Abstract:Artificial intelligence (AI) refers to the area of knowledge that develops computerised models to perform tasks that typically require human intelligence. These algorithms are programmed to learn and identify patterns from “training data,” that can be subsequently applied to new datasets, without being explicitly programmed to do so. AI is revolutionising the field of medical imaging and in particular of Cardiovascular Magnetic Resonance (CMR) by providing deep learning solutions for image acquisition, reconstruction and analysis, ultimately supporting the clinical decision making. Numerous methods have been developed over recent years to enhance and expedite CMR data acquisition, image reconstruction, post-processing and analysis; along with the development of promising AI-based biomarkers for a wide spectrum of cardiac conditions. The exponential rise in the availability and complexity of CMR data has fostered the development of different AI models. Integration in clinical routine in a meaningful way remains a challenge. Currently, innovations in this field are still mostly presented in proof-of-concept studies with emphasis on the engineering solutions; often recruiting small patient cohorts or relying on standardised databases such as Multi-ethnic Study on atherosclerosis (MESA), UK Biobank and others. The wider incorporation of clinically valid endpoints such as symptoms, survival, need and response to treatment remains to be seen. This review briefly summarises the current principles of AI employed in CMR and explores the relevant prospective observational studies in cardiology patient cohorts. It provides an overview of clinical studies employing undersampled reconstruction techniques to speed up the scan encompassing cine imaging, whole-heart imaging, multi-parametric mapping and magnetic resonance fingerprinting along with the clinical utility of AI applications in image post-processing, and analysis. Specific focus is given to studies that have incorporated CMR-derived prediction models for prognostication in cardiac disease. It also discusses current limitations and proposes potential developments to enable multi-disciplinary collaboration for improved evidence-based medicine. AI is an extremely promising field and the timely integration of clinician's input in the ingenious technical investigator's paradigm holds promise for a bright future in the medical field.

Abstract:Artificial intelligence (AI) integration in cardiac magnetic resonance imaging presents new and exciting avenues for advancing patient care, automating post-processing tasks, and enhancing diagnostic precision and outcomes. The use of AI significantly streamlines the examination workflow through the reduction of acquisition and postprocessing durations, coupled with the automation of scan planning and acquisition parameters selection. This has led to a notable improvement in examination workflow efficiency, a reduction in operator variability, and an enhancement in overall image quality. Importantly, AI unlocks new possibilities to achieve spatial resolutions that were previously unattainable in patients. Furthermore, the potential for low-dose and contrast-agent-free imaging represents a stride toward safer and more patient-friendly diagnostic procedures. Beyond these benefits, AI facilitates precise risk stratification and prognosis evaluation by adeptly analysing extensive datasets. This comprehensive review article explores recent applications of AI in the realm of cardiac magnetic resonance imaging, offering insights into its transformative potential in the field.

Abstract:Technical advances in artificial intelligence (AI) in cardiac imaging are rapidly improving the reproducibility of this approach and the possibility to reduce time necessary to generate a report. In cardiac computed tomography angiography (CCTA) the main application of AI in clinical practice is focused on detection of stenosis, characterization of coronary plaques, and detection of myocardial ischemia. In cardiac magnetic resonance (CMR) the application of AI is focused on post-processing and particularly on the segmentation of cardiac chambers during late gadolinium enhancement. In echocardiography, the application of AI is focused on segmentation of cardiac chambers and is helpful for valvular function and wall motion abnormalities. The common thread represented by all of these techniques aims to shorten the time of interpretation without loss of information compared to the standard approach. In this review we provide an overview of AI applications in multimodality cardiac imaging.

Abstract:In recent years, cardiovascular imaging examinations have experienced exponential growth due to technological innovation, and this trend is consistent with the most recent chest pain guidelines. Contrast media have a crucial role in cardiovascular magnetic resonance (CMR) imaging, allowing for more precise characterization of different cardiovascular diseases. However, contrast media have contraindications and side effects that limit their clinical application in determinant patients. The application of artificial intelligence (AI)-based techniques to CMR imaging has led to the development of non-contrast models. These AI models utilize non-contrast imaging data, either independently or in combination with clinical and demographic data, as input to generate diagnostic or prognostic algorithms. In this review, we provide an overview of the main concepts pertaining to AI, review the existing literature on non-contrast AI models in CMR, and finally, discuss the strengths and limitations of these AI models and their possible future development.

Abstract:Artificial intelligence (AI) has nowadays entered the realm of cardiac computed tomography (CT) offering new tools. In this review, we focus on the application of AI in the post‐processing workflow for coronary artery calcium score, computed tomography angiography, fractional flow reserve CT, myocardial CT perfusion, and epicardial adipose tissue. In the last decade, artificial intelligence (AI) has influenced the field of cardiac computed tomography (CT), with its scope further enhanced by advanced methodologies such as machine learning (ML) and deep learning (DL). The AI‐driven techniques leverage large datasets to develop and train algorithms capable of making precise evaluations and predictions. The realm of cardiac CT is expanding day by day and multiple tools are offered to answer different questions. Coronary artery calcium score (CACS) and CT angiography (CTA) provide high‐resolution images that facilitate the detailed anatomical evaluation of coronary plaque burden. New tools such as myocardial CT perfusion (CTP) and fractional flow reserve (FFRCT) have been developed to add a functional evaluation of the stenosis. Moreover, epicardial adipose tissue (EAT) is gaining interest as its role in coronary artery plaque development has been deepened. Seen the great added value of these tools, the demand for new exams has increased such as the burden on imagers. Due to its ability to fast compute multiple data, AI can be helpful in both the acquisition and post‐processing phases. AI can possibly reduce radiation dose, increase image quality, and shorten image analysis time. Moreover, different types of data can be used for risk assessment and patient risk stratification. Recently, the focus of the scientific community on AI has led to numerous studies, especially on CACS and CTA. This narrative review concentrates on AI's role in the post‐processing of CACS, CTA, FFRCT, CTP, and EAT, discussing both current capabilities and future directions in the field of cardiac imaging.

This review explores the integration of artificial intelligence (AI) in multimodal cardiac imaging, tracing its evolution from early X-rays to contemporary techniques such as CT, MRI, and nuclear imaging. AI, particularly machine learning and deep learning, significantly enhances cardiac diagnostics by estimating biological heart age, predicting disease risk, and optimizing heart failure management through adaptive algorithms without explicit programming or feature engineering. Key contributions include AI's transformative role in non-invasive coronary artery disease diagnosis, arrhythmia detection via wearable devices, and personalized treatment strategies. Despite substantial progress, challenges including data standardization, algorithm validation, regulatory approval, and ethical considerations must be addressed to fully harness AI's potential. Collaborative efforts among clinicians, scientists, industry stakeholders, and regulatory bodies are essential for the safe and effective deployment of AI in cardiac imaging, promising enhanced diagnostics and personalized patient care.

Abstract:Healthcare, conceivably more than any other area of human endeavour, has the greatest potential to be affected by artificial intelligence (AI). This potential has been shown by several reports that demonstrate equal or superhuman performance in medical tasks that aim to improve efficiency, diagnosis and prognosis. This review focuses on the state of the art of AI applications in cardiovascular imaging. It provides an overview of the current applications and studies performed, including the potential value, implications, limitations and future directions of AI in cardiovascular imaging.It is envisioned that AI will dramatically change the way doctors practise medicine. In the short term, it will assist physicians with easy tasks, such as automating measurements, making predictions based on big data, and putting clinical findings into an evidence-based context. In the long term, AI will not only assist doctors, it has the potential to significantly improve access to health and well-being data for patients and their caretakers. This empowers patients. From a physician's perspective, reliable AI assistance will be available to support clinical decision-making. Although cardiovascular studies implementing AI are increasing in number, the applications have only just started to penetrate contemporary clinical care.

Abstract:The technological development of Artificial Intelligence (AI) has grown rapidly in recent years. The applications of AI to cardiovascular imaging are various and could improve the radiologists' workflow, speeding up acquisition and post-processing time, increasing image quality and diagnostic accuracy. Several studies have already proved AI applications in Coronary Computed Tomography Angiography and Cardiac Magnetic Resonance, including automatic evaluation of calcium score, quantification of coronary stenosis and plaque analysis, or the automatic quantification of heart volumes and myocardial tissue characterization. The aim of this review is to summarize the latest advances in the field of AI applied to cardiovascular CT and MR imaging.

Abstract:Background and introduction: Cardiac imaging is crucial for diagnosing heart disorders. Methods like X-rays, ultrasounds, CT scans, and MRIs provide detailed anatomical and functional heart images. AI can enhance these imaging techniques with its advanced learning capabilities. Method: In this scoping review, following PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analyses) Guidelines, we searched PubMed, Scopus, Web of Science, and Google Scholar using related keywords on 16 April 2024. From 3679 articles, we first screened titles and abstracts based on the initial inclusion criteria and then screened the full texts. The authors made the final selections collaboratively. Result: The PRISMA chart shows that 3516 articles were initially selected for evaluation after removing duplicates. Upon reviewing titles, abstracts, and quality, 24 articles were deemed eligible for the review. The findings indicate that AI enhances image quality, speeds up imaging processes, and reduces radiation exposure with sensitivity and specificity comparable to or exceeding those of qualified radiologists or cardiologists. Further research is needed to assess AI's applicability in various types of cardiac imaging, especially in rural hospitals where access to medical doctors is limited. Conclusions: AI improves image quality, reduces human errors and radiation exposure, and can predict cardiac events with acceptable sensitivity and specificity.

This narrative review examines the current applications and future potential of artificial intelligence (AI) and machine learning (ML) in cardiovascular imaging. We discuss the integration of these technologies across various imaging modalities, including echocardiography, computed tomography, magnetic resonance imaging, and nuclear imaging techniques. The review explores AI-assisted diagnosis in key areas such as coronary artery disease detection, valve disorders assessment, cardiomyopathy classification, arrhythmia detection, and prediction of cardiovascular events. AI demonstrates promise in improving diagnostic accuracy, efficiency, and personalized care. However, significant challenges persist, including data quality standardization, model interpretability, regulatory considerations, and clinical workflow integration. We also address the limitations of current AI applications and the ethical implications of their implementation in clinical practice. Future directions point towards advanced AI architectures, multimodal imaging integration, and applications in precision medicine and population health management. With recent advancements in computer power and the availability of digital archives, artificial intelligence (AI) is rapidly gaining traction in the field of medical imaging, including nuclear medicine and molecular imaging. However, the complex and time-consuming workflow and interpretation involved in nuclear medicine and molecular imaging, limit their extensive utilization in clinical practice. To address this challenge, AI has emerged as a fundamental tool for enhancing the role of nuclear medicine and molecular imaging. It has shown promising applications in various crucial aspects of nuclear cardiology, such as optimizing imaging protocols, facilitating data processing, aiding in CVD diagnosis, risk classification and prognosis. In this review paper, we will introduce the key concepts of AI and provide an overview of its current progress in the field of nuclear cardiology. In addition, we will discuss future perspectives for AI in this domain.

Abstract:Cardiac magnetic resonance imaging (CMR) is the gold standard for cardiac function assessment and plays a crucial role in diagnosing cardiovascular disease (CVD). However, its widespread application has been limited by the heavy resource burden of CMR interpretation. Here, to address this challenge, we developed and validated computerized CMR interpretation for screening and diagnosis of 11 types of CVD in 9,719 patients. We propose a two-stage paradigm consisting of noninvasive cine-based CVD screening followed by cine and late gadolinium enhancement-based diagnosis. The screening and diagnostic models achieved high performance (area under the curve of 0.988 ± 0.3% and 0.991 ± 0.0%, respectively) in both internal and external datasets. Furthermore, the diagnostic model outperformed cardiologists in diagnosing pulmonary arterial hypertension, demonstrating the ability of artificial intelligence-enabled CMR to detect previously unidentified CMR features. This proof-of-concept study holds the potential to substantially advance the efficiency and scalability of CMR interpretation, thereby improving CVD screening and diagnosis.

Abstract:Artificial Intelligence (AI) is a broad discipline of computer science and engineering. Modern application of AI encompasses intelligent models and algorithms for automated data analysis and processing, data generation, and prediction with applications in visual perception, speech understanding, and language translation. AI in healthcare uses machine learning (ML) and other predictive analytical techniques to help sort through vast amounts of data and generate outputs that aid in diagnosis, clinical decision support, workflow automation, and prognostication. Coronary computed tomography angiography (CCTA) is an ideal union for these applications due to vast amounts of data generation and analysis during cardiac segmentation, coronary calcium scoring, plaque quantification, adipose tissue quantification, peri-operative planning, fractional flow reserve quantification, and cardiac event prediction. In the past 5 years, there has been an exponential increase in the number of studies exploring the use of AI for cardiac computed tomography (CT) image acquisition, de-noising, analysis, and prognosis. Beyond image processing, AI has also been applied to improve the imaging workflow in areas such as patient scheduling, urgent result notification, report generation, and report communication. In this review, we discuss algorithms applicable to AI and radiomic analysis; we then present a summary of current and emerging clinical applications of AI in cardiac CT. We conclude with AI's advantages and limitations in this new field.

Abstract:During a 2 years period the serological markers of viral infections were evaluated in the semen of 24 donors who were involved in assisted reproduction techniques. The presence of cytomegalovirus, hepatitis B virus and human immunodeficiency virus were investigated. The results were as expected, no data of viral infection at the time of donation being found because the study population was a highly selected one. Taking into account all the known microorganisms that can be transmitted through semen, a protocol for the prevention of infections in the mother and in the prospective newborn is suggested.

Abstract:Introduction: Cardiac magnetic resonance (CMR) is of diagnostic and prognostic value in a range of cardiopulmonary conditions. Current methods for evaluating CMR studies are laborious and time-consuming, contributing to delays for patients. As the demand for CMR increases, there is a growing need to automate this process. The application of artificial intelligence (AI) to CMR is promising, but the evaluation of these tools in clinical practice has been limited. This study assessed the clinical viability of an automatic tool for measuring cardiac volumes on CMR. Methods: Consecutive patients who underwent CMR for any indication between January 2022 and October 2022 at a single tertiary centre were included prospectively. For each case, short-axis CMR images were segmented by the AI tool and manually to yield volume, mass and ejection fraction measurements for both ventricles. Automated and manual measurements were compared for agreement and the quality of the automated contours was assessed visually by cardiac radiologists. Results: 462 CMR studies were included. No statistically significant difference was demonstrated between any automated and manual measurements ( p > 0.05; independent T -test). Intraclass correlation coefficient and Bland-Altman analysis showed excellent agreement across all metrics (ICC > 0.85). The automated contours were evaluated visually in 251 cases, with agreement or minor disagreement in 229 cases (91.2%) and failed segmentation in only a single case (0.4%). The AI tool was able to provide automated contours in under 90 s. Conclusions: Automated segmentation of both ventricles on CMR by an automatic tool shows excellent agreement with manual segmentation performed by CMR experts in a retrospective real-world clinical cohort. Implementation of the tool could improve the efficiency of CMR reporting and reduce delays between imaging and diagnosis.

Abstract:Background: Artificial intelligence (AI) can radically change almost every aspect of the human experience. In the medical field, there are numerous applications of AI and subsequently, in a relatively short time, significant progress has been made. Cardiology is not immune to this trend, this fact being supported by the exponential increase in the number of publications in which the algorithms play an important role in data analysis, pattern discovery, identification of anomalies, and therapeutic decision making. Furthermore, with technological development, there have appeared new models of machine learning (ML) and deep learning (DP) that are capable of exploring various applications of AI in cardiology, including areas such as prevention, cardiovascular imaging, electrophysiology, interventional cardiology, and many others. In this sense, the present article aims to provide a general vision of the current state of AI use in cardiology. Results: We identified and included a subset of 200 papers directly relevant to the current research covering a wide range of applications. Thus, this paper presents AI applications in cardiovascular imaging, arithmology, clinical or emergency cardiology, cardiovascular prevention, and interventional procedures in a summarized manner. Recent studies from the highly scientific literature demonstrate the feasibility and advantages of using AI in different branches of cardiology. Conclusions: The integration of AI in cardiology offers promising perspectives for increasing accuracy by decreasing the error rate and increasing efficiency in cardiovascular practice. From predicting the risk of sudden death or the ability to respond to cardiac resynchronization therapy to the diagnosis of pulmonary embolism or the early detection of valvular diseases, AI algorithms have shown their potential to mitigate human error and provide feasible solutions. At the same time, limits imposed by the small samples studied are highlighted alongside the challenges presented by ethical implementation; these relate to legal implications regarding responsibility and decision making processes, ensuring patient confidentiality and data security. All these constitute future research directions that will allow the integration of AI in the progress of cardiology.

An AI tool for automated analysis of large-scale unstructured clinical cine CMR databases

Abstract:Artificial intelligence (AI) techniques have been proposed for automating analysis of short axis (SAX) cine cardiac magnetic resonance (CMR), but no CMR analysis tool exists to automatically analyse large (unstructured) clinical CMR datasets. We develop and validate a robust AI tool for start-to-end automatic quantification of cardiac function from SAX cine CMR in large clinical databases. Our pipeline for processing and analysing CMR databases includes automated steps to identify the correct data, robust image pre-processing, an AI algorithm for biventricular segmentation of SAX CMR and estimation of functional biomarkers, and automated post-analysis quality control to detect and correct errors. The segmentation algorithm was trained on 2793 CMR scans from two NHS hospitals and validated on additional cases from this dataset (n=414) and five external datasets (n=6888), including scans of patients with a range of diseases acquired at 12 different centres using CMR scanners from all major vendors. Median absolute errors in cardiac biomarkers were within the range of inter-observer variability: <8.4mL (left ventricle volume), <9.2mL (right ventricle volume), <13.3g (left ventricular mass), and <5.9% (ejection fraction) across all datasets. Stratification of cases according to phenotypes of cardiac disease and scanner vendors showed good performance across all groups. We show that our proposed tool, which combines image pre-processing steps, a domain-generalisable AI algorithm trained on a large-scale multi-domain CMR dataset and quality control steps, allows robust analysis of (clinical or research) databases from multiple centres, vendors, and cardiac diseases. This enables translation of our tool for use in fully-automated processing of large multi-centre databases.

Abstract:Artificial Intelligence (AI), through deep learning, has brought automation and predictive capabilities to cardiac imaging. However, despite considerable investment, tangible health-care cost reductions remain unproven. Although AI holds promise, there has been insufficient time for both methodological development and prospective clinical trials to establish its advantage over human interpretations in terms of its effect on patient outcomes. Challenges such as data scarcity, privacy issues, and ethical concerns impede optimal AI training. Furthermore, the absence of a unified model for the complex structure and function of the heart and evolving domain knowledge can introduce heuristic biases and influence underlying assumptions in model development. Integrating AI into diverse institutional picture archiving and communication systems and devices also presents a clinical hurdle. This hurdle is further compounded by an absence of high-quality labelled data, difficulty sharing data between institutions, and non-uniform and inadequate gold standards for external validations and comparisons of model performance in real-world settings. Nevertheless, there is a strong push in industry and academia for AI solutions in medical imaging. This Series paper reviews key studies and identifies challenges that require a pragmatic change in the approach for using AI for cardiac imaging, whereby AI is viewed as augmented intelligence to complement, not replace, human judgement. The focus should shift from isolated measurements to integrating non-linear and complex data towards identifying disease phenotypes-emphasising pattern recognition where AI excels. Algorithms should enhance imaging reports, enriching patients' understanding, communication between patients and clinicians, and shared decision making. The emergence of professional standards and guidelines is essential to address these developments and ensure the safe and effective integration of AI in cardiac imaging.

Abstract:Recent advancements in Artificial Intelligence (AI) have significantly influenced the field of Cardiovascular Disease (CVD) analysis, particularly in image-based diagnostics. Our paper presents an extensive review of AI applications in image-based CVD analysis, offering insights into its current state and future potential. We systematically categorize the literature based on the primary anatomical structures related to CVD, dividing them into non-vessel structures (such as ventricles and atria) and vessel structures (including the aorta and coronary arteries). This categorization provides a structured approach to explore various imaging modalities like Magnetic Resonance Imaging (MRI), which are commonly used in CVD research. Our review encompasses these modalities, giving a broad perspective on the diverse imaging techniques integrated with AI for CVD analysis. Additionally, we compile a list of publicly accessible cardiac image datasets and code repositories, intending to support research reproducibility and facilitate data and algorithm sharing within the community. We conclude with an examination of the challenges and limitations inherent in current AI-based CVD analysis methods and suggest directions for future research to overcome these hurdles.

Abstract:Background: Accurate measurements from cardiovascular magnetic resonance (CMR) images require precise positioning of scan planes and elimination of motion artifacts from arrhythmia or breathing. Unidentified or incorrectly managed artifacts degrade image quality, invalidate clinical measurements, and decrease diagnostic confidence. Currently, radiographers must manually inspect each acquired image to confirm diagnostic quality and decide whether reacquisition or a change in sequences is warranted. We aimed to develop artificial intelligence (AI) to provide continuous quality scores across different quality domains, and from these, determine whether cines are clinically adequate, require replanning, or warrant a change in protocol. Methods: A three-dimensional convolutional neural network was trained to predict cine quality graded on a continuous scale by a level 3 CMR expert, focusing separately on planning and motion artifacts. It incorporated four distinct output heads for the assessment of image quality in terms of (a, b, c) 2-, 3- and 4-chamber misplanning, and (d) long- and short-axis arrhythmia/breathing artifact. Backpropagation was selectively performed across these heads based on the labels present for each cine. Each image in the testing set was reported by four level 3 CMR experts, providing a consensus on clinical adequacy. The AI's assessment of image quality and ability to identify images requiring replanning or sequence changes were evaluated with Spearman's rho and the area under receiver operating characteristic curve (AUROC), respectively. Results: A total of 1940 cines across 1387 studies were included. On the test set of 383 cines, AI-judged image quality correlated strongly with expert judgment, with Spearman's rho of 0.84, 0.84, 0.81, and 0.81 for 2-, 3- and 4-chamber planning quality and the extent of arrhythmia or breathing artifacts, respectively. The AI also showed high efficacy in flagging clinically inadequate cines (AUROC 0.88, 0.93, and 0.93 for identifying misplanning of 2-, 3- and 4-chamber cines, and 0.90 for identifying movement artifacts). Conclusion: AI can assess distinct domains of CMR cine quality and provide continuous quality scores that correlate closely with a consensus of experts. These ratings could be used to identify cases where reacquisition is warranted and guide corrective actions to optimize image quality, including replanning, prospective gating, or real-time imaging.
