Abstract:BACKGROUNDGastric duplication cysts (GDCs) are a relatively uncommon congenital developmental abnormality, mainly occurring in infants but very rarely in adults. Because of the variability in clinical presentation, it is often quite challenging to diagnose GDCs in adults. We are presenting a case report of an adult diagnosed operatively as having a GDC with a literature review to summarize clinical and imaging features and the treatment selections of GDCs in adults so that doctors could have a comprehensive understanding of this disease and make a precise diagnosis and a suitable therapeutic decision for patients.CASE SUMMARYA 51-year-old man presented with recurrent epigastric pain and fullness for two years. No significant findings were noted during physical examination and routine blood tests were unremarkable. An abdominal ultrasound revealed a large cyst in the upper left abdominal quadrant. A following contrast-enhanced abdominal computed tomography (CT) scan demonstrated a hypodense cystic lesion between the spleen and stomach. The lesion had scattered calcification in the cyst wall without any significant enhancement. The lesion was initially thought to be a cystic lymphangioma. The patient underwent a surgical resection and intraoperatively it was noted that the lesion was closely adherent to the greater curvature of the stomach. Subsequently, a resection of the gastric mass along with a partial gastrectomy was performed. The patient recovered quickly with a complete symptomatic relief and did not show any further complications during the 8-month follow-up.CONCLUSIONGDCs are quite rare in adults, with a multitude of symptoms, which is quite challenging for precise diagnosis before histological examination. Some imaging techniques involving CT, magnetic resonance imaging, and endoscopic ultrasound could provide valuable morphological features for differential diagnosis.
Analysis on Diagnosis and Treatment of 42 Cases with Remnant Gastric Cancer

Abstract:Objective Through analysing 42 cases with remnant gastric cancer to explore its clinical characteristics as well as diagnosis and treatment methods. Methods Retrospective analysis of clinical data of 42 cases with remnant gastric cancer from author's hospital between 2004 and 2012 yr. Results Among the 42 patients 4 underwent laparotomy(2 had gastrojejunostomy,1 had Jejunostomy,and 1 only received surgical exploration. The 4 cases received 5-FU particles),8 received palliative remnant gastrectomy,26 underwent total resection combined with gastric +D2 lymphadenectomy. After surgery 30 patients were followed up for more than 3years. The 4 patients underwent laparotomy survived less than 8 months. The survival rate of 5 patients who underwent palliative operation in year 1 was 60%(3 / 5)and none of them survived more than 3 years. Among the21 cases underwent radical surgery their survival rates were 100% and 71.4%(15 / 21) for year 1 and year 3,respectively. Conclusion Regular endoscopy within the first 10 years after partial resection of the stomach is the key to early diagnosis of remnant gastric cancer. Radical surgery is the primary method for treating remnant gastric cancer.
Resection of GIST in the Duodenum and Proximal Jejunum: A Retrospective Analysis of Outcomes

Abstract:Introduction: Gastrointestinal stromal tumors (GISTs), with a primary occurrence in the duodenum and proximal jejunum, are rare and treatment is poorly understood. This study aimed to evaluate the main factors influencing the prognosis of GIST resection in this complex anatomical structure. Materials and methods: This retrospective study included 47 patients who underwent surgery for primary GIST of the duodenum (20) and proximal jejunum (27) between 2012 and 2017. Perioperative clinical data as well as relapse and survival information were collected. Results: All patients underwent negative margin resection (R0) of duodenal and proximal jejunum GISTs. Complications occurred more frequently in treatment of duodenal GISTs than proximal jejunum GISTs (p = 0.003). GISTs in D3 (the 3rd portion of duodenum) were related to larger tumor size (p = 0.001), higher probability of severe complication rate (p = 0.042), longer hospital stays (p = 0.023) and fasting time (p = 0.020). More complications were found for patients with digestive reconstruction than limited resection (p = 0.010). Additionally, patients with a tumor mass larger than 5 cm or a mitotic index greater than 5 mitoses/50 HPFs showed poorer therapeutic outcomes. The 1- and 3-year overall survival was 97.9% and 86.1%, respectively and were not influenced by operation type (p = 0.061) or GIST position (p = 0.447). Conclusion: With negative operational margins, limited resection is a safe and feasible procedure for duodenal and proximal jejunum GIST patients and unnecessary digestive reconstruction should be avoided. Considering the severe complication rate, resection for GISTs in D3 should be performed with care. (C) 2019 The Authors. Published by Elsevier Ltd. Clinicopathological Characteristics and Prognosis of Remnant Stomach Cancer——report of 45 Cases

Abstract:OBJECTIVE:To investigate the clinicopathological characteristics and prognostic factors of remnant stomach cancer.METHODS:The clinicopathological and prognosis data of 45 patients with remnant stomach cancer were retrospectively analyzed.RESULTS:The remnant stomach cancer are likely to develop in males with a ratio of male to female: 44:1. Their initial operation modes of these patients were Billroth II subtotal gastrectomy in 40 patients, Billroth I subtotal gastrectomy in 4 and proximal subtotal gastrectomy in 1. The interval from the initial operation to the diagnosis of remnant stomach cancer was 5 to 42 years with an average of 23 years. Of these 45 patients, 28 had lesion at anastomotic site, 9 in the gastric cardia and 8 in other locations; 19 had radical resection, 16 palliative resection and 10 exploration alone except one who had an anastomosis of remnant stomach with the jejunum. The histology types included: 1 un-differentiated adenocarcinoma, 36 poorly-differentiated adenocarcinoma, 7 moderately-differentiated adenocarcinoma and 1 well-differentiated adenocarcinoma. The 1-, 3-, 5-year survival rates of patients with radical resection were significantly better than those treated with palliative resection, which was 100% vs. 62.5%, 78.8% vs. 25%, 47.2% vs. 0, respectively (P < 0.05). All ten patients without resection died within 2 years with an average survival time of 12 months. The 5-year survival rate of stage I, II, III and IV was 100%, 75%, 17.8% and 0, respectively (P < 0.05).CONCLUSION:Remnant stomach cancer prevalently occurs in the male usually 10 years after Birroth II gastrectomy. The lesions is mainly located at anastomotic site. Poorly-differentiated adenocarcinoma is found to be the prevalent histological type of advanced remnant stomach cancer. The prognosis of remnant stomach cancer is correlated with pTNM stage and whether having been treated with complete resection or not. Patients with early remnant stomach cancer may survive for a long time if radical resection can be done.
Laparoscopic ventral mesh rectopexy for obstructed defaecation syndrome: time for a critical appraisal

Abstract:Obstructed defaecation syndrome (ODS) is a symptom complex consisting of a sense of incomplete evacuation, prolonged straining and a persisting desire to defaecate. In patients with ODS, structural abnormalities of the colorectum and the pelvic floor (including rectal intussuception, rectocele, enterocele and prolapse of the middle pelvic compartment) are nearly always present [1]. The aetiology and nature of rectal intussusception are debatable. Some believe that internal prolapse is a primary condition that progressively leads to a complete external protrusion of the rectum, and others think that folding of the rectal wall is secondary, being related to excessive straining as a consequence of altered rectal motility and hyposensitivity [2]. Assuming that external rectal prolapse is the end-point of a successive rectal wall descent, it is obvious to consider the same surgical approach for rectorectal and rectoanal intussusception as for rectal prolapse. Over the years, more than 100 different procedures have been described for surgical treatment of rectal prolapse but the optimal surgical approach has yet not been determined. The existence of so many surgical options is an indicator of the lack of one single successful procedure. There is considerable controversy regarding the indications for the different surgical interventions, and most evidence is derived from case series and nonrandomized cohort settings with only short-term follow up. As for external rectal prolapse, a variety of different operations have been described for rectal intussusception. Generally, a folding of the rectal wall can be prevented either by resection or by fixation. The underlying concept for both techniques is an attempt to correct the structural abnormality of the rectum and the pelvic floor. Rectopexy, with or without resection of any redundant sigmoid colon, and with or without insertion of a mesh, can be performed by open or laparoscopic surgery. Detailed data on the functional and longterm outcome are limited, and the published series are small. Stapled transanal rectal resection, known as the STARR procedure, was first described by Longo [3]. By using a circular stapler, this operation aims to remove redundant rectal wall, but the descent of the rectum and the pelvic floor is not corrected. Several specialized circular staplers, such as the Contour Transtar stapler and the TST STARR stapler, have subsequently been developed with the aim of excising an increasing amount of prolapsing tissue to improve the outcome. Serious and sometimes life-threatening complications, such as rectal perforation, severe bleeding with large hematoma formation and sepsis, have been reported following the STARR procedure. Faecal incontinence, anovaginal fistula formation and intractable pelvic pain are other complications that are unacceptable sequelae after treatment for a benign disorder, and there is a certain scepticism among many surgeons of these techniques [4]. Laparoscopic ventral mesh rectopexy for rectal prolapse and ODS has achieved widespread use and popularity, especially in Europe, over the past decade. The method was first described in 2004 by D’Hoore et al. [5] who developed the technique as a laparoscopic nerve-sparing modification of the sutured posterior rectopexy that included complete mobilization of the rectum to the pelvic floor. Dissection posterior and lateral to the rectum may cause autonomic nerve damage and result in disturbance of the rectosigmoid motility and rectal reservoir capacity. Several previous studies have shown that up to 50% of all patients experience postoperative constipation and obstructed defaecation after posterior rectopexy [6,7]. Subsequently, laparoscopic ventral mesh rectopexy has been offered as a treatment option to patients with internal rectal prolapse and ODS. The anatomical abnormalities in the middle pelvic compartment are corrected by insertion of a mesh between the rectum and the vagina. The vaginal vault is fixed to the mesh, and the pouch of Douglas is thereby obliterated and the anterior rectal wall reinforced, aiming to prevent formation of enterocele, intussusception and rectocele during defaecation [5]. There are several controversies surrounding the increasing use of ventral mesh rectopexy. First, only Level 3 evidence exists. Unfortunately, there is a complete lack of systematic reviews and randomized controlled trials regarding this procedure for internal rectal prolapse, ODS or faecal incontinence. Abstract:Gastric cancer in remnant stomach is a rare tumor but with poor prognosis. Compared with conventional open surgery, laparoscopic gastrectomy has potential benefits for these patients due to advantages resulting from its minimally invasive approach. Herein, we report on three patients with gastric cancer in remnant stomach who underwent laparoscopic total gastrectomy with intracorporeal esophagojejunostomy successfully. The operative time was 280, 250 and 225 minutes, the estimated blood loss was 100, 80 and 50 ml and the length of postoperative hospital stay was seven, eight and nine days respectively. Our experience has suggested that laparoscopic total gastrectomy with intracorporeal esophagojejunostomy can be a safe, feasible and promising option for patients with gastric cancer in remnant stomach.
The mass effect of a slowly growing GIST

Abstract:Gastrointestinal stromal tumours (GISTs) are typically defined as solid masses arising from the GI tract, most commonly from the stomach and small intestine. They seldom present in a cystic form. Management of cystic masses arising from the GI tract may pose a diagnostic predicament. We had one such case that presented itself with complaints of a slow growing intra-abdominal mass. An ultrasound scan demonstrated a thick-walled cystic lesion arising from the pelvis. Further imaging evaluations in the form of a CT scan revealed a complex large cystic mass arising from left upper quadrant (see Figure 1). Due to the uncertainty of origin of this mass and lack of invasion or lymphadenopathy, it was thought to be benign. After a multidisciplinary meeting, it was concluded that an urgent surgical excision of this benign mass was the best treatment. The surgical treatment of which entailed a 10 hours surgery to resect this 10 kg lesion, which comprised 7 L fluid and 3 kg solid mass. Histopathology aided in the diagnosis of this lesion as a CD117-positive and DOG1-positive GIST.

Abstract:Background and Objectives: Gastrointestinal stromal tumors (GISTs) are rare mesenchymal neoplasms located mainly in the fundus (60-70%). The incidence of GIST is approximately 10 per million population per year in Europe, with a peak incidence at the age of 63. Recent studies suggest that morbidly obese patients have a higher incidence of GIST than the general population. The aim of this study was to analyze the incidence of GIST in patients undergoing laparoscopic sleeve gastrectomy (LSG) in our department. Materials and Methods: this paper present the retrospective study of prospectively collected data of 1564 patients who underwent LSG in a single large bariatric center from October 2013 to September 2021. After surgery, each sample of the resected stomach was sent for histopathological examination. For the analysis, we included patients diagnosed with GIST intraoperatively or postoperatively. Results: GISTs were found in five patients (0.31%). There were three men and two women. The mean age was 50.2 (range 32-63 ± 11.8) and the mean preoperative body mass index was 43.3 kg/m2 (40-49.4 ± 3.2). In four cases, GISTs were found in the fundus (80%), and in one in the pylorus (20%). None of the tumors were larger than 7 mm in diameter and all were diagnosed as a very low-risk category. No adjuvant treatment was required. All patients achieved good or satisfactory bariatric and metabolic results. Conclusions: The incidence of GIST in our study was estimated at 0.31%. All patients had a very low-risk GIST and no recurrence until follow-up. Recent literature suggests that the risk of GIST is higher in the obese population, and therefore surgeons should be aware of the risk of incidental GIST during LSG.

Abstract:In the United States, gastric cancer has an incidence of 5.6 per 100,000 individuals, and adenocarcinoma represents 90%-95% of all stomach cancers. Roux-en-y gastric bypass (RYGB) is a surgery performed by creating a small gastric pouch from the existing stomach along with a jejunojejunostomy and gastrojejunostomy; weight loss occurs through restriction and malabsorption. Late complications of RYGB include anastomotic strictures, fistulae, and various nutritional deficiencies. Rarely are cases of gastric cancer reported developing in the remnant stomach, such as a narrative review describing 17 cases in which 80% were adenocarcinoma.

Combined endoscopic/laparoscopic intragastric resection of gastric stromal tumors

Abstract:Myogenic neoplasms of the stomach are the most common submucosal mass. Their natural history is indeterminate, and surgical resection is advised regardless of size. These lesions have typically required open resection, but a variety of laparoscopic techniques have been described. We report results of endoscopically guided, laparoscopic intragastric resection. Fourteen lesions have been excised in 13 patients in the last 3.5 years. There were eight women and five men with a mean age of 57 years (range 34-72). All patients were asymptomatic, and no lesions had mucosal ulceration. Eight lesions were located at the gastroesophageal junction, two each at the incisura and posterior body, and one each in the fundus and anterior wall of the corpus. All lesions were predominantly intraluminal, and three were transmural. The diagnosis of a myogenic lesion was confirmed by endoscopic ultrasound in eight patients. The laparoscopic/endoscopic technique included two or three, 2 or 5 mm intragastric trocars; endoscopic suture passage and specimen removal; and laparoscopic intragastric suture repair of the gastric defect. The mean operative time was 186 minutes. The mean size of the resected specimens was 3.8 cm (range 1.5-7.0). There was no mitotic activity on histopathology, and all were considered pathologically benign. The median length of stay was 3.8 days (range 3-8). There was no mortality or operative morbidity. At a mean follow-up of 16.2 months (range 1-32) there has been no local recurrences. A combined laparoscopic/endoscopic intragastric resection is most appropriate for intraluminal, benign-appearing submucosal lesions of the proximal stomach.
Urinary bladder carcinoma: impact of smoking, age and its clinico-pathological spectrum.

Abstract:BACKGROUND Urinary bladder carcinoma is common urological malignancy. Although epidemiological evidence favors role of occupational exposure to chemical carcinogen as the aetiological factor of bladder carcinoma, many cases arise with no obvious occupational exposure to chemical carcinogen. Tobacco and cigarette smoking is common in both rural and urban areas of Nepal. OBJECTIVE The objective of this study was to determine the impact of smoking and age in urinary bladder carcinoma with related clinicopathological correlations. METHOD A total of 56 (44 males and 12 females) cases of urinary bladder cancer treated at Dhulikhel Hospital, Kathmandu University Teaching Hospital during time period of January 2004 to December 2013 were included in the study. Data of patients with Urinary bladder cancer were obtained from hospital records and evaluated for age, sex, history of smoking, clinical presentations, cystoscopic findings and histopathological characteristics. RESULTS Out of 56 cases, 51 (91.1%) of the patients had hematuria. History of smoking was found in 44 patients. Smoking was found much higher in males (88%) than females (41.66%). Transitional cell carcinoma (TCC) was the most common histological variety, which was seen in 51 (91.07%) patients. The significant impact of smoking was found in terms of grade of TCC. CONCLUSION The incidence of bladder carcinoma is higher in male and TCC is the most common variety of Urinary bladder malignancy. History of smoking correlated with grade.

Abstract:This paper presents new type of test structures based on the Pirani principle. They were realized together with a gyroscope in a 50 mum SOI (silicon-on-insulator)-based technology to measure the encapsulated pressure of Coriolis vibrating gyroscopes (CVG) but might also be adapted to other devices. Design and optimization of the test structures was carried out by an analytical model that accounts for the main technological parameters and enables a good estimate of the precision of the developed test method. The achieved measurement accuracy allows a good separation of encapsulated from non-encapsulated gyroscopes. Further improvements are expected by application of dynamic instead of static test methods.

Abstract:Background: Gastritis cystica profunda (GCP), commonly observed in remnant gastric anastomosis, is associated with developing gastric cancer. Case: This case report describes a patient with GCP in a previously unoperated stomach that mimicked a pyloric submucosal tumor and caused anorexia, which is rare in clinical practice. Patient concerns: A 72-year-old woman presented with loss of appetite and weight. Diagnoses: Gastroscopy detected a 20 mm diameter submucosal tumor near the pylorus. Computed tomography and magnetic resonance imaging identified a cystic lesion, unlike a usual submucosal tumor in the stomach. The diagnosis was difficult, even with endoscopic ultrasound-guided fine-needle aspiration. Interventions: Surgery was performed for diagnosis and treatment. The lesion was resected using a submucosal dissection technique after an incision of the gastric wall during open laparotomy. Histopathological examination confirmed the diagnosis of GCP and revealed no dysplasia or cancer. Outcomes: Anorexia resolved after the surgery. Residual or recurrent lesions were not detected during follow-up examinations performed 1 year after surgery. Lessons: GCP occurring in a previously unoperated stomach as a macroscopic lesion like a submucosal tumor causing some symptoms is rare. GCP is associated with a risk of developing cancer. Therefore, careful evaluation and management during treatment are required.

Significance of resected stomach measurements in postoperative delayed gastric emptying following laparoscopic pylorus-preserving gastrectomy

Abstract:Purpose: We investigated the relationship between the resected stomach measurements, the incidence of delayed gastric emptying (DGE), and food residue 1 year after surgery in patients who underwent laparoscopic pylorus-preserving gastrectomy (PPG). Materials and methods: The DGE group included 10 patients fasting due to nausea, vomiting, abdominal distension, or remnant stomach distension on radiographs; the control group included 36 patients without these symptoms. We compared the size and length of lesser and greater curvatures of the resected stomach and endoscopic findings after 1 year. Results: No significant differences were observed between groups in terms of sex, body mass index, gross type, histology, tumor progression, number of dissected lymph nodes, operating time, or blood loss. The DGE group was older, had a longer postoperative stay, and showed a smaller size and shorter greater curvature of the resected stomach than the control group (p < 0.01 for all). No difference was observed in the length of the lesser curvature of the resected stomach. In addition, there were no disparities in residual food, degree and extent of gastritis, or bile reflux 1 year after gastrectomy. Conclusions: Measurements of the resected stomach suggest that preventing DGE may be achievable by removing a larger area of the greater curvature and/or stomach during laparoscopic PPG. This implies potential surgical strategy improvements for better outcomes. Further multicenter trials are needed to validate and refine techniques.

Abstract:A new scheme, deduced from Khan’s homotopy perturbation transform method (HPTM) (Khan, 2014; Khan and Wu, 2011) via optimal parameter, is presented for solving nonlinear differential difference equations. Simple but typical examples are given to illustrate the validity and great potential of Khan’s homotopy perturbation transform method (HPTM) via optimal parameter in solving nonlinear differential difference equation. The numerical solutions show that the proposed method is very efficient and computationally attractive. It provides more realistic series solutions that converge very rapidly for nonlinear real physical problems. The results reveal that the method is very effective and simple. This method gives more reliable results as compared to other existing methods available in the literature. The numerical results demonstrate the validity and applicability of the method.
Metachronous remnant gastric cancer after proximal gastrectomy.

Abstract:260 Background: Proximal gastrectomy is frequently selected for early gastric cancer located at the upper third of the stomach. Annual postoperative endoscopy is considered to be useful for the early detection of metachronous remnant gastric cancer (MRGC). Despite that, we sometimes have also encountered cases that are not suitable for endoscopic resection (ER). The aim of this study is to identify the risk factors for the patients who developed MRGC after proximal gastrectomy, received annual endoscopy, but are not eligible for ER (non-ER). Methods: We retrospectively analyzed 191 patients who had received annual endoscopic surveillance after undergoing PG for cT1 gastric cancer at NCCH between 2006-2015. MRGCs were categorized into two groups: ER group and non-ER group. The remnant stomach was defined as three locations: pseudo-fornix (PF), corpus and antrum. Results: The median observation period was 73 (range 12-168) months. MRGC was observed in 29 cases with 32 lesions. ER was eligible for 20 lesions (ER group, 62.5%), but 12 were not (non-ER group, 37.5%). We compared the characteristics between of ER group and non-ER group in terms of sex, age (>65 years), interval from the initial surgery (>60months), the location (PF vs non-PF), and cross-sectional circumference (posterior wall vs. others). In both univariate and multivariate logistic regression analysis, only the location at PF was identified as a risk factor for non-ER group (OR 25.0, 95% CI: 1.8–339, p = 0.015). Among 12 lesions of the non-ER group, the location of PF was found in 6 (50%) lesions. As compared with MRGC at the non-PF, that at the PF (n = 6) was characterized by younger patients (57.5 (range 37-69) years vs. 69 (56-77) years), larger size (36 (7-57) mm vs. 9 (9-24) mm), deeper depth (MP or deeper; 5/6 vs. 0/6), more frequent undifferentiated-type histology (4/6 vs. 0/6) and frequent nodal metastasis (3/6 vs. 0/6). The interval to detect MRGC appears to be longer in the PF (78 (31-96) months than that of the non-PF (60 (31-96) months), but the difference was not statistically significant (p = 0.86). Then, we examined the visibility and the mucosal normality of the area developing MRGC in the annual follow-up endoscopy one year before the detection of MRGC (n =32). In 25 lesions at the non-PF, the visibility and the mucosal normality was secured in 21 (84%). while 5 PF lesions were difficult to observe (71.4%, p = 0.01) due to food residues in 4 (57%, p = 0.047) and insufficient expansion of the gastric mucosa in 4 (57%, p = 0.001). Conclusions: Detection of the MRGC at the PF at an early stage is crucial and challenging. With annual follow-up endoscopy, complete clearance and adequately extending the gastric mucosa could lead to the early detection of MRGC at the PF.

Abstract:Gastrointestinal stromal tumors (GISTs) are rare mesenchymal tumors occurring in the gastrointestinal tract particularly the stomach or small intestine originating from interstitial cells of Cajal. This case report describes a 50-year-old postmenopausal female presenting with a gradually increasing abdominal mass which clinically was thought to be a neoplasm originating in the ovaries. A clinical and imaging diagnosis of primary ovarian malignancy was made but during laparotomy, a mesenteric component to the malignancy as well as bilateral ovarian cysts were seen. The mass was removed with care and histopathological analysis confirmed it to be GIST. Follow-up of the patient was done for three years and there was no sign of any disease in the patient and she had an uncomplicated postoperative period. This case describes the intricacy of GISTs' diagnosis, the significance of detailed intraoperative analysis, and appropriate postoperative surveillance. Differences and similarities with other similar cases shed light on how such patients present themselves for treatment, thus encouraging differentiated care. Supervisory care is therefore vital in the monitoring of the patient for prolonged periods and to check for any relapse.
A rare case of gastric schwannoma mimicking GIST: diagnostic and therapeutic approach

Abstract:Gastric schwannomas are rare, benign neurogenic tumors originating from Schwann cells within the gastrointestinal tract, comprising only 0.2% of all gastric tumors. This report presents the case of a 32-year-old female patient who experienced severe periumbilical pain, nausea, and vomiting, ultimately diagnosed with gastric schwannoma. Initial imaging and endoscopic evaluations suggested a gastrointestinal stromal tumor (GIST), but postoperative histopathological analysis confirmed schwannoma, showing S-100 positivity and negativity for CD117, DOG-1, SMA, Desmin, and CD34. The patient underwent successful central gastrectomy with negative surgical margins and no metastasis. Despite a postoperative complication of small bowel obstruction, which was managed conservatively, the patient remained symptom-free with no recurrence over the follow-up period. This case underscores the importance of differential diagnosis, distinguishing schwannomas from GISTs and other submucosal lesions through thorough histopathological and immunohistochemical analyses, and highlights the efficacy of complete surgical resection in preventing recurrence.

Abstract:Introduction: Gastrointestinal stromal tumors (GISTs) represent a unique subset of neoplasms within the digestive tract. They can manifest in various organs throughout the digestive tract, ranging from the oral cavity to the anus, with a predilection for the stomach and small intestine. A distinct subtype of GISTs, known as Extra-gastrointestinal stromal tumors (EGISTs), originate outside the typical GIST organs such as the mesentery, retroperitoneum, and occasionally the omentum. EGISTs are relatively rare, accounting for <5 % of all GIST cases. Presentation of case: We present the case of a 30-year-old female patient who presented with an upper abdominal mass associated with anorexia, fever, and weakness. Radiographic imaging revealed a cystic mass beneath the left hypochondrium. Exploratory laparoscopy confirmed the presence of an isolated mass adherent to the gastrocolic ligament and greater omentum. Histopathological examination confirmed GIST, characterized by spindle-shaped cells with DOG1 and CD117/C-kit expression. The patient underwent successful tumor resection and was discharged home with postoperative imatinib therapy. Follow-up at 14 months showed no recurrence. Discussion: The rarity of Gastrointestinal Stromal Tumors (GISTs) in the Greater Omentum highlights diagnostic challenges and underscores the need for further research. Immunohistochemical analysis aids in diagnosis, with Ki-67 staining indicating a high-risk classification. Surgery remains the primary treatment, with potential adjuvant therapy utilizing Imatinib mesylate. Conclusion: Our case underscores the rare occurrence of GIST in the Greater Omentum. Despite their infrequency, EGISTs should be considered in intra-abdominal masses, emphasizing accurate diagnosis for appropriate management and the need for further research.
