Abstract:Inflammatory bowel disease (IBD), including Crohn disease and ulcerative colitis, with multifactorial etiologies has led to a global health-associated burden in many countries. Substantial efforts are devoted to understand the pathogenesis, behavioral and environmental triggers, which may be specifically valuable for the treatment of IBD. The specific pathogenesis underlying IBD is as yet incompletely understood. The use of anti-cytokine therapy and small molecule agents targeting the immune system is thought to restore the body's intestinal barrier function and relieve inflammation with manageable adverse effects. In this review, we report recent advances in anti-cytokine therapy and treatment with small molecule agents for the management of IBD.
The Effect of Probiotics on Treatments in Patients Affected by Inflammatory Bowel Disease: A Systematic Review

Abstract:Background: Gut dysbiosis has been associated with development and progression of inflammatory bowel disease (IBD). Over the last two decades, many publications have focused on the role of probiotics in IBD. However, studies regarding the treatment of IBD with probiotics have not yielded consistent results. Objective: To evaluate the efficacy and safety of probiotics in IBD therapy. Results: A total of seven studies met the criteria and were included in the meta-analysis, which included 879 participants. We finally selected five endpoints to evaluate the efficacy of probiotics, 3 indicators showed the statistical difference, including C-reactive protein (CRP)(OR: -2.45, 95% CI: -3.16, -1.73, P< 0.01), the number of fecal Bifidobacterium (OR: 3.37, 95%CI: 3.28, 3.47, P< 0.01) and Lactobacillus(OR: 2.00, 95%CI: 1.91, 2.09, P< 0.01) . The other two indicators including the disease activity of Crohn’s Disease (CD) (OR: -0.17, 95%CI: -1.46, 1.12, P=0.80) and ulcerative colitis(UC) (OR: -0.31, 95%CI: -1.36, 0.74, P=0.56) showed no statistical difference, while the OR reflected the beneficial relationship between probiotics and IBD. Conclusion: Supplementation of probiotics can improve clinical symptoms in IBD patients, reduce the serological inflammation level, and regulate the intestinal microenvironment, but more RCTs are needed.

Abstract:Proton pump inhibitors (PPIs) are the most used acid-inhibitory drugs, with a wide range of applications in the treatment of various digestive diseases. However, recently, there has been a growing number of digestive complications linked to PPIs, and several studies have indicated that the intestinal flora play an important role in these complications. Therefore, developing a greater understanding of the role of the gut microbiota in PPI-related digestive diseases is essential. Here, we summarize the current research on the correlation between PPI-related digestive disorders and intestinal flora and establish the altered strains and possible pathogenic mechanisms of the different diseases. We aimed to provide a theoretical basis and reference for the future treatment and prevention of PPI-related digestive complications based on the regulation of the intestinal microbiota.

Abstract:Proton pump inhibitors (PPIs) are the most commonly prescribed drugs for the treatment of non-erosive reflux disease (NERD), ulcers associated with non-steroidal anti-inflammatory drugs (NSAIDs), esophagitis, peptic ulcer disease (PUD), Zollinger–Ellison syndrome (ZES), gastroesophageal reflux disease (GERD), non-ulcer dyspepsia, and Helicobacter pylori eradication therapy. The drugs have the effect of inhibiting acid production in the stomach. According to research, PPIs can affect the composition of gut microbiota and modulate the immune response. Recently, there has been a problem with the over-prescription of such drugs. Although PPIs do not have many side effects, their long-term use can contribute to small intestinal bacterial overgrowth (SIBO) or C. difficile and other intestinal infections. Probiotic supplementation during PPIs therapy may provide some hope in the reduction of emerging therapy side effects. This review aims to present the most important effects of long-term PPI use and provides critical insights into the role of probiotic intervention in PPI therapy.

Abstract:Proton pump inhibitors (PPIs) are some of the most commonly prescribed drugs worldwide, but there are increasing concerns about digestive complications linked to PPIs. Next-generation sequencing studies have suggested that PPIs can significantly affect the composition of the gut microbiota, which in turn may substantially contribute to the development of these complications. Recently, emerging evidence has suggested that the translocation of oral microbes into the gut may be the primary mechanism underlying the alterations in the gut microbiota induced by PPIs in the presence of gastric acid suppression and impaired oral–gut barrier function. Moreover, the significance of oral–gut microbial translocation in health and disease conditions has gained increasing recognition. Consequently, it is imperative to enhance our understanding of the functions of the oral–gut microbiota axis in digestive disorders associated with PPI therapies. This review aims to summarize current research findings and further elucidate the contribution of the oral–gut microbiota to the pathogenesis of PPI-related digestive diseases. We aim to provide a theoretical foundation for future therapeutic and preventive strategies targeting PPI-related digestive complications through modulation of the oral–gut microbiota.

Abstract:Abstract Background Proton pump inhibitors (PPIs) are the most efficacious and common medications for gastric acid suppression. However, PPIs continue to perpetuate safety concerns due to the availability as an over-the-counter medication. This uncontrolled use of PPIs has recently been shown to be associated with the increased health risks. Main body of the abstract The inhibition of gastric acid production by irreversibly binding to and inhibiting the H + /K + ATPase enzyme system can cause structural and physiologic changes in the GI microbiome, GI physiology, and pH. With the recent guideline updates from American Gastroenterological Association regarding deprescription of PPIs, this review focuses on the complications of long-term use of PPIs on various systems, gut microbiome, intestinal barrier and inflammatory bowel disease (IBD). Short conclusion If PPI use in IBD patients is associated with increased risk of other adverse outcomes, considering the PPI-associated mineral, electrolyte and microbial alterations also needs rigorous evaluation.
The Relationship between Helicobacter pylori and Inflammatory Bowel Disease Based on Epidemiological Data: A Systematic Review and Meta-Analysis

Abstract:Background: Helicobacter pylori (H. pylori) infection may be a protective effect against inflammatory bowel disease (IBD). The previous meta-analysis was flawed. This study integrated epidemiological data and rearranged a meta-analysis to identify the correlation between IBD and H. pylori. Moreover, whether IBD medication and classification affecting H. pylori infection, and whether H. pylori eradication leading to recurrence of IBD were analyzed in this study. Methods: Articles published until May 1, 2019, in the PubMed, MEDLINE and Embase databases were searched. Spearman correlation analysis and meta-analysis were performed after collecting and collating the relevant data. Sensitivity analysis and meta-regression were used to evaluate the reliability and sources of heterogeneity. Findings: 59 studies of IBD prevalence, 127 studies of H. pylori prevalence, and 23 studies for meta-analysis were included. IBD, Crohn's disease (CD) and ulcerative colitis (UC) was negatively correlated to H. pylori prevalence respectively (p<0.001, p<0.001, p<0.001). In addition, meta-analysis results showed that IBD, CD and UC were 0.44, 0.36, 0.54 times more likely to have H. pylori infection compared to controls (OR=0.44, 95% CI=0.34-0.59; OR=0.36, 95% CI=0.26-0.49; OR=0.54, 95% CI=0.4-0.72). Moreover, IBD patients were 1.41 times (OR=1.41, 95% CI=1.25-1.58) more likely to relapse after H. pylori eradication compared to no H. pylori eradication. Finally, H. pylori infection was not related to IBD medication and classification. Interpretation: H. pylori prevalence was negatively correlated to IBD and H. pylori was a protective effect against IBD. Furthermore, H. pylori eradication can lead to the recurrence of IBD. Abstract:Proton pump inhibitors (PPIs) are known as a class of pharmaceutical agents that target H+/K+-ATPase, which is located in gastric parietal cells. PPIs are widely used in the treatment of gastric acid-related diseases including peptic ulcer disease, erosive esophagitis and gastroesophageal reflux disease, and so on. These drugs present an excellent safety profile and have become one of the most commonly prescribed drugs in primary and specialty care. Except for gastric acid-related diseases, PPIs can also be used in the treatment of Helicobacter pylori infection, viral infections, respiratory system diseases, cancer and so on. Although PPIs are mainly used short term in patients with peptic ulcer disease, nowadays these drugs are increasingly used long term, and frequently for a lifetime, for instance in patients with typical or atypical symptoms of gastroesophageal reflux disease and in NSAID or aspirin users at risk of gastrotoxicity and related complications including hemorrhage, perforation and gastric outlet obstruction. Long-term use of PPIs may lead to potential adverse effects, such as osteoporotic fracture, renal damage, infection (pneumonia and clostridium difficile infection), rhabdomyolysis, nutritional deficiencies (vitamin B12, magnesium and iron), anemia and thrombocytopenia. In this article, we will review some novel uses of PPIs in other fields and summarize the underlying adverse reactions.

Abstract:Inconsistent data exist on the use of proton pump inhibitors (PPI) in inflammatory bowel disease (IBD) patients. Some studies suggest that PPIs increase the risk of IBD complications. Others believe there is no increased IBD risk with PPI use after eliminating protopathic bias. This ongoing debate necessitates further research. While most studies focus on new IBD diagnoses or complications in patients already on PPIs, limited studies have assessed the risk of starting PPI in patients with IBD on biologics or immunomodulators. We aim to evaluate the risk of IBD complications, such as flares, bleeding, abscesses, fistulas, and intestinal obstruction, in this group of patients.

Abstract:BackgroundProton pump inhibitors are widely used as a class of drugs for the treatment of gastric acid‐related disorders. Their impact in liver diseases is also increasingly being noted.AimA systematic and comprehensive overview of the impact of proton pump inhibitors in liver diseases and their effects on the liver.MethodA manual and comprehensive search of the PubMed database was conducted to obtain relevant literature.ResultProton pump inhibitors alter the composition and function of the intestinal microflora and may lead to small intestinal bacterial overgrowth and bacterial translocation, which are associated with a range of adverse effects caused by its presence in liver diseases. They may increase the risk of hepatic encephalopathy, spontaneous bacterial peritonitis, different types of infections and increase mortality in cirrhosis. Proton pump inhibitors may lead to an increased risk of hepatic carcinoma, although the mechanism is unknown, and this relationship is still controversial. Proton pump inhibitors also have an impact on the direct‐acting antiviral regimen in patients with chronic hepatitis C. They have been associated with an increased risk of liver abscesses and increased mortality. It is possible that they may lead to a range of metabolic risk events such as liver steatosis and weight gain.ConclusionProton pump inhibitors have been associated with several adverse outcomes in liver diseases, although there is still no consistency in many respects. Abstract:Background: Proton pump inhibitors (PPIs) are among the most commonly used medications by patients due to its availability over the counter and frequent prescribing by physicians to treat and alleviate symptoms of gastroesophageal reflux disease. Recently, the FDA issued a warning with respect to the utilization of PPIs and risk of developing Clostridium difficile infections (CDI). The most commonly known medications to cause CDI are antibiotics. However, available studies suggest an association and increase in risk for CDI with PPI use as well. Objective: The purpose of this research is to review and summarize data currently available on the association between PPIs and CDI. Methods: To search for eligible studies, EBSCO engines were investigated using proton pump inhibitors or PPIs and Clostridium difficile or C. diff. as search terms. Meta analyses and systematic reviews published between 2000 and 2020 on adult patients were considered. Results: Eight meta-analyses and systematic reviews met the inclusion criteria. They included studies conducted in the US, Europe, Asia and Canada on inpatient and outpatient adults. The final result for all 8 studies showed a statistically significant association between PPIs and CDI ranging from mild to high risk. Conclusion: Currently available data suggest a positive association between PPIs and CDI.

Abstract:Experimental evidence supports the fact that changes in the bowel microflora due to environmental or dietary factors have been investigated as implicating factors in the etiopathogenesis of inflammatory bowel disease (IBD). The amassing knowledge that the inhabited microbiome regulates the gut physiology and immune functions in IBD, has led researchers to explore the effectiveness of prebiotics, probiotics, and synbiotics in treating IBD. This therapeutic approach focuses on restoring the dynamic balance between the microflora and host defense mechanisms in the intestinal mucosa to prevent the onset and persistence of intestinal inflammation. Numerous microbial strains and carbohydrate blends, along with their combinations have been examined in experimental colitis models and clinical trials, and the results indicated that it can be an attractive therapeutic strategy for the suppression of inflammation, remission induction, and relapse prevention in IBD with minimal side effects. Several mechanisms of action of probiotics (for <i>e.g., Lactobacillus</i> species, and <i>Bifidobacterium</i> species) have been reported such as suppression of pathogen growth by releasing certain antimicrobial mediators (lactic and hydrogen peroxide, acetic acid, and bacteriocins), immunomodulation and initiation of an immune response, enhancement of barrier activity, and suppression of human T-cell proliferation. Prebiotics such as lactulose, lactosucrose, oligofructose, and inulin have been found to induce the growth of certain types of host microflora, resulting in an enriched enteric function. These non-digestible food dietary components have been reported to exert anti-inflammatory effects by inhibiting the expression of tumor necrosis factor-α-related cytokines while augmenting interleukin-10 levels. Although pro-and prebiotics has established their efficacy in healthy subjects, a better understanding of the luminal ecosystem is required to determine which specific bacterial strain or combination of probiotics and prebiotics would prove to be the ideal treatment for IBD. Clinical trials, however, have given some conflicting results, requiring the necessity to cite the more profound clinical effect of these treatments on IBD remission and prevention. The purpose of this review article is to provide the most comprehensive and updated review on the utility of prebiotics, probiotics, and synbiotics in the management of active Crohn's disease and ulcerative colitis/pouchitis.

Abstract:Background & Aims: The aim of this study was to assess systematically the efficacy of proton pump inhibitors (PPIs) in the treatment of functional dyspepsia compared with placebo and to determine if any difference in the response exists between symptom subgroups of functional dyspepsia. Methods: A literature search was performed through September 2005 in PubMed, Medline, Embase, CINAHL, and Cochrane databases to include randomized, double-blind, placebo-controlled trials evaluating the efficacy of PPIs for the treatment of functional dyspepsia. Relative risk (RR) and relative risk reduction (RRR) and 95% confidence intervals (CI) were calculated under a random-effects model. Results: Seven studies with a total of 3725 patients were identified. PPIs were found to be more effective than placebo for reducing symptoms in patients with functional dyspepsia (RRR, 10.3%; 95% C1, 2.7%-17.3%). The estimated number needed to treat is 14.6 (95% CI, 8.7-57.1). When stratified analyses were performed, a significant difference in the efficacy was observed only in patients with ulcer-like (RRR, 12.8%; 95% CI, 7.2%-18.1%) and reflux-like dyspepsia (RRR, 19.7%; 95% CI, 1.8%-34.3%), but not in those with dysmotility-like (RRR, 5.1%; 95% CI, -10.9% to 18.7%) and unspecified dyspepsia (RRR, -8.0%; 95% CI, -23.7% to 5.6%). The effect of H pylori on the efficacy of PPIs remains unclear. Significant heterogeneity among studies was found for the overall analysis, dysmotility-like dyspepsia, H pylori-negative subgroup, and different dose subgroups. Conclusions: PPIs are more effective than placebo for the management of patients with ulcer-like and reflux-like functional dyspepsia.

Furthermore, the changes of gut microbiota in IBD's patients are also intriguing. Thus, this meta-analysis was to explore the clinical effects and gut microbiota changes of using probiotics, prebiotics and synbiotics in IBD.</p></span><span class="a-plus-plus abstract-section id-a-sec2"><h3 class="a-plus-plus">Methods</h3><p class="a-plus-plus">The search was performed in PubMed, Web of Science and the Cochrane library from inception to April 2020. Qualified randomized controlled trials were included. IBD's remission rate, disease activity index and recurrence rate were extracted and analyzed. Changes in the gut microbiota of patients with IBD are comprehensively described.</p></span><span class="a-plus-plus abstract-section id-a-sec3"><h3 class="a-plus-plus">Results</h3><p class="a-plus-plus">Thirty-eight articles were included. Probiotics, prebiotics and synbiotics can induce/maintain IBD's remission and reduce ulcerative colitis (UC) disease activity index (RR = 1.13, 95% CI 1.02, 1.26, <em class="a-plus-plus">P</em> < 0.05; SMD = 1.00, 95% CI 0.27, 1.73, <em class="a-plus-plus">P</em> < 0.05). In subgroup analyses of IBD remission rate and UC disease activity index, we obtained some statistically significant results in some subgroup (<em class="a-plus-plus">P</em> < 0.05). To some extent, probiotic supplements can increase the number of beneficial bacteria (especially <em class="a-plus-plus">Bifidobacteria</em>) in the intestinal tract of patients with IBD.</p></span><span class="a-plus-plus abstract-section id-a-sec4"><h3 class="a-plus-plus">Conclusions</h3><p class="a-plus-plus">Our results support the treatment of IBD (especially UC) with pro/pre/synbiotics, and synbiotics are more effective. Probiotic supplements that are based on <em class="a-plus-plus">Lactobacillus</em> and <em class="a-plus-plus">Bifidobacterium</em> or more than one strain are more likely to be beneficial for IBD remission. The dose of 10<sup class="a-plus-plus">10</sup>–10<sup class="a-plus-plus">12</sup> CFU/day may be a reference range for using probiotics to relieve IBD.</p></span>

Abstract:Proton pump inhibitors (PPIs) are among the most prescribed and widely used medications; however, the long-term effects of these medications are only beginning to be investigated. Since the introduction of omeprazole in 1989, PPIs have become the first-choice treatment for esophagitis, peptic ulcer disease, Zoster-Ellison syndrome, dyspepsia, and the prevention of ulcers with non-steroidal anti-inflammatory drugs. Recent studies have specifically examined the rise in celiac disease (CD) in this context. This review explores how PPIs may impact the development of CD and highlights the need for additional research into the environmental and genetic factors that influence the development and progression of the disease. A literature search was performed using the keywords celiac disease, proton pump inhibitors, human leukocyte antigen (HLA)-DQ2, HLA-DQ8. The pathogenesis of CD is multifactorial, and human leukocyte antigens are one factor that may contribute to its development. Additionally, pharmaceuticals, such as PPIs, that cause gut dysbiosis have been linked to the inflammatory response present in CD. Recent studies have suggested that the rise in CD could be attributed to changes in the gut microbiome, highlighting the significant role that gut microbiota is proposed to play in CD pathogenesis. Although PPI therapy is helpful in reducing acid production in gastroesophageal disorders, additional information is needed to determine whether PPIs are still an appropriate treatment option with the possibility of developing CD in the future, particularly in the context of HLA-DQ2 and HLA-DQ8 predispositions. This review emphasizes the importance of personalized medicine for individuals with gastroesophageal disorders that require long-term use of PPIs.

Abstract:Purpose of review: This review summarizes the recent literature on the potential side-effects of proton pump inhibitors (PPIs) and known interactions with the metabolism/absorption of other drugs. Recent findings: Data confirm that PPIs are a very well tolerated drug class. Their high safety, efficacy and wide distribution lead to overuse, inappropriate dosage or excessive duration of treatment. Despite the absorption of micronutrients or other plausible effects on the development of bacterial infections linked to PPI-induced hypochlorhydria, it is difficult to demonstrate an association between PPI and specific symptoms. A possible negative effect of PPIs on bone integrity appears weak, but hypomagnesemia is likely a PPI drug class effect. A higher risk of Clostridium difficile infection and other infectious diseases such as small intestinal bacterial overgrowth and spontaneous bacterial peritonitis remain controversial in PPI users. However, the careful use of PPIs in cirrhotic or otherwise fragile patients is mandatory. Short-term or long-term PPI use may trigger microscopic colitis, and the management of this condition may include PPI withdrawal. The effect of PPIs on stimulating exocrine or endocrine gastric cell proliferation is poorly understood. A diagnostic delay or masking of diseases such as gastrinoma is difficult to evaluate. Summary: Short-term standard dose PPI treatment is low risk. Long-term PPI use may complicate health conditions by various mechanisms linked to PPIs and/or to hypochlorhydria.

Abstract:Inflammatory bowel disease (IBD) is a chronic and recurrent nonspecific intestinal disorder, clinically characterized by symptoms such as abdominal pain, diarrhea, and rectal bleeding, accompanied by infiltration of large numbers of neutrophils and macrophages, leading to dysbiosis of intestinal microbiota and sustained intestinal mucosal inflammation. [ 1 , 2 ] Ulcerative colitis and Crohn disease are the 2 main forms of IBD, which often accompanied with intestinal mucosal injured, inflammatory hyperplasia, and persistent ulcers. [ 3 ] Currently, the primary clinical treatment for IBD still relies on antibiotics, steroids, immunosuppressants, etc, to alleviate intestinal immune stress in IBD patients, prevent the occurrence of related complications, and achieve satisfactory therapeutic effects in many IBD patients. [ 4 , 5 ] However, the efficacy of these drugs varies among individuals, and their short half-life and inevitable side effects from intravenous or subcutaneous administration pose challenges. Moreover, the pathogenesis of IBD remains unclear, and its recurrent nature and inability to be completely cured, which greatly distress patients mentally and physically, earning IBD the moniker "the cancer that never dies."

Abstract:Inflammatory bowel disease (IBD) is characterized by chronic and relapsing inflammation of gastrointestinal tract, with steadily increased incidence and prevalence worldwide. Although the precise pathogenesis remains unclear, gut microbiota, bile acids (BAs), and aberrant immune response play essential roles in the development of IBD. Lately, gut dysbiosis including certain decreased beneficial bacteria and increased pathogens and aberrant BAs metabolism have been reported in IBD. The bacteria inhabited in human gut have critical functions in BA biotransformation. Patients with active IBD have elevated primary and conjugated BAs and decreased secondary BAs, accompanied by the impaired transformation activities (mainly deconjugation and 7α-dehydroxylation) of gut microbiota. Probiotics have exhibited certain positive effects by different mechanisms in the therapy of IBD. This review discussed the effectiveness of probiotics in certain clinical and animal model studies that might involve in gut microbiota-BAs axis. More importantly, the possible mechanisms of probiotics on regulating gut microbiota-BAs axis in IBD were elucidated, which we focused on the elevated gut bacteria containing bile salt hydrolase or BA-inducible enzymes at genus/species level that might participate in the BA biotransformation. Furthermore, beneficial effects exerted by activation of BA-activated receptors on intestinal immunity were also summarized, which might partially explain the protect effects and mechanisms of probiotics on IBD. Therefore, this review will provide new insights into a better understanding of probiotics in the therapy targeting gut microbiota-BAs axis of IBD.

Probiotics and Their Use in Inflammatory Bowel Disease

Abstract:Context • Crohn's disease and ulcerative colitis result in similar gastrointestinal (GI) symptoms, including pain, diarrhea, stools with mucus or blood, and ulceration or tissue damage within the alimentary canal. Gut microbiota play a crucial role in triggering, maintaining, and exacerbating inflammatory bowel disease (IBD). Probiotics might help to rebalance the gut flora in a positive way, shifting from pro- to anti-inflammatory. Objectives • The study intended to investigate the safety and use of probiotics and the biological effects of probiotic bacteria on IBD. Design • The research team performed a literature review. The team conducted a database search in April 2015 using Google Scholar and PubMed to find studies relevant to probiotics and their use in IBD. Only papers that were published in English were considered, and all available years in each database were searched. The initial search identified 38 published articles, for which the research team obtained full texts and independently read them in full to identify those papers suitable for inclusion in the review. Setting • The study took place in the main library of the University of Lahore (Islamabad, Pakistan). Results • Many strains of probiotics exist, but the most common strains available today are (1) the Bifidobacterium species, (2) Enterococcus faecium, (4) the Lactobacillus strains, (4) Saccharomyces boulardii, (5) the Bacillus species, and (6) Pediococcus, all used to produce beneficial health effects. These species showed their beneficial effects on the host using different mechanisms involving (1) production of proteins, quorum sensing signaling inhibitors, butyrate, immunoglobulin A, and short-chain fatty acids; (2) decreased production of tumor necrosis factor alpha and interleukin 8; (3) increased expression of mucin 2; and (4) increased upregulation of defensin. Conclusions • Studies on probiotics in animal models of IBD are promising, and clinical results in IBD patients are encouraging; however, the data are limited, and few studies are placebo controlled. Additional placebo-controlled, double-blind studies in IBD are required before recommendations can be offered for routine use of probiotics in IBD. Additional organisms may eventually be developed through genetic engineering. The current evidence also indicates that probiotic effects are strain specific; they do not act through the same mechanisms nor are all probiotics indicated for the same health conditions. More research is needed to determine what strains and at what dose probiotics become more useful as part of a clinical intervention.
