Abstract:ObjectivesExtracorporeal membrane oxygenation (ECMO) has become an important life support technique during lung transplantation. We aimed to develop a rat model for lung transplantation using venoarterial (VA) ECMO support.MethodsAdult male Sprague-Dawley rats weighing 400 to 450 g were used in this study. ECMO circuits were created by obtaining venous access from the femoral vein with subsequent extracorporeal oxygen exchange, which was then returned to the circulatory system through the left carotid artery (ie, VA-ECMO). Simultaneously, the donor lungs were retrieved and immersed in cold, low-potassium dextran lung preservation solution. Orthotopic left lung transplantation supported by VA-ECMO was performed. Thereafter, a respiratory failure rat model was constructed using ventilation with a hypoxic and hypercapnic gas mixture, consisting of 6% oxygen, 8% carbon dioxide, and 86% nitrogen, before lung transplantation. Similarly, left lung transplantation supported by VA-ECMO was performed in rats with respiratory failure. Arterial blood gas levels were measured at designated time points throughout the experiment.ResultsWe found that VA-ECMO provided sufficient oxygenation and carbon dioxide removal to allow for smooth left lung transplantation in healthy rats and those with respiratory failure.ConclusionsWe established a rat model for lung transplantation using VA-ECMO. Left lung transplantation using VA-ECMO support is also feasible and safe in rat models of respiratory failure. These models provide efficient and economical models for translational medicine for lung transplantation using ECMO. Moreover, it will be invaluable to evaluate the physiological and pathophysiological roles of ECMO during lung transplantation.
Experimental Study of Ex Vivo Liposome-mediated Gene Transfer in Orthotopic Rat Left Lung Transplantation

Abstract:Objective To determine the feasibility of ex vivo liposome-mediated gene transfer ( Lac-Z gene ) to lung isografts and to get expression. Methods An orthotopic rat left lung transplant model was developed with the use of a modification of the “cuff” technique. 18 SD rats were divided into three groups randomly. Gene transfer group ( group 1 ): PSV-β-gal/ liposome ( Lipofectin R○ ) complex was transfered into the donor left lung via the left pulmonary vein route at the time of lung harvesting. Liposome group ( group 2 ) and Control group ( group 3 ) : Grafts underwent the same procedure but received liposome and 0 9% saline solution respectively. Donor lungs were harvested one day after transplantation. The transgene expression of Lac-Z gene was detected by histochemical staining. Results Transgene expression of β-gal in group 1 was detected in bronchial epithelial cells and alveolar epithelial cells. But no expression of β-gal was found in groups 2 and 3. Conclusion Ex vivo plasmid vector/liposome complex-mediated gene transfer to lung isografts via the left pulmonary vein route is feasible. This study provides the scientific proof to the feasibility of functional gene therapy of complications such as ischemia-reperfusion injury, rejection and infection after rat lung transplantation.
A Novel and Knotless Technique for Heterotopic Cardiac Transplantation in Mice

Abstract:Background: The development of microsurgical. techniques has facilitated the establishment of fully vascularized cardiac transplantation models in small mammals. Anastomotic stenosis and bleeding continue to hamper procedures and limit long-term graft survival. In this Study we assess a novel technique to improve Outcome after cardiac transplantation in mice.Methods: Our novel technique of murine heterotopic cardiac transplantation consists of three critical steps: (i) a novel procedure for graft harvest; (ii) a modified method for recipient vessel preparation; and (iii) a novel Suturing procedure for graft implantation. Importantly, a new knotless suturing technique for end-to-side vascular anastomosis was applied, which allows for adjustment of the anastomosis after transplantation, thus reducing the risk of anastomotic bleeding or stenosis.Results: The recipient survival rate based on this novel technique was between 90% and 98%, depending on physician expertise. Graft implantation time varied between 20 and 25 minutes after the initial 200 training cases. In comparing the standard knot microvascular Suturing technique to the new knotless technique carried out by an experienced surgeon, the latter was found to be more efficient by significantly reducing the rate of anastomotic stenosis (0% vs 8% with knot, p < 0.001, n = 200) and anastomotic bleeding (2% vs 7% with knot, p < 0.05, n = 200).Conclusions: This novel technique offers a rapid, easy and effective method for murine heterotopic cardiac transplantation. A Modified Orthotopic Left Lung Transplantation Model in Rats

Abstract:To enhance the operability of the rat orthotopic left lung transplantation model, we implemented several improvements and meticulously detailed the procedure. One hundred and thirty-one healthy male Sprague Dawley rats, weighing between 250 and 300 g, were utilized, with 64 serving as donors, 64 as recipients, and 3 as sham controls. We employed a modified three-cuff technique for the orthotopic left lung transplantation. Notably, our modified perfusion method could prevent donor lung edema, while waist-shaped cuffs minimized suture slippage during anastomosis. Additionally, positioning the recipient rat in a slightly left-elevated supine position during anastomosis reduced tension on the lung hilum, thus mitigating the risk of vascular laceration. The introduction of a unique two-person anastomosis technique significantly reduced operation time and substantially improved success rates. Furthermore, maximizing inflation of donor lungs both during preservation and surgery minimized the occurrence of postoperative atelectasis. Various other procedural refinements contributed to the enhanced operability of our model. Sixty-four rat orthotopic left lung transplantations were performed with only one surgical failure observed. The acquisition time for donor lungs averaged (19 ± 4) minutes, while (11 ± 1) minutes were allocated for donor lung hilum anatomy and cuff installation. Recipient thoracotomy and left lung hilar anatomy before anastomosis required (24 ± 8) minutes, with anastomosis itself taking (31 ± 6) minutes. Remarkably, the survival rate at the 4-h postoperative mark stood at 96.7 %. Even six months post-operation, transplanted left rat lungs continued to exhibit proper inflation and contraction rhythms, displaying signs of chronic pathological changes. In summary, our modified rat model of orthotopic left lung transplantation demonstrates robust operability, significantly reducing surgical duration, improving operation success rates, and enhancing postoperative survival rates. Furthermore, its long-term survival capacity enables the simulation of acute and chronic disease processes following lung transplantation.

Abstract:Objectives: The murine cervical heterotopic heart transplant model was initially designed to test the immune response to third-party allografts, modified by cuff techniques. While cuff techniques simplify the execution of this procedure, cutting of the carotid artery and the external jugular vein alters the blood supply to central nervous system and makes it difficult to achieve long-term graft survival. In the present study, we describe modified techniques that preserve the continuity and function of blood vessels and improve transplant integrity.Materials and Methods: The modified techniques in this study comprise the following aspects: (1) Preservation of the sternal head of the right sternocleidomastoid muscle, (2) use of the donor's intrathoracic inferior vena cave for anastomosis and (3) preservation of the function of the recipient's carotid artery and external jugular vein and thus, continuity of blood flow to the central nervous system.Results: Stable, long-term, disease-free allograft survival has been achieved with syngeneic transplants (> 200 days), whereas allografts from fully major histocompatibility complex-mismatched donors were acutely rejected in a time similar to the traditional abdominal heterotopic heart transplant model (8.2 +/- 1.3 vs 8.4 +/- 1.4 days; P = .73 in the Mantel Cox test, and P = .61 in the Gehan-Breslow-Wilcoxon test). Similar alloresponses could be induced in these 2 models.Conclusions: It is possible and feasible to achieve long-term graft survival in the mouse cervical heart transplant model using the modified procedures described in the present study.
The Improvement of Orthotopic Left Pulmonary Allograft Model in Rats

Abstract:Objective To establish rat orthotopic left lung allograft transplantation model by the improved "cuff-like" vessel and bronchial anastomosis technique. Methods Thirty-six male Sprague-Dawley rats were randomly allocated to donors and recipients. The donor right hilus was clipped for 5 mins(clipping test), then the blood samples were collected from carotid artery for arterial blood gas analysis as baseline. The graft's oxygenation function was also evaluated after 2-hour reperfusion. Dynamic compliances(Cdyn), resistances of airway(Raw) and peak inspiratory pressures(PIP) were monitored before the operation and after 2-hour reperfusion. Results After 2-hour reperfusion, Cdyn, Raw and PIP were worse than related baselines, but there weren't significant differences. The PaO_ 2 , OI, Qs/Qt, and A-aO_ 2 were significantly worse than related baseline values(P<0.01). This improved model makes the operation easier than ever. Conclusion This simpler model may be used to study lung transplantation.

Abstract:Objective To develop a model of rat heterotopic heart transplantation by using improved techniques. Methods The authors adopted the model of rats heart heterotopic transplantation established by Ono who anastomosed the donor ascending aorta and pulmonary artery to the recipiet abdominal aorta and inferior vena cava respectively, and some improvements were made in this experiment. The time of procedure and the ischemic time of the donor heart were shortened. Results The successful rate of 50 formal transplantations was 88%. Conclusion Heterotopic heart transplantation model in rats is a new, simple and useful technique. The main reason of surgical failure is bleeding from the anastomosis. To lessen the intraoperative bleeding is a key factor to make the procedure successfully.
A Model of Orthotopic Right Lung Transplantation in Rats

Abstract:Objective To report the preliminary experience with the techniques of orthotopic right lung transplantation in rats, and to evaluate an application of the model in the study on ischemia-reperfusion injury after experimental lung transplantation. Methods Pure breed SD rats (weighing 250～270 g) were chosen. Under an operation microscope (10) for both donor and recipient procedures, 12 consecutive orthotopic right lung transplantations in rats were performed. Under the general anes-thesia and mechanical ventilation, middle line laparothoracotomy was used to expose the heart and bi-lateral lungs. Heparin was given through the inferior vena cava. After opening the right ventricular outflow tract and exsanguinating through the left auricle, a perfusion catheter was placed in the pul-monary artery. Following lung perfusion, the donor lung was retrieved, and properly preserved. The techniques of general anesthesia and mechanical ventilation for the recipient were the same as the do-nor. Right posterior thoracotomy through the fourth intercostal space was performed. Pulmonary ves-sels and bronchus were isolated, clamped and excised as far from the clamps as possible. Donor lung was implanted, all bronchial and vessel end-to-end anastomoses were hand-sewn with 9-0 single strand running suture. The recipients" intraoperative and postoperative conditions were recorded. Results The mean cold ischemic time of the graft was (131.25 ± 20.24) min; mean time of lung transplanta-tion was (91.25 ± 15.97) min. A total of 8 recipients survived for the entire 24 h observation period. Success rate of right lung transplantation was 66.6%. Furthermore, 6 recipients survived for more than 4 weeks. Conclusion Rat right lung transplant model is a new option for ischemia-reperfusion in-jury study after lung transplantation.

Abstract:Objective:To build up and refine rat allograft orthotopic left lung transplantation model. Methods:One hundred male Sprague-Dawley rats,weight ranging from 200 to 250 grams,were selected and randomized into 50 pairs for donors and recipients. After general anesthesia with intubation,donor left lungs were harvested individually after perfusion with LPD solution through left main pulmonary artery,then were immersed into prepared fresh LPD solution. Muscle-sparing incision was adopted in recipient lung operations. Blunt dissection was carried out during dissect hilum of left lung. Cuff techniques were introduced in anastomosis of bronchus,pulmonary artery,and pulmonary vein. Results:All cuffs of the donor lungs were accomplished successfully. In all the 50 transplantations,we yielded a 90 percent success rate. Time for harvesting and cuff techniques was (25±2) min,and anastomosis time was (30±4) min. The reasons of 5 failed cases were as follows:disrupture of pulmonary vein in 3,disrupture of pulmonary artery in 1,and fatal bleeding in 1. Conclusion:Rat lung transplantation model was successfully built up and refined on some techniques. The animal model could be duplicated easily and the success rate was high.
Establishment of a rat lung transplantation model with non-heart beating donor

Abstract:BACKGROUND: A large number of experiments have shown that the donor lung tissue and pulmonary vascular function can be stored under appropriate conditions. When the donor heart beating stops, the pulmonary gas exchange system can be tolerant to the warm ischemia for 60 minutes. OBJECTIVE: To establish a simple and effective rat lung transplantation model with non-heart beating donor. METHODS: Sixty Sprague-Dawley rats were randomly divided into heart beating donor group, non-heart beating donor-ischemia 30 minutes group and non-heart beating donor-ischemia 60 minutes group, there were 20 rats in each group. The lungs of the donor rats in the heart beating donor group were obtained and stored in 4 ℃ low potassium dextran solution for 4 hours; rats in the non-heart beating donor-ischemia 30 minutes group were bloodletting sacrificed, and the lungs of the rats were maintained ventilation for 30 minutes, and then performed with cold preservation after hot ischemia for 30 minutes; the treatment method in the non-heart beating donor-ischemia 60 minutes group was similar to that in the heart beating donor-ischemia 30 minutes group, the difference was that the time of hot ischemia. The peak airway pressure and pulmonary venous blood oxygen partial pressure was measured at 1, 15, 30, 45 and 60 minutes after the connection of the circulation loop in each group. After perfusion, the wet weight and dry weight ratio and the malondialdehyde level were detected. RESULTS AND CONCLUSION: There was no significant difference of all the indexes between heart beating donor group and non-heart beating donor-ischemia 30 minutes group (P > 0.05). The peak airway pressure in each group was increased gradually along with the extension of time, but the peak airway pressure in non-heart beating donor-ischemia 60 minutes group was higher than that in the heart beating donor group and non-heart beating donor-ischemia 30 minutes group (P < 0.05). The pulmonary blood oxygen partial pressure in each group was gradually decreased with the extension of time, and the pulmonary blood oxygen partial pressure non-heart beating donor-ischemia 60 minutes groupwas lower than that in heart beating donor group and non-heart beating donor-ischemia 30 minutes group (P < 0.05). The wet/dry weight ratio and the malondialdehyde level in non-heart beating donor-ischemia 60 minutes group were higher than those in heart beating donor group and non-heart beating donor-ischemia 30 minutes group (P < 0.05). Non-heart beating donor-ischemia 30 minutes is a safe and effective method for lung transplantation.

Abstract:Over the past decade, our laboratory has made significant progress in developing and refining vascularized mouse lung transplantation models using an efficient and highly reliable "cuff technique" of transplantation. This article describes a sophisticated and comprehensive method for orthotopic lung transplantation in a vascularized orthotopic lung model, representing the most physiologic and clinically relevant model of mouse lung transplantation to date. The transplantation process consists of two distinct stages: donor harvest and subsequent implantation into the recipient. The method has been successfully mastered, and with several months of sufficient training, a skilled practitioner can perform the procedure in approximately 90 min from skin-to-skin. Surprisingly, once individuals overcome the initial learning curve, the survival rate during the perioperative period approaches nearly 100%. The mouse model allows for the use of multiple commercially available transgenic and mutant strains of mice, enabling the study of tolerance and rejection. Additionally, the unique features of this model make it a valuable tool for investigating tumor biology and immunology.

Comparation of the Effects of Ulinastatin and N-desulfated Heparin on Ischemia-reperfusion Injury after Lung Transplantation in Rats

Abstract:Objective: To build an orthotopic single left lung allograft animal model in rats and compare the effects of ulinastatin (UTI) and non-anticoagulant N-desulfated heparin (NNH) on lung ischemia-reperfusion injury after lung transplantation. Methods: 24 Male SD rats weighted 250-350g were randomly divided into 3 groups. At the time of reperfusion, the control group were given saline 1. 2ml/kg (n = 8); the UTI group were given UTI 10,000 U/kg (n=8); the NNH group were given NNH 12mg/kg , we completed 24 rat left lung transplantation assisted by mechanical ventilation and microscopy. 2 hours after reperfusion, the transplanted lung's pulmonary vein blood gases, wet dry ratio(W/D) and myeloperoxidase(MPO) were measured. Tumor necrosis factor-α(TNF-α) and interleukin 8(IL-8) were measured by method of ELISA. Intercilluar adhesion molecule-1( IC AM-1) mRNA in lung tissue were measured by method of RT-PCR, pathology slice were done routinely. Results: Among 24 transplanted rats, 21 survived at the end point, the transplantation success rate was 87. 5% and the procedure success rate was 100%. Among the 3 failure cases, 2 died in 75 minutes and 90 minutes after reperfusion respectively. Another one was still alive, however, thrombus was found in the pulmonary artery and vein at autopsy, thus the transplanted lung was not perfused. PaO2 in the UTI group(289. 68±5. 65mmHg) and the NNH group(315. 26 ± 14. 91mmHg) was significantly higher (P0.0001) than the control group (236. 80 ±13. 90mmHg ), and the NNH group was singnificantly higher than the UTI group(P 0.0001). W/D ratio has no difference(P=0.18) within the UTI group(4.74 ±0. 28) ,the NNH group (4. 87 ±0. 34) and the control group (5. 00 ± 0. 12), MPO activity in the UTI group(0. 52 ± 0. 25) was lower than the control group(0. 84±0. 22, P0.05), and the difference within the NNH group(0. 64 ± 0. 24), the control group and the UTI group was not apparent(P=0. 12, P=0. 29). In the lung tissue, the concentrations of TNF-a in the UTI group (138. 61 ± 15. 30) were significantly higher than the control group(77. 54 ± 10.14, P0. 0001), the NNH group(52. 85 ± 6. 55) was significantly lower than the control group(P0.0001). In the lung tissue, the concentrations of IL-8 in UTI group(173. 45± 27. 63) were significantly higher than the control group(76. 22 ?12. 42, P0.0001) and the NNH group(61. 34 ± 9. 86,P0. 0001), and there was no difference between the control group and the NNH group(P=0. 12). ICAM-1 mRNA in the UTI group(0. 37 ± 0. 14) and the NNH group(0. 38 ± 0. 11 )5. 00 ± 0. 12, P0. 01 and P0. 0001) were significantly lower than the control group(1. 74 ±0. 39,P0. 0001), and there was no difference between the UTI group and the NNH group(P=0. 94). Histological appearances showed less lung tissue damage in the UTI and NNH group. Conclusion: Rat left lung transplantation model with non-vessel endo-thelium cuff technique is easy to do and acceptable for short period research. UTI and NNH both prevented lung reperfusion injury after lung transplantation in rats. UTI can significantly reduce oxygen derived free radicals. NNH depressed the expression of proinflammatory fator and improved the function of transplanted lung, and have more significantly effect on preventing the lung reperfusion injury after lung transplantation in rats.
Establishment and Modification of Isolated Lung Transplantation and Reperfusion Model in Rats

Abstract:OBJECTIVE To establish and modify of the isolated lung transplantation and reperfusion model in rats with a relatively less-invasive way.METHODS Angiocatheters were inserted into femoral artery and jugular vein of the recipient rat to establish the controlled cross-circulatory.The venous cannula was connected to the pulmonary artery of the ex vivo lung block after preservation and cold storage to provide inflow,and Pulmonary effluent blood from the lung block was collected and returned to the support rat via the femoral artery for detoxification and deoxygenation.Each support rat was used to perfuse one lung block for at least one hour.The inflow sample to each lung block was analyzed for pH,pO2,pCO2,and Hb to follow alterations in support rat physiology.RESULTS There were no statistical differences in pH,PvO2,PvCO2 or Hb(P>0.05).CONCLUSION The support rat can deliver constant systemic venous blood at stable physiologic parameters to the ex vivo lung block and this animal model can be used in research of lung transplantation and ischemic-reperfusion injury.

Abstract:Elucidating the underlying mechanisms and developing protective strategies for the pathophysiological consequences of cardiopulmonary bypass (CPB) have been hampered due to the absence of a satisfactory long-term recovery animal model. The objective of this study was to establish a survival experimental model of CPB in rats to meet the requirement of these studies.Male SD rats (450-550 g) were randomly divided into CPB (n = 10) group and Sham group (n = 10). All rats were anaesthetized and mechanically ventilated. The femoral artery and vein were cannulated for continuous blood pressure recordings and fluid replacement, respectively. The CPB circuit comprised a venous reservoir, a membrane oxygenator, and a roller pump. Blood was drained from the right atrium via a jugular vein catheter and returned to the right carotid artery. Priming consisted of 8 ml of homologous blood and 8 ml of colloid. CPB was conducted for 60 min at a flow rate of 100-150 ml/kg/min in the CPB group. Haemodynamic investigations, blood gas analysis, and survival studies were performed subsequently.Our data show that the rat model principally simulated the clinical setting of CPB in terms of its construction, configuration, performance, material surface area, and priming volume to blood volume ratio. All CPB rats survived and the 2-week follow-up period remained uneventful.The rat model of CPB was easy to establish and was associated with excellent survival. This model should facilitate the investigation of the pathophysiological processes concerning CPB-related multiple organ dysfunction and possible protective interventions.
Evaluation of Rat Model Subjected to Cryopreserved Liver Transplantation

Abstract:Objective To study the impact of different cryopreservation time on the injury and regeneration of the donor liver after transplantation. Methods Orthotopic liver transplantation was performed using the two-cuff technique described by Kamada with modification. According to the different cryopreservation time of the donor liver in the University of Wisconsin (UW) solution, male SD rat recipients were randomly divided into four groups: group 1 (1 h), group 2 (8 h), group 3 (12 h) and group 4 (16 h), with 10 recipients and 10 donors in each group. After transplantation, the operation parameters, survival rate, liver injury and regeneration of the recipients were observed. Results The average anhepatic time was consistently kept below 15 min. The 168-hour survival rate was 100%, 80%, 50% and 20% in group 1, 2, 3 and 4, respectively. The survival rate in group 3 was significantly lower than that of group 1(P0.05). Submassive confluent necrosis and increased mitotic figures could be observed in group 3. Conclusion The SD rat that received the liver subjected to 12-hour cryopreservation may be the ideal model for the research of liver injury and regeneration during the early period after liver transplantation.
A novel and simple maneuver greatly improves rat liver transplantation

Abstract:Background: Orthotopic rat liver transplantation (OLT) is preferentially used to study transplant immunology; it remains challenging due to higher complications associated with 26-minute anhepatic time ceiling, here we safely extended the anhepatic time to improve success of OLT. Methods: Initially we performed OLT including whole graft OLT from inbred Sprague Dawley (SD) rat to SD (whole graft group, control group, n=21) and 30 minute anhepatic time (AHT) group (AHT group, n=11) to master this procedure. For generalization of this maneuver, partial OLT was performed from 50% Lewis allograft to Brown Norway (BN) rat to induce tolerance (half graft group, n=28), Cyclosporine A was injected once daily for 10 days in this group. Results: For whole graft group, 30-day survival rate was 85.5% (18/21), the reasons of death were gas embolism due to the missed suturing in 2 cases, blood loss in 1 case. For AHT group, 30-day survival rate was 72.7% (8/11), one recipient died from respiratory failure intra-operatively; two deaths were biliary leakage in 14 days. There were no differences in survival between whole graft group and AHT group (p=0.368). For tolerance group, 30-day survival rate was 82.0% (11/61), and the causes of death were diverse and recorded. Conclusion: The anhepatic time can be extended to 30 minutes simply through the change of clamping the diaphragm, which facilitates LT and improves its success good for the basic research.

Abstract:Objective To improve technique in making heart transplantation model in rats and decrease operative mortality.Method Modified surgical technique was used in donor heart collection, that is, right pulmonary artery instead of main pulmonary artery was anastomosed to inferior vena cava. Perioperative managements were also improved.Results 40 heterotopic heart transplantations were performed. 12 recipients died. Surgical technique, anesthetic accident, hypoxia, anastomotic bleeding may account for operative mortality.Conclusion Modified surgical technique and careful perioperative managements can contribute to success in heart transplantation in rats.
Development of a New Technique for Reconstruction of Hepatic Artery During Liver Transplantation in Sprague-Dawley Rat.

Abstract:BACKGROUND:Sleeve anastomosis is the most common technique used to rearterialize orthotopic liver transplants (OLT). However, this technique has a number of disadvantages, including difficulty of performance of the technique visually unaided. We herein describe a novel rearterialized OLT model in the rat.MATERIALS AND METHODS:Forty-six male Sprague Dawley rats (300-400 g) were used as donors and recipients. Based on Kamada's cuff technique, the new model involved performing a modified "sleeve" anastomosis between the celiac trunk of the donor and common hepatic artery of the recipient to reconstruct blood flow to the hepatic artery. An additional ten male Sprague Dawley rats underwent liver transplantation without artery reconstruction. Liver grafts were retrieved from the two groups and histological examination was performed following surgery.RESULTS:Total mean operating times were ~42 minutes for the donor liver extraction and 57 minutes for the recipient transplantation. Graft preparation took an additional 15 minutes and the time to fix the arterial bracket was ~3 minutes. During transplantation, the anhepatic phase lasted 18 ± 2.5 min and the artery reconstruction only required ~3 minutes. The patency rate was 94.44% and the 4-week survival rate was 90%. Histology indicated obvious fibrosis in the liver grafts without artery reconstruction, while normal histology was observed in the arterialized graft.CONCLUSIONS:This new method allows for the surgical procedure to be performed visually unaided with good survival and patency rates and represents an alternative model investigating OLT in rats.

Abstract:Although lung transplant is a life-saving therapy for some patients, primary graft dysfunction (PGD) is a leading cause of mortality and morbidity soon after a transplant. Ischemia reperfusion injury is known to be one of the most critical factors in PGD development. PGD is by definition an acute lung injury syndrome that occurs during the first 3 d following lung transplantation. To successfully translate laboratory discoveries to clinical practice, a reliable and practical large animal model is critical. This protocol describes a surgical technique for swine lung transplantation and postoperative management for a further 3 d post transplant. The protocol includes the background and rationale, required supplies, and a detailed description of the donor operation, transplant surgery, postoperative care, and sacrifice surgery. A pig lung transplant model is reliably produced in which the recipients survive for 3 d post transplant. This 3-d survival model can be used by lung transplant researchers to assess the development of PGD and to test therapeutic strategies targeting PGD. In total, the protocol requires 5 h for the surgeries, plus ~2 h in total for the postoperative care.

Abstract:Objective A model of left lung transplantation in rats was established by the means of microsurgical techniques to observe pulmonary function and pathological changes of orthotopic allografts in a long-term period.Methods Pure breed Wistar rats(weight,200～250 g) were chosen in this study.Twenty consecutive orthotopic left lung transplantations were performed in rats under opera-tion loupe(6×).The bronchial and venous anastomoses were hand-sewn with 9-0 interrupted sutures,and the artery was anastomosed with 10-0 interrupted sutures.The observation period was 100 d after transplantation.Results After restoring mechanical ventilation,all allografts reexpanded well.The entire lung of all recipients quickly had favorable blood supply and venous drainage following removal of clamps.Operation success rate was 85%(17/20),and the long-term survival rate of the recipients was 75%(15/20).Pathological findings showed that 12(60%) grafts had nearly normal pulmonary tissue architecture,whereas the structure of 2 had been completely destroyed,and 1 been partially destroyed.Conclusion The microsurgical anastomotic technique is advantageous for recipient's long-term survival and graft function after left lung transplantation in rats.
