Abstract:Due to the inevitable errors introduced by means of observations and estimation algorithms, anchors' locations are usually not accurate in practical applications. This paper proposes an Expectation-Maximization (EM)-based localization algorithm with inaccurate anchors and noisy range measurements in wireless sensor networks. A circularly symmetric Gaussian distribution is used to approximate the a posteriori distribution of anchor's position uncertainty by minimizing the Kullback-Leibler (KL) divergence, building on which, we are able to derive a close-form expression of the expectation step (E-step). Then, a gradient method is followed in the maximization step (M-step) to find the solution which maximizes the E-step. Simulation results show that the EM estimator for localization can mitigate the impact of the anchors' position uncertainties and outperforms the approximated Maximum Likelihood (ML) estimator which ignores the anchors' position uncertainties.

Abstract:Aging is a crucial factor influencing postural stability control and contributing to frequent falls, yet its underlying mechanisms remain incompletely understood. This study aims to explore the effects of aging on postural stability control by comparing differences in postural stability and node strength of electroencephalogram (EEG) brain network between elderly and young people under the conditions of congruent and incongruent visual-vestibular sensory inputs. Eighteen elderly volunteers without neuromuscular disorders and eighteen young individuals participated in the present study. Virtual reality (VR) technology was employed to manipulate visual rotation stimuli (clockwise and counterclockwise), and a horizontal rotating platform was used for vestibular rotation stimuli (clockwise). Based on the directional disparity of sensory input in the horizontal plane, visual-vestibular input consistency was categorized as congruent and incongruent. Postural stability was assessed by the center of pressure (COP) trajectory, and EEG signals were collected and analyzed using directed network analysis to observe EEG brain network node connectivity strength. The results revealed that, under conditions of incongruent visual-vestibular sensory inputs, the elderly exhibited significantly inferior postural stability performance in terms of COP anterior-posterior (Y-axial) sway speed, total path length, anterior-posterior and medial-lateral sample entropy, compared to the young adults. Moreover, the node connectivity strength of visual cortex in the elderly was notably higher, while node connectivity strength of superior temporal cortex was significantly lower than that in the young adults. These findings suggest that the elderly have a heightened reliance on visual information in postural control and an impaired ability to cope with sensory conflicts arising from incongruent visual-vestibular sensory inputs, leading to compromised postural stability. The outcomes of this study hold significant implications for future assessments of balance function in the elder and fall prevention trainings.
Reliability of Posturographic Parameters Assessing Sit-to-stand Movements in Healthy Participants

Abstract:Objective Currently,the sit-to-stand process is not well evaluated.The aim of the study is to evaluate quantitatively the reliability of posturographic parameters in the sit-to-stand process.Methods Seventy-four healthy participants were enrolled and required to finish 20-second sitting,standing and the sit-to-stand process.The computer calculated the posturographic parameters automatically.The test was repeated twice to assess the reliability at an interval of six days.Results During sitting and standing position,the intraclass correlation coefficients (ICCs) of all the parameters were above 0.50,indicating a good or fair reliability.During the standing stance and sitting phase of the sit-to-stand process,the ICCs of all the parameters were above 0.40 and 0.60 respectively,showing a good or fair reliability.Conclusions Sit-to-stand posturographic parameters are a new way to assess the position control ability among the healthy participants with a good reliability.Further study should be conducted among patients to result in better functional and effectiveness evaluation.

Abstract:To date, the human balance assessment is mainly achieved by measuring the center of pressure (COP) trajectory of upright posture. However, COP-based metrics provide inconclusive results under certain circumstances, due to the lack of relevance with the body balance mechanism. This paper developed a novel framework to measure the variability in body sway based on the Poincaré analysis to enhance the balance assessment. It provided nonlinear parameters SD1, SD2, and their ratio SD1/SD2 as quantitative indicators of the local and global balance regulation capacities. 136 healthy adults were categorized into young and elderly groups and received test on a force platform. COP signals were recorded for 30 s under eyes-opened and eyes-closed vision conditions. The results were analyzed using the two-way mixed-design analysis of variance and show that the technology is able to reveal the effect of age and vision on the balance control by SD1, SD2, and SD1/SD2. SD1 mainly responded to vision, and SD2 was more influenced by age. Due to the information about the subtle adjustment during quiet standing, these Poincaré indexes surpassed the conventional COP measurements in demonstrating the balance control behavior. It can be concluded that the supposed method of estimating the local and global variabilities is a valuable approach for the balance assessment.

Test-Retest Reliability of Movement Displacement during a 20-s Stepping-in-Place Test in Community-Dwelling Older Adults with and without Supportive Care

Abstract:Introduction: Our recent reports have shown that movement pattern and displacement assessed with an infrared depth sensor during a 20-s stepping-in-place test (ST) correlates with measures of balance and need for assistance in older individuals. This study investigated ST test-retest reliability in community-dwelling older adults with and without supportive care. Methods: Two groups were included: those not participating (HO: n = 25, 74.7 ± 5.2 years) and those participating (DSU: n = 41, 78.8 ± 5.8 years) in regular senior day services (supportive care). HO completed three ST trials, separated by 1 week, while DSU completed two ST trials during the same half-day supportive care visit. Testing was conducted with eyes open. ST measures included head movement path distance (TMD), maximum movement displacement (MMD), knee movement path length (KMD), and stepping rate (STEP). TMD×KMD-1 ratio indicated upper-body sway relative to lower-body. Test-retest reliability (intra-class correlation coefficients, ICCs) and between-trial and between group differences (ANCOVA, adjusting for age) were assessed. Results: After adjusting for age, TMD, KMD, TMD×KMD-1 were lesser and STEP was greater in HO than DUS. HO ST variables did not differ across testing days. HO ICC (1, 3) for TMD (0.911 [95% confidence interval: 0.827-0.958]), MMD (0.918 [0.814-0.961]), KMD (0.838 [0.685-0.923]), and TMD×KMD-1 (0.940 [0.884-0.872]) showed strong to very strong reliability. Similarly, DSU ST variables did not differ across same day trials and ICC (1, 2) for TMD, KMD, and TMD×KMD-1 displayed very strong reliability. Conclusion: These ST variables exhibited excellent test-retest reliability of discriminating between community-dwelling older adults with and without supportive care.

Stabilization of the head position in pitch plane: study of imbalance during a multisegment posturography examination

Abstract:Our purpose was to evaluate head stabilization in pitch plane in patients with different kinds of imbalance when standing over firm and movable surfaces. We used the STATITEST (Société Mumedia, Belgium), a multisegment platform for recording and computing postural adjustments at different body levels. We detected these data by two sensors placed on the head and hip of each subject in four situations: eyes open on a firm surface (EOFS); eyes closed on a firm surface (ECFS); eyes open on a movable surface (EOMS); and eyes closed on a movable surface (ECMS). For each recording, a magnetic field was broadcast by an antenna placed in front of the patients. First, as the manufacturer did not provide the normal range limits for shoulder sway, we conducted a statistical analysis on a population and selected a sample of 31 normal individuals. Second, we performed the experiment with 91 patients in two steps: standard examination and an assessment with sensors on the head and shoulder. The following values of increased head-shoulder relationship were found: on EOFS, 6; on ECFS, 5; on EOMS, 6; and on ECMS, 11. We noted the predominance of smaller head adjustments, reflecting accurate head stability. However, a small percentage of patients showed increased head movement, a pigeonlike sway. A correlation of these individuals with postural strategies revealed that almost all adopted ankle strategies.
Reliability and Validity of Force Platform Measures of Balance Impairment in Individuals With Parkinson Disease

Abstract:Abstract Background Complex movement and balance impairments in people with Parkinson disease (PD) contribute to high fall risk. Comprehensive balance assessment is warranted to identify intrinsic fall risk factors and direct interventions. Objective The purpose of this study was to examine the psychometric properties of 3 balance measures of a force platform (FP) system in people with PD. Methods Forty-two community-dwelling individuals with idiopathic PD completed the testing protocol. Test-retest reliability was assessed for the Limits of Stability Test (LOS), Motor Control Test (MCT), and Sensory Organization Test (SOT). Intraclass correlation coefficients (ICC [2,1]) were calculated to determine test-retest reliability and minimal detectable change. Validity was assessed by comparing the FP measures with criterion gait and balance measures using Pearson product moment correlations. Multiple regression analyses examined the contribution of PD characteristics to FP measures. Results All primary FP variables demonstrated excellent test-retest reliability (ICC=.78–.92). The SOT and LOS demonstrated fair to good correlations with criterion measures, whereas the MCT had fair correlations to balance measures only. Both SOT composite equilibrium and MCT average latency were moderately associated with disease severity. Limitations This study's sample had a relatively small number of participants with a positive fall history, which may limit the generalizability of the findings. Conclusions This study's findings provide support that FP measures are reliable and valid tests of balance impairment in people with PD. Disease severity was significantly associated with SOT and MCT measures, perhaps reflecting that these tests are meaningful indicators of decline in postural control with disease progression. Force platform measures may provide valuable quantitative information about underlying balance impairments in people with PD to guide therapeutic interventions for fall risk reduction.

Abstract:BackgroundPrevious research on balance mostly focused on the assessment, training, and improvements of balance through interventions. We investigated tools commonly used to study static balance. Differences in postural stability were analyzed using multiscale entropy (MSE) and feature analysis.MethodsA force plate and inertial sensor were used to collect acceleration and center-of-pressure (COP) nonlinear signals. MSE was also used to detect fractal correlations and assess the complexity of univariate data complexity. Fifteen healthy subjects participated in the experiments. Each stood on a force plate and wore a sensor while attempting to maintain postural stability for 30 s in four randomized experiments to evaluate their static balance via a copositive experiment with eyes open/closed and with standing on one foot or both feet. A Wilcoxon-signed rank test was used to confirm that the conditions were significant. Considering the effect of the assessment tools, the influence of the visual and lower limb systems on postural stability was assessed and the results from the inertial sensor and force plate experiments were compared.ResultsForce plate usage provided more accurate readings when completing static balance tasks based on the visual system, whereas an inertial sensor was preferred for lower-limb tasks. Further, the eyes-open-standing-on-one-foot case involved the highest complexity at the X, Y, and Z axes for acceleration and at the ML axis for COP compared with other conditions, from which the axial directions can be identified.ConclusionsThe findings suggested investigation of different evaluation tool choices that can be easily adapted to suit different needs. The results for the complexity index and traditional balance indicators were comparable in their implications on different conditions. We used MSE to determine the equipment that measures the postural stability performance. We attempted to generalize the applications of complexity index to tasks and training characteristics and explore different tools to obtain different results.Trial registrationThis study was approved by the Research Ethics Committee of National Taiwan University and classified as expedited on August 24, 2017. Assessing Proprioception in an Older Population: Reliability of a Protocol Based on Active Movement Extent Discrimination

Abstract:Proprioceptive ability - the sense of where body parts are located in space - is one of many factors thought to affect falls risk among the elderly. Active movement extent discrimination is an approach to measuring proprioception that is administered in an ecologically valid testing environment to better reflect the exercise of proprioceptive skills in daily life. The Active Movement Extent Discrimination Apparatus (AMEDA) was developed to objectively measure this proprioceptive discrimination. However, the current absolute identification testing protocol is cognitively demanding, and it yields results that are insufficiently reliable to assess performance at the individual level. The objectives of this pilot study were to test the reliability and feasibility of a proposed new AMEDA testing protocol and to explore how performance related to cognitive ability and any perceived dysfunction in the foot or ankle. We tested 42 participants (aged 19 - 94 years) three times on the ankle AMEDA using a newly developed protocol that asked participants to report whether a given angle of ankle inversion was shallower or deeper than the immediately preceding inversion. Participants also completed the Stroop test, as a measure of cognitive ability, and two validated questionnaires for identifying foot or ankle dysfunction (the Cumberland Ankle Instability Tool and the Foot and Ankle Ability Measure). The proportion of correct responses for the AMEDA test showed the expected sigmoid shape of the psychometric function as signal strength increased. The intraclass correlation coefficient measured over the three tests was 0.65 (95% confidence interval: 0.49 - 0.78), suggesting moderate reliability. We found a positive and statistically significant correlation between AMEDA performance and Stroop results but no relationship between the AMEDA score and questionnaire-measured foot or ankle dysfunction. This study confirmed that the alternative testing protocol was simple to administer and easily understood by participants.
Test–retest reliability of kinematic and kinetic parameters during dual-task stair walking in the elderly

Abstract:Objective: This study aims to evaluate the test–retest reliability of kinematics and kinetics during single and dual-task stair walking in the elderly. Methods: Fifteen healthy elderly adults were recruited. Kinematic and kinetic parameters were measured using an infrared motion analysis system (Vicon, Oxford Metrics Ltd., Oxford, United Kingdom) and force platforms (Switzerland, Kistler 9287BA and 9281CA). Participants were tested under single-task and dual-task (serial 3 subtractions or carrying a cup of water) conditions. Each participant completed two sessions on two separate days with a 1-week interval. Intraclass correlation coefficients (ICC), Pearson correlation coefficient (r), and Bland–Altman plot were used to assess the reliability of stair walking. Results: When ascending stairs, the ICC of kinematics and kinetics ranged from fair to excellent (ICC = 0.500–0.979) in the single and dual tasks, except for step length (ICC = 0.394) in the single task. The r value of kinematics and kinetics ranged from 0.704 to 0.999. When descending stairs, the ICC of kinematics and kinetics ranged from good to excellent (ICC = 0.661–0.963), except for min hip moment (ICC = 0.133) and min ankle moment (ICC = 0.057) in the manual task. The r value of kinematics and kinetics ranged from 0.773 to 0.960 in the single and dual tasks. In the Bland–Altman plots, all the zero values and most of the dots fell in the 95% confidence interval, and the mean difference was found to be close to zero for all the parameters during stair walking. Conclusion: These results obtained from this study show the good test-retest reliability of step cadence, step speed, and step width during single- and dual-task stair walking in the elderly, and the poor reliability of step length during ascending stairs. All the kinetic parameters, including min hip moment, max knee moment, and min ankle moment, had good test-retest reliability during single- and dual-task stair walking, but min hip moment and min ankle moment had poor reliability during manual-task descending stair. These results may help researchers in the assessment of biomechanics of dual-task stair walking in the elderly and to interpret the effect of interventions in this population.

Abstract:Objectives: Postural instability is one of the most disabling features in Parkinson's disease (PD), and often leads to falls that reduce mobility and functional capacity. The objectives of this study were to analyse the limit of stability (LOS) and influence of the manipulation of visual, somatosensorial and visual-vestibular information on postural control in patients with PD and healthy subjects. Design: Cross-sectional. Setting: Movement Disorders Unit, university setting. Participants: Eighty-two subjects aged between 37 and 83 years: 41 with Parkinson's disease in the 'on' state and 41 healthy subjects with no neurological disorders. Both groups were matched in terms of sex and age. Main outcome measures: Unified Parkinson's Disease Rating Scale (UPDRS)-motor score, modified Hoehn and Yahr staging, Dynamic Gait Index (DGI) and posturography with integrated virtual reality. The parameters analysed by posturography were LOS area, area of body centre of pressure excursion and balance functional reserve in the standing position in 10 conditions (open and closed eyes, unstable surface with eyes closed, saccadic and optokinetic stimuli, and visual-vestibular interaction). Results: The mean UPDRS motor score and DGI score were 27 [standard deviation (SD) 14] and 21 (SD 3), respectively. Thirteen participants scored between 0 and 19 points, indicating major risk of falls. Posturographic assessment showed that patients with PD had significantly lower LOS area and balance functional reserve values, and greater body sway area in all posturographic conditions compared with healthy subjects. Conclusions: Patients with PD have reduced LOS area and greater postural sway compared with healthy subjects. The deterioration in postural control was significantly associated with major risk of falls.

Abstract:BACKGROUND: Standing is a basic human function that healthy adults take for granted, yet it is a complex perceptual-motor process that requires sensation of position and motion from the sensory systems. OBJECTIVE: We assessed agreement between center of pressure data from a laboratory force-platform and head position data from an HTC Vive head-mounted display (HMD) for the evaluation of standing postural control. We investigated the impact of different statistical choices when assessing the relationship between two measurements. Specifically: 1) How does correlation and agreement statistics relate before and after logarithmic transformation? 2) Is there systemic or proportional bias between the force-platform and HMD measurements? METHODS: We tested 37 adults (26 controls, 11 with unilateral vestibular hypofunction) standing on foam, observing a static or dynamic visual scene projected from the HMD. We quantified anterior-posterior and medio-lateral sway via Directional Path, Root Mean Square Velocity, Variance, and Power Spectral Density (PSD) from a force-platform and the HMD. RESULTS: Intra-class correlations (ICCs) were moderate-to-good for the non-transformed data and good-to-excellent after logarithmic transformation for all outcomes except for PSD above 1 Hz. Correlations were higher than ICCs. Bland-Altman plots indicated proportional bias but not after logarithmic transformation. CONCLUSIONS: Both devices correlated linearly, and measure people's postural responses but cannot be used interchangeably, mostly because they appear to diverge with larger sway as evident on Bland-Altman plots of non-transformed data. Agreement between devices was excellent for low frequency movement but poor for high frequency small corrective movements.

Abstract:Background and Objective: Accelerations, ground reaction force data, and other quantitative data obtained from kinematic analyses of gait initiation serve as measures of dyskinesia in patients with Parkinson's disease. The objective of this study was to determine whether a separate center of pressure information could reveal new characteristics of reduced anticipatory postural adjustments in patients with Parkinson's disease. Methods: Ten healthy elderly subjects and 10 patients with Parkinson's disease participated in this study. Motions at gait initiation in response to a light signal to begin the task were analyzed. Anticipatory postural adjustments were characterized using trunk acceleration data and center of pressure displacement data. Results: The separated center of pressure in patients with Parkinson's disease showed longer peak latency (p < .01) and larger peak magnitude (p < .01) in the stance leg than in the stepping leg. In patients with Parkinson's disease, the displacement peak latency of the stance leg center of pressure correlated negatively with the Timed Up and Go time (r = −0.46, p < .05), while displacement peak latency of the stepping leg center of pressure correlated negatively with Unified Parkinson's Disease Rating Scale (r= −0.47, p < .05), and positively with Berg Balance Scale score (r = 0.50, p < .05). Conclusions: Patients with Parkinson's disease experience asymmetry between the stance leg and stepping leg at gait initiation. These findings may help clinicians understand the changes to motor function in patients with Parkinson's disease and suggest strategies for improved rehabilitation training.

Abstract:Due to the fact that no study to date has shown the experimental validity of ACC-based measures of body sway with respect to posturography for subjects with vestibular deficits, the aim of the present study was: i) to develop and validate a practical tool that can allow clinicians to measure postural sway derangements in an otoneurological setting by ACC, and ii) to provide reliable, sensitive and accurate automatic analysis of sway that could help in discriminating unilateral vestibular failure (UVF) patients. Thus, a group of 13 patients (seven females, 6 males; mean age 48.6 +/- 6.4 years) affected for at least 6 months by UVF and 13 matched healthy subjects were instructed to maintain an upright position during a static forceplate-based posturography (FBP) acquisition while wearing a Movit sensor (by Captiks) with 3-D accelerometers mounted on the posterior trunk near the body centre of mass. Pearson product moment correlation demonstrated a high level of correspondence of four time-domain and three frequency-domain measures extracted by ACC and FBP testing; in addition, t-test demonstrated that two ACC-based time- and frequency-domain parameters were reliable measures in discriminating UVF subjects. These aspects, overall, should further highlight the attention of clinicians and researchers to this kind of sway recording technique in the field of otoneurological disorders by considering the possibility to enrich the amount of quantitative and qualitative information useful for discrimination, diagnosis and treatment of UVF. In conclusion, we believe the present ACC-based measurement of sway offers a patient-friendly, reliable, inexpensive and efficient alternative recording technique that is useful - together with clinical balance and mobility tests - in various circumstances, as well as in outcome studies involving diagnosis, follow-up and rehabilitation of UVF patients.
Vertical Center-of-Mass Braking and Motor Performance during Gait Initiation in Young Healthy Adults, Elderly Healthy Adults, and Patients with Parkinson's Disease: A Comparison of Force-Plate and Markerless Motion Capture Systems

Abstract:Background. This study tested the agreement between a markerless motion capture system and force-plate system ("gold standard") to quantify stability control and motor performance during gait initiation. Methods. Healthy adults (young and elderly) and patients with Parkinson's disease performed gait initiation series at spontaneous and maximal velocity on a system of two force-plates placed in series while being filmed by a markerless motion capture system. Signals from both systems were used to compute the peak of forward center-of-mass velocity (indicator of motor performance) and the braking index (indicator of stability control). Results. Descriptive statistics indicated that both systems detected between-group differences and velocity effects similarly, while a Bland–Altman plot analysis showed that mean biases of both biomechanical indicators were virtually zero in all groups and conditions. Bayes factor 01 indicated strong (braking index) and moderate (motor performance) evidence that both systems provided equivalent values. However, a trial-by-trial analysis of Bland–Altman plots revealed the possibility of differences >10% between the two systems. Conclusion. Although non-negligible differences do occur, a markerless motion capture system appears to be as efficient as a force-plate system in detecting Parkinson's disease and velocity condition effects on the braking index and motor performance.

Elderly fall risk prediction using static posturography

Abstract:Maintaining and controlling postural balance is important for activities of daily living, with poor postural balance being predictive of future falls. This study investigated eyes open and eyes closed standing posturography with elderly adults to identify differences and determine appropriate outcome measure cut-off scores for prospective faller, single-faller, multi-faller, and non-faller classifications. 100 older adults (75.5 ± 6.7 years) stood quietly with eyes open and then eyes closed while Wii Balance Board data were collected. Range in anterior-posterior (AP) and medial-lateral (ML) center of pressure (CoP) motion; AP and ML CoP root mean square distance from mean (RMS); and AP, ML, and vector sum magnitude (VSM) CoP velocity were calculated. Romberg Quotients (RQ) were calculated for all parameters. Participants reported six-month fall history and six-month post-assessment fall occurrence. Groups were retrospective fallers (24), prospective all fallers (42), prospective fallers (22 single, 6 multiple), and prospective non-fallers (47). Non-faller RQ AP range and RQ AP RMS differed from prospective all fallers, fallers, and single fallers. Non-faller eyes closed AP velocity, eyes closed VSM velocity, RQ AP velocity, and RQ VSM velocity differed from multi-fallers. RQ calculations were particularly relevant for elderly fall risk assessments. Cut-off scores from Clinical Cut-off Score, ROC curves, and discriminant functions were clinically viable for multi-faller classification and provided better accuracy than single-faller classification. RQ AP range with cut-off score 1.64 could be used to screen for older people who may fall once. Prospective multi-faller classification with a discriminant function (-1.481 + 0.146 x Eyes Closed AP Velocity-0.114 x Eyes Closed Vector Sum Magnitude Velocity-2.027 x RQ AP Velocity + 2.877 x RQ Vector Sum Magnitude Velocity) and cut-off score 0.541 achieved an accuracy of 84.9% and is viable as a screening tool for older people at risk of multiple falls.

Abstract:BACKGROUND:Force-plate posturography offers a convenient way for quantitative assessment of postural stability in the elderly. However, studies focusing on routine balance assessment have usually not taken reginal shear distributions (i.e., arising from horizontal forces) into consideration.RESEARCH QUESTION:(1) Does plantar shear distribution differ between young and elderly subjects during upright standing? (2) How do the maximum plantar shear forces vary at different regions of the foot?METHODS:The new reginal shear measurement (RSM) method can simultaneously capture the three-dimensional force distributions at regional plantar sites while subjects maintaining standing balance. The feasibility of the proposed method in characterizing the magnitude and distribution of plantar shear forces was tested in thirty-two normal young and nineteen elderly subjects. Statistical analysis was performed using the independent samples t-test for both the continuous and ordinal variables.RESULTS:For regional AP shear forces, statistically significant differences were found between the two groups for the toe region of the right foot and the midfoot of both feet. For ML shear distributions, statistically significant differences were found at nearly all plantar sites expect for the hallux and lateral metatarsal. The maximum increase in ML shear forces occurred in the toe region of the right foot, where the peak shear values were 113.16% higher than those of the young subjects. The peak ML shear occurred in the midfoot were averagely 83.19% and 70.57% higher in the elderly's left and right feet, respectively.SIGNIFICANCE:The RSM method may offer unique solutions to identify functional decline in postural control of the elderly. The plantar shear pattern has potential to become an important parameter in evaluating one's balance performance during upright standing.
