Abstract:Superior vena cava (SVC) syndrome, once a rarity, has seen an uptick in cases with diverse origins. While this disease process is clinically diagnosable, imaging modalities and tissue biopsies further refine interventions. The clinical presentation includes but is not limited to edema of the arms, neck, and head, facial plethora, cyanosis, and or distention of subcutaneous vessels. SVC syndrome can be attributed to extrinsic compression or thrombosis in many cases. If symptoms are not life-threatening, the overall morbidity is based on the underlying root cause. Few cases have been reported with associated death due to epistaxis. However, the obstruction itself can be initially asymptomatic and then slowly progress over months to years. This case report highlights a distinct instance of SVC syndrome with notable risk factors: implantable cardioverter defibrillator placement and prior cardiac trauma status post-intervention.

Abstract:The superior vena cava (SVC) is mainly responsible for the return of blood flow from the head, upper limbs, and neck into the right atrium. The large vein can be subject to extrinsic tumor compression and invasive intraluminal tumors-metastatic and mediastinal tumors that can lead to complete or partial occlusion. SVC occlusion can also result from chronic inflammation or scarring of the vessel iatrogenically by pacemaker wires or venous access ports used for chemotherapy, long-term antibiotics, or hemodialysis. Patients with SVC occlusion present with a constellation of clinical abnormalities that make up SVC syndrome. SVC syndrome includes varying degrees of facial fullness, neck and upper extremity swelling, dyspnea, and classically dilated collateral veins in the upper chest. Very rarely do patients present with syncope, hoarseness, dysphagia, or acute encephalopathy. The diagnosis of SVC syndrome is best established on imaging such as CT Chest with contrast; however, on rare occasions, it can be discovered by endobronchial ultrasound. We present an unusual presentation of SVC syndrome- primarily presenting as frequent syncopal episodes- diagnosed via endobronchial ultrasound.
Superior Vena Cava Syndrome Masquerading as Angioedema: A Chemotherapy Port Complication

Abstract:Superior vena cava (SVC) syndrome is a constellation of symptoms that occur secondary to external compression of the SVC, most commonly by a mediastinal malignancy. With the increased use of implanted cardiac devices and indwelling central venous catheters, SVC syndrome from a benign cause has become quite common. This report follows a 62-year-old female who was initially admitted to the critical care unit for treatment of angioedema without a history of malignancy but was found to have a surgically placed port used to treat her rheumatoid arthritis. Despite treatment of what was presumed to be angioedema, her symptoms failed to resolve. Imaging of the thorax revealed a venous thrombosis in the previously placed port. The port was subsequently removed, and the patient's symptoms hastily resolved. This case report underscores the importance of obtaining a thorough history, maintaining a broad differential diagnosis, and revising the differential when the patient's symptoms fail to improve.
[An unusual presentation of superior vena cava syndrome]

Abstract:Superior vena cava syndrome (SVCS) is a clinical picture resulting from the obstruction of superior vena cava. SVCS usually causes symptoms and signs related to the head, the neck, the upper extremities and the upper thorax. We report a case of unusual clinical presentation of SVCS in a 25-year-old male patient who suffered from lymphoblastic lymphoma in childhood. Clinicians should be aware of abdominal wall presentations of SVCS especially in patients having undergone central venous catheterization, even years before. SVCS could be an emergency situation, requiring specific treatment or contraindicating surgical procedures.

Abstract:Background: Superior vena cava syndrome (SVCS) is a malignancy-related emergency. It is caused by obstruction of blood flow in the superior vena cava (SVC) secondary to intraluminal thrombosis, external compression, or direct invasion of tumor. Case summary: A 49-year-old male presented to the emergency department (ED) with acute hypoxemic respiratory failure. He was intubated and treated as pneumonia. Post-intubation, he became hypotensive, requiring fluid resuscitation and inotropic support. Resuscitative transesophageal echocardiography (TEE) showed external compression by a lung mass and an intraluminal thrombus causing SVC obstruction. Computed tomography (CT) angiography was performed, and it confirmed the TEE findings. A provisional diagnosis of lung carcinoma was made, and he underwent endovascular therapy for rapid symptomatic relief. Discussion: This case report highlights the role of resuscitative TEE in evaluating a hypotensive patient with clinical suspicion of SVCS at the emergency department. TEE performed at the bedside could help to diagnose and demonstrate the pathology causing SVCS in this case. TEE allowed high-quality image acquisition and was able to overcome the limitation of transthoracic echocardiography (TTE). TEE should be considered as an alternative ED imaging modality in the management of SVCS.

Abstract:Inferior vena cava (IVC) syndrome is a cluster of symptoms that occur due to obstruction or stenosis of the inferior vena cava. Symptoms include lower extremity swelling and erythema, shortness of breath, and abdominal discomfort. This case describes a patient who presented for evaluation of lower extremity swelling and weakness associated with nausea and vomiting. A plan was made to investigate a cardiopulmonary etiology; however, the initial bedside ultrasound that included the evaluation of the abdomen discovered a mass in the IVC and left kidney. Additional imaging and intervention that involved a posterior radical nephrectomy and IVC thrombectomy allowed confirmation of left renal cell carcinoma (RCC) extended into the IVC with a thrombus causing the constellation of symptoms. A detailed history is always important to configure the IVC syndrome, but a quick and easy bedside ultrasound can identify IVC and other abdominal pathologies. A multidisciplinary approach is highly recommended for optimal results.

Abstract:Superior vena cava (SVC) syndrome is an uncommon yet potentially fatal syndrome occurring after intrinsic or extrinsic compression to the SVC. While there are multiple emerging etiologies for this phenomenon, malignancy remains the most common. It is characterized by several symptoms including facial swelling, extremity swelling, shortness of breath, and headaches. We present the case of a 59-year-old female with a past medical history of cocaine abuse who was admitted for upper extremity swelling and facial edema. Imaging revealed a right suprahilar mass compressing a branch of the right pulmonary artery and SVC, in addition to bilateral segmental and subsegmental pulmonary emboli. She underwent an emergent biopsy and SVC stenting, with immunostaining revealing small cell lung cancer (SCLC). This case highlights a severe presentation of SVC syndrome caused by previously undetected SCLC.

Abstract:Malignant superior vena cava syndrome (SVCS) is a clinical problem that results from the obstruction of blood flow in the superior vena cava by an underlying malignancy. This may occur due to external compression, neoplastic invasion of the vessel wall, or internal obstruction with bland or tumor thrombus. Although symptoms are typically mild, SVCS can cause neurologic, hemodynamic, and respiratory compromise. Classic management options include supportive measures, chemotherapy, radiation therapy, surgery, and endovascular stenting. New targeted therapeutics and techniques have also recently been developed, which may have a role in management. Nevertheless, few evidence-based guidelines exist to guide treatment of malignant SVCS, and these recommendations are typically restricted to individual disease sites. Furthermore, there are no recent systematic literature reviews that address this question. Here, we present a theoretical case to frame this clinical problem and synthesize updated evidence published in the past decade relating to the management of malignant SVCS through a comprehensive literature review.

Abstract:Superior vena cava (SVC) syndrome constellates symptoms of plethora and facial and upper thoracic swelling with distension of neck veins, and the consequences of this condition range from mild discomfort to upper airway obstruction. The aetiology has a strong linkage with malignancy, and is always to be excluded. SVC syndrome is commonly a bilateral presentation, and unilateral SVC syndrome is rare, with very few non-malignant causes reported. Here, we demonstrate a unilateral occurrence with an uncommon cause.
Two cases of vertebral perfusion disturbances in computer tomography imitating metastatic lesions in the course of superior vena cava thrombosis

Abstract:Skeletal metastases are frequently observed in various malignancies. In some cases, they are asymptomatic and can be found incidentally in various imaging methods in patients without known malignant tumors. In this case study 2 cases of vertebral perfusion disturbances are presented that imitate vertebral metastatic lesions in computer tomography in the course of superior vena cava thrombosis. The first patient was referred to our clinic for chest and abdominal computer tomography (CT) for staging due to a known tumor in the anterior mediastinum. The second patient was referred for chest CT due to swelling in the upper extremity and neck, with the suspected diagnosis of a tumor or pulmonary embolism. In both cases, CT scans showed metastases suspected lesions in the upper thoracic vertebral bodies. In both cases, additionally, the thrombosis of superior vena cava (SVC) and vena brachiocephalica was confirmed (in the first case due to tumor compression in the upper mediastinum, in the second case due to the presence of pacemaker leads). In control CT scans after anticoagulation treatment, there were no suspected lesions in the vertebral bodies, which confirmed the diagnosis of vertebral perfusion disturbances in the course of SVC thrombosis in both patients. In conclusion, in rare cases of metastases suspected lesions of thoracic vertebral bodies in contrast-enhanced computer tomography among patients with a diagnosis of superior vena cava thrombosis vertebral perfusion disturbances should be included in differential diagnosis protocol.

Abstract:Superior vena cava syndrome (SVCS) is a clinical condition characterized by signs and symptoms resulting from the blockage or narrowing of the thin-walled superior vena cava (SVC). This obstruction can lead to significant morbidity and mortality. In this case, we report a 58-year-old patient who was diagnosed with SVCS due to a massive compressing anterior mediastinal mass leading to signs and symptoms of SVCS, including shortness of breath, dizziness, palpitations, and neck swelling, which was managed surgically by excision of the mass and reconstruction of the brachiocephalic vein using a synthetic graft.

Abstract:Scimitar syndrome is a rare, complex congenital anomaly characterized by an anomalous connection of the pulmonary vein with the inferior vena cava (IVC). A persistent left superior vena cava (PLSVC) is another thoracic venous anomaly. We present a rare case with scimitar syndrome associated with the PLSVC. These variations were confirmed by magnetic resonance imaging (MRI) and computed tomography (CT). The typical features and the embryonic development of these variations are described. Radiological evaluations and clinical implications are discussed.

Abstract:Purpose: To study CT appearances of superior vena cava syndrome (SVCS) from lung cancer. Materials and Methods:Fifty-one cases with lung cancer resulting in SVCS confirmed by pathological examination were analyzed retrospectively and statistically with emphasis on CT features of lung cancer, superior vena cava obstruction and secondary lesions of SVCS. Results: (1) Lung cancer resulting in SVCS in right upper pulmonary lobe, right middle pulmonary lobe, right low pulmonary lobe, left upper pulmonary lobe, left low pulmonary lobe and right complete pulmonary lobes was 32 cases, 3 cases, 5 cases, 2 cases, 5 cases and 4 cases respectively. Central type, right upper mediastinal type, peripheral type was 35 cases, 12 cases and 4 cases respectively; (2) CT appearances of lesions around obstructive segment of SVC involved in 11 cases of ringed encasement,5 cases of partial ringed encasement and 5 cases of sandwich-like encasement,10 cases of procession,15 cases of compression toward left and 5 cases of complete obstruction.At different degree of SVC obstruction,probability of the former five of them on CT was of difference (P<0.05);(3) On CT appearances of secondary lesions,cases of simple constitution and opening of collateral pathway,simple swelling of chest wall,both of the two and neither of the two were 14 cases,12 cases,16 cases and 9 cases respectively on CT. With SVC obstruction at different extent,constitution and opening of collateral pathway and/or swelling of chest wall were different in shown ratio on CT (P<0.05). Conclusion: SVCS resulted from lung cancer can be diagnosed by contrast enhanced CT.
Multidetector Computed Tomography Angiography Role in Diagnosing Superior Mesenteric Arteriovenous Fistula with Superior Mesenteric Vein Thrombosis: A Case Report

Abstract:Introduction: Arteriovenous fistulas (AVF) of the superior mesenteric vasculature are rare vascular abnormalities. It often occurs due to iatrogenic injuries during bowel resection or abdominal traumas. As the patient exhibits a variety of signs and symptoms and can present late, diagnosis may be difficult.Multidetector computed tomography (MDCT) is the most common modality for imaging the small bowel, vessels, and mesentery in patients with mesenteric ischemia. Case presentation: We present the case of a 25-year-old man who presented with features of acute intestinal obstruction and was operated on in an emergency. Resection of gangrenous small bowel and bowel adhesiolysis with double barrel ileostomy was performed. During exploratory laparotomy, gangrenous bowels were found, but no cause of mesenteric ischemia could be delineated. Finally, on computed tomography angiography (CTA) superior mesenteric arteriovenous fistula (SMAVF) with superior mesenteric vein (SMV) thrombosis was diagnosed. The patient was advised of aggressive anticoagulant therapy. Conclusion: Computed tomography angiography has become the most reliable diagnostic modality to confirm the diagnosis and guide the best treatment strategy. Abstract:The Journal of Vascular Access, Ahead of Print. Central vein stenosis (CVS) is a common and challenging complication in hemodialysis patients with chronic central venous catheters (CVCs). CVS often remains asymptomatic and is discovered incidentally during follow-up imaging. CVS symptoms include arm swelling, venous hypertension, impaired dialysis flow rates, and development of collateral veins. However, these symptoms can be nonspecific and overlap with other conditions, making the diagnosis challenging. Timely recognition and appropriate intervention are crucial to prevent complications and optimize patient outcomes. Diagnostic tools commonly used include duplex ultrasonography and venography to assess the degree and location of stenosis. Management strategies for CVS encompass a multidisciplinary approach involving nephrologists, interventional radiologists, and vascular surgeons. Initial conservative measures may include anticoagulation therapy, along with pharmacological interventions such as antiplatelet agents and thrombolytics. The endovascular approach is the first line for managing CVS by using balloon angioplasty either alone or in combination with stent placement, but CVS typically recurs frequently, requiring repeated interventions with an increased risk of complications. Additionally, alternative vascular access options such as arteriovenous fistulas or grafts may be considered. In this report, we describe a case of a 25-year-old woman who presented with an extensive history of multiple dialysis access failure for left internal jugular vein central venous tunneled catheter exchange. The procedure was complicated by a fatal superior vena cava rupture likely related to the dislodgment of the guidewire causing perforation into the pericardium space with subsequent cardiopulmonary collapse. The post-mortem autopsy showed severe organized stenosis of SVC and transmural defect above the SVC/atrial junction.

Unveiling absent inferor vena cava in young patients: Case reports and clinical insights

Abstract:Introduction: Inferior vena cava agenesis (IVCA), a rare congenital anomaly, contributes to approximately 5 % of deep venous thrombosis (DVT) cases lacking other risk factors. It can lead to chronic venous insufficiency and DVT when collateral circulation is insufficient, presenting diagnostic challenges due to its rarity. Case presentation: We present two cases of Absent IVC (AIVC) in young males. Case 1: a 22-year-old developed bilateral lower limb DVT post-appendectomy. Imaging revealed AIVC with azygos continuation. Treatment included Heparin and Rivaroxaban, achieving symptom resolution. Case 2: a 41-year-old with recurrent DVT and chronic venous insufficiency was diagnosed with AIVC via venography. Managed with warfarin and compression therapy, his symptoms stabilized. Clinical discussion: These cases underscore the importance of recognizing AIVC in young patients presenting with unexplained DVT. Diagnosis often requires advanced imaging techniques like CT venography. Management typically involves long-term anticoagulation and compression therapy to mitigate the risk of recurrence and chronic venous complications. Conclusion: Early identification of AIVC in young adults presenting with recurrent DVT is essential for appropriate management and prevention of long-term complications.
