Abstract:Breast cancer is the most common cancer affecting women in Ethiopia with increasing burden, and chemotherapy treatment produces a detrimental effect on individual wellbeing. Since last few years quality of life has been the primary goal of cancer treatment, yet little research has been conducted on quality of life of breast cancer patients under chemotherapy.To determine the quality of life and associated factors among patients with breast cancer under chemotherapy at Tikur Anbessa specialized hospital, Addis Ababa, Ethiopia.Institution based cross-sectional study was conducted on 404 patients with breast cancer, who took at least one cycle of chemotherapy treatment using face to face interview at oncology unit of Tikur Anbessa specialized hospital day care center from February to April 2018. The validated Amharic version of European organization for research and treatment of cancer core 30 (EORTC QLQ-C30) and quality of life questionnaire specific to breast (QLQ-BR23) was used to measure health related quality of life. Both descriptive and inferential statistics were used. For the purpose of interpretation quality of life score was dichotomized in to two using the calculated mean score, which is 53 as a cutoff point, then, bi-variable and multivariable logistic regression was used to describe association between dependent and independent variables. Hence, patients who score above 53 for quality of life were considered to have good quality of life.Of the total sample, overall response rate was 99.77%. The average quality of life score of patients with breast cancer under chemotherapy treatment was 52.98 (SD = 25.61). Majority of patients had scored poor in emotional functioning, sexual functioning, and financial difficulties. Educational status of college and above, being divorced, higher household income, higher scores of physical and social functioning were associated with significantly improved (better) quality of life. Lower scores of fatigue, insomnia, financial difficulties and systemic therapy side effects all were associated with better scores of quality of life of breast cancer patients. Whereas, patients receiving < = 2 cycles of chemotherapy had significantly lower scores of quality of life.Quality of life of breast cancer patients under chemotherapy treatment is poor in comparison with the reference data and international findings. Therefore, quality of life assessment should be incorporated in patient's treatment protocol. And financial aids may significantly improve the quality of life of breast cancer patients under chemotherapy treatment.

Abstract:Background: Cancer is a major public health problem around the world. Cancer by itself and its treatment modalities affect the quality of life (QoL) of patients with it. However, there were a paucity of studies about the QoL of patients receiving chemotherapeutic treatment in Ethiopia. This study was aimed at addressing such a gap. Accordingly, we investigated QoL and associated factors among cancer patients receiving chemotherapy at Dessie Comprehensive Specialized Hospital (DCSH), North East Ethiopia, in 2023. Methods: We employed a cross-sectional study from April 1 to May 30, 2023. The data was collected using the European Organization for Research and Treatment of Cancer Core QoL Questionnaire, version 3.0 (EORTC QLQ-C30). The data was entered and cleaned using EpiData version 4.6 and exported to Statistical Package for Social Sciences (SPSS) version 27 for analysis. The association between the dependent and independent variables was determined using Odds Ratios (ORs) at a p value < 0.05 with a 95% CI. Results: Data was collected from 394 patients. Their mean summary QoL score was 36.3 ± 9.0. About 39.3% demonstrated a good QoL summary score, whereas 60.7% were impacted by symptoms. A good functional QoL score was observed in 42.6% of the participants. About 54.8% and 31.7% reported good overall health status and good overall QoL, respectively. The most impacted functional domain was social functioning, affecting 64.5% of participants. The most common symptom was diarrhea, affecting 65.5% of the participants. Secondary school education level (Adjusted Odds Ratio-AOR = 3.16, 95% CI: 1.14-8.81), diploma and above education level (AOR = 4.90, 95% CI: 1.29-18.62), and urban residency (AOR = 1.74, 95% CI: 1.07-2.82) had a significant positive association with QoL, while being a civil servant (AOR = 0.13, 95% CI: 0.04-0.49), having stage III cancer (AOR = 0.14, 95% CI: 0.05-0.39), and stage IV cancer (AOR = 0.16, 95% 0.06-0.44) had a significant negative association with it. Conclusion: The QoL for cancer patients undergoing chemotherapy was significantly low and associated with their level of education, occupational status, area of residence, and stage of cancer. Incorporating psychosocial support is thus crucial in their treatment plans.

Abstract:Abstract Background Cervical cancer is the second most prevalent and the leading cause of cancer related deaths among Ethiopian women; and about three fourth are diagnosed at advanced stages. Cervical cancer can affect the health-related quality of life (HRQOL) in multiple ways. The main aim of this study was to describe the HRQOL of cervical cancer patients and the predictive factors using validated tools. Methods Institution based cross-sectional study was conducted among 264 cervical cancer patients using the validated Amharic version of European Organization for Research and Treatment of Cancer (EORTC) modules; QLQ-C30 and QLQ CX24. Descriptive statistics were used to summarize the raw data. One way ANOVA was used to determine the significance of mean differences between the dependent and independent variables. Binary and multivariable regression analysis were used to measure the association between Global Health Status and independent factors. The level of significance was set at p -value < 0.05. Results On EORTC QLQ-C30 scales, the mean Global Health Status (GHS) was 42.57 ± 23.31. The least and highest affected functions were physical and social, mean (SD) = 76.39 ± 23.24 and 50.40 ± 32.19, respectively. The financial difficulty was the most affected among the symptom scales, 57.83 ± 35.34. Only physical function and financial difficulty have shown an independent association with GHS, (AOR = 0.21, 95% CI = 0.05–0.84), (AOR = 0.21 95% CI = 0.07–0.59), respectively. Illiterate, can read and write, were among the predictor factors that showed an independent association with the Global Health Status. Among the EORTC QLQ-CX24 symptom scales, the highest affected score was for sexual worry, mean (SD) = 51.81 + 32.197. Conclusions In an effort to improve the Global Health Status of cervical cancer patients in Ethiopia; physical function and financial difficulty should be the priority areas. The Illiterate and those who lack formal education need due attention in order to improve the health-related quality-of-life.

Abstract:Background: This study aimed to identify the factors that influence Breast Cancer (BC) women's quality of life (QoL) based on the International Classification of Functioning, Disability and Health (ICF) framework. Method: A cross-sectional study was conducted among 188 women with BC. The dependent variable, QoL, was measured using the Quality of Life Index (QLI-c). The independent variables were measured using the following Arabic-validated questionnaires: Pittsburgh Sleep Quality Index (PSQI), Female Sexual Function Index (FSFI), Modified Fatigue Impact Scale (MFIS), Hospital Anxiety and Depression Scale (HADS), and the International Physical Activity Questionnaire (IPAQ). Results: There was a significant positive correlation between monthly income ( r = 0.17, p = 0.016) and QoL, and significant negative correlation between the stage of disease ( r = −0.221, p = 0.002) and duration of first diagnosis ( r = −0.280, p = 0.004) with QoL. Poor sleep quality, sexual dysfunction, fatigue, depression, and anxiety had significant negative correlations with QoL ( p < 0.01). Multiple regression analysis revealed that among the various factors that might affect QoL, sexual dysfunction, poor sleep quality, depression, and anxiety were significant predictors of QoL ( p ≤ 0.05). Conclusion: The ICF provided an excellent framework to explore the factors influencing QoL among women with BC. This study has given evidence for the relationship of demographic, clinical, and body functional factors with QoL among women with BC. Interestingly, sexual dysfunction, poor sleep quality, depression, and anxiety factors are predictors of QoL. Awareness of these factors that predict QoL will guide healthcare professionals to improve the health and QoL of BC women.

Abstract:Background Due to earlier detection and improved treatment and examinations, there has been a rise in the survival rate of patients with breast cancer (BC). It is imperative to examine the health-related quality of life (QoL) of these patients, as it can aid healthcare professionals and authorities in comprehending the variables that influence quality of life. Unfortunately, there is a dearth of information on the subject of women in Nineveh. In Nineveh, a study sought to delve into the quality of life experienced by women undergoing treatment or on follow-up for breast cancer at Mosul Oncology and Nuclear Medicine Hospital. Specifically, researchers wanted to see how age and treatment impacted the QoL of these women. Methods A cross-sectional study was conducted on 212 women with BC. Clinico-pathological-, social-, disease-, and treatment-related characteristics were reviewed. The Functional Assessment of Cancer Therapy-Breast (FACT-B) questionnaire was used for assessing QoL in this study. Results The overall FACT-B score was 75.7 (SD=19.99). Around 36.3% (n=77) of women with BC suffered from a lack of energy, and 39% (n=83) could not meet the needs of their families. The mean score of emotional well-being was the lowest among the FACT-B subscales. Patients aged 60 years and older had significantly worse QoL than younger patients (less than 60 years), and patients receiving chemotherapy had poor QoL. During the chemotherapy period, 51.4% (n=77) of patients were bothered by the side effects of treatment; 43.8% (n=65) suffered from pain; 35.3% (n=53) had nausea; and 39.1% (n=58) felt ill. Conclusions Patients with breast carcinoma who are older tend to experience a lower quality of life, according to the findings of a recent study. Interestingly, those who undergo systemic chemotherapy have a worse QoL than their counterparts who complete their chemotherapy cycles. As a result, healthcare providers must offer targeted interventions to improve quality of life, particularly for those who fall into the older age group and those receiving chemotherapy.

Abstract:Background: Breast cancer is the most common cancer in women worldwide. As the outcome of any cancer treatment is not only dependent on disease-free interval but also on the quality of life (QOL) of the survivor. Hence, this study was conducted to evaluate QOL of breast cancer patients and sociodemographic factors affecting QOL of patients. Subjects and Methods: A cross-sectional study was conducted among 50 breast cancer patients and 50 controls, which fulfilled the selection criteria. The sociodemographic profile and QOL using the World Health Organization QOL-BREF were analyzed using the descriptive statistical methods. Results: All the QOL indicators were statistically significantly ( P = 0.0001) lower among breast cancer patients. QOL indicators were statistically significantly ( P < 0.01) lower in young age. Not much difference was observed according to religion and these were significantly ( P < 0.05) higher among the homemakers. There was no significant difference in all the QOL indicators according to the marital status of the cases. However, these were significantly ( P < 0.05) higher among rural cases compared with urban one. The QOL indicators were statistically significantly ( P < 0.05) higher among the cases living in joint families than nuclear. No difference was observed among different socioeconomic status. Conclusion: QOL was significantly lower among the breast cancer patients. Age, occupational status, type of family, and locality were the significant factors that affect the QOL of breast cancer patients. Hence, psychological and social support can play an important role in improving QOL of cancer patient.
Quality of Life Outcomes and Determining Factors in Breast Cancer Patients Reporting to a Tertiary Care Centre in the Sub-Himalayan Region of India

Abstract:Purpose: Breast cancer is the most common malignancy among women worldwide. This study was conducted to determine the quality of life (QOL) outcomes among breast cancer patients in the sub-Himalayan region and, secondly, to identify factors affecting them. Method: The European Organization for Research and Treatment of Cancer (EORTC) QLQ-C30 and BR-45 questionnaires in English and Hindi translations were used. The BR-45 Hindi translation was obtained using the forward-backward translation method. To check internal consistency and validity, Cronbach's alpha was employed. EORTC scoring manuals were used to score the questionnaires. The analysis of variance test was used to determine the impact of different treatment and sociodemographic factors on QOL domains. Results: The English and Hindi translations had Cronbach's alpha values of 0.949 and 0.950, respectively, suggesting that the data gathered were reliable. The mean score for global health status was 64.4 ± 29.7, the functional scale (FS) of QLQ_C30 was 76.9 ± 21.5, the FS of BR45 was 64.6 ± 24.1, the symptom scale (SS) of QLQ_C30 was 20.3 ± 19.2, and the SS of BR45 was 22.5 ± 19.1. Factors adversely affecting global health status included younger age, pre/perimenopausal status, and ongoing chemotherapy. Functional scales were significantly affected by marital status and earlier stages of the disease. Symptom scales were adversely affected by ongoing chemotherapy, an earlier stage of the disease, and a duration of treatment of less than six months. Conclusion: Tailoring treatment to reduce radiotherapy, surgery, and systemic therapy-related side effects may improve QOL. Counselling and social support groups may help patients cope with the burden of family and societal roles.

Abstract:Breast cancer is a global health concern that significantly impacts the quality of life (QOL) of individuals. This study aims to comprehensively examine the interplay between QOL and depression among nonmetastatic breast cancer patients in Lebanon, a region with limited research in this context. A cross-sectional study was conducted at Hammoud Hospital-University Medical Center from January 2018 to January 2023. Data was collected through a self-administered questionnaire distributed as Google Forms via WhatsApp. A total of 193 patients had non-metastatic breast cancer. Out of these, 81 valid responses were obtained. The Patient Health Questionnaire and Quality of Life Scale were used to assess depression and QOL, respectively. A total of 81 patients were included with mean age 54.4 years. Results revealed that 77.8% of patients experienced provisional depression, with 35.8% meeting criteria for major depressive disorder. Financial status and chronic diseases were associated with the likelihood of developing major depressive disorder. The mean QOL score was 81.14, lower than the average for healthy individuals. Educational level and presence of chronic diseases were significant factors influencing QOL. Postsurgical depression prevalence is substantial, underscoring the importance of integrating mental health care. Economic status and comorbidities are influential factors, necessitating targeted interventions. Breast cancer's impact on QOL is profound, falling below that of other chronic conditions. Education empowers coping, while comorbidities impact QOL. Our findings emphasize the multidimensional nature of breast cancer care, advocating for holistic support and addressing emotional well-being.

Abstract:Journal of Oncology Pharmacy Practice, Ahead of Print. Assessing the quality of life (QoL) of breast cancer (BC) patients using a triangulation of tools is crucial for understanding their well-being and tailoring specific interventions to improve their overall experience. The study assessed the QoL of BC patients using a combination of generic and disease-specific validated questionnaires. The study utilized a self-administered questionnaire-based cross-sectional design among BC patients attending the Oncology clinic in a Nigerian teaching hospital. The 23-item EORTC-BR23 questionnaire and the 15-item HRQoL 15D questionnaire were provided to consenting eligible respondents for data collection. Descriptive (e.g., frequency, percentages, mean, median, etc.) and inferential (T-test and one-way ANOVA) statistical analyses were conducted on the cleaned data, with significant p values set at less than 0.05. A total of 60 female BC patients participated in the study. Respondents that were aged 41–50 years and 50–60 years were 20 (33.3%) and 19 (31.7%) respectively. Patients who were diagnosed with BC one year ago before the study were 22 (39.3%) with 51 (85%) reporting no positive family history of BC. Patients who had undergone surgery, radiotherapy, hormonal therapy, and chemotherapy were 52 (86.7%), 27 (45.0%), 14 (24.1%), and 54 (90%) respectively. The patients scored 30.00 ± 4.67% and 72.36 ± 2.93% for future perspective and body image, respectively, in the functional scales of the EORTC-BR23 with a maximum possible score of 100%. On the symptom scale, they scored 47.46 ± 2.52% and 63.40 ± 5.03% for side effects of therapy and being upset about hair loss, respectively. The patients' quality of life utility score in the 15-D tool was 0.79 ± 0.02. With p values less than 0.005, age, time since diagnosis, and cancer stage were influential determinants of patients' QoL. The QoL of the participants based on the HRQoL-15D was determined to be high. For the EORTC-BR23, respondents reported high quality of life for body image and sexual function but low quality of life for both sexual enjoyment and future perspective in terms of the functional scale. The symptom scale of EORTC-BR23 showed high symptoms for only the domain of upset by hair loss resulting in low QoL. Socio-demographic factor that affects the QoL of BC patients were age, number of years since diagnosis and stage of BC.

Abstract:Objective: Patients with cancer often undergo multiple extended treatments that decrease their quality of life. However, the quality of life of women with breast cancer after they undergo treatment remains underexplored in Brazil. Therefore, this study determined sociodemographic, behavioral, and clinical factors related to the post-treatment quality of life of women with breast cancer. Methods: This cross-sectional study involved 101 women diagnosed with breast cancer between 2014 and 2016 and treated at a Brazilian Oncology Reference Service. Data were collected from them using face-to-face surveys. Quality of life was evaluated using the European Organization for the Research and Treatment of Cancer Core Quality of Life questionnaire (EORTC QLQ-C30) and EORTC Breast Cancer-specific Quality of Life questionnaire (EORTC QLQ-BR23). The data collected were analyzed using Student's t-test and Mann-Whitney U test. Results: The median score on the global health, functional, and symptom scales of the EORTC QLQ-C30 was 75.00 (Interquartile range=33.33), 75.99 (Standard deviation [SD]=19.26), and 19.67 (SD=16.91), respectively. The mean score on the functional and symptom scales of the EORTC QLQ-BR23 was 61.89 (SD=17.21) and 20.12 (SD=16.94), respectively. Furthermore, higher post-treatment quality of life was found to be associated with being aged 50 or more, being Black, having eight or more years of education, having a partner, having a paying job, receiving treatment from the private healthcare system, having a higher income, living in the municipality where healthcare services are availed, engaging in physical activity, not smoking, being more religious, having more social support, not being overweight, having no comorbidities, and undergoing lumpectomy. Conclusion: Sociodemographic, behavioral, and clinical factors significantly impact the quality of life of women who undergo breast cancer treatment. Implementing interventions that improve health and reducing inequalities in the access to healthcare services can improve the quality of life of these patients. [Partial surgery in cancer of the upper respiratory and digestive tracts].

Abstract:Partial or functional surgery for cancers of the upper aerodigestive tract essentially concerns the larynx and the pharyngolaryngeal area. The aim of this surgery is to preserve voice and deglutition. Therefore fundamental anatomical structures such as the cricoid cartilage or at least one arytenoid cartilage must be spared, pointing the limitations of the techniques. Based on these principles, many authors have divided partial surgical procedures ranging from the simplest (cordectomy) to reconstruction on near-total laryngectomy. Cancers of the hypopharynx can benefit from similar techniques always founded on reconstruction by muscle and aponeurosis or by a free flap. Carcinologic results after partial surgery for cancer of upper aerodigestive tract are better than those obtained after total laryngectomy. This fact could be explained by the nature of these cancers that are limited. The carcinologic and functional success of this surgery needs a precise clinical indication and a perfect approach of the various surgical techniques.

Abstract:Introduction: Breast cancer is a type of cancer with the highest incidence among women worldwide. It has an impact on the patient’s physical, psychological, social, and functional health. Breast cancer patients have a greater burden of symptoms, which can lead to decreased quality of life. This study had the objective to describe breast cancer patients’ quality of life in a hospital.Methods: A descriptive observational study design was used. The research population was breast cancer patients in a public hospital. The sample in this study was based on the inclusion and exclusion criteria, with a total of 110 respondents obtained through the proportional sampling technique. The data were analyzed using descriptive analysis. Data were collected by referencing demographic data and distributing a quality-of-life questionnaire (FACT-B). Results: Regarding the quality-of-life variable, findings show that 32 patients had very bad physical well-being (29.1%). Most of the social well-being relations were good, with 40 respondents in this category (36.4%). The majority were in the bad category in terms of functional well-being, with 52 respondents being identified (47.3%). Emotional well-being was considered good for 66 respondents (60.0%). In terms of additional concerns, as many as 60 respondents were in the bad category (54.2%).Conclusions: Chronic conditions can negatively impact patients’ physical health and quality of life. Low quality of life is related to the burden of symptoms experienced by patients, such as anxiety, depression, and fatigue. This suggests that social support can strengthen effective coping and improve patients’ quality of life.Keywords: breast cancer; breast carcinoma; health-related quality of life; quality of life; patients

Abstract:Background: Chemotherapy-related toxicity affects the quality of life (QOL) of patients with cancer. Measuring the QOL in breast cancer (BC) patients has been the focus of clinical practices and research in recent decades. This study aimed to assess the impact of BC medications on QOL of Sudanese patients with BC. Methods: A cross-sectional study was conducted in Khartoum Oncology Hospital, Sudan, from November 2020 to March 2021. All patients diagnosed with BC were included in the study. QOL was assessed using the European Organization for Research and Treatment of Cancer quality of life (EORTC QLQ-C-30) and breast cancer supplementary module (QLQ-BR23). ANOVA, independent t-test and logistic regression analysis were used to assess the association between variables. Results: Two hundred patients were enrolled in the study, with a mean age of 50 ± 11.7 years. 52.5% of the patients were on a conventional therapy whereas 40.5% and 7% received hormonal and HER2-targeted therapies, respectively. In QLQ-C30 scale, the global health-QOL status score was (53.2 ± 1.9), with 54.0% of patients having poor global health-QOL status. In the functional scale, the cognitive functioning was the highest score (80.7 ± 1.8). In QLQ-C30 symptom scale, the most distressing issue was financial difficulties (63.7 ± 2.9). In QLQ-BR23 scale, body image scored the worse functioning (47.7 ± 2.7), with 54.5% of patients having poor QOL. In QLQ-BR23 symptoms scale, "being upset by hair loss" was the highest disturbing symptom (62.1 ± 3.3), with 68.6% of patients having poor QOL. Global health status (P = 0.000), social (P = 0.000), emotional (P = 0.002) and role functioning (P = 0.000) were significantly higher in patients taking HER2-targeted or hormonal therapy compared to conventional therapy. The level of symptomatology was significantly low in patients taking HER2-targeted therapy or hormonal therapy (P = 0.000) than those on conventional therapy. Hormonal (OR = 3.7, p = 0.01) and HER2-targeted therapies (OR = 10.2, p = 0.04 ) were positive predictors of QOL. Conclusion: BC survivors in Sudan had a low QOL/global health status. Hormonal and HER2-targeted therapies were predictors of good QOL.

Abstract:Background Breast cancer (BC) is the most common cancer among women worldwide. We conducted a systematic review and meta-analysis to cover the existing research gap and contribute to existing knowledge to provide both researchers and clinicians with a better profile on the topic and consequently help improve the quality of life (QoL) of patients with BC. Methods A comprehensive review of original articles published in English from January 2000 to October 2021 from databases including Embase, Scopus, PubMed and Web of Science was conducted based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement. Results Based on the meta-regression which examined a total of 9012 patients with BC, the QoL score calculated by EORTC QLQ-C30 was 64.72 (95% CI 59.24 to 70.20), while the score obtained from FACT-B was 84.39 (95% CI 64.24 to 104.54) and the scores from QLQ-BR23 and SF-36 were 66.33 (95% CI 62.76 to 69.90) and 57.23 (95% CI 47.65 to 66.82), respectively. A meta-analysis affirmed a significant direct relationship between the QoL score of patients with BC and their age (p=0.03). The results also revealed that the QoL scores of patients who had completed treatment were higher than those who were currently under treatment. Conclusion The present systematic review identified several factors that affect the QoL of women with BC worldwide and provided several implications for developing policy interventions to effectively improve the QoL of women with BC. In this way, clinicians can sufficiently give advice to their patients with the purpose of improving their QoL. PROSPERO registration number CRD42022309791.

Abstract:Background: Breast cancer in India it varies from as low as 5 per 100,000 female populations per year in rural areas to 30 per 100,000 female populations per year in urban areas. Medical treatments of surgery, chemotherapy etc. for cancer usually precipitates with serious adverse effects affecting overall quality of life in physical, psychological and social aspects. Objective of the Study: To evaluate health related quality of life in patients who had been followed up after the breast cancer surgery for chemotherapy at our hospital. Methods: Quality of life was documented using European Organization of Research and Treatment for Cancer Quality of life Questionnaire-Cancer 30 (EORTC QOL-C30) and European Organization of Research and Treatment for Cancer Quality of life Questionnaire-Breast 23 (EORTC QOL-BR23). Results: After analysis of EORTC QOL-C30 questionnaire, participants had maximum affection of cognitive, social and physical functions and reported high levels of pain, fatigue and insomnia. Domains of EORTC QOL-BR23, the such as worry about self body image, sexual dysfunction, upset by hair loss, systemic therapy side effects, breast and arm symptom were reported by maximum participants. Various domains of quality of life were maximally affected in patients of breast cancer surgery undergoing chemotherapy. Health care professionals in the field of Oncology should address the above mentioned domains of quality of life in Indian patients with breast cancer surgery undergoing chemotherapy. Conclusion: Comprehensive cancer management plan should include psychological counseling, emotional support, physiotherapy care for pain and other musculoskeletal problems etc. along with medical treatment of patients with breast cancer.

Abstract:Abstract Purpose This systematic review aims to summarize factors that influence the quality of life (QOL) of advanced cancer patients in palliative care (PC) in developing countries. Understanding this context in developing countries milieu is necessary; however, this outcome is rarely reported. Methods Following the PRISMA guidelines, the electronic databases MEDLINE, Embase, CINAHL, and Web of Science were systematically searched using the search terms: QOL, cancer, PC, and names of all developing countries. Studies with less than ten subjects, qualitative or pilot studies, reviews, conference abstracts, and that reported validation of QOL questionnaires were excluded. Results Fifty-five studies from 15 developing countries in the African ( n = 5), Latin America and the Caribbean ( n = 10), and Asian ( n = 40) region were included in the narrative synthesis. 65.4% were cross-sectional, 27.3% were cohort studies, 7.3% were RCTs or quasi-experimental studies. Around 30 QOL factors were studied with 20 different types of QOL instruments. Advanced cancer patients who were older, married/ever married, participated in additional care within PC, used complementary and alternative medicine (CAM), and practiced spirituality/religiosity showed higher QOL score. Low educational level and high depression were associated with a lower QOL. Conclusion Various factors affect QOL among cancer patients in PC. Patients valued the use of CAMs; however, the quality and safety aspects should be properly addressed. Important factors that influenced the QOL score were social and spiritual support. While there is a general need to develop PC strategies further, recognizing patients’ needs should be prioritized in national cancer programs.

Abstract:e24111 Background: Despite developments in breast cancer screening for early diagnosis and treatment, 20-85% of women go on to develop distant metastasis, based on tumor biology, stage at presentation and treatment modalities received. The primary goal of treating metastatic breast cancer (MBC) is to improve quality of life (QoL). This study assessed the change in QoL for patients with MBC seeking treatment at Komfo Anokye Teaching Hospital (KATH). Methods: Women with MBC seen at KATH had QoL assessed using the Functional Assessment of Cancer Therapy Breast (FACT-B) questionnaire before, and 3 months after they were supposed to have received systemic treatment - chemotherapy, hormonal, target therapy. Data was analyzed using STATA v17 software. The higher the FACT scores, the better the QoL. Results: Fifty-six women with MBC were interviewed. Their mean age was 51.7 years. Only 27 (48%) of them received some systemic treatment – chemotherapy, hormonal, target therapy – over the study period. Baseline median QoL scores in the wellbeing domains were physical (PWB) = 17.5, social/family (SWB) = 16.0, emotional (EWB) = 12.0, functional (FWB) = 8.0, breast cancer subscale (BCS) = 22.0. Their median total QoL scores were FACT-B = 77.5; FACT-B TOI = 48.5 and FACT-G = 54.5. At follow up, 28 (50%) of participants were alive. About a third (9/28) of them received no systemic treatment. Of the remaining 19; 4, 1 and 14 received one, two and three cycles of systemic treatment respectively. Their median QoL scores were PWB = 20.5, SWB = 16.0, EWB = 16.0, FWB = 15.0, BCS = 22.5, FACT-G = 72.0, FACT-B = 94.5, and FACT-B TOI = 61.0. Fourteen had improved, and 14 had deteriorated QoL. The median minimally important difference (MID) for the improved QoL group was MID FACT-B = -22, MID FACT-B TOI = -10 and MID BCS = -4, and for the deteriorated QoL group was MID FACT-B = 26.9, MID FACT-B TOI = 18 and MID BCS = 5. Being dead at follow up was related to receiving no treatment (AOR = 6.97, CI: 1.91 – 25.47, p-value = 0.003) and having one metastatic site at baseline (AOR = 5.23, CI:1.43 – 19.12, p-value = 0.01). Conclusions: Mortality among women with MBC was high (50%), especially among those who did not receive any systemic treatment or had one metastatic site. Of the survivors, equal proportions had significantly improved and significantly deteriorated QoL scores albeit varying treatment courses received. Further studies are needed to explore how systemic treatment or not impact QoL changes and survival outcomes of MBC patients in KATH.

Abstract:Background Significant advances in the diagnosis and treatment of breast cancer have led to an increase in patient survival. However, the treatment of breast cancer often involves a multimodal approach with surgeries and adjuvant therapies, which still have adverse effects that can affect several domains of quality of life (QoL) in the short and long term. Understanding the effects of breast cancer treatment on the QoL of these women is essential for the development of therapeutic interventions that address the needs of this population. Objective To assess the impact of surgical treatment on the QoL of women with breast cancer and the factors associated with a greater impact on QoL. Methods This is an observational study with a quantitative approach, carried out in Fortaleza/CE. Women between 18 and 80 years old, between 3 and 12 months after surgery for breast cancer and under outpatient follow-up, from May 2022 to March 2023 were included. Those with cognitive impairment assessed by the Mini-Mental State Examination (MMSE) were excluded. Sociodemographic and clinical data were collected, and the Functional Assessment of Cancer Therapy-Breast questionnaire (FACT-B+4) was applied through interviews to assess QoL. The Fact-B+4 score ranges from 0 to 164, in which the higher the score, the better the quality of life. The score is also evaluated in the following domains: Physical Well-being (PWB), Social/Family Well-being (SWB), Emotional Well-being (EWB), Functional Well-being (FWB), Breast Cancer Subscale (BCS) and Arm Subscale (ARM). For analysis of associations, Student's t test was used, assuming p< 0.05 for significance. Results 29 women with a mean age of 55.9±10.2 years were included. The mean FACT-B+4 total score was 104.3±16.9. The most impacted domains were ARM (16.3±3.6), FWB (17.5±4.2) and EWB (18.2±4.9). There was an association between postmenopausal status and lower scores in the EWB (p<0.01), FWB (p<0.01), PWB (p<0.01) and ARM (p=0.018) domains. Age (<60 years) was associated only with the ARM domain (p=0.03). Adjuvant therapy, through radiotherapy and hormone therapy, was associated with lower FWB scores (p=0.03; p=0.01 respectively), while chemotherapy was associated with lower EWB scores (p=0.01). There was no association between the surgical type and the axillary approach in the sample's QoL scores. Conclusion The FWB, EWB and ARM domains were the most affected after breast cancer treatment, which may be related to impaired self-image, autonomy and performance of activities of daily living. Age, menopausal status, and adjuvant therapy are associated with different impacts on QoL in this population. Implications Considering the multiplicity of factors that impact the QoL of women operated on for breast cancer is extremely important for the development of assistance and support strategies during the oncological treatment process, not only after surgery, but in the long-term during adjuvant therapy.
