Abstract:Abstract Background: Hormone receptor-positive/HER2-negative (HR+/HER2-) breast cancer (BC) is clinically and biologically heterogeneous. CDK4/6 inhibitors (CDK4/6i) are proven to be effective in different molecular subtypes of HR+/HER2- BC, including the PAM50 luminal B. However, not all patients benefit to the same extent and new biomarkers for response are needed. The aim of this study was to develop a computational workflow to predict the response of patients with luminal B BC to treatment with CDK4/6i in combination with endocrine therapy (ET). Methods: The main part of the workflow is a computational model representing the mechanisms of action of the drugs and their influence on protein signaling and cell proliferation in the tumor. The model was trained on publicly available data from BC cell lines (Western blot time courses and viability dose-response data). The model was then used to predict response in patients by incorporating gene expression profiles obtained at baseline. The output of the model was a score indicating how strongly the tumor responded to the treatment. Gene expression data determined using the nCounter BC360 panel was available at baseline from two patient cohorts: 1) the CORALLEEN phase II trial which evaluated neoadjuvant ribociclib plus letrozole vs multi-agent chemotherapy in postmenopausal patients with luminal B by PAM50, HR+/HER2- early BC (Prat et al. Lancet Oncology. 2020), 2) a retrospective study of postmenopausal patients with HR+/HER2-/Luminal B advanced BC treated with CDK4/6i plus ET in the first line setting at Hospital Clinic Barcelona (hereafter CDK cohort). PAM50 risk of relapse (ROR) and Ki67 levels were considered as outcome for the CORALLEEN patients. Progression free survival (PFS) was available in the CDK cohort. Area under the ROC Curve (AUC) was used to estimate the discrimination performance of the model. Differences were tested for statistical significance using Wilcoxon rank-sum test. Hazard ratios were estimated using Cox regression to investigate the association with PFS. Results: The computational model used here describes the dynamics of relevant protein-protein and drug-protein interactions to the treatment of CDK4/6i plus ET. The model showed high agreement with the training data obtained from cell lines (Pearson correlation of 0.88 and 0.95). The ability of the trained model to predict treatment outcome in patients was validated in baseline samples of the CDK4/6i arm (n=51) and the chemotherapy arm (n=52) of the CORALLEEN trial, and baseline Luminal B primary tumors of the CDK cohort (n=19). For each patient, a response score was calculated by the model using the expression of 6 genes at baseline as input. This response score was significantly associated with patient response in both cohorts. The model identified patients with high Ki67 (AUC of 0.80 (95% CI 0.64 - 0.92)) and high ROR (AUC of 0.78 (95% CI 0.64 - 0.89)) after treatment in the CDK4/6i arm of the CORALLEEN cohort and was used to stratify patients into different response groups. The AUC in the chemotherapy arm of CORALLEEN for identifying high Ki67 was 0.44 (95% CI 0.29 - 0.58), indicating that the predictions are specific to CDK4/6i plus ET treatment. Additionally, stratification of patients in the CDK cohort into either two or three groups based on the response score was linked to PFS (HR=3.71 (95% CI 0.97 - 14.16), p=0.055 and HR=2.92 (95% CI 1.08 - 7.86), p=0.034, respectively). Conclusion: A mechanistic model was developed, trained on cell line data, and then used to predict treatment outcome of patients with HR+/HER2-/PAM50 Luminal B BC treated with CDK4/6i plus ET. This approach showed significant association with observed outcome and could be used to assign patients to different response groups, indicating the usefulness of the model as a potential marker for response. Abstract:Background: Endocrine therapy is highly effective in blocking the estrogen receptor pathway in HR+/HER2- early breast cancer (EBC). However, up to 40% of patients experience relapse during or after adjuvant endocrine therapy. Here, we investigate molecular mechanisms associated with primary resistance to endocrine therapy and develop predictive models. Patients and Methods: In the WSG-ADAPT trial (NCT01779206), HR+/HER2- EBC patients underwent pre-operative short-term endocrine therapy (pET). Treatment response was determined by immunohistochemical in-situ labeling of cycling cells (G1 to M-phase) with Ki67 before and after pET. We performed targeted next generation sequencing and Infinium MethylationEPIC-based DNA methylation analysis post-pET in a discovery cohort (n=364, responder (R) and non-responder (NR) pairs matched for clinicopathologic features) and a validation cohort (n=270, unmatched). Predictive indices of endocrine resistance under both treatments were constructed using lasso penalized logistic regression. A subset of breast cancers from The Cancer Genome Atlas project (TCGA-BRCA) was used for external validation. Results: TP53 mutations were prominently associated with primary resistance to both tamoxifen (TAM) and aromatase inhibitors (AI), with AI non-responders exhibiting resistance in up to 32% of cases. Additionally, we identified distinct DNA methylation patterns in TAM and AI non-responders, with TAM non-responders showing global DNA methylation loss, associated with KRAS signaling, apical junctions and epithelial-mesenchymal transition (EMT). Conversely, we observed methylation gain in AI non-responders affecting developmental transcription factors, hypoxia and estrogen signaling. TAM or AI resistance was associated with increased methylation-inferred proportions of immune cells and decreased proportions of endothelial cells. Based on these findings and patient age, we developed the Predictive Endocrine ResistanCe Index (PERCI). PERCI stratified NR and R cases in both treatment groups and cohorts with high accuracy (ROC AUC TAM discovery 93.9%, validation 83%; AI discovery 98.6%, validation 76.9%). A simplified PERCI efficiently predicted progression-free survival in the TCGA-BRCA sub-cohort (Kaplan-Meier log-rank p-value = 0.03 between low and high PERCI groups). Conclusions: We identified genomic and epigenomic features associated with primary resistance to TMA and AI. By combining information on genomic alterations, patient age, differential methylation and tumor microenvironment (TME) composition, we developed PERCI TAM and PERCI AI as novel predictors of primary resistance, with potential additional prognostic value. Early prediction of clinical response to checkpoint inhibitor therapy in human solid tumors through mathematical modeling

Abstract:Background: Checkpoint inhibitor therapy of cancer has led to markedly improved survival of a subset of patients in multiple solid malignant tumor types, yet the factors driving these clinical responses or lack thereof are not known. We have developed a mechanistic mathematical model for better understanding these factors and their relations in order to predict treatment outcome and optimize personal treatment strategies. Methods: Here, we present a translational mathematical model dependent on three key parameters for describing efficacy of checkpoint inhibitors in human cancer: tumor growth rate ( α ), tumor-immune infiltration ( Λ ), and immunotherapy-mediated amplification of anti-tumor response ( μ ). The model was calibrated by fitting it to a compiled clinical tumor response dataset (n = 189 patients) obtained from published anti-PD-1 and anti-PD-L1 clinical trials, and then validated on an additional validation cohort (n = 64 patients) obtained from our in-house clinical trials. Results: The derived parameters Λ and μ were both significantly different between responding versus nonresponding patients. Of note, our model appropriately classified response in 81.4% of patients by using only tumor volume measurements and within 2 months of treatment initiation in a retrospective analysis. The model reliably predicted clinical response to the PD-1/PD-L1 class of checkpoint inhibitors across multiple solid malignant tumor types. Comparison of model parameters to immunohistochemical measurement of PD-L1 and CD8+ T cells confirmed robust relationships between model parameters and their underlying biology. Conclusions: These results have demonstrated reliable methods to inform model parameters directly from biopsy samples, which are conveniently obtainable as early as the start of treatment. Together, these suggest that the model parameters may serve as early and robust biomarkers of the efficacy of checkpoint inhibitor therapy on an individualized per-patient basis. The role of heme in the regulation of iron uptake during Friend cell differentiation.

Abstract:The inhibitory effect of hemin on iron uptake and heme synthesis was investigated in Friend erythroleukemia cells. In cells of line 707 the inhibition of iron uptake became more pronounced after the induction of erythroid differentiation by dimethyl sulfoxide. No such rise in sensitivity to hemin was found in cells of line Fw, which are deficient in ferrochelatase activity and do not synthesize hemoglobin after induction of differentiation by sodium butyrate. The possibility that hemin influences mainly the uptake of iron used directly for heme synthesis, and to a lesser extent intracellular storage of iron is discussed.

Abstract:CDK4/6i plus endocrine therapy is standard of care for patients with hormone receptor-positive/human epidermal growth factor receptor-negative advanced or metastatic breast cancer; however, patients frequently develop resistance to therapy. Identifying biomarkers that are associated with CDK4/6i resistance or response is key to selecting the most appropriate patients for treatment and could be used to develop novel agents or combination therapies. The aim of this study was to comprehensively describe the CDK4/6i biomarker landscape to guide future research. We used a combination of bioinformatics and AI techniques to examine and collate putative biomarkers. Data were mined from Uniprot to identify proteins related to CDK4/6. Biomarkers used in clinical trials were mined from the PharmaProjects database. Protein interaction networks were generated using Signor, STRING, and Bioplex 3.0. To explore relevant publications, we performed a Geneshot analysis and used the output to conduct an enrichment analysis using the MSigDB database. To find non-protein biomarkers such as non-coding RNA species or molecular signatures, we used deep learning text analytics to interrogate more than 50 million documents across several platforms including PubMed, medRxiv, and bioRxiv. Results were prioritized by manual curation and target tractability analysis. Bioinformatic database analyses identified 876 genes with known Uniprot functions potentially related to CDK4/6i response in breast cancer. Additionally, genes identified by Geneshot were contained within 42 oncogenic gene sets; 5 of these gene sets contained > 60 of the identified genes, with the most significant dysregulated pathways being TBK1, Rb, PDGF, SNF5, cyclin D1, Raf, and p53. Following target tractability analysis and literature review, we prioritized ∼20 promising candidates, including ERBB2, AKT1, and PIK3CA. The compendium developed using our multi-approach, AI-assisted review of existing knowledge of the CDK4/6i biomarker landscape will be central in supporting next generation research and drug development for breast cancer.

Abstract:e13603 Background: Oncology therapeutic development has high failure rates with substantial costs. Advances in technology, including molecular characterisations of cancer and computational power, provide the opportunity to better model therapeutic response for existing and emerging therapies. Methods: Concr adapted Bayesian statistical principles used by astrophysicists to effectively integrate multi-dimensional data (genomic & transcriptomic) for accurate modelling of cancer biology. Our proprietary computational framework was applied to predict clinical response and survival through construction of digital twins and in silico simulations of clinical trials. Predicted log odds ratios (LOR) were generated for drug response and compared to published data from 8 suitable historical clinical trials. Three distinct models underpin this framework: Drug Efficacy, Treatment Response & Overall Survival (OS). Three publicly available datasets were used for training: Cancer Therapeutic Response Portal ( in vitro dose-response data), Cancer Cell Line Encyclopaedia and TCGA. Results: For all 8 clinical studies, the digital twin model accurately simulated both trial arms, compared drug efficacy across arms and predicted which treatment was most active. Unblinded evaluation: Our model accurately predicted gemcitabine would have greater clinical benefit than 5-FU (LOR -0.10, P <0.0001) in metastatic pancreatic cancer. In advanced breast cancer, the model predicted docetaxel had a higher response rate than doxorubicin, and carboplatin response rates were similar to docetaxel in BRCA wild-type patients. In platinum-sensitive recurrent ovarian cancer, the model predicted cisplatin-based combination had a higher response rate than paclitaxel. Blinded evaluation: Using data for paclitaxel, the model correctly predicted that nab-paclitaxel+gemcitabine response rates were higher than gemcitabine in metastatic pancreatic cancer (predicted LOR -0.090, p = CMF, adjuvant TC > AC and adjuvant tamoxifen was superior to capecitabine. The underlying pan-tumour Overall Survival model used a Random Survival Forest approach, trained on 23 cancers with predictive accuracy of 0.78 AUC ROC and C-index 0.71. To enable cohort enrichment for drug response, the model segmented patient cohorts into treatment responder and non-responder. Using this approach, the log odds increased for drug response rate for 16 of the 19 different cancers and 14 of the different 17 drugs. Conclusions: Our digital twin model represents transformative technology for clinical trial simulation with strong potential for accelerating and de-risking clinical drug development, and utility for synthetic control arms.

Abstract:A significant number of breast cancers develop resistance to hormone therapy. This progression, while posing a major clinical challenge, is difficult to predict. Despite important contributions made by cell models and clinical studies to tackle this problem, both present limitations when taken individually. Experiments with cell models are highly reproducible but do not reflect the indubitable heterogenous landscape of breast cancer. On the other hand, clinical studies account for this complexity but introduce uncontrolled noise due to external factors. Here, we propose a new approach for biomarker discovery that is based on a combined analysis of sequencing data from controlled MCF7 cell experiments and heterogenous clinical samples that include clinical and sequencing information from The Cancer Genome Atlas. Using data from differential gene expression analysis and a Bayesian logistic regression model coupled with an original simulated annealing-type algorithm, we discovered a novel 6-gene signature for stratifying patient response to hormone therapy. The experimental observations and computational analysis built on independent cohorts indicated the superior predictive performance of this gene set over previously known signatures of similar scope. Together, these findings revealed a new gene signature to identify patients with breast cancer with an increased risk of developing resistance to endocrine therapy.

Abstract:3055 Background: Although cyclin-dependent kinase 4/6 inhibitors (CDK4/6i) are a primary treatment for hormone receptor-positive/HER2 negative MBC, data regarding resistance mechanisms are still an unmet need. The aim of the study was to highlight new resistance pathways using machine learning (ML) to detect multiparametric patterns in complex datasets. Methods: The study retrospectively analyzed a cohort of 610 hormone receptor positive HER2 negative MBC patients (pts) at Northwestern University, Massachusetts General Hospital and Washington University in St. Louis between 2015-2020 with baseline ctDNA testing by Guardant360. Pathways were defined based on previous work (Sanchez-Vega F et al, Cell. 2018) (i.e., RTK, RAS, RAF, MEK, NRF2, ER, WNT, MYC, P53, cell cycle, notch, PI3K). Only pathogenic variants according to OncoKB were included in the models. Associations among single nucleotide (SNVs) and copy number (CNVs) variations, pathway classification and previous exposure to CDK4/6i were explored through logistic regression and Gradient boosted machines (GBMs) ML algorithm. Results: at baseline, 322 pts (52.8%) were previously treated with CDK4/6i. The most detected pathway alterations were SNVs in PI3K (37.1%), P53 (31.8%), ER (29.2%) and RTK (22.3%). After stepwise logistic regression, RB1, NF1 and ESR1 SNVs were associated with previous exposure to CDK4/6i (respectively OR: 3.55 P = 0.017; OR: 3.06 P = 0.026 and OR: 1.82 P < 0.001), while SNVs in the ER pathway were associated with CDK4/6i (1.56 P < 0.001). Two GBMs models were designed based on gene variants (training AUC: 0.695, cross validation AUC: 0.631) and oncogenic pathways (training AUC: 0.713, cross validation AUC: 0.619). The highest relative importance (RI) was observed for ESR1 SNVs (RI: 35.35), TP53 SNVs (RI: 11.33), NF1 SNVs (RI: 3.45), SMAD4 SNVs (RI: 3.39) and RB1 SNVs (RI: 3.33). Alterations at a pathway level with the highest RI were ER SNVs (RI: 33.50), P53 SNVs (RI: 14.98), PI3K SNVs (RI: 14.40), RTK SNVs (RI: 10.55), RTK CNVs (RI: 10.26), cell cycle CNVs (RI: 6.99), cell cycle SNVs (RI: 6.77) and RAS SNVs (RI: 6.54). Of the previously highlighted pathway alterations, a significant impact on PFS after ctDNA collection was observed among de novo pts treated with CDK4/6i (165 pts) for ER SNVs (P < 0.0001), RTK SNVs (P = 0.0011), RTK CNVs (P = 0.0006), Cell cycle CNVs (P = 0.0010) and Cell cycle SNVs (P = 0.0143). No impact was observed on PFS for pts who had not received a CDK4/6i-based regimen. Conclusions: The combination of ctDNA-based datasets and machine learning algorithms defined novel resistance pathways for patients treated with CDK4/6i. Although preliminary, these results suggest that alterations of the ER, RTK and Cell cycle pathways might be crucial to optimize treatment strategy and drug development.

Machine-learning and mechanistic modeling of metastatic breast cancer after neoadjuvant treatment

Abstract:Clinical trials involving systemic neoadjuvant treatments in breast cancer aim to shrink tumors before surgery while simultaneously allowing for controlled evaluation of biomarkers, toxicity, and suppression of distant (occult) metastatic disease. Yet neoadjuvant clinical trials are rarely preceded by preclinical testing involving neoadjuvant treatment, surgery, and post-surgery monitoring of the disease. Here we used a mouse model of spontaneous metastasis occurring after surgical removal of orthotopically implanted primary tumors to develop a predictive mathematical model of neoadjuvant treatment response to sunitinib, a receptor tyrosine kinase inhibitor (RTKI). Treatment outcomes were used to validate a novel mathematical kinetics-pharmacodynamics model predictive of perioperative disease progression. Longitudinal measurements of presurgical primary tumor size and postsurgical metastatic burden were compiled using 128 mice receiving variable neoadjuvant treatment doses and schedules (released publicly at https://zenodo.org/records/10607753). A non-linear mixed-effects modeling approach quantified inter-animal variabilities in metastatic dynamics and survival, and machine-learning algorithms were applied to investigate the significance of several biomarkers at resection as predictors of individual kinetics. Biomarkers included circulating tumor- and immune-based cells (i.e., circulating tumor cells and myeloid-derived suppressor cells) as well as immunohistochemical tumor proteins (i.e., CD31 and Ki67). Our computational simulations show that neoadjuvant RTKI treatment inhibits primary tumor growth but has little efficacy in preventing (micro)-metastatic disease progression after surgery and treatment cessation. Machine learning algorithms that included support vector machines, random forests, and artificial neural networks, confirmed a lack of definitive biomarkers, which shows the value of preclinical modeling studies to identify potential failures that should be avoided clinically. Using simulations from a mechanistic mathematical model compared with preclinical data from surgical metastasis models, we found that anti-tumor effects of neoadjuvant receptor tyrosine kinase inhibitor treatment can differ between the primary tumor and metastases in the perioperative setting. Model simulations with variable drug doses and scheduling of neoadjuvant treatment revealed a contrasting impact on initial primary tumor debulking and metastatic outcomes long after treatment has stopped and the primary tumor has been surgically removed. Using machine-learning algorithms, we identified the limited power of several circulating cellular and molecular biomarkers in predicting metastatic outcomes, uncovering a potential fast-track strategy for assessing future clinical biomarkers by pairing patient studies with identical studies in mice.

Abstract:Introduction Neoadjuvant chemotherapy (NAC) is the standard-of-care (SOC) for patients with locally advanced triple-negative breast cancer (TNBC) [1]. Unfortunately, only about half of TNBC patients achieve a pathological complete response (pCR) at the completion of NAC [2]. With the recent FDA approval of immunotherapy, the response rate improved by about 8% [3]. To try to further improve NAC response rates, we build upon methods that predict treatment response to optimize interventions and outcomes. Our previous studies have demonstrated that excellent predictive accuracy of TNBC response to NAC was achieved through calibrating mechanism-based models to both pre- and on- treatment imaging data [4]. However, by requiring on-treatment imaging data, therapy optimization can only occur after a patient begins NAC. Here, our goal is to relax the requirement of on-treatment imaging data such that we can predict, and potentially optimize, a patient's response to NAC before initiating it. We will accomplish this through combining data-driven approaches with mechanism-based modeling to obtain spatiotemporal predictions of tumor response prior to NAC. More specifically, we integrate a reaction-diffusion equation based mathematical model with a U-Net based convolutional neural network (CNN). Methods The reaction-diffusion equation models the development of tumor cellularity over time as the sum of tumor cell diffusion, controlled by a fixed diffusivity, and tumor cell proliferation, controlled by a calibrated net proliferation rate. The net proliferation rate implicitly accounts for both proliferative and drug induced death effects. We calibrated the net proliferation rates in this model on a patient-specific basis for 128 patients from the ARTEMIS trial (NCT02276443) using three imaging timepoints – pre- (V1), post two cycles (V2), and post four cycles (V3) of A/C. We then employed a CNN to characterize the relationship between the calibrated net proliferation rates and the pre-treatment imaging data. The CNN takes pretreatment imaging inputs of the apparent diffusion coefficient map, percent enhancement maps from the dynamic contrast enhanced MRI timecourse, and precontrast T1 map. The CNN outputs an estimate of the calibrated net proliferation rate which can be used to run the mathematical model forward in time to predict cellularity maps at V2/3. In training the CNN, our loss considers both the concordance correlation coefficient (CCC) and the normalized root mean square error (NRMSE). We calculated loss on the net proliferation rate predictions as NRMSE with a CCC penalty to aid in characterizing the non-Gaussian distribution of the net proliferation rate. We calculated loss on the cellularity predictions as the CCC between predicted and measured voxel-wise cellularity. The CNN was trained using the Adam optimizer and a fivefold cross validation with a withheld test set of 26 patients. Results Using the CNN predictions to forward run the mechanism-based model in time to V3 we calculated change in total tumor cellularity and volume from V1 to V3. In the test cohort, we obtained CCC values between the predicted and measured changes of 0.95 and 0.91 for cellularity and volume, respectively. Discussion and summary Through integrating mechanistic modeling and deep learning, we can accurately predict the spatiotemporal development of TNBC response to NAC using only pretreatment imaging data. We will build upon these results by expanding our model to include separate proliferation and drug induced death terms to allow for therapy intervention and optimization. Abstract:BackgroundTumor microenvironment (TME) status is closely related to breast cancer (BC) prognosis and systemic therapeutic effects. However, to date studies have not considered the interactions of immune and stromal cells at the gene expression level in BC as a whole. Herein, we constructed a predictive model, for adjuvant decision-making, by mining TME molecular expression information related to BC patient prognosis and drug treatment sensitivity.MethodsClinical information and gene expression profiles were extracted from The Cancer Genome Atlas (TCGA), with patients divided into high- and low-score groups according to immune/stromal scores. TME-related prognostic genes were identified using Kaplan-Meier analysis, functional enrichment analysis, and protein-protein interaction (PPI) networks, and validated in the Gene Expression Omnibus (GEO) database. Least absolute shrinkage and selection operator (LASSO) Cox regression analysis was used to construct and verify a prognostic model based on TME-related genes. In addition, the patients’ response to chemotherapy and immunotherapy was assessed by survival outcome and immunohistochemistry (IPS). Immunohistochemistry (IHC) staining laid a solid foundation for exploring the value of novel therapeutic target genes.ResultsBy dividing patients into low- and high-risk groups, a significant distinction in overall survival was found (p < 0.05). The risk model was independent of multiple clinicopathological parameters and accurately predicted prognosis in BC patients (p < 0.05). The nomogram-integrated risk score had high prediction accuracy and applicability, when compared with simple clinicopathological features. As predicted by the risk model, regardless of the chemotherapy regimen, the survival advantage of the low-risk group was evident in those patients receiving chemotherapy (p < 0.05). However, in patients receiving anthracycline (A) therapy, outcomes were not significantly different when compared with those receiving no-A therapy (p = 0.24), suggesting these patients may omit from A-containing adjuvant chemotherapy. Our risk model also effectively predicted tumor mutation burden (TMB) and immunotherapy efficacy in BC patients (p < 0.05).ConclusionThe prognostic score model based on TME-related genes effectively predicted prognosis and chemotherapy effects in BC patients. The model provides a theoretical basis for novel driver-gene discover in BC and guides the decision-making for the adjuvant treatment of early breast cancer (eBC).

Abstract:CDK4/6 inhibitors are widely used in combination with endocrine therapy in the treatment of HR+/HER2- metastatic breast cancer. Generally the CDK4/6 inhibitors lead to a doubling of progression-free survival (PFS) over single agent endocrine therapy. In spite of a known mechanism of action, biomarkers to define the duration of response to CDK4/6 inhibitors with either aromatase inhibitors (AI) or fulvestrant (FUL) remain lacking. A cohort of over 300 patients receiving standard of care CDK4/6 inhibitor combination therapy for metastatic disease were consented for participation (NCT04526587). The duration of PFS in both AI and FUL treated sub-groups were comparable to that observed in randomized clinical trials. In this cohort progesterone receptor-status (HR 0.70, p=0.0123), visceral disease (HR 1.55, p=0.0013), prior endocrine therapy (HR 2.34, p< 0.001), and type of endocrine therapy (HR 2.16, p< 0.001) were associated with duration of PFS. To develop biomarkers, gene expression analyses was performed on 313 tumor samples representing over 200 individual patients. Focusing on the pre-treatment metastatic biopsy, established signatures associated with prognosis in HR+/HER2- breast cancer were employed to determine association with response to combination therapy in the metastatic setting. Proliferation-associated signatures were associated with shorter PFS. For example, an established RB loss signature was associated with PFS in both AI (HR 1.9, p=0.009) and FUL (HR 2.8, p=0.007) treated patients. Clinically employed signatures (e.g. OncotypeDx and Mammaprint) harbored variable associations with outcomes. Random Survival Forest feature selection was applied to individual and combined prognostic signatures. This approach resulted in a potent classifier for progression-free survival (HR 10, p< 1E-20). To enable applicability to different manifestations of disease, significant clinical pathological variables were incorporated into the model so that there was equivalent performance in AI and FUL treated patients for prediction of PFS. This predictive algorithm was also strongly associated with overall survival (HR 4.6, p=2.7E-8). Ongoing studies are evaluating the algorithms prospectively in NCT04526587. Abstract:Approximately 70% of patients have breast cancers that are oestrogen receptor alpha positive (ER+) and are therefore candidates for endocrine treatment. Many of these patients relapse in the years during or following completion of adjuvant endocrine therapy. Thus, many ER+ cancers have primary resistance or develop resistance to endocrine therapy during treatment. Recent improvements in our understanding of how tumours evolve during treatment with endocrine agents have identified both changes in gene expression and mutational profiles, in the primary cancer as well as in circulating tumour cells. Analysing these changes has the potential to improve the prediction of which specific patients will respond to endocrine treatment. Serially profiled biopsies during treatment in the neoadjuvant setting offer promise for accurate and early prediction of response to both current and novel drugs and allow investigation of mechanisms of resistance. In addition, recent advances in monitoring tumour evolution through non-invasive (liquid) sampling of circulating tumour cells and cell-free tumour DNA may provide a method to detect resistant clones and allow implementation of personalized treatments for metastatic breast cancer patients. This review summarises current and future biomarkers and signatures for predicting response to endocrine treatment, and discusses the potential for using approved drugs and novel agents to improve outcomes. Increased prediction accuracy is likely to require sequential sampling, utilising preoperative or neoadjuvant treatment and/or liquid biopsies and an improved understanding of both the dynamics and heterogeneity of breast cancer.

Abstract:Tumor heterogeneity contributes significantly to chemoresistance, a leading cause of treatment failure. To better personalize therapies, it is essential to develop tools capable of identifying and predicting intra- and inter-tumor heterogeneities. Biology-inspired mathematical models are capable of attacking this problem, but tumor heterogeneity is often overlooked in in-vivo modeling studies, while phenotypic considerations capturing spatial dynamics are not typically included in in-vitro modeling studies. We present a data assimilation-prediction pipeline with a two-phenotype model that includes a spatiotemporal component to characterize and predict the evolution of in-vitro breast cancer cells and their heterogeneous response to chemotherapy. Our model assumes that the cells can be divided into two subpopulations: surviving cells unaffected by the treatment, and irreversibly damaged cells undergoing treatment-induced death. MCF7 breast cancer cells were previously cultivated in wells for up to 1000 hours, treated with various concentrations of doxorubicin and imaged with time-resolved microscopy to record spatiotemporally-resolved cell count data. Images were used to generate cell density maps. Treatment response predictions were initialized by a training set and updated by weekly measurements. Our mathematical model successfully calibrated the spatiotemporal cell growth dynamics, achieving median [range] concordance correlation coefficients of > .99 [.88, >.99] and .73 [.58, .85] across the whole well and individual pixels, respectively. Our proposed data assimilation-prediction approach achieved values of .97 [.44, >.99] and .69 [.35, .79] for the whole well and individual pixels, respectively. Thus, our model can capture and predict the spatiotemporal dynamics of MCF7 cells treated with doxorubicin in an in-vitro setting.

Abstract:The study explored endocrine resistance by leveraging machine learning to establish the prognostic stratification of predicted Circulating tumor cells (CTCs), assessing its integration with circulating tumor DNA (ctDNA) features and contextually evaluate the potential of CTCs-based transcriptomics. 1,118 patients with a diagnosis of luminal-like Metastatic Breast Cancer (MBC) were characterized for ctDNA through NGS before treatment start, predicted CTCs were computed through a K nearest neighbor algorithm. Differences across subgroups were analyzed through chi square or Fisher's exact test according to sample size and corrected for False Discovery Rate. Differences in survival were tested by log-rank test and uni- and multivariable Cox regression. CTCs transcriptomics was performed through RNAseq after sorting with DEPArray NxT. Univariable and multivariable analysis adjusted for ctDNA alterations revealed a significant impact of CTCs predictive stratification on both progression-free survival (PFS) and overall survival (OS). Alterations in RTK and ER pathways were significantly correlated with predicted-Stage IVaggressive. The combined impact of CTCs stratification and RTK/ER pathway alterations influenced patient outcomes, with predicted-Stage IVaggressive having a negative impact on PFS regardless of the mutational status. The pilot exploratory CTCs transcriptomics analysis showed transcriptional changes linked to cell proliferation such as under expression of MALAT1 and overexpression of GREM1, GPR85 and OCM. Our data underline the potential of an integration between ctDNA and CTCs, both through quantification and transcriptomic analysis, for a deeper understanding of tumor biology and treatment response in HR-positive, HER2-negative MBC.

Abstract:AbstractBackgroundCDK4/6 inhibitors (CDK4/6i) have shown promising results in the treatment of hormone receptor‐positive (HR+) metastatic breast cancer (MBC) when combined with endocrine therapy (ET). It is crucial to evaluate the actual effectiveness and safety of CDK4/6i in clinical practice, as well as to analyze the factors that can predict their outcomes.MethodsPatients with HR+ MBC who received CDK4/6i‐based therapy between May 2016 and May 2023 at Hunan Cancer Hospital were evaluated for progression‐free survival (PFS). Adverse reactions were assessed based on the National Cancer Institute Common Toxicity Criteria (version 5.0).ResultsThis study included 344 patients, with a median PFS (mPFS) of 12.8 months (range: 10.4–15.2 months). After adjustment, Cox multivariate regression analysis revealed that visceral metastasis (specifically liver and brain metastases), Eastern Cooperative Oncology Group Performance Status (ECOG PS) ≥ 1, estrogen receptor ≤ 80%, progesterone receptor ≤ 10%, Ki‐67 > 30%, and treatment in later stages were significant factors associated with reduced PFS. Based on this, we created a prognostic nomogram and validated its performance, obtaining a C‐index of 0.714 (95% confidence interval: 0.640–0.787) as well as reliable calibration and clinical impact. The mPFS of CDK4/6i rechallenge was 7.7 months; for patients who initially discontinued CDK4/6i for reasons other than disease progression, CDK4/6i rechallenge still provided a mPFS of 11.4 months. The tolerability and safety of combining CDK4/6is with ET were manageable. Adverse events led to treatment discontinuation in 3.8% of patients. Neutropenia (29.1%), leukopenia (13.7%), and anemia (4.1%) were the primary grade 3/4 adverse reactions.ConclusionsThis real‐world study highlights the ample efficacy and reasonable safety of combined CDK4/6i and ET in patients with HR+ MBC. Individualized treatment decisions and ongoing safety monitoring are important to optimize the therapeutic benefit of CDK4/6i treatment.

Abstract:601 Background: Triple negative breast cancer (TNBC) is a biologically and clinically heterogenous disease, associated with poorer outcomes when compared with other subtypes of breast cancer. In early-stage TNBC, surgery with curative intent remains the mainstay of therapy. Neoadjuvant chemotherapy is often given prior to surgery and achieving pathological complete response (pCR) has been associated with improved long-term outcomes in terms of progression-free survival (PFS) and overall survival (OS). There is thus high clinical interest in the ability to accurately predict pCR status using baseline data. Methods: A retrospective cohort of 57 patients with early-stage TNBC treated with neoadjuvant chemotherapy was analyzed to develop a machine learning-based algorithm predictive of pCR likelihood at the individual patient level. Multimodal baseline data were collected including clinical, biological (e.g., histology, genomic profile including CDC2, CDC20, KPNA2, MYBL2, complete blood count, Ki67), imaging data (baseline PET/CT scan), the radiology report and clinical outcomes data (pCR, PFS, OS). For each patient, tumors were segmented in 3D by an experimented nuclear physician using the SOPHiA Radiomics platform. Radiomics features were then extracted following the IBSI standards and then combined with the other data modalities. A filter-based variable selection method was applied before training several machine learning algorithms. The optimization criterion was the Area Under the ROC Curve (AUC). Due to the small size of the cohort, a nested leave-pair-out cross-validation approach was used to properly estimate the model performance. Results: The best result was obtained with the SVM algorithm with a linear kernel, reaching an AUC of 0.82, a precision of 71%, a sensitivity of 71% and a specificity of 70%. The features with highest weight in the algorithm were a mix of radiological, radiomics, biological and clinical features, highlighting the importance of a truly multimodal analysis. Indeed, withdrawing a specific data modality (e.g., radiomics features or biological features), led to a decrease of ̃10% of the AUC. Patients were then stratified into two groups based upon their predicted pCR status. These two groups displayed a statistically significant difference in PFS (p<0.001), suggesting that baseline multimodal data analysis could help predict long-term outcomes. Conclusions: This proof of concept study suggests that machine learning applied to baseline multi-modal data can help predict pCR status after neoadjuvant chemotherapy for TNBC at the individual patient level, as well as stratify patients to inform long-term outcomes. Patients that would be predicted as non-pCR could benefit from concomitant treatment with immunotherapy, or dose intensification. This algorithm will be further validated in a larger, multicentric cohort.

Abstract:Abstract Background: Breast Cancer (BC) patients exhibit a wide variety of responses to neoadjuvant chemotherapy (NACT). This is driven by factors both intrinsic (e.g., mutations, dysregulation, metabolic reprogramming) and extrinsic (e.g., nutrient/drug perfusion, interactions with surrounding healthy tissues and the tumor microenvironment (TME)) to the cells that make up each tumor. The SimBioSys TumorScope is a platform for making individualized predictions of the response of each patient's tumor to NACT. It employs 3D biophysical simulations that explicitly model the dynamics of cellular response to the ever-changing chemical milieu of drugs and nutrients that perfuse the TME during treatment, in order to predict when and where different regions of the tumor are growing, dying, and ultimately how a given patient will respond to treatment. Methods: The SimBioSys TumorScope constructs 3D in silico models of each patient's tumor directly from pretreatment DCE-MRIs. It combines this spatial model with personalized genome-scale models of tumor and tissue metabolism, pharmacokinetics, and pharmacodynamics, and vascular perfusion (based on DCE-MRI timeseries). The combined model is then simulated using a custom high-performance reaction-diffusion-material mechanics simulation engine which produces a spatio-temporal trajectory of tumor size, morphology, intra- and extracellular biochemistry. We evaluated the ability of the TumorScope software to predict volumetric response to NACT. A validation set comprising the pretreatment records (including MRIs) of 780 BC patients that underwent NACT was used. These patients spanned a wide range of tumor sizes, molecular subtypes, and NCCN-recommended treatment regimens. Simulations were initialized using each patient’s pretreatment MRI and pathology data and run from the start of therapy to the specified surgical date. Simulated tumor volumetric percent response (calculated as the ratio of change in tumor volume to initial volume) at the time of surgery was then compared with actual tumor volumetric percent response extracted from presurgical MRIs. Among patients for which event free survival data was available (n = 480), we performed a Cox proportional hazard analysis. Results: The SimBioSys TumorScope predicted pre-surgical tumor volumetric response with a median error of 0.03% and median absolute deviation of 8.2%. Among the patients for which EFS data was available, we found a hazard ratio of 1.8 associated with having a final simulated volume greater than 0.01 cc (p = 0.00048). Conclusions: The SimBioSys TumorScope produces accurate patient specific predictions of response to NACT using only standard-of-care pre-treatment data. Such predictions can aid in decision making, enabling physicians to select less-toxic regimens for patients in which a robust response is predicted, and more aggressive treatments and/or clinical trial enrollment when response is likely to be poor. Practical prediction model of the clinical response to programmed death-ligand 1 inhibitors in advanced gastric cancer

Abstract:Abstract The identification of predictive biomarkers or models is necessary for the selection of patients who might benefit the most from immunotherapy. Seven histological features (signet ring cell [SRC], fibrous stroma, myxoid stroma, tumor-infiltrating lymphocytes [TILs], necrosis, tertiary lymphoid follicles, and ulceration) detected in surgically resected tissues ( N = 44) were used to train a model. The presence of SRC became an optimal decision parameter for pathology alone (AUC = 0.78). Analysis of differentially expressed genes (DEGs) for the prediction of genomic markers showed that C-X-C motif chemokine ligand 11 ( CXCL11 ) was high in responders ( P < 0.001). Immunohistochemistry (IHC) was performed to verify its potential as a biomarker. IHC revealed that the expression of CXCL11 was associated with responsiveness ( P = 0.003). The response prediction model was trained by integrating the results of the analysis of pathological factors and RNA sequencing (RNA-seq). When trained with the C5.0 decision tree model, the categorical level of the expression of CXCL11 , a single variable, was shown to be the best model (AUC = 0.812). The AUC of the model trained with the random forest was 0.944. Survival analysis revealed that the C5.0-trained model (log-rank P = 0.01 for progression-free survival [PFS]; log-rank P = 0.012 for overall survival [OS]) and the random forest-trained model (log-rank P < 0.001 for PFS; log-rank P = 0.001 for OS) predicted prognosis more accurately than the PD-L1 test (log-rank P = 0.031 for PFS; log-rank P = 0.107 for OS).
