Abstract:Background: Social determinants of health (SDOH), have been demonstrated to significantly impact health outcomes in spine patients. There may be interaction between opioid use and these factors in spine surgical patients. We aimed to evaluate the social determinants of health (SDOH) which are associated with perioperative opioid use among lumbar spine patients. Methods: This retrospective cohort study included patients undergoing spine surgery for lumbar degeneration in 2019. Opioid use was determined based on prescription records from the electronic medical records. Preoperative opioid users (OU) were compared with opioid-naïve patients regarding SDOH including demographics like age and race, and clinical data such as activity and tobacco use. Demographics and surgical data, including age, comorbidities, surgical invasiveness, and other variables were also collected from the records. Multivariate logistic regression was used for analysis of these factors. Results: Ninety-eight patients were opioid-naïve and 90 used opioids preoperatively. All OU had ≥3 months of use, had more prior spine surgeries (1.07 vs. 0.44, p<.001) and more comorbidities including diabetes, hypertension, and depression (p=.021, 0.043, 0.017). Patients from lower community median income areas, unemployed, or with lower physical capacity (METS<5) were more likely to use opioids preoperatively. Postoperative opioid use was strongly associated with preoperative opioid use, as well as alcohol use, and lower community median income. At one year postoperatively, OU had higher rates of opioid use [72.2% vs. 15.3%, p<.001]. Conclusions: Unemployment, low physical activity level, and lower community median income were associated with preoperative opioid use and longer-term opioid use postoperatively.

Abstract:INTRODUCTION: Long-term opioid use after elective lumbar spine surgery is a significant concern, given the current opioid epidemic and its impact on public health. Identifying risk factors for long-term opioid use after spine surgery is crucial in developing effective interventions to reduce the incidence of opioid dependence in this patient population. METHODS: A retrospective analysis of patient data was conducted using the Michigan Spine Surgery Improvement Collaborative (MSSIC) registry. Patients who underwent elective lumbar spine surgery between March 2018, and September 2021, were included, resulting in a total of 3599 patients at one year and 966 patients at two years follow-up. Poisson generalized estimating equation models were used for multivariate analyses. RESULTS: A total of 493 (13.7%) and 137 (14.2%) patients were still using opiates at one-year and two-years post-surgery respectively. Discharge morphine milligram equivalents (MMEs), baseline PROMIS PF functional scores, baseline pain scores, age, gender, education level, pre-operative opioid use, and not achieving the minimal clinically important difference (MCID) in PROMIS PF scores at follow-up were significantly associated with long-term opioid use. Specifically, pre-operative opioid use had the strongest association with opioid use at one year (IRR = 2.83, CI: 2.40-3.34, p < 0.001) and two years (IRR = 3.79, CI: 2.53-5.68, p < 0.001). Additionally, not achieving MCID in PROMIS scores at follow-up was strongly associated (IRR = 2.00, CI: 1.72-2.32, p < 0.001 and IRR = 2.18, CI: 1.64-2.91, p < 0.001) with opioid use at both time points (1-year and 2-year respectively). CONCLUSIONS: Pre-operative opioid use and not achieving MCID in PROMIS scores at follow-up were the strongest predictors of long-term opioid use after elective lumbar spine surgery. Identifying these risk factors can help clinicians target interventions to reduce opioid use in at-risk patients and improve postoperative outcomes.

Abstract:BACKGROUND: The risk factors for persistent opioid use after surgical discharge and the association between opioid prescription at discharge and postoperative emergency department visits, readmission, and mortality are unclear.METHODS: This population-based retrospective cohort study involved opioid-naive patients who underwent surgical procedures from January 1, 2000 to November 30, 2020. The data source was Hong Kong Hospital Authority Clinical Management System electronic health record. The primary outcome was the incidence of new persistent opioid use. Other study outcomes included 30-day emergency department visits, 30-day readmission, and 30-day all-cause mortality. Multivariable logistic regression models were used to estimate the association between opioid prescription at discharge and persistent opioid use, emergency department visits, readmission, and all-cause mortality.RESULTS: Over a median follow-up of 1 month with 36 104 person-years, 438 128 patients (opioid prescription: 32 932, no opioid prescription: 405 196) who underwent surgical procedures were analysed, of whom 15 112 (3.45%) had persistent opioid use after discharge. Prescribing opioids on discharge was associated with increased risks of developing persistent opioid use (odds ratio [OR]: 2.30, 95% confidence interval [CI]: 2.19-2.40, P<0.001), 30-day emergency department visits (OR: 1.28, 95% CI: 1.23-1.33, P<0.001), 30-day readmission (OR: 1.17, 95% CI: 1.13-1.20, P<0.001), and 30-day all-cause mortality (OR: 1.68, 95% CI: 1.53-1.86, P<0.001).CONCLUSIONS: In this large cohort of patients undergoing surgery, an opioid prescription on discharge was associated with a higher chance of persistent opioid use and increased risks of postoperative emergency department visits, readmission, and mortality. Minimising opioid prescriptions on discharge could improve perioperative patient outcomes.

Abstract:Although epidural steroid injections are commonly used to treat radicular pain [1, 2], a paucity of data exists with regard to the long-term impact on pain after subsequent spine surgery. We hypothesized that in a proportion of patients, preoperative epidural injections might delay surgery and efficient analgesia, with potential worsening of pain pathology, or could indicate severe disease and thus would be associated with postoperative chronic opioid use. Study approval was obtained by our Institutional Review Board (IRB #2017-0169). We conducted a retrospective study of patients undergoing anterior cervical discectomy and fusion (ACDF) and microdiscectomy captured in the Truven Health MarketScan database (Copyright ©2017 Truven Health Analytics, Inc.) in 2012–2016 by using International Classification of Diseases 9th/10th revision (ICD-9/10) procedure codes (Supplementary Data Appendix 1). The main exposure of interest, preoperative epidural steroid injection, was identified on the basis of Current Procedural Terminology (CPT) codes reported in Supplementary Data Appendix 2 and included visits with a diagnosis code for arm/neck pain (patients undergoing ACDF) or back pain (patients undergoing microdiscectomy). The outcome postoperative chronic opioid use was defined as filling prescriptions for at least 120 pills or filling 10 or more prescriptions for opioids within the period from 90 days to 1 year after surgery [3, 4].

Abstract:Global Spine Journal, Ahead of Print. Study DesignProspective multicenter database post-hoc analysis.ObjectivesOpioids are frequently prescribed for painful spinal conditions to provide pain relief and to allow for functional improvement, both before and after spine surgery. Amidst a current opioid epidemic, it is important for providers to understand the impact of opioid use and its relationship with patient-reported outcomes. The purpose of this study was to evaluate pre-/postoperative opioid consumption surrounding ASD and assess patient-reported pain outcomes in older patients undergoing surgery for spinal deformity.MethodsPatients ≥60 years of age from 12 international centers undergoing spinal fusion of at least 5 levels and a minimum 2-year follow-up were included. Patient-reported outcome scores were collected using the Numeric Rating Scale for back and leg pain (NRS-B; NRS-L) at baseline and at 2 years following surgery. Opioid use, defined based on a specific question on case report forms and question 11 from the SRS-22r questionnaire, was assessed at baseline and at 2-year follow-up.ResultOf the 219 patients who met inclusion criteria, 179 (81.7%) had 2-year data on opioid use. The percentages of patients reporting opioid use at baseline (n = 75, 34.2%) and 2 years after surgery (n = 55, 30.7%) were similar (P = .23). However, at last follow-up 39% of baseline opioid users (Opi) were no longer taking opioids, while 14% of initial non-users (No-Opi) reported opioid use. Regional pre- and postoperative opioid use was 5.8% and 7.7% in the Asian population, 58.3% and 53.1% in the European, and 50.5% and 40.2% in North American patients, respectively. Baseline opioid users reported more preoperative back pain than the No-Opi group (7.0 vs 5.7, P = .001), while NRS-Leg pain scores were comparable (4.8 vs 4, P = .159). Similarly, at last follow-up, patients in the Opi group had greater NRS-B scores than Non-Opi patients (3.2 vs 2.3, P = .012), but no differences in NRS-Leg pain scores (2.2 vs 2.4, P = .632) were observed.ConclusionsIn this study, almost one-third of surgical ASD patients were consuming opioids both pre- and postoperatively world-wide. There were marked international variations, with patients from Asia having a much lower usage rate, suggesting a cultural influence. Despite both opioid users and nonusers benefitting from surgery, preoperative opioid use was strongly associated with significantly more back pain at baseline that persisted at 2-year follow up, as well as persistent postoperative opioid needs.

Prolonged opioid use after single-level lumbar spinal fusion surgery in a Belgian population: a multicentric observational study

Abstract:Purpose: Lumbar spinal fusion surgeries are increasingly being performed in spinal degenerative disease, often accompanied by perioperative opioid prescriptions. The aim of this study is to analyze prolonged postoperative opioid use following a standardized opioid prescription after single-level lumbar spinal fusion surgery in a Belgian population. Methods: This prospective, multicentric observational study included patients undergoing single-level lumbar fusion surgery for degenerative disease. A standardized postoperative opioid protocol (Targinact 2 × 10 mg/5 mg, Paracetamol 4 × 1 g and Ibuprofen 3 × 600 mg) was applied uniformly. Prolonged opioid use was defined as continued opioid use six months after surgery. Patient data were collected using the Back-App®. Results: Among 198 participants, 32.8% continued opioid use six months post-surgery, with 8% utilizing strong opioids. Prolonged opioid use correlated with lower pre-operative back pain. Patients with prolonged opioid use and strong opioid use at six months show less improvement in disability compared to patients without prolonged opioid use. Moreover, patients with prolonged strong opioid use tend to have lesser improvement of the low back pain. The odds for prolonged opioid use decrease with the increase of the improvement in ODI. Conclusion: 1 in 3 patients undergoing single-level lumbar spinal fusion surgery is at risk for prolonged opioid use. The study underscores the importance of tailored pain management strategies, particularly given the rising prevalence of spinal fusion surgeries. The association between pre-operative low back pain, post-operative improvement in functionality (ODI), and prolonged opioid use emphasizes the need for judicious opioid prescribing practices and highlights the role of functional outcomes in treatment goals.

Abstract:Objective: To examine the association of prescription opioid fills over the year prior to surgery with postoperative outcomes. Background: Nearly one third of patients report opioid use in the year preceding surgery, yet an understanding of how opioid exposure influences patient-reported outcomes after surgery remains incomplete. Therefore, this study was designed to test the hypothesis that preoperative opioid exposure may impede recovery in the postoperative period. Methods: This retrospective cohort study used a statewide clinical registry from 70 hospitals linked to opioid fulfillment data from the state’s prescription drug monitoring program to categorize patients’ preoperative opioid exposure as none (naïve), minimal, intermittent, or chronic. Outcomes were patient-reported pain intensity (primary), as well as 30-day clinical and patient-reported outcomes (secondary). Results: Compared to opioid-naïve patients, opioid exposure was associated with higher reported pain scores at 30 days after surgery. Predicted probabilities was higher among the opioid exposed versus naive group for reporting moderate pain (43.5% [95% CI 42.6 – 44.4%] vs 39.3% [95% CI 38.5 – 40.1%]) and severe pain (13.% [95% CI 12.5 – 14.0%] vs 10.0% [95% CI 9.5 – 10.5%]), and increasing probability was associated increased opioid exposure for both outcomes. Clinical outcomes (incidence of ED visits, readmissions, and reoperation within 30-days) and patient-reported outcomes (reported satisfaction, regret, and quality of life) were also worse with increasing preoperative opioid exposure for most outcomes. Conclusions: This study is the first to examine the effect of presurgical opioid exposure on both clinical and non-clinical outcomes in a broad cohort of patients, and shows that exposure is associated with worse postsurgical outcomes. A key question to be addressed is whether and to what extent opioid tapering before surgery mitigates these risks after surgery.

A Predictive Model for Developing Long Term Opioid Use After Neurosurgery and Orthopedic Surgery

Abstract:This study aimed to identify patient characteristics that predict long-term opioid use after an orthopedic or neurosurgery procedure. Long-term opioid use was defined as opioid use for 90 or more days following the surgical procedure. A retrospective analysis was conducted of orthopedic and neurosurgery patients 18 years and older from 01/01/2011 through 12/31/2017 (n = 12,301). Characteristics included age, sex, race, length of hospital stay, body mass index, surgical procedure specialty, presence of opioid use before and after surgery, and opioid use 90 days or more after surgery. A multiple logistic regression model was used to model characteristics predictive of long-term use of opioids. In this cohort, 32.0% of patients had prescriptions for opioids 90 or more days after surgery. Statistically significant risk factors for long-term opioid use were being Caucasian, younger (18-25 years age group) or older than age 45 and being obese. People who were African American or Black, in the 25-45 years age group, underweight, and used opioids before surgery were less likely to use opioids 90 days after surgery. Nurse anesthetist awareness of predictive characteristics of long-term opioid use can lead to alternative options to prevent opioid abuse.
Antibodies against autologous tumor cell proteins in patients with small-cell lung cancer: association with improved survival.

Abstract:BACKGROUND The frequency and clinical relevance of human antitumor immune responses is not well known, and few target antigens have been identified. PURPOSE This study was designed to determine the frequency of antibodies reactive against extracts of autologous tumor cell lines and to correlate these data with survival. METHODS Serum samples were obtained from 40 lung cancer patients treated on National Cancer Institute protocols. These sera were used as probes in immunoblots against protein extracts from tumor cell lines derived from each of these patients. RESULTS We detected serum antibodies against autologous tumor cell proteins in 21 (58%) of the 36 patients with small-cell lung cancer (SCLC) and three (75%) of the four with non-small-cell lung cancer (NSCLC). Two patients' sera detected the p53 tumor suppressor gene product and two detected the product of the HuD gene (associated with paraneoplastic neurological syndromes) in their autologous tumor cell lysates. SCLC patients with antibodies against autologous tumor cell proteins had improved survivals compared with those in the antibody-negative group (P = .059). All patients who lived longer than 36 weeks were antitumor antibody positive. Sera from six (86%) of seven patients with limited disease were positive for antibodies that reacted against autologous tumor cells, compared with 15 of 29 (52%) of sera from patients with extensive disease. CONCLUSIONS Our results suggest that the sera from patients with SCLC frequently contain antibodies against tumor cell proteins and that these antibodies are associated with improved survival. IMPLICATIONS These data suggest that an antitumor immune response may affect tumor growth, and that the anonymous proteins detected by antitumor antibodies in lung cancer patient sera may represent proteins involved in the development of lung cancer or in its clinical manifestations.
Opioid Use Following Spine Surgery in Ambulatory Surgical Centers Versus Hospital Outpatient Departments

Study DesignRetrospective cohort study.ObjectiveTo assess the association between undergoing spine surgery in an ambulatory surgical center (ASC) vs a hospital outpatient department (HOPD) and (a) perioperative opioid prescription patterns and (b) prolonged opioid use.MethodsData from the Merative MarketScan Database included patients aged 18-64 who underwent single-level or multilevel anterior cervical discectomy and fusion (ACDF) or lumbar decompression between January 2017 and June 2021. Primary outcomes included receipt of a perioperative opioid prescription, perioperative oral morphine milligram equivalents (MMEs), and prolonged opioid use (defined as opioid prescription 91-180 days post-surgery). Secondary outcomes included the number of perioperative opioid prescriptions filled (single/multiple) and type of initial perioperative opioid filled (potent/weak). Analysis of prolonged opioid use was limited to opioid-naive patients. Propensity score matching (1 ASC to 3 HOPD cases) and logistic regression models were used for analysis.ResultsThe study included 11,654 ACDF and 26,486 lumbar decompression patients. For ACDF, ASCs had higher odds of an initial potent opioid prescription (OR = 1.18, 95% CI 1.08-1.30, P < .001) and higher total adjusted mean MMEs (+21.14, 95% CI 3.08-39.20, P = .02). For lumbar decompression, ASCs had increased odds of an initial potent opioid (OR = 1.23, 95% CI 1.16-1.30, P < .001) but lower odds of multiple opioid prescriptions (OR = 0.90, 95% CI 0.85-0.96, P < .001). There was no significant association between the surgery setting and prolonged opioid use.ConclusionDifferences in perioperative opioid prescribing were observed between ASCs and HOPDs, but there was no increase in prolonged opioid use in ASCs. Further research is needed to optimize postoperative pain management in different outpatient settings.

Abstract:Long-term patency of modified Cabrol technique in composite aortic root replacement is not well documented. We report a unique case in which a patient presented for prosthetic valve dysfunction with patent Cabrol grafts 22 years after composite root replacement requiring redo aortic root. This case also demonstrates how modified Cabrol grafts can be reutilized in aortic root replacement without compromising adequate fluid dynamics.
Force reserve of the diaphragm in patients with chronic obstructive pulmonary disease.

Abstract:The fatigue threshold of the human diaphragm in normal subjects corresponds to a transdiaphragmatic pressure (Pdi)-inspiratory time integral (TTdi) of about 15% of Pdimax. The TTdi of resting ventilation was measured in 20 patients with chronic obstructive pulmonary disease (COPD) and ranged between 1 and 12% of Pdimax (mean 5%). TTdi was significantly related to total airway resistance (Raw) (r = 0.57; P less than 0.05). Five of these patients were asked to voluntarily modify their TI/TT (ratio of inspiratory time to total cycle duration; from 0.33 to 0.49) so as to increase their TTdi from a control value of 8% to an imposed value of 17% of Pdimax. The imposed pattern induced a progressive decline in the high-frequency (150-350 Hz)/low-frequency (20-40 Hz) power ratio (H/L) of the diaphragm electromyogram (fatigue pattern), quantitatively similar to that seen in normal subjects breathing with similar TTdi levels. The decay in H/L was followed by a progressive fall in mean Pdi meanly due to decrease in gastric pressure swings. It is concluded that 1) the force reserve of the diaphragm in COPD patients is decreased because of a decrease in Pdimax; 2) the remaining force reserve of the diaphragm can be exhausted by even minor modifications in the breathing pattern; and 3) at a TI/TT of 0.40 our COPD patients can increase their mean Pdi 3-fold before reaching a fatiguing pattern of breathing compared with 8-fold in normal subjects.
Discharge Opioid Prescription and Consumption Following Surgery: The POPCORN Observational Study

Abstract:Background: Few studies have evaluated opioid consumption after various inpatient surgical procedures. Objectives: To describe opioid prescription patterns and to characterize patient-reported use of opioids after surgery. Methods: This single-centre prospective observational study was conducted between February and October 2021 at the Jewish General Hospital in Montréal, Quebec. Patients 18 years of age or older who underwent a surgical procedure, were hospitalized for 24 hours or longer after the procedure, and had an opioid prescription at the time of discharge were included. Data were collected for the quantity of opioids prescribed, as documented in hospital records, and the quantity consumed, as reported by participants. Various potential predictors of opioid consumption were explored, and data were also collected on patients' use of non-opioid coanalgesia, scores on the Numeric Rating Scale for pain, opioid renewal requests, and proper opioid disposal during the 30-day follow-up period. Results: A total of 150 participants completed the study. The median dose prescribed was 10 opioid pills (75.0 morphine milligram equivalents). By the end of the follow-up period, a median of 1 pill (7.5 morphine milligram equivalents) had been consumed from the total amount in the discharge prescription. Overall, 66 participants (44.0%) did not consume any of the opioids prescribed at discharge. Of the total number of pills prescribed, 58.2% (1193/2050) were unused, and 7.0% (5/71) of participants with unused pills disposed of them properly. Conclusions: Following discharge from hospital, postoperative patients consumed a median proportion of only 10% of prescribed opioid pills. More than half of all prescribed pills were unused. Protocols implementing specific prescribing strategies warrant further investigation to evaluate their potential impact on opioid prescription and consumption.
Patterns of pre-operative opioid use affect the risk for complications after total joint replacement

Abstract:Abstract Preoperative opioid use has been shown to increase the risk for complications following total joint arthroplasty (TJA); however, these studies have not always accounted for differences in co-morbidities and socio-demographics between patients that use opioids and those that do not. They have also not accounted for the variation in degree of pre-operative use. The objective of this study was to determine if preoperative opioid use is associated with risk for surgical complications after TJA, and if this association varied by degree of use. Population-based retrospective cohort study. Older adult patients undergoing primary TJA of the hip, knee and shoulder for osteoarthritis between 2002 and 2015 in Ontario, Canada were identified. Using accepted definitions, patients were stratified into three groups according to their preoperative opioid use: no use, intermittent use and chronic use. The primary outcome was the occurrence of a composite surgical complication (surgical site infection, dislocation, revision arthroplasty) or death within a year of surgery. Intermittent and chronic users were matched separately to non-users in a 1:1 ratio, matching on TJA type plus a propensity score incorporating patient and provider factors. Overall, 108,067 patients were included in the study; 10% (N = 10,441) used opioids on a chronic basis before surgery and 35% (N = 37,668) used them intermittently. After matching, chronic pre-operative opioid use was associated with an increased risk for complications after TJA (HR 1.44, p = 0.001) relative to non-users. Overall, less than half of patients undergoing TJA used opioids in the year preceding surgery; the majority used them only intermittently. While chronic pre-operative opioid use is associated with an increased risk for complications after TJA, intermitted pre-operative use is not.

Abstract:Background: Opioid use prior to spinal surgery is common among patients with workers’ compensation (WC) claims. Extended opioid use for pain management in this population is associated with several adverse outcomes including delayed return to work (RTW). Objective: This systematic review and meta-analysis aim to assess the evidence on the association of preoperative opioid use with stable RTW and RTW within 1-year after spinal surgery. Material and methods: We searched MEDLINE, Embase, PsycINFO, Emcare, CINAHL Plus, Scopus and Web of Science from inception to January 14, 2023. We included studies that compared any preoperative opioid use with no opioid use, and those that enabled a comparison of different durations of preoperative opioid use. The primary outcome was stable RTW after spinal surgery. Secondary outcomes were RTW within 1-year after surgery and cost of WC claims. A random effect model was assumed to pool the effect estimate. The GRADE approach was applied to evaluate the certainty of evidence. Results: From 2589 records, 10 studies were included, and of these, nine were considered for quantitative synthesis. All studies were observational with eight retrospective cohort and two case-control studies. Five studies each investigated cervical and lumbar disorders. With moderate certainty evidence, the odds of postoperative stable RTW reduced by half (OR: 0.51, 95% CI, 0.43-0.59; 5549 participants) in patients using opioids preoperatively. Similarly, moderate certainty evidence from 2348 participants demonstrated that the odds of RTW within 1-year after surgery were reduced by more than half in patients with preoperative opioid prescriptions (OR: 0.46, 95% CI 0.36-0.59). Conclusions: This systematic review and meta-analysis shows that preoperative opioid use is associated with a reduction in odds of postoperative RTW by half in patients with WC-funded spinal surgery.

Abstract:By tradition ‐ and for historical reasons ‐ reflex pathways and interneurones have been named by their dominating sensory input. Later studies have demonstrated that each individual interneurone, as a rule, receives a broad convergence from a large variety of sensory modalities, as well as inputs from one or more descending tracts. It is thus possible that the traditional nomenclature inadvertently has served as a ‘straightjacket’ for conceptual development in this field. Indeed, there is now much evidence in favour of the view that the many classes of spinal interneurones may be seen as ‘functional units’ representing different levels of muscle synergies, parts of movements, or even more integrated motor behaviour. Such ‘functional units’ may be used by (different) descending pathways to mediate the motor commands from the brain and integrate the appropriate (multimodal) sensory feedback into the central command. A given sensory stimulus would then be able to affect the motor output through a number of parallel, or alternative, segmental pathways belonging to different ‘functional units’. If this were correct it would indeed be predicted, rather than coming as a surprise, that a given sensory stimulus can result in different outputs ‐ even with a different sign ‐ depending on the preceding selection of active ‘functional units’, i.e. the type of motor activity initiated by the brain.
347 Short-Term Pain Outcomes and Pain Medication Utilization Among Urine Toxicology-Identified Opioid and Marijuana Users Following Elective Spine Surgery

Abstract:INTRODUCTION: Postoperative pain outcomes may be influenced by preoperative substance use, which is often underreported due to associated stigma. METHODS: Patients undergoing elective spinal surgery between September 2020 and May 2022 were recruited for this prospective cohort study. Detailed chart review was completed to collect demographic, urine toxicology, VAS, and pain medication regimen data. Comparisons between self-reported and urine toxicology-identified substance use, pre- and post-operative VAS ratings, and postoperative pain medication use were made using χ 2 tests, student’s t-tests, and logistic regression, respectively. Models were adjusted for age, sex, and race. RESULTS: Among 111 participants (mean age 58 years, 59% female, 95% with >1 comorbidity), urine toxicology overestimated drug use (47% vs 16%, p < 0.001) and underestimated alcohol use (16% vs 56%, p < 0.001) at preoperative baseline relative to patient reports. Two-weeks postoperatively, participants on preoperative opioids as determined by urine toxicology reported no significant improvements in pain from baseline (6.67 preop vs 5.92 postop, p = 0.288) unlike non-users (6.56 preop vs 4.61 postop, p < 0.001). They also had worse postoperative VAS (5.92 vs 4.61, p = 0.030) and heavier reliance on opioid medications (OR = 3.09, 95% CI = 1.21-7.89, p = 0.019). Conversely, participants on preoperative marijuana reported similar improvements in pain from baseline (users: 6.88 preop vs 4.36 postop, p = 0.001; non-users: 6.49 preop vs 5.07 postop, p = 0.001), similar postoperative pain (4.36 vs 5.07, p = 0.238), and similar postoperative reliance on opioid medications (OR = 0.96, 95% CI = 0.38-2.44, p = 0.928). CONCLUSIONS: While urine toxicology-identified preoperative opioid use was associated with poor postoperative pain relief and continued reliance on postoperative opioids for pain management following elective spinal surgery, preoperative marijuana use was not. Preoperative marijuana use, hence, should not delay or be a contraindication to elective spinal surgery.

Abstract:Background: The aim of this retrospective cohort study was to determine the rate of prolonged opioid use and identify associated risk factors after perioperative opioid exposure for tibial shaft fracture surgery. Materials and Methods: We used the MarketScan Commercial Claims and Encounters database (IBM) to identify patients 18 to 64 years old who filled a peri-operative opioid prescription after open reduction and internal fixation of a tibial shaft fracture from January 2016 to June 2020. Multivariable logistic regression identified factors (eg, demographics, comorbidities, medications) associated with prolonged opioid use (ie, filling an opioid prescription 91 to 180 days postoperatively); adjusted odds ratios (ORs) and 95% CIs were reported. Results: The rate of prolonged opioid use was 10.5% (n=259/2475) in the full cohort and 6.1% (n=119/1958) in an opioid-naive subgroup. In the full cohort, factors significantly associated with increased odds of prolonged use included preoperative opioid use (OR, 4.76; 95% CI, 3.60–6.29; P <.001); perioperative oral morphine equivalents in the 4th (vs 1st) quartile (OR, 2.68; 95% CI, 1.75–4.09; P <.001); age (OR, 1.03; 95% CI, 1.02–1.04; P <.001); and alcohol or substance-related disorder (OR, 1.66; 95% CI, 1.15–2.40; P =.01). Patients in the Northeast and North Central (vs South) regions had decreased odds of prolonged use (OR, 0.61–0.69; P =.02–.04). When removing preoperative use, findings were similar in the opioid-naive subgroup. Conclusion: Prolonged opioid use is not uncommon in this orthopedic trauma population, with the strongest risk factor being preoperative opioid use. Nevertheless, shared risk factors exist between the opioid-naive and opioid-tolerant subgroups that can guide clinical decision-making. [ Orthopedics . 2024;47(4):e188–e196.]

Abstract:Pathogenicity of feline reovirus serotype 2 isolates, Nos. 114 and 140 strains recovered from the feces of cats with diarrhea, was experimentally determined. Kittens orally inoculated with No. 114 strain did not show any clinical signs. Fecal consistency of kittens inoculated with No. 140 strain became rather diarrheic on the 3rd and 4th days after inoculation, but any other clinical abnormalities were not observed. Virus was excreted for the first five to six days after inoculation. Seroconversion was observed in the sera of all the inoculated kittens. Virus excretion and seroconversion were also noted in all contact control kittens. These results may indicate that feline reovirus serotype 2 is mildly pathogenic in cats.
