Abstract:BACKGROUND:Urinary tract infection (UTI) is one of major nosocomial infections significantly affecting the outcomes of immobile stroke patients. Previous studies have identified several risk factors, but it is still challenging to accurately estimate personal UTI risk.AIM:To develop predictive models for UTI risk identification for immobile stroke patients.METHODS:Research data were collected from our previous multicentre study. Derivation cohort included 3982 immobile stroke patients collected from November 1st, 2015 to June 30th, 2016; external validation cohort included 3837 patients collected from November 1st, 2016 to July 30th, 2017. Six machine learning models and an ensemble learning model were derived, based on 80% of derivation cohort, and effectiveness was evaluated with the remaining 20%. Shapley additive explanation values were used to determine feature importance and examine the clinical significance of prediction models.FINDINGS:In all, 2.59% (103/3982) patients were diagnosed with UTI in derivation cohort, 1.38% (53/3837) in external cohort. The ensemble learning model performed the best in area under the receiver operating characteristic (ROC) curve in internal validation (82.2%); second best in external validation (80.8%). In addition, the ensemble learning model performed the best sensitivity in both internal and external validation sets (80.9% and 81.1%, respectively). Seven UTI risk factors (pneumonia, glucocorticoid use, female sex, mixed cerebrovascular disease, increased age, prolonged length of stay, and duration of catheterization) were also identified.CONCLUSION:This ensemble learning model demonstrated promising performance. Future work should continue to develop a more concise scoring tool based on machine learning models and prospectively examining the model in practical use, thus improving clinical outcomes.

Abstract:Background: Detailed and invasive clinical investigations are required to identify the causes of haematuria. Highly unbalanced patient population (predominantly male) and a wide range of potential causes make the ability to correctly classify patients and identify patient-specific biomarkers a major challenge. Studies have shown that it is possible to improve the diagnosis using multi-marker analysis, even in unbalanced datasets, by applying advanced analytical methods. Here, we applied several machine learning algorithms to classify patients from the haematuria patient cohort (HaBio) by analysing multiple biomarkers and to identify the most relevant ones. Materials and methods: We applied several classification and feature selection methods (k-means clustering, decision trees, random forest with LIME explainer and CACTUS algorithm) to stratify patients into two groups: healthy (with no clear cause of haematuria) or sick (with an identified cause of haematuria e.g., bladder cancer, or infection). The classification performance of the models was compared. Biomarkers identified as important by the algorithms were also analysed in relation to their involvement in the pathological processes. Results: Results showed that a high unbalance in the datasets significantly affected the classification by random forest and decision trees, leading to the overestimation of the sick class and low model performance. CACTUS algorithm was more robust to the unbalance in the dataset. CACTUS obtained a balanced accuracy of 0.747 for both genders, 0.718 for females and 0.803 for males. The analysis showed that in the classification process for the whole dataset: microalbumin, male gender, and tPSA emerged as the most informative biomarkers. For males: age, microalbumin, tPSA, cystatin C, BTA, HAD and S100A4 were the most significant biomarkers while for females microalbumin, IL-8, pERK, and CXCL16. Conclusions: CACTUS algorithm demonstrated improved performance compared with other methods such as decision trees and random forest. Additionally, we identified the most relevant biomarkers for the specific patient group, which could be considered in the future as novel biomarkers for diagnosis. Our results have the potential to inform future research and provide new personalised diagnostic approaches tailored directly to the needs of the individuals.

Abstract:578 Background: Radical cystectomy (RC) is a standard treatment for patients with muscle invasive and certain high-risk non-invasive bladder cancers. Existing clinical risk calculators based for RC on standard regression methods have demonstrated poor, inaccurate predictions of post-operative risks and adverse outcomes. We aimed to build a risk calculator using novel machine learning methods to predict complication rates and other outcomes based on clinical characteristics of patients undergoing RC. Methods: Patients who had an RC for bladder cancer between 2019 and 2021 were pulled from the National Surgical Quality Improvement Database. Potential predictors included clinically meaningful characteristics such as patient demographics, pre-operative labs, comorbidities, pre-operative bowel and antibiotic preparations, and prior treatment history, as well as cystectomy-specific characteristics such as diversion type and surgical approach. Outcomes included risk of any complication, infectious complication, mortality, and readmissions. Logistic regression, random forest, and support vector machine algorithms were used to build predictive models. Models were optimized assessed with area under the ROC curve (AUROC) and average precision (AP). Results: 8,936 patients were included in the sample dataset. Random forest classifiers outperformed other models for predicting risk of any complication (AUROC = 0.662; AP = 0.66), infectious complication (AUROC = 0.615; AP = 0.20), and mortality (AUROC = 0.603; AP = 0.03). The logistic regression model best predicted readmissions (AUROC = 0.587; AP = 0.26). Top predictors of complication risk included operation time, glomerular filtration rate, and type of procedure (robotic vs. open). Infectious complication was best predicted by wound classification and diversion type, mortality was best predicted by BMI and operation time, and readmissions was predicted most by pre-existing renal failure and emergent cases. Conclusions: Machine learning-based risk calculators were more effective in predicting morbidity and mortality after RC compared to more standard predictive algorithms. Machine learning algorithms therefore demonstrate greater predictive potential when given a wide range of clinical characteristics. [Table: see text]

Abstract:Importance: Acute kidney injury (AKI) is associated with increased morbidity and mortality in hospitalized patients. Current methods to identify patients at high risk of AKI are limited, and few prediction models have been externally validated.Objective: To internally and externally validate a machine learning risk score to detect AKI in hospitalized patients.Design, Setting, and Participants: This diagnostic study included 495 971 adult hospital admissions at the University of Chicago (UC) from 2008 to 2016 (n = 48 463), at Loyola University Medical Center (LUMC) from 2007 to 2017 (n = 200 613), and at NorthShore University Health System (NUS) from 2006 to 2016 (n = 246 895) with serum creatinine (SCr) measurements. Patients with an SCr concentration at admission greater than 3.0 mg/dL, with a prior diagnostic code for chronic kidney disease stage 4 or higher, or who received kidney replacement therapy within 48 hours of admission were excluded. A simplified version of a previously published gradient boosted machine AKI prediction algorithm was used; it was validated internally among patients at UC and externally among patients at NUS and LUMC.Main Outcomes and Measures: Prediction of Kidney Disease Improving Global Outcomes SCr-defined stage 2 AKI within a 48-hour interval was the primary outcome. Discrimination was assessed by the area under the receiver operating characteristic curve (AUC).Results: The study included 495 971 adult admissions (mean [SD] age, 63 [18] years; 87 689 [17.7%] African American; and 266 866 [53.8%] women) across 3 health systems. The development of stage 2 or higher AKI occurred in 15 664 of 48 463 patients (3.4%) in the UC cohort, 5711 of 200 613 (2.8%) in the LUMC cohort, and 3499 of 246 895 (1.4%) in the NUS cohort. In the UC cohort, 332 patients (0.7%) required kidney replacement therapy compared with 672 patients (0.3%) in the LUMC cohort and 440 patients (0.2%) in the NUS cohort. The AUCs for predicting at least stage 2 AKI in the next 48 hours were 0.86 (95% CI, 0.86-0.86) in the UC cohort, 0.85 (95% CI, 0.84-0.85) in the LUMC cohort, and 0.86 (95% CI, 0.86-0.86) in the NUS cohort. The AUCs for receipt of kidney replacement therapy within 48 hours were 0.96 (95% CI, 0.96-0.96) in the UC cohort, 0.95 (95% CI, 0.94-0.95) in the LUMC cohort, and 0.95 (95% CI, 0.94-0.95) in the NUS cohort. In time-to-event analysis, a probability cutoff of at least 0.057 predicted the onset of stage 2 AKI a median (IQR) of 27 (6.5-93) hours before the eventual doubling in SCr concentrations in the UC cohort, 34.5 (19-85) hours in the NUS cohort, and 39 (19-108) hours in the LUMC cohort.Conclusions and Relevance: In this study, the machine learning algorithm demonstrated excellent discrimination in both internal and external validation, supporting its generalizability and potential as a clinical decision support tool to improve AKI detection and outcomes.

Abstract:254 Background: Clinicians in oncology are often challenged to identify when patients with cancer are at high risk for adverse outcomes and would benefit from more intensive clinical care. Preemptive identification of these patients may improve efficiency and improve patient care. The objective of this quality improvement pilot was to prospectively validate a machine learning (ML)-based clinical tool designed to identify patients with cancer who are at high risk for an emergency department (ED) visit, and whether they met eligibility criteria for clinical services at home, such as Huntsman at Home (H@H). Methods: Patients with cancer who received care at Huntsman Cancer Institute (HCI) between Jan. 4 and Feb. 7, 2020 were included in the analysis. For patients with HCI contact in a given week, the ML-based tool predicted the probability of an ED visit in the next 60 days to identify “high risk” patients using real-time structured EHR data (e.g. demographic characteristics, vital signs, and laboratory values). Risk of an ED visit was used as a proxy for eligibility for H@H. Patients were randomized to two cohorts to assess eligibility precision and outcome forecast precision. Eligibility precision was defined as the percentage of ML-classified high risk patients who were confirmed by a nurse practitioner to be eligible for admission to H@H. Outcome forecast precision was defined as the percentage of ML-classified high risk patients who experienced a future ED event within 60 days and was compared to the baseline prevalence of ED visits within 60 days during the same time period. The IRB determined this to be a quality improvement project. Results: This quality improvement pilot included 1,236 patients; 53% were women, median age was 65 years, and 84% were Caucasian. The most common cancers, excluding non-melanoma, were breast, prostate, lung, and myeloma. The observed prevalence of an ED visit within 60 days was 7%. The tool classified 9% of patients as high risk. The eligibility precision was 0.76 (95% CI: 0.62-0.89), demonstrating concordance with clinician assessment among patients classified as high risk. Compared to a baseline of 0.07, the outcome forecast precision was 0.22 (95% CI: 0.12-0.34) for future ED events. Conclusions: This quality improvement pilot demonstrates the potential application of an ML-based tool to identify patients with cancer who may benefit from further support through the H@H program. The approach creates a framework for ML-based tools to enhance clinical services at home.

Abstract:Several studies have shown that microsatellite changes can be profiled in urine for the detection of bladder cancer. The use of microsatellite analysis (MSA) for bladder cancer detection requires a comprehensive analysis of as many as 15 to 20 markers, based on the amplification and interpretations of many individual MSA markers, and it can be technically challenging. Here, to develop fast, more efficient, standardized, and less costly MSA for the detection of bladder cancer, we developed three multiplex-polymerase-chain-reaction-(PCR)-based MSA assays, all of which were analyzed via a genetic analyzer. First, we selected 16 MSA markers based on 9 selected publications. Based on samples from Johns Hopkins University (the JHU sample, the first set sample), we developed an MSA based on triplet, three-tube-based multiplex PCR (a Triplet MSA assay). The discovery, validation, and translation of biomarkers for the early detection of cancer are the primary focuses of the Early Detection Research Network (EDRN), an initiative of the National Cancer Institute (NCI). A prospective study sponsored by the EDRN was undertaken to determine the efficacy of a novel set of MSA markers for the early detection of bladder cancer. This work and data analysis were performed through a collaboration between academics and industry partners. In the current study, we undertook a re-analysis of the primary data from the Compass study to enhance the predictive power of the dataset in bladder cancer diagnosis. Using a four-stage pipeline of modern machine learning techniques, including outlier removal with a nonlinear model, correcting for majority/minority class imbalance, feature engineering, and the use of a model-derived variable importance measure to select predictors, we were able to increase the utility of the original dataset to predict the occurrence of bladder cancer. The results of this analysis showed an increase in accuracy (85%), sensitivity (82%), and specificity (83%) compared to the original analysis. The re-analysis of the EDRN study results using machine learning statistical analysis proved to achieve an appropriate level of accuracy, sensitivity, and specificity to support the use of the MSA for bladder cancer detection and monitoring. This assay can be a significant addition to the tools urologists use to both detect primary bladder cancers and monitor recurrent bladder cancer.

Abstract:Background: Several clinical prediction scores have been designed to assess the risk of cancer-associated thrombosis (CAT). The most commonly used in current clinical practice is the Khorana score, however it is applicable only to patients prior to initiation of chemotherapy. We now apply machine learning with clinical, demographic, and genomics parameters to predict CAT events. Methods: The random survival forest (RSF) ensemble learning method was selected to illustrate a machine approach to CAT prediction. The cohort consisted of 14,223 individuals with a solid tumor malignancy and MSK IMPACT somatic genomic data collected during the years 2014 to 2016. CAT was defined as the diagnosis of lower extremity deep vein thrombosis (proximal or distal) or pulmonary embolism, incidental or symptomatic. Covariates considered for inclusion in the model consisted of tumor type, metastatic status, age, exposure to cytotoxic chemotherapy in the month before cohort entry, time elapsed since cancer diagnosis, time elapsed since tumor sampling, normalized mean blood cell counts (white cell count, hemoglobin, platelet count) in the prior 3 months, normalized mean prothrombin time (PT) and activated partial thromboplastin time (aPTT) in the prior 3 months, body mass index (BMI), and presence or absence of a somatic genetic alteration for oncogenes/tumor suppressor genes with an alteration frequency ≥ 1.5% (n = 56). The primary endpoint consisted of time to CAT episode. The C-index for models including different covariates was derived from the test holdout sample using repeated 10-fold cross-validation. The C-index, measuring the relative agreement between the RSF predicted risk and the CAT times of patients, has values between 0.5 and 1.0 with the latter indicating perfect agreement. Results: 12,040 patients were included in the final analysis. There were 855 CAT events during the observation period. The most common tumor types were lung (17%), breast (15%) and colorectal cancer (9%). Blood cell count data and coagulation parameters were missing for 8% and 51% of patients respectively. Using cross-validation, the baseline model with cancer type and metastatic status had a C-index of 0.62 (95% CI = 0.61-0.64), which increased to 0.65 (95% CI = 0.63-0.66) with the addition of chemotherapy, age, time from tissue sampling, time from cancer diagnosis and BMI. Further adding genetic data increased the C-index to 0.68 (95% CI = 0.66-0.69). Replacing genetic data in this model with cell counts and coagulation parameters resulted in a C-index of 0.69 (95% CI = 0.68-0.70). The model with all available covariates had a C-index of 0.70 (95% CI = 0.69-0.71). The cumulative incidence of CAT at 6 months for 5 categories of predicted risk using the model with all available covariates is plotted in Figure A. Scaled Ishwaran-Kogalur Variable Importance (VIMP) values, presented in Figure B, indicate that cancer type and prior chemotherapy are the two top factors for model performance. Conclusions: Machine learning is a promising approach in the search of more accurate and generalizable models for prediction of CAT. In the application described here, the use of random survival forests performed well without information about future chemotherapy administration. Additional work is needed to identify the optimal algorithm and covariates, including better delineation of which cancer genomic information should be retained. Abstract:BACKGROUND/AIM: Prostate cancer (PCa) is lethal. Our aim in this retrospective cohort study was to use machine learning-based methodology to predict PCa risk in patients with benign prostate hyperplasia (BPH), identify potential risk factors, and optimize predictive performance.PATIENTS AND METHODS: The dataset was extracted from a clinical information database of patients at a single institute from January 2000 to December 2020. Patients newly diagnosed with BPH and prescribed alpha blockers/5-alpha-reductase inhibitors were enrolled. Patients were excluded if they had a previous diagnosis of any cancer or were diagnosed with PCa within 1 month of enrolment. The study endpoint was PCa diagnosis. The study utilized the extreme gradient boosting (XGB), support vector machine (SVM) and K-nearest neighbors (KNN) machine-learning algorithms for analysis.RESULTS: The dataset used in this study included 5,122 medical records of patients with and without PCa, with 19 patient characteristics. The SVM and XGB models performed better than the KNN model in terms of accuracy and area under curve. Local interpretable model-agnostic explanation and Shapley additive explanations analysis showed that body mass index (BMI) and late prostate-specific antigen (PSA) were important features for the SVM model, while PSA velocity, late PSA, and BMI were important features for the XGB model. Use of 5-alpha-reductase inhibitor was associated with a higher incidence of PCa, with similar survival outcomes compared to non-users.CONCLUSION: Machine learning can enhance personalized PCa risk assessments for patients with BPH but more research is necessary to refine these models and address data biases. Clinicians should use them as supplementary tools alongside traditional screening methods.

Abstract:PURPOSE: Microhematuria is a prevalent condition and the American Urological Association has developed a new risk-stratified approach for the evaluation of patients with microhematuria. Our objective was to provide the first evaluation of this important guideline.MATERIALS AND METHODS: This multinational cohort study combines contemporary patients from 5 clinical trials and 2 prospective registries who underwent urological evaluation for hematuria. Patients were stratified into American Urological Association risk strata (low, intermediate or high risk) based on sex, age, degree of hematuria, and smoking history. The primary end point was the incidence of bladder cancer within each risk stratum.RESULTS: A total of 15,779 patients were included in the analysis. Overall, 727 patients (4.6%) were classified as low risk, 1,863 patients (11.8%) were classified as intermediate risk, and 13,189 patients (83.6%) were classified as high risk. The predominance of high risk patients was consistent across all cohorts. A total of 857 bladder cancers were diagnosed with a bladder cancer incidence of 5.4%. Bladder cancer was more prevalent in men, smokers, older patients and patients with gross hematuria. The cancer incidence for low, intermediate and high risk groups was 0.4% (3 patients), 1.0% (18 patients) and 6.3% (836 patients), respectively.CONCLUSIONS: The new risk stratification system separates hematuria patients into clinically meaningful categories with differing likelihoods of bladder cancer that would justify evaluating the low, intermediate and high risk groups with incremental intensity. Furthermore, it provides the relative incidence of bladder cancer in each risk group which should facilitate patient counseling regarding the risks and benefits of evaluation for bladder cancer.

Abstract:Objective Patients with ulcerative colitis (UC) diagnosed with low-grade dysplasia (LGD) have increased risk of developing advanced neoplasia (AN: high-grade dysplasia or colorectal cancer). We aimed to develop and validate a predictor of AN risk in patients with UC with LGD and create a visual web tool to effectively communicate the risk. Design In our retrospective multicentre validated cohort study, adult patients with UC with an index diagnosis of LGD, identified from four UK centres between 2001 and 2019, were followed until progression to AN. In the discovery cohort (n=246), a multivariate risk prediction model was derived from clinicopathological features using Cox regression. Validation used data from three external centres (n=198). The validated model was embedded in a web tool to calculate patient-specific risk. Results Four clinicopathological variables were significantly associated with AN progression in the discovery cohort: endoscopically visible LGD >1 cm (HR 2.7; 95% CI 1.2 to 5.9), unresectable or incomplete endoscopic resection (HR 3.4; 95% CI 1.6 to 7.4), moderate/severe histological inflammation within 5 years of LGD diagnosis (HR 3.1; 95% CI 1.5 to 6.7) and multifocality (HR 2.9; 95% CI 1.3 to 6.2). In the validation cohort, this four-variable model accurately predicted future AN cases with overall calibration Observed/Expected=1.01 (95% CI 0.64 to 1.52), and achieved 100% specificity for the lowest risk group over 13 years of available follow-up. Conclusion Multicohort validation confirms that patients with large, unresected, multifocal LGD and recent moderate/severe inflammation are at highest risk of developing AN. Personalised risk prediction provided via the Ulcerative Colitis-Cancer Risk Estimator ( www.UC-CaRE.uk ) can support treatment decision-making.

Interpretable Machine Learning Models for Predicting Critical Outcomes in Patients with Suspected Urinary Tract Infection with Positive Urine Culture

Abstract:(1) Background: Urinary tract infection (UTI) is a leading cause of emergency department visits and hospital admissions. Despite many studies identifying UTI-related risk factors for bacteremia or sepsis, a significant gap remains in developing predictive models for in-hospital mortality or the necessity for emergent intensive care unit admission in the emergency department. This study aimed to construct interpretable machine learning models capable of identifying patients at high risk for critical outcomes. (2) Methods: This was a retrospective study of adult patients with urinary tract infection (UTI), extracted from the Medical Information Mart for Intensive Care IV Emergency Department (MIMIC-IV-ED) database. The critical outcome is defined as either in-hospital mortality or transfer to an intensive care unit within 12 h. ED visits were randomly partitioned into a 70%/30% split for training and validation. The extreme gradient boosting (XGBoost), random forest (RF), and support vector machine (SVM) algorithms were constructed using variables selected from the stepwise logistic regression model. The XGBoost model was then compared to the traditional model and clinical decision rules (CDRs) on the validation data using the area under the curve (AUC). (3) Results: There were 3622 visits among 3235 unique patients diagnosed with UTI. Of the 2535 patients in the training group, 836 (33%) experienced critical outcomes, and of the 1087 patients in the validation group, 358 (32.9%) did. The AUCs for different machine learning models were as follows: XGBoost, 0.833; RF, 0.814; and SVM, 0.799. The XGBoost model performed better than others. (4) Conclusions: Machine learning models outperformed existing traditional CDRs for predicting critical outcomes of ED patients with UTI. Future research should prospectively evaluate the effectiveness of this approach and integrate it into clinical practice.

Abstract:Introduction: Urinary incontinence (UI) is a common side effect of prostate cancer treatment, but in clinical practice, it is difficult to predict. Machine learning (ML) models have shown promising results in predicting outcomes, yet the lack of transparency in complex models known as "black-box" has made clinicians wary of relying on them in sensitive decisions. Therefore, finding a balance between accuracy and explainability is crucial for the implementation of ML models. The aim of this study was to employ three different ML classifiers to predict the probability of experiencing UI in men with localized prostate cancer 1-year and 2-year after treatment and compare their accuracy and explainability. Methods: We used the ProZIB dataset from the Netherlands Comprehensive Cancer Organization (Integraal Kankercentrum Nederland; IKNL) which contained clinical, demographic, and PROM data of 964 patients from 65 Dutch hospitals. Logistic Regression (LR), Random Forest (RF), and Support Vector Machine (SVM) algorithms were applied to predict (in)continence after prostate cancer treatment. Results: All models have been externally validated according to the TRIPOD Type 3 guidelines and their performance was assessed by accuracy, sensitivity, specificity, and AUC. While all three models demonstrated similar performance, LR showed slightly better accuracy than RF and SVM in predicting the risk of UI one year after prostate cancer treatment, achieving an accuracy of 0.75, a sensitivity of 0.82, and an AUC of 0.79. All models for the 2-year outcome performed poorly in the validation set, with an accuracy of 0.6 for LR, 0.65 for RF, and 0.54 for SVM. Conclusion: The outcomes of our study demonstrate the promise of using non-black box models, such as LR, to assist clinicians in recognizing high-risk patients and making informed treatment choices. The coefficients of the LR model show the importance of each feature in predicting results, and the generated nomogram provides an accessible illustration of how each feature impacts the predicted outcome. Additionally, the model's simplicity and interpretability make it a more appropriate option in scenarios where comprehending the model's predictions is essential.

Abstract:Background. Noncancer death accounts for a high proportion of all patients with bladder cancer, while these patients are often excluded from the survival analysis, which increases the selection bias of the study subjects in the prediction model. Methods. Clinicopathological information of bladder cancer patients was retrieved from the Surveillance, Epidemiology, and End Results (SEER) database, and the patients were categorized at random into the training and validation cohorts. The random forest method was used to calculate the importance of clinical variables in the training cohort. Multivariate and univariate analyses were undertaken to assess the risk indicators, and the prediction nomogram based on the competitive risk model was constructed. The model's performance was evaluated utilizing the calibration curve, consistency index (C index), and the area under the receiver operator characteristic curve (AUC). Results. In total, we enrolled 39285 bladder cancer patients in the study (27500 patients were allotted to the training cohort, whereas 11785 were allotted to the validation cohort). A competitive risk model was constructed to predict bladder cancer-specific mortality. The overall C index of patients in the training cohort was 0.876, and the AUC values were 0.891, 0.871, and 0.853, correspondingly, for 1-, 3-, and 5-year cancer-specific mortality. On the other hand, the overall C index of patients in the validation cohort was 0.877, and the AUC values were 0.894, 0.870, and 0.847 for 1-, 3-, and 5-year correspondingly, suggesting a remarkable predictive performance of the model. Conclusions. The competitive risk model proved to be of great accuracy and reliability and could help clinical decision-makers improve their management and approaches for managing bladder cancer patients.

Abstract:Bladder cancer is the most recurrent form of cancer, requiring frequent screening and surveillance, which contributes to its status as the costliest per capita. While urine cytology serves as a crucial adjunct to invasive cystoscopy, it is high volume, labor-intensive, and relatively inconsistent. Despite the implementation of The Paris System, which introduced updated quantitative guidelines, implementation of digital tools for urine cytology still trails the technological advancements seen in semi-automated Pap smear screenings. This lag can be attributed to the diverse cytomorphology of urine specimens and variation in specimen preparation and screening. Our team has developed AutoParis-X (APX), a state-of-the-art technology leveraging deep learning algorithms to extract objective (e.g., nucleus-to-cytoplasm ratio) and subjective (e.g., hyperchromasia) features from whole slide images (WSI). APX transforms these features into the Atypia Burden Score (ABS), offering a quantitative assessment of malignancy and recurrence risks on a scale from 0 to 1. Initially validated with 1300 samples (ThinPrep; Leica Aperio GT450) at Dartmouth Hitchcock, achieving an AUROC of 0.9, APX has evolved into APX-WEB, a user-friendly, web-based tool for cytopathologists. Notable challenges persist, including: 1) validating across nationally representative cohorts with varied specimen preparations and imaging, 2) addressing the intricacies of z-stacking for imaging non-monolayer preparations, and 3) integration of genomics to heighten the sensitivity of imaging assays. This study aimed to establish a performance baseline for APX's adaptability to these challenges without algorithmic adjustments. APX was applied across three distinct cohorts: 1) a preliminary national validation on 500 ThinPrep WSI in collaboration with Hologic; 2) assessment on non-monolayer specimen preparations (e.g., SurePath), using z-stacking, conducted with Johns Hopkins (n=100 WSI); and 3) a pilot study at Dartmouth (n=19) for integrating APX's imaging features with whole exomic sequencing (WES). Preliminary findings indicate: 1) Promising performance on a held-out Hologic national cohort (sensitivity=0.87, specificity=0.73; original APX study: sensitivity=0.88, specificity=0.83). 2) In SurePath z-stack WSI, the number of detected urothelial cells per cluster decreased with distance from the focus plane (rho =-0.13, p<0.001). 3) Of eight cases deemed negative or atypical by cytology, three exhibited TERT mutations, suggesting subclinical recurrence, in contrast to the consistent findings in all nine suspicious or positive cases. This highlights the potential of integrating genomics with image-based classifiers to enhance diagnostic precision. The findings underscore the need for future multicenter work aimed at refining machine learning models to achieve enhanced predictive performance across various operational parameters and settings, facilitating their implementation. Abstract:Background: Urothelial bladder cancer (UBC) is the 5th most common cancer in Western societies. The most common symptom of UBC is haematuria. Cystoscopy the gold standard for UBC detection, allows direct observation of the bladder, but is expensive, invasive, and uncomfortable. This study examines whether an alternative new urine-based diagnostic test, the DCRSHP, is cost-effective as a triage diagnostic tool compared to flexible cystoscopy in the diagnosis of UBC in haematuria patients. Methods: A model-based cost-utility analysis using cost per quality adjusted life year and life year gained, parameterised with secondary data sources. Results: If the DCRSHP is targeted at haematuria patients at lower risk of having bladder cancer e.g. younger patients, non-smokers, then it can be priced as high as £620, and be both effective and cost-effective. Sensitivity analysis found that DCRSHP is approximately 80% likely to be cost-effective across all willingness to pay values (for a QALY) and prevalence estimates. Conclusion: This analysis shows the potential for a non-invasive test to be added to the diagnostic pathway for haematuria patients suspected of having UBC. If the DCRSHP is applied targeting haematuria patients at low risk of UBC, then it has the potential to be both effective and cost-effective.
Prediction of risk of acquiring urinary tract infection during hospital stay based on machine-learning: A retrospective cohort study

Abstract:Background Healthcare associated infections (HAI) are a major burden for the healthcare system and associated with prolonged hospital stay, increased morbidity, mortality and costs. Healthcare associated urinary tract infections (HA-UTI) accounts for about 20–30% of all HAI’s, and with the emergence of multi-resistant urinary tract pathogens, the total burden of HA-UTI will most likely increase. Objective The aim of the current study was to develop two predictive models, using data from the index admission as well as historic data on a patient, to predict the development of UTI at the time of entry to the hospital and after 48 hours of admission (HA-UTI). The ultimate goal is to predict the individual patient risk of acquiring HA-UTI before it occurs so that health care professionals may take proper actions to prevent it. Methods Retrospective cohort analysis of approx. 300 000 adult admissions in a Danish region was performed. We developed models for UTI prediction with five machine-learning algorithms using demographic information, laboratory results, data on antibiotic treatment, past medical history (ICD10 codes), and clinical data by transformation of unstructured narrative text in Electronic Medical Records to structured data by Natural Language Processing. Results The five machine-learning algorithms have been evaluated by the performance measures average squared error, cumulative lift, and area under the curve (ROC-index). The algorithms had an area under the curve (ROC-index) ranging from 0.82 to 0.84 for the entry model (T = 0 hours after admission) and from 0.71 to 0.77 for the HA-UTI model (T = 48 hours after admission). Conclusion The study is proof of concept that it is possible to create machine-learning models that can serve as early warning systems to predict patients at risk of acquiring urinary tract infections during admission. The entry model and the HA-UTI models perform with a high ROC-index indicating a sufficient sensitivity and specificity, which may make both models instrumental in individualized prevention of UTI in hospitalized patients. The favored machine-learning methodology is Decision Trees to ensure the most transparent results and to increase clinical understanding and implementation of the models.

Abstract:Background:Quick and accurate identification of urinary calculi patients with positive urinary cultures is critical to the choice of the treatment strategy. Predictive models based on machine learning algorithms provide a new way to solve this problem. This study aims to determine the predictive value of machine learning algorithms using a urine culture predictive model based on patients with urinary calculi.Methods:Data were collected from four clinical centers in the period of June 2016, to May 2019. 2,054 cases were included in the study. The dataset was randomly split into ratios of 5:5, 6:4, and 7:3 for model construction and validation. Predictive models of urine culture outcomes were constructed and validated by logistic regression, random forest, adaboost, and gradient boosting decision tree (GBDT) models. Each ratio's construction and verification were repeated five times independently for cross-validation. The Matthews correlation coefficient (MMC), F1-score, receiver operating characteristic (ROC) curve with the area under curve (AUC) was used to evaluate the performance of each prediction model. The additive net reclassification index (NRI) and absolute NRI were used to assess the predictive capabilities of the models.Results:Four prediction models of urinary culture results in patients with urinary calculi were constructed. The mean AUCs of the logistic regression, random forest, adaboost, and GBDT models were 0.761 (95% CI: 0.753-0.770), 0.790 (95% CI: 0.782-0.798), 0.779 (95% CI: 0.766-0.791), and 0.831 (95% CI: 0.823-0.840), respectively. Moreover, the average MMC and F1-score of GBDT model was 0.460 and 0.588, which was improved compared to logistic regression model of 0.335 and 0.501. The additive NRI and absolute NRI of the GBDT and logistic regression models were 0.124 (95% CI: 0.106-0.142) and 0.065 (95% CI: 0.060-0.069), respectively.Conclusions:Our results indicate that machine learning algorithms may be useful tools for urine culture outcome prediction in patients with urinary calculi because they exhibit superior performance compared with the logistic regression model.

Abstract:1557 Background: Cancer and its treatment cause undesirable cancer events (UCEs). Automated warning systems could reduce the frequency and severity of UCEs by alerting the healthcare team and allocating preventative interventions. Most previous studies predicted single UCEs at the initiation of treatment. In AIM2REDUCE, we developed and evaluated a general-purpose system for predicting UCEs during outpatient systemic anti-cancer therapy. Methods: Each time a patient receives treatment, AIM2REDUCE applies machine learning to the preceding data in the electronic medical record (EMR) to predict future UCEs. We identified patients treated for aerodigestive cancers from the EMR at Princess Margaret Cancer Centre, who were randomly split into development, validation, and test cohorts. Features included cancer diagnosis, treatment sessions with doses, laboratory tests, and patient-reported symptoms. UCEs are listed in the Table. We trained LASSO regression and random forests models in the training cohort and tuned hyperparameters in the validation cohort using Bayesian optimization. We evaluated performance across discrimination, calibration, and net benefit in the test cohort. Results: The cohort included 5,760 patients who received 175,565 treatment sessions, with 13,612,746 unique data points across 102 features. Of these patients, 2,352 (40.8%) were female, the median age was 64.0 years (interquartile range 14.0), the most common diagnoses were lung cancer (2,071, 36.0%) and pancreatic cancer (926, 16.1%), and the most common treatment regimens were weekly gemcitabine (433, 7.5%) and maintenance pemetrexed (417, 7.2%). The Table shows the performance of AIM2REDUCE. Conclusions: We demonstrate that longitudinal machine learning systems trained using EMR data can accurately predict a wide range of UCEs. Based on these results, automated warning systems should be implemented and evaluated in real-time clinical practice. [Table: see text]

Abstract:BACKGROUND AND OBJECTIVE: Assessment of patients with hematuria (aH) remains a challenge in urological practice, balancing the benefits of diagnosing a potentially underlying bladder cancer (UCa) against the risks of possibly unnecessary diagnostic interventions. This study analyzes the potential of an mRNA-based urine assay, the Xpert® Bladder Cancer Detection- CE-IVD (Xpert BC-D), in patients with hematuria. MATERIALS AND METHODS: Overall, 368 patients with newly observed painless hematuria and no history of UCa were included in this observational study. Patients received urological workup, including urethrocystoscopy (WLC), upper tract imaging, urine cytology and Xpert BC-D. Patients with positive WLC were recommended to undergo tumor resection (TUR-B). RESULTS: After excluding non-assessable cases, 324 patients were considered for analysis (188 males, 136 females; median age: 61 years). Eight of twenty-eight patients with a positive TUR-B had Ta low grade (LG) tumors; the others were diagnosed with high grade (HG) lesions (Ta: 4, CIS: 2, T1:11, > T1:3). The Xpert BC-D was more sensitive than urine cytology (96% vs. 61%) (p = 0.002). Increased risk ratios (RR) were observed for gross hematuria, gender, urine cytology, and positive Xpert BC-D (all p < 0.05). Age and positive Xpert BC-D remained independent predictors of UCa in multivariate analysis. Simulating a triage with WLC restricted to patients with positive Xpert BC-D could have saved 240 (74.1%) assessments at the cost of missing one pTa LG tumor. CONCLUSIONS: The results suggest a potential role for Xpert BC-D in preselecting patients with hematuria for either further invasive diagnosis or an alternate diagnostic procedure.
