Abstract:The routine genomic surveillance of pathogens in diverse geographical settings and equitable data sharing are critical to inform effective infection control and therapeutic development. The coronavirus disease 2019 (COVID-19) pandemic highlighted the importance of routine genomic surveillance of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) to detect emerging variants of concern. However, the majority of high-income countries sequenced >0.5% of their COVID-19 cases, unlike low- and middle-income countries. By the end of 2022, many countries around the world had managed to establish capacity for pathogen genomic surveillance. Notably, Beta and Omicron; 2 of the 5 current SARS-CoV-2 variants of concern were first discovered in Africa through an aggressive sequencing campaign led by African scientists. To sustain such infrastructure and expertise beyond this pandemic, other endemic pathogens should leverage this investment. Therefore, countries are establishing multi-pathogen genomic surveillance strategies. Here we provide a catalog of the current landscape of sequenced and publicly shared pathogens in different countries in Africa. Drawing upon our collective knowledge and expertise, we review the ever-evolving challenges and propose innovative recommendations.

Abstract:The growing awareness of colonialism's role in global health partnerships between HICs and LMICs and the associated calls for decolonization in global health has led to discussion for a paradigm shift that would lead to new ways of engagement and partnerships, as well as an acknowledgement that colonialism, racism, sexism, and capitalism contribute to inequity. While there is general agreement among those involved in global health partnerships that the current system needs to be made more equitable, suggestions for how to address the issue of decolonization vary greatly, and moving from rhetoric to reform is complicated. Based on a comprehensive (but not exhaustive) review of the literature, there are several recurring themes that should be addressed in order for the inequities in the current system to be changed. The degree to which decolonization of global health will be successful depends on how the global health community in both the HICs and LMICs move forward to discuss these issues. Specifically, as part of a paradigm shift, attention needs to be paid to creating a more equal and equitable representation of researchers in LMICs in decision-making, leadership roles, authorship, and funding allocations. There needs to be agreement in defining basic principles of best practices for global partnership, including a universal definition of 'decolonization of global health'; the extent to which current policies allow the perpetuation of power imbalance between HICs and LMICs; a set of principles, best practices, and models for equitable sharing of funds and institutional costs among partners; a mechanism to monitor progress prospectively the equitable sharing of credits (e.g., leadership, authorship), including a set of principles, best practices, and models; and, a mechanism to monitor progress prospectively the extent to which decolonialization will contribute to strengthening institutional capacity in the LMIC institutions.

Abstract:Whole-genome sequencing has revolutionised the field of infectious disease surveillance, enabling near real-time detection of pathogens and tracking how infections may spread. We performed a scoping review of studies that have applied genomic epidemiology to zoonotic disease transmission across One Health domains (human, animal, and environment). We identified 114 records published between 2005 and 2022 which reported original multi-domain genomic data of zoonotic pathogens integrated into phylogenetic models. Most studies investigated bacterial pathogens, highlighting key knowledge gaps for other zoonotic agents, particularly arboviruses. Sampling and sequencing efforts vastly varied across domains: the median number and range of pathogen genomes analysed were highest for humans (23; 1-29,586) and lowest for the environment domain (13; 1-956). Infectious disease genomics was leveraged to track zoonotic disease outbreaks and cross-domain transmission, to enhance pathogen surveillance, and to disentangle evolutionary dynamics driving lineage diversification and virulence. Our study informs effective study design for future genomic applications to multi-domain and cross-species transmission of zoonoses, with the potential to identify key infection sources and inform interventions for local and global health security.

Abstract:The role that human rights can play in the global response to HIVIAIDS is crucial. People around the world continue to be placed at risk of HIV due to ongoing human rights violations. In this article--based on a public lecture he gave at "From Evidence and Principle to Policy and Action", the 2nd Annual Symposium on HIV, Law and Human Rights, held on 10-12 June 2010 in Toronto, Canada--Michel Kazatchkine, Executive Director of the Global Fund to Fight AIDS, Tuberculosis and Malaria, discusses how the lack of support for programs that protect and promote human rights is one of the failures in the response to AIDS. He stresses that advocates must reinvigorate efforts for human rights and treatment and prevention for all, including for the most marginalized populations.

Abstract:American fertility has gone through dramatic changes in the past century including the "baby boom" after World War II and the "baby bust" of the 1960s and 1970s which brought births below the re- placement level of 2.11 births per woman for the first time in recorded history. In contrast the average American woman in 1800 gave birth to seven children. Back then the U.S. was an agrarian society so children had economic value. Cities saw a trend toward lower fertility: Families in Nantucket for example began limiting the number of children as early as the 1730s. Raising kids was costly and being whalers residents had less incentive to have many children. Birth rates began dropping nationally during the 19th century because of urbanization and the decreasing supply of farmland which lessened the need for extra hands. The most popular methods of family limitation were coitus interruptus followed by the douche and the condom. By 1930 the U.S. birth rate had fallen to about a third of that recorded in 1800. Abstract:The colonial origins and power imbalances between western high-income countries (HICs) and low and middle-income countries (LMICs) are barriers to self-reliance and sustained structural improvements to healthcare systems. Radiologists working in global health (Global Radiologists) are tasked with improving the state of imaging in LMICs while mitigating the effects of colonial structures and processes. To accomplish this, we need to be aware of factors such as colonialism, neocolonialism, parachute research, and brain drain that contribute to global health inequities. Potential solutions to decolonizing global radiology include commitment to understanding local context; strengthening local capacity for technology advancement, research, and development; and policies and educational programs to combat medical brain drain from LMICs. In this article, we describe how the legacies of colonialism can interfere with improving health in LMICS, despite the best intentions and provide a call to action for decolonizing our field with intentional approaches and equitable partnerships that emphasize investments in sustainable infrastructure, robust training of personnel, and policies that support self-reliance to match true health system strengthening with our passion for addressing health equity.

Abstract:Antimicrobial resistance in healthcare-associated bacterial pathogens and the infections they cause are major public health threats affecting nearly all healthcare facilities. Antimicrobial-resistant bacterial infections can occur when colonizing pathogenic bacteria that normally make up a small fraction of the human microbiota increase in number in response to clinical perturbations. Such infections are especially likely when pathogens are resistant to the collateral effects of antimicrobial agents that disrupt the human microbiome, resulting in loss of colonization resistance, a key host defense. Pathogen reduction is an emerging strategy to prevent transmission of, and infection with, antimicrobial-resistant healthcare-associated pathogens. We describe the basis for pathogen reduction as an overall prevention strategy, the evidence for its effectiveness, and the role of the human microbiome in colonization resistance that also reduces the risk for infection once colonized. In addition, we explore ideal attributes of current and future pathogen-reducing approaches.

Abstract:Metagenomic next-generation sequencing (mNGS) is the process of sequencing all genetic material in a biological sample. The technique is growing in popularity with myriad applications including outbreak investigation, biosurveillance, and pathogen detection in clinical samples. However, mNGS programs are costly to build and maintain, and additional obstacles faced by low- and middle-income countries (LMICs) may further widen global inequities in mNGS capacity. Over the past two decades, several important infectious disease outbreaks have highlighted the importance of establishing widespread sequencing capacity to support rapid disease detection and containment at the source. Using lessons learned from the COVID-19 pandemic, LMICs can leverage current momentum to design and build sustainable mNGS programs, which would form part of a global surveillance network crucial to the elimination of infectious diseases.

Abstract:Emerging fungal pathogens pose a serious, global and growing threat to food supply systems, wild ecosystems, and human health. However, historic chronic underinvestment in their research has resulted in a limited understanding of their epidemiology relative to bacterial and viral pathogens. Therefore, the untargeted nature of genomics and, more widely, -omics approaches is particularly attractive in addressing the threats posed by and illuminating the biology of these pathogens. Typically, research into plant, human and wildlife mycoses have been largely separated, with limited dialogue between disciplines. However, many serious mycoses facing the world today have common traits irrespective of host species, such as plastic genomes; wide host ranges; large population sizes and an ability to persist outside the host. These commonalities mean that -omics approaches that have been productively applied in one sphere and may also provide important insights in others, where these approaches may have historically been underutilised. In this review, we consider the advances made with genomics approaches in the fields of plant pathology, human medicine and wildlife health and the progress made in linking genomes to other -omics datatypes and sets; we identify the current barriers to linking -omics approaches and how these are being underutilised in each field; and we consider how and which -omics methodologies it is most crucial to build capacity for in the near future.

Abstract:Contemporary global health education is overwhelmingly skewed towards high-income countries (HICs). HIC-based global health curricula largely ignore colonial origins of global health to the detriment of all stakeholders, including trainees and affected community members of low- and middle-income countries. Using the Consortium of Universities for Global Health’s Global Health Education Competencies Tool-Kit , we analyse the current structure and content of global health curricula in HICs. We identify two major areas in global health education that demand attention: (1) the use of a competency-based education framework and (2) the shortcomings of curricular content. We propose actionable changes that challenge current power asymmetries in global health education.

Abstract:Global Health Promotion, Ahead of Print. The Covid-19 pandemic reified pre-existing inequalities predicated on anti-Black racism, imperial geographical cartography, and the violent language of biomilitarism. In this reflective essay I deploy tools of historical sociology to underscore the importance of race, racism, racialization, and global responses to pandemics. I considerer the following questions. First, how can world society develop ideas and concepts for the imagination of a post-imperial global health regime? Second, can alternative futures be imagined if the monopolistic control of power, global scientific processes and knowledge regime is framed around a problematic lexicography of a Eurocentric totalizing project of being human? Lastly, if there is a scientific consensus that we need alternative futures, what kinds of knowledge is needed to bring about a post-imperial liberated order? The future of global health regime is a decolonial one predicated on a new biopolitics. I provide four paradigmatic approaches to subvert imperial global health: (i) pivoting ecocide in the imperial global health regime; (ii) abandonment of a Eurocentric conceptualization of racial hierarchy and modernity; (iii) disbanding the commodification of public health; and (iv) organizing a new world order through health reparations.

Abstract:Genomics and bioinformatics are increasingly contributing to our understanding of infectious diseases caused by bacterial pathogens such as Mycobacterium tuberculosis and parasites such as Plasmodium falciparum. This ranges from investigations of disease outbreaks and pathogenesis, host and pathogen genomic variation, and host immune evasion mechanisms to identification of potential diagnostic markers and vaccine targets. High throughput genomics data generated from pathogens and animal models can be combined with host genomics and patients' health records to give advice on treatment options as well as potential drug and vaccine interactions. However, despite accounting for the highest burden of infectious diseases, Africa has the lowest research output on infectious disease genomics. Here we review the contributions of genomics and bioinformatics to the management of infectious diseases of serious public health concern in Africa including tuberculosis (TB), dengue fever, malaria and filariasis. Furthermore, we discuss how genomics and bioinformatics can be applied to identify drug and vaccine targets. We conclude by identifying challenges to genomics research in Africa and highlighting how these can be overcome where possible.

[Contributions of postcolonial and decolonial criticisms to the contextualization of the concept of culture in Epidemiology]

Abstract:This epistemological essay aims to inform about contributions of postcolonial and decolonial criticisms to the contextualization of the concept of culture in Epidemiology. The postcolonial, based on the questioning of the British imperialism, and the decolonial thought, of Latin American origin, reveal the persistence of coloniality in culture and science. Culture with its symbolic dimension encompassing diverse meanings ranging from conservation to change and creating hierarchies that favor oppression is a polysemic concept with interdisciplinary motion since Anthropology studies. The text outlines the perspective and shape of the gaze, emphasizing the essay format: the concept of culture is presented; the relation between post- and decolonial thought and culture are indicated as relevant to epidemiology; the use of the concept is approached in the interdisciplinarity between Anthropology and Epidemiology, reviewing proposals close to the reflection on colonization; and the potential contributions in epistemic appreciation on Epidemiology and the concept of culture are summarized. Colonizing ties as the sense of domination of nature, subordination to Western biomedicine, the use of classifications and variables forged according to binarized hierarchization and oppositions, and the disregard of popular knowledge can be exposed by postcolonial and decolonial criticisms in the bases of Collective Health, and especially in hegemonic theoretical-conceptual epidemiological ordering formulated according to cultural patterns foreign to local realities. Postcolonial and decolonial criticisms can reveal the presence of scientific and cultural racism concealed in Epidemiology.

Abstract:Over the past century, classical approaches from microbiology and immunology have produced spectacular results in the control of infectious diseases. However, the recent SARS-COV-2 pandemic has highlighted our continued failure to control some infections. Other microorganisms still pose a threat to humanity such as HIV, Ebola, and influenza viruses. It seems that conventional approaches are not able to solve all the current problems caused by infectious diseases. Human genetics has shown that infections have a strong genetic determinism that can lead to a predisposition or resistance to infections. This explains much of the clinical variability observed in individuals infected with the same pathogen. The identification of the genetic etiology allows a better understanding of the pathogenesis of infectious diseases and, consequently, the consideration of appropriate preventive and therapeutic strategies. This review provides insights into the genetic theory and the concrete evidence to support it. We highlight the role of primary immunodeficiencies in the discovery of Mendelian and monogenic susceptibility to infections, then we show how genetic and phenotypic heterogeneity, redundancy, and resistance to infection manifest in the context of this genetic determinism. To effectively combat the constant threat of microbes, it is essential to integrate human genetics with microbiology to examine the interactions between pathogens and our immune system.

Abstract:Our view of the microbial world and its impact on human health is changing radically with the ability to sequence uncultured or unculturable microbes sampled directly from their habitats, ability made possible by fast and cheap next-generation sequencing (NGS) technologies. Such recent developments represent a paradigmatic shift in the analysis of habitat biodiversity, be it the human, soil, or ocean microbiome. We review here some research examples and results that indicate the importance of microbiomes in our lives and then discus some of the challenges faced by metagenomic experiments and the subsequent analysis of the generated data. We then analyze the economic and social impact on genomic medicine and research in both developing and developed countries. We support the idea that there are significant benefits in building capacities for developing high-level scientific research in metagenomics in developing countries. Indeed, the notion that developing countries should wait for developed countries to make advances in science and technology that they later import at great cost has recently been challenged.

Abstract:Understanding how human genetics influence infectious disease susceptibility offers the opportunity for new insights into pathogenesis, potential drug targets, risk stratification, response to therapy and vaccination. As new infectious diseases continue to emerge, together with growing levels of antimicrobial resistance and an increasing awareness of substantial differences between populations in genetic associations, the need for such work is expanding. In this Review, we illustrate how our understanding of the host–pathogen relationship is advancing through holistic approaches, describing current strategies to investigate the role of host genetic variation in established and emerging infections, including COVID-19, the need for wider application to diverse global populations mirroring the burden of disease, the impact of pathogen and vector genetic diversity and a broad array of immune and inflammation phenotypes that can be mapped as traits in health and disease. Insights from study of inborn errors of immunity and multi-omics profiling together with developments in analytical methods are further advancing our knowledge of this important area.

Abstract:The current decolonizing global health movement is calling us to take a post-colonial perspective at the research and practice of global health, an area that has been re-defined by contemporary scholars and advocates with the purpose of promoting equity and justice. In this article, we summarize the main points of discussion from the Symposium organized by the editorial board of Global Health Research and Policy, convened in July 2021 in Wuhan, China. Experts participating in the symposium discussed what decolonizing global health means, how to decolonize it, and what criteria to apply in measuring its completion. Through the meeting, a consensus was reached that the current status quo of global health is still replete with various forms of colonial vestiges-ideologies and practices-, and to fully decolonize global health, systemic reforms must be taken that target the fundamental assumptions of global health: does investment in global health bring socioeconomic development, or is it the other way around? Three levels of colonial vestiges in global health were raised and one guiding principle was proposed when thinking of solutions for them. More theoretical discussion needs to be explored to guide practices to decolonize global health.
Ethics and governance challenges related to genomic data sharing in southern Africa: the case of SARS-CoV-2

Abstract:Data sharing in research is fraught with controversy. Academic success is premised on competitive advantage, with research teams protecting their research findings until publication. Research funders, by contrast, often require data sharing. Beyond traditional research and funding requirements, surveillance data have become contentious. Public health emergencies involving pathogens require intense genomic surveillance efforts and call for the rapid sharing of data on the basis of public interest. Under these circumstances, timely sharing of data becomes a matter of scientific integrity. During the COVID-19 pandemic, the transformative potential of genomic pathogen data sharing became obvious and advanced the debate on data sharing. However, when the genomic sequencing data of the omicron (B.1.1.529) variant was shared and announced by scientists in southern Africa, various challenges arose, including travel bans. The scientific, economic, and moral impact was catastrophic. Yet, travel restrictions failed to mitigate the spread of the variant already present in countries outside Africa. Public perceptions of the negative effect of data sharing are detrimental to the willingness of research participants to consent to sharing data in postpandemic research and future pandemics. Global health governance organisations have an important role in developing guidance on responsible sharing of genomic pathogen data in public health emergencies.
