Abstract:In this paper, attribute reduction in variable precision rough set model is discussed. Several kinds of reducts preserving some of lower approximations, upper approximations, boundary regions and the unpredictable region are discussed. Relations among those kinds of reducts are investigated. As a basis for reduct computation, Boolean function representations of the preservation of lower approximations, upper approximations, boundary regions and the unpredictable region are discussed. Throughout this paper, the great difference between the analysis using variable precision rough sets and the classical rough set analysis is emphasized.
"It might be a statistic to me, but every death matters.": An assessment of facility-level maternal and perinatal death surveillance and response systems in four sub-Saharan African countries

Abstract:Background: Maternal and perinatal death surveillance and response (MPDSR) systems aim to understand and address key contributors to maternal and perinatal deaths to prevent future deaths. From 2016-2017, the US Agency for International Development's Maternal and Child Survival Program conducted an assessment of MPDSR implementation in Nigeria, Rwanda, Tanzania, and Zimbabwe. Methods: A cross-sectional, mixed-methods research design was used to assess MPDSR implementation. The study included a desk review, policy mapping, semistructured interviews with 41 subnational stakeholders, observations, and interviews with key informants at 55 purposefully selected facilities. Using a standardised tool with progress markers defined for six stages of implementation, each facility was assigned a score from 0-30. Quantitative and qualitative data were analysed from the 47 facilities with a score above 10 ('evidence of MPDSR practice'). Results: The mean calculated MPDSR implementation progress score across 47 facilities was 18.98 out of 30 (range: 11.75-27.38). The team observed variation across the national MPDSR guidelines and tools, and inconsistent implementation of MPDSR at subnational and facility levels. Nearly all facilities had a designated MPDSR coordinator, but varied in their availability and use of standardised forms and the frequency of mortality audit meetings. Few facilities (9%) had mechanisms in place to promote a no-blame environment. Some facilities (44%) could demonstrate evidence that a change occurred due to MPDSR. Factors enabling implementation included clear support from leadership, commitment from staff, and regular occurrence of meetings. Barriers included lack of health worker capacity, limited staff time, and limited staff motivation. Conclusion: This study was the first to apply a standardised scoring methodology to assess subnational- and facility-level MPDSR implementation progress. Structures and processes for implementing MPDSR existed in all four countries. Many implementation gaps were identified that can inform priorities and future research for strengthening MPDSR in low-capacity settings.

Abstract:Background: Ethiopia is investing in the routine Health Management Information System. Improved routine data are needed for decision-making in the health sector. Objective: To analyse the quality of the routine Health Management Information System data and triangulate with other sources, such as the Demographic and Health Surveys. Methods: We analysed national Health Management Information System data on 19 indicators of maternal health, neonatal survival, immunization, child nutrition, malaria, and tuberculosis over the 2012–2018 time period. The analyses were conducted by 38 analysts from the Ministry of Health, Ethiopia, and two government agencies who participated in the Operational Research and Coaching for Analysts (ORCA) project between June 2018 and June 2020. Using a World Health Organization Data Quality Review toolkit, we assessed indicator definitions, completeness, internal consistency over time and between related indicators, and external consistency compared with other data sources. Results: Several services reported coverage of above 100%. For many indicators, denominators were based on poor-quality population data estimates. Data on individual vaccinations had relatively good internal consistency. In contrast, there was low external consistency for data on fully vaccinated children, with the routine Health Management Information System showing 89% coverage but the Demographic and Health Survey estimate at 39%. Maternal health indicators displayed increasing coverage over time. Indicators on child nutrition, malaria, and tuberculosis were less consistent. Data on neonatal mortality were incomplete and operationalised as mortality on day 0–6. Our comparisons with survey and population projections indicated that one in eight early neonatal deaths were reported in the routine Health Management Information System. Data quality varied between regions. Conclusions: The quality of routine data gathered in the health system needs further attention. We suggest regular triangulation with data from other sources. We recommend addressing the denominator issues, reducing the complexity of indicators, and aligning indicators to international definitions.

Abstract:Pregnancy-related death is a cause for maternal and newborn mortality and morbidity as well as an obstacle for economic growth. Three-quarters of mothers' lives can be saved if women have access to a skilled health worker at delivery and emergency obstetric care. This evaluation was conducted to assess skilled delivery service implementation level by using three dimensions (availability, compliance, and acceptability) and identify major contributing issues for underutilization of the service. The evaluation design is cross-sectional. The study included 846 mothers who gave birth in Hadiya zone within one year prior to study period, using one year delivery records. Epi Info 3.5.3 and SPSS version 16 were employed for data analysis. Based on selected indicators, resource availability was inadequate for health facilities, human resource medical equipment, and rooms. On the compliance dimension, skilled delivery service coverage (34.8%), active management of third stage labor (32.7%), and health information at discharge and in postnatal care (PNC) visit (7.1%) critically complied with or poorly agreed to the guidelines and targets. Regarding skilled delivery service acceptability, welcoming, privacy keeping, reassurance during labor pain, follow-up, baby care, comfortability (rooms, beds, and clothing), cost of service, and episiotomy (without local anesthesia) were not acceptable.

Abstract:Background: Despite reports of universal access to and modest utilization of maternal and newborn health services in Ethiopia, mothers and newborns continue to die from preventable causes. Studies indicate this could be due to poor quality of care provided in health systems. Evidences show that high quality health care prevents more than half of all maternal deaths. In Ethiopia, there is limited knowledge surrounding the status of the quality of maternal and newborn health care in health facilities. This study aims to assess the quality of maternal and neonatal health care provision at the health facility level in four regions in Ethiopia. Methodology: This study employed a facility-based cross-sectional study design. It included 32 health facilities which were part of the facilities for prototyping maternal and neonatal health quality improvement interventions. Data was collected using a structured questionnaire, key informant interviews and record reviews. Data was entered in Microsoft Excel and exported to STATA for analysis. Descriptive analysis results are presented in texts, tables and graphs. Quality of maternal and neonatal health care was measured by input, process and outputs components. The components were developed by computing scores using standards used to measure the three components of the quality of maternal and neonatal health care. Result: The study was done in a total of 32 health facilities: 5 hospitals and 27 health centers in four regions. The study revealed that the average value of the quality of the maternal and neonatal health care input component among health facilities was 62%, while the quality of the process component was 43%. The quality of the maternal and neonatal health output component was 48%. According to the standard cut-off point for MNH quality of care, only 5 (15.6%), 3 (9.3%) and 3 (10.7%) of health facilities met the expected input, process and output maternal and neonatal health care quality standards, respectively. Conclusion: This study revealed that the majority of health facilities did not meet the national MNH quality of care standards. Focus should be directed towards improving the input, process and output standards of the maternal and neonatal health care quality, with the strongest focus on process improvement.

Abstract:A recombinant fowlpox virus vaccine expressing key protective Mycoplasma gallisepticum antigens could facilitate in the prevention both of fowlpox virus and M. gallisepticum infections. Vectormune FP-MG vaccine, a recombinant fowlpox virus expressing both M. gallisepticum 40k and mgc genes, was assessed for its safety in 8-wk-old specific-pathogen-free White Leghorn chickens. The vaccine virus was serially passaged 5 times by wing-web inoculation. Based on the postinoculation clinical observation, gross pathological examination of air sacs and peritoneum, genetic stability evaluation, virus shedding and tissue distribution detection, horizontal transmission ability determination, and protection against fowlpox virus challenge, the Vectormune FP-MG vaccine possesses a high level of safety.
Exploring barriers to the use of formal maternal health services and priority areas for action in Sidama zone, southern Ethiopia

Abstract:Background: In 2015 the maternal mortality ratio for Ethiopia was 353 per 100,000 live births. Large numbers of women do not use maternal health services. This study aimed to identify factors influencing the use of maternal health services at the primary health care unit (PHCU) level in rural communities in Sidama zone, south Ethiopia in order to design quality improvement interventions. Methods: We conducted a qualitative study in six woredas in 2013: 14 focus group discussions (FGDs) and 44 in-depth interviews with purposefully selected community members (women, male, traditional birth attendants, local kebele administrators), health professionals and health extension workers (HEWs) at PHCUs. We digitally recorded, transcribed and thematically analysed the interviews and FGDs using Nvivo. The 'three delay model' informed the analytical process and discussion of barriers to the use of maternal health services. Results: Lack of knowledge on danger signs and benefits of maternal health services; cultural and traditional beliefs; trust in TBAs; lack of decision making power of women, previous negative experiences with health facilities; fear of going to an unfamiliar setting; lack of privacy and perceived costs of maternal health services were the main factors causing the first delay in deciding to seek care. Transport problems in inaccessible areas were the main contributing factor for the second delay on reaching care facilities. Lack of logistic supplies and equipment, insufficient knowledge and skills and unprofessional behaviour of health workers were key factors for the third delay in accessing quality care. Conclusions: Use of maternal health services at the PHCU level in Sidama zone is influenced by complex factors within the community and health system. PHCUs should continue to implement awareness creation activities to improve knowledge of the community on complications of pregnancy and benefits of maternal health services. The health system has to be responsive to community's cultural norms and practices. The mangers of the woreda health office and health centres should take into account the available budgets; work on ensuring the necessary logistics and supplies to be in place at PHCU.

Abstract:Accurate reporting of maternal mortality is essential for effective health policy planning and achieving maternal death reduction in Sustainable Development Goals Target 3 (SDGs). This study aimed to identify gaps between facility-based maternal death reviews and the hospital Health Management Information System (HMIS) reports in Mzimba South District, Malawi from July 2013 to June 2018. A retrospective hospital-based medical record review was conducted to identify maternal deaths among women aged 15 to 49 years with all death causes. Out of 447 mortality records identified from the hospital wards, 89 maternal mortality cases were identified by the study review compared to 83 cases in the HMIS report. The HMIS report showed an underreporting rate of 6.7% (6/89) and a misclassification rate of 13.5% (12/89) within five years. These findings highlight the need for establishing mechanisms for the verification and monitoring of maternal mortality data reporting in health facilities. Improving the quality of maternal death reporting could help inform evidence-based interventions and policies that address the root causes of maternal mortality, and achieve SDGs in Malawi.

Abstract:This study evaluated an early warning, alert and response system for a crisis-affected population in Doolo zone, Somali Region, Ethiopia, in 2019–2021, with a history of epidemics of outbreak-prone diseases. To adequately cover an area populated by a semi-nomadic pastoralist, or livestock herding, population with sparse access to healthcare facilities, the surveillance system included four components: health facility indicator-based surveillance, community indicator- and event-based surveillance, and alerts from other actors in the area. This evaluation described the usefulness, acceptability, completeness, timeliness, positive predictive value, and representativeness of these components.

Abstract:Introduction: Fifteen million babies are born prematurely, before 37 weeks gestational age, globally. More than 80% of these are in sub-Saharan Africa and Asia. 35% of all deaths in the first month of life are due to prematurity and the neonatal mortality rate is eight times higher in low-income and middle-income countries (LMICs) than in Europe. Early Warning Scores (EWS) are a way of recording vital signs using standardised charts to easily identify adverse clinical signs and escalate care appropriately. A range of EWS have been developed for neonates, though none in LMICs. This paper reports the findings of early work to examine if the use of EWS is feasible in LMICs. Methods: We conducted an observational study to understand current practices for monitoring of preterm infants at a large national referral hospital in Nairobi, Kenya. Using hospital records, data were collected over an 8-week period in 2019 on all live born infants born at <37 weeks and/or <2500 g (n=294, 255 mothers) in the first week of life. Using a chart adopted from the EWS developed by the British Association of Perinatal Medicine, we plotted infants' vital signs. In addition, we held group discussions with stakeholders in Kenya to examine opinions on use of EWS. Results: Recording of vital signs was variable; only 63% of infants had at least one temperature recorded and 53% had at least one heart rate and respiratory rate recorded. Stakeholders liked the traffic-light system and simplicity of the chart, though recognised challenges, such as staffing levels and ability to print in colour, to its adoption. Conclusion: EWS may standardise documentation and identify infants who are at higher risk of an adverse outcome. However, human and non-human resource issues would need to be explored further before development of an EWS for LMICs.

Abstract:This study aimed at assessing the predictors of knowledge about the Maternal and Perinatal Deaths Surveillance and Response (MPDSR) system among health workers in the Morogoro region. It was an analytical cross-sectional study, conducted from April 27 to May 29, 2020. A multistage sampling technique was used to recruit 360 health workers. A semi-structured questionnaire was used to collect the data. Statistical Package for Social Science (SPSS v.20) software was used for data entry and analysis. Bivariate and multivariate logistic regression analyses were used to assess factors associated with knowledge of MPDSR. A total of 105 (29.2%) health workers in the Morogoro region had adequate knowledge of the MPDSR system. After controlling for confounders, predictors of knowledge on the MPDSR system were the level of health facility a health worker was working (n [hospital [adjusted odds ratio [AOR] = 2.668 at 95% confidence intervals [CI] = 1.497–4.753, P = .001]), level of education of a health worker (diploma [AOR = 0.146 at 95% CI = 0.038–0.561, P = .005]), and status of training on MPDSR (trained [AOR = 7.253 at 95% CI = 3.862–13.621, P ≤ .001]). The proportion of health workers with adequate knowledge about the MPDSR system in the Morogoro region is unacceptably low. Factors associated with adequate knowledge were those working in hospitals with higher levels of professional training and those who had ever had training in MPDSR. A cost-effective strategy to improve the level of knowledge regarding MPDSR in this region is highly recommended.

Abstract:Long-term prognosis was analyzed by a retrospective study of 56 patients with malignant melanoma of the conjunctiva and the eyelids. Survival rate at 5 years was 85 p. cent, at 10 years 68 p. cent, and at 15 or 20 years 55 p. cent; neither the localization of the tumor nor the type of initial treatment had a significant influence on these rates. Local or nodal recurrences occurred in 80 p. cent of cases, and were frequent following conservative surgery; prognosis is bad after radiotherapy.
Assessment of the efficiency of the routine epidemiological surveillance system for malaria at the Tambacounda Health District, Senegal

Abstract:Introduction: as part of the fight against malaria, epidemiological surveillance (ES) is one of the key pillars of the global technical strategy 2016-2030 to combat this disease. However, in the south-east of Senegal, where malaria poses a major public health problem, epidemiological surveillance has until recently been very neglected. To help reduce malaria-related morbidity and mortality in Senegal, an evaluation of the routine malaria ES system was conducted in the Tambacounda Health District in 2021. Methods: we conducted a cross-sectional, descriptive survey of 27 health structures in Tambacounda district from 20th February to 1st March 2022. Results: overall, the routine ES system in the district was acceptable according to its users, with satisfactory tool filling time in 96.3% of the structures in our study, a cumulative completeness of reports at 92% despite a 77% promptness. The data collected at the services delivery points (SDP) level allowed a representativeness of the ES system in 100% of health facilities. The ES system was also rated as simple by 74.1% of SDP managers even though only 55.6% of them were trained. However, the stability of the system was low because 55.6% of SDP had staff to ensure the continuity of ES service despite the availability of management tools (100%) and the telephone network (96.3%). The same was true for the usefulness of the ES because only 25.9% of SDP managers analyzed their produced data. The reported ES and malaria morbidity data were not adequate. On the other hand, the ES system was reactive with a speed of transmission of information at 96.3% and a possibility of rapid diagnosis and management of cases at 100%. Conclusion: the routine malaria ES system at the level of health facilities in Tambacounda District was acceptable, simple, flexible, representative, and responsive. However, an increase in qualified staff at the health posts, capacity strengthening of the ES staff and regular supervision of SDP are needed essentials to make the district's malaria surveillance system more efficient.
Assessing Maternal Adherence to WHO's Essential Newborn Care Practices in Libo Kemekem District: A Community-Based Study

Abstract:Background. Around half of child deaths that occur before the age of 5 are related to newborn-related causes. The advice on caring for newborns may not always match with local traditions and beliefs. That's why it's important to understand the current newborn care practices and what influences them. This will help implement efforts to reduce newborn deaths. The goal of this research was to assess essential newborn care practices and the factors associated with them among women who have recently given birth. Methods. A community-based cross-sectional study was conducted in Libo Kemekem District from April 1 to May 1, 2021, among 601 women. Face-to-face interviews using a structured questionnaire were employed for data collection. The completeness of the questionnaires was checked, and errors were manually corrected. The data was then entered into EpiData v3.1 and transferred to SPSS v26.0 for analysis. Descriptive statistics were used to calculate frequencies, percentages, means, and standard deviations of the findings. Bivariate and multivariate analyses were performed to assess the association between variables for early newborn care practices (ENCP). In the bivariable models, independent variables that showed a statistically significant relationship at a P-value less than .2 with the outcome variable were considered candidates for the multivariable logistic regression models. In the multivariate regression, a P-value < .05 was considered statistically significant. Results were presented in text, tables, and graphs. Additionally, we checked for multicollinearity and performed the Hosmer and Lemeshow goodness-of-fit test to ensure the validity of the multivariate logistic regression analysis. Result. The study revealed that 54.2% (95% CI, 50%-58%) of mothers practiced comprehensive ENBC. In the multivariable binary logistic regression analysis, several factors were found to be significantly associated with ENBC. Primary education was associated with a reduced likelihood of practicing essential newborn care, with an adjusted odds ratio (AOR) of 0.34 (95% CI: 0.23-0.78). Similarly, secondary education was also associated with a reduced likelihood, with an AOR of 0.51 (95% CI: 0.34-0.89). On the other hand, mass media exposure was associated with an increased likelihood of practicing essential newborn care, with an AOR of 2.03 (95% CI: 1.35-3.07). The number of ANC visits was also a significant factor, with an AOR of 0.403 (95% CI: 0.237-0.686. Furthermore, knowledge about ENBC and newborn danger signs were both important predictors, with AORs of 3.93 (95% CI: 2.09-7.37) and 0.31 (95% CI: 0.12-0.74), respectively. Conclusion. This study reveals a critically low level of essential newborn care practices in the local community, influenced by education, mass media exposure, ANC visits, mothers' knowledge, and awareness of newborn danger signs. Addressing these factors through targeted interventions can reduce neonatal mortality rates, emphasizing the need for investments in education, health literacy programs, and healthcare services to enhance mothers' and newborns' health outcomes.
Understanding performance data: health management information system data accuracy in Southern Nations Nationalities and People’s Region, Ethiopia

Abstract:Health management information system (HMIS) is a system whereby health data are recorded, stored, retrieved and processed to improve decision-making. HMIS data quality should be monitored routinely as production of high quality statistics depends on assessment of data quality and actions taken to improve it. Thus, this study assessed accuracy of the routine HMIS data.Facility based cross-sectional study was conducted in Southern Nations Nationalities and People's region in 2017. Document review was done in 163 facilities of different levels. Statistical Package for the Social Sciences (SPSS) for windows version 20 was used to perform data analysis. Data accuracy was presented in terms of mean and standard deviation of data verification factor.Though inaccuracy was noted for all data elements, 96.9 and 84.7% of facilities reported institutional maternal death and skilled birth attendance within acceptable range respectively while confirmed malaria (45.4%), antenatal care fourth visit (46.6%), postnatal care (55.2%), fully immunized (55.8%), severe acute malnutrition (54.6%) and total malaria (50.3%) were reported accurately only by about half of facilities. Antenatal care fourth visit was over reported by 24% while total malaria was under reported by 28%. Reasons for variations included technical, behavioral and organizational factors.Majority of facilities over reported services while under reporting diseases. Data quality should be monitored routinely against data quality parameters quantitatively and/or qualitatively to catch-up country's information revolution agenda.

Abstract:BackgroundAmong Millennium Development Goals, achieving the fifth goal (MDG-5) of reducing maternal mortality poses the greatest challenge in Sub-Saharan Africa. Ethiopia has one of the highest maternal mortality ratios in the world with unacceptably low maternal health service utilization. The Government of Ethiopia introduced an innovative community-based intervention as a national strategy under the Health Sector Development Program. This new approach, known as the Health Extension Program, aims to improve access to and equity in essential health services through community based Health Extension Workers.ObjectiveThe objective of the study is to assess the role of Health Extension Workers in improving women’s utilization of antenatal care, delivery at health facility and postnatal care services.MethodsA cross sectional household survey was conducted in early 2012 in two districts of northern and south central parts of Ethiopia. Data were collected from 4949 women who had delivered in the two years preceding the survey. Logistic regression analysis was performed to determine the association between visit by Health Extension Workers during pregnancy and use of maternal health services, controlling for the effect of other confounding factors.ResultsThe non–adjusted analysis showed that antenatal care attendance at least four times during pregnancy was significantly associated with visit by Health Extension Workers [Odds Ratio 3.46(95% CI 3.07,3.91)], whereas health facility delivery (skilled attendance at birth) was not significantly associated with visit by Health Extension Workers during pregnancy [Odds Ratio 0.87(95% CI 0.25,2.96)]. When adjusted for other factors the association of HEWs visit during pregnancy was weaker for antenatal care attendance [Adjusted Odds Ratio: 1.35(95% CI: 1.05, 1.72)] but positively and significantly associated with health facility delivery [Adjusted Odds Ratio 1.96(1.25,3.06)].ConclusionIn general HEWs visit during pregnancy improved utilization of maternal health services. Health facility delivery is heavily affected by other factors. Meaningful improvement in skilled attendance at birth (health facility delivery) should include addressing other factors on top of visits by HEWs during pregnancy and specific target oriented interventions during visits by HEWs to support skilled attendance at birth.

Factors influencing integrated disease surveillance and response system in selected districts in the Eastern Region of Ghana

Abstract:Background: Ghana adopted the Integrated Disease Surveillance and Response (IDSR) system, which is an integration of the various programs in the surveillance system and can contain disease outbreaks and pandemics. Implementation of the IDSR is influenced by several factors which can affect its functionality and ability to contain disease outbreaks. This study assessed the factors influencing the IDSR system in selected districts in the Eastern Region of Ghana. Method: A cross-sectional study was conducted between February-March, 2022 in Fanteakwa North, Abuakwa South and New Juaben South districts in the Eastern Region of Ghana among health care workers who are involved in IDRS activities. Both primary and secondary data were collected and analyzed using descriptive statistics and regression analysis at 0.05 significant level with 95% confidence interval. Results: Three hundred and forty-seven (347) health care workers participated in the study with 56.2% (195/347) indicating that rumor registers were available at the health facilities. Most of the respondents (64.8%, 225/347) had means of transport for disease surveillance activities while majority (61.9%, 215/347) had case-based forms for case investigation. About half (51.9%, 180/347) of the participants revealed that they did not receive any feedback from the next higher level in the past year. Availability of transport for IDSR activities was almost 3.4 times more likely to contribute positively to IDSR system compared to facilities without transport (AOR = 3.36; 95% CI = 1.44-7.83; p = 0.005). Respondents who have the capacity to apply case definition are 2 times more likely to contribute to an effective IDSR system compared to health workers who cannot apply case definition (AOR = 1.94; 95% CI = 1.17-3.21; p = 0.013). Respondents who did not receive feedback from the next higher level were 52% less likely to have an effective IDSR system compared to respondents who received feedback from the next higher level (AOR = 0.48; 95% CI = 0.23-1.00; p = 0.05). Conclusion: Effective operation of IDSR is affected by the application of case definition and means of transport at health facilities. In addition, the capacity of health care workers to provide feedback can influence the smooth operation of the IDSR in the studied area in Ghana.
