Abstract:The shoulder joint has a complex anatomy and biomechanics. It is a ball and socket joint made by the articulation surface of the humeral head (ball) and glenoidal fossa (socket) of the scapula. Shoulder arthroplasty is done when parts of the shoulder joint are severely affected and damaged beyond repair. The damaged parts are replaced with artificial parts. Prosthetic implants are typically made of metal or plastic material. Implants come in various sizes and shapes. There are three types of surgical arthroplasty: total shoulder arthroplasty, partial shoulder arthroplasty, and reverse arthroplasty. Indications of shoulder arthroplasty may include osteoarthritis, fractures, rotator cuff injuries, osteonecrosis, and rheumatoid arthritis. This case study aims to provide a case of anteroinferior dislocation of the left shoulder with humeral head comminuted fracture confirmed by an investigation like radiograph and CT scan operatively managed by left shoulder hemiarthroplasty. In this case study, a 58-year-old male cannot lift his arm and perform actions of the shoulder joint independently after the operative procedure, thus reducing the functional status and quality of life. After the left shoulder hemiarthroplasty repair post due to inadequate rehabilitation, there was a failure in achieving the ranges and gaining back the strength of the muscles. The patient has a combined plan of action, which consists of pharmacological interventions along with physiotherapy rehabilitation. The physiotherapy protocol consists of goals like using electrical muscle stimulation, activation exercises of muscles, strengthening protocol, stretches, and counselling. By the end of the physiotherapy treatment, the patient showed significant progress in re-establishing the ranges and enhanced muscle strength, which resulted in a positive self-boost along with improved functional independence quotient, thereby increasing quality of life.

Abstract:Multiple fractures are frequently encountered in adults following road traffic accidents. A 32-year-old male presented with multiple fractures in his right lower extremity, including a femoral shaft fracture, distal third fractures of the tibia and fibula, as well as a calcaneal fracture. The patient provided a history indicative of a road traffic accident. X-rays were performed on both hip joints, both knee joints, and the ankle joints. Treatment involved open reduction and internal fixation (ORIF) with interlocking nailing for the femur, tibia, and fibula, alongside ORIF with plating using a screw-out set (SOS) and cannulated cancellous (CC) screw fixation for the calcaneal fracture. Additionally, the Ilizarov procedure was conducted following debridement over the right foot. Post-surgery, the patient experienced primary symptoms of hip joint pain and restricted hip joint movement. Physiotherapy was initiated to address these issues. Evaluation of outcome measures indicated a reduction in joint pain, significant enhancement in joint mobility, and an increase in muscle strength.

Abstract:Femoral shaft fracture occurs through the diaphyseal region of the femur. The intertrochanteric (IT) fracture is an extracapsular fracture that occurs through the proximal metaphyseal area of the femur. The ipsilateral femoral shaft and IT fractures happen in rare cases following high-energy trauma. These fractures are difficult to manage due to their complexity. They are usually managed surgically and require prompt physiotherapeutic management postoperatively. The postoperative complications involve pain, stiffness, reduced muscle strength, deep vein thrombosis, muscular weakness, and atrophy. These complications occur primarily due to immobilization. In our case, the 30-year-old male patient was a known case of schizophrenia for three years. He met with a road traffic accident (RTA) while driving a car and acquired ipsilateral femoral shaft and IT fractures. He presented with the chief complaints of pain, swelling, deformity over the right thigh, and unable to bear weight over the right lower limb. X-rays revealed a right-side ipsilateral IT fracture and a femoral shaft fracture. The patient underwent an operation that involved open reduction and internal fixation with a proximal femoral nail under spinal epidural anesthesia. We started physical therapy management on postoperative day three. On clinical evaluation, there was a decrease in the range of motion and muscle strength of the right lower limb. This case posed us with a challenge to deliver postoperative physiotherapeutic intervention without elicitation of the symptoms associated with schizophrenia. The physiotherapy protocol involved virtual reality-based (VR-based) interventions in adjunct to conventional therapeutic interventions like strengthening exercises, range of motion exercises, application of electrical modality, balance training, gait training, VR-based relaxation, and aerobic exercises. We used the Lower Extremity Functional Scale, Manual Muscle Testing, Range of Motion, Brief Psychiatric Rating Scale, and the Numerical Pain Rating Scale as outcome measures. The comparison of pre-and post-outcome measure scores demonstrates a significant improvement.

Abstract:The term "Holstein-Lewis fracture" describes a spiral fracture that occurs in the shaft of the humerus at its distal third, which has been linked to radial nerve palsy in adults, and operative treatment is the preferred method of treating the trapped nerve at the fracture site. This paper describes a clinical case involving a 20-year-old male patient demonstrating a humeral fracture syndrome accompanied by complications associated with radial nerve palsy. After the necessary investigation, he was diagnosed with a Holstein-Lewis fracture with radial nerve paralysis; he underwent open reduction internal fixation (ORIF), after which he was referred to physical therapy. Developing a successful postoperative rehabilitation program that consists mostly of functional physical therapy interventions is essential for the treatment of this condition. Outcome measures like the Numerical Pain Rating Scale (NPRS), Disabilities of the Arm, Shoulder, and Hand (DASH) score, and Patient-Rated Wrist Evaluation (PRWE) score were recorded before and after rehabilitation, and pain reduction, improvement in strength, range of motion (ROM), grip strength, and activities of daily living (ADL) were found. The purpose of this case report is to present a comprehensive treatment plan that includes ROM exercises, cryotherapy, and strengthening of grip using a robotic glove for a patient who had a wrist drop and underwent ORIF surgery. This tailored intervention was effective in speeding up the return of functional abilities and improving function in ADLs.

Abstract:Traumatic brachial plexus injury (BPI) is a debilitating condition predominantly affecting young males, often resulting from traction or direct injuries. Due to the complicated neuronal network, the damage is often classified as preganglionic or postganglionic injuries. It includes upper-limb mobility impairments as well as reduced muscular strength and sensitivity. We discuss a case of a 30-year-old female who suffered a displaced mid-shaft clavicular fracture and BPI after a road traffic collision. The patient experienced significant pain, restricted movement, and sensory and motor loss in her left arm. Imaging studies revealed additional complications, including epidural collection and pseudomeningoceles. She underwent open reduction and internal fixation of the clavicle, followed by a structured rehabilitation program focusing on pain management, muscle re-education, and functional recovery. This case highlights the complexity of managing clavicular fractures with concurrent BPI, requiring a multidisciplinary approach involving imaging, surgical intervention, and comprehensive physiotherapy for optimal recovery and functional restoration. Rehabilitation strategies were employed to address the diverse symptoms, including multi-sensory strategies, sensory re-education, graded motor imagery rehabilitation, and gradual restoration of upper extremity (UE) range, strength, and endurance and to develop neuromuscular control. Effective management of clavicular fractures with BPI requires early diagnosis, surgical intervention, and structured rehabilitation to improve functional outcomes and quality of life.

Abstract:INTRODUCTION: Brachial plexus injuries are very much devastating and lead to significant socioeconomic and mental burden to the affected person and his family. In the early days of 20th century many authors were in favour of conservative management even the amputation of affected limb. But in later decades due to development in technological advancement of operating microscope, intraoperative electrophysiological monitoring, biological tissue glue etc. the results of surgical repair are beyond the imagination that could be done previously. METHODS: Our study is a Retrospective observational analytical study in which we included patients of Pan brachial plexus injury operated under the Department of Neurosurgery from April 2011 till February 2023 (excluding covid period from March 2020 to September 2021) at apex trauma center AIIMS Delhi, India. Demographic details, mode of injury, surgery performed and outcome were analyzed. Data was extracted from patient’s casualty, ICU and OT records and analyzed using SPSS version 29.0.0.0. Analysis of outcome was taken in terms of improvement in motor power and quality of life. RESULTS: There were 113 patients diagnosed to have Pan brachial plexus injury by clinical examination and supported with brachial plexus MRI, NCV and EMG of the affected limb. 24 (21.24%) cases underwent Neurolysis only, 17 (15.04%) cases underwent Supraclavicular neurotization only, 24 (21.24%) cases were operated by infraclavicular neurotization and 48 (42.48%) patients were operated by supraclavicular and infraclavicular neurotization both. Respectively 25%, 23.52%, 25% and 35.42% cases had motor and sensory improvement and were able to perform few of their daily activities with lesser support. CONCLUSIONS: In our study at least 25% Patients of Pan brachial plexus injury after surgery got benefited in their routine activity and quality of life thus lesser affected in socioeconomic and psychological front.

Abstract:This report presents an individualized physical therapy program and the therapeutic results for a 36-year-old man with a posttraumatic left scapular body fracture with minimal displacement and without ribs fractures scapular fracture. Because the glenoid fossa was intact, conservative treatment was recommended. The patient reports arm activities dysfunctions. The rehabilitation was conducted in an outpatient physical therapy clinic from Craiova under direct supervision for 12 weeks. At the end of the follow-up period, the level of pain was considerably decreased. Despite the functional deficit from the beginning of the rehabilitation program, normal function was restored. The patient was fully involved and cooperative during the physical therapy session.

Abstract:A 35-year old healthy male trauma surgery chief resident, suffered a high-speed motor vehicle collision. The patient sustained the following injuries: a Gustilo-Anderson grade 2 open comminuted intra-articular fracture of the left distal femur (AO 33C3.3), a Hawkins 1A neck fracture of the right talus (AO 81.2A), an undisplaced Lisfranc injury of the right foot comprising avulsion fractures at the base of the 1st, 2nd and 5th metatarsal as well as the cuboid bone suggesting ligament injury and 2nd to 5th carpometacarpal dislocations of the right (non-dominant) hand with comminuted fractures of the capitate, hamate, trapezoid and the base of the fifth metacarpal bone. A staged-treatment approach ensued. An external fixator (ex-fix) was placed over the left knee, followed by definitive fixation of the distal femoral fracture using a Qwix screw, Non-Contact Bridging (NCB) plate and Locking Compression Plate (LCP). An ex-fix was placed over the right wrist, followed by open reduction and k-wire fixation. The talar fracture of the right foot was treated with a single lag screw and the Lisfranc injury was treated non-operatively with four weeks of non-weight bearing cast immobilization. An intensive clinical rehabilitation program was started, including early use of Continuous Passive Motion (CPM), daily non-weightbearing swimming pool exercises, hand, physical and recreational therapy. One year after the injury the patient was rehabilitated and resumed his surgical residency. Two years after the injury, limited flexion and pain in the left leg remains, possibly related to partial union of the femoral fracture. Range of motion (ROM) of the right ankle and wrist remains limited, not causing significant functional impairment. Lessons learned from a patient experience combined with detailed descriptions of injuries, rehabilitation and long term outcomes can be used as a reference for treating patients with comparable injuries.

Abstract:In adults, partial rotator cuff injuries can frequently be the root cause of pain in the shoulder. One recurrent pathology that may significantly impact a broad spectrum of individuals, including athletes, laborers, and sedentary people, is partial rotator cuff tears (RCTs). Physical therapy, anti-inflammatories, painkillers, rest or activity modifications, and corticosteroid injections are a few nonoperative treatment options for partial RCTs. We report a case of a 27-year-old male who sustained a rotator cuff injury of the right shoulder. The patient presented with pain and restriction of the right shoulder joint following the injury, which had occurred while throwing a ball forcefully with his right hand. The rehabilitation program emphasized pain management, restoring range of motion (ROM), increasing strength of muscles, and functional activities to optimize the patient's recovery. Concurrently, isometrics, ROM, and strengthening exercises were integrated into the rehabilitation program to enhance muscle strength, prevent tightness, and maintain ROM. The patient's progress was monitored by using the Disabilities of Arm, Shoulder, and Hand (DASH) score and the Upper Extremity Functional Scale (UEFS) at specific intervals during rehabilitation. The treatment and healing of a patient with an RCT who underwent both pre and postoperative physiotherapy are explored in this case report.

Abstract:Proximal tibia fractures are generally open fractures resulting in life-threatening complications. There is an increase in the prevalence of compartment syndrome post-tibial fractures. Though fasciotomy and wound debridement is the choice of treatment in such conditions, amputation becomes a necessity when there is widespread muscle and tissue damage. Specifically, compartment syndrome involving the lower limb is distressing as its symptoms can be misleading at times resulting in delayed diagnosis and hence life-threatening complications. A 48-year-old male met with a road accident and presented to the hospital with complaints of pain and an open wound over the anterior aspect of the right lower leg 14 days ago. He was managed by Ilizarov external fixator. Later he developed compartment syndrome as a complication of tibial plateau fracture, for which decompressive fasciotomy was performed. But due to irreversible tissue loss and gangrene, he had to undergo above-knee amputation followed by physiotherapy rehabilitation. We mainly focused on postoperative/pre-prosthetic rehabilitation. During postoperative rehabilitation, we concentrated on reducing phantom limb pain, preventing complications, and improving strength and endurance. Current literature claims that mirror therapy is effective in reducing phantom limb pain in post amputees, but there are only a few case reports emphasizing mirror therapy in particularly lower limb amputees. Therefore, we emphasized using mirror therapy for phantom limb pain in this case of lower limb amputation. It resulted in positive outcomes. Our broader aim was to strengthen the upper limbs and the intact lower limb so that the patient's overall functional independence can be enhanced. Further prosthetic rehabilitation was planned in which we focused on gait and balance training. Physiotherapy rehabilitation improved the patient's quality of life and independence.

Abstract:Summary X‐linked anaemia (sla) has been studied in the foetus of the mutant mice of Falconer & Isaacson (1962). The affected male is anaemic by the 19th–20th day of gestation. At this time there is no indication of a defect in the transport of iron to the foetus as indicated by the transfer of 59Fe and by the content of elementary iron in the total carcass. This suggests that a mechanism other than, or in addition to, intestinal malabsorption is operative in the production of sla.

Abstract:Distal radius and ulnar fractures usually have a significant role in affecting the functions of hand. They are the most common fractures of the forearm caused by either direct trauma (hit by bat) or indirect trauma (fall). Important clinical features are pain, swelling and decreased forearm and hand functions. These fractures are diagnosed by X-rays and usually managed either conservatively if the fragments are not displaced or by open reduction if the fragments are displaced. The present case report is of a female of age 55 years met with an accident 3 weeks prior. She was operated on the next day with open reduction and internal fixation and physiotherapy started immediately the next day. Patient was given early physiotherapy training to fingers and shoulder joint of left side to prevent disuse atrophy and to improve circulation. Along with this the patient was also encouraged to perform gripping activities of affected hand, Thumb movements an also generalised active movements for rest of the extremities both upper and lower limbs. Patient was even taught about the breathing exercises and breathing control training to promote active participation. Patient was receiving physiotherapy for two weeks. At the end of two weeks patient was not having any swelling or oedema in the extremity and movements were preserved in other joints of affected extremity and unaffected upper limb and lower limbs. Patient was able to do her activities of daily living by herself and has good balance while sitting, standing and walking. This case report signifies the importance early physiotherapy for compound fractures of forearm.

Abstract:Tibial fractures occur following low-energy and high-energy trauma resulting from indirect and direct impacts, respectively. High-velocity trauma like road traffic accidents usually results in open fractures of the tibia associated with acute compartment syndrome, posing a serious threat. Thus, this injury requires prompt operative management with fasciotomy followed by fixating the fracture with an external fixator to avoid infection. Despite all the efforts, sometimes this condition may lead to osteomyelitis of the tibia requiring further care. Meanwhile, the patient has to be kept immobilized for a longer duration of time, which allows further complications to occur. Therefore, structured physiotherapeutic management of patients postoperatively is of immense necessity to prevent complications associated with prolonged immobility and achieve an optimal level of functional independence, thereby facilitating the patient to come back to near-normal life as soon as possible. Our case report provides a structured early-stage postoperative physical therapy treatment protocol for a patient with a proximal tibial fracture with acute compartment syndrome and neurovascular deficit managed with Ilizarov fixation.

Abstract:A tensile force, normally caused by a high-energy force, including an automobile or industrial accident, crushes and elevates the pressure within the hand abruptly. To achieve good results in its recovery, mutilated arm and its treatment require a thorough knowledge of rehabilitative methods and a thorough knowledge of hand anatomy. Early wound excision, aggressive hand rehabilitation for perioperative care, low reconstructive surgery threshold and rehabilitative care is recommended Patient’s main concerned was stiffness in wrist and elbow joint, reduced range of motion with difficulty in doing daily activities. She was diagnosed with crush injury over right forearm back while working on a machine. Main clinical findings were reduced range of motion over elbow and wrist joint. Joint stiffness over elbow joint with reduced muscle strength. Diagnosis of this case was crush injury over right forearm which resulted into scar adhesion and joint stiffness. For six weeks, the patient received physiotherapy treatment. Session included range of motion exercises, strengthening exercises, patient education, scar mobilization, joint mobilization, gripping exercises and home program. Alexios Pitsillides and Dimitrios Stasinopoulos suggested that every third day, tendons and ligaments are treated to maintain a balance between collagen synthesis and degradation and also because type I collagen's response to high load in a normal tendon peaks around 3 days after intense loading in a normal tendon. Treatment plan made proved beneficial to the patient we were able to increase the range of motion and patient was able to perform daily activities. Abstract:Rotator cuff (RC) muscles give the shoulder joint stability in addition to movement. The case report outlines the physical rehabilitation therapy, condition evaluation, and diagnostic testing that was given to a 62-year-old female field worker who had been complaining of pain in her left shoulder. To improve functional mobility and lessen discomfort, the patient underwent physiotherapy. In this instance, a physical therapy program was put in place to treat rotator cuff syndrome (RCS), enhance range of motion (ROM), and promote long-term recovery. Part of the assessment included a detailed examination of the biomechanics and potential mitigating variables for the persistent problems. The intervention plan's multimodal approach comprised physical therapy, stretching, and strengthening exercises, as well as patient counseling and health management education. Throughout the physiotherapy sessions, the patient's functional mobility improved and their level of discomfort gradually decreased. This case adds to the body of knowledge regarding successful physiotherapy techniques for RC injuries by emphasizing the value of a comprehensive approach to help patients with chronic shoulder pain achieve favorable outcomes. It also highlights how crucial it is to treat chronic RCS with a customized physical therapy program that takes into account the patient's unique preferences and characteristics that can exacerbate the problem.

Abstract:The posterior cruciate ligament (PCL), one of the key ligaments in the knee, serves to prevent backward movement of the tibia relative to the femur. A simultaneous occurrence of a PCL avulsion fracture and a femur shaft fracture in a pediatric patient suggests a complex orthopedic injury resulting from significant trauma to the knee and thigh area. This study describes the rehabilitation process of a 12-year-old female involved in a road traffic accident, who suffered both a midshaft femur fracture and a PCL avulsion fracture. Following surgical procedures, the patient underwent a comprehensive physiotherapy regimen utilizing X-Sens inertial sensor technology. The rehabilitation plan comprised multiple stages targeting pain alleviation, muscle strengthening, flexibility exercises, gait retraining, and balance improvement. Various interventions including contrast baths, cryotherapy, patellar mobilization, isotonic and resistance exercises, and progressive gait training were integrated across different phases of the rehabilitation program. Over subsequent follow-up periods, the patient demonstrated significant enhancements in pain management, range of motion, muscle strength, functional capabilities, and gait metrics. This case report underscores the efficacy of a systematic physiotherapy strategy incorporating advanced technology in the successful recovery from intricate lower limb fractures, underscoring the importance of prompt intervention and multidisciplinary collaboration for optimal patient outcomes.
THE EFFECT OF TAPING ON SHOULDER KINEMATICS AND POSTURE IN PATIENTS WITH SHOULDER IMPINGEMENT SYNDROME

Abstract:Taping is commonly used in the conservative treatment of shoulder impingement syndrome. However, the underlying mechanism of taping and its effect on kinematics are not yet understood and there is little evidence to support its use. There are two main taping techniques used in physical rehabilitation, Rigid and Elastic taping. In this study, a motion capture system was used to track reflective markers attached to the thorax, humerus, the scapula locator and to two clusters attached to the thoracic and lumbar spine. An observer used the scapula locator to track the movement of the scapula whilst maintaining low pressures on the three scapular landmarks. Bilateral elevation and lowering in the scapular and sagittal planes were measured before taping and after the application of the Rigid and Elastic taping techniques, Visual Analogue Scale was used as a measure of pain intensity during motion. The results showed that both taping techniques had similar effects on the shoulder kinematics although the magnitude of these changes varied between techniques and across subjects. Rigid taping in particular increased the glenohumeral rotation and reduced upward rotation leading to an increase in the scapoulohumeral rhythm. Pain intensity was more reduced by Elastic tape application in both planes but this did not seem to be correlated to the changes in kinematics.

Utilization of axillary brachial plexus block in the postoperative rehabilitation of intra-articular fractures of the distal humerus

Abstract:Objectives: An effective rehabilitation program is essential to prevent joint stiffness and regain range of motion after surgical treatment of intra-articular fractures of the distal humerus. We evaluated the effect of a physiotherapy program on functional results, that involved passive resistive stretching exercises performed under axillary brachial plexus block after radiographic observation of bone union of intra-articular fractures of the distal humerus treated with open reduction and internal fixation. Methods: The study included 21 patients (7 females, 14 males; mean age 34+/-5 years; range 21 to 57 years) who underwent open reduction and internal fixation for intra-articular fractures of the distal humerus. All the patients had closed fractures. Six patients had AO type C1, six patients had C2, and nine patients had C3 fractures. Surgical treatment consisted of a posterior incision, olecranon osteotomy, and fixation of the metaphyseal fragments using two reconstruction plates placed medially and laterally. Active range of motion exercises were started on the third postoperative day. To prevent early development of heterotopic ossification, passive range of motion exercises were avoided. Active stretching exercises were initiated three weeks after surgery. Upon radiographic observation of bone union, axillary brachial plexus block was performed. The physiotherapy program involved passive stretching exercises during nerve block, and active weight exercises after recovery from motor block. The catheter remained in the axillary region for three months, during which functional rehabilitation was continued 2-3 times a week on an outpatient basis. Functional results were evaluated according to the criteria of Jupiter et al. after a mean follow-up period of 31 months (range 24 to 46 months). Results: All fractures united within a mean of 11.9 weeks (range 9 to 17 weeks) except for one type C3 fracture. Functional results were excellent in 10 patients (47.6%), good in eight patients (38.1%), moderate in two patients (9.5%), and poor in one patient (4.8%). Two patients with a moderate outcome had associated multiple fractures in the ipsilateral extremity. Distribution of the functional results according to the type of fractures were 4 excellent, 2 good in type C1; 4 excellent, 2 good in C2; and 2 excellent, 4 good, 2 moderate, and 1 poor in C3 fractures. The mean loss of elbow extension was 16 degrees. The mean elbow flexion, pronation, and supination were measured as 131 degrees, 90 degrees, and 75 degrees, respectively. None of the patients had nonunion at the olecranon osteotomy site, superficial or deep infection, or heterotopic ossification. Three patients developed transient ulnar nerve neuropraxia that resolved spontaneously during the follow-up period. There were no complications related to axillary catheterization. Conclusion: Following surgical treatment of intra-articular fractures of the distal humerus, a regular and pain-free physiotherapy program performed under axillary brachial plexus block on an outpatient basis increases patient compliance and enables early return to daily activities.
