Abstract:The progress in medicine has contributed to the fact that for 50 years life expectancy in Germany has increased by three to four years per decade. The number of older people has considerably increased and at the same time there has been a decline in the birthrate with the effect that the portion of the people over 60 years, now a quarter of the population, will rise to a third of the population in 2040. The dual relationship between patient and physician has changed due to the extension of the statutory health insurance to more than 90% of the population. Today collective contracts made with the statutory health insurance determine the preconditions for entitlement, but often with uncertain legal concepts. The expenditure which occurred due to the demographic and epidemiologic changes caused the limitations of the contribution rates by the legislator. The health insurance funds have implemented further regementations by means of health care duties and action for recourse in case of exceeding the budget. Thus the politically intended limitation of the resources can bring the physician in a decision emergency condition or even in an ethical dilemma. He has to observe legal rules, especially in the field of the criminal law and the liability law. Thus the financial, organizational and structural deficits in the health care system that are resulting from the social legislation are shifted by means of the liability law to the individual physician or to other service providers. Even treatment guidelines cannot protect them from this because the "standard patient" does not exist. What we really need are appropriate reforms including adjustments of the insurance structures which offer the insured alternatives: Expansion of the financial elements of the statutory health insurance Adjustments of the health care structures with integration between the outpatient and inpatient sector Restriction of the legislator to the definition of framework provisions and service limits Orientation of the health care necessities to the health care needs of the patients Economy must not dominate over humanity Adjustment of the social legislation and the liability law Settlement of the details by the joint self-administration of physicians and health insurance funds.

Abstract:Up to the early 1990's the health care system was essentially characterised through:--the insured' right of choice of therapist,--therapeutic freedom of patients and physicians, and--the freedom of establishment for medical doctors.--In accordance with the Hospital Funding Act the hospital system was--in compliance with federal constitutional law using capacity requirements--based on the "fire-fighting" principle, i.e. that if required, every patient should have access to a suitable hospital bed within about 15 minutes.--The responsibility for ensuring the provision of general and specialist health care services had been conferred by the government to the National Association of Statutory Health Insurance Physicians (1955) in the legal form of a public corporation. In the face of a foreseeable rise in expenses as a result of advances in medicine and a higher demand for health care services because of the demographic development (long-life society) the Advisory Council for Concerted Action in Health Care concludes in its Annual Report that maintaining this level of health care for all people is not financially viable any longer. This is why the state--on the basis of the Health Care Reform Act of 2002 and the Statutory Health Insurance System Modernisation Act of 2004--retreated from the provision of services in the ambulatory and inpatient setting by privatising these sectors and by proclaiming competition (introduction of diagnosis-related groups). Presently, the once liberal performance tradition is more and more turning into a centrally planned system in the spirit of "From Therapeutic Freedom to Therapeutic Programmes". The guidelines that on the basis of the methods of evidence-based medicine were developed by the international community of physicians for the treatment of patients with defined diagnoses and intended to be decision aids for individual treatment decisions are now used to implement disease management programmes for the provision of health care to people with certain chronic diseases. Another steering instrument of the above mentioned Modernisation Act is the definition of a list stating all the benefits available under the statutory health insurance system. The assessment of the health gain of new medical drugs and other medical procedures has been assigned to the Institute for Quality and Efficiency in Health Care (IQWiG) specifically founded to that purpose. With regard to the assessment of diagnostic and therapeutic procedures the Joint Federal Committee (Gemeinsamer Bundesausschuss, G-BA) in the summer of 2005 gave itself a Code of Procedures that defines uniform cross-sector criteria for the appraisal of diagnosis and treatment. In Germany the principle of evidence-based health care has by law--and this is unique as compared to other countries--fully penetrated everyday health care where the decisions of the Joint Federal Committee in the form of directives have mandatory effect for health care providers and hence for the insured, too. This is why the German Medical Association and the National Association of Statutory Health Insurance Physicians have embarked on the implementation of the "National Programme for Disease Management Guidelines" and the "Health Services Research" Project as a means of continuously evaluating health care provision which are intended to guide the future political control of the system of statutory health insurance in terms of target-performance comparisons and for the purpose of identifying health care deficits.

Abstract:Human arylamine N-acetyltransferases 1 and 2 (NAT1 and NAT2) are polymorphic phase II xenobiotic-metabolising enzymes (XME) that acetylate arylamine compounds. They therefore play an important role in the detoxication and/or metabolic activation of certain therapeutic drugs, occupational chemicals and carcinogens. Although the use of the term "xenobiotic" implies that XME form a separate and distinct class of enzymes, possible endogenous substrates may exist. Unlike NAT2, NAT1 is produced in most tissues. Polymorphism at the NAT1 locus has been associated with the existence of at least 26 allelic variants, generating phenotypic variations in terms of NAT1 catalytic activity. This genetic variation affects the acetylator status of individuals, leading to interindividual differences in drug response and predisposition to disease in humans. Recent studies have shown that non-genetic factors may also regulate NAT1 activity at the posttranslational level, with potentially important consequences for drug toxicity. In this mini-review, we summarise what is currently known about the regulation of NAT1 activity by non-genetic factors, including substrates and oxidative stress. Recent findings presented here may account for the genotype/phenotype relationship for the NAT1 locus being less clear-cut than that for human NAT2.

Abstract:Insulin induces the expression of the 70-kDa heat shock protein (Hsp70) in rat hearts. In this study, we examined insulin- and heat shock-treated hearts for improved contractile recovery after 30 min of ischemia, activation of the heat shock transcription factor, and localization of the Hsp70 in relation to dystrophin and alpha-tubulin. Adult male Sprague-Dawley rats were assigned to groups: 1) control, 2) sham control, 3) insulin injected (200 microU/g body wt), 4) heat shock treated (core body temperature 42 degrees C for 15 min), and 5) heat shock and insulin treated. Six hours later, hearts were isolated for Langendorff perfusion to determine cardiac function, or myocardial tissues were collected and prepared for either electrophoretic mobility shift assay, Western blot analysis, or immunofluorescence microscopy. Insulin treatment with 6 h of recovery enhances postischemic myocardial recovery of contractile function and increases Hsp70 expression through activation of the heat shock transcription factor. Insulin-treated hearts had elevated levels of Hsp70, particularly in the membrane fraction. In contrast, heat-shocked hearts had elevated levels of Hsp70 in the cytosol, membrane, and pellet fractions. After insulin treatment, Hsp70 was mostly colocalized to the plasma membrane with dystrophin. In contrast, after heat shock, Hsp70 was localized mostly between cardiomyocytes in apparent vascular or perivascular elements. The localization of Hsp70 is dependent on the inducing stimuli of either heat shock or insulin treatment. The cell membrane versus vascular localization of Hsp70 suggests the interesting possibility of functionally distinct roles for Hsp70 in the heart, whether induced by insulin or heat shock treatment.

Abstract:German Health Care System, which is mainly founded on the Compulsory Health Care System imported by Bismarck dated to 1883, is a successful model most widely imitated throughout the world. However, along with the slowing down of German economic growth, the deterioration of the aging of population and the impacts of globalization, the systematic problems of financing, social solidarity and efficiency of health care provision have gradually emerged. The reform of the health care system seems inevitable and becomes the focus of the economic and social reforms implemented by the Merkel government. But there remain broad controversies as to the concrete steps to take. Based on the introduction of the status quo and the basic structure of German Health Care System, the paper analyzes the background of the health care reform carried out by the Merkel government, the different reform protocols proposed by different parties and the different notions underlying each protocol. This paper also comments on the main contents of the final act titled "Health Care Reform 2007", and points out the validity of the general aim of this reform which uses competition enhancement as the main instrument, covers unprecedented broad fields and which is oriented to multiple objectives. However, as a programme resulted from compromises by different parties, it is difficult to foresee its final result, which may not be positive.

Abstract:Industrialized countries are confronting the challenge of providing their population with accessible, high-quality, safe and efficient healthcare. They face the growing challenge of caring for an increasing number of patients suffering from chronic and complex diseases. Chronically ill patients see multiple clinicians at different healthcare sites, which increases the risks of errors and poor coordination. These experiences together with rising healthcare costs force the different healthcare systems in the industrialized countries to introduce and test new ways to offer healthcare while taking into account the entire chain of care. Disease management programmes together with institutions and structures like medical homes which are accessible and help to coordinate care make a difference for those patients. The German way to a better coordination of and a better access to healthcare is a step in the right direction, but there is a lack of data demonstrating the increase in effectiveness and efficiency. The opportunity has been missed to conduct pilot studies in order to identify measurable, reliable, and valid outcome indicators and thus create greater transparency. The question is not "whether" but "how" new ways of offering care can be introduced to increase the performance of the whole system, to decrease coordination problems, and to stabilise healthcare spending.

Abstract:Verschiedene Ansätze zur Reform des schweizerischen Gesundheitswesens werden diskutiert, ohne dass ein klarer Entscheid für eine mehr planwirtschaftliche oder eine mehr wettbewerbliche Ausrichtung vorliegt. Trotz gleichbleibendem Leistungsauftrag bezüglich Angebot, Menge und Qualität, werden die Mittel für die öffentlichen Spitäler bei zum Teil sinkenden Preisen (= Tarifen) reduziert. Das Rationalisierungspotential beim Personal ist dabei weitestgehend ausgeschöpft, weshalb nun zunehmend eine verdeckte Rationierung einsetzt. Dieser Leistungsabbau führt zu Qualitätsverlust in der Behandlung der Patienten, zu Burn-out beim Pflegepersonal und zu Frustrationen bei den verantwortlichen Kaderärzten. Die bedrohte Qualitäts- und Sicherheitslage erfordert deshalb vom Leistungserbringer Schritte in Richtung einer Qualitätstransparenz zur Monitorisierung der Folgen einer einseitigen auf Kosten fokussierten Gesundheitspolitik und die Einführung eines integralen Risikomanagements zur Gewährleistung grösstmöglicher Sicherheit für den Patienten in einem System mit beschränkten Ressourcen. Es wird zudem eine Bewertung möglicher Auswege aus der Falle der verdeckten Rationierung vorgenommen. Diskutiert werden folgende Lösungsansätze: 1. explizite Rationierung, 2. Ansätze für eine grundlegende Reform des Gesundheitswesens in der Schweiz, 3. Restrukturierung der Leistungserbringer unter Berücksichtigung von Skaleneffekten und 4. Wettbewerb. In jedem Fall aber hat sich auch die Rechtssprechung den beschränkten Mitteln anzupassen und ihre Optik sollte die politische und ökonomische Sicht der Rationierung im Gesundheitswesen, als neues Element neben der Patientensicherheit mit berücksichtigen. Die Ärzte dürfen nicht allein in die Rolle der Rationierer gedrängt werden, sie wollen und können diese Rolle nicht übernehmen. Dies ist Aufgabe der Politik.
[A paradigm change in German academic medicine. Merger and privatization as exemplified with the university hospitals in Marburg and Giessen]

Abstract:1. The intended fusion of the university hospitals Marburg and Giessen in the state of Hessia is "a marriage under pressure with uncalculated risk" (Spiegel 2005). In the present political and financial situation it hardly appears to be avoidable. From the point of the view of the faculty of medicine in Marburg it is difficult to understand, that the profits of this well guided university hospital with a positive yearly budget should go to the neighboring university hospital which still had a fair amount of deficit spending in the last years.2. Both medical faculties suffer from a very low budget from the state of Hessia for research and teaching. Giessen much more than Marburg, have a substantial need for investments in buildings and infrastructure. Both institutions have a similar need for investments in costly medical apparatuses. This is a problem, which many university hospitals face nowadays.3. The intended privatisation of one or both university hospitals will need sound answers to several fundamental questions and problems:a) A privatisation potentially endangers the freedom of research and teaching garanteed by the German constitution. A private company will undoubtedly influence by active or missing additional support the direction of research in the respective academic institution. An example is the priorisation of clinical in contrast to basic research.b) With the privatisation practical absurdities in the separation of research and teaching on one side and hospital care on the other will become obvious with respect to the status of the academic employees, the obligatory taxation (16%) when a transfer of labor from one institution to the other is taken into account. The use of rooms for seminars, lectures and bedside with a double function for both teaching, research and hospital care has to be clarified with a convincing solution in everyday practice.c) The potential additional acquisition of patients, which has been advocated by the Hessian state government, may be unrealistic, when the 4th biggest university hospital in Germany will be created by the merger. University hospitals recrute the patients for high end medicine beyond their region because of the specialized academic competence and advanced technical possibilities. Additional recruitment of patients for routine hospital can hardly be expected.d) A private management will have to consider primarily the "shareholder value", even when investing in infrastructure and buildings, as it can be expected for one partner. On the longterm this will not be possible without a substantial reduction of employees in both institutions. There are, however, also substantial efforts of some private hospital chains in clinical research, e. g. by Helios in Berlin and Rhön Gmbh at the Leipzig Heart Center.e) There is a yet underestimated but very substantial risk because of the taxation for the private owner when academic staff is transferred from the university to hospital care in their dual function as academic teachers and doctors. This risk also applies for the university if the transfer should come from hospital to the university. These costs would add to the financial burden, which has to be carried in addition to the DRGs.

Abstract:According to the Euro Health Consumer Index, the Dutch healthcare system is the champion of Europe. Yet we are living for longer, prosperity is increasing and the population is growing. If we do not continue to adjust our healthcare system to these trends, medical expenses in the Netherlands will double to € 174 billion (in 2040). We are also facing job market difficulties in healthcare. We risk a shortfall of 125 thousand employees in 2022. It is therefore time to restructure healthcare. Not tomorrow but today. Healthcare will increasingly be organised around the day-to-day lives of patients—at home or work, with offline and online healthcare seamlessly matching up to each other. The shortage of personnel also demands a different attitude to healthcare provision. As a health insurer we can help to improve healthcare, for instance by giving healthcare providers the financial capacity to organise their care differently. Or by conducting independent research so that conclusions can be drawn on the legal criterion of ‘the state of science and practice’. This is how it was possible, in conjunction with the Dutch Cardiology Centres, the Netherlands Society of Cardiology and FocusCura, to include the ‘Hartwacht’ telemonitoring service in health insurance policies.

Abstract:Im Rahmen der umfassenden Reform des Krankenhaussystems in Deutschland erschien am 29.09.2023 die Achte Stellungnahme und Empfehlung der Regierungskommission für eine moderne und bedarfsgerechte Krankenhausversorgung zum Thema der Reform und Weiterentwicklung der sog. "Psych-Fächer" (Psychiatrie, Psychosomatik und Kinder- und Jugendpsychiatrie). Diese spielten im Rahmen der geplanten Krankenhausreform trotz eines Anteils von 15% der Gesamtbetten (i. e. fast 74.000 Krankenhausbetten, davon allein in der Psychiatrie ca. 57.000) und der hohen Krankheitsprävalenz psychischer Störungen erstaunlicherweise bislang kaum eine Rolle. Abstract:Prekäre Lebenssituationen sind neben Erwerbslosigkeit, Erwerbsarmut, oft ungesicherten Wohnverhältnissen und erhöhter Morbidität nicht selten zusätzlich auch durch eine fehlende oder unzureichende Krankenversicherung gekennzeichnet. In der Bundesrepublik Deutschland lebt eine nur schwer quantifizierbare Zahl von Personen, die aus unterschiedlichen Gründen über einen nur eingeschränkten oder überhaupt keinen Krankenversicherungsschutz verfügt. Dazu zählen Deutsche, EU-Bürger*innen, Personen aus Drittstaaten, Wohnungs- und Obdachlose sowie Personen ohne Papiere. Die gesundheitliche Versorgung der Betroffenen erfolgt häufig weitgehend außerhalb des Regelsystems. Seit einigen Jahren gibt es in einigen Bundesländern und Kommunen zusätzliche Projekte, um diesen Problembereich durch Clearingstellen und/oder Kostenübernahmen für die gesundheitliche Versorgung anzugehen und Übergänge in die Regelversorgung zu schaffen. Im vorliegenden Beitrag werden fünf dieser Projekte hinsichtlich ihrer Struktur und Arbeitsweise vergleichend untersucht. Die Ergebnisse zeigen, dass vielen Menschen damit geholfen werden kann, allerdings muss für eine Deckung des hohen und weiter zunehmenden Bedarfs neben der sicher notwendigen Ausweitung und Verstetigung der Projekte auch an umfassenderen Lösungen gearbeitet werden. Gleichzeitig sollten die statistischen und epidemiologischen Datengrundlagen dringend verbessert werden. In addition to unemployment, poor living and housing conditions and increased morbidity, precarious living situations are often characterized by a lack of or inadequate health insurance. In the Federal Republic of Germany, there is a difficult-to-quantify number of people who, for various reasons, have only limited health insurance coverage or none at all. Those affected include German citizens, EU citizens, and non-EU-citizens who are frequently homeless and/or undocumented. Health care for those groups is predominantly provided outside the German health care system by voluntary non-governmental and welfare organizations. Several German states and municipalities together with non-governmental organizations have addressed this problem by establishing clearing centers often combined with cost reimbursement for medical treatment. In this article, five of these projects are analysed and compared with each other with regard to their structure and mode of operation. The results show that many people can be helped in this way, but in order to meet the high and increasing demand, effort must be put towards more comprehensive solutions as well as on the expansion and sustainability of these projects. Ungeachtet der gesundheitspolitischen Großwetterlage wird die Krankenhausreform und -vergütung die Radiologie die nächsten Jahre begleiten und herausfordern. Dabei soll die Radiologie als diagnostisches Querschnittsfach und zwingend auch in ihrer therapeutischen Funktion den Stellenwert erhalten, der ihrer Rolle in der Versorgungswirklichkeit gerecht wird. However, the prerequisite for the provision of outpatient services is that this does not lead to any impairment in the quality of care or of patient safety. The ultimate goal should not be to reduce costs but rather to maintain and, ideally, improve the quality of care. This requires that services are not just defined by the surgical procedure but also by the entire treatment chain, including, for example, psychosocial support, and are remunerated accordingly. It is particularly worrying that the final decision as to whether an outpatient operation is possible is not the responsibility of the operating unit, but of the "Medizinischer Dienst," with the corresponding options and threats of sanctions. This situation is unique internationally and requires a paradigm shift. Furthermore, structural prerequisites must be maintained which currently only exist inadequately in Germany. Since a substantial proportion of planned outpatient operations require immediate or secondary inpatient treatment, there must be a barrier-free transition between the outpatient and inpatient sectors. This will require the creation of networks between outpatient service providers and one or more hospitals that are equipped and competent to manage even complex complications. It is important to create structures that, with intensive involvement of the operating unit, include adequate preoperative evaluation and patient education as well as needs-oriented postoperative care at home. The current separation of sectors is a significant hinderance. Moreover, when expanding and promoting outpatient surgery, the aspect of training and further education of specialist staff must be taken into account, as well as cross-sectoral quality assurance. Based on a review of the international literature, this article presents 13 recommendations for adequate structures when providing outpatient services which should serve as a prerequisite for the greatest possible guarantee of patient safety. Abstract:Physicians owe their patients the necessary duty of care in both examination and treatment. A physician who disregards this basic level of care acts negligently and may be held liable for damages. The same level of duty applies to contracting physicians (Sect. 76 para. 4 SGBV). The efficiency principle of Title Five of the German Social Code (SGB V), and in particular the jurisdiction of the Federal Social Court has already taken into account the strained financial situation of the Statutory Health Insurance Systems through increasing rationing elements: Panel patients' entitlement to benefits is limited by the procurement rights of the contracting physician so that actually the two-class medicine strictly denied by politicians has become reality long since. For the physician, a new legal obligation to inform his patient arises from the divergence between the civil and criminal part of the medical statutes on the one hand and the restrictive regulations for physicians practising under SGB V, creating considerable potential for conflict the extent of which will generally remain hidden from the medically ignorant patient. In its decision of December 6, 2005, the Federal Constitutional Court (BVerfG) has set limits to the increasingly restrictive jurisdiction of the Federal Social Court referring to the welfare state principle. The dictate of humanity of Section 70 SGB V must not be degraded to the rank of a mere political statement.

Abstract:Die Ereignisse bezüglich einer Reform der Notfallversorgung überschlagen sich, und ein klares Konzept ist noch nicht erkennbar, wenngleich überfällig. Kliniken und Praxen erleben im Fachgebiet der Hals-Nasen-Ohrenheilkunde (HNO) einen deutlichen Zuwachs entsprechender Patienten, die Gründe sind mannigfaltig: von dem Bedürfnis Betroffener einer sofortigen Abklärung bestimmter Symptome bis hin zu veränderten Notdienststrukturen. Verschärft hat sich die Situation durch das jüngste Urteil zur Sozialversicherungspflicht der Poolärzte mit einer "Notbremse" im ärztlichen Bereitschaftsdienst bspw. der Kassenärztlichen Vereinigung (KV) Baden-Württemberg. Zu allem Überfluss wurde die vom Gemeinsamen Bundesausschuss erstellte Erstfassung der Richtlinie zur Ersteinschätzung des Versorgungsbedarfs in der Notfallversorgung (Ersteinschätzungsrichtlinie) durch das Bundesministerium für Gesundheit beanstandet und liegt aktuell auf Eis. Es ist allen Akteuren klar, dass in jedem Fall eine grundlegende Reform zur Filterung und Steuerung der Patientenströme notwendig wird. Die Vorschläge reichen von einer Aufklärung der Bürger zur Erlangung einer basalen Gesundheitskompetenz über die Wiedereinführung der Notfallgebühr bis hin zu den wahrscheinlichen Szenarien einer zentralen Notfallnummer, eines gemeinsamen Tresens (i.e. vertragsärztliche Notfallpraxis und entsprechende Klinik-Notfallambulanzen in unmittelbarer räumlicher Nähe) und einer Triage durch geschultes Personal mit Vergabeoptionen in die geeigneten Versorgungsbereiche. Die Regierungskommission für eine moderne und bedarfsgerechte Krankenhausversorgung hat mit Stellungnahmen zur Reform der Notfall- und Akutversorgung zu den Bereichen "Rettungsdienst und Finanzierung" und "Integrierte Notfallzentren und Integrierte Leitstellen" Empfehlungen abgegeben. Hierzu sollen a.e. im Verlauf des Jahres, nach Möglichkeit unter Beteiligung der Fachgesellschaften und Berufsverbände, belastbare Grundlagen geschaffen sein, auch verknüpft mit der Hoffnung, die dringend notwendige Entlastung von medizinischem Personal in Klinik und Praxis einzuleiten. Das vorliegende Manuskript beschreibt die gesundheitspolitische Historie sowie aktuelle Probleme der Notfallversorgung, wobei Lösungsvorschläge mit Bezug auf die HNO-ärztlichen Besonderheiten in den Fokus genommen werden. Das vorliegende Positionspapier ist verbunden mit einem Appell an Selbstverwaltung und Politik, nunmehr zeitnah ein tragfähiges Konzept für die Notfallversorgung in Kraft zu setzen, da Finanzierung und Personalverfügbarkeit immer kritischer werden und der ungeregelten Welle an Notfallpatienten eine ordnende Hand gereicht werden muss. Clinics and practices in the field of ear, nose and throat medicine (ENT) are experiencing a significant increase in the number of emergency patients, which has multiple reasons. There is broad consent that a reform of emergency structures is necessary. The government commission for modern and needs-based hospital care has made recommendations with statements on the reform of emergency and acute care in the areas of "emergency services and financing" and "integrated emergency centers and integrated control centers". For this purpose a reliable foundation will be created with participation of specialist societies and professional associations, also linked to the hope of initiating the urgently needed relief of medical staff in clinics and practices. The present manuscript describes the health policy history and current problems in emergency care, focusing on proposed solutions with reference to the special features of ENT medicine. This position paper is linked to an appeal to self-administration and politicians to quickly implement a sustainable concept for emergency care, as financing and staff availability are becoming increasingly critical and the unregulated wave of emergency patients must be given a helping hand. [Development and consequences of physician shortages in Bavaria]

Abstract:Aim: Germany's growing shortage of doctors is a current topic of numerous discussions in healthcare policy. The objective of this study is to show the medical supply situation and migratory movement of doctors in the outpatient and inpatient physician care sector, with particular emphasis on the immigration of foreign doctors to Germany and its consequences. Subjects and methods: The annual statistical reports from the Bavarian Medical Association and the Federal Medical Association were investigated with regard to foreign doctors and migratory movements. To establish Bavaria's situation regarding outpatient physician care, unpublished case reports and planning sheets of the Statutory Health Insurance Physicians in Bavaria (KVB) were analysed. A survey amongst Bavarian hospitals shows the current situation of the inpatient care sector. Results: The trend of emigration by German doctors continues unabated, especially to Switzerland, UK, USA and Austria. In Bavaria, outpatient care by GPs or specialists is still standard or in oversupply. However, the survey was able to confirm a considerable lack of doctors for inpatient care. So far, it has been possible to compensate existing staff shortages in hospitals by employing foreign doctors, despite significant language deficits. Discussion and conclusions: To resolve the shortage of doctors in future, a reform of the Medical Requirements Planning in combination with structural improvements and measures to integrate immigrant doctors is essential.
WORK–LIFE BALANCE OF GENERAL PRACTITIONERS IN GERMANY CONSIDERING RECENT REGULATIONS

Abstract:Background: In the light of demographic changes in the population and among the doctors, the German government has in the last two decades taken several regulatory steps to address work–life balance and related issues. Objective: This article seeks to identify crucial pieces of legislation that may increase the attractiveness for the GP profession in Germany. Method: The article summarizes important regulatory steps and their results in terms of increasing the attractiveness of the GP profession. Results: Some regulations in Germany including the “Vertragsarztrechtsänderungsgesetz” led to more flexibilization in terms of the location and the internal structure of GP practices, while others addressed the further digitalization. A central element for creating attractive working conditions for GPs is the possibility of selective and direct contracting between the insurance companies and the GPs, parallel to the more general collective contracting. Discussion: While some of these regulations helped to make the GP profession more attractive, they have failed to increase the number of GPs especially in rural socially deprived areas. Conclusion: The government will need to invest more resources in scientific studies (e.g. international comparison) to identifying regulatory measures that help address the growing gap between the amount of GP care needed by the aging population and the level of care provided with the current human resources.
