Abstract:Protamine sulfate reversibly inhibits serum-induced mitogenic stimulation of several nontransformed and neoplastic cell types in vitro. Fifty percent inhibition was induced by approximately 120-150 micrograms protamine sulfate/ml. Cells were affected directly, and inhibition depended on the duration of cell exposure. Heparin, chondroitin sulfate, heparan sulfate, and dextran sulfate neutralized protamine sulfate effects during the early stages of treatment. Nontransformed cells [bovine aortic endothelial cells, adult human gingival fibroblasts (strains 423 and 1101), fetal rat skin (strain 921-K) and muscle fibroblasts] required longer exposure to induce inhibition than did neoplastic cells [rat 3-methylcholanthrene-induced fibrosarcoma cell lines (MCA-6 and MCA-9), a macrophage-like cell line (NCTC-3749), Walker 256 rat carcinoma cells (ATCC-CCL-38), rat Morris hepatoma cells (ATCC-CCL-144), murine melanoma cells (B16), and rat bladder squamous cell carcinoma cells (804-G)]. Other polycationic compounds, including histone type VIII-S, poly-L-lysine, poly-L-arginine, and protamine (free base), were also effective inhibitors, whereas the basic proteins cytochrome c and lysozyme had no effect. Poly-L-histidine, poly-L-glutamic acid, poly-L-aspartic acid, and dextran blue also had no inhibitory effect.
Harm reduction social work with people who use drugs: a qualitative interview study with social workers in harm reduction services in Sweden

Abstract:Abstract Background Social work with people who use drugs (PWUD) has traditionally focused on abstinence and rehabilitation. In recent years, harm reduction has gained an increasingly more important role in social work with PWUD, and social workers are key professionals in many harm reduction services. This study investigates how social workers in harm reduction services for PWUD in Sweden understand the concept of harm reduction and how it relates to goals of rehabilitation, and how they assess and deal with dilemmas and challenges in everyday work. Methods The study is based on interviews with 22 social workers in harm reduction services for PWUD in the Scania region of Sweden. A thematic analysis in three steps was used in coding and processing the data. Results The social workers pointed to similar values between social work and harm reduction and argued for combining the two fields to improve services for PWUD. Three overarching principles for Harm Reduction Social Work (HRSW) were developed based on the social workers accounts: (1) Harm reduction is a prerequisite for rather than a counterpoint to rehabilitation and recovery, (2) motivational work must be non-mandatory and based on the client’s goals, (3) a holistic perspective is crucial for Harm Reduction Social Work. Challenges in doing HRSW concerned restrictive laws, policies, and guidelines, resistance from managers, difficulties in setting boundaries between client autonomy and life-saving interventions, and the risk of normalizing high-risk behaviors. Conclusions We use the concept of Harm Reduction Social Work to show how social work with PWUD can have a primary focus on reducing harm and risks, while at the same time it involves a holistic perspective that facilitates motivation and change. The suggested principles of HRSW can provide guidance in practical social work with vulnerable PWUD. Social workers can have important roles in most harm reduction settings and may act to enable recovery.

"It should be hard to be a drug abuser" An evaluation of the criminalization of drug use in Sweden

Abstract:Background: Drug use was criminalized in Sweden in 1988 with aim of reducing the number of consumers and drug-related risks and harms. Imprisonment was introduced into the penalty scale in 1993 to improve the legislation's effectiveness. The criminalization has never been evaluated. Method: Goal-attainment evaluation based on a range of indicators from surveys, case-finding estimates, healthcare and cause-of-death data, and crime statistics. Comparative drug policy analysis is conducted with other Nordic countries. Results: The criminalization is not followed by a reduced or more expensive drug supply, reduced consumption levels, problematic drug use or healthcare needs, or to fewer drug-related deaths. Most of the indicators instead show the opposite. Control costs are high, and trends are no better than those of other Nordic countries, despite Sweden's more repressive drug policy. Conclusion: Criminalization emerges as an ineffective, expensive and harmful means of dealing with the drugs problem.
Publics in local media reporting on harm reduction: Rightfully worried local witnesses or uneducated obstacles to change

Abstract:Background: In 2018, the planned opening of a second Needle and Syringe Exchange Program (NSP) unit in Stockholm, Sweden, was stopped with reference to protests from the public. Local Stockholm media cited stakeholders who claimed that the initiative was led by politicians with "zero knowledge about what makes citizens upset" and referred to reported public concern over a preschool located near the planned NSP unit. This case highlights the significant role of the public - and the idea of public opinion - in relation to political and medial aspects of alcohol and other drug (AOD) issues. Our aim is to scrutinize how "the public" is produced in local print media reports on harm reduction measures such as the NSP, to illuminate how these representations operate and what reality/ies they co-produce. Methods: We analyzed 171 articles reporting on harm reduction in local Stockholm print media from 2012 to 2023. The themes identified in the analysis emerged from a combination of data-driven empirical observations and a theory-driven approach grounded in the influential literature on publics and counterpublics by Michael Warner and Nancy Fraser. Results: The overarching articulation in the material is that of a singular and homogeneous public. Public opinion regarding local experiences of individual drug use and harm reduction is depicted as being driven by fear and worry over living alongside "messy others", thereby producing a public of worried local community witnesses. This production of the public takes on two different meanings depending on the narrative of the articles: 1) as righteous and entitled, 2) as ignorant and irrational. As a result, the public comes to operate as either a consulted public deserving consideration in the implementation of harm reduction policies or as an uneducated political obstacle to change. Consequently, the public is assigned both a counterpublic and a dominant public identity. Conclusions: When the representation of the worried public is repeatedly echoed by the media, it becomes hard to ignore in policy-making processes. The implications of such media representations are significant, as they risk disguising the complex nature of publics as a diverse group of individuals while reproducing taken-for-granted ideas about local communities opposing harm reduction measures. In addition, the appropriation of a counterpublic identity narrows the discursive space for action. Taken together, the repetition of a singular worried public and the appropriation of the counterpublic position make it nearly impossible to imagine alternative public responses to harm reduction. As a consequence, this can limit well-needed policy responses to AOD issues.
Drug dealing and drug use prevention - a qualitative interview study of authorities' perspectives on two open drug scenes in Stockholm

Abstract:Background: The use of illicit substances causes various health and social problems globally. Moreover, open drug use and dealing in urban areas, i.e., open drug scenes, can cause public order problems, lead to the recruitment of marginalized and young people for drug use or sale, and induce feelings of insecurity in the general public. Although some international studies have described various ways to manage open drug scenes, such as law enforcement and social interventions, there is limited knowledge about the facilitators and barriers promoting or impeding the implementation of such interventions. The aim of the current study was to explore how different authorities describe the nature of two open drug scenes in Stockholm and to derive authorities' perspective on facilitators and barriers to implementing interventions to reduce open drug dealing, drug use, and related problems at these locations. Methods: Semi-structured interviews with police officers, security staff, social workers, and municipality officials (n = 21) in the municipality of Stockholm were conducted and analyzed by qualitative content analysis. Results: The analysis of the interviews generated the following categories: Problems, Interventions, Organizational factors, and External factors, revealing information about the strategic and daily counteracting work occurring at the open drug scenes as well as authorities' perceptions of facilitators and barriers to implementing interventions to reduce open drug dealing, drug use, and related problems. Facilitators included motivated and skilled professionals and organized collaboration between key actors. Prominent barriers were a lack of resources to maintain personnel continuity at the scenes, policies that impede information sharing and put security staff in danger, and people who use or sell drugs without having residence permits. Conclusions: To increase the possibility of successful implementation of interventions to counteract open drug dealing, politicians and authorities should pay attention to collaboration between key actors, sufficient resource allocation, possible modification of policy governing professional duties, and remedies to the vulnerability of individuals without residence permits.

Abstract:Introduction Public spaces where alcohol and other drugs are openly used and marginal citizens gather, exist in many Nordic cities. The biggest open drug scenes in the Nordic countries are in cities like Oslo and Copenhagen; however, there are smaller scenes in other cities and suburbs, centered around shed-like structures, offering some form of shelter and a designated space for marginalized people involved in heavy drug and alcohol use who hang out in public space. In this paper, we investigate, in a comparative perspective, the characteristics and functions of smaller open alcohol and drug scenes, and how their existence is negotiated in the local community and among the citizens using them. Methods and material This article is a comparative case-study based on data from fieldwork (participating observation and interviews) carried out in two specific, yet somewhat similar, locations in Denmark and Norway between 2017 – 2022. A cross-case analysis was performed to identify commonalities and differences. Results Smaller open alcohol and drug scenes are non-regulated spaces of ambivalence and ongoing negotiation in local communities. Based on the data across locations, they represent possibilities for informal care and community for citizens in marginalized situations. The scenes are further, across location, characterized by a mutual agreement of performing decency, e.g., not allowing minor drug sale/use. Discussion To enable public spaces as smaller alcohol and drug scenes can play a role in reducing harm for marginalized citizens. Communication and dialogue between citizens using an open drug scene and the wider community may help reduce stigma.

"I am not a junkie": Social categorization and differentiation among people who use drugs

Abstract:Background: Substance use stigma is a form of group-based exclusion, and delineating pathways from stigma to poor health requires a deeper understanding of the social dynamics of people who use drugs (PWUD). Outside of recovery, scant research has examined the role of social identity in addiction. Framed by Social Identity Theory/Self-Categorization Theory, this qualitative study investigated strategies of within-group categorization and differentiation among PWUD and the roles these social categories may play in shaping intragroup attitudes, perceptions, and behaviors. Methods: Data come from the Rural Opioid Initiative, a multi-site study of the overdose epidemic in rural United States. We conducted in-depth interviews with people who reported using opioids or injecting any drug (n=355) living in 65 counties across 10 states. Interviews focused on participants' biographical histories, past and current drug use, risk behaviors, and experiences with healthcare providers and law enforcement. Social categories and dimensions along which categories were evaluated were inductively identified using reflexive thematic analysis. Results: We identified seven social categories that were commonly appraised by participants along eight evaluative dimensions. Categories included drug of choice, route of administration, method of attainment, gender, age, genesis of use, and recovery approach. Categories were evaluated by participants based on ascribed characteristics of morality, destructiveness, aversiveness, control, functionality, victimhood, recklessness, and determination. Participants performed nuanced identity work during interviews, including reifying social categories, defining 'addict' prototypicality, reflexively comparing self to other, and disidentifying from the PWUD supra-category. Conclusion: We identify several facets of identity, both behavioral and demographic, along which people who use drugs perceive salient social boundaries. Beyond an addiction-recovery binary, identity is shaped by multiple aspects of the social self in substance use. Patterns of categorization and differentiation revealed negative intragroup attitudes, including stigma, that may hinder solidary-building and collective action in this marginalized group.
Living a frozen life: a qualitative study on asylum seekers' experiences and care practices at accommodation centers in Sweden

Abstract:Background: Forced migrants fleeing conflict and violence face a high risk of mental health problems due to experiences before displacement, perilous journeys, and conditions in the new host societies. Asylum seekers seem to be in particularly vulnerable situations, indicated by higher prevalence rates of mental health problems compared to resettled refugees. Asylum seekers' mental health is highly influenced by the conditions they face in host countries while awaiting a decision on their case. In Sweden, 40% of asylum seekers reside in state-provided accommodation centers during the asylum process. Collective accommodation centers for asylum seekers have been said to impose restrictive social conditions and to be associated with poorer mental health outcomes than other housing forms (e.g., self-organized housing). However, there seems to be a scarcity of qualitative studies exploring the experiences of asylum seekers in different contexts. The aim of this study was therefore to explore the experiences of asylum seekers and how they manage their mental wellbeing while living at accommodation centers in Sweden. Methods: Fourteen semi-structured interviews with asylum seekers were conducted at two accommodation centers in Sweden. Participants were recruited using purposeful sampling and represented a diverse group of asylum seekers regarding age, background, and gender. The data was analyzed using content analysis. Results: Three overarching categories were identified; 1) Frozen life, 2) Constant worrying and "overthinking", and 3) Distractions and peer support. Participants experienced a state of being that could be characterized as a frozen life, which was associated with intense feelings of psychological distress, mostly described as manifesting itself in consuming patterns of ruminative thoughts, for instance overthinking and constant worrying. However, despite high levels of distress, participants demonstrated agency in managing negative mental health outcomes through self-care practices, peer support, and the development of care practices in caring for others in need. Conclusion: This study offers new insights into the everyday challenges that asylum seekers at accommodation centers face. Furthermore, it offers valuable observations of how asylum seekers at accommodation centers cope through self-care practices, peer support, and care practices in caring for peers in need. In order to enable sustainable and empowering support, mental health and psychosocial support services must identify and address both challenges and strengths, be grounded in the lived reality of asylum seekers, and build on existing resources. Moreover, further policy work needs to be done to enable faster asylum processes.

Abstract:BACKGROUND: The emergence of the drug user as a political problem in Sweden during the 1960s presented politicians with the problem of how to fit this new character into the existing democratic order. The aim of this article is to examine how Swedish politics sought to regulate democratic participation by establishing norms that conditioned who is recognized as a political subject as well as what counts as political speech and action.METHODS: The analysis is based on a close reading of parliamentary debates, political motions, and public reports and covers the period 1966-1979.RESULTS: During the examined period, Swedish politics constituted the ideal subject of democratic politics, homo politicus, as a subject embedded in a community of active and politically conscious citizens endowed with the capacity to cooperate and engage in the collective formulation of the common good. Drug use therefore posed a threat to the democratic order due to its passivizing effects that inhibited the cooperation needed to uphold the democratic polity.CONCLUSION: The perceived individualism, passivity, and inability of the drug user to engage in cooperation within a politically conscious community of citizens positioned the drug user as a threat to the democratic order. The drug user thereby became a useful figure in the political regulation of the democratic sphere and the constitution of homo politicus, the ideal subject of democratic politics.

Abstract:Background and aims: For people trying to stop using alcohol or other drugs (AOD), the process is often characterised by periods of abstinence followed by relapse into their previous drug-related way of life and subsequent re-entry into the treatment system. There is a call for greater attention to the how of these transitions, with a special focus on the phase of leaving treatment. The aim of this article was to get a better understanding of the transformation of practice when moving from a drug-using to a non-using lifestyle by exploring the experience of (1) the involvement in treatment settings, (2) the process of leaving treatment, and finally, (3) the early phase of changing everyday practice into a drug-free way of living. Method: The article takes on a social practice approach, in particular Bourdieu’s concepts of habitus, “doxa” and field to analyse 17 in-depth interviews with Norwegian men and women seeking treatment for problems resulting from the use of drugs and/or alcohol. Results: The study shows that the support of professionals operates as transitional relations that can bridge the transformation from a drug-using to a drug-free life, by providing a social web of relations, positions, settings and activities. However, leaving treatment and establishing AOD-free practice involves moving into something unknown in the sense that there is no embodied, taken-for-granted knowledge about how to relate to a world where drug use is not the focal point of existence. Conclusions: The process of change, then, involves being exposed to an existential feeling of maladjustment. To get beyond the feeling of maladjustment, and get into the doxic mode takes time, and involves reconfiguration of habitus through reorientation of social settings, relations and networks. The treatment system could potentially develop a continuum of support during these transformations.

Abstract:Background: Health and human rights organizations have endorsed drug decriminalization to promote public health-oriented approaches to substance use. In the US, policymakers have begun to pursue this via prosecutorial discretion-or the decision by a prosecutor to decline criminal charges for drug possession in their jurisdiction. This study characterizes drivers of adoption, policy design and implementation processes, and barriers to impact and sustainability of this approach to inform evolving policy efforts promoting the health of people who use drugs (PWUD). Methods: We conducted n=22 key informant interviews with policymakers and national policy experts representing 13 jurisdictions implementing de facto drug policy reforms. Analyses were informed by the Exploration, Preparation, Implementation and Sustainment (EPIS) framework and analyzed using a hybrid inductive-deductive approach. Results: Drivers of policy adoption included racial inequities, perceived failures of criminalization, and desires to prioritize violent crime given resource constraints. Three distinct policy typologies are described with varying conditions for eligibility, linkage to services, and policy transparency and dissemination. Public misinformation, police resistance and political opposition were seen as threats to sustainability. Conclusions: Given evidence that criminalization amplifies drug-related harms, many policymakers are adopting de facto drug policy reforms in the absence of formal legislation. This is the first study to systematically describe relevant implementation processes and emerging policy models. Findings have implications for designing rigorous evaluations on health outcomes and informing sustainable evidence-based policies to promote health and racial equity of PWUD in the US.

Abstract:Introduction: The treatment gap for addictive disorders is one of the largest in health care. Several studies have investigated barriers to treatment for different addictive disorders, but very few studies conducted have explored whether the barriers differ depending on substance or behavior or if they are common among all addictive disorders. In Sweden, addiction care is provided both by the healthcare and social services, where the latter is common, but also less popular. To our knowledge, there are no studies exploring whether the barriers are different depending on where the treatment is given. Aim: The aim was to thoroughly explore both which general and social services-specific barriers to treatment that are common, which barriers that differs, and how the barriers are described among individuals with a problematic use of alcohol, cannabis and/or gambling. Method: A mixed method convergent parallel design was conducted. For the quantitative measures, surveys including the validated Barriers to Treatment Inventory as well as questions regarding barriers in the Swedish multi-provider landscape, were collected from individuals with a problematic use of alcohol (n = 207), cannabis (n = 51), and gambling (n = 37). In parallel, 17 semi-structured interviews from the same population were conducted and analyzed with thematic analysis. Thereafter, the quantitative and qualitative data was compared, contrasted, and at last, interpreted. Results: The quantitative data showed that the largest general barriers in all groups were privacy concern and poor availability, and the largest barriers for seeking help from the social services was stigma, unawareness of what is offered, and fear of consequences for all groups. The qualitative data resulted in five general barriers: stigma, ambivalence, accessibility, fear of consequences, and lack of knowledge about addiction and its' treatments, and three barriers specifically towards social services: social services reputation, fear of meeting acquaintances, and lack of knowledge. The themes were developed from data from all groups, but different aspects of the themes were mentioned by different groups. Conclusion: There are details and aspects that differentiates both the general and social service-specific barriers to treatment between individuals with a problematic use of alcohol, cannabis, and gambling, but in large they perceive similar barriers.
"Returning to Ordinary Citizenship": A Qualitative Study of Chinese PWUD's Self-Management Strategies and Disengagement Model of Identity

Abstract:How PWUD (people who use drugs) live under drug governance is an important research question. This study adopts a qualitative research method to explore how PWUD in China self-manage after perceiving the dilemma of incomplete citizenship and the social pressure brought by drug control arrangements. Through analysis of 130 PWUD's files and in-depth interviews with 10 interviewees (from the 24 preliminary interviews), this study found that PWUD developed action strategies of hidden mobility (spatial isolation), disconnection of past experiences (time isolation), instrumental actions, as well as narrative strategies of reframing themselves as ordinary citizens with attempts of reversing identity disadvantages. Further, PWUD's self-management strategies manifest as a disengagement model in which the actors (PWUD, not rehabilitation agencies) do not intend to develop integrative positive identities through dispersed, practiced behavioral strategies, but attempt to return to pre-addiction, non-socially exclusionary citizenship experiences. The disengagement model and its negative effect on PWUD's social integration help us reflect on the current implementation of rehabilitation projects and institutional settings of drug governance.
Reframing Dutch drug policies: a new era for harm reduction

Abstract:In this article the authors offer their perspective on the changes in the Dutch harm reduction field. From the 1970s to the 1990s, the Netherlands emerged as a leader in harm reduction services, driven by grassroots movements like the Medisch-sociale Dienst Heroïne Gebruikers (MDHG) (Medisch-sociale Dienst Heroïne Gebruikers (MDHG) translates to Medical-Social Service Heroin Users in English) in Amsterdam and Junkiebond in Rotterdam. These organisations advocated for health-centred policies, initiated needle exchange programmes, and created safe consumption spaces. Their efforts led to significant public health improvements and policy shifts towards harm reduction, reducing HIV and hepatitis rates among people who use drugs. By the 1980s, harm reduction became institutionalised within local health and social care systems, leading to notable declines in drug-related harm and crime. However, from the 2000s, a shift towards security and crime prevention emerged, influenced by socio-political changes. Increased criminal justice measures and budget cuts for harm reduction services strained the system, making it harder to address emerging drug trends and the complex needs of people who use drugs. Despite challenges, there is renewed momentum for reform, particularly at the local level, advocating for the responsible regulation of psychoactive substances. Amsterdam Mayor Femke Halsema's 2024 conference on drug regulation exemplifies this shift, calling for policies that address prohibition failures and centre harm reduction. International bodies like the UN High Commissioner for Human Rights support this approach, emphasising a health and rights-based framework. As the Netherlands navigates these evolving dynamics, there is a pressing need to reinvest in harm reduction infrastructure, ensuring it meets diverse community needs and reaffirms its foundational rights-affirming principles.

Abstract:Introduction: People who use drugs have a long history of mobilising to reduce harm within their communities, significantly influencing harm reduction efforts globally. Peers with lived experience contribute through needle exchange programs, harm reduction education, and community-based research. Despite facing stigma, their initiatives have generated multiple benefits for communities. Collaborations between peers and researchers are increasingly recognised, emphasising meaningful participation in decisions affecting their lives. This paper focuses on the role of peers in mitigating drug-related risks and harms through community care. Methods: Semi-structured interviews were conducted with 30 people who use drugs, exploring peer support and harm reduction. Data were analysed by the lead author and a coding framework was developed in which key theme-categories were organised. Theoretical framing from science and technology studies informed analysis, recognising the political dimensions of research. Results: Peers play a crucial role in bridging gaps in formal services, offering stigma-free, relational care spaces. These spaces, both physical and social, affirm dignity and solidarity, countering marginalisation. Participants highlight the importance of peer involvement in driving change, promoting safer use practices, and advocating for a holistic harm reduction approach that considers systemic factors. Conclusions: Our data highlight the vital role of peer connections and peer-led harm reduction practices in fostering safety, solidarity, and connection among communities of people who use drugs. Future research should continue to explore peer-led initiatives within evolving healthcare contexts, considering broader social dynamics and employing innovative conceptual frameworks to promote equitable peer-led harm reduction strategies.
Emission Reduction From Burner System By Varying Swirler Blade Angle

Abstract:Kertas kerja ini mempersembahkan pengaruh mengubah sudut bilah pemusar udara aliran jejarian yang kecil dalam mengurangkan emisi seperti oksida nitrogen (NO x ) dan karbon monoksida (CO). Dalam kajian ini, satu sistem pembakar berbahan api cecair menggunakan empat pemusar udara aliran jejarian yang berlainan sudut bilah diuji di dalam kebuk pembakar berdiameter 163 mm dan panjang 280 mm. Ujian dijalankan menggunakan kerosin sebagai bahan api. Pemusar udara aliran jejarian dengan bilah terlengkung berdiameter keluaran 40 mm dipasang pada satah masukan pembakar bagi menjana aliran berpusar. Bahan api dipancitkan pada plat belakang keluaran pemusar menggunakan pemancit bahan api pusat dengan dua nozel bahan api menghala keluar secara paksi. Sudut pemusar udara dan nisbah setara diubah–ubah. Ujian dilaksanakan menggunakan empat pemusar udara yang berlainan sudut, iaitu masing–masing 30°, 40°, 50° dan 60°. Pengurangan NO x melebihi 50 peratus dicapai untuk sudut bilah 60° berbanding pada sudut bilah 30°. Emisi CO juga dikurangkan sebanyak 72 peratus pada sudut bilah 60° berbanding pada sudut bilah 30°. Kata kunci: Aliran berpusar; pembakar industri; emisi NOx; emisi CO; bilah terlengkung This paper presents the effect of varying the blade angle of a small radial air swirler on reducing emissions such as oxides of nitrogen (NO x ) and carbon monoxide (CO). In this research a liquid fuel burner system with four different radial air swirler blade angles has been investigated using 163 mm inside diameter combustor of 280 mm length. Tests were conducted using kerosene as fuel. A radial flow air swirler with curved blades having 40 mm outlet diameter was inserted at the inlet plane of the combustor to produce swirling flow. Fuel was injected at the back plate of the swirler outlet using central fuel injector with two fuel nozzles pointing axially outwards. The swirler blade angles and equivalence ratios were varied. Tests were carried out using four different air swirlers having 30°, 40°, 50° and 60° blade angles, respectively. A NO x reduction of more than 50 percent was achieved for blade angle of 60° compared to the 30° blade angle. CO emissions were also reduced by 72 percent for 60° blade angle compared 30° blade angle. Key words: Swirling flow; industrial burner; NO x emissions; CO emissions; curved blade
Male drug abuse, criminality and subcultural affiliation in a career perspective

Abstract:Degree of connection to the criminal underworld was the basis for typologic research on 698 male drug abusers; interviews as well as official records were used. Four types were distinguished: The addicted criminals seemed to resemble the groups dominant in Sweden from the 1950s to the 1970s. An early crime debut and criminal offenses in youthful years characterized their deviant careers. Drugs and criminal activities coexisted with an often very high intake of alcohol and the most difficult childhood and adolescence conditions compared to the other types. The criminal addicts had fewer recorded acts of juvenile delinquency. Their drug abuse was severe and occurred later in life, as criminality did, but tended to accelerate very rapidly. Their subcultural affiliation was probably as strong as that of the addicted criminals. A large group called low-crime addicts had a weak subgroup affiliation. The "normal" abuse pattern, with cannabis as the first substance used and a gradual shift to more severe opioid and CNS stimulant abuse, was most true of this type. Probably the drug abuse played a role in the development of the criminal pattern. Emotionally unstable addicts with little or no criminality had the best education, job situation and social relations. Multiple drug abuse and abuse of legal drugs were common. Mental ill-health was characteristic for this group. The results show that drug abusers in Sweden cannot be seen as a homogeneous group of individuals, that they do not commit crimes only in order to finance their habit, and that the history of narcotics use in Sweden, with its strong connection to a criminal subculture, is highly relevant to this sample.

Abstract:The paper describes and analyzes the development of prevention and care of alcohol problems in Sweden, from the emergence of the temperance issue as a public concern at the turn of the 19th century to the economic and ideological changes that seem to challenge the “Swedish model” of dealing with alcohol problems 100 years later. By applying an analytical model that transcends the common “badness-illness” dualism and provides a bridge between a social-political and a clinically oriented perspective, an attempt is made to relate the historical succession of dominant approaches to alcohol problems to the changing roles of the welfare state. The paper concludes by discussing the future prospects in Sweden of the type of community approach to handling alcohol problems that was first launched at the beginning of the century in Ivan Bratt's pioneer work but still awaits a major breakthrough in practice.
