Abstract:Ultrasound (US) is the most important imaging method for the assessment of structural disorders of the thyroid. A precise volume determination is relevant for therapy planning and outcome monitoring. However, the accuracy of 2D-US is limited, especially in cases of organ enlargements and deformations. Software-based "stitching" of separately acquired 3D-US data revealed precise volume determination in thyroid phantoms. The purpose of this study is to investigate the feasibility and accuracy of 3D-US stitching in patients with structural thyroid disease. A total of 31 patients from the clinical routine were involved, receiving conventional 2D-US (conUS), sensor-navigated 3D-US (3DsnUS), mechanically-swept 3D-US (3DmsUS), and I-124-PET/CT as reference standard. Regarding 3DsnUS and 3DmsUS, separately acquired 3D-US images (per thyroid lobe) were merged to one comprehensive data set. Subsequently, anatomical correctness of the stitching process was analysed via secondary image fusion with the I-124-PET images. Volumetric determinations were conducted by the ellipsoid model (EM) on conUS and CT, and manually drawn segmental contouring (MC) on 3DsnUS, 3DmsUS, CT, and I-124-PET/CT. Mean volume of the thyroid glands was 44.1 ± 25.8 mL (I-124-PET-MC = reference). Highly significant correlations (all p < 0.0001) were observed for conUS-EM (r = 0.892), 3DsnUS-MC (r = 0.988), 3DmsUS-MC (r = 0.978), CT-EM (0.956), and CT-MC (0.986), respectively. The mean volume differences (standard deviations, limits of agreement) in comparison with the reference were −10.50 mL (±11.56 mL, −33.62 to 12.24), −3.74 mL (±3.74 mL, −11.39 to 3.78), and 0.62 mL (±4.79 mL, −8.78 to 10.01) for conUS-EM, 3DsnUS-MC, and 3DmsUS-MC, respectively. Stitched 3D-US data sets of the thyroid enable accurate volumetric determination even in enlarged and deformed organs. The main limitation of high time expenditure may be overcome by artificial intelligence approaches.

Abstract:Ultrasound often represents the first diagnostic step for thyroid nodule evaluation in clinical practice, but baseline US alone is not always effective enough to achieve thyroid nodule characterization. In the last decades new ultrasound techniques, such as CEUS, have been introduced to evaluate thyroid parenchyma as recommended by EFSUMB guidelines, for use in clinical research field, although its role is not yet clear. Several papers show the potential utility of CEUS in the differential diagnosis of benign and malignant thyroid nodules and in the analysis of lymph node involvement in neoplastic pathology. Therefore, we carried out an evaluation of the literature concerning the role of CEUS in three specific areas: the characterization of the thyroid nodule, the evaluation of minimally invasive treatment and loco-regional staging of the lymph node in proven thyroid cancer. According to evidence reported, CEUS can also play an operative role in nodular thyroid pathology as it is able to guide ablation procedures on thyroid nodule and metastatic lymph nodes, to assess the radicality of surgery, to evaluate disease relapse at the level of the margins of ablated regions and to monitor the clinical evolution of necrotic areas in immediate post-treatment setting.

Abstract:Ultrasound three-dimensional visualization, a cutting-edge technology in medical imaging, enhances diagnostic accuracy by providing a more comprehensive and readable portrayal of anatomical structures compared to traditional two-dimensional ultrasound. Crucial to this visualization is the segmentation of multiple targets. However, challenges like noise interference, inaccurate boundaries, and difficulties in segmenting small structures exist in the multi-target segmentation of ultrasound images. This study, using neck ultrasound images, concentrates on researching multi-target segmentation methods for the thyroid and surrounding tissues.

Abstract:Introduction: Currently, B-mode ultrasound (US) is the primary imaging modality in diagnosing thyroid nodules (TNs). B-mode is a two-dimensional US (2D US) imaging display. Recent studies suggest a role for strain and shear wave elastography for evaluating TN as well. Three-dimensional US (3D-US) has the potential to enhance the diagnostic accuracy and precision for thyroid cancer (TC) detection. Materials and Methods: An experienced ultrasonographer (G.A.) evaluated the patient using the following techniques: B-mode, strain and shear wave elastography, and 3D-US followed by fine needle aspiration biopsy (FNAB). Laboratory measurements were performed at LabCorp. Informed consent was obtained. Case: A 28-year-old woman referred for hypothyroidism. Her primary doctor initiated levothyroxine 50 mcg daily 6 months prior. At the time of her visit, her thyroid stimulating hormone (TSH) was 2.8 (0.45-4.5 uIU/mL) and both thyroid peroxidase and thyroglobulin antibodies were elevated, suggestive of Hashimoto's thyroiditis. Her thyroid US showed a heterogeneous gland with an isoechoic TN in the right lobe measuring 7.7 × 6.3 × 7 mm. Strain elastography showed diffuse and patchy tissue stiffness throughout the gland, suggestive of tissue fibrosis caused by Hashimoto's thyroiditis. This study did not distinguish target TN from the surrounding tissue. Shear wave elastography of the TN revealed moderately increased stiffness compared with surrounding tissue. The shear wave velocity (SWV) measurement for the TN was 3.1 m/s. 3D-US examination demonstrated an isoechoic TN with irregular margins, and the volume was 0.119 cm3. FNAB of the TN was performed. Cytopathology was diagnostic for papillary thyroid cancer (PTC), Bethesda Category VI. Subsequent total thyroidectomy confirmed a 7 mm PTC with positive surgical margins caused by thyroid capsule invasion and no clear-cut evidence of extra-thyroid extension. Discussion: This case showcases the recent technological advances in TN imaging. Our objective is to provide an improved approach to TN management. The American College of Radiology Thyroid Imaging Reporting and Data System stratifies the malignancy risk of TN primarily based on the size and B-mode US features. This model does not recommend FNAB for any TN <10 mm regardless of malignancy risk.1 This is our observation that with 3D-US the size cutoff of TN might not be an issue as with B-mode or elastography. Irregularities of the TN can be seen with 3D-US with small and large nodules equally. The finding of irregular margins on 3D-US and consulting with the patient lead us to perform FNAB. Recent publications in the journal of VideoEndocrinology showed utilizations of 3D-US in diagnosing parathyroid adenomas and TNs. 3D-US technology improves view of the target lesion by adding a third dimension, coronal view, to the transverse and longitudinal views of B-mode US.2,3 B-mode imaging provides excellent view of TNs. However, it has a low sensitivity for predicting TC.4 Prospective TN studies have demonstrated that adding elastography to B-mode imaging improves sensitivity of US technology for detecting TC.5-10 In a prospective study with 707 TN, we showed that a single cutoff analysis for predicting malignancy in TNs, a maximum SWV of 3.54 m/s had the best sensitivity. The mean SWV for benign nodules was 2.71 m/s. The mean SWV for malignant nodules was 3.96 m/s.6 In this particular case strain and shear wave were not as helpful. The discrepancy between the two systems has been described in cases with severe Hashimoto's thyroiditis associated with tissue fibrosis.6 In our experience, the presence of autoimmune thyroid disease increases the risk for malignancy. Recent publications reported an association between differentiated TC and autoimmune thyroid disease and/or TSH when all Bethesda classifications were included.11-13 Conclusion: 3D-US technology in conjunction with B-mode may improve diagnostic accuracy in detecting TC. Abstract:Thyroid cancer is the fastest growing age and gender adjusted cancer in 2011 according the American Cancer Society. The majority of the clinically diagnosed thyroid nodules are benign while less than 5% represent intrathyroidal cancers. Currently, the clinical gold-standard procedure for assessing the thyroid nodules is needle biopsy, a procedure that is associated with significant financial burden as well as pain and risk for patients. Therefore, a noninvasive, affordable, and potentially widely available method to differentiate between benign and malignant thyroid nodules can play an important role in reducing the number of unnecessary biopsies. In this study, we investigate the feasibility of two acoustic radiation force elastography techniques, shear wave dispersion ultrasound vibrometry (SDUV) and comb-push ultrasound shear wave elastography (CUSE imaging), in identifying thyroid nodules (imaging) and differentiating between benign and malignant pathologies based on their elasticity and viscosity (SDUV measurements). Our preliminary results show that the measured shear elasticity and shear viscosity parameters depend on tissue type; hence, these measurements may be utilized to differentiate between healthy normal thyroid tissue, benign nodules, and malignant nodules. Further studies on a large population of patients is required to better evaluate the role of the combination of elasticity and viscosity properties of tissue in differentiating various thyroid nodules.

Abstract:The segmentation and estimation of thyroid volume in 3D ultrasound images have attracted the research community's attention because of their great importance in clinical diagnosis. Usually, thyroid volume estimation is based on the segmentation of 3D ultrasound images, which is difficult due to various disorders, including non-homogeneous texture distribution within the thyroid region, artifacts, speckles, and the nature of the thyroid shape. This paper presents an approach to segmenting all individual slices and then reconstructing them into a 3D object to overcome these difficulties. The process involves four techniques. The VOI initialization encompasses the probable thyroid gland; it greatly affects the segmentation results. Multi-gene genetic programming determines the appropriate textural features. The block-matching technique estimates the thyroid gland's change in size and location from slice to slice. Finally, the ITKSNAP software reconstructs the 3D volume. The proposed method is compared with state-of-the-art methods to prove its effectiveness in medical image analysis. Sixteen 3D images from an ultrasound thyroid image dataset were used for the experiments. The analysis of the results based on performance evaluation metrics shows that the proposed method is more efficient than the state-of-the-art methods.

Abstract:Three-dimensional (3D) ultrasound imaging can more accurately represent the volume, shape, and location of the lesion region than the two-dimensional (2D) ultrasound imaging. In this study, we propose a 3D multi-modality free-hand ultrasound imaging system (MFUSI) and focus on its clinical applications. The proposed MFUSI system locates the spatial position of the 2D ultrasound image and reconstructs the 3D ultrasound image through capturing an optical calibration board fixed on the ultrasound probe with a super high-resolution optical camera. In addition to 3D B-mode ultrasound imaging, our proposed MFUSI system is also compatible with 3D ultrasound Doppler imaging and 3D ultrasound elasticity imaging. In terms of clinical application, the MFUSI system has been applied to clinical diagnosis of scoliosis, thyroid nodules, and muscular atrophy, and it also has the potential to be applied in muscle strains, peripheral arterial disease, chronic venous disease, and deep vein thrombosis. We hope that our imaging system might enrich the clinical application of 3D ultrasound imaging.
Establishment and preliminary application of personalized three‐dimensional reconstruction of thyroid gland with automatic detection of thyroid nodules based on ultrasound videos

Abstract:Purpose A well display of the spatial location of thyroid nodules in the thyroid is important for surgical path planning and surgeon‐patient communication. The aim of this study was to establish a three‐dimensional (3D) reconstruction method of the thyroid gland, thyroid nodule, and carotid artery with automatic detection based on two‐dimensional (2D) ultrasound videos, and to evaluate its clinical value. Methods Ultrasound videos, including the thyroid gland with nodule, isthmus of thyroid gland, and ipsilateral carotid artery, were recorded. BC‐UNet, MTN‐Net, and RDPA‐U‐Net network models were innovatively employed for segmentation of the thyroid glands, the thyroid nodules, and the carotid artery respectively. Marching Cubes algorithm was used for reconstruction, while Laplacian smoothing algorithm was employed to smooth the 3D model surface. Using this model, 20 patients and 15 surgeons completed surveys on the effectiveness of this model for the pre‐surgery demonstration of nodule location as well as surgeon‐patient communication. Results The thyroid gland with nodule, isthmus of gland, and carotid artery were reconstructed and displayed. With the 3D model, the understanding of the spatial location of thyroid nodules improved in all three surgeon groups, eliminating the influence of professional levels. In the patient survey, the patients' understanding of the thyroid nodule location and procedure for surgery were significantly improved. In addition, with the 3D model, the time for doctors to explain to patients was significantly reduced (16.75 vs. 8.85 min, p = 0.001). Conclusion To our knowledge, this is the first report of constructing a 3D thyroid model using a deep learning technique for personalized thyroid segmentation based on 2D ultrasound videos. The preliminary clinical application showed that it was conducive to the comprehension of the location of thyroid nodules for surgeons and patients, with significant improvement on the efficiency of surgeon‐patient communication.

Ultrasound super-resolution imaging for the differential diagnosis of thyroid nodules: A pilot study

Abstract:Objective Ultrasound imaging provides a fast and safe examination of thyroid nodules. Recently, the introduction of super-resolution imaging technique shows the capability of breaking the Ultrasound diffraction limit in imaging the micro-vessels. The aim of this study was to evaluate its feasibility and value for the differentiation of thyroid nodules. Methods In this study, B-mode, contrast-enhanced ultrasound, and color Doppler flow imaging examinations were performed on thyroid nodules in 24 patients. Super-resolution imaging was performed to visualize the microvasculature with finer details. Microvascular flow rate (MFR) and micro-vessel density (MVD) within thyroid nodules were computed. The MFR and MVD were used to differentiate the benign and malignant thyroid nodules with pathological results as a gold standard. Results Super-resolution imaging (SRI) technique can be successfully applied on human thyroid nodules to visualize the microvasculature with finer details and obtain the useful clinical information MVD and MFR to help differential diagnosis. The results suggested that the mean value of the MFR within benign thyroid nodule was 16.76 ± 6.82 mm/s whereas that within malignant thyroid was 9.86 ± 4.54 mm/s. The mean value of the MVD within benign thyroid was 0.78 while the value for malignant thyroid region was 0.59. MFR and MVD within the benign thyroid nodules were significantly higher than those within the malignant thyroid nodules respectively ( p < 0.01). Conclusions This study demonstrates the feasibility of ultrasound super-resolution imaging to show micro-vessels of human thyroid nodules via a clinical ultrasound platform. The important imaging markers, such as MVD and MFR, can be derived from SRI to provide more useful clinical information. It has the potential to be a new tool for aiding differential diagnosis of thyroid nodules.

Abstract:Three-dimensional (3D) ultrasound (US) imaging addresses the limitation in field-of-view (FOV) in conventional two-dimensional (2D) US imaging by providing 3D viewing of the anatomy. 3D US imaging has been extensively adapted for diagnosis and image-guided surgical intervention. However, conventional approaches to implement 3D US imaging require either expensive and sophisticated 2D array transducers, or external actuation mechanisms to move a one-dimensional array mechanically. Here, we propose a 3D US imaging mechanism using actuated acoustic reflector instead of the sensor elements for volume acquisition with significantly extended 3D FOV, which can be implemented with simple hardware and compact size. To improve image quality on the elevation plane, we introduce an adaptive-delay synthetic aperture focusing (AD-SAF) method for elevation beamforming. We first evaluated the proposed imaging mechanism and AD-SAF with simulated point targets and cysts targets. Results of point targets suggested improved image quality on the elevation plane, and results of cysts targets demonstrated a potential to improve 3D visualization of human anatomy. We built a prototype imaging system that has a 3D FOV of 38 mm (lateral) by 38 mm (elevation) by 50 mm (axial) and collected data in imaging experiments with phantoms. Experimental data showed consistency with simulation results. The AD-SAF method enhanced quantifying the cyst volume size in the breast mimicking phantom compared to without elevation beamforming. These results suggested that the proposed 3D US imaging mechanism could potentially be applied in clinical scenarios.

Abstract:Noninvasive diagnosis of bone density and mechanical properties using non-radiation imaging modality is an emerging area with promising in early prediction of osteopenia and treatment effectiveness in the clinic and functional disuse, i.e., long-term bedrest and space mission. Advances in quantitative ultrasound have shown advantages in measuring both bone density and mechanical strength, non-radiation, imaging capability, and easy to use. The challenge that remained is the poor penetration of ultrasound signals passing through trabecular and cortical bones and acoustic energy scattering. A new scanning confocal ultrasound technology is developed in this lab to detect the alteration of bone to provide diagnostic results in bone density and structure properties. A software-controlled flexible ultrasound system with 2-D dual array transducer is developed and proposed for the purpose of noninvasive bone density diagnosis and assessment of bone loss. Transmitting (Tx) transducer elements are divided into sub-blocks to excite the ultrasound signals in sequence to decrease the system complexity while maintaining beam pattern properties by the signal processing procedure at receiving (Rx) side. Apodization is also applied to reduce acoustic side lobes and to make the resolution in the ultrasound field of view (FOV) more uniform. This study may provide basic understanding of modulated confocal ultrasound beam forming for tissue characterization, such as trabecular bone structual and strength properties.

Abstract:Ultrasound is the go‐to modality for thyroid nodule management, however it has drawbacks which can be solved by using three‐dimensional (3D) ultrasound (US). With this technology, development of computer‐aided diagnosis (CAD) and intervention (CAI) systems will be enabled. The end result can be improvements in diagnosis, treatment monitoring and follow‐up. Ultrasound, the primary imaging modality in thyroid nodule management, suffers from drawbacks including: high inter‐ and intra‐observer variability, limited field‐of‐view and limited functional imaging. Developments in ultrasound technologies are taking place to overcome these limitations, including three‐dimensional‐Doppler, ‐elastography, ‐nodule characteristics‐extraction, and novel machine‐learning algorithms. For thyroid ablative treatments and biopsies, perioperative use of three‐dimensional ultrasound opens a new field of research. This review provides an overview of the current and future applications of ultrasound, and discusses the potential of new developments and trends that may improve the diagnosis, therapy, and follow‐up of thyroid nodules.

Abstract:Medical ultrasound is extensively used to define tissue textures and to characterize lesions, and it is the modality of choice for detection and follow-up assessment of thyroid diseases. Classical medical ultrasound procedures are performed manually by an occupational operator with a hand-held ultrasound probe. These procedures require high physical and cognitive burden and yield clinical results that are highly operator-dependent, therefore frequently diminishing trust in ultrasound imaging data accuracy in repetitive assessment. A robotic ultrasound procedure, on the other hand, is an emerging paradigm integrating a robotic arm with an ultrasound probe. It achieves an automated or semi-automated ultrasound scanning by controlling the scanning trajectory, region of interest, and the contact force. Therefore, the scanning becomes more informative and comparable in subsequent examinations over a long-time span. In this work, we present a technique for allowing operators to reproduce reliably comparable ultrasound images with the combination of predefined trajectory execution and real-time force feedback control. The platform utilized features a 7-axis robotic arm capable of 6-DoF force-torque sensing and a linear-array ultrasound probe. The measured forces and torques affecting the probe are used to adaptively modify the predefined trajectory during autonomously performed examinations and probe-phantom interaction force accuracy is evaluated. In parallel, by processing and combining ultrasound B-Mode images with probe spatial information, structural features can be extracted from the scanning volume through a 3D scan. The validation was performed on a tissue-mimicking phantom containing thyroid features, and we successfully demonstrated high image registration accuracy between multiple trials.

Abstract:Accurately imaging bones using ultrasound has been a long-standing challenge, primarily due to the high attenuation, significant acoustic impedance contrast at cortical boundaries, and the unknown distribution of sound velocity. Furthermore, two-dimensional (2-D) ultrasound bone imaging has limitations in diagnosing osteoporosis from a morphological perspective, as it lacks stereoscopic spatial information. To address these limitations, a three-dimensional (3-D) bone ultrasound tomography (USCT) method was proposed in this work. The compensations of directivities for the emitting and receiving elements on artifact elimination were incorporated. Moreover, the application of acoustic reciprocity further reduced the amount of data storage and improved the efficiency of the method. Two ex-vivo bovine femurs were employed to demonstrate the effectiveness of the method for 3-D cortical bone reconstruction and osteoporosis diagnosis. The reconstructed 3-D images were in good agreement with the reference images scanned by micro computed tomography (μCT) in terms of the volume. The mean relative errors of 3-D volumes for the ex-vivo experiments were all smaller than 13%. Unlike 2-D imaging, 3-D reconstruction can quantitatively provide the bone mass loss (i.e., 2.85% for sample 1 and 3.72% for sample 2, respectively), rather than simply offering a qualitative description of 2-D morphology of the osteoporosis part. The proposed bone USCT method has been demonstrated to be an effective way for 3-D cortical bone imaging and osteoporosis diagnosis.

Abstract:Thyroid volumetry is crucial in the diagnosis, treatment, and monitoring of thyroid diseases. However, conventional thyroid volumetry with 2D ultrasound is highly operator-dependent. This study compares 2D and tracked 3D ultrasound with an automatic thyroid segmentation based on a deep neural network regarding inter- and intraobserver variability, time, and accuracy. Volume reference was MRI. 28 healthy volunteers (24—50 a) were scanned with 2D and 3D ultrasound (and by MRI) by three physicians (MD 1, 2, 3) with different experience levels (6, 4, and 1 a). In the 2D scans, the thyroid lobe volumes were calculated with the ellipsoid formula. A convolutional deep neural network (CNN) automatically segmented the 3D thyroid lobes. 26, 6, and 6 random lobe scans were used for training, validation, and testing, respectively. On MRI (T1 VIBE sequence) the thyroid was manually segmented by an experienced MD. MRI thyroid volumes ranged from 2.8 to 16.7 ml (mean 7.4, SD 3.05). The CNN was trained to obtain an average Dice score of 0.94. The interobserver variability comparing two MDs showed mean differences for 2D and 3D respectively of 0.58 to 0.52 ml (MD1 vs. 2), −1.33 to −0.17 ml (MD1 vs. 3) and −1.89 to −0.70 ml (MD2 vs. 3). Paired samples t-tests showed significant differences for 2D ( p = .140, p = .002 and p = .002) and none for 3D ( p = .176, p = .722 and p = .057). Intraobsever variability was similar for 2D and 3D ultrasound. Comparison of ultrasound volumes and MRI volumes showed a significant difference for the 2D volumetry of all MDs ( p = .002, p = .009, p <.001), and no significant difference for 3D ultrasound ( p = .292, p = .686, p = 0.091). Acquisition time was significantly shorter for 3D ultrasound. Tracked 3D ultrasound combined with a CNN segmentation significantly reduces interobserver variability in thyroid volumetry and increases the accuracy of the measurements with shorter acquisition times.

Abstract:Quantitative ultrasound (QUS) offers a non-invasive and objective way to quantify tissue health. We recently presented a spatially adaptive regularization method for reconstruction of a single QUS parameter, limited to a two dimensional region. That proof-of-concept study showed that regularization using homogeneity prior improves the fundamental precision-resolution trade-off in QUS estimation. Based on the weighted regularization scheme, we now present a multiparametric 3D weighted QUS (3D QUS)imaging system, involving the reconstruction of three QUS parameters: attenuation coefficient estimate (ACE), integrated backscatter coefficient (IBC) and effective scatterer diameter (ESD). With the phantom studies, we demonstrate that our proposed method accurately reconstructs QUS parameters, resulting in high reconstruction contrast and therefore improved diagnostic utility. Additionally, the proposed method offers the ability to analyze the spatial distribution of QUS parameters in 3D, which allows for superior tissue characterization. We apply a three-dimensional total variation regularization method for the volumetric QUS reconstruction. The 3D regularization involving N planes results in a high QUS estimation precision, with an improvement of standard deviation over the theoretical rate achievable by compounding N independent realizations. In the in vivo liver study, we demonstrate the advantage of adopting a multiparametric approach over the single parametric counterpart, where a simple quadratic discriminant classifier using feature combination of three QUS parameters was able to attain a perfect classification performance to distinguish between normal and fatty liver cases.

Abstract:In nuclear medicine, radioiodine therapy is prescribed to treat diseases like hyperthyroidism. The calculation of the prescribed dose depends, amongst other factors, on the thyroid volume. This is currently estimated using conventional 2D ultrasound imaging. However, this modality is inherently user-dependant, resulting in high variability in volume estimations. To increase reproducibility and consistency, we uniquely combine a neural network-based segmentation with an automatic robotic ultrasound scanning for thyroid volumetry. The robotic acquisition is achieved by using a 6 DoF robotic arm with an attached ultrasound probe. Its movement is based on an online segmentation of each thyroid lobe and the appearance of the US image. During post-processing, the US images are segmented to obtain a volume estimation. In an ablation study, we demonstrated the superiority of the motion guidance algorithms for the robot arm movement compared to a naive linear motion executed by the robot in terms of volumetric accuracy. In a user study on a phantom, we compared conventional 2D ultrasound measurements with our robotic system. The mean volume measurement error of ultrasound expert users could be significantly decreased from $20.85\pm 16.10\%$ to only $8.23\pm 3.10\%$ compared to the ground truth. This tendency was observed even more in non-expert users where the mean error improvement with the robotic system was measured to be as high as 85 which clearly shows the advantages of the robotic support.

Abstract:The TI‐RADS classification standard scores suspicious nodules that are difficult to determine benign and malignant as grade 4, and further pathological puncture is recommended clinically, which may lead to a large number of unnecessary biopsies and operations. Thyroid nodule is a common and frequently occurring disease in the neck in recent years, and ultrasound has become the preferred imaging diagnosis method for thyroid nodule due to its advantages of noninvasive, nonradiation, real‐time, and repeatable. The thyroid imaging, reporting and data system (TI‐RADS) classification standard scores suspicious nodules that are difficult to determine benign and malignant as grade 4, and further pathological puncture is recommended clinically, which may lead to a large number of unnecessary biopsies and operations. Including conventional ultrasound, ACR TI‐RADS, shear wave elastography, super microvascular imaging, contrast enhanced ultrasound, "firefly," artificial intelligence, and multimodal ultrasound imaging used in combination. In order to identify the most clinically significant malignant tumors when reducing invasive operations. This article reviews the application and research progress of multimodal ultrasound imaging in the diagnosis of TI‐RADS 4 thyroid nodules.
