T-cell lymphomas are exceedingly rare diseases. Intestinal T-cell lymphoma NOS, as a "wastebasket" category, is difficult to diagnosis. Endoscopy can identify abnormal mucosa in most patients at a reasonably early stage. Therefore, it is crucial to increase the understanding of endoscopists in terms of the endoscopic characteristics of ITCL. Patient concerns: A 74‐year‐old male alone with wasting as the major complaint, had multiple polypoid lesions in the large intestine. The patient then had endoscopic care. Diagnoses: Only 1 polypoid lesion on white-light endoscopy in the sigmoid colon was pathologically diagnosed as intestinal T-cell lymphomas, not otherwise specified (ITCL-NOS). Interventions: The patient underwent intensity-reduced CHOP therapy. Outcomes: The patient is still with controlled disease but developed chemotherapy-related side effects. Lessons: In the individual with unexplained anemia and waste, endoscopy should not be delayed. For each of polypoid lesion on white-light endoscopy, the endoscopist need to remain cautious, because every lesion in the same patient can exhibit the independence of histopathological features. Meanwhile, we suggest that endoscopists should routinely observe the terminal ileum, even take biopsy samples if necessary.

Differential Diagnosis between Primary Intestinal Lymphoma and Crohn's Disease

Abstract:Background: The incidence rate of primary intestinal lymphoma is relatively low, and its clinical manifestations are non-specific, which is easy to confuse with other gastrointestinal tract diseases, especially Crohn's disease (CD). Aims: To improve the skill of differential diagnosis between primary intestinal lymphoma and CD by analyzing the clinical characteristics of these two diseases. Methods: The clinical features, endoscopic findings and histopathologic characteristics of patients with primary intestinal lymphoma and CD were reviewed retrospectively. Results: The median ages of primary intestinal lymphoma and CD were 50 and 35 years old, respectively. Perianal lesions were observed in 5 (12.2%) patients with CD, internal fistula in 4 (9.8%) patients with CD and extraintestinal manifestations in 3 (7.3%) patients with CD, while anal fistula was observed in only 1 (5.9%) patient with primary intestinal lymphoma. The most frequently involved location was ileocecum in primary intestinal lymphoma (41.7%) and small bowel with colon was affected in 48.8% of patients with CD. Protruding lesions were the most frequently endoscopic features in primary intestinal lymphoma (41.7%), while ulcerative and cobblestoning appearance were found in CD, including complications of intestinal stricture in 18 and fistulization in 4 patients with CD. Of 17 patients with primary intestinal lymphoma, 13 were B-cell and 4 T-cell lymphoma. 12 patients with CD were noncaseous granuloma. Conclusions: The differential diagnosis between primary intestinal lymphoma and CD is difficult to make clinically. Multiple submucosal biopsies under endoscopy are helpful for the differential diagnosis.

Abstract:Small bowel intussusception is a rare occurrence in adults and typically indicates an underlying gastrointestinal (GI) pathology, which may include solid masses or chronic inflammatory conditions like Crohn's disease. We present a complex case involving a patient with a history of Crohn's and GI bleeding, who was found to have a bleeding lipoma serving as a lead point for intussusception.
