Abstract:BackgroundClosure of traumatic macular hole (TMH) can be achieved spontaneously or by surgical intervention. Thus far, there exist no prospective comparative studies that have analyzed the difference between the two modalities. This study aimed to compare the anatomical and visual recovery of eyes with TMH following either an immediate vitrectomy or six-month observation.MethodsThis was a multicenter prospective comparative study. Eight centers participated in the study. Patient data from 40 eyes with a recent history of blunt ocular trauma and newly formed full-thickness TMH were recruited in this study. The participating patients selected between an early vitrectomy or a six-month observation after a doctor explained the potential benefits and risks of both strategies in an unbiased manner. Twenty-five patients underwent an immediate vitrectomy, and 15 patients received six-month observation. Patients were assessed by spectral-domain optical coherence tomography (SD-OCT) and best-corrected visual acuity (BCVA).ResultsClosure rates were 66.7% for the observational group, and 100% for the surgical group (P=0.002). There were no vision-threatening ocular complications in both groups. For the observational group, the mean closure time was 2.5±1.6 months, and 80% of the hole closure occurred within 3 months; cystic edema on the edge of the hole at baseline was significantly more frequent in the non-closed subgroup than in the closed subgroup (P=0.03). There were no significant differences in the foveal microstructure and in the final visual outcome between the spontaneously closed cases and the surgically closed cases.ConclusionsTMH had a moderately high incidence of spontaneous closure, but an immediate vitrectomy achieved an even higher closure rate. Vitrectomy was effective and safe to treat TMH, while a 3-month observation for spontaneous closure may be an alternative modality for TMH management. Cystic edema on the edge of the hole may be an unfavorable factor for the spontaneous closure of TMH.
Retinal Microstructure Based on Optical Coherence Tomography Predicts Visual Prognosis in Diabetic Macular Edema with Extended Intraretinal Cysts and Subretinal Fluid.

Abstract:Abstract Purpose We investigated the association between the retinal microstructure on optical coherence tomography (OCT) and the visual outcomes of diabetic macular edema (DME) with extended intraretinal cysts (IRCs) (maximum diameter ≥ 200 μm) and serous retinal detachment (SRD) who underwent anti-vascular endothelial growth factor intravitreal injections. Methods We retrospectively reviewed 58 eyes from 43 DME patients with extended IRCs and subretinal fluid who were treated with three consecutive intravitreal injections of ranibizumab with subsequent as-needed administrations. The OCT images were assessed at baseline and at 1, 3, 6, and 12 monthsafter treatment. The association of the baseline retinal microstructure on OCT with photoreceptor disruption at 12 months was investigated. We also evaluated the relationships between visual outcomes and baseline OCT parameters. Results The mean logarithmic minimum angle of resolution (logMAR) best-corrected visual acuity (BCVA) decreased from 0.66 ± 0.34 before treatment to 0.46 ± 0.32 after treatment (p < 0.001). ELM (R = 0.535, p < 0.001) and EZ (R = 0.612, p < 0.001) disruption at 12 months, but not CFT at 12 months (R = 0.185, p = 0.164), were associated with the logMAR BCVA at 12 months. The extent of EZ disruption (R = 0.256, p = 0.039) and the area ratio of SRD spaces (R = 0.409, p = 0.002) at baseline were positively correlated with the extent of EZ disruption at 12 months. The extent of ELM disruption (R = 0.641, p < 0.001) and the area ratio of IRCs (R = 0.287, p = 0.008) at baseline were positively correlated with the extent of ELM disruption at 12 months. The area ratio of SRD spaces (R = 0.400, p = 0.006) and the extent of ELM disruption (R = 0.393, p = 0.014) at baseline were predictive of the logMAR BCVA at 12 months. Conclusion The extent of EZ and ELM disruption was significantly associated with baseline OCT imaging characteristics. Both the baseline area ratio of SRD spaces and ELM disruption might be useful OCT biomarkers to predict the visual outcomes of DME patients with extended IRCs and SRD spaces. Abstract:AIM:To report the long-term surgical outcomes of pathologic myopic foveoschisis (MF) following vitrectomy.METHODS:We performed a retrospective case series analysis of 50 consecutive patients diagnosed with MF who experienced vision loss due to progression of foveoschisis. The 50 patients (67 eyes) were treated in our hospital with vitrectomy with internal limiting membrane (ILM) peeling from December 2004 to September 2010. Best corrected visual acuity (BCVA), refractive error, optical coherence tomography (OCT), and routine examination results were analysed. The changes of BCVA, foveal anatomical features on OCT scan, and complications were the main outcome measures.RESULTS:The mean follow-up duration was 42±17mo (range 24 to 93mo). BCVA improved significantly postoperatively (0.76±0.65 logMAR) compared with preoperative baselines (1.31±0.78 logMAR, P<0.0001), and in 53 eyes (79%) including 3 lines gain in 44 eyes (66%) at the last follow-up visit. OCT scans showed that central retinal thickness decreased from 580.0±270.0 µm preoperatively (n=67) to 179.7±84.7 µm postoperatively (n=58, P<0.0001). Total resolution of foveoschisis occurred in 41 eyes (61%). Preoperative BCVA correlated well with postoperative BCVA, whereas other factors such as age, axial length, and refractive error were not correlated. The most common complications were cataract and full-thickness macular hole formation in 14 and 9 cases, respectively.CONCLUSION:Patients with progressive vision loss due to MF who were treated with vitrectomy with ILM peeling show favourable outcomes. In most eyes, visual acuity and foveal structure remain stable during long-term observation.
[Fortuitous discovery of a voluminous mandibular cyst in a child].

Abstract:In a five and a half year-old child, a voluminous mandibular cyst was discovered by a dental check-up. The originality of this case consisted in the great volume of this asymptomatic lesion and the anterior localization in the mandible. Treatment was surgical, conserving the deciduous teeth and the underlying germs. The histopathological examination of the lesion confirmed the clinical and radiological aspects of follicular cyst. A regular clinical and radiologic surveillance was necessary until bone regeneration was complete, the risk of per and post-operative fracture being major.
Postoperative large intraretinal cavity and schisis with paravascular inner break in high myopia

Abstract:Purpose: To report distinct structural changes following surgery for myopic traction maculopathy (MTM). Study design: A single-center, retrospective case series. Methods: Patients with MTM who underwent vitrectomy and had persistent large intraretinal cavities and schisis were reviewed. The pre- and postoperative clinical findings and optical coherence tomography characteristics are described. Results: Five eyes of five patients were included in the study. Three patients had prominent schisis, and four had retinal/foveal detachment before surgery. All patients underwent fovea-sparing internal limiting membrane peeling during vitrectomy and had improved vision at 3 months after surgery. Large intraretinal cavities were noted on optical coherence tomography at the first postoperative exam in three patients and 12 to 18 months postoperation in the other two. The median height of the postoperative intraretinal cavities was 704 μm (range, 445-1287 μm). Inner retinal breaks were identified in all eyes, of which four were located in the paravascular area. The intraretinal cavity and schisis remained stable during the initial follow-up for an average of 35 months; later on, at > 5 years after the initial presentation two patients developed retinal detachment and one patient underwent vitrectomy with the retina reattached postoperatively. Conclusion: Large intraretinal cavities and schisis can be observed after MTM surgery, associated with paravascular inner breaks. The condition can progress after a long stable period; therefore, continuous follow-up is necessary.
Efficacy of Vitrectomy With Tamponade Versus No Tamponade for Myopic Traction Maculopathy: A Multicenter Study (SCHISIS Report No.1)

Abstract:Purpose: To evaluate the influence of tamponade on the visual and anatomic outcomes of pars plana vitrectomy for myopic traction maculopathy (MTM). Design: Multicenter, retrospective clinical cohort study. Methods: Consecutive eyes that underwent vitrectomy for advanced MTM with tamponade of air, sulfur hexafluoride (SF6), or perfluoropropane (C3F8) or without tamponade with a minimum follow-up of 12 months were included. Main outcome measures included postoperative visual acuity (VA) at 12 months in eyes with vs without tamponade. Results: We included a total of 193 eyes (193 patients) in this study; 136 eyes (70%) treated with tamponade were compared with 57 eyes (30%) treated without tamponade. Baseline characteristics did not differ significantly between the groups. Both groups showed significant visual improvement at 12 months (both P < .001). However, postoperative visual acuity and visual improvement at 12 months were significantly better (P = .003 and P = .028, respectively) in eyes without tamponade, although the MTM in these eyes without tamponade took longer to resolve (P = .039). Retinal thickness and the ellipsoid zone were more preserved in eyes without tamponade (P < .001 and P = .001, respectively). Complications such as macular holes did not differ between the groups. A novel imaging finding of "schisis bending (accordioning)" was identified during MTM resolution. Conclusions: Vitrectomy either with or without tamponade for MTM was effective in improving vision in this study. However, eyes without tamponade experienced even better visual improvement and preserved retinal anatomy, despite a longer schisis resolution time. Surgery without tamponade may achieve better visual outcomes.
Postoperative anatomical and functional outcomes of different stages of high myopia macular hole

Abstract:Background: Recently it was suggested that high myopia macular holes (HMMH) and macular holes accompanied by retinal detachment occur in the advanced stages of myopia traction maculopathy (MTM), while macular retinoschisis, shallow retinal detachment without holes, and lamellar macular holes occur in the early stages of MTM. Complete vitreous cortex removal associated with internal limiting membrane peeling is now widely used to treat HMMH. However, it remains uncertain at what HMMH stage patients would benefit most from surgical intervention. Our study was aimed to evaluate the postoperative anatomical changes and functional outcomes of high myopia macular holes (HMMH). Methods: Patients were retrospectively collected between March 2009 and August 2011. Before and 1st, 3rd, and 9th month after 23G pars plana vitrectomy, all patients underwent a complete ophthalmologic examination, spectral domain optical coherence tomography (SD-OCT) and MP-1. At each follow-up, best-corrected visual acuity (BCVA), photoreceptor inner and outer segments (IS/OS) defects, and retinal sensitivity (RS) were investigated. According to different preoperative macular hole morphologies, patients were divided into three groups: Group 1, macular hole with epiretinal membrane (ERM) traction and macular retinoschisis; Group 2, full-thickness macular hole (FTMH); Group 3, FTMH with subretinal fluid. Results: 43 eyes from 43 patients met the inclusion criteria. The mean age was 60 years. BCVA and RS were significantly improved after vitrectomy; the mean IS/OS defect was significantly reduced. At 9 postoperative months, 11 of 43 (25.6 %) eyes achieved IS/OS junction integrity; 9 of these 11 (81.8 %) eyes belonged to Group 1, 2 (18.2 %) belonged to Group 2. Conclusions: Pars plana vitrectomy combined with ILM peeling and gas tamponade results in limited functional outcomes in patients with HMMH. The appearance of subretinal fluid indicates a worse prognosis for surgical intervention.
Observations on vitrectomy for vitreomacular traction syndrome

Abstract:Objective To evaluate the efficacy of vitreous surgery for patients with vitreomacular traction syndrome. Methods Of 30 patients (30 eyes) who underwent vitrectomy for vitreomacular traction syndrome were retrospectively analyzed. Results Vitreomacular traction was released successfully, and a better visual acuity was obtained in 20 eyes. Mean macular thickness decreased by 267μ m postoperatively. The eyes showed statistically significant improvement in visual acuity and central macular thickness (P <0.05). Optical coherence tomography (OCT) and fluorescein fundus angiography (FFA) showed macular edema gradually alleviated after vitreous surgery. Preoperative high myopia was associated with prognosis of postoperative visual acuity (P<0.05). Conclusions Vitrectomy can relieve macular traction, and is effective for decreasing macular thickness and improvement of vision in vitreomacular traction syndrome. OCT and FFA are useful for evaluation and follow-up for vitreomacular traction syndrome.
Vitrectomy Outcomes in Eyes with High Myopic Macular Hole Without Retinal Detachment.

Abstract:Purpose: The purpose of this study is to evaluate the vitrectomy outcomes for high myopic patients with macular hole (MH) without retinal detachment and to compare these outcomes with those of control patients with idiopathic MH undergoing similar surgeries by the same surgeon.Methods: The study was designed as a matched, case-control, retrospective chart review. We performed a retrospective chart review of consecutive cases that underwent vitrectomy for MH without retinal detachment from 2000 through 2008. Twenty-four eyes of 24 patients were selected to participate in this study. They were divided into 2 groups: 12 study eyes with high myopic MH without retinal detachment and 12 control eyes with idiopathic MH. The main outcomes were preoperative, postoperative best-corrected visual acuity (BCVA), MH closure rates, and complications in both groups.Results: There was a closure rate of 100% in both the study and control groups, and no cases of reopening were reported during the follow-up visits. After surgery, the mean BCVA of the study group remained stable. There was no significant improvement of mean BCVA after surgery (P = 0.35) in the study group. The mean BCVA of the control group improved significantly (P = 0.00) after surgery. The mean postoperative BCVA in the study group was significantly lower than that in the control group in a comparison using the Student's t-test (P = 0.01). In the study group, the BCVA improved or remained stable after surgery in 8 eyes (67%). In the control group, the BCVA of all eyes improved or remained stable. The proportion of eyes with BCVA improvement after surgery was significantly higher in the study group than in the control group (P = 0.03). Disappearance of the outer nuclear layer and photoreceptor layer in the foveola and thinning of the foveola was showed in the postoperative optical coherence tomography images of four eyes in the study group while in none of the eyes in the control group.Conclusion: In summary, vitrectomy results in satisfactory anatomical improvement in patients with high myopic MH without retinal detachment. However, the visual acuity outcomes of the high myopic MH patients without retinal detachment were poorer than those for patients with idiopathic MHs. RETINA 32:275-280, 2012

Abstract:Purpose: To evaluate the incidence, pathogenesis, risk factors, and treatment outcomes of postoperative macular hole (MH) after pars plana vitrectomy (PPV) for myopic traction maculopathy (MTM). Design: Multicenter, interventional, retrospective case series. Subjects: Consecutive eyes that underwent PPV for MTM with a minimum 6-month follow-up. Methods: We investigated the characteristics and treatment outcomes of postoperative MH after MTM surgery. Main outcome measures: Incidence, risk factors, and anatomic and visual outcomes of postoperative MH. Results: We included 207 eyes (207 patients) with a mean follow-up of 25.9 months. During follow-up, 24 (11.6%) eyes developed MH (10 with concurrent MH retinal detachment); 15 eyes within 30 days (early), 4 eyes between 31 and 180 days (intermediate), and 5 eyes after 180 days (late). Logistic regression analysis revealed male gender (odds ratio [OR], 2.917; 95% confidence interval [CI], 1.198-7.100; P = 0.018), thinner preoperative choroidal thickness (OR, 0.988; 95% CI, 0.976-1.000; P = 0.048), and use of indocyanine green for internal limiting membrane peeling (OR, 2.960; 95% CI, 1.172-7.476; P = 0.022) as significant risk factors for postoperative MH. Internal limiting membrane peeling with a fovea-sparing technique tended to protect against postoperative MH, but it was not statistically significant (P = 0.096), because 1 eye still developed MH. Postoperative MHs were treated by observation (6 eyes), in-office octafluoropropane (C3F8) gas injection (7 eyes), or PPV (11 eyes). Macular hole closure was achieved in 20 eyes (83%). The hole closure rate was 67% (4/6 eyes) after observation, 71% (5/7 eyes) after C3F8 gas injection, and 91% (10/11 eyes) after PPV. However, visual outcomes were significantly worse for eyes with postoperative MH than those without (0.38 ± 0.43 vs. 0.68 ± 0.46; P = 0.002). Conclusions: Postoperative MH may occur in 11.6% of patients with MTM at any time after surgery. Retreatment resulted in relatively favorable anatomic closure but unfavorable visual outcomes. [Therapeutic Effects of Pars Plana Vitrectomy Combined with Internal Limiting Membrane Peeling on High Myopic Foveoschisis].

Abstract:Objective: To observe the clinical efficacy of vitrectomy combined with foveal-sparing circular internal limiting membrane peeling with the central fovea reserved in patients with high myopic foveoschisis. Methods: Case-control study. A total of 72 patients (86 eyes) with foveoschisis due to high myopia who underwent vitrectomy combined with internal limiting membrane peeling in Lishui Municipal Central Hospital of Zhejiang Province from June 2016 to May 2017 were enrolled, including 18 males (18 eyes) and 54 females (68 eyes), aged (50.5±8.9) years. The patients were randomly divided into the observation group (34 cases, 43 eyes) and the control group (38 cases, 43 eyes) using a random number table and random number residual grouping method. Vitrectomy was performed in all patients. In the control group, the limiting membrane in the macular area was removed during surgery. In the observation group, the foveal annular internal limiting membrane was preserved during surgery. In severe cases, cataract extraction was performed. Results: In the observation group, the mean central foveal thickness (CFT) on optical coherence tomography was (723.49±130.95) μm preoperatively and decreased to (286.33±210.73) μm postoperatively, and the difference had statistical significance (t=17.059, P=0.000). In 39 eyes with complete or partial healing of the foveal split, the best corrected visual acuity (BCVA) was (0.99±0.40) logMAR before the treatment and increased to 0.68±0.24 after the treatment, and the difference had statistical significance (t=7.585, P=0.000). In the control group, the mean CFT was (726.98±140.62) μm and (297.88±241.56.) μm before and after the treatment, respectively, and the difference had statistical significance (t=16.271, P=0.000). In 38 eyes with complete or partial healing of the foveal split, the BCVA increased from preoperative 1.04±0.47 to postoperative 0.69±0.21, and the difference had statistical significance (t=6.707, P=0.00). With the follow-up time as the efficacy determination time, there was no significant difference in BCVA (t=0.22, P=0.983) and CFT (t=0.236, P=0.814) between the observation group and the control group (P>0.05); there was significant difference in the incidence rate of macular holes (P=0.026). There was no statistically significant difference in the rate of complete macular morphology healing between the two groups (P=0.816). Conclusion: Vitrectomy combined with foveal-sparing circular internal limiting membrane peeling can effectively treat high myopic macular holes and reduce the formation of postoperative macular holes. Natural Course of Myopic Traction Maculopathy and Factors Associated With Progression or Resolution

Abstract:PURPOSE: To investigate the natural course of myopic traction maculopathy and to identify the factors associated with its progression or resolution in 207 highly myopic eyes.DESIGN: Retrospective, consecutive case series.METHODS: We investigated 207 highly myopic eyes with myopic traction maculopathy by optical coherence tomography (OCT) and followed the patients for at least 24 months. The size of the macular retinoschisis was categorized according to its extent and location relative to the fovea. The clinical data, status of the myopic traction maculopathy, and changes in the posterior vitreous and inner limiting membrane (ILM) were analyzed during the natural course of the disease process.RESULTS: During a mean follow-up of 36.2 ± 6.2 months, 8 of 207 eyes (3.9%) showed a decrease or complete resolution of the macular retinoschisis. Of these 8 eyes, 6 had release of retinal traction in the OCT images, and 4 of the 6 eyes developed posterior vitreous detachment. A spontaneous disruption of the ILM occurred in 2 eyes before the improvement of the myopic traction maculopathy. Of the 207 eyes, 24 (11.6%) experienced progression of the myopic traction maculopathy. The eyes with more extensive macular retinoschisis showed progression significantly more commonly (42.9%) than the eyes having less extensive macular retinoschisis areas (6.7%).CONCLUSIONS: The natural course of myopic traction maculopathy varies, and eyes with more extensive macular retinoschisis tend to progress more commonly. A complete resolution of macular retinoschisis can occur after a release of traction on the retina. These findings can be used to determine whether surgery should be performed.

Abstract:Abstract Background To describe the natural course of myopic traction maculopathy (MTM) and determine predictive factors for its progression and visual prognosis. Methods This retrospective observational study included 113 MTM patients (113 eyes). Best-corrected visual acuity (BCVA) measurements and optical coherence tomography findings were recorded. Results Over a mean follow-up of 38.2 ± 11.1 months, 49 of 113 eyes (43.4 %) progressed. The progression rate of outer schisis prominently located in the fovea or posterior staphyloma was significantly higher than that of outer schisis prominently located in paravascular areas ( P = 0.0011). MTM with partial posterior vitreous detachment during the follow-up progressed more rapidly than MTM without ( P = 0.0447). Patients with older age (> 65 years), without domed-shaped macula and with defects in the ellipsoid zone (EZ) had worse BCVA at the last visit ( P = 0.0416, P = 0.0494and P = 0.0130). Multiple linear regression analysis showed that BCVA and defects in the EZ at baseline were significantly associated with the final BCVA ( P < 0.0001 and P = < 0.0001, respectively). Conclusions MTM has a high possibility for progression. Outer schisis located predominantly in the fovea or posterior staphyloma or with partial posterior vitreous detachment exhibits rapid progression. The integrity of the EZ is related to visual prognosis.

Abstract:OBJECTIVE:To evaluate the conversion of macular function in diabetic macular edema (DME) after vitreous surgery.METHODS:Seventeen eyes of 16 patients were assessed after pars plana vitrectomy for DME. Multifocal electroretinogram (mfERG) techniques were used to evaluate the effects of treatment on macular visual function, and retinal foveal thickness was determined by optical coherence tomography before and after vitrectomy. Patients were followed up continually for 7 months. The correlation between the change of tomographic features and visual functions were analyzed.RESULTS:During the follow-up period, the retinal function in macular areas recovered and the foveal thickness decreased gradually. While compared with preoperative values, the response of positive wave (P1) have shown decreases in implicit time and increases in amplitude since the forth and fifth postoperative month respectively (P < 0.05). While compared between each postoperative period, the P1 responses have increased significantly since the forth postoperative month. The correlation between the increase of P1 amplitude in macular area and the decrease of retinal foveal thickness was significant (r = 0.954, P < 0.001).CONCLUSIONS:Macular function get improvement gradually in DME after vitreous surgery; The restoration of macular function is coincident with the reduction of macular edema.
Optical coherence tomographic characteristics and surgical outcome of eyes with myopic foveoschisis

Abstract:Aim To describe the optical coherence topographic (OCT) characteristics and outcome of vitrectomy with triamcinolone acetonide (TA)-assisted epiretinal tissue removal and gas tamponade in eyes with myopic foveoschisis (MF). Methods We examined 101 myopic eyes, and the OCT characteristics of 18 eyes (18%) with an MF were studied. Six eyes underwent vitrectomy with TA-assisted epiretinal tissue removal and gas tamponade. The postoperative outcome measures were best-corrected visual acuity (BCVA), resolution of the MF as determined by OCT, and complications. Results OCT scans showed an isolated MF in three (17%) eyes, and the MF were associated with two or more macular anomalies in 15 (83%) eyes. A foveal detachment was found in 13 (72%), lamellar macular hole in 4 (22%), and a tractional epimacular membrane in 11 (61%) of the 18 eyes. Six eyes underwent surgery, in which the epiretinal tissue was made visible by intravitreal TA and completely removed. The median BCVA significantly improved from 20/400 preoperatively (range, 20/1000–20/160) to 20/160 postoperatively (range, 20/200–20/40; P =0.03, Wilcoxon signed-rank test). The mean VA improved by 5.6 lines. OCT showed a complete reattachment of the fovea in four (67%) eyes and a small subfoveal or parafoveal detachment in two (33%) eyes. Conclusions MF is commonly observed in highly myopic eyes and most are associated with one or more macular anomalies. Vitrectomy with TA-assisted complete removal of all tractional premacular structures followed by gas tamponade resulted in good visual and anatomical outcomes for treating MF.

Abstract:PURPOSE: To study the long-term natural course of myopic retinoschisis (MRS) with a dome-shaped macula (DSM) and to identify the factors affecting its development and visual prognosis.METHODS: In this retrospective case series study, we followed 25 MRS eyes with a DSM and 68 MRS eyes without a DSM for at least two years and observed changes in optical coherence tomography morphological features and best-corrected visual acuity (BCVA).RESULTS: During the mean follow-up of 48.3±13.24 months, the difference in the rate of MRS progression between the DSM and non-DSM groups was not significant (P = 0.7462). In the DSM group, the patients whose MRS progressed were older and had a higher refractive error than those whose MRS was stable or improved (P = 0.0301 and 0.0166, respectively). The patients whose DSM was located in the central fovea had a significantly higher progression rate than those whose DSM was located in the parafovea (P = 0.0421). For all DSM eyes, BCVA did not decrease significantly in eyes with extrafoveal retinoschisis (P = 0.2500), patients whose BCVA decreased more than 2 lines had a greater central foveal thickness initially than those whose BCVA decreased less than 2 lines during the follow-up period (P = 0.0478).CONCLUSION: A DSM did not delay the progression of MRS. The development of MRS in DSM eyes was associated with age, myopic degree and DSM location. A higher schisis cavity predicted visual deterioration, and a DSM protected visual function in extrafoveal MRS eyes during the follow-up period.

Abstract:To evaluate the long-term effect of cataract surgery on highly myopic patients with myopic traction maculopathy (MTM) and the risk factors associated with MTM progression. Highly myopic patients with cataract and MTM were included. Phacoemulsification surgery was performed on patients who had vision loss below 20/63 and were willing to operation. Exclusion criteria included full thickness macular hole, foveal/retinal detachment, history of vitreoretinal surgery, myopic choroidal neovascularization, macular chorioretinal atrophy, peripheral lattice degeneration, incomplete follow up, or intraoperative complications. All patients underwent a complete ophthalmological examination. Optical coherence tomography examinations and microperimetry examinations were performed. A total of 229 patients (mean age: 57 ± 6 years) were recruited, including 179 operated patients and 50 unoperated patients. Both the best corrected visual acuity (BCVA) and macular sensitivity (MS) were significantly improved after cataract surgeries throughout the follow-up period (p = 0.000). No difference was found in the proportion of MTM staging and in the rate of resolving/stable or progressive MTM (p = 0.757) between the operated and the unoperated groups. Of all patients, those with S2 to S4 MTM at baseline had significantly higher risk of progressive MTM (p < 0.001). Patients with absence of posterior vitreous detachment or with longer axial length at baseline had higher risks of progressive MTM. Cataract surgery generally improves the BCVA and MS of highly myopic patients with MTM. Preoperative vitreoretinal adhesion, longer axial length, and S2 to S4 MTM are risk factors for progressive MTM. A long-term follow-up on the development of MTM is recommended.

Abstract:Objective . To observe the fellow eye in patients undergoing surgery on one eye for treating myopic traction maculopathy. Methods . 99 fellow eyes of consecutive patients who underwent unilateral surgery to treat MTM were retrospectively evaluated. All patients underwent thorough ophthalmologic examinations, including age, gender, duration of follow-up, refraction, axial length, intraocular pressure, lens status, presence/absence of a staphyloma, and best-corrected visual acuity (BCVA). Fundus photographs and SD-OCT images were obtained. When feasible, MP-1 microperimetry was performed to evaluate macular sensitivity and fixation stability. Results . At an average follow-up time of 24.7 months, 7% fellow eyes exhibited partial or complete MTM resolution, 68% stabilized, and 25% exhibited progression of MTM. Of the 38 eyes with “normal” macular structure on initial examination, 11% exhibited disease progression. The difference in progression rates in Groups 2, 3, and 4 was statistically significant. Refraction, axial length, the frequency of a posterior staphyloma, chorioretinal atrophy, initial BCVA, final BCVA, and retinal sensitivity all differed significantly among Groups 1–4. Conclusions . Long axial length, chorioretinal atrophy, a posterior staphyloma, and anterior traction contribute to MTM development. Patients with high myopia and unilateral MTM require regular OCT monitoring of the fellow eye to assess progression to myopic pre-MTM. For cases exhibiting one or more potential risk factors, early surgical intervention may maximize the visual outcomes.

Abstract:Objective To observe the changes of retinal microstructure in lamellar macular hole (LMH)after vitrectomy.Methods A retrospective clinical observational study.Forty patients (41 eyes) with LMH and received vitrectomy in Ophthalmology Department of Peking University People's Hospital from January 2014 to September 2018 were included in this study.Among them,14 patients (15 eyes) were males and 26 patients (26 eyes) were females,with an average age of 67.8±8.6 years.There were 37 eyes with a lens and 4 eyes with an IOL.There were 29 eyes with LMH of tractional type,7 eyes of degenerative type,and 5 eyes of mixed type.All patients underwent BCVA and OCT examinations.The BCVA examination was performed using the international standard visual acuity chart,which was converted into logMAR visual acuity.The average logMAR BCVA was 0.57±0.27;the mean macular retinal thickness (CRT) was 192.3 ± 108.9 μtm,the mean macular thickness (MRT) was 427.5± 110.2 μm.Among the 29 eyes of tractional type,there were 17 eyes with retinal cavity,8 eyes with macular retinoschisis,and 3 eyes with incomplete ellipsoid zone.Among the 7 eyes of degenerative type,there were 5 eyes with lamellar hole-associated epiretinal proliferation (LHEP),5 eyes with retinal cavity,and 5 eyes with incomplete ellipsoid zone.Among the 5 eyes of mixed type,2 eyes with LHEP,1 eye with macular epiretinal membrane,and 4 eyes with incomplete ellipsoid zone.The average follow-up time after surgery was 12.8±5.2 months.Among them,10 eyes were followed up for equal or greater than 24 months.After the surgery,the same equipment and method before the surgery were used for relevant examination.The changes of BCVA,CRT,and MRT before and after surgery were observed.Continuous variables were compared by t test.Results At the last follow-up,the mean logMAR BCVA was 0.37± 0.26.Compared with before surgery,the difference was statistically significant (t=5.98,P＜0.01).The mean CRT and MRT were (245.2 ± 90.8) and (347.0 ±46.7) μtm,respectively.Compared with before surgery,the differences were statistically significant (t=-2.49,-5.24;P＜ 0.05,＜ 0.01).CRT and MRT changed greatly within 6 months after surgery,and then tended to be gentle.Among the 3 eyes with incomplete ellipsoid zone of tractional type before surgery,ellipsoid zone recovered in 2 eyes and partially recovered in 1 eye.Among the 17 eyes with retinal cavity and 8 eyes with macular retinoschisis before surgery,there were still 4 eyes with retinal cavity,but all the retinoschisis were disappeared.Among the 5 eyes with retinal cavity of degenerative type before surgery,there were still 2 eyes with retinal cavity and all the eyes with incomplete ellipsoid zone.Among 10 eyes with a follow-up time of equal or greater than 24 months,the macular ganglion cell complex partially atrophied in 6 eyes,and the nerve fiber layer separated in 2 eyes.There was no full-thickness macular hole after surgery.Conclusion For most LMH patients,vitrectomy can effectively improve the visual acuity and promote the recovery of retinal microstructure.
Long-term Incidence and Risk Factors of Macular Fibrosis, Macular Atrophy, and Macular Hole in Eyes with Myopic Neovascularization

Abstract:PURPOSE: To identify the risk factors associated with myopic macular neovascularization (mMNV)-related complications in patients treated with intravitreal anti-VEGF agents.DESIGN: Longitudinal cohort study.PARTICIPANTS: Myopic eyes (n = 313) with active mMNV and median (interquartile range) follow-up of 42 months (interquartile range, 18-68 months) after initiation of anti-VEGF treatment.METHODS: Data regarding patients' clinical and mMNV-related characteristics were collected at baseline. Subsequent OCT scans were inspected for mMNV-related complications. Best-measured visual acuity (BMVA) values were retrieved from each visit.MAIN OUTCOME MEASURES: Incidence rate and hazard ratio (HR, with 95% confidence interval [CI]) of risk factors for fibrosis and macular atrophy calculated with Kaplan-Meier curves and Cox regression models. Crude incidence of macular hole (MH). Longitudinal BMVA changes.RESULTS: Five-year incidence of fibrosis, atrophy, and MH were 34%, 26%, and 8%, respectively. The rate of fibrosis was 10.3 (95% CI, 8.25-12.6) per 100 person-years. Risk factors were subfoveal mMNV location (HR [95% CI] = 12.7 [2.70-56.7] vs. extrafoveal, P = 0.001) and intraretinal fluid at baseline (HR [95% CI] = 1.75 [1.05-2.98], P = 0.03). The rate of macular atrophy was 6.5 (95% CI, 5-8.3) per 100 person-years. Risk factors were diffuse (HR, 2.20 vs. tessellated fundus; 95% CI, 1.13-5.45; P = 0.02) or patchy chorioretinal atrophy (HR, 3.17 vs. tessellated fundus; 95% CI, 1.32-7.64; P = 0.01) at baseline and more numerous anti-VEGF injections before baseline (HR, 1.21; 95% CI, 1.06-1.38 for each treatment; P = 0.005). Eyes with fibrosis and macular atrophy had faster BMVA decay over follow-up. Twenty eyes (6%) developed MH. Two subtypes of MH were identified: "atrophic" and "tractional."CONCLUSIONS: Myopic MNV-related complications are common in the long term despite initially successful treatment and have detrimental effects on visual acuity. Insights into their incidence and risk factors may help for future treatments to mitigate sight-threatening outcomes.
