Abstract:The diagnosis and treatment of congenital lung cysts producing severe symptoms are discussed. The cases of six infants form the basis of this report. All had such severe dyspnea and cyanosis that emergency operations were necessary. All six infants made uneventful recoveries. The youngest child was five days old.

Abstract:Background: The liver, a central organ in human metabolism, is often the primary target for drugs. However, conditions such as viral hepatitis, cirrhosis, non-alcoholic fatty liver disease (NAFLD), and hepatocellular carcinoma (HCC) present substantial health challenges worldwide. Existing treatments, which suffer from the non-specific distribution of drugs, frequently fail to achieve desired efficacy and safety, risking unnecessary liver harm and systemic side effects. Purpose: The aim of this review is to synthesise the latest progress in the design of liver-targeted prodrugs, with a focus on passive and active targeting strategies, providing new insights into the development of liver-targeted therapeutic approaches. Methods: This study conducted an extensive literature search through databases like Google Scholar, PubMed, Web of Science, and China National Knowledge Infrastructure (CNKI), systematically collecting and selecting recent research on liver-targeted prodrugs. The focus was on targeting mechanisms, including the Enhanced Permeability and Retention (EPR) effect, the unique microenvironment of liver cancer, and active targeting through specific transporters and receptors. Results: Active targeting strategies achieve precise drug delivery by binding specific ligands to liver surface receptors. Passive targeting takes advantage of the EPR effect and tumour characteristics to enrich drugs in liver tumours. The review details successful cases of using small molecule ligands, peptides, antibodies and nanoparticles as drug carriers. Conclusion: Liver-targeted prodrug strategies show great potential in enhancing the efficacy of drug treatment and reducing side effects for liver diseases. Future research should balance the advantages and limitations of both targeting strategies, focusing on optimising drug design and targeting efficiency, especially for clinical application. In-depth research on liver-specific receptors and the development of innovative targeting molecules are crucial for advancing the field of liver-targeted prodrugs.

Abstract:Liver diseases, particularly hepatitis B virus infections, liver cirrhosis and hepatocellular carcinoma continue to pose a significant health challenge worldwide due to the dire lack of curative treatment options besides liver resection and transplantation. Nanoparticle therapeutics, comprising of drugs, nucleic acids or proteins in association with a carrier, have emerged as safe and efficient systems in the treatment of the respective liver diseases. This review describes targeting strategies employed in relation to liver anatomy and disease etiologies, summarizes recent advances in the field and discusses the challenges and future perspectives for the effective treatment of liver diseases using polymer- and lipid-based nanoparticle therapeutics.

Abstract:Abstract The widespread use of nanomaterials (NMs) has raised concerns that exposure to them may introduce potential risks to the human body and environment. The liver is the main target organ for NMs. Hepatotoxic effects caused by NMs have been observed in recent studies but have not been linked to liver disease, and the intrinsic mechanisms are poorly elucidated. Additionally, NMs exhibit varied toxicokinetics and induce enhanced toxic effects in susceptible livers; however, thus far, this issue has not been thoroughly reviewed. This review provides an overview of the toxicokinetics of NMs. We highlight the possibility that NMs induce hepatic diseases, including nonalcoholic steatohepatitis (NASH), fibrosis, liver cancer, and metabolic disorders, and explore the underlying intrinsic mechanisms. Additionally, NM toxicokinetics and the potential induced risks in the livers of susceptible individuals, including subjects with liver disease, obese individuals, aging individuals and individuals of both sexes, are summarized. To understand how NM type affect their toxicity, the influences of the physicochemical and morphological (PCM) properties of NMs on their toxicokinetics and toxicity are also explored. This review provides guidance for further toxicological studies on NMs and will be important for the further development of NMs for applications in various fields.

Abstract:Primary liver cancer is the seventh-most-common cancer worldwide and the fourth-leading cause of cancer mortality. In the current era of precision medicine, the diagnosis and management of liver cancer are full of challenges and prospects. Mesoporous nanoparticles are often designed as specific carriers of drugs and imaging agents because of their special morphology and physical and chemical properties. In recent years, the design of the elemental composition and morphology of mesoporous nanoparticles have greatly improved their drug-loading efficiency, biocompatibility and biodegradability. Especially in the field of primary liver cancer, mesoporous nanoparticles have been modified as highly tumor-specific imaging contrast agents and targeting therapeutic medicine. Various generations of complexes and structures have been determined for the complicated clinical management requirements. In this review, we summarize these advanced mesoporous designs in the different diagnostic and therapeutic fields of liver cancer and discuss the relevant advantages and disadvantages of transforming applications. By comparing the material properties, drug-delivery characteristics and application methods of different kinds of mesoporous materials in liver cancer, we try to help determine the most suitable drug carriers and information media for future clinical trials. We hope to improve the fabrication of biomedical mesoporous nanoparticles and provide direct evidence for specific cancer management.

Abstract:This study explores and reviews systemic nanoparticle delivery barriers and highlights nanoparticle design and biological modulation strategies to mitigate nanoparticle clearance by biological systems. A literature analysis systematically evaluating the in vivo performance of current nanoparticle design strategies is conducted to clarify how nanoparticle physicochemical properties affect their pharmacokinetics and biodistribution in preclinical mouse models. Most nanomedicines require efficient in vivo delivery to elicit meaningful diagnostic and therapeutic effects. However, en route to their intended tissues, systemically administered nanoparticles often encounter delivery barriers. To describe these barriers, the term "nanoparticle blood removal pathways" (NBRP) is proposed, which summarizes the interactions between nanoparticles and the body's various cell‐dependent and cell‐independent blood clearance mechanisms. Nanoparticle design and biological modulation strategies are reviewed to mitigate nanoparticle‐NBRP interactions. As these interactions affect nanoparticle delivery, the preclinical literature from 2011–2021 is studied, and the nanoparticle blood circulation and organ biodistribution data are analyzed. The findings reveal that nanoparticle surface chemistry affects the in vivo behavior more than other nanoparticle design parameters. Combinatory biological‐PEG surface modification improves the blood area under the curve by ≈418%, with a decrease in liver accumulation of up to 47%. A greater understanding of nanoparticle‐NBRP interactions and associated delivery trends will provide new nanoparticle design and biological modulation strategies for safer, more effective, and more efficient nanomedicines.

Abstract:Most patients are at advanced stages when they are diagnosed with hepatocellular carcinoma, leading to poor prognosis and a low 5-year survival rate. Serological markers, ultrasound, computed tomography, magnetic resonance imaging, positron emission tomography, and liver biopsy are the common clinical diagnostic techniques for liver cancer. Effective interventions in the early stage will be beneficial to improve the prognosis of liver cancer patients and reduce the global burden. Therefore, it is urgent to develop new diagnostic methods to improve the diagnosis and management of liver cancer. Nanotechnology has become a new frontier subject in medical detection along with the application of nanomaterials in the manufacture of drug carriers, diagnostic tools, and therapeutic devices. Many studies have shown that nanoparticles (NPs) can be applied to the clinical diagnosis of liver cancer in combination with existing technologies, providing a new method for the early diagnosis of liver cancer. In this review, we elaborate on the theoretical basis and characteristics of NPs in the diagnosis of liver cancer, and the research progress and prospects of NPs in the diagnosis of liver cancer are summarized.

Abstract:Hepatocellular carcinoma (HCC) is one of the cancers that has the highest morbidity and mortality rates. In clinical practice, there are still many limitations in surveilling, diagnosing, and treating HCC, such as the poor detection of early HCC, the frequent post-surgery recurrence, the low local tumor control rate, the therapy resistance and side effects. Therefore, improved, or innovative modalities are urgently required for early diagnosis as well as refined and effective management. In recent years, nanotechnology research in the field of HCC has received great attention, with various aspects of diagnosis and treatment including biomarkers, ultrasound, diagnostic imaging, intraoperative imaging, ablation, transarterial chemoembolization, radiotherapy, and systemic therapy. Different from previous reviews that discussed from the perspective of nanoparticles' structure, design and function, this review systematically summarizes the methods and limitations of diagnosing and treating HCC in clinical guidelines and practices, as well as nanomedicine applications. Nanomedicine can overcome the limitations to improve diagnosis accuracy and therapeutic effect via enhancement of targeting, biocompatibility, bioavailability, controlled releasing, and combination of different clinical treatment modalities. Through an in-depth understanding of the logic of nanotechnology to conquer clinical limitations, the main research directions of nanotechnology in HCC are sorted out in this review. It is anticipated that nanomedicine will play a significant role in the future clinical practices of HCC.

Abstract:Hepatobiliary diseases are serious universal health concerns that affects the liver and is responsible for around 2 million deaths annually. The limitations of standard drug-delivery systems, such as unequal and nonspecific drug distribution, which result in adverse effects on healthy tissues and a reduction in drug bioavailability, make successful therapy difficult even when choices for surgery and medicine are available. With the liver's distinct physiological and anatomical features, nanotechnology offers a potential approach to targeted medication delivery, making it a prime target for nanomedicine. In the context of liver illnesses, a number of nanomaterial categories, such as polymer, inorganic and multifunctional nanoparticles (NPs), have been investigated as possible agents for targeting this organ. Nanomaterials can be specifically targeted to liver tissue or hepatocytes by surface modification and functionalization, increasing the delivery of medications and reducing their adverse effects. Although there are many advantages to using nanomaterials, their toxicity and stability can cause problems for living things, including inflammation and protein adsorption. Notwithstanding the difficulties associated with the creation of nanoparticles, further study and development hold much promise for the use of nanotechnology to the targeted treatment of liver disorders. The nanomaterials used to address liver diseases nanozymes, Nano-based oligonucleotide, Lipid-based nanostructures and nanoemulsions and Extracellular Vesicles are first discussed in this study. Additionally, the ways in which nanomaterials target liver illness are examined. This study concludes with a discussion of the present issues and potential future avenues for this line of inquiry.

Abstract:Anti-cancer drugs with various mechanisms emerge in succession, but most of free therapeutics are accompanied by some disadvantages, such as poor safety and effectiveness, inability to target tumor cells, and short circulation time. Nanotechnology provides new solutions for overcoming the intrinsic limits of free therapeutics and navigating biological barriers. Considerable technological and clinical success have been achieved in cancer nanomedicines, but the main obstacle to the clinical translation of nanomedicine is an incomplete understanding of the requirements of clinical trials in new and established research scientists and the latest research of nanomedicine in clinicians. In this review, by searching the authoritative database, we introduce the progress and challenges of nanomedicines that have entered the clinical market or are ubiquitous in clinical trials for cancer therapy, including lipid-based nanoparticles, polymeric nanoparticles and albumin-based nanoparticles. Furthermore, we introduce several biomolecule-based nanomedicines that are promising for clinical application in the future, including biomimetic membranes, virus-like particles and DNA-based nanomachine, and highlight their advanced nanoparticle designs, advantages and challenges.

Abstract:The incidence and mortality of hepatocellular carcinoma have continued to increase over the last few years, and the medicine-based outlook of patients is poor. Given great ideas from the development of nanotechnology in medicine, especially the advantages in the treatments of liver cancer. Some engineering nanoparticles with active targeting, ligand modification, and passive targeting capacity achieve efficient drug delivery to tumor cells. In addition, the behavior of drug release is also applied to the drug loading nanosystem based on the tumor microenvironment. Considering clinical use of local treatment of liver cancer, in situ drug delivery of nanogels is also fully studied in orthotopic chemotherapy, radiotherapy, and ablation therapy. Furthermore, novel therapies including gene therapy, phototherapy, and immunotherapy are also applied as combined therapy for liver cancer. Engineering nonviral polymers to function as gene delivery vectors with increased efficiency and specificity, and strategies of co-delivery of therapeutic genes and drugs show great therapeutic effect against liver tumors, including drug-resistant tumors. Phototherapy is also applied in surgical procedures, chemotherapy, and immunotherapy. Combination strategies significantly enhance therapeutic effects and decrease side effects. Overall, the application of nanotechnology could bring a revolutionary change to the current treatment of liver cancer.

Abstract:Hepatocellular carcinoma (HCC) is a prevalent malignant tumor with a range of risk factors, including viral infections, alcoholic liver disease, exposure to fungal toxins, obesity, and type 2 diabetes. Despite advancements in early detection and treatment, HCC continues to exhibit a high rate of recurrence. Patients in advanced stages have a poor prognosis, and the survival rate after cancer metastasis is notably low. Consequently, there exists an urgent necessity for novel treatment strategies. Nanomedicine possesses superior attributes, such as targeted administration and responsive drug release within the tumor microenvironment. These systems demonstrate potential in HCC treatment by facilitating the transportation of diverse therapeutic drugs. This comprehensive review aims to delve into the application of nanoparticle drug delivery systems in the management of liver tumors, with particular emphasis on formulation, targeted strategies specific to liver tumors, and various methods of drug release. By offering insights into the utilization of nanoparticle drug delivery systems in the realm of liver tumor treatment, this review endeavors to assist readers in exploring their vast potential.

Abstract:The precise treatment of liver cancer is receiving much research attention. Surgery, chemoradiotherapy, and other methods remain the mainstream of this treatment, but many chemotherapeutic drugs used to treat advanced liver cancer cause adverse reactions and have unstable efficiencies. Active ingredients used in traditional Chinese medicine (TCM) have been examined widely in anti-cancer research due to their advantages of multi-pathway targeting and rich pharmacological effects. However, these active components have poor water solubility, bioavailability, and targeting efficiency. Nanomedicine has been applied to improve the efficacy of TCM ingredients in the treatment of liver cancer. Nanoparticles (NPs) show great potential in this context due to their excellent bioavailability, high controlled agent release efficiency, and clear targeting. This paper reviews the application of NPs loaded with active TCM ingredients in the treatment of liver cancer, with the aim of facilitating new vector development and improving the precision treatment of liver cancer.

Abstract:Liver disorders present a significant global health challenge, necessitating the exploration of innovative treatment modalities. Liposomal nanocarriers have emerged as promising candidates for targeted drug delivery to the liver. This review offers a comprehensive examination of the mechanisms and applications of liposomal nanocarriers in addressing various liver disorders. Firstly discussing the liver disorders and the conventional treatment approaches, the review delves into the liposomal structure and composition. Moreover, it tackles the different mechanisms of liposomal targeting including both passive and active strategies. After that, the review moves on to explore the therapeutic potentials of liposomal nanocarriers in treating liver cirrhosis, fibrosis, viral hepatitis, and hepatocellular carcinoma. Through discussing recent advancements and envisioning future perspectives, this review highlights the role of liposomal nanocarriers in enhancing the effectiveness and the safety of liver disorders and consequently improving patient outcomes and enhances life quality.

Imaging decreased brain docosahexaenoic acid metabolism and signaling in iPLA2β (VIA)-deficient mice

Abstract:Ca2+-independent phospholipase A2β (iPLA2β) selectively hydrolyzes docosahexaenoic acid (DHA, 22:6n-3) in vitro from phospholipid. Mutations in the PLA2G6 gene encoding this enzyme occur in patients with idiopathic neurodegeneration plus brain iron accumulation and dystonia-parkinsonism without iron accumulation, whereas mice lacking PLA2G6 show neurological dysfunction and neuropathology after 13 months. We hypothesized that brain DHA metabolism and signaling would be reduced in 4-month-old iPLA2β-deficient mice without overt neuropathology. Saline or the cholinergic muscarinic M1,3,5 receptor agonist arecoline (30 mg/kg) was administered to unanesthetized iPLA2β−/−, iPLA2β+/−, and iPLA2β+/+ mice, and [1-14C]DHA was infused intravenously. DHA incorporation coefficients k* and rates Jin, representing DHA metabolism, were determined using quantitative autoradiography in 81 brain regions. iPLA2β−/− or iPLA2β+/− compared with iPLA2β+/+ mice showed widespread and significant baseline reductions in k* and Jin for DHA. Arecoline increased both parameters in brain regions of iPLA2β+/+ mice but quantitatively less so in iPLA2β−/− and iPLA2β+/− mice. Consistent with iPLA2β’s reported ability to selectively hydrolyze DHA from phospholipid in vitro, iPLA2β deficiency reduces brain DHA metabolism and signaling in vivo at baseline and following M1,3,5 receptor activation. Positron emission tomography might be used to image disturbed brain DHA metabolism in patients with PLA2G6 mutations.
Personal recognition using geometric features in the phase space of electrocardiogram

Abstract:Electrocardiogram (ECG) is one of the biomedical properties that have recently been studied for biometrics. We prop ose a new method for human recognition by phase-space reconstruction (PSR) of a single-lead ECG signal. To reconstruct a single-lead ECG signal into phase space, we used a time delay technique We extracted the geometric features through the trajectory from the phase space and analyzed it using our method. The ECG signals used in this study were measured in various situations such at rest, during exercise, while listening to music, and watching a video. We performed phase space reconstruction by applying some time-delay to the measured ECG signal and extracted 21 geometric al features to find the best identifiable time-delay value through Support vector machine learning. The results were performed on 1 3 subjects. The accuracy was 97.8% when the delay was 8ms. Based on this result personal authentication was conducted. The results show 97.7% accuracy, 1.5% FAR(False Acceptance Ratio) and 2.9% FRR(False Rejection Ratio).

Abstract:Nanomedicines are of increasing scrutiny due to their improved efficacy and/or mitigated side effects. They can be integrated with many other therapeutics to further boost the clinical benefits. Among those, herbal medicines are arousing great interest to be combined with nanomedicines to exert synergistic effects in multifaceted mechanisms. The in vivo performance of nanomedicines which determines the therapeutic efficacy and safety is believed to be heavily influenced by the physio-pathological characters of the body. Activation of multiple immune factors, e.g., complement system, phagocytic cells, lymphocytes, and among many others, can affect the fate of nanomedicines in blood circulation, biodistribution, interaction with single cells and intracellular transport. Immunomodulatory effects and metabolic regulation by herbal medicines have been widely witnessed during the past decades, which alter the physio-pathological conditions and dramatically affect in vivo delivery of nanomedicines. In this review, we summarize recent progress of understanding on the in vivo delivery process of nanomedicines and analyze the major affecting factors that regulate the interaction of nanomedicines with organisms. We discuss the immunomodulatory roles and metabolic regulation by herbal medicines and their effects on in vivo delivery process of nanomedicines, as well as the prospective clinical benefits from the combination of nanomedicines and herbal medicines.
Strategies to improve tumor penetration of nanomedicines through nanoparticle design

Abstract:Nanoparticles (NPs) have emerged as an effective means to deliver therapeutic drugs for cancer treatment, as they can preferentially accumulate at tumor site through the enhanced permeability and retention effect. Various forms of NPs including liposomes, polymeric micelles, and inorganic particles have been used for therapeutic applications. However, the therapeutic benefits of nanomedicines are suboptimal. Although many possible reasons may account for the compromised therapeutic efficacy, the inefficient tumor penetration can be a vital obstacle. Tumor develops characteristic pathological environment, such as abnormal vasculature, elevated interstitial fluid pressure, and dense extracellular matrix, which intrinsically hinder the transport of nanomedicines in the tumor parenchyma. The physicochemical properties of the NPs such as size, shape, and surface charge have profound effect on tumor penetration. In this review, we will highlight the factors that affect the transport of NPs in solid tumor, and then elaborate on designing strategies to improve NPs' penetration and uniform distribution inside the tumor interstitium.
