Abstract:A 15-year-old white adolescent was first diagnosed with a right temporo-parietal anaplastic ependymoma at the age of 8 years. A tumor of 7 3 4 cm-size was macroscopically completely resected and adjuvant chemotherapy was applied (Fig. 1). Within the next 31⁄2 years, he suffered from three local relapses. Stabilization of disease was achieved by combined treatment of further surgery, chemotherapy, and irradiation. However, 7 years after the initial diagnosis, the boy presented with dyspnoea, thoracic pain on inspiration, and back pain since a few days. Diagnostic procedures included a chest X-ray film that showed a round lesion in the right lower lung suggestive of a metastasis and a massive pleural effusion on the left side. A CT scan of the lung revealed extensive bilateral pleural involvement and a large tumor in the lower lobe of the right lung as well as several enlarged hilar lymph nodes (Fig. 2). Pathohistologic analysis confirmed the diagnosis of metastastic anaplastic ependymoma (Fig. 3). In accordance with the patient and his parents, a therapeutic trial with whole body thermochemotherapy was performed, from which the patient only very slowly recovered. Unfortunately, a subsequent CT scan of the lung demonstrated tumor progression. No further therapy was administered according to the wish of the patient and his parents and 3 months later the patient died from respiratory failure. Ependymomas arise from the ependymal cell layer lining the ventricular system and the central canal of the spinal cord. They belong to the common brain tumors of childhood and primarily affect young children with a mean age at diagnosis of 5 years. In general, the prognosis of childhood ependymomas is relatively poor, with an overall 5-year survival rate of approximately 50% [1]. Ependymoma is usually complicated by local recurrence or much less commonly by CNS metastases. Extraneural metastases, most of which originate from primary tumors of the supratentorial compartment, are a very infrequent complication of ependymoma and very few cases of pleuropulmonary metastases in children have been reported to date. Clinicopathologic studies reveal extraneural metastases or relapses of primary intracranial tumors in only 0.4–2% of all cases. This increases to 25% when autopsies are performed [2].
