Abstract:Abstract We used machine learning methods to investigate if body composition indices predict hypertension. Data from a cohort study was used, and 4663 records were included (2156 were male, 1099 with hypertension, with the age range of 35–70 years old). Body composition analysis was done using bioelectrical impedance analysis (BIA); weight, basal metabolic rate, total and regional fat percentage (FATP), and total and regional fat-free mass (FFM) were measured. We used machine learning methods such as Support Vector Classifier, Decision Tree, Stochastic Gradient Descend Classifier, Logistic Regression, Gaussian Naïve Bayes, K-Nearest Neighbor, Multi-Layer Perceptron, Random Forest, Gradient Boosting, Histogram-based Gradient Boosting, Bagging, Extra Tree, Ada Boost, Voting, and Stacking to classify the investigated cases and find the most relevant features to hypertension. FATP, AFFM, BMR, FFM, TRFFM, AFATP, LFATP, and older age were the top features in hypertension prediction. Arm FFM, basal metabolic rate, total FFM, Trunk FFM, leg FFM, and male gender were inversely associated with hypertension, but total FATP, arm FATP, leg FATP, older age, trunk FATP, and female gender were directly associated with hypertension. AutoMLP, stacking and voting methods had the best performance for hypertension prediction achieving an accuracy rate of 90%, 84% and 83%, respectively. By using machine learning methods, we found that BIA-derived body composition indices predict hypertension with acceptable accuracy.

Abstract:Abstract Type 2 Diabetes Mellitus (T2DM) is a significant public health problem globally. The diagnosis and management of diabetes are critical to reduce the diabetes complications including cardiovascular disease and cancer. This study was designed to assess the potential association between T2DM and routinely measured hematological parameters. This study was a subsample of 9000 adults aged 35–65 years recruited as part of Mashhad stroke and heart atherosclerotic disorder (MASHAD) cohort study. Machine learning techniques including logistic regression (LR), decision tree (DT) and bootstrap forest (BF) algorithms were applied to analyze data. All data analyses were performed using SPSS version 22 and SAS JMP Pro version 13 at a significant level of 0.05. Based on the performance indices, the BF model gave high accuracy, precision, specificity, and AUC. Previous studies suggested the positive relationship of triglyceride-glucose (TyG) index with T2DM, so we considered the association of TyG index with hematological factors. We found this association was aligned with their results regarding T2DM, except MCHC. The most effective factors in the BF model were age and WBC (white blood cell). The BF model represented a better performance to predict T2DM. Our model provides valuable information to predict T2DM like age and WBC.

Abstract:Objectives: This study aimed to develop a model to predict fasting blood glucose status using machine learning and data mining, since the early diagnosis and treatment of diabetes can improve outcomes and quality of life.Methods: This crosssectional study analyzed data from 3376 adults over 30 years old at 16 comprehensive health service centers in Tehran, Iran who participated in a diabetes screening program. The dataset was balanced using random sampling and the synthetic minority over-sampling technique (SMOTE). The dataset was split into training set (80%) and test set (20%). Shapley values were calculated to select the most important features. Noise analysis was performed by adding Gaussian noise to the numerical features to evaluate the robustness of feature importance. Five different machine learning algorithms, including CatBoost, random forest, XGBoost, logistic regression, and an artificial neural network, were used to model the dataset. Accuracy, sensitivity, specificity, accuracy, the F1-score, and the area under the curve were used to evaluate the model.Results: Age, waist-to-hip ratio, body mass index, and systolic blood pressure were the most important factors for predicting fasting blood glucose status. Though the models achieved similar predictive ability, the CatBoost model performed slightly better overall with 0.737 area under the curve (AUC).Conclusions: A gradient boosted decision tree model accurately identified the most important risk factors related to diabetes. Age, waist-to-hip ratio, body mass index, and systolic blood pressure were the most important risk factors for diabetes, respectively. This model can support planning for diabetes management and prevention.

Abstract:Background Previous conventional models for the prediction of diabetes could be updated by incorporating the increasing amount of health data available and new risk prediction methodology. Objective We aimed to develop a substantially improved diabetes risk prediction model using sophisticated machine-learning algorithms based on a large retrospective population cohort of over 230,000 people who were enrolled in the study during 2006-2017. Methods We collected demographic, medical, behavioral, and incidence data for type 2 diabetes mellitus (T2DM) in over 236,684 diabetes-free participants recruited from the 45 and Up Study. We predicted and compared the risk of diabetes onset in these participants at 3, 5, 7, and 10 years based on three machine-learning approaches and the conventional regression model. Results Overall, 6.05% (14,313/236,684) of the participants developed T2DM during an average 8.8-year follow-up period. The 10-year diabetes incidence in men was 8.30% (8.08%-8.49%), which was significantly higher (odds ratio 1.37, 95% CI 1.32-1.41) than that in women at 6.20% (6.00%-6.40%). The incidence of T2DM was doubled in individuals with obesity (men: 17.78% [17.05%-18.43%]; women: 14.59% [13.99%-15.17%]) compared with that of nonobese individuals. The gradient boosting machine model showed the best performance among the four models (area under the curve of 79% in 3-year prediction and 75% in 10-year prediction). All machine-learning models predicted BMI as the most significant factor contributing to diabetes onset, which explained 12%-50% of the variance in the prediction of diabetes. The model predicted that if BMI in obese and overweight participants could be hypothetically reduced to a healthy range, the 10-year probability of diabetes onset would be significantly reduced from 8.3% to 2.8% (P<.001). Conclusions A one-time self-reported survey can accurately predict the risk of diabetes using a machine-learning approach. Achieving a healthy BMI can significantly reduce the risk of developing T2DM.

Abstract:Rising diabetes rates have led to increased health-care costs and health complications. An estimated half of diabetes cases remain undiagnosed. Early and accurate diagnosis is crucial to mitigate disease progression and associated risks. This study addresses the challenge of predicting diabetes prevalence in Canadian adults by employing machine learning (ML) techniques to primary care data. We leveraged the Canadian Primary Care Sentinel Surveillance Network (CPCSSN), Canada’s premier multi-disease electronic medical record surveillance system, and developed and tuned seven ML classification models to predict the likelihood of diabetes. The models were tested and validated, focusing on clinical patient characteristics influential in predicting diabetes. We found XGBoost performed best out of all the models, with an AUC of 92%. The most important features contributing to model prediction were HbA1c, LDL, and hypertension medication. Our research aims to aid healthcare professionals in early diagnosis and to identify key characteristics for targeted interventions. This study contributes to an understanding of how ML can enhance public health planning and reduce healthcare system burdens.

Abstract:In this study, we delve into the intricate relationships between diabetes and a range of health indicators, with a particular focus on the newly added variable of income. Utilizing data from the 2015 Behavioral Risk Factor Surveillance System (BRFSS), we analyze the impact of various factors such as blood pressure, cholesterol, BMI, smoking habits, and more on the prevalence of diabetes. Our comprehensive analysis not only investigates each factor in isolation but also explores their interdependencies and collective influence on diabetes. A novel aspect of our research is the examination of income as a determinant of diabetes risk, which to the best of our knowledge has been relatively underexplored in previous studies. We employ statistical and machine learning techniques to unravel the complex interplay between socio-economic status and diabetes, providing new insights into how financial well-being influences health outcomes. Our research reveals a discernible trend where lower income brackets are associated with a higher incidence of diabetes. In analyzing a blend of 33 variables, including health factors and lifestyle choices, we identified that features such as high blood pressure, high cholesterol, cholesterol checks, income, and Body Mass Index (BMI) are of considerable significance. These elements stand out among the myriad of factors examined, suggesting that they play a pivotal role in the prevalence and management of diabetes.

Abstract:In modern times, obesity has become a significant threat all over the world. Obesity means an unnatural or excessive amount of fat that is present in our bodies. People are constantly moving towards an unhealthy lifestyle, eating excessive junk food, late-night sleep, spend a long time sitting down. Especially, adolescents are being affected because of their unconscious attitudes. It is a medical problem known as a very complex disease. It promotes the spread of complex illnesses, stroke, heart disease, liver cancer. Consequently, as an aware multitude of Bangladesh, we have to move forward to prevent this risk of obesity. The purpose of this paper is to move towards a machine-learning-based pathway for predicting the risk of obesity using machine-learning algorithms. The great thing about this paper is that people will know the risk of obesity and the reasons behind their obesity. We collect more than 1100 data from many varieties of people of different ages and collect information from both are suffering obesity and non-obesity. For this research, we apply nine prominent machine learning algorithms. We used the algorithm of k-nearest neighbor (k-NN), random forest, logistic regression, multilayer perceptron (MLP), support vector machine (SVM), naïve Bayes, adaptive boosting (ADA boosting), decision tree, and gradient boosting classifier, and we have measured the performance of each of these classifications in terms of some prominent performance metrics. From the experimental results, we determine the obesity of high, medium, and low. The Logistic Regression Algorithm achieves the highest accuracy of 97.09% as compared to the other classifiers. In addition, the gradient boosting algorithm gave the poorest accuracy of 64.08% as well as the lowest metric values.

Abstract:Abstract: Diabetes is a chronic condition that could lead to a catastrophe in the world's healthcare system. 537 million people worldwide have diabetes, according to the International Diabetes Federation. By 2045, this number is anticipated to reach 783 million. A rise in blood glucose levels can result in diabetes. Numerous symptoms, which includes frequent urination, increased thirst, and increased appetite, are brought on by this raised blood glucose level. It is a significant contributor to heart failure, stroke, kidney failure, blindness, and amputations. The objective of the given study mainly is to develop the single collective system that combines result of many different types machine learning techniques, including Logistic Regression, Linear Regression, Support Vector Machine, and Random Forest, to more accurately predict diabetes in a patient. It collects the patient's records based on their pregnancy, blood sugar levels, blood pressure, insulin levels, body mass index, and many other factors. Each of the strategies will be used to determine the model's correctness, however, we found that the Support vector machine method had the highest accuracy (77%). The diabetes forecast is then made using the model with the highest accuracy.

Abstract:BackgroundDiabetes mellitus is a chronic disease that impacts an increasing percentage of people each year. Among its comorbidities, diabetics are two to four times more likely to develop cardiovascular diseases. While HbA1c remains the primary diagnostic for diabetics, its ability to predict long-term, health outcomes across diverse demographics, ethnic groups, and at a personalized level are limited. The purpose of this study was to provide a model for precision medicine through the implementation of machine-learning algorithms using multiple cardiac biomarkers as a means for predicting diabetes mellitus development.MethodsRight atrial appendages from 50 patients, 30 non-diabetic and 20 type 2 diabetic, were procured from the WVU Ruby Memorial Hospital. Machine-learning was applied to physiological, biochemical, and sequencing data for each patient. Supervised learning implementing SHapley Additive exPlanations (SHAP) allowed binary (no diabetes or type 2 diabetes) and multiple classification (no diabetes, prediabetes, and type 2 diabetes) of the patient cohort with and without the inclusion of HbA1c levels. Findings were validated through Logistic Regression (LR), Linear Discriminant Analysis (LDA), Gaussian Naïve Bayes (NB), Support Vector Machine (SVM), and Classification and Regression Tree (CART) models with tenfold cross validation.ResultsTotal nuclear methylation and hydroxymethylation were highly correlated to diabetic status, with nuclear methylation and mitochondrial electron transport chain (ETC) activities achieving superior testing accuracies in the predictive model (~ 84% testing, binary). Mitochondrial DNA SNPs found in the D-Loop region (SNP-73G, -16126C, and -16362C) were highly associated with diabetes mellitus. The CpG island of transcription factor A, mitochondrial (TFAM) revealed CpG24 (chr10:58385262, P = 0.003) and CpG29 (chr10:58385324, P = 0.001) as markers correlating with diabetic progression. When combining the most predictive factors from each set, total nuclear methylation and CpG24 methylation were the best diagnostic measures in both binary and multiple classification sets.ConclusionsUsing machine-learning, we were able to identify novel as well as the most relevant biomarkers associated with type 2 diabetes mellitus by integrating physiological, biochemical, and sequencing datasets. Ultimately, this approach may be used as a guideline for future investigations into disease pathogenesis and novel biomarker discovery.

Abstract:Machine learning is becoming a popular and important approach in the field of medical research. In this study, we investigate the relative performance of various machine learning methods such as Decision Tree, Naïve Bayes, Logistic Regression, Logistic Model Tree and Random Forests for predicting incident diabetes using medical records of cardiorespiratory fitness. In addition, we apply different techniques to uncover potential predictors of diabetes. This FIT project study used data of 32,555 patients who are free of any known coronary artery disease or heart failure who underwent clinician-referred exercise treadmill stress testing at Henry Ford Health Systems between 1991 and 2009 and had a complete 5-year follow-up. At the completion of the fifth year, 5,099 of those patients have developed diabetes. The dataset contained 62 attributes classified into four categories: demographic characteristics, disease history, medication use history, and stress test vital signs. We developed an Ensembling-based predictive model using 13 attributes that were selected based on their clinical importance, Multiple Linear Regression, and Information Gain Ranking methods. The negative effect of the imbalance class of the constructed model was handled by Synthetic Minority Oversampling Technique (SMOTE). The overall performance of the predictive model classifier was improved by the Ensemble machine learning approach using the Vote method with three Decision Trees (Naïve Bayes Tree, Random Forest, and Logistic Model Tree) and achieved high accuracy of prediction (AUC = 0.92). The study shows the potential of ensembling and SMOTE approaches for predicting incident diabetes using cardiorespiratory fitness data.

Abstract:High blood sugar levels characterize diabetes and is a chronic disease with long-lasting effects on human health. Accurately predicting diabetes occurrence presents challenges due to the limited availability of labeled data and outliers or missing values in diabetes datasets. In diabetes research, machine learning (ML) algorithms are extensively employed to analyze datasets and predict the onset of the disease. In this study, diabetes data from Bangladesh, India, and Germany were examined using various ML models. The experimental results demonstrate that the Bangladesh dataset performs better using boosting ML algorithms such as AdaBoost, CatBoost, Gradient Boost, and XGBoost. These algorithms effectively predict the occurrence of diabetes. Additionally, satisfactory performance was observed with basic models like Random Forests and Decision Trees, as evaluated by performance metrics. Early detection of diabetes plays a crucial role in mitigating associated risk factors and severity. ML algorithms have emerged as valuable tools in diabetes prediction, leveraging the available data to make accurate predictions. The study's findings underscore the potential of boosting ML algorithms, such as AdaBoost, CatBoost, Gradient Boost, and XGBoost, in predicting diabetes based on the Bangladesh dataset. Furthermore, the study acknowledges the acceptable performance of basic models like Random Forests and Decision Trees in evaluating diabetes data. In conclusion, this study contributes to the understanding of diabetes prediction by analyzing datasets from multiple countries. The results highlight the effectiveness of ML algorithms, particularly boosting algorithms, in accurately predicting diabetes occurrence. Abstract:ObjectiveDiabetes is a chronic fatal disease that has affected millions of people all over the globe. Type 2 Diabetes Mellitus (T2DM) accounts for 90% of the affected population among all types of diabetes. Millions of T2DM patients remain undiagnosed due to lack of awareness and under resourced healthcare system. So, there is a dire need for a diagnostic and prognostic tool that shall help the healthcare providers, clinicians and practitioners with early prediction and hence can recommend the lifestyle changes required to stop the progression of diabetes. The main objective of this research is to develop a framework based on machine learning techniques using only lifestyle indicators for prediction of T2DM disease. Moreover, prediction model can be used without visiting clinical labs and hospital readmissions.MethodA proposed framework is presented and implemented based on machine learning paradigms using lifestyle indicators for better prediction of T2DM disease. The current research has involved different experts like Diabetologists, Endocrinologists, Dieticians, Nutritionists, etc. for selecting the contributing 1552 instances and 11 attributes lifestyle biological features to promote health and manage complications towards T2DM disease. The dataset has been collected through survey and google forms from different geographical regions.ResultsSeven machine learning classifiers were employed namely K-Nearest Neighbour (KNN), Linear Regression (LR), Support Vector Machine (SVM), Naive Bayes (NB), Decision Tree (DT), Random Forest (RF) and Gradient Boosting (GB). Gradient Boosting classifier outperformed best with an accuracy rate of 97.24% for training and 96.90% for testing separately followed by RF, DT, NB, SVM, LR, and KNN as 95.36%, 92.52%, 90.72%, 90.20%, 90.20% and 77.06% respectively. However, in terms of precision, RF achieved high performance (0.980%) and KNN performed the lowest (0.793%). As far as recall is being concerned, GB achieved the highest rate of 0.975% and KNN showed the worst rate of 0.774%. Also, GB is top performed in terms of f1-score. According to the ROCs, GB and NB had a better area under the curve compared to the others.ConclusionThe research developed a realistic health management system for T2DM disease based on machine learning techniques using only lifestyle data for prediction of T2DM. To extend the current study, these models shall be used for different, large and real-time datasets which share the commonality of data with T2DM disease to establish the efficacy of the proposed system.
Machine learning models for prediction of co-occurrence of diabetes and cardiovascular diseases: a retrospective cohort study

Abstract:Abstract Background Diabetic mellitus (DM) and cardiovascular diseases (CVD) cause significant healthcare burden globally and often co-exists. Current approaches often fail to identify many people with co-occurrence of DM and CVD, leading to delay in healthcare seeking, increased complications and morbidity. In this paper, we aimed to develop and evaluate a two-stage machine learning (ML) model to predict the co-occurrence of DM and CVD. Methods We used the diabetes complications screening research initiative (DiScRi) dataset containing >200 variables from >2000 participants. In the first stage, we used two ML models (logistic regression and Evimp functions) implemented in multivariate adaptive regression splines model to infer the significant common risk factors for DM and CVD and applied the correlation matrix to reduce redundancy. In the second stage, we used classification and regression algorithm to develop our model. We evaluated the prediction models using prediction accuracy, sensitivity and specificity as performance metrics. Results Common risk factors for DM and CVD co-occurrence was family history of the diseases, gender, deep breathing heart rate change, lying to standing blood pressure change, HbA1c, HDL and TC\HDL ratio. The predictive model showed that the participants with HbA1c >6.45 and TC\HDL ratio > 5.5 were at risk of developing both diseases (97.9% probability). In contrast, participants with HbA1c >6.45 and TC\HDL ratio ≤ 5.5 were more likely to have only DM (84.5% probability) and those with HbA1c ≤5.45 and HDL >1.45 were likely to be healthy (82.4%. probability). Further, participants with HbA1c ≤5.45 and HDL <1.45 were at risk of only CVD (100% probability). The predictive accuracy of the ML model to detect co-occurrence of DM and CVD is 94.09%, sensitivity 93.5%, and specificity 95.8%. Conclusions Our ML model can significantly predict with high accuracy the co-occurrence of DM and CVD in people attending a screening program. This might help in early detection of patients with DM and CVD who could benefit from preventive treatment and reduce future healthcare burden.

Abstract:Abstract This article uses linear regression in machine learning and LightGBM algorithm for data mining, and compares the linear regression and LightGBM algorithm’s ability of fast running speed and high accuracy under the same data conditions. The least squares method is used to make error judgments to realize the rapid and accurate prediction and judgment of the probability of diabetes in a large amount of data. At the same time, the importance and correlation of the variables are compared between the variables. Among them, it is found that Body Mass Index (BMI) has the greatest impact on diabetes, and the factors affecting BMI are weight and height. Height and age can be input into system, automatically determining the BMI value, and providing doctors with a basis for judgment.

Abstract:Prediabetes is a critical health condition characterized by elevated blood glucose levels that fall below the threshold for Type 2 diabetes (T2D) diagnosis. Accurate identification of prediabetes is essential to forestall the progression to T2D among at-risk individuals. This study aims to pinpoint the most effective machine learning (ML) model for prediabetes prediction and to elucidate the key biological variables critical for distinguishing individuals with prediabetes. Utilizing data from the Canadian Primary Care Sentinel Surveillance Network (CPCSSN), our analysis included 6,414 participants identified as either nondiabetic or prediabetic. A rigorous selection process led to the identification of ten variables for the study, informed by literature review, data completeness, and the evaluation of collinearity. Our comparative analysis of seven ML models revealed that the Deep Neural Network (DNN), enhanced with early stop regularization, outshined others by achieving a recall rate of 60%. This model’s performance underscores its potential in effectively identifying prediabetic individuals, showcasing the strategic integration of ML in healthcare. While the model reflects a significant advancement in prediabetes prediction, it also opens avenues for further research to refine prediction accuracy, possibly by integrating novel biological markers or exploring alternative modeling techniques. The results of our work represent a pivotal step forward in the early detection of prediabetes, contributing significantly to preventive healthcare measures and the broader fight against the global epidemic of Type 2 diabetes.

Machine Learning-Based Hyperglycemia Prediction: Enhancing Risk Assessment in a Cohort of Undiagnosed Individuals

Abstract:Background: Noncommunicable diseases continue to pose a substantial health challenge globally, with hyperglycemia serving as a prominent indicator of diabetes. Objective: This study employed machine learning algorithms to predict hyperglycemia in a cohort of individuals who were asymptomatic and unraveled crucial predictors contributing to early risk identification. Methods: This dataset included an extensive array of clinical and demographic data obtained from 195 adults who were asymptomatic and residing in a suburban community in Nigeria. The study conducted a thorough comparison of multiple machine learning algorithms to ascertain the most effective model for predicting hyperglycemia. Moreover, we explored feature importance to pinpoint correlates of high blood glucose levels within the cohort. Results: Elevated blood pressure and prehypertension were recorded in 8 (4.1%) and 18 (9.2%) of the 195 participants, respectively. A total of 41 (21%) participants presented with hypertension, of which 34 (83%) were female. However, sex adjustment showed that 34 of 118 (28.8%) female participants and 7 of 77 (9%) male participants had hypertension. Age-based analysis revealed an inverse relationship between normotension and age (r=-0.88; P=.02). Conversely, hypertension increased with age (r=0.53; P=.27), peaking between 50-59 years. Of the 195 participants, isolated systolic hypertension and isolated diastolic hypertension were recorded in 16 (8.2%) and 15 (7.7%) participants, respectively, with female participants recording a higher prevalence of isolated systolic hypertension (11/16, 69%) and male participants reporting a higher prevalence of isolated diastolic hypertension (11/15, 73%). Following class rebalancing, the random forest classifier gave the best performance (accuracy score 0.89; receiver operating characteristic-area under the curve score 0.89; F1-score 0.89) of the 26 model classifiers. The feature selection model identified uric acid and age as important variables associated with hyperglycemia. Conclusions: The random forest classifier identified significant clinical correlates associated with hyperglycemia, offering valuable insights for the early detection of diabetes and informing the design and deployment of therapeutic interventions. However, to achieve a more comprehensive understanding of each feature's contribution to blood glucose levels, modeling additional relevant clinical features in larger datasets could be beneficial.

Abstract:Diabetes Mellitus is among chronic diseases and lots of people are suffering with this disease. It may cause many complications and have a high risk of diseases like heart disease, kidney disease, stroke, eye problem, nerve damage, etc. There is no doubt that this alarming figure needs great attention. With the rapid development of Machine Learning, machine learning has been applied to many aspects of medical health. There are several Machine learning algorithms that are used to perform predictive analysis in various fields. Predictive analysis in healthcare is a challenging task but ultimately can help practitioners make data informed about a patient's health and treatment. In this project, for experiment purposes, we have taken a dataset which is originally from the National Institute of diabetes and digestive and kidney diseases. All patients here are females at least 21 years old of Pima Indian heritage. By studying the dataset, we must find hidden information, hidden patterns to discover knowledge from the data and predict outcomes accordingly. The objective of this project is to diagnostically predict whether the patient has diabetes or not, based on certain diagnostic measurements included in the dataset. We have proposed a diabetes prediction model for better classification of diabetes by applying some popular machine learning algorithms namely, Logistic Regression, Random Forest Algorithm and KNN Algorithm to predict Diabetes.
