Abstract:Esophageal metastasis from a primary lesion is considered rare, especially when deriving from endometrial adenocarcinomas. Here, we describe a case of a 58-year-old woman presenting with a 3-month history of progressive dysphagia and lower abdominal pain. Subsequently, a poorly differentiated esophageal carcinoma was confirmed by endoscopic biopsy. The pathological and immunohistochemical analyses of the esophageal lesion were similar to that of the endometrial adenocarcinoma that had been removed 2 years earlier, suggesting that the esophageal tumor derived from her previous endometrial adenocarcinoma. The patient received 6 cycles of paclitaxel plus carboplatin first-line chemotherapy and 2 cycles of epirubicin plus cisplatin second-line chemotherapy, with continued disease progression. After switching to apatinib, computed tomography (CT) scans revealed stable disease after 2 months of treatment. Our goal in this case report was to increase the awareness of clinicians and pathologists regarding the possibility of esophageal metastasis originating from endometrial adenocarcinoma, although it is a rare occurrence. Apatinib may be effective for treating metastatic endometrial carcinoma. However, further investigations are warranted to determine the optimal targeted agent for this condition.
An Ovarian Large-Cell Neuroendocrine Carcinoma Accompanied by Clear Cell Carcinoma with Specific High Level of AFP: Case Report and Review of the Literature

Abstract:Large cell neuroendocrine carcinoma (LCNEC) is a rare histological subtype of ovarian cancer. A few cases have been reported in the literature with extreme invasiveness and a poor prognosis. However, there still have not been accepted criteria for diagnosis and treatment of LCNEC. Here we report an unmarried 37 year-old woman who was diagnosed with LCNEC associated with clear cell carcinoma and the tumor index was manifested with a specific increase of AFP. The case received six courses of etoposide and carboplatin chemotherapy as an adjuvant therapy after primary curative surgery. However, she relapsed within 6 months after surgery and metastasized rapidly to distant organs despite combined chemotherapy of paclitaxel, cisplatin, and bevacizumab, and died 18 months after primary surgery. This is the first reported case of LCNEC manifested with a specific increase of AFP and characteristically metastasized to the spine as recurrence. Reviewing our case as well as previously reported cases, LCNEC present with aggressive malignancy and vulnerable to distant metastasis through a hematogenous approach, we conjectured that adding Bevacizumab in primary chemotherapy may be beneficial to extend disease-free survival. But so far there is no recommendation of this regimen for treatment of LCNEC in current guidelines. Further research is needed to confirm this view so as to find the best treatment of LCNEC and improve the prognosis of these patients.
Large cell neuroendocrine carcinoma of urinary bladder and ureter: a case report and review of the literature

Abstract:Objective To investigate the clinicopathological features,diagnosis,treatment and prognosis of a case of large cell neuroendocrine carcinoma (LCNEC) in urinary bladder and ureter.Methods The clinical pathological data of a patient with LCNEC in urinary bladder and ureter was retrospectively analyzed and the related literatures were reviewed.It was a male patient,78 years old,presenting with total painless gross hematuria.The ultrasonography showed nodules on the left lateral wall of bladder.Cystoscopy showed 2 cm cauliflower-like mass on the left lateral wall of bladder,and pathologic biopsy showed bladder urothelial carcinoma.Results Transurethral resection of bladder tumor was performed.The tumor cells were large in size,with vesicular nuclei,prominent nucleoli and nested/trabecular architecture.Immunohistochemical analysis demonstrated that tumor cells were positive for synaptophysin,chromogranin A and CD56.The proliferation index evaluated with Ki-67 was about 80％.The pathology report revealed a large cell neuroendocrine bladder tumor with focal areas of high-grade urothelial carcinoma.After intravesical instillation therapy for 2 months,tumor recurrence was detected by CT.Laparoscopic radical cystectomy and ureteral segmental resection was performed.The pathology report revealed a LCNEC with focal areas of highgrade invasive urothelial carcinoma in the bladder and ureter.CT showed tumor recurrence with liver metastasis 1 month after the operation.The overall survival was 9 months.Conclusions The clinical and imaging findings of LCNEC in bladder and ureter are nonspecific.Diagnosis must depend on the pathological and immunohistochemical staining.The tumor is aggressive with high risk of recurrence and metastasis and has a poor prognosis.It is mainly treated with radical resection combined with chemotherapy.

Abstract:Pulmonary large cell neuroendocrine carcinoma (LCNEC) is a rare but destructive tumor type, accounting for approximately 1% of all lung cancers, associated with poor prognosis. LCNEC is challenging diagnosing using biopsy specimens. While current LCNEC therapies include surgery, radiotherapy, and chemotherapy, it has not yet proved its greatest treatment strategies. Immunotherapy is rapidly emerging as a possibility for lung cancer treatment. However, there are scant reports in the literature regarding LCNEC immunotherapy. Therefore, the author here reports a case of LCNEC by immunotherapy, and retrospective reviews the present research status and progress of LENCE and corresponding clinical treatment progress. This case will supply valuable information for the treatment options for LCNEC. A 64-year-old male smoker was treated for one month for blood in his sputum. Chest radiography and computed tomography revealed a 3-cm solitary tumor in the left upper lung. We treated the patient with thoracoscopic radical surgery for upper left lung cancer. Postoperative pathology shows pulmonary LCNEC. We performed postoperative chemotherapy with a double-drug regimen holding platinum. Then, bevacizumab, paclitaxel, and the PD-L1 checkpoint inhibitor nivolumab were applied, but the patient progressed rapidly. Immunotherapy is an ineffective treatment possibility for these patients, even if PD-L1 expression is positive. A possible contributing factor is the timing of immunotherapy too late.

Abstract:Neuroendocrine neoplasm (NEN) is developing from the enterochromaffin cells in the neuroendocrine tissue throughout the body. [ 1 ] Digestive system NEN has been classified by the World Health Organization in 2019 into 2 main categories: neuroendocrine tumor (NET) and neuroendocrine carcinoma (NEC). NET includes the well-differentiated neuroendocrine tumors (G1 and G2), and the poorly differentiated neuroendocrine tumors (G3). NEC includes the small cell neuroendocrine carcinoma (SCNEC) and LCNEC. NEN is a rare tumor, and gastric LCNEC is a much rarer high-grade neuroendocrine carcinoma that tends to have a poor prognosis. During the last years, only a few sporadic cases of gastric LCNEC have been reported. Usually, these tumors are only found at the time of metastasis making them even more difficult to treat. Therefore, we reported 2 patients with gastric LCNEC. The clinical manifestation, pathology morphology, immunohistochemical characteristics, cell proliferative index and prognosis of these patients were summarized in this study, along with an in-depth literature discussion, in order to improve the pathologist accurate diagnosis, the understanding more about this uncommon tumor and further to provide more experience to treat this disease.

Abstract:Background: Large-cell neuroendocrine carcinoma (NEC) is an uncommon type of tumor that can occur in the endometrium. This aggressive cancer requires definitive management. Here, we describe the clinical characteristics and treatment of a postmenopausal woman with large cell NEC of the endometrium. Case summary: A 55-year-old Asian female presented with a 1-year history of postmenopausal vaginal bleeding. Transvaginal ultrasound revealed a thickened endometrium (30.2 mm) and a hypervascular tumor. Computed tomography revealed that the tumor had invaded more than half of the myometrium and spread to the pelvic lymph nodes. The tumor marker, carcinoembryonic antigen, was elevated (3.65 ng/mL). Endocervical biopsy revealed high-grade endometrial carcinoma. She underwent radical hysterectomy, bilateral salpingo-oophorectomy, omentectomy, and bilateral pelvic and para-aortic lymph node dissection. Pathological examination revealed mixed neuroendocrine and endometrioid adenocarcinoma, pT2N0M0, grade 3, and International Federation of Gynecology and Obstetrics stage 2. Immunohistochemistry showed moderate estrogen and progesterone receptor expressions (20% and 1%, respectively), focal CD56 expression (NEC marker), positive staining for vimentin, p53 (wild type), and ki67 (90%), and loss of expression of PMS2 (Lynch syndrome marker). The patient received five cycles of cisplatin and etoposide after surgery. No recurrence was noted after 5 mo. Conclusion: We report the characteristics and successful management of a rare case of large-cell endometrial NEC concomitant with Lynch syndrome.

Abstract:<h3 class="Heading">Background</h3><p class="Para">There is no consensus on the optimal chemotherapy regimen and the prognostic factors for ovarian large cell neuroendocrine carcinoma (LCNEC), a rare type of tumor. The objective of the present study is to present the case of a recent encounter of pure ovarian LCNEC and perform a brief review to summarize the clinicopathological features and prognostic factors of 57 cases of LCNEC patients that have been previously reported.</p><h3 class="Heading">Method: case presentation</h3><p class="Para">Eligible studies were searched for online and 57 cases with clear follow-up data were found to have been reported. We present the 58th case, which is of a 70-year-old woman with stage IIIc primary pure LCNEC of the ovary. The initial symptom of this patient was abdominal distension (more than 2 months). A recent ultrasound test showed a solid-cystic mass occupying the pelvic and abdominal cavity. She received two courses of cisplatin-etoposide chemotherapy as an adjuvant therapy. No signs of nonclinical or radiological evidence of disease recurrence was found at follow-up examinations during the first 3 months after operation. A retrospective review of these 58 cases was conducted and survival curves were estimated. Using the Kaplan-Meier method.</p><h3 class="Heading">Conclusion</h3><p class="Para">The patients included were aged between 18 and 80 years. A Kaplan-Meier survival curve revealed that the median overall survival was 10.000 months, while 26 (44.83%) patients died within 12 months. We compared the overall mean survival time of all patients with that of stage I patients (42.418 vs 42.047 months), which suggests that ovarian LCNEC has a very poor prognosis even at stage I. Mean survival was longer for patients who had undergone postoperative chemotherapy than for those without postoperative chemotherapy (48.082 vs 9.778 months). A small series, such as this, does not provide adequate data to establish a firm correlation between the postoperative chemotherapy and prognosis (<em class="EmphasisTypeItalic">p</em> = 0.176). In our review of 58 cases with ovarian LCNEC, prognosis was unfavorable in most cases. Given the rarity of LCNEC, it is highly recommended that a global medical database of ovarian LCNEC and a standard system of diagnosis and treatment is established.</p>

Abstract:BACKGROUND: Gastric neuroendocrine carcinoma (GNEC) is a rare histological subtype of gastric cancer, which is categorized into small cell and large cell neuroendocrine carcinomas. It is characterized by strong invasiveness and poor prognosis. Mixed large and small cell neuroendocrine carcinoma (L/SCNEC) is an extremely rare pathological type of gastric cancer, and there have been no reports on this situation until now.CASE SUMMARY: Herein, we first present a 57-year-old patient diagnosed with L/SCNEC of the stomach. A 57-year-old Chinese male presented with epigastric discomfort. Outpatient gastroscopic biopsy was performed, and pathological examination revealed that the cardia was invaded by adenocarcinoma. The patient underwent laparoscopic-assisted radical proximal subtotal gastrectomy and was diagnosed with L/SCNEC. He refused adjuvant treatment and was followed up every 3 mo. Eight months after the operation, the patient showed no evidence of local recurrence or distant metastasis.CONCLUSION: We advocate conducting further genomic studies to explore the origin of gastric large cell and small cell neuroendocrine carcinoma and using different chemotherapy schemes according to large or small cell neuroendocrine carcinoma of the stomach for clinical research to clarify the heterogeneity of GNEC and improve the prognosis of patients with GNEC.

Abstract:<span><strong>Purpose:</strong> The aim of this study was to review the clinicopathologic characteristics, treatments, and outcomes of patients with primary large cell neuroendocrine carcinoma of the bladder (LCNEC).<strong>Patients and Methods:</strong> We report one patient diagnosed with primary pure LCNEC of the bladder in Sun Yat-sen Memorial Hospital. In addition, we performed a systematic literature review, in April 2020, on case report and case series of LCNEC of the bladder. The clinicopathologic characteristics, treatments and outcomes of this rare disease were analyzed.<strong>Results:</strong> A total of 39 patients were included in our analysis (1 case from our institution and 38 cases from the literature). Most patients (79.5%) were male. The average age at the surgery for the patients is 61.5 years (range 19–85 years). The most common symptom was hematuria (<i>n</i> = 20, 76.9%). Almost all patients (38, 97.4%) underwent surgery, with 26 (66.7%) receiving multimodality therapy. Out of 24 patients with available data, regional or distant recurrences developed in 14 patients (58.3%). The median overall survival of the patients was 11.5 months, with 1- and 3-year survival rates of 54.0 and 21.4%, respectively. In the survival analysis, theT1–2 tumors (<i>P</i> = 0.025), no distant metastases at diagnosis (<i>P</i> = 0.001), and multimodality therapy (<i>P</i> = 0.017) were associated with better overall survival (OS).<strong>Conclusions:</strong> LCNEC of the bladder is an extremely rare neoplasm. The available data suggest that the disease has an aggressive natural history with poor prognosis. Early pathologic stage and multimodality treatment may be important factors in determining prognosis.</span>

Abstract:Neuroendocrine prostate neoplasms, encompassing small cell carcinoma, carcinoid, and large cell carcinoma, are infrequently observed in malignant prostate tumors. The occurrence of large cell neuroendocrine prostate cancer (LCNEPC) is exceedingly rare. In this study, the patient initially presented with a persistent dysuria for a duration of one year, accompanied by a serum prostate-specific antigen (PSA) level of 17.83ng/mL. Prostate magnetic resonance imaging (MRI) and chest computed tomography (CT) scan showed that a neoplastic lesion was considered, and prostate biopsy confirmed prostate adenocarcinoma with a Gleason score of 7 (4 + 3). Then, thoracoscopic lung tumor resection was performed, and the pathological examination revealed the presence of primary moderately differentiated invasive adenocarcinoma of the lung and metastatic prostate adenocarcinoma, the Gleason score was 8 (4 + 4). After 1 year of endocrine therapy with goserelin acetate and bicalutamide, he underwent a laparoscopic radical prostatectomy (LRP), the pathological report indicated the presence of adenocarcinoma mixed with NE carcinoma. Two months after the LRP, the patient experienced gross hematuria and sacral tail pain. Further examination revealed multiple metastatic lesions throughout the body. He also underwent transurethral resection of bladder tumor (TURBT) for bladder tumor and received etoposide+ cisplatin chemotherapy three weeks post-surgery. The patient eventually died of multi-organ failure due to myelosuppression after chemotherapy. This case report presents an uncommon instance of LCNEPC with widespread systemic metastases, while also providing a comprehensive review of existing literature to facilitate improved management and treatment strategies for similar patients in subsequent cases.

Abstract:Pulmonary large cell neuroendocrine carcinoma (LCNEC) is a high-grade neuroendocrine carcinoma (HGNEC) accounting for 3% of primary lung cancer, and characterized by strong invasion, high heterogeneity, and extremely poor prognosis. At present, the diagnosis and treatment of LCNEC remains controversial and refer to therapeutic strategy of small cell lung cancer (SCLC), lacking precise therapy. Recently, the genetic analysis and clinical trials of LCNEC gradually emerged, providing more evidence for precise diagnosis and treatment. Here, we review the diagnosis, molecular characteristics, and treatment of LCNEC based on the existing research and frontier progress to provide a potential direction for future diagnosis and treatment of LCNEC.

Delay-optimal rate allocation in multiaccess communications: a cross-layer view

Abstract:The literature on multiaccess communications has traditionally treated "network-layer" issues such as source burstiness, network delay, and buffer overflow, apart from "physical-layer" issues such as channel modeling, coding, and detection. The recent work of Telatar and Gallager [I. E. Telatar and R. Gallager, Combining Queueing Theory with Information Theory for Multiaccess, August 1995] [I.E. Telatar, Multiple Access Information Theory and Job Scheduling, 1995] have sought to bridge this unfortunate division. We extend this line of inquiry by examining a multiaccess communication scenario where users' packets arrive randomly into separate queues and transmission rates are allocated from the information-theoretic multiaccess capacity region based on the respective users' queue states. In the symmetric case, a longer-queue-higher rate (LQHR) allocation strategy is shown to minimize the average system delay of packets. Such a policy can be interpreted in the coding context as adaptive successive decoding. The delay performance of the LQHR policy provides a fundamental lower bound to the performance for multiaccess coding schemes which seek to meet any given level of decoding error probability.

Abstract:Background Large cell neuroendocrine carcinoma (LCNEC) is a rare, aggressive cancer with a dismal prognosis. The majority of cases occur in the lung and the gastrointestinal tract; however, it can occur throughout the body. Recently advances in the understanding of the molecular underpinnings of this disease have paved the way for additional novel promising therapies. This review will discuss the current best evidence for management of LCNEC and new directions in the classification and treatment of this rare disease. Methods We performed a PubMed search for “Large cell neuroendocrine carcinoma” and “High grade neuroendocrine carcinoma.” All titles were screened for relevance to the management of LCNEC. Papers were included based on relevance to the management of LCNEC. Results Papers were included reviewing both pulmonary and extra pulmonary LCNEC. We summarized the data driven best practices for the management of both early and advanced stage LCNEC. We describe emerging therapies with promising potential. Discussion LCNEC are rare and aggressive neoplasms. In advanced disease, the historical regimen of platinum based therapy in combination with etoposide or irinotecan remains among the commonly used first line therapies, however for extra thoracic LCNEC regimens like FOLFOX, FOLFOIRI and CAPTEM can also be used. Further effective and safe treatment options are desperately needed. Recently, new advances including a new understanding of the genetic subcategories of LCNEC and immunotherapy agents may guide further treatments.

Abstract:BACKGROUND Large cell neuroendocrine carcinoma (LCNEC) of the bladder is a rare non-urothelial tumor of the bladder. The treatment of LCNEC of the bladder is different from that of urothelial carcinoma (UC); therefore, early and accurate diagnosis is particularly important. As LCNEC of the bladder is rare and its clinical symptoms and radiographic features are similar to those of urothelial tumors, the clinical diagnosis of the disease remains challenging. CASE SUMMARY We report a 72-year-old female patient who presented with gross hematuria for 3 mo. A solitary tumor located in the anterior wall of the bladder was found by cystoscopy. Pathological examination after biopsy suggested UC of the bladder in the absence of immunohistochemical assessment. The patient underwent partial cystectomy and was finally diagnosed with LCNEC (pT2bN0M0) based on the results of postoperative immunohistochemical examination. During the 10-mo follow-up, no signs of tumor recurrence or metastasis were found. CONCLUSION Immunohistochemical examination is essential for diagnosing LCNEC of the bladder. Accurate diagnosis and multidisciplinary treatment in the early stage of the disease are crucial for improving the prognosis.

Abstract:Large cell neuroendocrine carcinoma (LCNEC) is part of lung neuroendocrine tumors. LCNEC represents an extremely rare entity with aggressive behavior and poor prognosis. Primary surgery is the mainstream of treatment, although it is rarely amenable due to local or systemic tumor metastasis at the time of the diagnosis. We present a case report of a female patient diagnosed with large cell neuroendocrine lung cancer metastatic to the brain. Noting the low incidence of the disease, the lack of relevant clinical data has resulted in a challenge in diagnosis and management.
Clinicopathological characteristics and prognostic factors of pulmonary large cell neuroendocrine carcinoma: A large population-based analysis

Abstract:Background: The study was conducted to compare the clinicopathological characteristics, survival outcomes, and metastatic patterns between pulmonary large cell neuroendocrine carcinoma (LCNEC) and other non-small cell lung cancer (ONSCLC), and to identify the prognostic factors of LCNEC. Methods: Data of patients diagnosed with LCNEC and ONSCLC from 2004 to 2014 were obtained from the Surveillance, Epidemiology, and End Results dataset. Pearson's chi-square tests were used to compare differences in clinicopathological characteristics. The Kaplan-Meier method was used for survival analysis. A propensity score was used for matching and a Cox proportional hazards model was used for multivariate and subgroup analyses. Results: A total of 2368 LCNEC cases and 231 672 ONSCLC cases were identified. LCNEC incidence increased slightly over time. Except for marital status, LCNEC patients had obviously different biological features to ONSCLC patients. Survival analysis showed that LCNEC had poorer outcomes than ONSCLC. Multivariate analysis revealed that female gender, black race, surgery, radiation, and chemotherapy were protective factors for LCNEC. Matched subgroup analysis further demonstrated that most subgroup factors favored ONSCLC, especially in early stage. Early-stage LCNEC patients had a higher risk of lung cancer-specific death than early-stage ONSCLC patients. Moreover, metastatic patterns were different between LCNEC and ONSCLC. LCNEC patients with isolated liver metastasis or combined invasion to other organs had poorer survival rates. Conclusions: LCNEC has totally different clinicopathological characteristics and metastatic patterns to ONSCLC. LCNEC also has poorer survival outcomes, primarily because of isolated liver metastasis or combined invasion to other organs. Most subgroup factors are adverse factors for LCNEC.

Abstract:Large-cell neuroendocrine carcinoma (LCNEC) of the extrahepatic bile duct is an exceedingly rare and aggressive neoplasm. [ 1 , 2 ] Due to its low incidence, there is a scarcity of case reports and a limited understanding of its clinical behavior, optimal treatment strategies, and prognosis. [ 3 , 4 ] Most of the available literature highlights the aggressive nature and poor prognosis associated with LCNEC, emphasizing the need for early detection and comprehensive treatment. [ 5 , 6 ] Neuroendocrine tumors of the biliary tract are uncommon, with the majority of reported cases originating from the gallbladder or the intrahepatic bile ducts. [ 2 , 7 ] LCNEC, a high-grade variant of neuroendocrine carcinoma, is characterized by large cells with abundant cytoplasm, prominent nucleoli, and high mitotic activity. The rarity of LCNEC in the extrahepatic bile duct poses significant diagnostic and therapeutic challenges for clinicians.

Abstract:Large cell neuroendocrine carcinoma (LCNEC) of lung is a rare neuroendocrine carcinoma subtype with difficulty in early diagnosis and poor prognosis which is treated with standard strategies of small cell lung cancer and non-small cell lung cancer. In recent years, the precise types of LCNEC and its response to therapy have been identified by next-generation sequencing. Some researches have also found the correlation between different subtypes of LCNEC and the efficacy of chemotherapy regimens. However, there is no consensual agreement of its therapy. Recently, immune checkpoint inhibitors (ICIs) has provided a new option for LCNEC patients based on some retrospective research data and case reports. In this review, we aimed to summarize the epidemiological characteristics, standard therapy, the advances of molecular subtypes and clinical applications of ICIs of LCNEC, so as to provide optimal systemic clinical decision-making for LCNEC patients. .
