Abstract:Objective: The purpose of this study was to describe variations in blood-based resuscitation in an injured cohort. We hypothesize that distinct transfusion trajectories are present. Background: Retrospective studies of hemorrhage utilize the concept of massive transfusion, where a set volume of blood is required. Patterns of hemorrhage vary and massive transfusion does little to describe these differences. Methods: Patients were prospectively included from June 2012 to 2013. Time of transfusion for each packed red blood cell (PRBC) transfused was recorded, in minutes, for all patients. Additional measures included demographic and injury data, admission laboratory values, and vital signs and outcomes including mortality, tempo of transfusion, and operative requirements. Group-based trajectory modeling was utilized to describe transfusion trajectories throughout the cohort. Results: Three hundred sixteen patients met the inclusion criteria. Among them, 72% were men and median age was 35 years (interquartile range [IQR] 24–50), median injury severity score was 13 (IQR 9–22), median 24-hour transfusion volume was 4 units of PRBCs (IQR 2–8), and mortality was 14%. Six transfusion trajectories were identified. Among the patients, 35% received negligible transfusions (group 1). Groups 2 and 3 received greater than 15 units PRBCs—the former as early resuscitation, whereas the latter intermittently throughout the day. Groups 4 and 5 had similar small resuscitations with distinct demographic differences. Group 6 suffered blunt injuries and required rapid resuscitation. Conclusions: Traditional definitions of massive transfusion are broad and imprecise. In cohorts of severely injured patients, there are distinct, identifiable transfusion trajectories. Identification of subgroups is important in understanding clinical course and to anticipate resuscitative and therapeutic needs.
The Sector Analysis as a Coastal Management Tool for Sustainable Tourism Development on the Mediterranean Coast of Morocco

Abstract:Beaches are ecologically valuable ecosystems and sites that attract many tourists from all over the world, therefore, knowledge of their environmental conditions to establish sound management strategies is of extreme relevance. This study aims to assess and classify 50 beaches through an innovative coastal management approach called "Sector Analysis", which integrates Litter grading, the coastal scenic quality and beach typology in order to classify sites into one of three sectors: Green (high value sites), Red (low value sites) and Yellow (sites with contradictory values). Litter Grade makes it possible to classify a site according to the quantity of litter using four grades from "A" (low) to "D" (high amount). The Coastal Scenic Evaluation System (CSES) allows to classify sites into five classes, ranging from extremely attractive natural sites (Class I) to unattractive degraded and urbanized sites (Class V). This innovative methodology for sustainable coastal area management can be easily applied to any beach in the world. The results on Litter Grade and CSES considered in this paper were obtained from previous studies and used to obtain the Sector Analysis that showed only 8 sites (16%) are in the Green sector, 18 sites (36%) in the Red sector and 24 sites (48%) in the Yellow sector. The significant percentage of sites in the Red sector (one third of the sites studied) clearly indicates the degradation that the Moroccan Mediterranean coast has undergone due to considerable anthropogenic activities and the lack of adequate coastal management programs. In this study, various management interventions were proposed to conserve and improve the aesthetic quality of beaches and reduce the impact and presence of litter in the coastal areas.

Abstract:In Africa, Morocco is the 10th largest producer of plastic. The severity of this plastic has attracted increasing amounts of attention in the Moroccan Atlantic and Mediterranean in recent years. However, at the national level, there is limited knowledge of plastic pollution. To obtain an exhaustive and comprehensive evaluation of plastic pollution levels in Morocco, large-scale monitoring is needed on all the coasts of the country. In this context, this paper examined the composition, abundance, distribution, source and quality of beaches on two Moroccan coasts using four beach quality indices along 29 beaches. During two seasons, a total of 72,105 items were counted. The mean litter abundance was 0.31 items/m 2, and the Mediterranean beaches were more dense than the Atlantic beaches. In particular, litter density was greater in spring (0.35 items/m 2 ) than in summer (0.29 items/m 2 ). The data indicate considerable differences in the density of marine debris according to the seasonality, beach typology and presence of rivers. Hazardous litter items were collected along both Moroccan coasts, constituting 8.41 % of the total collected items, with a mean of 0.026 items/m 2 . The use of environmental indices allowed us to classify Moroccan beaches as "moderate cleanliness", "moderate abundance" of plastics, "moderately safe" presence of hazardous litter and "mediocre" environmental status. The findings of the present study indicate that the sources of litter on both Moroccan coasts come mainly from recreational activities and dumping. The waste management practices recommended for Moroccan beaches include reducing sources, mitigating mitigation measures and changing littering behavior.

Abstract:Coastal tourism's surge raises concerns for Morocco's Agadir marine environment, notably with Taghazout Bay's impact. Our study assesses Taghazout's health, employing a comprehensive approach. Our study evaluates Taghazout's health, adopting a comprehensive approach covering physicochemical, microbiological aspects, macrobenthic fauna, metal pollution, and biomarkers in D. trunculus mollusks. Seawater quality aligns with Moroccan standards, indicating good bathing water. The intertidal zone hosts ten species, dominated by D. trunculus. Biomarker responses in D. trunculus suggest chemical stress. Land-use maps expose significant changes driven by the Taghazout Bay project, impacting approximately 37.99 % of the landscape. Construction activities notably encroached upon the Arganeraie and the coastal zone, creating a stark contrast from 2003. These findings form a crucial database for future studies, contributing significantly to environmental management and sustainable development, aiding informed decision-making and effective coastal ecosystem preservation strategies.

Conflict Between Coastal Tourism Development and Sustainability : case of Mostaganem , Western Algeria

Abstract:Dependency on oil and gas incomes has urged Algeria to support tourism development as a means of creating other sources of wealth. However, this is not done without environmental damages. Indeed, the insufficient number of host accommodations at the coastal areas has led to chaotic construction projects. Mostaganem is an example. It attracts recently 11 million tourists yearly, although it lacks in host accommodation. To overcome this issue, new tourist infrastructures close to the shoreline and among the coastal dunes have been programed regardless of the Littoral law that protects the coastal strip (300 m). Moreover, the wastewater issue is neither discussed nor solved, in spite of a lack of wastewater treatment plants in this area. This work is based on data collected from the Algerian Government, and the Ministry of Land-use Planning, Tourism, and Handicraft. Shore urbanization endangers the sustainability of the beaches as a socioeconomic resource. Seaside tourism could be developed without any beach urbanization by constructing in the nearby coastal rural municipalities. Tourists would enjoy the beach during the day and would return to their accommodation in the rural municipality. This would benefit economically both the local population and the environment.

Abstract:The degradation of coastlines is a result of global warming, which is closely accompanied by rising sea and ocean levels, as well as anthropogenic activities. The systematic fixation of mobile dunes has proven to be a positive environmental practice thus far. To accurately assess and predict future trends, it is essential to analyse the position of the coastline and understand its dynamics over time. In line with this goal, a study was conducted to investigate the decadal evolution of the sand shoreline along a 25 km stretch of the high-energy low-coast of Essaouira (Morocco, NW Africa). Landsat satellite images were used spanning a period of 30 years (1990, 2000, 2010, and 2020) and employed the digital shoreline analysis system model (DSAS) in conjunction with morpho-sedimentary dynamics analysis. Additionally, Google Earth satellite images from 2007 to 2022 were consulted to gain insights into the cyclical morpho-sedimentary processes that affect the beach/dune system. The findings revealed that the Essaouira coast has experienced landward migration of the coastline, leading to the retreat of beach/dune systems. Transgressive dunes, which had been fixed previously, were reactivated as part of this process, contributing to the morpho-sedimentary impacts observed along the coast.

Abstract:Urbanization sprawls at the expense of the surrounding land corresponding generally to agricultural land or natural areas. Therefore, evaluating land use and land cover change (LULCC) is important. In this study, LULCC and its environmental impact in Béni-Mellal town and the surrounding communes (Morocco) are investigated between 2002 and 2016 using remote sensing, geographical information system (GIS), and fuzzy analytical hierarchy process (FAHP). Remotely sensed imagery combined with GIS served to evaluate the typical LULCC dynamics. FAHP allowed calculating the weights of each LULC class used to generate the map of the environmental impact of LULCC. The results showed that arable fields, built-up and bare soil classes are increased about 2.75%, 177.81%, and 4.47%, respectively, while arboriculture and forest LULC classes are decreased about 40.64% and 53.85%, respectively. The environmental degradation map produced with regard to the LULCC indicated that the lands severely degraded are those transformed from arboriculture to built-up and bare soil, followed by those transformed from arable fields to built-up and bare soil, while moderate degradation is marked in areas of arboriculture transformed to arable fields. To mitigate the degradation of natural resources such as fertile agricultural soil and fruit trees, we suggest that urban construction should be encouraged in hilly areas in the mountain communes, and the regional and communal management plans must be developed and respected. As highlighted by this study, combining remote sensing and GIS could be an effective approach to LULC planning and improving the environment.

Abstract:Background. The Client-centred Strategies Framework consists of strategies for facilitating therapists' application of client-centred approaches. Purpose. The purpose of this study was to explore the application of the strategies and the utility of the framework to implement client-centred approaches. Methods. The study used a sequential mixed-methods procedure. The quantitative phase consisted of a survey of 230 Canadian occupational therapists. The qualitative phase consisted of telephone focus groups with a sample of 14 Canadian respondents to the initial survey. Findings. Results indicated that occupational therapists experience challenges in implementing strategies, particularly related to community organizing, coalition advocacy, and political action. Therapists identified multiple factors that influenced the implementation of strategies and ways of incorporating strategies into daily practice. The Client-centred Strategies Framework was viewed as a useful tool for increasing dialogue about occupational therapists' role in client-centred practice. Implications. The results of this study encourage an expanded view of client-centred strategies and the application of strategies to daily practice.

Abstract:The Red Sea coastal zone is characterized by its sensitive, fragile, unique natural resources and habitats. In the Hurghada coastal region, major changes in the tourism industry have taken place in the last few decades. The detection of environmental changes, in a selected site of the Red Sea coastal zone, will be helpful to protect and develop this coastal environment. A methodology for separating natural and man-made changes in satellite images was developed. It was based on the following assumptions: (1) slow changes, which occur within the range of the class reflectance, represent a natural change rather than an anthropogenic one; (2) natural changes tend to be in the same land-use/land-cover class in each date, i.e. slow changes in the reflectance, not leading to changes in the type of land-use/land-cover class from the master image to the destination one; and (3) rapid changes in the reflectance of the Earth's objects are usually related to anthropogenic activities. This technique is used to identify and assess changes along the coast of Hurghada and Ras Abu Soma, the Red Sea. Results indicate serious human impacts and the necessity for control measures and monitoring. Recommendations are presented.

Abstract:Coastal waters are confronted with multiple stressors, including chemical contamination/pollution and climate change. Water quality along the Atlantic coast of Morocco was assessed, in this study, at six stations over four seasons, including three heavily industrialized stations (S2, S4, and S6) and three clean stations (S1, S3, and S5), spreading over a distance of ∼300 km from Azemmour to Essaouira cities. The physicochemical characteristics (temperature, salinity, pH, dissolved oxygen, suspended matter, and sulfate), and chlorophyll-a, as well as nutrients (nitrite, nitrate, ammonium, total phosphorus, orthophosphate, and silicate) were analyzed to investigate their spatial and seasonal trends in the study areas. The results revealed significant spatiotemporal variations in the distribution of the hydrological characteristics across the sampling sites. In addition, the observed sulfate and nutrient concentrations showed significant spatial and seasonal differences. The disturbed stations showed the highest concentrations of nutrients, while the clean stations exhibited the highest concentrations of chlorophyll-a, particularly in the spring season. As expected, the compressive pollution index (CPI) and water pollution index (WPi) results supported the pollution status of stations. Accordingly, the surface seawater quality was better in the clean stations compared to the polluted stations. According to the CPI and WPi values, the sampling stations were classified in the following descending order of pollution, from strongly and moderately polluted stations to slightly or unpolluted stations (S4 > S2 > S6 > S1> S3 > S5). The total phosphorus, orthophosphate, sulfate, and chlorophyll-a concentrations were substantially higher than the surface water quality standards of Morocco, especially at the most industrialized stations in the winter.The physicochemical, biological, and nutrient data obtained in this study provide a valuable reference for assessing the ecological quality status of the Atlantic coastal environments in the study areas.

Abstract:This review provides a critical analysis of all the available literature on plastic pollution in Morocco's marine environment between 2014 and 2023. It discusses the abundance, composition, and sources of plastic litter, the adopted sampling methods, and characterizations, as well as the impact on the biota and ecosystem services. Moreover, it also covers national marine litter management policies. Our analysis reveals that, up to this point, there have been limited studies conducted and a noticeable scarcity of data, particularly pertaining to the prevalence of microplastics in marine biota, sediment and water. While many of the studies have been conducted in diverse coastal regions of the Mediterranean (60 %), the Atlantic coasts remain largely unexplored (40 %). Data from various studies indicate that plastic litter persists as a significant issue along Moroccan coasts (53–94 % of total litter collected), with reported litter (0.58 items/m2) and microplastic densities (7680 MPs/kg to 34,200 MPs/kg) generally surpassing those in neighboring countries. Plastic litter poses a considerable threat to Moroccan biodiversity, primarily through ingestion (16 species) and entanglement (18 species). This review can serve as a valuable resource to inform strategies aimed at reducing the leakage of plastic litter into aquatic environments, as well as to guide future research.

Soil erosion and hydroclimatic hazards in major African port cities: the case study of Tangier

Abstract:Land degradation and soil erosion are becoming increasingly problematic in Africa's rapidly developing urban areas, particularly in Major Port Cities. Uncontrolled expansion and human pressures are hindering planning, adaptation, and conservation efforts. To understand the extent of these issues, this study combined morphometric analysis, soil loss calculation, field monitoring, and remote sensing and GIS tools to assess soil erosion in the Metropolis of Tangier (Morocco) located at the confluence of the Mediterranean Sea and the Atlantic Ocean at the Strait of Gibraltar. The study relied on data from 13 rain gauge stations, official reports, and remote sensing acquisitions, as well as field observations. Results showed an average soil erosion rate of 24.2 t/ha/year, equivalent to an annual soil loss of 588,051 t/year. This high rate was largely due to areas with a high erosion risk (99.8%), covering only 8.3% of the territory, which were characterized by recently burned topsoil, fallow land, and steep slopes. These areas included both uncontrolled neighbourhoods and areas for planned urban and industrial expansion, posing a threat to the landscape's sustainability and socio-economic prospects. The morphometric analysis revealed its high vulnerability to erosion and degradation, with the highest soil loss rates observed in the eastern and western regions. The study also found that flash floods caused by hydroclimatic hazards can lead to significant damage to infrastructure and equipment, particularly in western sub-basins and mountainous regions. In conclusion, the use of remote sensing and GIS technologies provided valuable insights into the physical characteristics and vulnerability of the Tangier Metropolis to land degradation and soil erosion. These findings emphasize the need for effective land management practices and conservation measures to mitigate the impacts of land degradation and soil erosion in the face of climate change. This information is crucial for decision-makers and stakeholders to develop strategies to address these pressing issues.

Abstract:Groundwater remains the main water resource for consumption of potable water, irrigation in the agricultural field, and for the use in industrial and touristic domains by inhabitants of cities and villages in the coastal areas with arid and semi-arid climate. In order to protect the groundwater table of the coastal zone of Essaouira, located in the west central part of Morocco, a study of the intrinsic vulnerability to pollution was tackled by the use of the Geographic Information Systems (GIS) from the DRASTIC method. This tool serve to highlight the overexploitation problems and water quality degradation and help decision makers to take good decisions to solve them. Vulnerability cards are fundamental documents for spatial planning. They represent excellent support for the establishment of all kinds of human activities taking into account the pollution of underground waters. The analysis of the vulnerability map for the pollution of the Plio-Pleistocene aquifer of the coastal zone of Essaouira, shows a heterogeneous variation in vulnerability across the area. With values of the DRASTIC index (ID) range from 45 to 149 spatially distributed in the study area, the vulnerability map established shows four (04) vulnerability classes: low (45–77), medium (78–92), high (93–112), and very high (113–149). Analysis of this map shows a dominance of low and medium vulnerability zones in most of the water table, with the percentage of each class is as follows: low (18%), medium (53%), high (22%), and very high (7%); this vulnerability to pollution calculated by the standard DRASTIC method is essentially dependent on the depth of the groundwater table and the unsaturated zone. These two parameters have a very important influence compared to other parameters that make up this method. The most dominant class is the low and middle class (71%), which represents almost the entire plain and corresponds to uncertain hydrogeological protection of groundwater by considering the hydrogeological vulnerability scale as certainly well protected.

Abstract:Coastal lagoons are valuable ecosystems, providing socioeconomic benefits and supporting human populations and biodiversity. However, these systems face several challenges, making them vulnerable to both natural and human factors. In this study, we apply the Driver-Pressure-State-Impact-Response (DPSIR) Approach to conduct a comprehensive socioeconomic and environmental assessment of the Khenifiss Lagoon to promote sustainable development and support decision-makers. Located on the southern Atlantic coast of Morocco, the lagoon was designated a natural reserve in 1962, a biological reserve in 1983, and a protected wetland under the Ramsar Convention since 1980. This study represents the initial endeavor to conduct a comprehensive global and multidisciplinary environmental assessment of the lagoon by using a wide range of data sources, including relevant publications and reports, satellite images and remote sensing data, field observations, and interviews, all analyzed under the DPSIR framework. Our findings show that both natural and human factors have an impact on the ecosystem. Natural Factors associated with the geomorphological features of the region likely contribute to the silting of the lagoon, possibly intensified by a large shipwreck stuck at its inlet. Meanwhile, human factors encompass population growth (at a rate of 2% per year), tourism, shellfish farming, fishing, shellfish harvesting, and salt extraction. Our results reveal significant changes in the lagoon's condition in recent years, including a reduction in water body extent, a probable decrease in depth, and an increase in the accumulation of solid waste, plastics, and wastewater in three sectors spanning a total surface area of 464 ha (equivalent to 7% of the lagoon), a substantial expansion of the salt mining area encompassing 368 ha, and a remarkable loss of biodiversity, manifested in declining fish stocks and seabird populations. This study showed that the lagoon is positioned as a potential site for economic growth and serves to alert stakeholders and the local population to the ecosystem's environmental issues. Based on the findings of this study, we highly recommend regulating human activities within the lagoon, the removal of the wreck at the entrance, proper waste management, community awareness programs, and strict monitoring and enforcement of regulations to protect the environment.

Abstract:In a prospective trial in 124 patients with acute myocardial infarction, Holter and surface ECG recordings were obtained simultaneously and compared for their ability to assess thrombolysis-induced ST segment changes. Accuracy in predicting patency of the infarct-related artery was evaluated in both methods. Success or failure of thrombolysis was determined angiographically 90 min after the start of therapy. For both methods, sensitivity, specificity, and positive predictive value for correct prediction of the perfusion status ranged between 64% and 92%. However, the negative predictive value was considerably lower (40-53%). There were no significant differences in any parameter evaluated for either method. Thus, two surface ECG recordings before and 2 h after the start of therapy yield the same predictive value as continuous Holter monitoring with respect to thrombolysis-induced coronary artery reperfusion. However, for triage of patients to early coronary interventions, more sophisticated methods are needed for non-invasive prediction of coronary artery patency due to the low negative predictive value of ST segment analysis.

Abstract:The quality of water in marine environments is of paramount importance, particularly for aquatic biodiversity and public health, especially during the summer season. With the sustained port activity of the Port of Oran, rapid degradation of water quality is a major concern, as pollution can spread under the influence of waves. The objective of this study was to assess the water quality of the port using remote sensing data for the period 2016–2023. For this purpose, data from the Sentinel-3 OLCI sensor, specifically Oa03, Oa04, Oa05, and Oa07, were utilized, followed by a validation process of reliability through the NDCI and chlorophyll-a concentrations (GCOM). The findings revealed that the Port of Oran exhibits relatively clean water quality with low nutrient levels and is characterized by an oligotrophic trophic state (< 2 mg/m3). However, careful monitoring of concentration variations, especially from 2020 onward, is necessary. The continuous evolution of concentrations suggests a possible deterioration of water quality toward an "oligotrophic to mesotrophic" state or even more. Additionally, with correlation coefficients of 18.5%, 12%, 0.1%, and 3.1% for Oa03, Oa04, Oa05, and Oa07, respectively, it is likely that the observed pollution stems directly from port activities and other anthropogenic sources rather than from water temperature variations. These results can provide an essential foundation for guiding environmental management policies and marine resource preservation initiatives.

Abstract:This study underscores the importance of accounting for spatial dimensions and temperatures in evaluating payments and environmental interactions. Exploiting natural spaces can foster sustainable development, especially in vulnerable areas, but urbanization poses significant challenges. The Mostaganem Plateau, spanning 78,100 hectares with diverse landscapes, is highly sensitive to environmental pressures like abrasion. Urbanization, agriculture, industry, deforestation, and heavy port activities threaten its biodiversity. Using the plateau as a diagnostic tool helps identify the main causes of its deterioration. Analyzing agricultural and industrial impacts on soil and water highlights the severity of the situation. These analyses, along with in-depth geographic studies, reveal the complex decomposition processes driven by natural and human factors. Implementing effective political policies and management practices is crucial to counter negative influences and promote sustainable development. Protecting the Mostaganem Plateau's fragile ecosystem requires vigilant efforts from the scientific community and social and economic stakeholders. Addressing environmental and ecological impacts is essential for long-term development. Practical responses are needed to mitigate severe degradation and preserve the plateau's environmental integrity for future generations. The study emphasizes the urgent need for monitoring and intervention to safeguard this vulnerable region, ensuring its sustainability and resilience against ongoing threats. Anthropogenic Microparticles Abundance in Sandy Beach Sediments along the Tetouan Coast (Morocco Mediterranean)

Abstract:Despite the widespread presence of anthropogenic microparticles (AMs) in beach sediments, research on their occurrence on Moroccan Mediterranean beaches is still limited. This study is the first report on AM pollution in four sandy beaches along the Tetouan coast (Morocco Mediterranean). The findings reveal an average AM concentration of 483.12 ± 157.04 AMs/kg of beach sediment. The most common AM types were fibers (75.54%) and fragments (24.06%). AMs were predominantly black, red, and blue, measuring between 0.1 and 1 mm. The evaluation of the anthropogenic microparticles pollution index (AMPI) and the coefficient of anthropogenic microparticles impact (CAMI) for the study area indicated a “very high abundance” of AMs and an “extreme” level of impact. The polymers identified in these areas included PS, PE, PP, and PET. Tourism, fishing, domestic activities, and poor solid waste management practices are the primary sources of AM pollution in this region. To protect Moroccan beaches, the implementation of a consistent plastic waste management strategy is recommended.
